Abstract:Objective To investigate the variation of anti-coagulation and fibrinolysis function in orthotopic liver transplantation(OLT).Methods The anti-coagulation and fibrinolytic function were assessed systematically in 14 cases of OLT.Results The anti-coagulation function increased at first,then reduced gradually and recovered at last.The fibrinolytic function increased gradually during operation,reached the highest level in the reinfusion and 1 hour after reinfusion.Conclusions The detections of anti-coagulation and fibrinolysis function in OLT and the effective measures to rectify anti-coagulation and fibrinolysis function by monitoring the changes timely and accurately in OLT.

Abstract:Objective To investigate the clinical implications of portal vein bloodletting immediately before reperfusion during orthotopic liver transplantation(OLT).Methods Thirty-two patients with end-stage liver diseases undergoing non veno-venous OLT were divided into bloodletting group (n=21)and control group(n=11).During anhepatic phase,we maintained mean arterial pressure ＞70 mm Hg,cardiac index＞2.5 L·min-1·m-2 by infusion,norepinephrine and dopamine.Blood samples were taken at the time when portal vein was clamped(T1),the time when portal vein was unclamped (T2),10 minutes after neohepatic phase(T3),neohepatic phase 30 minutes(T4)for electrolytes,blood gas and plasma inflammatory cytokines.Hemodynamic and ventilation parameters were also recorded.Results There was no significant difference in mortality(X2=1.12,P＞0.05)and arrhythmia incidence (X2=1.73,P＞0.05)between the two groups.Serum calcium,magnesium were both significantly lower than normal.After anhepatic phase,potassium,tumor necrosis factor alpha,interleukin-6 in radial artery didn't alter significantly;Bloodletting had no effect on lactic acid.There was no significant difference in hemodynamic and ventilation parameters among four time periods.Conclusion Bloodletting seemed to have no effect on changes of internal environment.
Preconditioning and Postconditioning Reduce Hepatic Ischemia-Reperfusion Injury in Rats

Abstract:BACKGROUND:Ischemia-reperfusion injury occurs when ischemic tissues or organs suffer from further functional and structural damage when their blood supply recovers. This study aimed to contrast the protective effects of ischemic preconditioning and ischemic postconditioning in hepatic ischemia-reperfusion injury in rats.METHODS:Thirty-two healthy male Wistar rats were randomly divided into four groups: sham-operated (SO), ischemia-reperfusion (IR), ischemic preconditioning (I-pre), and ischemic postconditioning (I-post). Blood samples and hepatic tissue were taken from all groups after the experiments.RESULTS:There were significant differences between the IR, I-pre and I-post groups in alanine aminotransferase and aspartate aminotransferase levels, NF-kappaB p65 expression, apoptosis index and superoxide dismutase activity in hepatic tissue. There were no significant differences between the I-pre and I-post groups.CONCLUSIONS:Ischemic postconditioning and ischemic preconditioning reduce hepatic ischemia-reperfusion injury, but in clinical practice the former is a more appropriate choice.
Effects of liver ischemia-reperfusion injury on respiratory mechanics and driving pressure during orthotopic liver transplantation

