Abstract:Objective To report our experience of channel transurethral resection of the prostate (channel TURP) in patients with post-brachytherapy urinary retention,and discuss the indications, timing of surgery, and its effect on clinical results of brachytherapy. Methods Clinical date of 8 cases of urinary retention after brachytherapy with channel TURP treated from March 2009 to January 2013 were retrospectively analyzed. Results Channel TURP was performed in 1.5 to 4.5 months after implantation in the 8 patients. There were not intraoperative and postoperative complications.The operation time ranged from 30 to 50 minitues and 0 to 5 seeds were removed. According to American Brachytherapy Society Urinary Symptom Score, symptoms were relieved postoperatively. With a mean followup of 15 months after brachytherapy (range 6 to 19) 1 patient had biochemical reccurence and androgen deprivation therapy was performed.And for the other 7 patients, mean serum PSA at the end of followup was 0.42 ng/ml (range 0.02 to 0.82). Conclusions Channel TURP was effective in relieving retention after brachytherapy and could be done safely in early stage without effect on clinical results of brachytherapy.
Early Channel Transurethral Resection of the Prostate for Patients with Urinary Retention after Brachytherapy

Abstract:Objective It is recommended that transurethral resection of the prostate (TURP) after brachytherapy should not be performed at an early stage after implantation. Herein we report our experiences and the results of channel TURP (cTURP) within six months post-implant for patients with refractory urinary retention. Methods One hundred and ninety patients with localized prostate cancer of clinical stages T1c to T2c were treated by brachytherapy as monotherapy at our institution from February 2009 to July 2013. Nine patients who developed refractory urinary retention and underwent cTURP within six months after brachytherapy were retrospectively reviewed and analyzed. Results The median interval between prostate brachytherapy and cTURP was three months (range 1.5 to 5.0 months). There were no intraoperative or postoperative complications and no incontinence resulting from the surgery. All urinary retention was relieved per the American Brachytherapy Society urinary symptom score. With a mean follow-up time of 16 months (range 6 to 26 months) after cTURP, no patient experienced biochemical recurrence. The mean serum prostate-specific antigen (PSA) of the patients who underwent cTURP was 0.42 ng/ml (range 0.08 to 0.83 ng/ml) at the end of their follow-up. Conclusions Early cTURP was found to be safe and effective in relieving urinary retention after brachytherapy and could be performed without compromising its therapeutic efficacy.

Abstract:Objective To investigate the efficacy and adverse effects of brachytherapy for patients with prostate cancer.Methods During 2001 to 2010,67 patients with prostate cancer received brachytherapy and were followed up for PSA and adverse effects.The relative factors which might affect the efficacy of brachytherapy were analyzed.Results A total of 64 patients(95.5%)were followed up for an average of 33.9(4～112)months.The preoperative PSA and nadir postoperative PSA were 20.04 ng/mL and 1.15 ng/mL in average,respectively.The nadir PSAs of 40 patients(62.5%)were less than1 ng/mL,and those of 26 patients(40.6%)were less than0.5 ng/mL.The average time to reach the nadir PSAs was 11(ranging 1～26) months after operation.Common short-term postoperative adverse events included:fever in 4 patients,hematuria in 8 patients,and hematochezia in 3 patients.Long-term postoperative adverse events included:irritation in19 patients,hematochezia in 7 patients,hematuria in2 patients,incontinence in2 patients,and urinary retention in1 patient.Conclusion Brachytherapy is an effective method to treat localized prostate cancer,with relatively low rate of complications and rare occurrence of severe adverse events.

