Abstract:Background. To evaluate the differences in the corneal higher-order aberrations (HOAs) and optical quality of the ptosis eyes compared with the normal fellow eyes in the unilateral congenital ptosis patients. Methods. A matched case-control study was performed in 17 pairs of eyes in 17 unilateral congenital ptosis patients. The ptosis eye was enrolled in the ptosis group while the normal fellow eye was enrolled in the normal group. The HOAs obtained from Pentacam HR and the optical quality parameters obtained from Optical Quality Analysis System (OQAS) were compared between the two groups. Results. There were significant differences in a vertical coma and vertical trefoil on the anterior corneal surface between the ptosis group and the normal group p<0.05. The OQAS parameters in the ptosis group were significantly different from those in the normal group p<0.05. The vertical fissure height (VFH) showed a significant correlation with the total HOAs, 3rd HOAs, and vertical coma on the anterior corneal surface p<0.05. Conclusions. Compared with the normal fellow eyes, the ptosis eyes showed significantly degraded optical quality. The eyelid position was important for the changes of corneal HOAs in the ptosis eyes. The reasons for the discrepancy of the OQAS parameters between the ptosis eyes and normal fellow eyes were multiple.
Changes of Corneal Wavefront Aberrations in Dry Eye Patients after Treatment with Artificial Lubricant Drops

Abstract:Purpose.To evaluate the corneal aberration changes in dry eye patients after treatment with artificial eye drops.Methods.Thirty mild to moderate dry eye patients treated with artificial eye drops and twenty comparable dry eye patients were recruited as controls. Anterior corneal aberrations over 3 mm and 5 mm analytical zones including total, 3rd to 5th high order aberrations (HOAs), spherical aberration (SA), and vertical (V-coma) and horizontal coma (H-coma) obtained from corneal topography data at baseline and 2 weeks after treatment were evaluated.Results.For 3 mm zone, trefoils, V-coma, H-coma terms, and 3rd and 5th HOAs were significantly decreased (p<0.05) in the treatment group. For 5 mm zone, instillation of eye drops reduced H-coma, SA terms, 3rd to 5th orders, and total HOAs all showed a significant decrease (p<0.05). The root mean square analysis of the Zernike terms also showed similar statistical results. For control group, all individual terms and total HOAs did not have significant changes over 3 mm and 5 mm zones (p>0.05).Conclusions.Treatment with artificial eye drops can effectively improve the corneal optical quality of dry eye patients by ameliorating the HOAs of anterior corneal surface.
Changes of Corneal Tomography in Patients with Congenital Blepharoptosis

Abstract:The study aimed to evaluate the effect of drooped eyelid on corneal tomography in congenital blepharoptosis patients. Sixty-four patients with congenital blepharoptosis and 64 age- and sex- matched healthy subjects were included. According to the eyelid margin to corneal light reflex distance (MRD), eyes with congenital blepharoptosis were categorized as mild, moderate, or severe. The eyes were scanned using the rotating Scheimpflug camera. Increased topometric parameters were observed in moderate and severe blepharoptosis. Back corneal elevations at the thinnest point were significant higher for mild (P = 0.009), moderate (P < 0.001), and severe (P < 0.001) congenital blepharoptosis compared with controls. Maximum Ambrósio’s relational thickness (ART) was decreased in eyes with severe blepharoptosis (P < 0.001). Fnal D values were significantly higher in moderate (P < 0.001) and severe blepharoptosis (P < 0.001) groups than that of controls. There were significant correlations between MRD and most corneal tomographic parameters. Our findings indicated there was a trend toward subclinical keratoconus-like changes in the corneas of congenital blepharoptosis, with the increase of ptosis severity.
Measurement of anterior segment in myopic eyes with Pentacam scheimpflug system