Abstract:Background: During orthotopic liver transplantation (OLT), liver graft ischemia-reperfusion injury (IRI) triggers a cytokine-mediated systemic inflammatory response, which impairs graft function and disrupts distal organ homeostasis. The objective of this prospective, observational trial was to assess the effects of IRI on lung and chest wall mechanics in the intraoperative period of patients undergoing OLT. Methods: In 26 patients undergoing OLT, we measured elastance of the respiratory system (ERS), partitioned into lung (EL) and chest wall (ECW), hemodynamics, and fluid and blood product intake before laparotomy (T1), after portal/caval surgical clamp (T2), and immediately (T3) and, at 90 and 180 minutes post-reperfusion (T4 and T5, respectively). Interleukin-6 (IL-6), monocyte chemotactic protein-1 (MCP-1), IL-1β and tumor necrosis factor-α plasma concentrations were assessed at T1, T4 and T5. Results: EL significantly decreased from T1 to T2 (13.5±4.4 vs 9.7±4.8 cmH2O/L, P<0.05), remained stable at T3, while at T4 (12.3±4.4 cmH2O/L, P<0.05) was well above levels recorded at T2, reaching its highest value at T5 (15±3.9 cmH2O/L, P<0.05). Variations in ERS, EL, driving pressure (∆P) and trans-pulmonary pressure (∆PL) significantly correlated with changes in IL-6 and MCP-1 plasma concentrations, but not with changes in wedge pressure, fluid amounts, and red blood cells and platelets administered. No correlation was found between changes in cytokine concentrations and ECW. Conclusions: We found that EL, ECW, ∆P and ∆PL underwent significant variations during the OLT procedure. Further, we documented a significant association between the respiratory mechanics changes and the inflammatory response following liver graft reperfusion.
Effects of Different Ways of Orthotopic Liver Transplantation without Venovenous Bypass on Anesthetic Management

Abstract:Objective To compare the effects of piggyback and classic orthotopic liver transplantation (OLT) without venovenous bypass on anesthetic management. Methods A retrospective review was conducted on the clinical data of 50 cases of OLT without venovenous bypass performed from November 2003 to December 2006, including 25 piggyback (group A) and 25 classic (group B) OLT. The perioperative hemodynamics, hemostatic conditions, hepatic and renal functions, changes of internal environment, and fluid therapy were compared between the two groups. Results At the beginning of anhepatic stage, the mean arterial pressure (MAP), central venous pressure (CVP), and cardiac index (CI) in the group A were significantly higher than those in the group B [MAP: (68±6) mm Hg vs (64±5) mm Hg; t=2.561, P=0.014. CVP: (5.4±3.3) mm Hg vs (3.5±2.3) mm Hg; t=2.362, P=0.022. CI: (3.7±0.8) L· min-1· m-2 vs (3.2±0.6) L· min-1· m-2;t=2.500, P= 0.016]. At the beginning of neohepatic phase, the MAP in the group A was significantly higher than that in the group B [(66±6) mm Hg vs (62±5) mm Hg; t=2.561,P=0.014], while the CVP was similar between the two groups [(8.4±4.0) mm Hg vs (10.6±4.2) mm Hg; t=-1.897,P=0.064]. The morbidity rate of reperfusion syndrome in the group A was significantly lower than that in the group B (8.0% vs 32.0%, χ2=4.500, P=0.034).At the early neohepatic stage, the blood pH and base excess (BE) values in the group A were significantly higher than those in the group B [pH: 7.34±0.05 vs 7.31±0.04; t=2.343, P=0.023. BE: (-4.9±2.3) mmol /L vs (-6.3±2.3) mmol /L; t=2.152, P=0.036].No significant differences were detected in the hemostatic states, hepatic and renal functions, intraoperative blood loss, urine output, fluid replacement, and component transfusion during OLT between the two groups (P>0.05). The requirement of vasoactive drugs in the group A was significantly smaller than that in the group B (P<0.05). Conclusions The hemodynamics and acid-base balance are relatively stable in the piggyback OLT compared to the classic OLT without venovenous bypass. Monitoring and regulation of hemodynamics, internal environment, coagulation state, hepatic and renal functions, and fluid administration should be emphasized during the OLT by using both the techniques.