Abstract:Abstract Objective Brachytherapy is minimally invasive with few complications, so may be a good alternative to surgery for patients with prostate cancer. This study aimed to compare the therapeutic effects of brachytherapy and radical prostatectomy in the treatment of localized prostate cancer at different stages. Methods A total of 16532 patients pathologically diagnosed with prostate cancer who underwent radical prostatectomy and 16627 patients who underwent brachytherapy between 2010 and 2012 were collected through the Surveillance, Epidemiology, and End Results (SEER) Stat software. These patients were randomly matched based on age and prostate specific antigen (PSA) level at first diagnosis, and were divided into five subgroups, including G2T1c, G3T1c, G2T2a, G3T2a and G3T3b. Differences in survival were analyzed by Kaplan-Meier survival curves and compared by log-rank test. Results The overall survival (OS) and prostate cancer-specific death time (PCSDT) in the radical prostatectomy group at G3T1c stage were significantly longer than in the brachytherapy group (P 0.05). Conclusion The radical prostatectomy prognosis was superior to brachytherapy in the treatment of early-stage low-risk localized prostate cancer, whereas there was no significant difference in the prognosis of patients with moderate or high-risk localized prostate cancer.
[Review on the effectiveness of prostate cancer brachytherapy]

Abstract:Ultrasound-guided transperineal prostate brachytherapy is now widely used modality in the treatment of prostate cancer. The overall prostate-specific antigen (PSA) progression-free survival at 10 years is 80-90% for low-risk patients. The results of long-term follow-up have showed better biochemical disease-free survival after I-125 and Pd-103 brachytherapy than after conventional external-beam radiotherapy and similar survival after radical prostatectomy. The most commonly reported dosimetric quantifiers include D90 (the dose that covers 90% of the prostate volume outlined on postimplant computed tomography images) and V100 (the fractional volume of the prostate that receives 100% of prescription dose). The biochemical disease-free survival correlates with the dose. In low-risk patients, achieving a D90 dose of 140-160 Gy might be adequate for prostate-specific antigen control. However, high-risk disease might require a D90 dose higher than 200 Gy. In the immediate posttreatment period, the most common complication is acute urinary retention. Urinary symptoms such as frequency, nocturia, and dysuria occur commonly and are documented in about 80% of patients complaining of symptoms 2-3 months after treatment. Late urinary complications of brachytherapy include urethral stricture and incontinence. Incontinence is rare and mainly occurs in patients who had transurethral resection of the prostate either prior or after brachytherapy. Rectal complications (proctitis, rectal bleeding) are rare after prostate brachytherapy. Brachytherapy like external-beam radiotherapy but unlike surgery preserves ejaculation; potency rates also appear to be relatively high after brachytherapy, at 50-85%, and most patients' sexual quality and function are preserved. Since July 2007, the real-time I-125 prostate brachytherapy has been started in Lithuania and Baltic countries at the Hospital of Kaunas University of Medicine. A total of 150 low-risk patients (< or =T2a, Gleason < or =6, PSA < or =10) were treated by this method. Permanent prostate brachytherapy is an appropriate method for standard treatment of localized prostate cancer.

Abstract:Prostate cancer (PC) is one of the most prevalent cancers in men. More PCs are diagnosed in older populations, with a high prevalence of comorbidities. Low dose-rate brachytherapy (LDR-BT), either as a monotherapy or combined with external beam radiotherapy (EBRT), exhibits better tolerability and improved biochemical control in clinical practice compared to radical prostatectomy (RP) for the treatment of intermediate- to high-risk, as well as low-risk, PC. [ 1 ] Increasing evidence suggests that ethnicity plays an important role in prostate cancer prognosis. [ 2 ] Herein, we report long-term clinical outcome data for patients with cT1-cT3b PC receiving LDR-BT-based treatment approaches, and our findings suggest that these approaches should result in tumor control efficacy in Chinese populations similar to those reported for ethnic groups.

Abstract:Objective To evaluate the clinical efficacy of permanent 125I prostate brachytherapy monotherapy in patients with early prostate cancer. Methods A total of 18 patients with prostate cancer was diagnosed by prostate biopsy,with Gleason score 5 - 6,average PSA 5.09 ng/ml (range,0.38 -8.73 ),and clinical stage T1c - T2a.All patients underwent 3-dimensional conformal brachytherapy with transperineal ultrasound-guide 125I seeds prostate implantation. Results None of the patients had any adverse events during the operation processes.The operation time was from 37 to 52 minutes and postoperative hospitalization time was from 3 to 4 days.The mean number of implanted 125I seeds was 54.All patients were followed up for 3 to 57 months with a mean of 15 months,while one patient was lost during followingup.The survival rate of PSA progression free was 100％ (17/17).The PSA was under 0.2 ng/ml for a long time in the 7 cases who had been followed-up for more than 18 months.The PSA did not show any rising trend in all 17 cases.No severe complications occurred in all cases. Conclusions Brachytherapy monotherapy with 125I seeds implantation is safe and effective for patients with early prostate cancer.