Abstract:Objective: To compare the biological parameters of normal and myopic eyes with Pentacam system and establish the preliminary data of various degree of myopic eyes. Methods: Repeatability was determined from 3 successive measurements of 50 healthy eyes with Pentacam system. Pentacam scheimplug imaging system was used to measure the anterior segment on 523 cases (1 024 eyes). Measured eyes were divided into emmetropia (240 eyes), low myopia (280 eyes), moderate myopia (272 eyes) and high myopia (232 eyes) according to the mean spherical equivalent. Results: The variations of ocular anterior segment were as follows in 50 healthy eyes: coefficient of cornea anterior keratometry 0.30%, posterior keratometry 0.64%, corneal apical pachymetry 0.70%, central anterior chamber depth (ACD) 0.70%, cornea volume 0.81%, anterior chamber volume 1.98%, anterior chamber angle 2.68%, pupil diameter 4.73%. The anterior keratometry elevated from (43.34 ± 1.46) D to (43.83 ± 1.28) D and pupil diameter decreased from (3.13 ± 0.65) mm to (2.92 ± 0.43) mm with the increase of myopic diopter of eyes (P < 0.05, P < 0.05). The other biometric data had no statistically significant difference among the four groups (P > 0.05). Conclusion: Pentacam scheimpflug system can provide rapid, simple and accurate measurement to ocular anterior segment, which is helpful in establishing standards of anterior segment of eyes.
Evaluation of Corneal Thickness and Volume Parameters of Subclinical Keratoconus Using a Pentacam Scheimflug System.

Abstract:PURPOSE:To evaluate the efficacy of corneal thickness and volume parameters measured with the Pentacam Scheimflug system in discriminating subclinical keratoconus corneas and normal corneas.METHODS:This study included 19 patients (19 eyes) with subclinical keratoconus and 29 patients (29 eyes) with myopic astigmatism as the control group. Corneas were evaluated using a Pentacam Scheimflug system (2002 model, Oculus, Berkshire, UK). We evaluated corneal thickness and volume parameters, including central corneal thickness (CCT), minimum corneal thickness (MCT), depressed corneal thickness (DCT), maximum progression index (MaxPI), average progression index (AvPI), minimum progression index (MinPI), and corneal volume values for the 3.0 mm (CV3), 5.0 mm (CV5), and 7.0 mm (CV7) central circles. Receiver operating characteristic (ROC) curves were used to compare the sensitivity and the specificity of the parameters. Partial least squares (PLS) analysis was used to construct models with corneal thickness and volume parameters for early diagnosis.RESULTS:There were significant differences in all parameters (p < 0.001), except CV3 and CV5, between subclinical keratoconus and normal corneas. The diagnostic accuracy for all parameters was high, with an area-under-the-curve (AUC) of approximately 1. The optimal cutoff values for DCT, MinPI, AvPI, and MaxPI were 5.5, 0.95, 1.05, and 1.45 for subclinical keratoconus, respectively. The PLS model for subclinical keratoconus fit well to the CCT, MCT, MaxPI, AvPI, MinPI, CV3, and CV7 data.CONCLUSION:Thickness and volume parameters measured by the Pentacam Scheimflug system effectively discriminate subclinical keratoconus corneas from normal corneas.
Structural Imaging of Benign Conjunctival Proliferative Diseases Using Optical Coherence Tomography

Abstract:Objective: To use anterior segment optical coherence tomography (AS-OCT) for structural imaging and accurate measurements of pinguecula and pterygium in order to understand the structural features of these lesions. Methods: This study was a prospective parallel control study. Lateral bulbar conjunctiva of 25 cases (30 eyes) that included pinguecula (10 eyes of 8 cases), pterygium (10 eyes of 9 cases), and normal eyes (10 eyes of 8 cases), were enrolled from the Department of Ophthalmology of the Fourth Affiliated Hospital of Zhejiang Medical University from January to April 2018. The anterior segment module of an Optovue optical coherence tomography angiography (OCTA) was used for measurements. The thickness of the bulbar conjunctiva epithelium and propria tissues at sites at a distance of 0.0 mm, 0.5 mm, 1.0 mm, 1.5 mm, 2.0 mm and 2.5 mm from the scleral process were stratified within the 2.5 mm range, were measured, compared and further analyzed. The data were described by means of mean±standard deviation. Repeated measurement analysis of variance was used for comparison of each point in the group, one-way analysis of variance was used for comparison between groups. Results: Compared with normal eyes and pinguecula eyes, pterygium showed thickened epithelium in 0.5 mm point (P<0.05). In the 2.5 mm range on the nasal side, pterygium and pinguecula showed thickened subepithelium tissues (P<0.05), especially for pterygium, compared to normal eye tissue. Conclusions: AS-OCT can image microstructures in benign conjunctival proliferative diseases, resulting in accurate quantitative diagnosis and evaluation. The thickness of subepithelium tissues increases in pinguecula and pterygium.
Anterior Segment Biometry with Phenylephrine and Tropicamide During Accommodation Imaged with Ultralong Scan Depth Optical Coherence Tomography