Abstract:Increasing evidence has shown that reactive oxygen species (ROS) are important mediators in liver ischemia/reperfusion injury(IRI). ROS include hydrogen peroxide (H(2)O(2)), superoxide anion (O(-2)), and hydroxyl radical (HO(-)), which may be generated by activated Kupffer cells in the liver, contributing to reperfusion injury. Hepatic IRI is a multistep process that damages liver graft function. To establish a series of therapeutic strategies to improve the outcome of liver transplantation, a good understanding of the mechanisms of IRI is essential. However, the detail mechanisms of how ROS lead to hepatocyte damage in IRI remains unclear. The aim of this review was to describe recent developments in the field of oxidative stress research. The first part of this review focused on the key roles and possible mechanisms of ROS in hepatic IRI. The second part of this review summarizes some findings including novel and classic antioxidant methods to ameliorate the hepatocyte damage during IRI.
Intracerebral Hemorrhage after Liver Transplantation

Abstract:We report 5 patients with intracerebral hemorrhage after orthotopic liver transplantation (OLT) and identify the possible risk factors. Between November 1991 and April 1999, 75 adult patients received 77 orthotopic liver transplants at Queen Mary Hospital, Hong Kong. Five patients (6.5%) developed intracerebral hemorrhage postoperatively. Clinical and laboratory data were reviewed, and potential risk factors were analyzed. The 5 patients developed intracerebral hemorrhage within 40 days (range, 1 to 37 days; median, 4 days) after OLT. The mortality rate was 80% (4 of 5 patients). The intraoperative blood transfusion volume (median, 17,200 mL; range, 15,750 to 30,360 mL) administered to patients who developed intracerebral hemorrhage postoperatively was significantly greater than that (median, 6,990 mL; range, 1,840 to 22,680 mL) for patients without the complication (P =.0008). Massive intraoperative transfusion (>15,000 mL) was required in all 5 patients (100%) with intracerebral hemorrhage but only 9 of 72 patients (12.5%) in the other group (P =.0001). Four of 5 patients (80%) with intracerebral hemorrhage had intraoperative hypotension compared with 7 of 72 patients (9.7%) in the other group (P =.001). No significant difference was found in age, prothrombin time (PT), activated partial thromboplastin time (APTT), incidence of hypertension, bleeding at extracerebral sites, cyclosporine A neurotoxicity, thrombocytopenia, hemodialysis, and sepsis between the patients with and without intracerebral bleeding. However, the median cumulative score of coagulation parameters (PT, APTT, platelet count) was significantly greater in the group with than without intracerebral bleeding (median score, 3 v 1; P =.023). Intracerebral hemorrhage is 1 of the most disastrous complications after OLT. Intraoperative hypotension, massive intraoperative transfusion, and coagulopathy may be correlated with this complication.
Changes of blood coagulation/fibrinolysis and its influential factors in orthotopic liver transplantation

Abstract:Objective To investigate intraoperative variations in blood coagulation/fibrinolysis and its influential factors in orthotopic liver transplantation(OLT).Methods Eighty-six patients undergoing orthotopic liver transplantation were divided into three groups:chronic hepatic cirrhosis(CHC)group,hepatocellular carcinoma(HCC)group,and fulminant liver failure(FLF).The coagulant,fibrinolytic function,blood routine and biochemical parameters were determined before and during operation.Change of every value reflecting blood coagulation function and acid-base metabolism were observed in all groups.Perioperative influential factors of blood coagulation/fibrinolysis and their relations were analyzed.Results The average level of blood coagulation in CHC group was between HCC group and FLF group,and the average blood platelet count reduced to 35.43×109/L,less than in HCC and FLF groups significantly(P<0.05).Compared with the values before operation,prothrombin time(PT),activated partial thromboplastin time(APTT),international normalized ratio(INR)gradually prolonged in CHC group,then peaked at 30 min after reperfusion,higher than in any other period of the operation(P<0.05),while fibrinogen(FIB)level got lowest in anheptic stage(P<0.05).The average blood coagulation states in HCC group were better than in CHC and FLF groups groups during whole operation.Their relative parameters in HCC group were almost normal before living donor liver transplantation(LDLT)and the change during OLT was less than that of HCC and FLF groups(P<0.05).Coagulation state in FLF group was worst with prolonged PT,INR and reduced FIB(P<0.05).However,these patients recovered in a short time after reperfusion.Except anheptic period,FIB level in every period was lower in FLF group than in HCC and CHC groups(P<0.05).Hemoglobin(Hb),albumin and TCO2 in three groups declined significantly in anhepatic period.The t-PA and D-D level gradually increased during OLT and peaked at 1 h after reperfusion(P<0.05).MELD score was negatively correlated with intraoperative bleeding and blood transfusion(r=0.638,P<0.05).Conclusion Blood coagulation disorder occurs mainly in anhepatic and early neohepatic periods.Changes of blood coagulation function and acid-base metabolism are different with different characteristics of primary diseases in hepatic transplantation.Coagulopathy should be treated individually according their characteristics to reduce intraoperative bleeding and acid-base metabolism disorder and to be good for recovery smoothly.
Functional Changes of the Heart and Lung in Perioperative Period of Orthotopic Liver Transplantation.

