Abstract:Objective To evaluate the difference of diffusion weighted imaging(DWI) ADC value and rADC value of different degree of differentiation cervical cancer and correlation with cervical cancer cell proliferation activity (PCNA and Ki-67).Methods Forty patients with pathological diagnosis for cervical cancer were selected for this study.Preoperative magnetic resonance imaging(MRI) and DWI was performed before operation.Meanwhile,the PCNA and Ki-67 immunohistochemical staining were performed.The expression of PCNA and Ki-67 in different differentiation degree of cervical cancer were analysed by analysis of variance,and the correlation with ADC and the rADC value were also analysed by Pearson correlation methods.SPSS 20.0 was used to calculated the relevant data and significance was set at ＜ 0.05.Results The ADC values and rADC value in poorly differentiated group is less than high differentiation group,and the difference was statistically significant(t =2.118,P ＜ 0.05).There was no significant difference between the poorly differentiated group and middle differentiated group(P ＞ 0.05).With the degree of differentiation is reduced,the expression of PCNA and Ki-67 is increased.With the increase of the degree of expression of PCNA and Ki-67,ADC values and the rADC value decreased,the difference had statistical significance (P ＜ 0.05).Pearson correlation test showed that ADC values to be inversely associated with the degree of Ki-67 and PCNA expression.Conclusion DWI imaging and the ADC,rADC value measurement for cervical cancer can help to evaluate the cervical cancer with different pathological level and has the potential to be a non-invasive method for identifying tumor proliferation.
The Diagnostic Role of Diffusion-Weighted Magnetic Resonance Imaging in Hypopharyngeal Carcinoma.

Abstract:The aim of the present study was to assess the role of diffusion-weighted magnetic resonance imaging (DWI) and apparent diffusion coefficient (ADC) values in hypopharyngeal carcinoma. A total of 40 hypopharyngeal carcinoma tissues and 15 benign lesion tissues were retrospectively analyzed. DWI, and T1- and T2-weighted magnetic resonance imaging (MRI) was performed. The sensitivity, specificity and accuracy of conventional MRI were 97.5, 66.7, and 89.1%, respectively. The mean ADC value [diffusion sensitive factor (b)=1,000× sec/mm2) for hypopharyngeal carcinomas was (1.0285±0.0328)×10-3 mm2/sec, which was significantly lower than the mean ADC value for benign lesions [(1.5333±0.1061)×10-3 mm2/sec; P<0.001]. Receiver operating characteristic (ROC) curve analysis revealed that the area under the curve (AUC) was 0.921 while the optimal threshold for the cut-off point of the ADC was 1.075×10-3 mm2/sec. The mean ADC value of the metastatic nodes was (0.9184±0.0538)×10-3 mm2/sec, lower than the mean value for the benign nodes [(1.2538±0.1145)×10-3 mm2/sec; P=0.005]. Two groups were created according to the mean of the ADC value of hypopharyngeal carcinomas [≤(1.0285±0.0328)×10-3 mm2/sec vs. >(1.0285±0.0328)×10-3 mm2/sec]. The 2-year survival rates of the two groups were 55.6 and 100.0%, respectively (P=0.024). ADC values may aid in distinguishing hypopharyngeal carcinomas from benign lesions and differentiating metastatic lymph nodes of hypopharyngeal squamous cell carcinomas from reactive cervical lymph nodes. In conclusion, mean ADC values may be useful prognostic factors in univariate analysis of hypopharyngeal carcinoma.
Differentiating Laryngeal Carcinomas from Precursor Lesions by Diffusion-Weighted Magnetic Resonance Imaging at 3.0 T: a Preliminary Study.