Abstract:Purpose To investigate the influence of phenylephrine and tropicamide on anterior segment biometry with ultralong scan depth optical coherence tomography (UL-OCT) during accommodation. Methods In this study, 20 left eyes of healthy volunteers with a mean ± standard deviation age of 31.05 ± 5.84 years and a mean refraction of −1.16 ± 1.11 diopters (range 0∼−3.0 D) were imaged using UL-OCT after instillation of artificial tears, phenylephrine, and tropicamide in three follow-up trials, respectively. At each follow-up trial, two repeated measurements were performed at states of relax and 5D accommodative stimulation. The dimensional parameters included central corneal thickness (CCT), anterior chamber depth (ACD), pupil diameter (PD), lens thickness (LT), and horizontal radii of the lens anterior and posterior surface curvatures (LAC and LPC). Results Tropicamide led to larger pupil, deeper ACD, thinner LT, and flatter crystalline lens surface (P < 0.05). Phenylephrine induced an increase in PD (P < 0.05), while no significant changes were seen in ACD, LT, LAC, and LPC (P > 0.05). CCT did not change after both phenylephrine and tropicamide instillation in this study (P > 0.05). Tropicamide induced the loss of accommodation and phenylephrine achieved pupil dilation without affecting the accommodation. PD, ACD decreased, LT increased significantly and the anterior and posterior surface of the lens in a 6.294 mm of diameter optical zone became steeper during accommodation after administration of phenylephrine (P < 0.05). Conclusion The anterior segment physiology changed after tropicamide instillation. Besides, tropicamide induced the loss of accommodation and phenylephrine preserved the accommodation with a larger pupil. And, the anterior and posterior surface of lens in a 6.294 mm of diameter optical zone became steeper during the accommodation.

Abstract:To measure by ultralong scan depth optical coherence tomography (UL-OCT) dimensional changes in the anterior segment of human eyes during accommodation.Evaluation of diagnostic test or technology.Forty-one right eyes of healthy subjects with a mean age of 34 years (range, 22-41 years) and a mean refraction of -2.5 ± 2.6 diopters were imaged in 2 repeated measurements at minimal and maximal accommodations.A specially adapted and designed UL-OCT instrument was used to image from the front surface of the cornea to the back surface of the crystalline lens. Custom software corrected the optical distortion of the images and yielded the biometric measurements. The coefficient of repeatability and the intraclass correlation coefficient were calculated to evaluate the repeatability and reliability.Anterior segment parameters and associated repeatability and reliability upon accommodation. The dimensional results included central corneal thickness (CCT), anterior chamber depth and width (ACD, ACW), pupil diameter (PD), lens thickness (LT), anterior segment length (ASL = ACD + LT), lens central position (LCP = ACD + 1/2LT), and horizontal radii of the lens anterior and posterior surface curvatures (LAC, LPC).Repeated measurements of each variable within each accommodative state did not differ significantly (P>0.05). The coefficients of repeatability (CORs) and intraclass correlation coefficients for CCT, ACW, ACD, LT, LCP, and ASL were excellent (1.2%- 3.59% and 0.998-0.877, respectively). They were higher for PD (18.90%-21.63% and 0.880-0.874, respectively) and moderate for LAC and LPC (34.86%-42.72% and 0.669-0.251, respectively) in the 2 accommodative states. Compared with minimal accommodation, PD, ACD, LAC, LPC, and LCP decreased and LT and ASL increased significantly at maximal accommodation (P<0.05), whereas CCT and ACW did not change (P>0.05).The UL-OCT measured changes in anterior segment dimensions during accommodation with good repeatability and reliability. During accommodation, the back surface of the lens became steeper as the lens moved forward.The authors have no proprietary or commercial interest in any of the materials discussed in this article.
Effects of eyelid pressure on corneal tomography and biomechanics: Quantitative analysis using a novel blepharo-tensiometer