Abstract:OBJECTIVETo investigate the variation of functions of the heart and lung during orthotopic liver transplantation (OLT).METHODSPulmonary artery cannula and right radial artery cannula were indewelled before anaesthesia and the parameters of hemodynamics in different periods were monitored. Analysis of variance was used to reveal the variation among the groups. T hypothesis test in paired data was used to compare the preoperative parameters with those in each period during operation and after operation respectively, and to compare the parameters immediately after operation with those in each period after operation respectively.RESULTSDuring the operation, heart rate increased, but blood pressure decreased significantly at the beginning of no-liver period, increased again in a short period and then increased progressively 12 h after operation. Pulmonary artery pressure (PAP) increased from before the no-liver period to 60 h after operation. Pulmonary wedge pressure changed in accordance with the variation of PAP. Cardiac output was maintained at a high-output level from before operation to 60 h after operation. Systemic vascular resistance (SVR) was within the normal limits before operation, whereas pulmonary vascular resistance (PVR) was lower than normal. In the no-liver period during the operation, SVR decreased significantly. Both SVR and PVR increased progressively and returned to normal postoperatively.CONCLUSIONSThe patients undergoing OLT have a high cardiac output and low resistance obstacle before and during the operation, and will recover gradually after operation. Monitoring hemodynamics during the peri-operative period is of significance in the prevention and treatment of pneumonedema and cardiac functional insufficiency.

Abstract:Steatotic livers are particularly vulnerable to ischemia/reperfusion (I/R) injury, resulting in poor outcomes following liver surgery and transplantation. Therapeutic approaches for I/R injury in steatotic livers are currently under intensive investigation. This review summarizes and discusses the approaches developed during the last few years to prevent hepatic I/R injury in steatotic livers. Among the proposed approaches, ischemic preconditioning and intermittent clamping are the two most promising approaches that have been applied in some clinical centers for liver surgery and transplantation, but most of others have not reached clinical application yet.

Abstract:Objective:To study the effect of oxygen free radicals on hepatic ischemia-reperfusion injury during liver transplantation.Methods:Plasma LPO,SOD,TAC and NO of18patients were measured preop-eratively,an hepatic phase and1hour after reperfusion.Results:The plasma LPO and TAC declined,SOD and NO elevated after reperfusion.Conclusion:Oxygen free radicals play an important role in hepatic ischemia-reperfusion injury during liver transplantation.After reperfusion,protective mechanism was activated.Blood transfusion and some drug may alleviate the injury caused by free radicals and protect body.

