Abstract:Objective: To present our classification for peripheral nerve schwannomas as well as explore the surgical strategies and operative management of peripheral nerve schwannomas based on the intraoperative neurophysiological monitoring (INM) technique and to decrease the risk of postoperative neurological deficits in the management of these schwannomas. Materials and methods: A retrospective study was conducted on 92 cases of peripheral nerve microsurgery performed, using the INM technique. We also made the classification for peripheral nerve schwannomas into two types according to operative findings and proceeded corresponding surgical strategies. Results: All tumors were removed completely under microscopy and INM. Three patients developed residual neurological deficits at final follow-up. There were different results about temporary (18/92, 19.6%) and permanent (3/92, 3.3%) neurological deficits. The incidence of temporary and permanent neurological deficits in type II group was significantly higher than that in type I group (p < 0.01). The incidence of permanent neurological deficits in larger size tumors was significantly higher than that of smaller size (p < 0.01). Conclusions: We made the classification for peripheral nerve schwannomas according to operative findings based on INM that is helpful to our surgical strategies. Intracapsular enucleation was the preferred strategy with satisfactory results and low risk of nerve injury. The size and location of tumors seem to be related to the risk of fascicular injury. Percutaneous Endoscopic Removal of Cervical Foraminal Schwannoma via Interlaminar Approach: A Case Report.

Abstract:BACKGROUND AND IMPORTANCE: Cervical foraminal schwannomas commonly originate from spinal nerves that pass through the intervertebral foramen of the cervical vertebrae. Because of the proximity of this type of tumor to the vertebral artery and spinal nerves, surgical management remains a major challenge. Conventional open spine surgery usually requires the removal of the articular process and is supplemented by a simultaneous posterolateral spine fusion surgery. To decrease the associated risks of surgical complications by further reducing invasiveness, percutaneous spinal endoscopy may be used for resection of foraminal spinal neoplasm. C LINICAL PRESENTATION: A 52-yr-old female who presented with neck pains with duration of 1 yr was admitted to our hospital. Physical examination revealed moderate rigidity in the neck and grade 5 muscle strength in both upper and lower limbs. Preoperative magnetic resonance imaging (MRI) scans demonstrated a left-sided lesion at the C3-C4 intervertebral foraminal area. Under C-arm fluoroscopy navigation and neuromonitoring, the endoscope was properly positioned on the same side of the tumor, and a small part of the left C3 inferior and C4 superior lamina were first removed by an endoscopic drill to enlarge the interlaminar space. Next, through an endoscopic working canal, the left intervertebral ligamentum flavum was removed to fully expose the tumor. The tumor mass was finally resected in a piecemeal approach. Postoperative MRI confirmed complete tumor resection. CONCLUSION: This is the first case report of a total removal of a cervical foraminal schwannoma with a percutaneous spinal endoscopic procedure.

Abstract:Intraosseous schwannoma of the mandible is rare, with diagnostic and therapeutic challenges. The aims of this study were to report new cases of intraosseous schwannoma of the mandible and to propose a clinical classification, providing suggestions for treatment methods. The cases of 13 patients treated at the authors' hospital and 86 cases reported previously in the literature were reviewed. The most common clinical feature was facial swelling (60/93). The rate of cortical thinning or expansion was 44.8% (43/96); widening of the inferior alveolar nerve canal on radiographs was observed in 15 patients.

Abstract:We report two rare cases of spinal intraosseous schwannoma (SIS) with sustained myelopathy symptoms and provide an updated review regarding SIS in the literature. A 71-year-old man experienced right lumbocrural pain and gait disturbance accompanied with paresthesia and right leg weakness. Imaging examinations revealed a mass with lesions in L4 vertebral body causing bone destruction and spinal cord compression. Complete resection of the well-demarcated tumor and posterior fusion were performed. A 54-year-old female reported bilateral gait disturbance, paresthesia, and numbness without weakness, and imaging revealed a posterior mass from T9 causing spinal cord compression and bone erosion. The tumor was completely separated from the spinal nerve root. The tumors from both patients were confirmed as schwannomas. Tumor recurrence was not observed at the 2-4 year follow-up. Although rare, SIS should be considered during differential diagnosis and can affect treatment planning. SIS symptoms vary depending on tumor location, and fusion is frequently necessary for spinal reconstruction after complete tumor resection.