Abstract:Purpose: This study aimed to validate a blepharo-tensiometer, and investigate the relationship between eyelid pressure (ELP) and corneal tomography/biomechanics. Methods: Repeatability of the blepharo-tensiometer was evaluated at different inclination angles: 10°, 15°, and 20° for the upper eyelid (U10, U15, and U20) and 45°, 50°, and 55° for the lower eyelid (L45, L50, and L55). Reproducibility was evaluated in terms of different operators and inclination angles. Both the maximum and average ELP were evaluated. Best-corrected visual acuity, manifest refraction spherical equivalent, eyelid types, palpebral fissure height, exophthalmometry, axial length, and corneal tomographic/biomechanical parameters were measured. Spearman analysis and generalized estimating equations were used to explore potential correlations. Results: This study included 36 eyes from 36 subjects. The ICCs for repeatability were comparable between different inclination angles, so U15 and L50 were selected for further analysis. The ICCs for reproducibility in terms of different operator or inclination angle also achieved good to excellent outcomes. Certain associations have been revealed between the ELP and corneal tomographic/biomechanical parameters. Tomographically, ELP influences corneal front J0, front asphericity, and the index of surface variance; whereas biomechanically, ELP affects the time at first applanation, deformation amplitude at highest concavity, and biomechanically corrected intraocular pressure. Conclusions: The tensiometer showed good precision. The study also showed that ELP causes a flattening effect on the peripheral cornea, with greater ELP linked to better corneal biomechanical properties in healthy subjects. These findings may potentially illuminate new avenues in the field of corneal disorders and beyond.
Evaluation of Meibomian Gland and Tear Film Changes in Patients with Pterygium

Abstract:Background: Recent studies have demonstrated that pterygium has a close relationship with dry eye disease. This study is to determine abnormalities in meibomian gland and tear function in patients with pterygium and to assess the relationship between the variables. Materials and Methods: Forty eyes from forty patients with primary nasal pterygium and forty eyes from forty volunteers without ocular pathologies were enrolled in this study. Ocular surface disease index scores, meibomian gland expression scores, lid margin abnormality scores, meiboscore, tear film breakup time (BUT), Schirmer test (SIT) value, and the lower tear meniscus height (TMH), tear meniscus depth (TMD), and tear meniscus area (TMA) using Fourier domain optical coherence were performed. Analysis of variance was applied for intergroup comparisons. A statistical significance level of P < 0.05 was considered. Results: Ocular symptom scores, BUT scores, lid margin abnormality, meibomian gland expression, and meiboscore were significantly higher in pterygium patients than in controls (P < 0.01 for all scores). However, the SIT scores, the lower TMH, TMD, and TMA values did not revealed a significant difference between two groups (all P > 0.05). Multivariate regression analysis demonstrated that meiboscore significantly correlated with ocular symptom scores, BUT, lid margin abnormality scores, and meibomian gland expression scores. Conclusions: Meibomian gland function may be altered in pterygium patients, which is associated with uncomfortable ocular symptoms. Being aware of meibomian gland changes seems essential to understand the complex relationship among pterygium, tear film functions, and ocular surface changes.
[Measurement of Central Corneal Thickness in Myopic Eyes with Ultrasound and Pentacam Scheimpflug System].

Abstract:OBJECTIVE:To assess the accuracy and variability of central corneal thickness measurements in myopic eyes obtained by ultrasound pachymetry and the Pentacam scheimpflug imaging system.METHODS:First, corneal thickness in 50 healthy eyes was measured three times with the Pentacam system to evaluate the repeatability. Then, Scheimpflug imaging system (Pentacam) and A-scan ultrasonic pachymeter (SP-3000) were used to measure the corneal thickness on 217 myopic cases (433 eyes) undergoing laser in situ keratomileusis (LASIK) and 63 cases (126 eyes) 6 months after LASIK. The results were compared.RESULTS:Repeated-measures ANOVA showed the repeatability of the Pentacam measurements was high (P > 0.05). Pre-operatively, the value of corneal central thickness measured by ultrasonic pachymetry and the Pentacam system was (537.26 +/- 31.61) microm and (538.08 +/- 32.72) microm, respectively. There was no significant difference between the results of these two methods (t = 1.867, P > 0.05). The difference between these two methods ranged -35 - 34 microm, and was within -20 - 20 microm in 95.38% cases. Post-operatively, the value of corneal central thickness measured by ultrasonic pachymetry and the Pentacam system was (464.21 +/- 41.04) microm and (465.17 +/- 42.65) microm, respectively. There was no significant difference between these two methods, too (t = 0.913, P > 0.05). There was a significant linear correlation between the two methods (P < 0.001). The thinnest point of cornea was mostly located surrounding the apex and inclined towards inferotemporal.CONCLUSION:The Pentacam scheimpflug system offers an acceptable method for evaluating corneal pachymetry, which is useful in corneal refractive surgery.
Application of a Scheimpflug-Based Biomechanical Analyser and Tomography in the Early Detection of Subclinical Keratoconus in Chinese Patients