Abstract:BACKGROUND:Diffusion-weighted magnetic resonance imaging (DWI) has been introduced in head and neck cancers. Due to limitations in the performance of laryngeal DWI, including the complex anatomical structure of the larynx leading to susceptibility effects, the value of DWI in differentiating benign from malignant laryngeal lesions has largely been ignored. We assessed whether a threshold for the apparent diffusion coefficient (ADC) was useful in differentiating preoperative laryngeal carcinomas from precursor lesions by turbo spin-echo (TSE) DWI and 3.0-T magnetic resonance.METHODS:We evaluated DWI and the ADC value in 33 pathologically proven laryngeal carcinomas and 17 precancerous lesions.RESULTS:The sensitivity, specificity, and accuracy were 81.8%, 64.7%, 76.0% by laryngostroboscopy, respectively. The sensitivity, specificity, and accuracy of conventional magnetic resonance imaging were 90.9%, 76.5%, 86.0%, respectively. Qualitative DWI analysis produced sensitivity, specificity, and accuracy values of 100.0, 88.2, and 96.0%, respectively. The ADC values were lower for patients with laryngeal carcinoma (mean 1.195±0.32×10(-3) mm(2)/s) versus those with laryngeal precancerous lesions (mean 1.780±0.32×10(-3) mm(2)/s; P<0.001). ROC analysis showed that the area under the curve was 0.956 and the optimum threshold for the ADC was 1.455×10(-3) mm(2)/s, resulting in a sensitivity of 94.1%, a specificity of 90.9%, and an accuracy of 92.9%.CONCLUSIONS:Despite some limitations, including the small number of laryngeal carcinomas included, DWI may detect changes in tumor size and shape before they are visible by laryngostroboscopy. The ADC values were lower for patients with laryngeal carcinoma than for those with laryngeal precancerous lesions. The proposed cutoff for the ADC may help distinguish laryngeal carcinomas from laryngeal precancerous lesions.
Correlations between HPV16/18 infection and p16 expression in endocervical adeno carcinomas

Abstract:Objective: To analyze the relationship between the high risk human papillomavirus(HPV16/18) DNA,p16 protein expression and the clinical factors of endocervical adenocarcinomas(ECAs),and explore the relationship between HPV16 /18 DNA and diffuse / strong p16 expression in ECAs.Methods: Fourty eight cases of ECAs were collected and detected for HPV16 /18 DNA by using in situ hybridization assay and for p16 protein expression by using immunohistochemistry streptavidin-biotin method.Results: The positive rate of HPV16 /18 DNA was 70.83%(34 /48).There was no significant correlation between HPV16/18 infection and age,clinical stage,lymphnode etastasis,differentiation and histological subtype.There was no statistically significant between strong p16 expression and age,lymphnode metastasis and differentiation of ECAs.But the strong p16 expression level in different clinical stages and histological subtypes each were not identical.Thirhty two cases in forty eight HPV16 /18 DNA-positive ECAs exhibited high levels of p16 expression(94.12%,32 / 34),while only two in forteen HPV16 /18 DNA-negative cases(14.29%,2 /14).There was significant difference(P=0.000).Conclusion: There might be no correlation between HPV16 /18 infection and the progression of ECAs.The strong expression of p16 might be related to both clinical stage and histological subtype of ECAs,and it may serve as a sign for HPV16 /18 infection in ECAs.

Abstract:ObjectivesTo evaluate the potential value of apparent diffusion coefficient (ADC) measurement in the assessment of cervical cancer.MethodsOne hundred twelve patients with cervical cancer and 67 control subjects underwent diffusion-weighted imaging (DWI) in addition to routine MR imaging at 3.0-T MRI before therapy. All ADCs were calculated from b = 0, 600 s/mm2 and b = 0, 1,000 s/mm2.ResultsThe ADCs of cervical cancer were significantly lower than those of normal cervix for both ADC maps. There was a statistically significant difference between the ADCs of well-/moderately differentiated (G1/2) tumours and poorly differentiated (G3) tumours, between the ADCs of squamous cell carcinoma and adenocarcinoma, between the pretherapy ADCs of tumour recurrence or metastasis and tumour free patients after radical hysterectomy for both ADC maps. There was no significant difference among the ADCs of cervical cancer when divided by other features (FIGO, lymph node status, tumour size and age groups) for both ADC maps.ConclusionADC values were reliable for differentiating cervical cancer from normal cervix with high diagnostic accuracy. The ADCs can be used to indicate the degree and histological type of cervical cancer, although there is some overlap. G3 tumours and lower ADCs may indicate poor prognosis. Abstract:OBJECTIVE:The aim of this study was to explore the application of whole-lesion histogram analysis of apparent diffusion coefficient (ADC) values of cervical cancer. METHODS:A total of 54 women (mean age, 53 years) with cervical cancers underwent 3-T diffusion-weighted imaging with b values of 0 and 800 s/mm prospectively. Whole-lesion histogram analysis of ADC values was performed. Paired sample t test was used to compare differences in ADC histogram parameters between cervical cancers and normal cervical tissues. Receiver operating characteristic curves were constructed to identify the optimal threshold of each parameter. RESULTS:All histogram parameters in this study including ADCmean, ADCmin, ADC10%-ADC90%, mode, skewness, and kurtosis of cervical cancers were significantly lower than those of normal cervical tissues (all P < 0.0001). ADC90% had the largest area under receiver operating characteristic curve of 0.996. CONCLUSIONS:Whole-lesion histogram analysis of ADC maps is useful in the assessment of cervical cancer.

