Abstract:OBJECTIVE:The aim of this study was to explore the application of whole-lesion histogram analysis of apparent diffusion coefficient (ADC) values of cervical cancer. METHODS:A total of 54 women (mean age, 53 years) with cervical cancers underwent 3-T diffusion-weighted imaging with b values of 0 and 800 s/mm prospectively. Whole-lesion histogram analysis of ADC values was performed. Paired sample t test was used to compare differences in ADC histogram parameters between cervical cancers and normal cervical tissues. Receiver operating characteristic curves were constructed to identify the optimal threshold of each parameter. RESULTS:All histogram parameters in this study including ADCmean, ADCmin, ADC10%-ADC90%, mode, skewness, and kurtosis of cervical cancers were significantly lower than those of normal cervical tissues (all P < 0.0001). ADC90% had the largest area under receiver operating characteristic curve of 0.996. CONCLUSIONS:Whole-lesion histogram analysis of ADC maps is useful in the assessment of cervical cancer.

Abstract:The purpose of our study was to evaluate the value of whole-lesion apparent diffusion coefficient (ADC) histogram parameters in distinguishing cervical cancers with different differentiation. A cohort of 18 patients (mean age, 53 years) with biopsy confirmed cervical cancers were involved in this prospective study. All of them underwent 3-T diffusion weighted imaging (DWI) with b values of 0 and 800 s/mm2. ADC histogram parameters were obtained from the entire tumor, including mean ADC values (ADCmean), the percentile ADC values i.e. ADC5%, ADC10%, ADC25%, ADC50%, ADC75%, and ADC90%, skewness, kurtosis, s-sDlowest and s-sDav distribution width. According to Mann-Whitney U-test, the s-sDav and 90th percentile ADC values of well or moderately differentiated cervical cancers were significantly higher than those of poorly differentiated cervical cancers (Both P < 0.05). Whereas the other measurements showed no significant differences. The ROC analysis demonstrated that the area under the ROC (AUC) of s-sDav (0.925) was higher than that of 90th percentile ADC values (0.85). All parameters showed excellent performance in inter-observer stability assessment (all ICC > 0.95). In conclusion, whole-lesion histogram analysis of ADC maps is helpful in discriminating well or moderately differentiated cervical cancers from poorly differentiated cervical cancers.

Abstract:Background: Apparent diffusion coefficient (ADC) histogram analysis has been widely used in determining tumor prognosis.Purpose: To investigate the dynamic changes of ADC histogram parameters during concurrent chemo-radiotherapy (CCRT) in patients with advanced cervical cancers.Material and Methods: This prospective study enrolled 32 patients with advanced cervical cancers undergoing CCRT who received diffusion-weighted (DW) magnetic resonance imaging (MRI) before CCRT, at the end of the second and fourth week during CCRT and one month after CCRT completion. The ADC histogram for the entire tumor volume was generated, and a series of histogram parameters was obtained. Dynamic changes of those parameters in cervical cancers were investigated as early biomarkers for treatment response.Results: All histogram parameters except AUC(low) showed significant changes during CCRT (all P < 0.05). There were three variable trends involving different parameters. The mode, 5th, 10th, and 25th percentiles showed similar early increase rates (33.33%, 33.99%, 34.12%, and 30.49%, respectively) at the end of the second week of CCRT. The pre-CCRT 5th and 25th percentiles of the complete response (CR) group were significantly lower than those of the partial response (PR) group.Conclusion: A series of ADC histogram parameters of cervical cancers changed significantly at the early stage of CCRT, indicating their potential in monitoring early tumor response to therapy.