Abstract:Background context Intramedullary schwannoma is a kind of relatively rare tumors with only a few literature reports. This study was aimed to report the clinical characteristics of intramedullary schwannoma and discuss imaging findings and treatment strategies. Methods The inclusion criterion was consecutive patients with intramedullary schwannomas who were surgically treated in our institution between 2017 and 2022. Data included clinical characteristics, radiological features, surgical management and prognosis. Clinical and follow-up details of all cases were collected and reviewed. Results There were three males and eight females. The mean age of all patients was 45 years old (Range 26-77 years). Cervical spine (4 cases, 36.4%), thoracic spine (4 cases, 36.4%) and lumbarsacral spine (3 cases, 27.3%) involvement were found respectively. Weakness, numbness and pain of limbs were the main symptoms at administration. Pre-operative MRI all demonstrated lesion with spinal cord medullary invasion and well demarcated margins. The post-operative histological examination all showed benign lesions and confirmed the schwannoma. Conclusions Intramedullary schwannoma is a very rare tumor. This article presented a series of 11 cases of intramedullary schwannoma with sharp margins and well-enhanced feature. Prognosis and functional recovery were good after gross total resection.

Abstract:Intraosseous schwannoma is a rare benign nerve sheath tumor comprising < 1% of bone tumors. Relatively common locations for this tumor include the skull and mandible, and, to a lesser degree, the spine and sacrum. Intraosseous schwannoma involving the appendicular skeleton is exceedingly rare. The clinical and imaging presentation, as in this case, is nonspecific and includes pain in the setting of a lytic bone lesion. Abstract:Objective To investigate the clinical features and surgical treatment of intramedullary spinal schwannomas.Methods 8 cases of iutramedullary spinal schwannomas were respectively reviewed and related clinical data,including manifestations,imaging features,operative outcomes was analyzed.Results Tumors were totally removed in all the 8 patients.During follow - up period from 3 months to 9 years,muscle strength significantly improved in 5 patients with limb weakness before operation.6 cases complained of abnormal sensitivity.No tumor recurrence was found in the rechecking MRI of the 8 patients.Conclusions Intramedullary spinal tumors,locating at dorsalateral side,with clear boundary,homogenous enhancement and neuralgic pain,should be highly suspected intramedullary shwannomas.Once diagnosed,surgical resection is the best choice.Clinical outcome is closely related to the surgical approach and skill.

Abstract:Background: Sacral schwannoma is relatively rare and both the early diagnosis and appropriate treatment of sacral schwannomas are equally very challenging.Methods: 11 sacral schwannoma cases operated at the Second Affiliated Hospital, School of Medicine, Zhejiang University, from 2012 to 2016, were investigated retrospectively and 10 were followed up. All patients were assessed with X-ray, CT and MRI, and underwent an intralesional excision.Results: One patient was male, nine were female and the average age was 48 (ranging between 31 and 63). Three patients suffered from back and leg pain, and seven had no obvious symptoms. The average blood loss during surgery was 980ml (ranging between 100 and 2,000ml). Six patients underwent preoperative biopsy. The surgeries were performed via the combination of an anterior and posterior approach in two patients, a posterior approach in seven patients, and an anterior approach in one patient. Residual tumors were not detected in all patients after surgery. Unfortunately, the postoperative complications occurred in three patients, namely bowel and bladder dysfunction (two patients) and cerebrospinal fluid leakage with secondary intracranial infection (one patient). The average follow-up was 22.7 months (8-44 months). All patients were relieved from preoperative symptoms after the last follow-up.Conclusions: The typical findings of our cases in MRI were a well-circumscribed lesion with a heterogenous signal intensity on T2-weighted image, which may be helpful for preoperative decision-making. Intralesional excision can be successfully performed using single anterior or single posterior or both, and is an important procedure in the treatment of sacral schwannomas.

