Abstract:Deep sternal wound infection (DSWI) is a severe complication in patients after open heart surgery (OHS). But there is a lack of appropriate imaging tool to detect the infection sites, which may lead to incomplete debridement. The present study aims to investigate the value of 18 F-fluorodeoxyglucose positron emission tomography/computed tomography (18 F-FDG PET/CT) in comparison with CT scan in diagnosing and localising DSWI. A total of 102 patients with DSWI after OHS were retrospectively collected from January 2012 to December 2017 in our hospital. All the patients had surgical debridements for DSWI with pretreatment imaging of either 18 F-FDG PET/CT or CT scan. The sensitivity, specificity, and accuracy of localising infection sites were compared between PET/CT and CT groups, with surgical, microbiological, and histopathological findings as the gold standard. The length of hospital stays and the rate of recurrence were also compared. Ten patients in the PET/CT group had a follow-up PET/CT scan after debridement, and the correlations between the changes of PET/CT findings and surgical outcomes were analysed. 18 F-FDG PET/CT is more accurate than CT in diagnosing and localising DSWI after OHS, which leads to a more successful surgical debridement with a lower rate of recurrence and a shorter length of hospital stay. In addition, follow-up PET/CT after debridement could evaluate the treatment effect.

Abstract:Objective:To evaluate the qualitative and localization diagnostic value of PET/CT in chronic sternal median incision infection after heart surgery and the guiding function of PET/CT image in debridement .Methods:Twenty‐one patients with chronic sternal median incision infection admitted to our hospital from February 2015 to December 2015 underwent PET/CT examinations within one week prior to surgery were selected .Detection of extent of sternal wound infection and localization diagnoses of costal chondritis were conducted by using PET/CT and CT ,and the results were compared with the diagnosis results during surgery .The qualitative and localization diagnostic value of PET/CT in chronic sternal median incision infection were evaluated .Debridement was guided by the PET/CT images .Patients were followed up for 6‐16 months after operation ,and the debridement and reconstructive surgery effect on chronic sternal median incision infection was evaluated . Results:There were 20 cases of sternal osteomyelitis ,18 cases of mediastinitis ,and 9 cases of costal chondritis .Accuracies of PET/CT in above diagnoses were all 100% .Sensitivity ,specificity ,positive predictive value ,negative predictive value ,and accuracy of PET/CT in localizing involved costal cartilages were 85 .71% ,99 .74% ,96 .00% ,98 .99% ,and 98 .81% , respectively .Sensitivity , negative predictive value , and accuracy of PET/CT in localizing involved costal cartilages were significantlyhigherthanthoseofCT(Pvalueswere0.002,0.008,and0.01).Curerateofpatientswithcostalchondritisafter first debridement operation was 85 .7% .Conclusions:PET/CT is of high accuracy in qualitative diagnosis and localization compared with CT in chronic sternal median incision infection after heart surgery , which is promising in guidance of debridement operation .
18F-FDG PET/CT for the Early Prediction of the Response Rate and Survival of Patients with Recurrent or Metastatic Breast Cancer.

Abstract:The present study aimed to explore the value of fludeoxyglucose F 18 positron emission tomography-computed tomography (PET/CT) for the early prediction of chemotherapy remission rates and survival in patients with recurrent and metastatic breast cancer. A total of 24 patients diagnosed with recurrent or metastatic breast cancer between 2009 and 2014 were enrolled. All patients underwent a PET/CT examination prior to (PET/CT1) and following (PET/CT2) chemotherapy. Differences of PET/CT1 maximal standardized uptake values (SUVmax), PET/CT2 SUVmax, ΔSUVmax and the ΔSUVmax% between objective remission (OR) and non-OR groups were measured. Survival differences between OR and non-OR groups and the overall survival (OS) between metabolic responsive and metabolic non-responsive groups were analyzed. In the present study, it was revealed that ΔSUVmax and ΔSUVmax% were significantly higher in the OR group compared with the non-OR group (P<0.001). Overall survival was significantly prolonged in the OR and metabolic responder groups compared with their respective control groups (P<0.001 and P<0.01, respectively). ΔSUVmax% were significantly positively associated with OS (r2=0.266; P<0.01). In conclusion, PET/CT may be valuable for the early prediction of the chemotherapy efficacy and survival of patients with recurrent or metastatic breast cancer.

