Abstract:Schwannoma is a proliferation of neoplastic Schwann cells. Schwannomas comprise 8-10% of all primary intracranial tumors. Primary intraorbital schwannoma arising from the ciliary nerves in the uvea, which accounts for 1-2% of all intracranial tumors, is a rare intraocular neoplasm. Intraocular schwannoma frequently masquerades as melanoma, reflecting the difficulty in clinically distinguishing it from malignant melanoma and the requirement for a histopathological diagnosis. The aim of the present study was to report a case series of 3 patients diagnosed with intraocular schwannoma at the Department of Ophthalmology, Peking University People's Hospital (Beijing, China). Patients with intraocular schwannoma were identified by searching the computerized database and patient medical records of the Department of Ophthalmology of Peking University People's Hospital. The patients (2 men and 1 woman; mean age, 34 years; age range, 25-48 years) were all treated by trans-scleral local resection, and schwannoma was confirmed by biopsy. The study found that choroidal schwannoma has a variety of clinical manifestations, with iridodialysis, subluxation of the lens and exudative detachment of the retina observed. The present study indicates that a pathological biopsy is required for diagnosis and that the optimal therapy is local resection.

Abstract:Intraosseous schwannoma of the mandible is rare, with diagnostic and therapeutic challenges. The aims of this study were to report new cases of intraosseous schwannoma of the mandible and to propose a clinical classification, providing suggestions for treatment methods. The cases of 13 patients treated at the authors' hospital and 86 cases reported previously in the literature were reviewed. The most common clinical feature was facial swelling (60/93). The rate of cortical thinning or expansion was 44.8% (43/96); widening of the inferior alveolar nerve canal on radiographs was observed in 15 patients.

Abstract:Intraventricular schwannomas are extremely rare, typically benign tumors originating from Schwann cells, which are not normally found within the ventricular system. Their presence challenges conventional understanding of tumor origins and complicates diagnosis and management. We report the case of a 19-year-old female presenting with a drop attack and headache, with no significant medical history. MRI revealed a heterogeneously enhancing lesion in the right lateral ventricle. Differential diagnoses included malignant tumors; however, histopathological examination post-surgical resection confirmed an intraventricular schwannoma. Postoperative outcomes were favorable, with successful CSF diversion via a right occipital ventriculoperitoneal shunt for isolated right temporal hydrocephalus. This case is notable for its atypical presentation in a young patient, challenging the conventional understanding that intraventricular schwannomas primarily affect older individuals. In addition, the correct diagnosis and successful management of a rare intraventricular schwannoma underscores the importance of considering this rare diagnosis in patients with nonspecific neurological symptoms and intraventricular lesions. This case, alongside the literature review, enriches the body of evidence on intraventricular schwannomas, highlighting the critical role of surgical intervention and the need for a comprehensive diagnostic approach.

Abstract:Schwannomas are the most common type of spinal tumor, and they most commonly occur in intradural extramedullary locations. Intramedullary schwannomas of the central nervous system are very rare and are difficult to diagnose using preoperative imaging. Here, we report a rare, tiny cervical intramedullary schwannoma and review the literature regarding the clinical presentation, magnetic resonance imaging, pathology, and surgical experience associated with this rare tumor type.

Abstract:Background context Intramedullary schwannoma is a kind of relatively rare tumors with only a few literature reports. This study was aimed to report the clinical characteristics of intramedullary schwannoma and discuss imaging findings and treatment strategies. Methods The inclusion criterion was consecutive patients with intramedullary schwannomas who were surgically treated in our institution between 2017 and 2022. Data included clinical characteristics, radiological features, surgical management and prognosis. Clinical and follow-up details of all cases were collected and reviewed. Results There were three males and eight females. The mean age of all patients was 45 years old (Range 26-77 years). Cervical spine (4 cases, 36.4%), thoracic spine (4 cases, 36.4%) and lumbarsacral spine (3 cases, 27.3%) involvement were found respectively. Weakness, numbness and pain of limbs were the main symptoms at administration. Pre-operative MRI all demonstrated lesion with spinal cord medullary invasion and well demarcated margins. The post-operative histological examination all showed benign lesions and confirmed the schwannoma. Conclusions Intramedullary schwannoma is a very rare tumor. This article presented a series of 11 cases of intramedullary schwannoma with sharp margins and well-enhanced feature. Prognosis and functional recovery were good after gross total resection.