Abstract:Objective To evaluate the difference of diffusion weighted imaging(DWI) ADC value and rADC value of different degree of differentiation cervical cancer and correlation with cervical cancer cell proliferation activity (PCNA and Ki-67).Methods Forty patients with pathological diagnosis for cervical cancer were selected for this study.Preoperative magnetic resonance imaging(MRI) and DWI was performed before operation.Meanwhile,the PCNA and Ki-67 immunohistochemical staining were performed.The expression of PCNA and Ki-67 in different differentiation degree of cervical cancer were analysed by analysis of variance,and the correlation with ADC and the rADC value were also analysed by Pearson correlation methods.SPSS 20.0 was used to calculated the relevant data and significance was set at ＜ 0.05.Results The ADC values and rADC value in poorly differentiated group is less than high differentiation group,and the difference was statistically significant(t =2.118,P ＜ 0.05).There was no significant difference between the poorly differentiated group and middle differentiated group(P ＞ 0.05).With the degree of differentiation is reduced,the expression of PCNA and Ki-67 is increased.With the increase of the degree of expression of PCNA and Ki-67,ADC values and the rADC value decreased,the difference had statistical significance (P ＜ 0.05).Pearson correlation test showed that ADC values to be inversely associated with the degree of Ki-67 and PCNA expression.Conclusion DWI imaging and the ADC,rADC value measurement for cervical cancer can help to evaluate the cervical cancer with different pathological level and has the potential to be a non-invasive method for identifying tumor proliferation.

Abstract:Objective This study aimed to investigate the application value of apparent diffusion coefficient (ADC) values in evaluating histological type as well as pathologic grade of uterine cervical cancer; and to investigate whether ADC values could reflect tumor cellular density. Methods Ninety-eight patients with histopathologically proven uterine cervical cancer were included in this study. Mean ADC value and minimum ADC value of the tumor were measured. Tumor cellular density was counted using colored multifunction imaging analyzing system. Results Both mean ADC value and minimum ADC value of squamous cell carcinoma were significantly lower than that of adenocarcinoma ( P = 0.001; P = 0.000). Using mean ADC criteria (≤0.965 × 10 −3 mm 2 /s) and minimum ADC criteria (≤0.844 × 10 −3 mm 2 /s), the sensitivity and specificity for differentiating squamous cell carcinoma from adenocarcinoma were 83.5% and 76.9%, and 77.6% and 92.3%, respectively. Receiver operating characteristic analysis revealed that there was no statistically significant difference in the Az values between them ( P = 0.990). Tumor cellular density, mean ADC value, and minimum ADC value of different pathological grade varied significantly ( P = 0.000, P = 0.000, P = 0.000). There was a significant positive linear correlation between tumor cellular density and pathological grade of tumor ( P = 0.000). Both mean ADC value and minimum ADC value correlated negatively with cellular density ( P = 0.000, P = 0.000) and the pathological grade of tumor ( P = 0.000, P = 0.000). Comparisons of correlation coefficients showed no significant differences ( P = 0.656, P = 0.631). Conclusions Diffusion-weighted magnetic resonance imaging has a potential ability to indicate the histologic type of uterine cervical cancer. Apparent diffusion coefficient measurements of uterine cervical cancer can represent tumor cellular density, thus providing a new method for evaluating the pathological grade of tumor.

Abstract:PurposeTo investigate the potential of apparent diffusion coefficient (ADC) histogram parameters in epithelial ovarian cancer (EOC) for distinguishing different tumor stages and determining lymph node status and correlations between ADC values and p53 and Ki-67 expression.ProceduresForty-nine EOC patients underwent preoperative magnetic resonance imaging. Staging and lymph node status were determined postoperatively. ADC values were measured using histogram analysis and compared between groups. Relationships between ADCs and Ki-67 and p53 expression were explored.ResultsDC parameters differed significantly between stage I vs II, I vs III, and I vs IV. The parameters were significantly lower in the lymph node-positive group than in the lymph node-negative group, were significantly negatively correlated with Ki-67 labeling index, and were all significantly lower in the mutation-type p53 group than in the wild-type p53 group.ConclusionsADC histogram analysis can help discriminate stage I from advanced-stage EOC and predict lymph node metastasis. ADC parameters were correlated with Ki-67 labeling index; the parameters may help indicate p53 expression.