Abstract:Abstract Background In vivo corneal biomechanics evaluation has been used to help screen early keratoconus in recent years. This study is to evaluate the value of a Scheimpflug-based biomechanical analyser combined with tomography in detecting subclinical keratoconus by distinguishing normal eyes from frank keratoconus (KC) and forme frusta keratoconus (FFKC) eyes in Chinese patients. Methods Study design: diagnostic test. This study included 31 bilateral frank keratoconus patients, 27 unilateral clinically manifesting keratoconus patients with very asymmetric eyes, and 79 control subjects with normal corneas. Corneal morphological and biomechanical parameters were measured using a Pentacam HR and a Corvis ST (OCULUS, Wetzlar, Germany). The diagnostic ability of computed parameters reflecting corneal biomechanical and morphological traits [including the Belin-Ambrósio deviation index (BAD_D), the Corvis biomechanical index (CBI) and the tomographic and biomechanical index (TBI)] was determined using receiver operating characteristic (ROC) curve analysis and compared by the DeLong test. Additionally, the area under the curve (AUC), the best cut-off values, and the Youden index for each parameter were reported. A novel corneal stiffness parameter, the stress-strain index (SSI), was also compared between KC, FFKC and normal eyes. Results Every morphological and biomechanical index analysed in this study was significantly different among KC, FFKC and normal eyes (P = 0.000). The TBI was most valuable in detecting subclinical keratoconus (FFKC eyes), with an AUC of 0.928 (P = 0.000), and both forms of corneal ectasia (FFKC and frank KC eyes), with an AUC of 0.966 (P = 0.000). The sensitivity and specificity of the TBI was 97.5 and 77.8 % in detecting FFKC and 97.5 and 89.7 % in detecting any KC, respectively, with a cut-off value of 0.375. The morphological index BAD_D and the biomechanical index CBI were also very useful in distinguishing eyes with any KC from normal eyes, with AUCs of 0.965 and 0.934, respectively. The SSI was significantly different between KC, FFKC and normal eyes (P = 0.000), indicating an independent decrease in corneal stiffness in KC eyes. Conclusions The combination of a Scheimpflug-based biomechanical analyser and tomography could increase the accuracy in detecting subclinical keratoconus in Chinese patients. The TBI was the most valuable index for detecting subclinical keratoconus, with a high sensitivity and specificity. Evaluation of corneal biomechanical properties in refractive surgery candidates could be helpful for recognizing potential keratoconic eyes and increasing surgical safety.
Clinical measurement of anterior chamber depth and lens thickness in myopic eyes with ultrasound and pentacam scheinpflug system

Abstract:Objective: Anterior chamber depth (ACD) and lens thickness (LT) measurement gives important details in cataract or glaucoma surgery. And ACD is also strictly required in refractive surgery, such as excimer laser photorefractive keratectomy or phakic anterior chamber lens implant. This investigation was to assess the accuracy of ultrasound and Pentacam scheinpflug imaging system measuring ACD, LT in myopic eyes. Methods: Thirty-eight eyes of 38 subjects aged 18-28 years underwent non-contact tonometry, autorefraction and subjective refraction examination. After accommodative paralysis, ACD and LT from 38 right eyes were measured with scheimplug imaging system (Pentacam) and A sonography. Results: The ACD value measured by Pentacam and A sonography was (3.33 ± 0.24) mm and (3.81 ± 0.24) mm, respectively, indicating a significant difference between the results of these two methods (P < 0.01) and a positive linear correlation between them (r = 0.967, P < 0.01). The LT value measured by Pentacam and A sonography was (3.34 ± 0.26) mm and (3.54 ± 0.17) mm respectively with the significant difference (P < 0.01) and a significant linear correlation between these two outcomes (r = 0.815, P < 0.01). The difference in LT between the two methods was higher with the increase of diopter of refractive error. Conclusion: Pentacam, as a new 3-dimensional mathematical model of the anterior segment, presents some different results from A sonography in the measurement of ACD and LT. Enough consideration should be payed to clinical evaluation of these results.