Abstract:BACKGROUND: Prostate cancer is common among male renal transplant recipients and can present challenges for medical management and patient survival. It is imperative to have a comprehensive understanding of available treatment options in this population to determine the most effective and safe therapies. Brachytherapy, a safe and effective treatment for localized prostate cancer, has not been sufficiently studied in this patient population. Therefore, this study aimed to evaluate the safety and effectiveness of brachytherapy in treating prostate cancer in renal transplant recipients.METHODS: We retrospectively reviewed our brachytherapy database to identify patients with a previous history of renal transplantation who underwent seed implantation for localized prostate cancer. Long-term prostate-specific antigen control and treatment-related toxicity, including graft dysfunction and urinary and rectal complications, were assessed and compared with published outcomes. Results were analyzed to evaluate the efficacy and safety of seed implantation in this patient population.RESULTS: We identified 2 patients with previous renal transplantation who underwent permanent seed implantation for localized prostate cancer. Follow-ups ranged from 53 to 57 months, and both patients remained free of prostate-specific antigen progression with normal graft function. No acute and late complications occurred.CONCLUSION: Brachytherapy is a safe and effective treatment option for post-renal transplant prostate cancer. Given the paucity of reports on brachytherapy in this population, the findings of this study, despite a small sample size, contribute to the increasing body of evidence supporting the use of brachytherapy in this patient population.

Abstract:Objective:To study the clinical efficacy of ~(125)I brachytherapy combined with surgical castration for treating local advanced prostate cancer.Methods:40 patients with advanced prostate cancer underwent ~(125)I transperineal brachytherapy guided by rectal ultrasound-B and orchiectomy at the same time.The age of patients ranged from 52-87 years old,with the average of 73 years old.Gleason Score ranged from 3-9,PSA from 0.24-2736.46 ng/ml,and clinical stage was T_3N_0M_0.Results:All procedures were completed smoothly,the average were 71±15 particles.Follow up ranged from 10-76 months,with the median of 44.5 months.Long term complications(>1 year) included urinary incontinence 2.5%(1/40),mild bloody stools 10%(4/40) and urethra rectal fistula 2.5%(1/40).1 case died in 69 months post-operation.5 cases had biochemical progression an average of 15.8 months post-operation.Cumulative PSA progression free survival rate was 86.9%.Conclusions:Combination of ~(125) I brachytherapy and surgical castration is an effective treatment for local advanced prostate cancer only with a few complications.

Abstract:Low-dose-rate (LDR) brachytherapy with I-125 seeds is one of the most common primary tumor treatments for low-risk and low-intermediate-risk prostate cancer. This report aimed to present an analysis of single-institution long-term results. We analyzed the treatment outcomes of 119 patients with low- and intermediate-risk prostate cancer treated with LDR brachytherapy at our institution between 2014 and 2020. The analysis focused on biochemical recurrence rates (BRFS), overall survival (OS), cumulative local recurrence rate (CLRR), and the incidence of acute and late toxicities. Patient-reported quality of life measures were also evaluated to provide a holistic view on the treatment's impact. The median follow-up period was 46 months. CLRR was 3.3% (4/119), five-year BRFS was 87%, and the five-year OS rate was 95%. Dysuria was the most common acute urinary toxicity, reported in 26.0% of patients as grade 1 and 13.4% as grade 2. As a late side effect, 12.6% of patients experienced mild dysuria. Sexual dysfunction persisted in 6.7% of patients as grade 1, 7.5% as grade 2, and 10.0% as grade 3. LDR brachytherapy in patients with prostate cancer is an effective treatment, with favorable clinical outcomes and manageable toxicity. The low CLRR and high OS rates, as well as low incidence of severe side effects, support the continued use of LDR brachytherapy as a primary treatment modality for localized prostate cancer.