Abstract:Abstract Background The aim was to investigate the prognostic value of MR apparent diffusion coefficients (ADC) using histogram analysis (HA) in predicting disease-free survival (DFS) of cervical cancer after chemo-radiation therapy. Methods We retrospectively analyzed 103 women with pathologically proven squamous cell uterine cancer who received chemo-radiation therapy between 2009 and 2013. All patients were followed up for more than 2 years. Pre-treatment MR images were retrieved and imported for HA using an in-house developed software program based on 3D Slicer. Regions of interest of whole tumors were drawn manually on DWI with reference to T2WI. HA features (mean, max, min, 50, 10, 90%, kurtosis, and skewness) were extracted from apparent diffusion coefficient (ADC) maps and compared between the recurrence and non-recurrence groups after the 2-year follow-up. Univariate and multivariate Cox regression analysis was used to correlate ADC HA features and relevant clinical variables (age, grade, maximal diameter of tumor, FIGO stage, SCC-Ag) with DFS. Results One hundred three patients with stage IB-IV cervical cancers were followed up for 2.0–94.6 months (median 48.9 months). Twenty patients developed recurrence within 2 years. In the recurrence group, the min (P = 0.001) and 10% (P = 0.048) ADC values were significantly lower than those of the non-recurrence group. Univariate and multivariate Cox regression analysis revealed that ADCmin (P = 0.006, HR = 0.110) was significantly correlated with DFS. Conclusion Pre-treatment volumetric ADCmin in histogram analysis is an independent factor that is correlated with DFS in cervical cancer patients treated with chemo-radiation therapy.
Analysis of the Early Response to Chemotherapy in Lung Cancer Using Apparent Diffusion Coefficient Single-Slice Histogram

Abstract:Purpose: To evaluate the application of apparent diffusion coefficient (ADC) values derived from diffusion-weighted imaging (DWI) using single-slice histogram analysis to study the chemotherapy responses in lung cancer. Methods: A total of 22 chemotherapy patients with advanced lung cancer from the Nanjing Drum Tower Hospital (Nanjing, China) were included in the study We obtained DWI before and during chemotherapy, performed single-slice histogram analysis of ADC values, and assessed responses after 3 months of chemotherapy. Differences in ADC histogram parameters were compared between the responder and non-responder groups. Results: After therapy, we classified 13 as responders and 9 patients as non-responders The recorded peak ADC value (ADC(peak)) and lowest ADC value (ADC(lowest)) did not show any significant difference in baseline ADC(lowest) and ADC(peak) between responders and non-responders After chemotherapy, 13 responders had significant increase in ADC(lowest) and ADC(peak) compared with pre-treatment values (p < 0.001) ADC(lowest) significantly increased in 9 non-responders (p < 0.05), although ADC(peak) did not significantly increase. ADC(peak) changes were significantly larger in the responder group than in the non-responder group (p = 0.024). ADC(lowest) changes after treatment were larger in the responder group than in the non-responder group, though not significantly. Conclusion: ADC values derived from single-slice histogram analysis may provide a useful and clinically feasible method for monitoring early chemotherapy response in patients with lung cancer.