You can register here Intramedullary schwannomas in the conus medullaris are very rare and are usually not associated with syringomyelia. We report a unique case of intramedullary schwannoma in the conus medullaris with long-segment syringomyelia. The patient was a 60-year-old male, initially presenting with left dorsalgia, subsequently developing weakness in the right lower extremity. As the symptoms progressed, the patient exhibited ataxia in gait, accompanied by sphincter insufficiency and voiding dysfunction. Lumbar MRI revealed the presence of two tumors at the L3 and T11-L1 levels, accompanied by syringomyelia extending from T4 to T10. During surgery, it was determined that the tumor located at the T11-L1 vertebral level was intramedullary, whereas the tumor situated at the L3 level exhibited an extramedullary intradural configuration. Pathological examination conclusively identified both the intramedullary and extramedullary tumors as schwannomas. Although intramedullary schwannomas at the conus medullaris are very rare, schwannoma remains a diagnosis that cannot be ignored when facing patients with intramedullary tumors with syringomyelia. Intramedullary schwannoma can have a good neurological prognosis after surgical treatment.

Abstract:BACKGROUND CONTEXT Cellular schwannoma (CS) is a rare tumor that accounts for 2.8–5.2% of all benign schwannomas. There is a dearth of up-to-date information on spinal CS in the literature. PURPOSE The aims of this study were to identify the proportion of CS cases amongst spinal benign schwannoma, describe the clinical features of spinal CS, and identify prognostic factors for local recurrence by analyzing data from 93 consecutive CS cases. STUDY DESIGN Retrospective review. PATIENT SAMPLE We analyzed 93 PSGCT screened from 1706 patients with spine CS who were treated at our institute between 2008 to 2021. OUTCOME MEASURES Demographic, radiographic, operative and post-operative data were recorded and analyzed. METHODS We compared the clinical features of spinal CS from the cervical, thoracic, lumbar and sacral segments. Prognostic factors for local recurrence-free survival (RFS) were identified by the Kaplan-Meier method. Factors with p ≤ 0.05 in univariate analysis were subjected to multivariate analysis by Cox regression analysis. RESULTS The proportion of spinal CS in all benign schwannomas was 6.7%. The mean and median follow-up times for the 93 patients in this study were 92.2 and 91.0 months respectively (range 36 to 182 months). Local recurrence was detected in 11 cases, giving an overall recurrence rate of 11.7%, with one patient death. Statistical analysis revealed that tumor size ≥ 5 cm, intralesional resection, and Ki-67 ≥ 5% were independent negative prognostic factors for RFS in spinal CS. CONCLUSIONS Whenever possible, en bloc resection is recommended for spinal CS. Long-term follow-up should be carried out for patients with tumor size ≥ 5cm and postoperative pathological Ki-67 ≥ 5%.

Well-Differentiated Intraosseous Osteosarcoma in the Sacrum: A Case Report

Abstract:Hereby we report a case of well-differentiated intraosseous osteosarcoma in the sacrum. A 32-year-old woman was admitted to our hospital with a low echo-level mass in the pelvis searched by ultrasound in a routine physical examination. Radiographically, the mass was misdiagnosed as a benign bony tumor originating from the sacrum. The tumor was completely resected and pathological diagnosis was intraosseous well-differentiated osteosarcoma. Twelve months after operation, the patient was well and there was no evidence of recurrence and distal metastasis. This is a peculiar case of well-differentiated osteosarcoma involving an unusual site of the sacrum. The radiographic appearance and the differential diagnosis are discussed. We consider that dense trabeculated-like bone within an intraosseous solid mass might be suggestive of a well-differentiated osteosarcoma that was valuable in guiding the treatment and prediction of its prognosis. Well-differentiated osteosarcoma, although malignant, may be mistaken for a benign condition. Local excision has almost always been associated with recurrence. For this case, the patient had a wide excision and had no recurrence and metastasis. Therefore, it is very important to identify the radiological features and to distinguish this tumor from benign lesions and high-grade osteosarcomas before operation.
A Case Report of Primary Multiple Obturator Nerve Schwannomas

Abstract:Primary multiple obturator nerve schwannomas originate from Schwann cells and are extremely rare. Patients with schwannomas are asymptomatic and a retroperitoneal schwannoma is often misdiagnosed as an adnexal mass. In the present study, we describe a 58-year-old woman in whom a right adnexal mass accompanied by endometrial polyp was found incidentally through transvaginal ultrasound. The mass was diagnosed as multiple obturator nerve schwannomas after laparoscopy. Immunohistochemical assay confirmed the schwannomas to be positive for SOX10. To our knowledge, this is the first report to demonstrate a case of multiple schwannomas originating from the obturator nerve and treated by laparoscopic resection.
Intraosseous mandibular schwannoma managed via submandibular approach: a case report with a review of previously published cases