Early allograft dysfunction occurred in two recipients (6%) randomized to IFLT and in eight (24%) randomized to CLT (difference -18%; 95% CI -35% to -1%; p = 0.044). Post-reperfusion syndrome occurred in three recipients (9%) randomized to IFLT and in 21 (64%) randomized to CLT (difference -54%; 95% CI -74% to -35%; p &lt;0.001). Non-anastomotic biliary strictures diagnosed with protocol magnetic resonance cholangiopancreatography at 12 months were observed in two recipients (8%) randomized to IFLT and in nine (36%) randomized to CLT (difference, -28%; 95% CI -50% to -7%; p = 0.014). Abstract:Abstract Cardiovascular instability is common during the reperfusion phase of orthotopic liver transplantation (OLT), and some patients experience a postreperfusion syndrome (PRS). However, there are no reports comparing the cardiac dysfunction between patients with PRS and those without. Thus, the aim of this study was to evaluate cardiac dysfunction in patients exhibiting PRS. This observational retrospective study included 34 patients who underwent OLT and were monitored with transesophageal echocardiography (TEE). The right ventricular/left ventricular (RV/LV) end diastolic area, tricuspid annular plane systolic excursion (TAPSE), left ventricular ejection fraction (LVEF) by Simpson method, pulsed Doppler of the mitral valve, and tissue Doppler motion of the mitral annulus were determined. Echocardiographic measurements were registered at the beginning of surgery and at 1 and 30 min after vascular unclamping. Patients with PRS (PRS group) were identified, and their echocardiographic parameters of ventricular function were compared with those in patients without PRS (non-PRS group). To check the evolution of diastolic-systolic dysfunction, general linear model-repeated measures were estimated. No patient presented systolic/diastolic dysfunction on the basal echocardiogram. One minute after vascular unclamping, the incidence of RV dilation was 4.5 times greater in patients with PRS ( Cramer ́s V > 0.6), and the incidence of RV systolic dysfunction was 62.5% in patients with PRS compared to 15.40% in patients without PRS ( Cramer ́s V = 0.45). The incidence of LV systolic dysfunction was 25% in patients with PRS compared to 0% in those without ( Cramer ́s V = 0.45), and left ventricular diastolic dysfunction was 4.8 times greater in patients with PRS ( Cramer ́s V = 0.45). No patient presented diastolic dysfunction type III. There were significant differences between groups in the evolutionary pattern at 1 and 30 min after unclamping for RV dilation ( p = 0.008) and for TAPSE ( p = 0.014). Liver graft reperfusion may alter cardiac function. Cardiac dysfunction was more frequent in patients with PRS. These patients exhibited temporary dysfunction of the RV associated with a varying degree of LV diastolic-systolic dysfunction. Trial registration : clinicaltrials.gov (NCT05175534). Abstract:Orthotopic liver transplantation (OLT) is the most effective treatment for patients with end-stage liver disease (ESLD). Hepatic insufficiency within a week of OLT, termed early allograft dysfunction (EAD), occurs in 20% to 25% of deceased donor OLT recipients and is associated with morbidity and mortality. Primary nonfunction (PNF), the most severe form of EAD, leads to death or retransplantation within 7 days. The etiology of EAD is multifactorial, including donor, recipient, and surgery-related factors, and largely driven by ischemia-reperfusion injury (IRI). IRI is an immunologic phenomenon characterized by dysregulation of cellular oxygen homeostasis and innate immune defenses in the allograft after temporary cessation (ischemia) and later restoration (reperfusion) of oxygen-rich blood flow. The rising global demand for OLT may lead to the use of marginal allografts, which are more susceptible to IRI, and thus lead to an increased incidence of EAD. It is thus imperative the anesthesiologist is knowledgeable about EAD, namely its pathophysiology and intraoperative strategies to mitigate its impact. Intraoperative strategies can be classified by 3 phases, specifically donor allograft procurement, storage, and recipient reperfusion. During procurement, the anesthesiologist can use pharmacologic preconditioning with volatile anesthetics, consider preharvest hyperoxemia, and attenuate the use of norepinephrine as able. The anesthesiologist can advocate for normothermic regional perfusion (NRP) and machine perfusion during allograft storage at their institution. During recipient reperfusion, the anesthesiologist can optimize oxygen exposure, consider adjunct anesthetics with antioxidant-like properties, and administer supplemental magnesium. Unfortunately, there is either mixed, little, or no data to support the routine use of many free radical scavengers. Given the sparse, limited, or at times conflicting evidence supporting some of these strategies, there are ample opportunities for more research to find intraoperative anesthetic strategies to mitigate the impact of EAD and improve postoperative outcomes in OLT recipients.