Abstract:Purpose To assess whether apparent diffusion coefficient (ADC) metrics can be used to assess tumor-infiltrating lymphocyte (TIL) levels in breast cancer, particularly in the molecular subtypes of breast cancer. Methods In total, 114 patients with breast cancer met the inclusion criteria (mean age: 52 years; range: 29–85 years) and underwent multi-parametric breast magnetic resonance imaging (MRI). The patients were imaged by diffusion-weighted (DW)-MRI (1.5 T) using a single-shot spin-echo echo-planar imaging sequence. Two readers independently drew a region of interest (ROI) on the ADC maps of the whole tumor. The mean ADC and histogram parameters (10 th , 25 th , 50 th , 75 th , and 90 th percentiles of ADC, skewness, entropy, and kurtosis) were used as features to analyze associations with the TIL levels in breast cancer. Additionally, the correlation between the ADC values and Ki-67 expression were analyzed. Continuous variables were compared with Student’s t-test or Mann-Whitney U test if the variables were not normally distributed. Categorical variables were compared using Pearson’s chi-square test or Fisher’s exact test. Associations between TIL levels and imaging features were evaluated by the Mann-Whitney U and Kruskal-Wallis tests. Results A statistically significant difference existed in the 10 th and 25 th percentile ADC values between the low and high TIL groups in breast cancer ( P =0.012 and 0.027). For the luminal subtype of breast cancer, the 10 th percentile ADC value was significantly lower in the low TIL group ( P =0.041); for the non-luminal subtype of breast cancer, the kurtosis was significantly lower in the low TIL group ( P =0.023). The Ki-67 index showed statistical significance for evaluating the TIL levels in breast cancer ( P =0.007). Additionally, the skewness was significantly higher for samples with high Ki-67 levels in breast cancer ( P =0.029). Conclusions Our findings suggest that whole-lesion ADC histogram parameters can be used as surrogate biomarkers to evaluate TIL levels in molecular subtypes of breast cancer.

Differentiation of Prostate Cancer and Stromal Hyperplasia in the Transition Zone with Histogram Analysis of the Apparent Diffusion Coefficient

Abstract:Background Prostate cancer and stromal hyperplasia (SH) in the transition zone (TZ) are difficult to discriminate by conventional magnetic resonance imaging (MRI) and diffusion-weighted imaging (DWI). Purpose To investigate the apparent diffusion coefficient (ADC) of prostate cancer and SH in the TZ with histogram analysis and the ability of ADC metrics to differentiate between these two tissues. Material and Methods Thirty-three cancer and 29 SH lesions in the TZ of 54 patients undergoing preoperative DWI (b-value 0, 1000 s/mm2) were analyzed. All the lesions on the MR images were localized based on histopathologic correlations. The 10th, 25th, and 50th percentiles, and the mean ADC values were calculated for the two tissues and compared. The efficiencies of the 10th, 25th, and 50th ADC percentiles in differentiating the two tissues were compared with that of the mean ADC with receiver operating characteristic (ROC) analysis. Results The 10th, 25th, and 50th percentiles and mean ADC values (×10−3 mm2/s) were 0.86 ± 0.15, 0.89 ± 0.16, 0.94 ± 0.16, and 1.03 ± 0.17 in SH and 0.64 ± 0.12, 0.69 ± 0.12, 0.72 ± 0.16, and 0.83 ± 0.15 in TZ cancer, respectively. The parameters were all significantly lower in cancer than SH. The 10th ADC percentile yielded an area under the ROC curve (AUC) of 0.87 for the differentiation of carcinomas from SH, which was higher than the mean ADC (0.80) (P < 0.05), and the AUCs of the 25th (0.82) and 50th (0.83) percentiles exhibited no differences from those of the mean ADC (P > 0.05). Conclusion Histogram analysis of ADC values may potentially improve the differentiation of prostate cancer from SH in the TZ.
The value of apparent diffusion coefficient in the assessment of cervical cancer