Abstract:Objective To explore the nursing cooperation in brachytherapy on the patients with clinically lo-calized intermediate and high-risk prostate cancer. Methods A total of 119 prostate cancer patients were en-rolled for brachytherapy from May 2013 to May 2014. Results All the procedures were uneventful,and the effi-cacy shows satisfactory. Conclusions The nursing experience and the behavior criterion are necessary for the pro-cedure.

Dosimetric and Radiobiological Comparison of Volumetric Modulated Arc Therapy, High-dose-rate Brachytherapy and Low-dose-rate Permanent Seeds Implant for Localized Prostate Cancer

Abstract:To assess the dosimetric and radiobiological differences among volumetric modulated arc therapy (VMAT), high-dose-rate (HDR) brachytherapy, and low-dose-rate (LDR) permanent seeds implant treatment of localized prostate cancer. Ten patients with localized prostate cancer were retrospectively selected for this study. Volumetric modulated arc therapy, high-dose-rate brachytherapy and low-dose-rate permanent seeds implant plans were created for each patient. For volumetric modulated arc therapy, planning target volume (PTV) was created by adding a margin of 5 mm to the clinical target volume. Bladder, rectum, femoral heads, urethra, and pelvic tissue were considered as organs at risk. 78 Gy in 39 fractions were prescribed for PTV. The dose prescription was D90 of 34 Gy in 8.5 Gy per fraction and 145 Gy to clinical target volume for HDR (192Ir) and LDR (125I), respectively. The dose and dose volume parameters were evaluated for target and organs at risk. Physical dose were converted to dose based on 2-Gy fractions (equivalent dose in 2 Gy per fraction, EQD2) for comparison of three techniques. All three techniques could achieve the prescribed dose to the target and clinically satisfactory plans. The volume of rectum and bladder exposed to 30-70 Gy (EQD2) was reduced for LDR and HDR when compared with VMAT. The sparing of normal tissue and femoral heads was also significantly improved in LDR and HDR compared with VMAT. The mean dose to urethra was lower in HDR than LDR. Brachytherapy techniques were clearly superior in terms of bladder, rectum, femoral heads and normal tissue sparing compared with VMAT. HDR could further improve the sparing of urethra compared with LDR.
Comparative Effectiveness of Brachytherapy As Local Treatment Versus No Local Treatment for Metastatic Prostate Cancer: A SEER Analysis

Abstract:Currently, there is no level 1 evidence to show whether brachytherapy (BT) as a definite local treatment (LT) has an impact on survival of patients with metastatic prostate cancer (mPCa) at diagnosis. We performed a population-based analysis to compare BT and no local treatment (NLT) for mPCa. We hypothesized that BT as a LT was associated with survival benefit for mPCa. Data on men with mPCa (M1a-M1c stage) at diagnosis were obtained from Surveillance Epidemiology and End Results database (2004–2013) and were divided based on definitive treatment of the prostate (brachytherapy [BT]) or NLT (no surgery or radiation therapy). The last follow-up was on December 31, 2013. Endpoints of interest were overall survival (OS) and cancer-specific survival (CSS). This study was exempt from human subjects review because only deidentified and previously collected data were used. Kaplan-Meier survival curves were used to describe survival probability of OS and CSS. Standard multivariable Cox regression analyses (MVA) was employed to test the relationship between BT and OS and CSS. Propensity score matching (PSM) analysis was used to generate study cohorts with comparable baseline characteristics after adjusting for cofounding factors. After PSM, Kaplan-Meier survival curves and MVA were conducted again. SPSS v.22.0 (SPSS Inc, Chicago, IL) was used for all analyses, with a p value <0.05 considered significant. Results: A total of 11,447 patients were identified: BT (n= 159) and NLT (n=11,288). BT group (mean survival time [MST]: 68.4, 95%CI: 60.6-76, months) had significant longer OS than NLT group (MST: 39.8, 95%CI: 39-40.7, months) (p<0.001). Similarly, BT group (MST: 77.6, 95%CI: 69.5-85.7, months) had significant longer CSS than NLT group (MST: 49.4, 95%CI: 48.4-50.5, months) (p<0.001). After adjusting for clinicopathological factors, MVA showed that BT was associated with 48.1% lower risk of all-cause mortality (HR=0.519, 95%CI=0.413-0.652, p<0.001) and 46.2% lower risk of prostate cancer-specific mortality (HR=0.538, 95%CI=0.415-0.698, p<0.001) compared with NLT. In PSM-adjusted analysis, BT group (MST: 67.2, months) still had better OS than NLT group (MST: 52 months) (p=0.017). BT group (MST: 76.1 months) also obtained longer CSS than NLT group (MST: 59.2 months) (p=0.018). MVA showed BT was associated with better OS (adjusted HR: 0.647, 95%CI=0.457-0.917, p=0.015) and better CSS (adjusted HR= 0.641, 95%CI=0.434-0.946, p=0.025). Brachytherapy as a local treatment improves survival for patients with mPCa at diagnosis. BT was potentially recommended for selected mPCa patients in clinical settings. Our results warrant prospective evaluation in clinical trials.