Abstract:Intraparenchymal schwannomas are extremely rare intracranial tumors. Although their occurrence has been reported in various age groups, they are most frequently seen in individuals under 30 years of age. A 65-year-old woman was brought to the emergency department with impaired consciousness due to spontaneous intracerebral hemorrhage in the right basal ganglia (BG). Approximately 3 months later, the patient was taken to another hospital due to decreased consciousness, and a subsequent brain computed tomography scan showed a mass-like lesion in the right BG. The tumor was removed, and the biopsy result revealed an intraparenchymal schwannoma with no involvement of the ventricular ependymal lining. An immunohistochemical analysis revealed a high Ki-67 labeling index, indicating rapid growth without malignancy.

Abstract:BACKGROUND Schwannomas are uncommon tumors originating from Schwann cells, forming the neural sheath. They account for approximately 2%-6% of all mesenchymal tumors and are most commonly identified in peripheral nerve trunks, with rarity in the gastrointestinal tract. Among gastrointestinal locations, the stomach harbors the majority of nerve sheath tumors, while such occurrences in the sigmoid colon are exceptionally infrequent. CASE SUMMARY This study presented a clinical case involving a 60-year-old female patient who, during colonoscopy, was diagnosed with a submucosal lesion that was later identified as a nerve sheath tumor. The patient underwent surgical resection, and the diagnosis was confirmed through immunohistochemistry. This study highlighted an exceptionally uncommon occurrence of a nerve sheath tumor in the sigmoid colon, which was effectively managed within our department. Additionally, a comprehensive review of relevant studies was conducted. CONCLUSION The preoperative diagnosis of nerve sheath tumors poses challenges, as the definitive diagnosis still relies on pathology and immunohistochemistry. Although categorized as benign, these tumors have the potential to demonstrate malignant behavior. Consequently, the optimal treatment approach entails the complete surgical excision of the tumor, ensuring the absence of residual lesions at the margins.

Abstract:Introduction and importance: Gastric schwannoma is a rare and benign tumor originating from the peripheral nerves of the stomach. Despite its benign nature, this tumor typically remains asymptomatic for an extended period, and its radiological and endoscopic presentation poses challenges in distinguishing it from other gastric mesenchymal tumors. Case presentation: Here, we present a rare case of a patient experiencing gastric pain and melena secondary to a gastric mass. The initial preoperative diagnosis indicated a gastrointestinal stromal tumor, but subsequent pathological and immunohistochemical staining of the surgical specimen confirmed the presence of gastric schwannoma. Discussion: To gain insights into this uncommon condition, we conducted an electronic search on PubMed using the keywords "gastric schwannoma" and "gastric neurinoma." Our focus centered on case series containing more than five cases of gastric localization, resulting in the analysis of 14 case series involving a total of 321 patients. Our review aims to comprehensively discuss the clinical, radiological, and therapeutic aspects associated with this rare disease. Conclusion: In the absence of a definitive preoperative diagnosis, the surgical approach is considered the primary treatment for resectable gastric schwannoma, given its excellent long-term outcomes. However, further studies are imperative to better define the role of endoscopic resection in managing this condition.

Abstract:Background: Intraparotid facial nerve schwannoma (IFNS) is rare and its definite preoperative diagnosis is challenging.Objective: To improve available knowledge regarding the diagnosis of IFNS and to suggest an appropriate treatment plan.Material and methods: We retrospectively analyzed medical records of IFNS patients at our hospital. Inclusion criteria were surgery (from January 2000, to December 2016) for a parotid mass, pathologically diagnosed as a schwannoma.Results: The study included 42 eligible patients who had undergone tumor resection from 5977 parotid tumor patients. Mostly presented hard-textured (18/39) or medium-textured (15/39), with limited mobility (21/39) mass (three tumors were not palpable). Their facial nerve function outcomes were House-Brackmann Grade I (n=14), Grade II (n=7), Grade III (n=11), Grade IV (n=5), Grade V (n=3), and Grade VI (n=2). Significant differences were noted in results based on different surgical methods used (p=.000) and tumor involvement (p=.002).Conclusions and significance: A hard-textured tumor with limited mobility mass in the parotid gland should prompt the diagnosis of a schwannoma. Tumors involving main trunk usually lead to unsatisfactory facial nerve outcomes. Facial nerve preservation should always be essential, and stripping surgery or intracapsular enucleation could be the preferred surgical methods of choice.