Abstract:A 40-year-old female presented with right mandibular swelling. A panoramic radiograph showed a unilocular radiolucency from the mandibular angle to tooth #46. Biopsy confirmed a schwannoma. Surgical resection was performed via a submandibular approach with a reconstruction plate. Teeth 46 and 47 were extracted. Surgery was complication-free, and histopathology confirmed the tumor's benign nature. The patient was discharged on the second postoperative day. At the 1-year follow-up, she had no paresthesia, normal mouth opening, and full mandibular motion. The reconstruction plate was intact. This case adds to the limited literature on intraosseous schwannomas, emphasizing early detection, thorough radiological assessment, and meticulous surgical planning.
Spinal Cellular Schwannoma: an Analysis of Imaging Manifestation and Clinicopathological Findings.

Abstract:OBJECTIVES:Cellular schwannoma that occurs in the spine is relatively rare. Herein, we retrospectively analyzed the clinical and imaging data from nine cases of spinal cellular schwannoma.MATERIALS AND METHODS:The clinical, imaging data and pathological manifestations were retrospectively analyzed from nine cases of pathologically confirmed spinal cellular schwannoma.RESULTS:All nine cases were treated with surgical excision; seven cases were solitary lesions characterized by a dumbbell-shaped soft tissue mass growing out across the intervertebral foramen and two cases were multiple nodular lesions in the spinal canal. Six cases had compressive bony destruction, one case displayed osteolytic and compressive bony destruction, and two cases were without apparent bony destruction. Computed Tomography (CT)scans showed three cases were isodense and two cases were miscellany density. Magnetic Resonance Imaging (MRI) scans showed that the solid part of the tumor was isointense on the T1-weighted imaging (T1WI); five cases were isointense and three cases were slightly hyperintense on T2-weighted imaging (T2WI). Five cases exhibited hyperintense cystic regions. MRI enhanced scanning showed that 3/7 cases showed moderately to markedly heterogeneous enhancement and the remaining 4/7 cases showed markedly homogeneous enhancement. Except for one recurrence, 8/9 cases had a good prognosis.CONCLUSION:Spinal cellular schwannoma should be diagnosed combining the imaging manifestation with clinical performance and pathological examination in order to distinguish it from other tumors, such as classical schwannoma and malignant peripheral nerve sheath tumors.

Abstract:Schwannomas are the most common type of spinal tumor, and they most commonly occur in intradural extramedullary locations. Intramedullary schwannomas of the central nervous system are very rare and are difficult to diagnose using preoperative imaging. Here, we report a rare, tiny cervical intramedullary schwannoma and review the literature regarding the clinical presentation, magnetic resonance imaging, pathology, and surgical experience associated with this rare tumor type.
Intralesional Marginal Resection for Osteoblastoma in the Mobile Spine: Experience From a Single Center

Abstract:Osteoblastoma (OB) is a benign bone tumor with aggressive behavior and a tendency for local recurrence. The appropriate surgical strategy for spinal OB remains unclear. This retrospective study aimed to verify the clinical efficacy and safety of intralesional marginal resection of OB in the mobile spine. We enrolled 50 consecutive patients with spinal OB between January 2009 and December 2019. The tumors were staged based on the Enneking system, with 21 and 29 lesions being determined as stage 2 (St.2) and stage 3 (St.3), respectively. Among them, 42 patients underwent intralesional marginal resection, five underwent extensive curettage, and three underwent en bloc resection successfully since their lesions were limited to the posterior element in a single vertebra. We analyzed clinical characteristics, perioperative and follow-up images, surgical details, and follow-up data. Within a median follow-up duration of 50 (range: 24-160) months, six (12.0%) patients had local recurrence. The recurrence rates among patients who underwent intralesional marginal resection, curettage, en bloc resection were 7.1%(3/42), 60.0%(3/5), and 0%(0/3), respectively. The recurrence rate of intralesional marginal resection of St.3 lesions was slightly higher than that of St.2 lesions (7.7%[2/26] vs. 6.3%[1/16]). There were 16(38.1%), 3(60.0%), and 0 patients with surgical complications among those who underwent intralesional marginal resection, curettage, and en bloc resection, respectively. Local recurrence was observed in five (5/14, 35.7%) patients who had vertebral artery extension and in none who did not have vertebral artery extension (p = 0.02). Our findings suggest that intralesional marginal resection could be an appropriate treatment choice for patients with spinal OB, both St.2 and St.3 lesions, with an acceptable local recurrence rate and a low risk of complications. Vertebral artery extension could be a strong risk factor for local recurrence in patients with spinal OB.
Surgical Management of Mandibular Intraosseous Schwannomas