Abstract:Objective This study evaluated the usefulness of fluorodesoxyglucose marked by fluorine-18 (18F-FDG) positron emission tomography ( PET) and computed tomography ( CT) in patients with suspected cardiovascular implantable electronic device (CIED) infection. Methods From Jan 2013 to Jun 2014, patients treated for CIED infection in our center were enrolled. The patients, suspected as CIED infection without confirm evidence from symptom, blood culture, echocardiography, underwent 18F-FDG PET/CT. Post 1 year follow up, analysis the accuracy of diagnosis depended on 18F-FDG PET/CT. Result From Jan 2013 to Jun 2014, there were 227 patients treated for CIED infection in our cen-ter. 10 of 227 cases was suspected as CIED infection without confirm evidence from symptom, blood culture, echocardio-graphy. Eight were suspected as infective endocarditis (IE), 2 as pocket infection. The results of 18F-FDG PET/CT sug-gested no infection in 2 patients with suspected pocket infection, 1 with suspected IE. One patient with suspected IE was diagnosed as bacteremia. 1 patient with vegetation, suspected IE, was diagnosed as no infection. All of the cases received standard treatment based on 18F-FDG PET/CT. Post follow up of 1 year, there were no recurrence and new infection ca-ses. Conclusions PET/CT is useful in diagnosis of CIED infection and classification.

Abstract:Due to untypical clinical manifestations, difficulty of diagnosis and treatment, and poor prognosis, the early and accurate diagnosis of cardiovascular infection is very important. 18F-fluorodeoxyglucose (FDG) PET/ CT has been widely applied in the diagnosis of infection. This article reviews the role of 18F-FDG PET/ CT in the diagnosis and treatment of cardiovascular infection, such as infectious endocarditis, cardiac implantable electronic device infection, and vascular prosthetic graft infection.

Abstract:BACKGROUND:While fever of unknown origin (FUO) remains a challenging problem in clinical practice, fluorine-18 fluorodeoxyglucose ((18)F-FDG) positron emission tomography/computed tomography (PET/CT) has been considered helpful in diagnosing its cause. The present study is set to evaluate the diagnostic value of PET/CT for patients with FUO.METHODS:We analyzed the records of 48 patients with FUO (34 men and 14 women; mean age of 57-year-old with a range between 24- and 82-year-old). The patients were examined by (18)F-FDG PET/CT and the results were compared to a final diagnosis that was established by additional procedures.RESULTS:A final diagnosis was established for 36 patients (75%). Among them, 15 patients had infectious diseases, 12 patients had malignancies, and 9 patients had non-infectious inflammatory diseases. Thirty-two abnormal PET/CT results correctly revealed the source of fever (true-positives). Abnormal PET/CT results were considered false-positives for 8 patients without diagnoses. Normal PET/CT results in 4 patients with no diagnoses were classified as true-negatives. Four patients with normal PET/CT results with diagnosed cause for FUO were considered false-negatives. Therefore, PET/CT had a positive predictive value of 80%, a negative predictive value of 50%, a sensitivity of 89%, and a specificity of 33% in patients with FUO.CONCLUSIONS:Our study demonstrated that FDG-PET/CT is a valuable imaging tool for the identification of the etiology in patients with FUO. The results suggest that this procedure may be considered as a second-line test, especially when conventional structural imaging was normal or unable to distinguish lesions from benign and malignant.