Abstract:Hemorrhage is a common complication in the early postoperative period after orthotopic liver transplantation (OLT) and surgical reintervention may be necessary. We sought to assess the incidence as well as to identify potential risk factors for bleeding requiring surgical reintervention in the early postoperative period. From January 2003 to December 2005, we retrospectively reviewed the courses of 261 patients who underwent OLT. We analyzed the pretransplantation parameters, transplantation features, and clinical data for surgical reintervention due to early postoperative hemorrhage. Twenty-two of 261 patients (8.4%) had early postoperative hemorrhage requiring urgent surgical reintervention during the initial hospital stay. In-hospital mortality of the patients with hemorrhage (9/22; 41%) was significantly higher than that of other patients (29/239; 12.1%; P < .001). The surgical problem was the main cause of hemorrhage (18/22; 81.8%). More intraoperative blood transfusions were necessary for patients with hemorrhage than for other patients. Furthermore, a greater number of blood transfusions, including red blood cells, plasma, and platelet concentrates, during the transplantation procedure correlated with a greater mortality. In conclusion, early postoperative hemorrhage requiring urgent surgical reintervention is a severe complication with a high mortality. It is mainly caused by errors in surgical technique. Blood transfusion during transplantation was correlated with a higher mortality.
Impact of Intra-Operative Cholangiography and Parenchymal Resection to Donor Liver Function in Living Donor Liver Transplantation

Abstract:BACKGROUND:Living donor liver transplantation (LDLT) has been widely accepted over the past decade, and hepatic dysfunction often occurs in the donor in the early stage after liver donation. The present study aimed to evaluate the effect of intra-operative cholangiography (IOC) and parenchymal resection on liver function of donors in LDLT, and to assess the role of IOC in influencing the biliary complications and improving the overall outcome. METHODS:Data from 40 patients who had donated their right lobes for LDLT were analyzed. Total bilirubin (TB), alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP) and gamma-glutamyl transpeptidase (GGT) at different time points were compared, and the follow-up data and the biliary complications were also analyzed. RESULTS:The ALT and AST values were significantly increased after IOC (P<0.001) and parenchymal resection (P<0.001). However, the median values of TB, ALP and GGT were not significantly influenced by IOC (P>0.05) or parenchymal resection (P>0.05). The biochemical changes caused by IOC or parenchymal resection were not correlated with the degree of post-operative liver injury or the recovery of liver function. The liver functions of the donors after operation were stable, and none of the donors suffered from biliary stenosis or leakage during the follow-up. CONCLUSIONS:IOC and parenchymal resection may induce a transient increase in liver enzymes of donors in LDLT, but do not affect the recovery of liver function after operation. Moreover, the routine IOC is helpful to clarify the division line of the hepatic duct, thus reducing the biliary complication rate.
The Impact of Intraoperative Transfusion of Platelets and Red Blood Cells on Survival After Liver Transplantation

Abstract:BACKGROUND: Intraoperative transfusion of red blood cells (RBC) is associated with adverse outcome after orthotopic liver transplantation (OLT). Although experimental studies have shown that platelets contribute to reperfusion injury of the liver, the influence of allogeneic platelet transfusion on outcome has not been studied in detail. In this study, we evaluate the impact of various blood products on outcome after OLT.METHODS: Twenty-nine variables, including blood product transfusions, were studied in relation to outcome in 433 adult patients undergoing a first OLT between 1989 and 2004. Data were analyzed using uni- and multivariate stepwise Cox's proportional hazards analyses, as well as propensity score-adjusted analyses for platelet transfusion to control for selection bias in the use of blood products.RESULTS: The proportion of patients receiving transfusion of any blood component decreased from 100% in the period 1989-1996 to 74% in the period 1997-2004. In uni- and multivariate analyses, the indication for transplantation, transfusion of platelets and RBC were highly dominant in predicting 1-yr patient survival. These risk factors were independent from well-accepted indices of disease, such as the Model for End-Stage Liver Disease score and Karnofsky score. The effect on 1-yr survival was dose-related with a hazard ratio of 1.377 per unit of platelets (P = 0.01) and 1.057 per unit of RBC (P = 0.001). The negative impact of platelet transfusion on survival was confirmed by propensity-adjusted analysis.CONCLUSION: This retrospective study indicates that, in addition to RBC, platelet transfusions are an independent risk factor for survival after OLT. These findings have important implications for transfusion practice in liver transplant recipients.