Abstract:Objective: To investigate the value of apparent diffusion coefficient (ADC) in predicting the pathomechanism of non-small-cell lung carcinoma (NSCLC). Materials and Methods:Four-six patients with NSCLC conformed by histopathology were examined with diffusion weighting imaging (DWI). ADCs were calculated on multiple b-value imaging. Correlations between the ADCs and tumor cellularity, nuclear-cytoplasmic ratio and nuclear-kytoplasm ratio were analysed by Pearson rank correlation. Results:The ADCs of NSCLC correlated negatively with tumorous cellular density on multiple b-value imaging and as b was 300 s/mm2,which can achieve the best correlation (b=300 s/mm2, r=-0.792, P<0.01). There were inverse correlations between ADCs and nuclear-cytoplasmic ratio, nuclear-kytoplasm ratio (b=300 s/mm2, r=-0.739, P<0.01,-0.607, P=0.0187, respectively). Conclusions:Preliminary results revealed the ADCs of NSCLC had close correlation with tumorous histopathology characteristic.

Abstract:Human Papillomavirus (HPV) is reported in 60-100% of cervical adenocarcinoma (CADC) globally. We investigated this relationship in a hospital-based survey in China. 718 CADC samples from nine Chinese regions were analysed. Expert pathologists reviewed cases with p16 and progesterone receptor immunostaining. Cases were tested for HPV using whole-tissue sections (WTS) and laser-capture microdissection. All cases were HPV-tested by L1 based broad-spectrum SPF10-DEIA-LiPA(25) PCR. Negative cases were tested for DNA adequacy and with E6 oncogene, type-specific HPV PCRs. Using WTS-PCR CADC showed overall 75% HPV-positivity (33-100% for different histological types). LCM-PCR showed that none of minimal deviation or serous CADC, and <10% of all clear cell and endometrioid CADC were HPV-positive in tumour cells. Usual and adenosquamous CADC showed a single HPV genotype in 60 and 78% cases. In some cases, HPV was found in adjacent cervix but not in tumour. HPV 16, 18 and 45 accounted for 90% of HPV in tumour cells. Patients with HPV-positive tumours were on average 6 years younger and presented at a lower clinicopathological stage as compared to patients with HPV-negative cancers. CADC is diverse pathologically and in HPV status. Special histopathological tumor subtypes may develop through different cellular and molecular pathways. Between 20 and 40% usual and adenosquamous types, in particular these diagnosed in older women and at advanced FIGO stages, are not driven by oncogenic HPV. In these cases HPV may not be involved in carcinogenisis or maybe lost during tumour progression.
Efficacy of Apparent Diffusion Coefficient in Predicting Aggressive Histological Features of Papillary Thyroid Carcinoma.