Abstract:Background: The current study sought to demonstrate the advantages offered by fluorine 18-fluorodeoxyglucose ((18)F-FDG) positron emission tomography/computed tomography (PET/CT) in patients supported with continuous-flow left ventricular assist devices (CF-LVADs) in detecting infection and the consequent effect on clinical decisions. Methods: Between April 2009 and September 2013, 40 PET examinations were performed in 31 patients (78.1% men; mean age, 51.0 ± 14.9 years) supported with a CF-LVAD. In group A (19 examinations in 14 patients), PET/CT was performed to detect infectious focus in patients without external signs of driveline involvement but with at least two of the following infection signs: recurrent fever, positive blood culture, or elevated infectious indicators. In group B (21 examinations in 17 patients), PET/CT aimed to assess the internal extension of infection in patients with external signs of driveline infection. Results: In 50% of the cases of the patients in group A, abnormal (18)F-FDG uptake (9 patients) was related to VAD components. Matching the results with the final diagnosis, we reported 9 true-positive, 8 true-negative, no false-negative, and 2 false-positive findings. New information unrelated to VAD was found in 9 patients (50%): pneumonia in 3, colon diverticulitis in 3, sternal dehiscence in 1, paravertebral abscess in 1, and erysipelas in 1. In group B, superficial abnormal (18)F-FDG uptake was found at the piercing site of the driveline in 2 patients, deeper extension of infection along the driveline in 10, initial involvement of the pump housing in 2, and full involvement of the device in 4. These findings contributed to changing the clinical management in 84.2% of group A patients and in 85.7% of group B patients: 16 patients were scheduled for urgent transplantation, 2 underwent surgical revision of the driveline, 7 required prolonged antibiotic therapy, and 3 underwent colonoscopy. Conclusions: This single-center experience highlights the diagnostic value of PET/CT in detecting the localization and internal extension of infection to internal VAD components. Moreover, this information notably influences the therapeutic management.

Abstract:Aim: Presence and consequent extent of infection in patients on continuous-flow left ventricular assist devices (CF-LVADs) can be challenging with the current diagnostic tools. The present study sought to demonstrate the diagnostic power of 18F-Fluorodeoxyglucose-Positron-Emission Tomography/Computed Tomography (18F-FDG PET/CT) in detecting infection in patients supported with CF-LVAD. Background: The present study sought to demonstrate the diagnostic power of 18F-fluorodeoxyglucose-positron-emission tomography/computed tomography (18F-FDG PET/CT) in detecting infection in patients supported with CF-LVAD. Methods and results: Between July 2009 and April 2016, 61 PET/CT examinations were performed in 47 patients (median age 64.13 years, IQR 18.77) supported with a CF-LVAD. PET/CT assessments were performed qualitatively and quantitatively at three different levels: at the piercing site of driveline (first level), along the intracorporeal course of driveline (second level), and around the device (third level). Final diagnosis of LVAD infection was prospectively performed and was based upon microbiological samples taken at hospital admission, during the surgical revision/transplantation and recurrence of symptoms on long-term follow-up. At last follow-up a total of 40 (65.57%) final diagnoses of LVAD-infection could be ascertained. Matching the final diagnosis with the PET/CT assessments the sensitivity, specificity, and positive and negative predictive value were 90.0, 71.4, 85.71, and 78.94%, respectively. Level sub-analyses of SUV max showed an optimal discriminator power for levels 1 and 2 (AUC of level 1-0.824, P < 0.001; AUC of level 2-0.849, P < 0.001, respectively). At the third level semi-quantitative analysis showed poor discriminator power (AUC 0.589, P = 0.33). Qualitative visual analysis instead indicated a trend toward significance (P = 0.07). Conclusions: Quantitative 18F-FDG PET/CT is an optimal diagnostic tool in detecting superficial and deep driveline infections. However, diagnostic accuracy with regard to the diagnosis of pump housing infection is limited. Here, clinical and qualitative PET/CT analyses must be better considered.

Abstract:18F-FDG-PET/CT is indicated in the workup of patients with suspected infective endocarditis to detect intra-cardiac and disseminated infections, as well as its source. We present the case of a 66-year-old female patient known for recurrent diabetic foot infection, with equivocal TTE results and persistent MRSA bacteremia despite medical management. PET/CT revealed evidence of left foot osteomyelitis. Whole body PET/CT diagnosed native mitral valve infective endocarditis (IE) and right lower lobe segmental pulmonary artery uptake, consistent with septic pulmonary embolism (PE).