Abstract:Objective Intraocular lymphoma is very rare and its pathologic diagnosis is difficult because of less samples cart be obtained.To know more of the diagnosis and treatment rule by analyzing the clinical features and treatment outcomes and reviewing the articles of intraocular lymphoma.Design Three cases of clinically diagnosed intraocular lymphoma were retrospectively analyzed and some articles were reviewed.Outcomes Case Ⅰ was first clinically diagnosed bilateral intraocular lymphoma and cerebellums lymphoma happened half year after eyes radiotherapy.Case 2 was first diagnosed as left ganglion basilare lymphoma and left ocular symptoms occurred 3 monthes after systemic chemotherapy and cranial radiotherapy.Case 3 was diagnosed as bilateral intraocular lympha.All of three cases presented with decreased visualacuity in suffered eyes.Fundus lesion manifestered with multiple yellowish white pathby dominated in 2 patients.Vitrina lesions were obvious in all of the three patients.Radiotherapy was administered to SUffered eyes for all three patients with total dose 30-40 Gy in 1.8-1.qGy fractions.The vision improved to some extent after radiotherapy in all of the 3 patients and all alive and under following up.Conclusion Diagnosis of intraocular lymphoma is difficult.Sever vitrina lesions and charateristic finding of fundus are helpful for the diagnosis of intraualar lympha.Radiotherapy of the eye is a very effective treatment method and increased attention to systemic or intrathecal Chemotherapy is needed.Ocular radiotherapy is effective to control primary lesion.

Abstract:European Journal of Ophthalmology, Ahead of Print. The optic nerve sheath is a rare site for schwannoma, to our knowledge 19 optic nerve sheath schwannoma (ONSS) cases have been reported. Difficulty can arise in diagnosis as imaging findings can be relatively non-specific. We describe a case of ONSS that mimicked orbital cavernous venous malformation pre-operatively. A 43-year-old woman presented with right subacute visual loss, reduced vision to 6/48 and signs of optic neuropathy. Endocapsular excision of the mass, which was adherent to the optic nerve, was accomplished from an endoscopic endonasal approach utilizing a 5-hand technique of retrocaruncular dissection. Histology showed spindle-shaped tumour cells with S100 positivity, consistent with ONSS. At 6 months post-operatively vision had improved to 6/6. We show that an endoscopic endonasal approach, augmented by transcaruncular retraction, can be utilized to excise ONSS with an excellent outcome. We review published cases of ONSS, including demographic and clinical features, differential diagnosis based on radiological features, and described outcomes, which are generally poor.

Abstract:Background This study aimed to investigate the clinical features, diagnostic criteria, treatment options, and prognosis of patients with gastric schwannoma (GS). Methods We collected the clinical data of all patients pathologically diagnosed with GS in Zhejiang Provincial People's Hospital from May 2012 to October 2021. Results A total of 26 cases of GS were analyzed clinicopathologically, where the sizes of the tumor were found to be in the range of 1–6 cm (mean: 3.16 cm, median: 3.05 cm). A computed tomography (CT) scan analysis revealed that most masses were either moderately progressive or uniformly enhanced. According to ultrasound gastroscopy results, most of them were hypoechoic masses. There were 23 cases of surgery and three cases of endoscopic submucosal tumor dissection. Immunohistochemistry demonstrated that S100 was positive in 26 patients, immunomarker SOX10 was positive in five, whereas CD34, CD117, and SMA were negative in most patients. CK (Pan), Dog-1, and Desmin were also found negative. All 26 cases were followed up after the conclusion of the study where no evidence of recurrence or metastasis was observed. Conclusions GS is a unique form of peripheral schwannoma. The diagnosis of this type of tumor depends on the pathology and immunohistochemistry of the individual. The key to treating this type of tumor is endoscopy and surgery. Follow up and related literature review showed that GS was a benign tumor with little possibility of malignant transformation.

Abstract:Intraocular malignant tumors including primary and metastatic tumors, are mainly found in Retina and uvea, and very few cases originate from the sclera and optic nerve. Intraocular tumors can endanger the patient's vision and even life, and proper treatment is vital. There have been several traditional treatments for intraocular tumors, such as radiotherapy, chemotherapy and surgery. In recent years, new methods have been developed in clinical applications including anti-VEGF and gene therapy. This paper aims to provide a timely review about recent progress in the treatment of intraocular malignant tumor.

Abstract:Purpose Schwannoma is a rare mesenchymal tumor. In this study, we analyzed clinicopathologically 99 schwannomas.This retrospective study delves into the clinical, pathological, and immunohistochemical dimensions of abdominal schwannomas. Results A cohort of 99 cases, comprising 4 malignant and 95 benign schwannomas, was meticulously examined. Clinical analysis revealed a notable gender distribution (1:1.7, male to female) and an average age of 58.5 years. The majority of cases were asymptomatic. Tumor sizes ranged from 0.5 to 30 cm, with distinct locations in the stomach for most benign cases and the abdomen/small intestine for malignancies. Initial misdiagnoses were frequent. Pathological evaluations revealed distinct features, including Antoni A and B patterns, spindle cells, and lymphatic sheath structures in benign schwannomas. Malignant cases exhibited atypical cells, ulcers, and invasive growth. Immunohistochemical markers, such as S100, SOX10, and vimentin, consistently demonstrated positivity by contributing to accurate diagnoses. Treatment outcomes indicated a poor prognosis in malignant cases, with overall survival ranging from 10 to 41 months. Conversely, benign cases displayed no recurrence or metastasis during follow‐up, despite atypical behaviors. Conclusion This study underscores the rarity of abdominal schwannomas and underscores the need for a comprehensive diagnostic morphology and immunohistochemistry. SOX10 emerges as a crucial and specific marker for accurate diagnosis. Further research is imperative to refine diagnostic protocols and enhance our understanding of the clinical behavior of abdominal schwannomas.