Abstract:ObjectivesTo evaluate the potential value of apparent diffusion coefficient (ADC) measurement in the assessment of cervical cancer.MethodsOne hundred twelve patients with cervical cancer and 67 control subjects underwent diffusion-weighted imaging (DWI) in addition to routine MR imaging at 3.0-T MRI before therapy. All ADCs were calculated from b = 0, 600 s/mm2 and b = 0, 1,000 s/mm2.ResultsThe ADCs of cervical cancer were significantly lower than those of normal cervix for both ADC maps. There was a statistically significant difference between the ADCs of well-/moderately differentiated (G1/2) tumours and poorly differentiated (G3) tumours, between the ADCs of squamous cell carcinoma and adenocarcinoma, between the pretherapy ADCs of tumour recurrence or metastasis and tumour free patients after radical hysterectomy for both ADC maps. There was no significant difference among the ADCs of cervical cancer when divided by other features (FIGO, lymph node status, tumour size and age groups) for both ADC maps.ConclusionADC values were reliable for differentiating cervical cancer from normal cervix with high diagnostic accuracy. The ADCs can be used to indicate the degree and histological type of cervical cancer, although there is some overlap. G3 tumours and lower ADCs may indicate poor prognosis. Abstract:OBJECTIVE: To compare the value of whole-lesion apparent diffusion coefficient (ADC) histogram analysis derived from readout-segmented echo-planar imaging (RS-EPI) and single-shot echo-planar imaging (SS-EPI) diffusion-weighted imaging (DWI) in evaluating normal-sized lymph node metastasis (LNM) in cervical cancer.METHODS: Seventy-six pathologically confirmed cervical cancer patients (stages IB and IIA) were enrolled, including 61 patients with non-LNM (group A) and 15 patients with normal-sized LNM (group B). The recorded tumor volume on T2-weighted imaging was the reference against which both DWIs were evaluated. Each ADC histogram parameter (including ADC max , ADC 90 , ADC median , ADC mean , ADC 10 , ADC min , ADC skewness , ADC kurtosis , and ADC entropy ) was compared between SS-EPI and RS-EPI and between the 2 groups.RESULTS: There was no significant difference in tumor volume between the 2 DWIs and T2-weighted imaging (both P > 0.05). Higher ADC max and ADC entropy but lower ADC 10 , ADC min and ADC skewness were found in SS-EPI than those in RS-EPI (all P < 0.05). For SS-EPI, lower ADC 90 and higher ADC kurtosis were found in group B than those in group A (both P < 0.05). For RS-EPI, lower ADC 90 and higher ADC kurtosis and ADC entropy were found in group B than those in group A (all P < 0.05). Readout-segmented echo-planar imaging ADC kurtosis showed the highest area under the curve of 0.792 in the differentiation of the 2 groups (sensitivity, 80%; specificity, 73.77%).CONCLUSIONS: Compared with SS-EPI, the ADC histogram parameters derived from RS-EPI were more accurate, and ADC kurtosis held great potential in differentiating normal-sized LNM in cervical cancer.

Abstract:Objectives. To evaluate the value of the whole volume apparent diffusion coefficient (ADC) histogram in distinguishing between benign and malignant breast lesions and differentiating different molecular subtypes of breast cancers and to assess the correlation between ADC histogram parameters and Ki-67 expression in breast cancers. Methods. The institutional review board approved this retrospective study. Between September 2016 and February 2019, 189 patients with 84 benign lesions and 105 breast cancers underwent magnetic resonance imaging (MRI). Volumetric ADC histograms were created by placing regions of interest (ROIs) on the whole lesion. The relationships between the ADC parameters and Ki-67 were analysed using Spearman’s correlation analysis. Results. Of the 189 breast lesions included, there were significant differences in patient age ( P < 0.001 ) and lesion size ( P = 0.006 ) between the benign and malignant lesions. The results also demonstrated significant differences in all ADC histogram parameters between benign and malignant lesions (all P < 0.001 ). The median and mean ADC histogram parameters performed better than the other ADC histogram parameters (AUCs were 0.943 and 0.930, respectively). The receiver operating characteristic (ROC) analysis revealed that the 10th percentile ADC value and entropy could determine the human epidermal growth factor receptor 2 (HER-2) status (both P = 0.001 ) and estrogen receptor (ER)/progesterone receptor (PR) status ( P = 0.020 and P = 0.041 , respectively). Among all breast cancer lesions, 35 tumours in the low-proliferation group ( Ki − 67 < 14 % ) and 70 tumours in the high-proliferation group ( Ki − 67 ≥ 14 ) were analysed with ROC curves and correlation analyses. The ROC analysis revealed that entropy and skewness could determine the Ki-67 status ( P = 0.007 and P < 0.001 , respectively), and there were weak correlations between ADC entropy ( r = 0.383 ) and skewness ( r = 0.209 ) and the Ki-67 index. Conclusion. The volumetric ADC histogram could serve as an imaging marker to determine breast lesion characteristics and may be a supplemental method in predicting tumour proliferation in breast cancer.