Abstract:Objective: This study compared survival of prostate cancer patients with low prostate specific antigen level (PSA <= 10 ng/ml) and high-grades of Gleason score (GS) of 8-10 with different treatment options (i.e., radical prostatectomy [RP], external beam radiotherapy [EBRT], or external beam radiotherapy with brachytherapy [EBRT+BT]). Materials and Methods: The Surveillance, Epidemiology and End Results (SEER) database data (2004-2013), and overall survival (OS) and prostate cancer-specific mortality (PCSM), were evaluated using the Cox proportional hazards regression model and Fine and Gray competing risk model. Results: The SEER data contained 9,114 patients, 4,175 of whom received RP, 4,114 received EBRT, and 825 received EBRT+BT with a median follow-up duration of 47 months. RP patients had significantly better OS than patients with EBRT and EBRT+BT (adjusted HR [AHR]: 3.36, 95% CI: 2.43-4.64, P < 0.001; AHR: 2.15, 95% CI: 1.32-3.48, P = 0.002; respectively). There was no statistical difference in PCSM between RP and EBRT+BT (AHR: 1.31, 95% CI: 0.61-2.80, P = 0.485), while EBRT had worse OS (P < 0.05). The subgroup analysis revealed that there was no statistical difference in prognosis of patients with age of >70 years old, or PSA levels of <= 2.5 ng/ml between RP and EBRT+BT (P > 0.05). Conclusion: RP patients with low PSA levels and high GS had better OS compared to either EBRT or EBRT+BT, while RP and EBRT+BT resulted in significantly lower PCSM, compared to EBRT. Moreover, EBRT+BT and RP were associated with similar survival of patients with age of > 70 years old, or PSA levels of <= 2.5 ng/ml.

Abstract:Objective: The brachytherapy (BT) and radical prostatectomy (RP) are two methods recommended in current guidelines for the treatment of localized prostate cancer (PCa). It is difficult to compare the oncological results of these two treatments because of differences in baseline characteristics and treatment selection.we sought to compare the efficacy of BT and RP after propensity score matching(PSM)analysis. Methods: Between January 2009 and December 2021, our institution treated 657 patients with localized PCa (BT: n = 198; RP: n = 459)and followed up for > 2 years. Biochemical recurrence was defined as prostate-specific antigen (PSA) levels of nadir plus 2 ng/ml or higher (Phoenix definition) for BT, and as PSA0.2 ng/ml or greater for RP. PSM was applied based on the age, body mass index, PSA, prostate volume, clinical T-stage, Gleason grade, percentage of positive puncture needles ≥ 1/2, maximum tumor diameter ≥ 5 mm, and follow-up period. Results: Median follow-up was 63 months for BT and 52 months for RP. After propensity score adjustment, a total of 294 (147 each) patients remained for further analysis.Kaplan-Meier curves showed no statistically significant difference in clinical relapse-free survivals (cRFS) (p = 0.637),overall survival (OS) (p = 0.726),and cancer-specific survival (CSS) (p = 0.505).BT was associated with improved biochemical relapse-free survivals (bRFS) compared to RP (p = 0.022), Logistic multivariate analysis based on the whole cohort revealed that clinical T stage ≥ T2b (p = 0.043) and tumor maximum diameter ≥ 5 mm (p = 0.044) were associated with significantly bRFS. Conclusion: The BT and RP group patients exhibited similar cRFS, OS, and CSS. However, patients in the BT groups exhibited better bRFS than those in the RP group.Clinical T stage ≥ T2b and a maximum tumor diameter ≥ 5 mm were independent prognostic factors.
Tumor burden and location as prognostic factors in patients treated by iodine seed implant brachytherapy for localized prostate cancers