Abstract:Angiosarcoma arising in a schwannoma is extremely rare with only eleven cases having been reported in the English literature. We describe here three further cases occurring in adult males with a pre‐existing longstanding schwannoma. The tumor arose each from the vagus, ischiadic and adrenergic nerve respectively. None of the patients had von Recklinghausen's disease. Microscopically, the tumor was composed of a mixture of a benign schwannoma and an epithelioid angiosarcoma. The two components changed abruptly within the tumor. The endothelial cell differentiation was confirmed by immunohistochemistry. A review of published reports, including the present cases, suggests a poor prognosis with a high rate of local recurrence, distant metastasis and mortality.

Abstract:Objective To investigate the clinical features and surgical treatment of intramedullary spinal schwannomas.Methods 8 cases of iutramedullary spinal schwannomas were respectively reviewed and related clinical data,including manifestations,imaging features,operative outcomes was analyzed.Results Tumors were totally removed in all the 8 patients.During follow - up period from 3 months to 9 years,muscle strength significantly improved in 5 patients with limb weakness before operation.6 cases complained of abnormal sensitivity.No tumor recurrence was found in the rechecking MRI of the 8 patients.Conclusions Intramedullary spinal tumors,locating at dorsalateral side,with clear boundary,homogenous enhancement and neuralgic pain,should be highly suspected intramedullary shwannomas.Once diagnosed,surgical resection is the best choice.Clinical outcome is closely related to the surgical approach and skill.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Neurosynovial tumors, originating from Schwann cells within nerve sheaths, are benign entities, with 25% to 45% manifesting in the head and neck region. However, occurrences in the pterygopalatine fossa (PPF) are exceptionally rare, and only a handful of cases have been documented. In this report, we present the unique case of a 6-year-old child exhibiting a sizable soft tissue mass in the left PPF, extending into the inferior orbital fissure. The patient underwent successful intranasal endoscopic removal of PPF schwannoma utilizing the prelacrimal recess approach, with postoperative pathology confirming the diagnosis of schwannoma. Schwannomas within the PPF are particularly uncommon, and instances of such tumors in pediatric patients are even more exceptional. This case highlights the diagnostic and therapeutic challenges associated with PPF schwannomas in children, emphasizing the significance of a multidisciplinary approach for optimal management. In addition, a comprehensive literature review is presented to provide insights into the existing knowledge on this rare entity, further contributing to the understanding of pediatric PPF schwannomas.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Introduction</h3><p>Synovial sarcoma (SS) is a tumor of unknown origin and is extremely rare in the central nervous system. Most studies on intracranial SS included only one or two cases. To better understand the disease, we review a series of primary intracranial SS.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Method and materials</h3><p>16 primary intracranial SS in Tiantan Hospital during 2008–2017 were included. Radiological results showed that the supratentorial region (81.25%) was the most common site of the brain involved. All patients were misdiagnosed as non-SS tumors. Gross total resection (GTR) was achieved in 12 cases (75.0%), and subtotal resection (STR) was achieved in 4 cases. All cases showed the characteristic SYT-SSX fusion gene, as detected by RT-PCR. The mean progression-free survival time (PFS) was 10.0 months and the mean overall survival time (OS) was 15.5 months. Multivariate analysis revealed that GTR and postoperative adjuvant radiotherapy were independent factors for PFS (HR = 6.143, 95% CI = 1.491–25.312; <em>P</em> = 0.012, HR = 6.143, 95% CI = 1.491–25.312; <em>P</em> = 0.012 respectively) and OS (HR = 9.000, 95% CI = 1.627–49.773; <em>P</em> = 0.012, HR = 0.017, 95% CI = 0.001–0.213; <em>P</em> = 0.002 respectively).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Intracranial SS were more frequently observed in the supratentorial region and in young patients without sex predilection. We recommend adjuvant radiation regardless of the extent of resection. More patients and longer follow-up periods were needed to further elucidate the biological features of intracranial SS.</p>

Abstract:The extraventricular neurocytoma of the sellar region (EVNSR) is a rare disease, it is difficult to make exact diagnosis of and operate on patients.