Abstract:Background: Infection of Cardiac Implantable Electronic Devices (CIEDI) is a real public health problem. The main aim of this study was to determine the diagnostic performance of 18F-FDG PET/CT in the diagnosis of CIEDI. Methods: A total of 48 patients, who performed 18F-FDG PET/CT for the clinical suspicion of CIEDI were retrospectively analyzed; all patients were provided with a model with procedural recommendations before the exam. Sensitivity (Se), specificity (Sp), positive predictive value (PPV), negative predictive value (NPV) and diagnostic accuracy (DA) of 18F-FDG PET/CT were calculated; the reproducibility of qualitative analysis was assessed with Cohen’s κ test. The semi-quantitative parameters (SUVmax, SQR and TBR) were evaluated in CIEDI+ and CIEDI− patients using the Student’ t-test; ROC curves were elaborated to detect cut-off values. The trend of image quality with regards to procedural recommendation adherence was evaluated. Results: Se, Sp, PPV, NPV and DA were respectively 96.2%, 81.8%, 86.2%, 94.7% and 89.6%. The reproducibility of qualitative analysis was excellent (K = 0.89). Semiquantitative parameters resulted statistically different in CIEDI+ and CIEDI− patients. Cut-off values were SUVmax = 2.625, SQR = 3.766 and TBR = 1.29. Trend curves showed increasing image quality due to adherence to procedural recommendations. Conclusions: 18F-FDG-PET/CT is a valid tool in the management of patients suspected of CIEDI and adherence to procedural recommendations improves its image quality.

Abstract:Background: One of the most drastic complications of median sternal incision is deep sternal wound infection (DSWI), as it can lead to prolonged hospitalization, increased expected costs, re-entry into the ICU and even reoperation. Since the pectoralis major muscle flap (PMMF) technique was proposed in the 1980s, it has been widely used for sternal reconstruction after debridement. Although numerous studies on DSWI have been conducted over the years, the literature on DSWI in Chinese population remains limited. The purpose of this study was to investigate the clinical characteristics of DSWI in patients and the clinical effect of the PMMF at our institution. Methods: This study retrospectively analyzed all 14,250 consecutive patients who underwent cardiac surgery in the Department of Cardiothoracic Surgery of Drum Tower Hospital from 2001 to 2020. Ultimately, 134 patients were diagnosed with DSWI.,31 of whom had recently undergone radical debridement and transposition of the PMMF in the cardiothoracic surgery or burns and plastic surgery departments because of DSWIs, while the remaining patients had undergone conservative treatment or other methods of dressing debridement. Results: In total, 9,824 patients were enrolled in the study between 2001 and 2020, of whom 134 met the DSWI criteria and 9690 served as controls. Body mass index (OR = 1.08; P = 0.02; 95% CI, 1.01 similar to 1.16) and repeat sternotomy (OR = 5.93; P< 0.01; 95% CI, 2.88 similar to 12.25) were important risk factors for DSWI. Of the 134 patients with DSWI, 31 underwent the PMMF technique, and the remaining 103 served as controls. There were significant differences in coronary artery bypass grafting (CABG) (P< 0.01), valve replacement (P = 0.04) and repeat sternotomy (P< 0.01) between the case group and the control group. The postoperative extubation time (P < 0.001), ICU time (P < 0.001), total hospitalization time (P < 0.001) and postoperative hospitalization time (P< 0.001) in the PMMF group were significantly lower than those in the control group. The results of multivariate regression analysis showed that PMMF surgery was an important protective factor for the postoperative survival of DSWI patients (OR = 0.12; P = 0.04; 95% CI, 0.01 similar to 0.90). Conclusions: Staphylococcus aureus was the most common bacteria causing DSWI, which was associated with BMI and reoperation, and can be validly treated with PMMF.

Abstract:ObjectiveThe purpose of this study was to estimate the value of a novel imaging technique in necrotic tissue debridement of diabetic foot ulcers (DFU), using near-infrared (NIR) fluorescence molecular imaging (FMI) of indocyanine green (ICG).MethodsThe patients with DFU were included and divided into a FMI navigation debridement group and a traditional group. The FMI of the wounds and the debridement effect of each group was analyzed, including debridement frequency, length of hospital stay, and the histological examination of removed tissue.ResultA total of 40 patients with DFU (23 cases in the FMI group and 17 cases in the traditional group) were included in this study. The positive rate of wound fluorescence in the FMI group was 96%, and no patients had related side effects. The pathological examination confirmed that the tissue emitting NIR fluorescence was totally necrotic tissue. In addition, the debridement frequency in the FMI group is less than that in the traditional group, but there is no difference in the length of hospital stay between the two groups.ConclusionICG FMI technique can accurately illuminated necrotic tissue, with high sensitivity, easy operation and good safety, which would be a new way to precisly guide the process of debridement in the clinic.