To determine the corneal biomechanical properties by using the Ocular Response Analyzer™ and to investigate potential factors associated with the corneal biomechanics in healthy myopic subjects. Methods. 135 eyes from 135 healthy myopic subjects were included in this cross-sectional observational study. Cornea hysteresis (CH), corneal resistance factor (CRF), cornea-compensated intraocular pressure (IOPcc), and Goldmann-correlated intraocular pressure (IOPg) were determined with the Reichert Ocular Response Analyzer (ORA). Univariate and multivariate regression analyses were performed to investigate factors associated with corneal biomechanics. Results. The mean CH and CRF were 9.82 ± 1.34 mmHg and 9.64 ± 1.57 mmHg, respectively. In univariate regression analysis, CH was significantly correlated with axial length, refraction, central corneal thickness (CCT), and IOPg (r = -0.27, 0.23, 0.45, and 0.21, resp.; all with p ≤ 0.015), but not with corneal curvature or age; CRF was significantly correlated with CCT and IOPg (r = 0.52 and 0.70, resp.; all with p < 0.001), but not with axial length/refraction, corneal curvature, or age. In multivariate regression analysis, axial length, IOPcc, and CCT were found to be independently associated with CH, while CCT and IOPg were associated with CRF. Conclusions. Both CH and CRF were positively correlated with CCT. Lower CH but not CRF was associated with increasing degree of myopia. Evaluation of corneal biomechanical properties should take CCT and myopic status into consideration.
Evaluation of Corneal Biomechanical Indices in Distinguishing Between Normal, Very Asymmetric, and Bilateral Keratoconic Eyes

Abstract:Purpose: To evaluate the ability of biomechanical indices provided by the Ocular Response Analyzer (ORA; Reichert Ophthalmic Instruments) and dynamic Scheimpflug analyzer (Corvis ST; Oculus Optikgeräte GmbH) to distinguish between normal eyes and eyes with very asymmetric ectasia (VAE) and mild and moderate keratoconus. Methods: This prospective, observational, and monocentric study included normal eyes (defined as keratoconus percentage index < 60, Belin/Ambrósio total deviation value [BAD-D] < 1.6, inferior-superior keratometry [I-S value] < 1.45 and maximum keratometry [Kmax] < 47) and eyes with clinical bilateral keratoconus (mild and moderate) and VAE (defined as unilateral keratoconus, where one eye showed a clinical keratoconus and the fellow eye was topographically normal [VAE-NT] or topographically and tomographically normal [VAE-NTT]). All eyes were measured by the Pentacam (Oculus Optikgeräte GmbH), ORA, and Corvis ST. Receiver operating characteristic curve analysis was performed to test the diagnostic ability. Results: Fifty-eight normal eyes and 238 ectatic eyes were included. The highest area under the curve (AUC) was provided by the Corvis Biomechanical Index (CBI) with an AUC of 0.979, followed by ORA corneal resistance factor (0.865), and corneal hysteresis (0.824) separating normal eyes from all ectatic subgroups. The AUC of the CBI was statistically significantly higher than all other parameters (DeLong test, P < .001). A sensitivity of 100% and 70.9%, respectively, and a specificity of 93.1% was found to distinguish normal eyes from VAE-NT and VAE-NTT using a cut-off value of 0.2. Conclusions: The assessment of biomechanical properties is an additional important method to evaluate corneal ectasia independent of its stage. The CBI provides further information for ectasia screening in cases where corneal topography and tomography are clinically not suspicious by using a cutoff of 0.2. Abstract:PURPOSE: The purpose of this study was to evaluate the anterior segment measurements according to refractive status in a sample of the Iranian population.SETTING: Rassoul Akram Hospital, Tehran University of Medical Sciences.MATERIAL AND METHODS: IN THIS STUDY, REFRACTIVE SURGERY CANDIDATES WERE ASSIGNED ACCORDING TO THE REFRACTIVE ERROR TO ONE OF THREE GROUPS : emmetropia, myopia, and hyperopia. Myopic eyes were further divided to four subgroups : simple myopic group, simple myopic astigmatism group, high myopic group, and high myopia with astigmatism group. Anterior segment measurements with the Pentacam Scheimpflug system were performed in the right eye of all subjects.RESULTS: The study sample was comprised of 283 subjects with a mean age of 29.1 ± 7.5 (standard deviation) years. Mean keratometry reading, Anterior chamber depth (ACD) and volume measurements were significantly higher in the myopic group and mean keratometry reading and anterior chamber angle measurements were significantly lower in the hyperopic group (P < 0.05, all comparisons). Maximum anterior elevation (AEmax) and maximum posterior elevation (PEmax), Q value, progression index, minimum corneal thickness, and corneal volume measurements were similar for all groups (P > 0.05, all comparisons). In the myopic subgroups, AEmax and PEmax and maximum keratometry (Kmax) were significantly higher, and ACD was lower in the astigmatic groups (P < 0.05, all comparisons). The Q value was less negative in low myopia (P < 0.05).CONCLUSIONS: Myopic eyes had steeper corneas than hyperopic eyes and anterior chamber measurements were significantly higher in the myopic eyes. In myopic eyes, AEmax and PEmax and Kmax measurements were higher, and ACD measurements were lower in the astigmatic groups.