Abstract:AIM: To explore the value of whole-lesion apparent diffusion coefficient (ADC) first-order statistics and texture features in clinical staging of cervical cancers. MATERIALS AND METHODS: Fifty-six women (mean age, 51 years) with histopathologically confirmed cervical cancers underwent 3 T pelvic magnetic resonance imaging including diffusion-weighted imaging (b = 0, 800 s/mm(2)) prospectively. The ADC first-order statistics and texture features derived from the whole volume of cervical cancers were correlated with International Federation of Gynecology and Obstetrics (FIGO) stages (i.e., stages I, II, III, and IV). RESULTS: The first-order statistics of skewness, kurtosis, and entropy, and the texture features of entropy(H) and homogeneity correlated positively, while the texture feature of energy correlated negatively with FIGO stages (all p < 0.05). Skewness, kurtosis, entropy, entropy(H), and homogeneity were significantly higher, while energy was significantly lower in cervical cancers at higher (IIB-IVA) than lower (IB-IIA) FIGO stages (all p < 0.05). Kurtosis and energy had the largest areas under the receiver operating characteristic (ROC) curve of 0.749 and 0.746 in differentiating cervical cancers at lower (IB-IIA) from higher (IIB-IVA) FIGO stages. CONCLUSION: Whole-lesion ADC first-order statistics and texture features proved relevant and meaningful in the clinical staging of cervical cancers. (C) 2017 The Royal College of Radiologists. Abstract:Abstract Background The present study aims to investigate the role of histogram analysis of intravoxel incoherent motion (IVIM) in the differential diagnosis of benign and malignant breast lesions. Methods The magnetic resonance imaging and clinical data of 55 patients (63 lesions) were retrospectively analyzed. The multi-b-valued diffusion-weighted imaging image was processed using the MADC software to obtain the gray-scaled maps of apparent diffusion coefficient (ADC)-slow, ADC-fast and f. The MaZda software was used to extract the histogram metrics of these maps. Combined with the conventional sequence images, the region of interest (ROI) was manually drawn along the edge of the lesion at the maximum level of the gray-scale image, and the difference of the data was analyzed between the benign and malignant breast lesions. Results There were 29 patients with 37 benign lesions, which included 23 fibroadenomas, 6 adenosis, 1 breast cysts, 4 intraductal papillomas, and 3 inflammations of breast. Furthermore, 26 malignant lesions in 26 patients, which included 20 non-specific invasive ductal carcinomas, 5 intraductal carcinomas and 1 patient with squamous cell carcinoma. The ADC-slow (mean and the 50th percentile) and f (minimum, mean, kurtosis, the 10th percentile and 50th percentile) of these malignant breast lesions were significantly lower than those of benign lesions ( P < 0.05), while ADC-fast (kurtosis) and f (variance, skewness) of these malignant breast lesions were significantly higher than those of benign lesions ( P < 0.05). Conclusion The histogram analysis of ADC-slow (mean and the 50th percentile), ADC-fast (kurtosis) and f (minimum, mean, kurtosis, the 10th percentile and 50th percentile. Variance, skewness) can provide a more objective and accurate basis for the differential diagnosis of benign and malignant breast lesions.