Abstract:Intraosseous schwannomas of the mandible are rare tumors that usually arise from peripheral nerve sheaths of the main trunk or branches of the inferior alveolar nerve (IAN).A systematic review of the English literature through PubMed was conducted from 1945 to 2016. Forty-nine patients of mandibular intraosseous schwannomas were identified. The most common location of the tumor was the posterior mandible (61.2%). There were 28 patients in whom the tumor involved the IAN (57.1%). The most commonly performed procedures included enucleation through a mucoperiosteal flap and a bone window (73%), curettage (10%), segmental mandibulectomy (10%), and sagittal split ramus osteotomy (6%). The recurrence rate was 6%: all 3 recurrent patients occurred in the posterior area. The authors report a 72-year-old female with a schwannoma of the IAN which was removed through a sagittal split of the mandible, although there was a bony defect of the external cortex after biopsy of the lesion. The nerve was reconstructed with a sural nerve graft.Due to the slow growing nature and encapsulation of schwannomas, surgical resection seems the best treatment with optimal functional outcomes and low recurrence rate even with a minimally invasive approach. Sagittal split ramus osteotomy and segmental mandibulectomy are reserved to large or posterior lesions to aid with radical resection or improve exposure and reconstruction.
Resection of a giant intraspinal and extraspinal schwannoma with cystic change using a two-step surgery: a case report

Abstract:Background: Schwannomas originating from the intravertebral canal rarely extend into the paravertebral region or form large masses. There are few reports on such medical cases, and their clinical diagnosis and management are poorly understood. Here, we report a case of an intraspinal schwannoma with a giant extraspinal mass in a middle-aged Chinese woman and the clinical implications of the symptoms, diagnosis, and treatment of thoracic vertebral schwannoma. Case presentation: A 59-year-old female patient who presented with sudden lower limb numbness and difficulty in walking was referred to our department. Magnetic resonance imaging (MRI) revealed a tumor compressing the T11-T12 spinal canal and extensively invading the left thoracic cavity to form a giant paravertebral mass. Schwannoma was diagnosed after intraoperative pathological examination, in which tumor cells were immunoreactive to S100 and Sox10 but negative for smooth muscle actin (SMH). Intraspinal mass resection through the posterior approach and extraspinal chest mass resection under video-assisted thoracoscopy through the left lateral approach were performed simultaneously, and instrumentation was used to maintain thoracic spinal stability. Conclusion: In patients with large schwannomas, customized treatment plans are crucial. Tailoring the approach to an individual's specific condition by considering factors such as tumor size and location is essential for optimal outcomes.

Abstract:Objective To study the clinicopathological features,diagnosis and differential diagnosis of intraspinal nerve sheath myxoma.Methods We described a case of intradural extramedullary spinal nerve sheath myxoma and reviewed the related literature.Results A 53-year-old male patient presented with bilateral lower limbs weakness and MRI revealed a well-demarcated intradural extramedullary tumor at the L1 lever.Histological study revealed characteristic myxoid lobules that were separated by fibrous septa or bands of more compact cellular area.The tumor ceils were diffusely positive for S-100 at cytoplasm and nucleus.Conclusion Intraspinal nerve sheath myxoma is extremely rare.Diagnosis is confirmed by combining pathology with immunohistochemistry.Literatures revealed most intraspinal nerve sheath myxomas are located at lower segment of spinal cord.There is a slight predominance in males.No recurrences are reported.