[Value of (18)F-Fluorodeoxyglucose Positron Emission Tomography/computed Tomography in Early Diagnosis of Lung Cancer].

Abstract:OBJECTIVE:To evaluate the application value of (18)F-fluorodeoxyglucose ((18)F-FDG) positron emission tomography/computed tomography (PET/CT) in early diagnosis of lung cancer.METHODS:Retrospective analyses were conducted for 347 cases of pulmonary space-occupying lesions at Zhongshan Hospital from June 2010 to October 2012. The diagnostic validity of PET/CT and fluorodeoxyglucose maximum standardized uptake value (SUVmax) of lesions were compared respectively. Among different morphological characteristics, pathologic types and levels of tumor markers. The diagnostic value of PET/CT was also evaluated along with serum tumor markers for lung cancer.RESULTS:SUVmax was positively correlated with lesion size (r = 0.484, P < 0.05) and negatively with tumor differentiation degree (r = -0.232, P < 0.01). It was significantly higher in tumor marker positive group than the negative group (10.6 ± 5.5 vs 7.6 ± 5.4, P < 0.05). The diagnostic specificity, sensitivity and accuracy of PET/CT were 50.0%, 96.6% and 89.3% in lung cancer. And the greater the lesion, the higher the diagnostic accuracy (P < 0.05). PET/CT plus serum tumor markers could boost the diagnostic specificity of lung cancer by 30% (P < 0.01).CONCLUSIONS:(18)F-FDG PET/CT has high diagnostic values for early stage pulmonary nodules. It can also suggest the differentiation degree of lung cancer. Combined use of serum tumor markers and (18)F-FDG PET/CT increases the early diagnostic specificity of lung cancer.

Abstract:Purpose: To systematically review and conduct a meta-analysis of the available evidence for PET/CT using F-18-fluorodeoxyglucose as tracers in the imaging of infectious endocarditis (IE). Methods: Databases, including PubMed, Embase, and Web of Science, were searched for studies that examined the diagnostic value of F-18-FDG PET/CT for patients with IE. The reference lists following review articles and those of the included articles were checked to complement the electronic searches. The data extraction and methodological quality assessment were completed by two independent reviewers, and the meta-analysis was then conducted using Meta-Disc software, version 1.4. Results: A total of six studies involving 246 patients was included. The results of the meta-analysis indicated that the pooled sensitivity was 61% (95% confidence interval (CI), 52 - 88%), and the pooled specificity was 88% (95% CI, 80 - 93%). The positive likelihood ratio was 3.24 (95% CI, 1.67 - 6.28, p = 0.224), and the negative likelihood ratio was 0.50 (95% CI, 0.32 - 0.77, p = 0.015). The diagnostic odds ratio was 6.98 (95% CI, 2.55 19.10, p = 0.145), the overall area under the curve (AUC) was 0.8230 (SE = 0.1085), and the Q* value was 0.7563 (SE = 0.0979). Conclusions: F-18-FDG PET/CT is currently not sufficient for the diagnosis of IE because of its low sensitivity. However, F-18-FDG PET/CT might be a useful tool for the diagnosis of skin and pocket cardiovascular implantable electronic device (CIED) infections and prosthetic valve endocarditis (PVE).

Abstract:The key challenge in the treatment of bacterial infection is rapid identification of bacteremia at an early stage of the diseases. Currently available imaging systems such as computed tomography (CT) and magnetic resonance imaging (MRI) can only detect bacterial infection after they have become systemic or have caused significant anatomical tissue damage, and at this stage infection are challenging to treat due to the high bacterial burden. To this day positron emission tomography (PET) imaging has showed the great potential for improving the diagnosis of bacterial infection because of the high sensitivity of PET radionuclides, capability of detecting molecular biology in details (even prior to anatomic change). Fluorodeoxyglucose (FDG) PET has been developed for bacterial imaging with incredible success. Whole body PET imaging with FDG for the diagnosis of bacterial infection and monitoring response to treatment has been well established. FDG-PET will not only help to accelerate the diagnosis of infection but improve the bacterial treatment. In this review, we focus on FDG-PET imaging for diagnosing bacterial infection in the clinic.