Abstract:PURPOSE:We aimed to evaluate preoperative diffusion-weighted magnetic resonance imaging (DWI) for predicting aggressive histological features in papillary thyroid cancer (PTC). METHODS:This prospective study included 141 PTC patients, who underwent DWI prior to thyroidectomy; 88 patients with 88 PTC lesions were finally analyzed. Multiple comparisons of mean and minimum apparent diffusion coefficient (ADC) values (ADCmean and ADCmin) and ADC of the solid component (ADCsolid) between the lowly aggressive PTC, highly aggressive PTC without hobnail, and hobnail variant PTC groups were performed by one-way ANOVA or the Welch test. The nonparametric Kruskal-Wallis H-test was used to assess lesion size differences. Receiver-operating characteristic (ROC) curve analysis was also performed. RESULTS:ADC values in the lowly aggressive PTC group were found to be significantly higher than those in the highly aggressive PTC without hobnail group (ADCmean: 1.35±0.20×10-3 mm2/s vs. 1.16±0.17×10-3 mm2/s, P = 0.003; ADCmin: 1.10±0.17×10-3 mm2/s vs. 0.88±0.16×10-3 mm2/s, P < 0.001; ADCsolid: 1.26±0.23×10-3 mm2/s vs. 1.04±0.17×10-3 mm2/s, P < 0.001). No significant differences for the ADCmean, ADCmin, and ADCsolid were observed between the lowly aggressive and hobnail variant PTC groups (all P > 0.05). Lesion sizes in the hobnail variant PTC group was significantly elevated compared with the lowly aggressive PTC group (2.19±1.21 cm vs. 0.93±0.37 cm, P < 0.001). Areas under the curves (AUCs) for ADCmean, ADCmin, and ADCsolid between the lowly aggressive PTC and highly aggressive PTC group without hobnail were 0.758, 0.851, and 0.787, respectively. The AUC for size between the lowly aggressive and hobnail variant PTC group was 0.896. CONCLUSION:ADCmin from DWI could potentially provide quantitative information to differentiate lowly aggressive PTC from highly aggressive PTC lesions without hobnail variants.

Abstract:Purpose: To assess diffusion-weighted magnetic resonance imaging (DWI-MRI) performed with apparent diffusion coefficient (ADC) values for the detection of cervical lymphadenopathy.Materials and Methods: Studies evaluating DWI-MRI for the detection of cervical lymphadenopathy were systematically searched for in the MEDLINE, EMBASE, Cancerlit, and Cochrane Library and other database from January 1995 to November 2010. By node-based data analyses, Cochrane methodology was used for the results of this meta-analysis.Results: Eight studies enrolling a total of 229 individuals were eligible for inclusion. Significant differences were found between malignant nodes and benign nodes of the mean ADC value (WMD [weighted-mean difference]: 1.19, 95% CI: [1.02, 1.35] x 10(-3) mm(2)/s, [P < 0.05]). In the secondary outcomes, significant differences were found between lymphomatous nodes and benign nodes (WMD: 1.33, 95% CI: [0.89, 1.77] x 10(-3) mm(2)/s), and nodes originating from highly or moderately differentiated cancer (WMD: 0.24, 95% CI: [0.21, 0.28] x 10(-3) mm(2)/s, [P < 0.05]), and nodes originating from poorly differentiated cancers (WMD: 0.10, 95% CI: [0.06, 0.14] x 10(-3) mm(2)/s, [P < 0.05]).Conclusion: DWI-MRI performed with ADC values shows significant differences among malignant nodes, lymphomatous nodes, and benign nodes in cervical lymphadenopathy. Diffusion Weighted MRI in Cervical Carcinomas:ADC Values and Its Relationship with Clinical and Pathological Features

Abstract:Objective To evaluate the role of diffusion weighted imaging(DWI)in uterine cervical carcinoma by exploring the relationship of ADC values with the lesion size,the oncologic therapeutic approach and various pathological grades of squamous cell carcinoma.Materials and Methods Totally 105 consecutive cases with pathologically proved cervical cancers were included in this study.MR examinations,including routine imaging and DWI were performed before therapy on a 1.5T scanner.The size,treatment,and pathological type were recorded.Analysis and measurement of DWI were performed on a post-process workstation with a pixel-analysis-software.Results Mean ADC values(1.128±0.1468×10-3mm2/s)were much higher than median ADC values(1.083±0.1591×10-3mm2/s)(P0.001),and got very strong positive linear correlation(R=0.990).There was a slightly negative correlation(R=-0.265~-0.310)between ADC values and tumor size.The sizes of lesions in operation group were significantly smaller than that of(chemo-)radiation group(P0.01),while the ADC values were higher in operation group(P0.05).There were no significant differences found in ADC values between different grades of squamous cell carcinomas.Conclusion The ADC value in a whole tumor has normal distribution,with slight shift to left.There is a tendency that the larger the size,the lower the ADC values.Lesions with smaller size and higher ADC values may get more opportunity for operation.