Abstract:Objective The aim of this study is to investigate the influence of pterygium on corneal biomechanical properties by comparing the corneal hysteresis (CH) and the corneal resistance factor (CRF) measured by an ocular response analyzer (ORA) in normal and pterygium eyes.Methods In a case control study,40 patients with monocular nasal simple pterygiums were randomly selected.The contralateral eyes were used for comparison.CH,CRF,IOPg and IOPcc were measured with an ORA and corneal curvature were measured with an corneal curvemeter.A paired t test,linear correlation and regression analysis were used in this study.Results Both the mean CH and CRF in pterygium eyes were both lower than in normal eyes and the differences were statistically significant (t=2.637,2.415,P＜0.05).Both the mean horizontal corneal curvature and vertical curvature in pterygium eyes were lower than in normal eyes and the differences were statistically significant (t=12.447,P＜0.01; t=2.578,P＜0.05).In linear correlation and regression analysis,the D-values of CH and CRF between normal and pterygium eyes were both positively corrclated with that of corneal horizontal curvature (r=0.622,0.678,P＜0.01),and corneal astigmatism (r=0.525,0.515,P＜0.01).Conclusion The CH and CRF were significantly lower in pterygium eyes than in normal eyes,and the D-values of CH and CRF between normal and pterygium eyes were correlated with horizontal corneal curvature.Using the measurements of CH,CRF and corneal curvature is helpful to assess the severity of pterygium's influence on the cornea.
Analysis of Corneal Spherical Aberrations in Chinese Bilateral Ectopia Lentis Patients

Abstract:Purpose: To analyze the anterior, posterior, and total corneal spherical aberrations (ASA, PSA, and TSA) in patients with Chinese bilateral ectopia lentis (EL). Methods: A cross-sectional study was conducted to evaluate corneal spherical aberration (CSA) using a Pentacam system at the 6-mm optical zone. Axial length, keratometry, astigmatism, and corneal asphericity were also determined. Results: This study included 247 patients (420 eyes) with a mean age of 18.1 years. The values of ASA, PSA, and TSA were 0.136 ± 0.100 μm, −0.118 ± 0.030 μm, and 0.095 ± 0.095 μm, respectively. In the EL patients with Marfan syndrome (MFS), ASA and TSA were significantly lower than in the non-MFS patients (0.126 ± 0.094 μm vs. 0.155 ± 0.107 μm, P = 0.004 for ASA; 0.085 ± 0.091 μm vs. 0.114 ± 0.099 μm, P = 0.003 for TSA), whereas PSA was not significantly different ( P = 0.061). The values of ASA and TSA were significantly higher in the patients with EL aged ≥ 40 years old than in younger patients, whereas ASA and PSA were lower in patients aged <10 years old than in older patients (all P < 0.05). In the multiple linear regression analysis, age, keratometry, astigmatism, anterior asphericity, higher-order aberration (HOA), and lower-order aberration (LOA) were positively or negatively correlated with TSA in the patients with EL ( r = 0.681, P < 0.001). Conclusions: Corneal spherical aberration was low in the patients with EL especially for MFS and tended to increase with aging. Preoperatively, individual measurement of CSA was necessary for bilateral EL patients with MFS.