Abstract:Purpose: The aim of this study is to examine the relevant factors affecting the duration of mechanical ventilation after orthotopic liver transplantation.Materials and Methods: The 96 patients who underwent liver transplantation were divided into 2 groups according to whether or not the duration of mechanical ventilation after operation was longer than 24 hours. Nineteen variables, including clinical and experimental variables, were analyzed by t test for continuous variables and chi(2) test for discrete variables. The variables with significance (P <.05) were then analyzed with stepwise logistic regression.Results: Nine continuous preoperative clinical and experimental variables, including preoperative Child-Pugh stage, time of operation, volume of intraoperative liquid transfusion, volume of intraoperative blood loss, volume of intraoperative blood transfusion, volume of intraoperative urine, time of intraoperative hypotension, postoperative renal failure, and postoperative pulmonary edema revealed significant differences between the 2 groups. Stepwise logistic regression analysis for 9 variables indicated that volume of intraoperative blood loss, volume of intraoperative urine, and postoperative renal failure are relevant independent risk factors.Conclusion: The relevant risks affecting the time of ventilation in patients after orthotopic liver transplantation are multiple. The volume of intraoperative blood loss, volume of intraoperative urine, and postoperative renal failure are independent risk factors. (C) 2010 Elsevier Inc. All rights reserved.
Effects of Different Biliary Warm Ischemia Time on Bile Duct and Peribiliary Vascular Plexus after Autologous Orthotopic Liver Transplantation in Rats

Abstract:Objective Effects of different biliary warm ischemia time on the bile duct and the peribiliary vascular plexus have been the most challenging problem for ischemic type biliary complications after orthotopic liver transplantation.The aim of this study is to explore the effects of different biliary warm ischemia time on the bile duct and the peribiliary vascular plexus after autologous orthotopic liver transplantation in rats.Methods Forty eight SD rats were randomly divided into sham-operation group and the other three operation groups with the secondary biliary warm ischemia time of 0 min,10 min and 20 min.The model of autologous orthotopic liver transplantation in rats with external biliary drainage was established,and blood and liver samples were harvested at 6 hours and 24 hours after transplantation.We detected the concentration of vascular endothelial growth factor-A(VEGF-A) in the blood,and conducted the immunohistochemical staining to detect the proliferation of the bile duct and the peribiliary vascular plexus.The apoptosis test of bile duct epithelial cell and conventional pathological examination were also conducted in this study.Results With the extension of the biliary warm ischemia time,VEGF-A levels were significantly decreased at 24 h in the three operation groups with no significant differences at 6 h.With the increase of the biliary tract secondary warm ischemia time,a corresponding significant increase of bile duct epithelial cell apoptosis was shown.The number of bile duct,blood vessels,bile duct surrounding with blood vessels and bile duct epithelial cell proliferation index in the early postoperative portal area decreased with the extended secondary biliary ischemia time,and the number of bile duct which was not surrounded by blood vessels increased significantly.The pathologic examinations of the rats showed that the inflammation of the rat portal area increased,and the injury of biliary epithelial cell was significantly worse.Conclusion The extension of biliary warm ischemia time would result in the increased injury of the bile duct and the peribiliary vascular plexus in autologous orthotopic liver transplantation.The secondary biliary warm ischemia time in liver transplantation should be minimized to reduce the injury of the bile duct and the peribiliary vascular plexus.