Abstract:The purpose of our study was to evaluate the value of whole-lesion apparent diffusion coefficient (ADC) histogram parameters in distinguishing cervical cancers with different differentiation. A cohort of 18 patients (mean age, 53 years) with biopsy confirmed cervical cancers were involved in this prospective study. All of them underwent 3-T diffusion weighted imaging (DWI) with b values of 0 and 800 s/mm2. ADC histogram parameters were obtained from the entire tumor, including mean ADC values (ADCmean), the percentile ADC values i.e. ADC5%, ADC10%, ADC25%, ADC50%, ADC75%, and ADC90%, skewness, kurtosis, s-sDlowest and s-sDav distribution width. According to Mann-Whitney U-test, the s-sDav and 90th percentile ADC values of well or moderately differentiated cervical cancers were significantly higher than those of poorly differentiated cervical cancers (Both P < 0.05). Whereas the other measurements showed no significant differences. The ROC analysis demonstrated that the area under the ROC (AUC) of s-sDav (0.925) was higher than that of 90th percentile ADC values (0.85). All parameters showed excellent performance in inter-observer stability assessment (all ICC > 0.95). In conclusion, whole-lesion histogram analysis of ADC maps is helpful in discriminating well or moderately differentiated cervical cancers from poorly differentiated cervical cancers.
Value of MR diffusion-weighted imaging in diagnosis and outcome prediction for uterine cervical cancer

Abstract:Objective To investigate the clinical application of diffusion weighted imaging (DWI) in uterine cervical cancer and the apparent diffusion coefficient (ADC) value in diagnosis and predicting treatment response.Methods Twenty-eight patients with advanced primary cervical cancer confirmed by pathology and 10 cases of normal uterine cervix as control were recruited in this prospective clinical trial.To analyze the correlation between tumor volume measured in DWI and tumor maximum diameter measured according to the RECIST criteria.To compare the ADC value differences among the uterine cervical cancer,uterine myometrium,and normal uterine cervix.To compare the ADC values in 17 cancer patients before and after treatment.Results The illustration of tumor boundary in DWI was superior to conventional T2WI and post-enhancement T1WI.The DWI with higher b value (2000 s/mm2) had a better signal-to-noise ratio.The tumor volume measured in DWI has good correlation with tumor maximum diameter according to RECIST criteria (r =0.759,P ＜ 0.01).When b =800 s/mm2,the ADC values of the uterine cervical cancer,uterine myometrium,and normal uterine cervix were (9.85 ± 1.55) × 10-3 mm2/s,(14.20 ± 2.80) ×10-3 mm2/s,and (14.14 ±0.45) × 10-3 mm2/s.When b =2000 s/mm2,the ADC values of the uterine cervical cancer,uterine myometrium and normal uterine cervix were (7.38 ±0.98) × 10-3 mm2/s,(8.52 ± 2.38) × 10-3 mm2/s,and (8.60 ±0.63) × 10-3 mm2/s,respectively.There were significant differences between the cervical cancer and normal cervix or uterine myometrium (P ＜0.001 for both).When b =800 s/mm2,the ADC value was (9.85 ± 1.55) × 110-3 mm2/s before and (13.41 ± 2.93) × 10-3 mm2/s after treatment (P ＜ 0.001).When b =2000 s/mm2,the ADC value was (7.38 ±0.98) × 10-3 mm2/s before and (8.93 ± 1.92) × 10-3 mm2/s after treatment (P =0.008).Univariate logistic regression analysis showed that 25％ ADC,50％ ADC,and 75％ ADC in the tumor ADC value histogram before treatment were significantly correlated to the treatment outcome of cervical cancer (P ＜ 0.05 for all).Multivariate regression analysis showed that 25％ ADC,50％ ADC,and 75％ ADC in the tumor ADC value histogram before treatment were not significantly correlated to the treatment outcome of cervical cancer (P ＞ 0.05 for all).The values of ROC curves were 25％ADC =0.818,50％ADC =0.775,and 75％ADC =0.716 (P ＞0.05),however,the 25％ ADC showed a relatively stronger statistical power.Conclusions DWI helps to confirm the morphology and exact target zone of the tumor for radiotherapy.DWI volume measurement is well correlated with RECIST criteria,particularly in volume measurement of irregular tumors.ADC value has a potential in quantitatively monitoring treatment response and predicting outcome of cervical cancers.
The Variable Characteristics of Human Papillomavirus in Squamous Cell Carcinoma and Adenocarcinoma of Cervix in China.