Abstract:OBJECTIVE:To assess the value of stretched-exponential and mono-exponential diffusion-weighted imaging (DWI) in predicting the aggressiveness of PCa.METHODS:This retrospective study included 36 cases of PCa with 48 lesions in the peripheral zone diagnosed by DWI with b-values of 0, 500, 1000, and 2000 s/mm2. We reconstructed the apparent diffusion coefficient (ADC), distributed diffusion coefficient (DDC) and α maps on the post-processing workstation, performed a histogram analysis on the largest slice of PCa on T2WI and Spearman's rank-order analysis on the correlation of the histogram variables with Gleason grade grouping (GG). Then, we assessed the values of the histogram variables in differentiating low-grade from high-grade PCa using the receiver operating characteristic (ROC) curve.RESULTS:The percentile and mean ADCs and DDCs were correlated with GG (ρ: 0.392－0.641) but not the α value, skewnesses and kurtosises (ρ: 0.055－0.266). High-grade PCa exhibited significantly lower 10th-, 25th-, 50th- and 75th-percentile and mean ADCs (490 ± 141, 591 ± 137, 695 ± 137, 781 ± 139 and 888 ± 135 mm2/s) and DDCs (420 ± 146, 534 ± 167, 666 ± 182, 787 ± 190 and 912 ± 175 mm2/s) than low-grade PCa (ADCs: 636 ± 74, 727 ± 86, 825 ± 85, 907 ± 85 and 975 ± 117 mm2/s; DDCs: 542 ± 80, 666 ± 93, 806 ± 108, 910 ± 110 and 1023 ± 105 mm2/s), but there were no statistically significant differences between low- and high-grade PCa in the α value (0.67 ± 0.042 vs 0.64 ± 0.036), kurtosises (ADC 0.105 vs 0.078; DDC －0.027 vs －0.401) or skewnesses (ADC －0.042 vs 0.067; DDC －0.058 vs 0.162). Both 10th-percentile ADCs and DDCs showed a higher efficiency than the mean ones in differentiating high- from low-grade PCa, though with no statistically significant difference (P > 0.05).CONCLUSIONS:Histogram variables DDCs and ADCs, rather than the α value, can be used to predict the aggressiveness of PCa, even more efficiently at the 10th percentile than on the mean.

Abstract:Objective To investigate the efficacy of histogram analysis of the whole-volume in apparent diffusion coefficient (ADC) for T and N staging in rectal cancer. Methods Thirty-five patients were enrolled and underwent preoperative DWI exam-ination. Tumors were delineated on each slice of the ADC maps. A series of histogram parameters were calculated and correlated with T and N staging by Spearman correlation analysis. The diagnostic performance of each parameter for distinguishing T and N staging was assessed by using the area under the receiver operating characteristic curve(AUC). Repeatability test was used to as-sess the repeatability between observers. Results There were significant correlations between histogram parameters (Histogram width and Skew) and T-staging ( P =0.036,r =0.355;P =0.033,r =0.361). Among all the histogram parameters, histo-gram width and Skew had larger AUC value in differentiating T3from T1/T2tumor, which were 0.716 and 0.720 respectively. However,we didn't find any positive result in whether lymph node metastasis. The intraclass correlation coefficient of 75% histo-gram parameters were more than 0.8 in repeatability test. Conclusion The repeatability of most histogram parameters in rectal cancer is high. Histogram width and Skew correlated significantly with T staging.

Abstract:PURPOSE: We aimed to evaluate the role of histogram analysis of apparent diffusion coefficient (ADC) maps for differentiating benign and malignant orbital tumors.METHODS: Fifty-two patients with orbital tumors were enrolled from March 2013 to November 2014. Pretreatment diffusion-weighted imaging was performed on a 3T magnetic resonance scanner with b factors of 0 and 800 s/mm2, and the corresponding ADC maps were generated. Whole-tumor regions of interest were drawn on all slices of the ADC maps to obtain histogram parameters, including ADCmean, ADCmedian, standard deviation (SD), skewness, kurtosis, quartile, ADC10, ADC25, ADC75, and ADC90. Histogram parameter differences between benign and malignant orbital tumors were compared. The diagnostic value of each significant parameter in predicting malignant tumors was established.RESULTS: Age, ADCmean, ADCmedian, quartile, kurtosis, ADC10, ADC25, ADC75, and ADC90 parameters were significantly different between benign and malignant orbital tumor groups, while gender, location, SD, and skewness were not significantly different. The best diagnostic performance in predicting malignant orbital tumors was achieved at the threshold of ADC10=0.990 (AUC, 0.997; sensitivity, 96.2%; specificity, 100%).CONCLUSION: Histogram analysis of ADC maps holds promise for differentiating benign and malignant orbital tumors. ADC10 has the potential to be the most significant parameter for predicting malignant orbital tumors.