Abstract:Abstract Background Iodine seed implant brachytherapy is indicated for low risk and selected favorable intermediate risk prostate cancers. A percentage of positive biopsies > 50% is usually considered as a contra-indication, and the tumor location could also influence the treatment efficacy. We studied the association of the percentage of positive biopsy cores, and tumor location, with progression-free survival. Methods Among the 382 patients treated at our center by permanent implant iodine seed brachytherapy for a localized prostate cancer between 2006 and 2013, 282 had accessible detailed pathology reports, a minimum follow-up of 6 months, and were included. Progression was defined as a biochemical, local, nodal, or distant metastatic relapse. We studied cancer location on biopsies (base, midgland or apex of the prostate) and percentage of positive biopsy cores, as well as potential confounders (pre-treatment PSA, tumor stage, Gleason score, risk group according to D’Amico’s classification modified by Zumsteg, adjunction of androgen deprivation therapy, and dosimetric data). Results Most patients (197; 69.9%) had a low risk, 67 (23.8%) a favorable intermediate risk, 16 (5.7%) an unfavorable intermediate risk, and 1 (0.3%) a high-risk prostate cancer. An involvement of the apex was found for 131 patients (46,5%), of the midgland for 149 (52,8%), and of the base for 145 (51,4%). The median percentage of positive biopsy cores was 17% [3–75%]. The median follow-up was 64 months [12–140]. Twenty patients (7%) progressed: 4 progressions (20%) were biochemical only, 7 (35%) were prostatic or seminal, 6 (30%) were nodal, and 3 (15%) were metastatic. The median time to failure was 39.5 months [9–108]. There were more Gleason scores ≥7 among patients who progressed (40% vs 19%; p = 0.042). None of the studied covariates (including tumor location, and percentage of positive biopsy cores), were significantly associated with progression-free survival. The risk group showed a trend towards an association ( p = 0.055). Conclusions Brachytherapy is an efficient treatment (5-year control rate of 93%) for patients carefully selected with classical criteria. The percentage and location of positive biopsies were not significantly associated with progression-free survival. A Gleason score ≥ 7 was more frequent in case of progression.

Abstract:Objective To evaluate the clinical efficacity of permanent ~(125)I prostate brachytherapy in patients with stage C hormone refractory prostate cancer. Methods A total of 11 patients with prostate cancer was diagnosed by prostate biopsy, Gleason score of them was 6～9, average PSA was 10.91ng/ml at the progression to hormone refractory prostate cancer, and clinical staging was C stage. All patients underwent 3-dimensional conformal brachytherapy with transperineal ultrasound-guide ~(125)I seeds prostate implantation. Results None of the patients had any adverse events during the operation processes. The operation time was from 30 to 70 mins and hospital staying time was from 3 to 6 days. The mean number of implanted ~(125)I seeds was 49. All patients were followed up for 3 to 71 months with a mean of 31 months. Of them, 3 cases had complete reaction, 1 partial reaction, 3 stable condition and 3 developed progressive disease(PD), one patient lost during following-up. The survival rate of PSA progression free was 70 % (7/10). For other 3 PD patients, 2 patients' PSA progression free survival time was longer than 2 years. No severe complication occurred in all cases. Conclusion Brachytherapy with ~(125)I seeds implantation is a safe and effective treatment for patients with stage C hormone refractory prostate cancer.