Abstract:Objective A hospital-based multicenter, retrospective study was conducted to compare the distribution of human papillomavirus (HPV) in squamous cell carcinoma (SCC) and cervical adenocarcinoma (CADC) in China. Methods Paraffin-embedded tissue blocks diagnosed as SCC and CADC across China were collected, as well as the total number of diagnosed invasive cervical cancer of the 9 selected centers. DNA enzyme immunoassay, reverse hybridization, and multiplex type-specific polymerase chain reaction were used for HPV genotyping. Results The ratios of CADC to SCC were increasing from 2005 to 2010, in parallel with HPV prevalence in CADC. In 630 patients with SCC (mean ± SD age, 45.40 ± 10.30) and 718 patients with CADC (mean ± SD age, 46.09 ± 10.59) recruited, HPV prevalence rates were 97.6% and 74.5%, respectively. Human papillomavirus viral load for SCC is significantly higher than that for CADC. Most common HPV types distributed in SCC and CADC were HPV-16 (78.5%, 75.1%–81.6%; 47.1%, 42.9%–51.3%), HPV-18 (8.0%, 6.1%–10.4%; 41.1%, 37.0%–45.3%), HPV-52 (2.3%, 1.4%–3.8%; 5.6%, 4.0%–7.9%), and HPV-45 (1.1%, 0.6%–2.3%; 3.9%, 2.6%–5.9%). Different diagnostic mean ± SD age for HPV-16/HPV-18 versus other high-risk HPV types were observed: SCC (44.5 ± 9.94 vs 51.0 ± 10.83, p < .05) and CADC (44.1 ± 9.44 vs 47.4 ± 10.41, p = .006). For HPV-negative cases, mean ± SD age was 46.1 ± 10.73 in SCC and 50.3 ± 11.85 in CADC, which were older than the positive (45.4 ± 10.31, 44.5 ± 9.64). HPV-16 and HPV-18 were the most frequent HPV types in both histological types, and HPV-18 was more frequent in CADC than in SCC. Conclusions Human papillomavirus infection was identified more often in SCC than in CADC. Women with HPV-associated cancers, especially HPV-16/HPV-18, were of a younger age at diagnosis when compared with non–HPV-associated cancers.
Diagnostic value of apparent diffusion coefficient in detecting cervical involvement of endometrial carcinoma

Abstract:Objective: To observe the value of apparent diffusion coefficient (ADC) in detecting cervical involvement of endometrial carcinoma (EC). Methods: Twenty-six patients with cervical involvement (CI) of EC were selected as experiment group, while 26 patients without CI were enrolled as the control group. ADC values were measured in the regions of interest within the intrauterine endometrial cancer, the internal and the external os of the cervix of all patients. Receiver operating characteristic (ROC) curve analysis was performed to investigate whether ADC values could help to discriminate CI. Results: There was no significant difference of mean ADC value between intrauterine cancer ([0.90±0.28]×10 -3 mm 2/s) and the cervical internal os ([0.92±0.24]×10 -3 mm 2/s) in the experiment group (t=-0.45, P=0.66). The mean ADC value of the intrauterine endometrial carcinoma ([0.83±0.21]×10 -3 mm 2/s) was significantly lower than that of the cervical internal os ([1.32±0.38]×10 -3 mm 2/s) in the control group (t=-6.86, P<0.001). Meanwhile, the mean ADC values of the intrauterine endometrial carcinoma showed no significant difference between the two groups (t=0.74, P=0.47). Area under curve (AUC) of ADC was 0.86. The optimal ADC threshold value was 1.02×10 -3 mm 2/s, the sensitivity and the specificity was 73.1% and 96.2%, respectively. Conclusion: ADC measurement can provide useful information in detecting the cervical involvement of endometrial carcinoma.