A Novel Method for Pachymetry Mapping of Human Precorneal Tear Film Using Pentacam with Fluorescein

Abstract:PURPOSE. To report a novel method for pachymetry mapping of human precorneal tear film using Pentacam.METHODS. Precorneal tear film is routinely undetected by Pentacam but could be well visualized with the aid of fluorescein. Twenty patients with dry eye and 20 age-matched control subjects with normal eyes were enrolled in this prospective study. The right eye of each subject was scanned once with Pentacam and rescanned after instillation of 1 mu L 0.1% fluorescein, and the differential map of corneal thickness between the two measurements was identified as the pachymetry map of tear film. Then the central tear film thickness was evaluated, and the pattern of each pachymetry map was determined.RESULTS. Mean central tear film thickness in dry eyes (22.2 +/- 4.5 mu m) was less than in normal eyes (24.7 +/- 3.9 mu m) (Student's t-test, P = 0.0614). Additionally, the tear film pachymetry map could be classified into three patterns: pattern 1, thickening upward; pattern 2, uniform distribution; pattern 3, thickening downward. Tear film pachymetry maps of normal eyes consisted of pattern 1 (40%), pattern 2 (40%), and pattern 3 (20%), whereas those of dry eyes consisted of pattern 1 (70%), pattern 2 (20%), and pattern 3 (10%). Dry eyes tended to have a higher proportion of pattern 1 pachymetry maps than normal eyes, though still no significant difference was found between two groups (Cochran-Mantel-Haenszel chi(2) test, P = 0.0852).CONCLUSIONS. This novel method feasibly could be used to map tear film thickness, and it provides a valuable means to investigate the spatial distribution of tear film. Evaluation of Corneal Biomechanics Using Brillouin Microscopy in Chinese Adults With Myopia

Abstract:Purpose: To evaluate the corneal biomechanical metrics of Chinese adults with myopia and identify relevant factors of Brillouin microscopy. Methods: In this cross-sectional study, corneal biomechanics in Chinese adults with myopia were quantified and analyzed using Brillouin microscopy and the Corvis ST (CST) (Oculus Optikgeräte GmbH) and analyzed. Univariate linear regression was used with potential factors including age, sex, spherical equivalent (SE), intraocular pressure (IOP), central corneal thickness (CCT), and mean keratometry (Kmean). Results: The study included 87 eyes of 87 participants (mean age: 24.47 ± 6.27 years). Central, Mean, maximum (Max), minimum (Min), standard deviation, and Max-Min Brillouin modulus (BM) values obtained from Brillouin microscopy exhibited values of 2.826 ± 0.039, 2.827 ± 0.027, 2.864 ± 0.034, 2.790 ± 0.038, 0.108 ± 0.042, and 0.074 ± 0.041 GPa, respectively. No significant correlations were found between BM parameters and age, sex, SE, IOP, or CCT. However, the Mean (β = -0.251, P = .019), Min (β = -0.315, P = .003), and Max-Min (β = 0.229, P = .033) BM values were significantly associated with Kmean. The Central, Mean, Min, and Max BM values negatively correlated with the Tomographic Biomechanical Index measured by CST (Spearman's r = -0.24, -0.35, -0.29, and -0.23, respectively, all P < .05). Conclusions: Brillouin microscopy accurately reflects corneal biomechanical parameters in Chinese adults with myopia, independent of IOP and CCT, with a good correlation with CST. Concurrent evaluation of the corneal curvature is imperative when employing Brillouin microscopy in clinical practice. [J Refract Surg. 2024;40(10):e768-e776.].
