METHODS We retrospectively reviewed the data of 612 consecutive upper tract urothelial carcinoma patients (341 with renal pelvic tumors and 271 with ureteral tumors) treated at Peking University First Hospital from 1999 to 2011. The clinical features, pathological outcomes, the methylation status of ten gene promoter (ABCC6, BRCA1, CDH1, GDF15, HSPA2, RASSF1A, SALL3, THBS1, TMEFF2 and VIM), and oncologic prognosis including cancer specific survival (CSS), overall survival (OS), bladder recurrence and contralateral recurrence were compared. RESULTS For clinical features, more patients with renal pelvic tumors presented with gross hematuria (p<0.001), while patients with ureteral tumors tended to be older (p=0.002), and was associated with worse pre-operative renal function (p<0.001), higher proportion of hypertension (p=0.038) and the presence of ipsilateral hydronephrosis (p<0.001). As for pathological outcomes, patients with renal pelvic tumor were featured for more lymph node metastasis (p=0.045) and sessile architecture (p=0.023), but less G3 tumor grade (p<0.001). With reference to gene promoter methylation status, more frequent methylation was present in renal pelvic tumors, with the mean number methylated genes 3.71±2.33 versus 2.85±2.19 in ureteral tumors (p<0.001), and there were significantly more methylated promoters of TMEFF2, GDF15, RASSIFA, SLL3 and ABCC6 in renal pelvic tumors (all p<0.05). Regarding prognosis, after a median follow-up duration of 64 months, location failed to reach independent predictor status with respect to CSS, OS, bladder recurrence or contralateral recurrence (all p>0.05), and gene methylation status was found to be an important risk factor for survival. CONCLUSIONS Although renal pelvic and ureteral tumors had similar prognosis, they exhibited significant difference in clinical-pathological characteristics and epigenetic biomarkers.