Abstract:Background High-dose-rate brachytherapy (HDR-BT) plays an important role in the treatment of locally recurrent prostate cancer after definitive treatment. The objective of this study is to summarize the efficacy and toxicity of HDR-BT in these patients. Methods We performed a systematic review of PubMed and EMBASE from inception to July 2023. The primary endpoint was relapse-free survival (RFS) in different subgroups, and the secondary endpoint was gastrointestinal (GI) and genitourinary (GU) toxicity. A semi-automated tool (WebPlotDigitizer) and a new Shiny application combined with R software (R: a language and environment for statistical computing. R Foundation for Statistical Computing, Vienna, Austria; https://www.R-project.org/) helped to reconstruct survival curves. Results Twenty-six studies met the inclusion criteria for quantitative analysis, including 1447 patients. A total of 761 patients from 13 studies were included in survival reconstruction, and the median RFS time was 61.2 months (57.6–72.0 months). The estimated 2‐, 3‐, and 5‐year rates were 75.9% (95% confidence interval [CI] 72.8 ~ 79.2%), 66.7% (95% CI 63.0 ~ 70.5%), and 52.3% (95% CI 47.5 ~ 57.4%), respectively. Whole-gland irradiation with multiple fractions (≥ 2 F) resulted in better RFS compared with focal gland irradiation with fewer fractions (1 F mostly; hazard ratio [HR]: 0.60, 95% CI 0.47–0.77, p < 0.0001). According to the different median time from primary treatment to salvage therapy (TRS) and median age at recurrence, short median TRS (56–67.2 months vs. 70–120 months; HR 0.52, 95% CI 0.68–0.40; p < 0.0001) and younger median age (60–70 years vs. 71–75 years; HR 0.58, 95% CI 0.46–0.74; p < 0.0001) were positive factors for RFS. The cumulative incidences estimated for grade ≥ 3 acute and late GU toxicities were 1% (95% CI 0 ~ 1%) and 5% (95% CI 4 ~ 7%), respectively. Three patients (3/992) experienced grade ≥ 3 late GI toxicity, and no cases of grade ≥ 3 acute GI toxicity were reported. Conclusion HDR-BT has a high safety profile and good RFS benefit for salvage treatment of radiorecurrent prostate cancer. In terms of RFS, whole-gland irradiation with multiple fractions seems to be better than focal gland irradiation with fewer fractions, while short TRS and younger age are good prognostic factors. In view of the low level of evidence in the included studies and the large heterogeneity of each study, these conclusions still need to be confirmed by randomized controlled trials.

Abstract:Objective To evaluate the effect of local treatment for the prognosis of hormonesensitive metastatic prostate cancer.Methods A systematic review of the literature about local treatment for the prognosis of hormone-sensitive metastatic prostate cancer was performed,searching Medline,Embase,Cochrane Library,CBM,CNKI,VIP and Wan Fang database from January 2001 to October 2016.Two authors reviewed the records to identify comparative studies.A meta-analysis was conducted using Review Manager 5.0.Results Five studies were enrolled,including 29 354 patients.Meta-analysis showed that,compared with no local treatment,local treatment had a significant beneficial effect in 5-year overall survival rate (OR =0.18,95 ％ CI 0.15-0.22,P ＜ 0.01),5-year disease specific survival rate (OR =0.39,95 ％ CI 0.31-0.49,P＜0.01) as well as 3-year overall survival rate (OR =0.47,95％ CI 0.35-0.62,P＜0.01).Sub-group analysis showed that,compared with no local treatment,prostatectomy had a significant beneficial effect in 5-year overall survival rate (OR =0.17,95 ％ CI 0.12-0.25,P ＜ 0.01),5-year disease specific survival rate (OR =0.30,95％ CI 0.22-0.40,P ＜0.01) as well as 3-year overall survival rate (OR=0.25,95％CI 0.19-0.32,P ＜0.01),and external beam radiation therapy only had a significant beneficial effect in 3-year overall survival rate (OR =0.54,95％ CI 0.42-0.69,P ＜ 0.01).Conclusions Prostatectomy and external beam radiation therapy may have a survival benefit for hormone-sensitive metastatic prostate cancer.Prospective randomized controlled studies are necessary because of the limits of current studies.