Abstract:BACKGROUND: The role of apparent diffusion coefficient (ADC)-based radiomics features in evaluating histopathological grade of cervical cancer is unresolved.PURPOSE: To determine if there is a difference between radiomics features derived from center-slice 2D versus whole-tumor volumetric 3D for ADC measurements in patients with cervical cancer regarding tumor histopathological grade, and systematically assess the impact of the b value on radiomics analysis in ADC quantifications.STUDY TYPE: Prospective.SUBJECTS: In all, 160 patients with histopathologically confirmed squamous cell carcinoma of uterine cervix.FIELD STRENGTH/SEQUENCE: Conventional and diffusion-weighted MR images (b values = 0, 800, 1000 s/mm2 ) were acquired on a 3.0T MR scanner.ASSESSMENT: Regions of interest (ROIs) were drawn manually along the margin of tumor on each slice, and then the center slice of the tumor was selected with naked eyes in the course of whole-tumor segmentation. A total of 624 radiomics features were derived from T2 -weighted images and ADC maps. We randomly selected 50 cases and did the reproducibility analysis.STATISTICAL TESTS: Parameters were compared using Wilcoxon signed rank test, Bland-Altman analysis, t-test, and least absolute shrinkage and selection operator (LASSO) regression with crossvalidation.RESULTS: In all, 95 radiomics features were insensitive to ROI variation among T2 images, ADC map of b800, and ADC map of b1000 (P > 0.0002). There was a significant statistical difference between the performances of 2D center-slice and 3D whole-tumor radiomics models in both ADC feature sets of b800 and b1000 (P < 0.0001, P < 0.0001). Compared with ADC features of b800 (0.3758 ± 0.0118), the model of b1000 ADC features appeared to be slightly lower in overall misclassification error (0.3642 ± 0.0162) (P = 0.0076).DATA CONCLUSION: Several radiomics features extracted from T2 images and ADC maps were highly reproducible. Whole-tumor volumetric 3D radiomics analysis had a better performance than using the 2D center-slice of tumor in stratifying the histological grade of cervical cancer. A b value of 1000 s/mm2 is suggested as the optimal parameter in pelvic DWI scans.LEVEL OF EVIDENCE: 1 Technical Efficacy: Stage 1 J. Magn. Reson. Imaging 2019;49:280-290.

Abstract:Purpose Accurate histologic grade assessment is helpful for clinical decision making and prognostic assessment of sinonasal squamous cell carcinoma (SNSCC). This research aimed to explore whether whole-tumor histogram analysis of apparent diffusion coefficient (ADC) maps with machine learning algorithms can predict histologic grade of SNSCC. Methods One hundred and forty-seven patients with pathologically diagnosed SNSCC formed this retrospective study. Sixty-six patients were low-grade (grade I/II) and eighty-one patients were high-grade (grade III). Eighteen histogram features were obtained from quantitative ADC maps. Additionally, the mean ADC value and clinical features were analyzed for comparison with histogram features. Machine learning algorithms were applied to build the best diagnostic model for predicting histological grade. The receiver operating characteristic (ROC) curve was used to evaluate the performance of each model prediction, and the area under the ROC curve (AUC) were analyzed. Results The histogram model based on three features (10th Percentile, Mean, and 90th Percentile) with support vector machine (SVM) classifier demonstrated excellent diagnostic performance, with an AUC of 0.947 on the testing dataset. The AUC of the histogram model was similar to that of the mean ADC value model (0.947 vs 0.957; P = 0.7029). The poor diagnostic performance of the clinical model (AUC = 0.692) was improved by the combined model incorporating histogram features or mean ADC value ( P < 0.05). Conclusion ADC histogram analysis improved the projection of SNSCC histologic grade, compared with clinical model. The complex histogram model had comparable but not better performance than mean ADC value model.
