Abstract:Objective To explore the long-term oncological outcomes in children undergoing laparoscopy for local adrenal neuroblastoma.Methods A retrospective review was conducted for 43 children with local adrenal neuroblastoma hospitalized at our hospital between July 2005 and July 2013.They fulfilled the inclusion criteria for adrenal neuroblastoma and underwent resection.Results Laparoscopic adrenalectomy was performed.There were 1 1 males and 4 females with a mean age of 39.47 (2-158) months.There were left (n =2) and right (n =13) adrenal lesions.According to the international neuroblastoma staging system,there were stage Ⅰ (n =12),stage Ⅱ (n =1) and stage Ⅳ (n =2).Two patients were converted into open surgery because of adhesions to renal vessels and diaphragmatic rupture.The tumor size,operative duration and blood loss volume were (3.92 ± 1.31) cm,(138.13 ± 45.63)min and (13.75 ± 11.32)ml respectively.The mean follow-up period was 26.93 (2-63) months.There was no recurrence for laparoscopic surgery.The overall 5-year survival rate of laparoscopy was 100％.Conclusions Laparoscopic resection of adrenal neuroblastoma is feasible and it may be performed with equivalent recurrence and mortality rates to open resection.For tumor size ＜6 cm without vascular encasement,laparoscopic surgery of adrenal neuroblastoma is preferred.

Abstract:Objective:To compare and evaluate the clinical value between laparoscopic adrenalectomy and open adrenalectomy for adrenal myelolipoma as the treatment of adrenal diseases. Methods: The clinical data of 37 cases underwent laparoscopic adrenectomy and 10 cases underwent open adrenalectomy were analyzed retrospectively. The differences in operating time, average bleeding volume during operation, hospitalization stay after operation. recovery time of GI function of the two procedures were compared. Results: In 37 laparoscopic adrenectomies 1 were converted to open surgery due to the thickness of perirenal fat layer, others were succeed. The mean operation time, average bleeding volume during operation, recovery time of GI function, hospitalization stay after operation of the laparoscopic group and open surgery group were:( 128.3±49.6)min vs(140.5±43.9)min,(53.3±57.1)ml vs (145.5±110.5)ml, (2.1±0.7)days vs(2.9±0.7)days, (5.6±1.0) days vs(10.7±4.9)days, respectively. Conclusions:In laparoscopic adrenalectomy group, the operation time, average bleeding volume during operation, hospitalization stay after operation, recovery time of GI function and occurrence of complication were superior to those in the open adrenalectomy group. Retroperitoneal laparoscopic adrenalectomy would become the first choice of the adrenal myelolipoma surgery.

Abstract:Objective To compare the clinical outcomes and the follow-up of retroperitoneal laparoscopic adrenalectomy and open adrenalectomy for the treatment of adrenal tumors in children. Methods The clinical data of 38 children underwent retroperitoneal laparoscopic adrenalectomy and 32 children underwent open adrenalectomy from June,2002 to March,2009 were analyzed retrospectively.The therapeutic effects and the follow-up data of the two groups were compared. Results No statistically significant difference was found in age,tumor size,and average operating time between the two groups.The intraoperative bleeding loss(95.4±32.0 ml),the mean time to oral intake(1.5±0.4 d),use of analgesic(21.1%),duration of postoprative hospital stay(5.5±0.8 d) and incidence of complications with group A were significantly superior to those(155.3±41.7 ml),(2.8±1.5 d),(50%),(8.5±2.1 d) with group B(P0.05).There was no statistically significant difference in the 5-year recurrence free survival rate and the 5-year overall survival rate between the two groups. Conclusions As compared with open adrenalectomy,retroperitoneal laparoscopic adrenalectomy can be used initially for adrenal tumors in children,with equivalent oncologic outcomes to open approaches while providing advantages of minimal invasion,less suffering,quicker recovery and less complication.

Abstract:Objective:To evaluate the reliability and safety of two different surgical approaches of laparoscopic adrenalectomy for children with adrenal neuroblastoma.Methods:A retrospective study was performed for five children undergoing laparoscopic adrenalectomy from February 2018 to August 2020.There were 6 boys and 3 girls with an average age of 22(1-65) months.The involved side was left ( n=3) and right ( n=6). There were abdominal pain ( n=2), hematuria ( n=1), diarrhea ( n=1), abdominal distension ( n=1) and non-discomfort ( n=4). Retroperitoneal ( n=2) and transperitoneal ( n=3) laparoscopies were performed.All procedures were successfully performed without any conversion into open surgery.The average operative duration was 137 min, the mean estimated volume of blood loss 16.3(5-40) ml and the mean tumor diameter 4.3(2.5-5.4) cm.Histopathological examination hinted at neuroblastoma ( n=5), ganglioneuroblastoma ( n=3) and ganglioneuroma ( n=1). Results:All of them recovered well postoperatively during a mean follow-up period of 8.6(2-35) months.There was no onset of recurrence or metastasis.Conclusion:Two different approaches of laparoscopic adrenalectomy may be safely and effectively performed for neuroblastoma.Retroperitoneal laparoscopy offers an advantage over exposing and handling blood vessels while transperitoneal laparoscopy is superior for operating space.

Abstract:Objective To evaluate clinical application of laparoscopic adrenalectomy for adrenal diseases.Methods Clinical data of the patients undergoing laparoscopic adrenalectomy (LA, n=160) and open adrenalectomy (OA, n=52) were retrospectively analyzed. Results There were 4 cases of open conversion(2.5%).The operation time were (115.03±48.67) min and (131.44±73.63) min in the LA and OA group, respectively( P>0.05).The blood loss during operation was (101.51±107.80) ml vs. (358.05±401.80) ml( t=-7.28 , P<0.05), and postoperative hospitalization was(4.50±1.90) days vs. (9.81±3.93 )days, respectively( t=-12.96 , P<0.05). There were 4 and 5 cases of complication in LA and OA group, respectively. The mean follow-up was (14.94±5.82)months(range 2～90 months), and there was no statistics difference between the two groups in terms of hypokalemia and hypertension improvement. Conclusion Compared with open adrenalectomy, laparoscopic adrenalectomy has advantages of minimal morbidity, small incision, less bleeding and rapid recovery. This method could be considered as the first option in the treatment of adrenal diseases.

Abstract:Objective:To compare laparoscopic adrenalectomy(LA)with open adrenalectomy(OA)for benign adrenal masses larger than 6cm.Method:The data of 50patients with large adrenal benign masses(6cm)from Jan.2009to Dec.2011were reviewed.Twenty-six patients underwent LA and 24for OA.The tumor size,the age of patients and operative time,volume of blood loss during operation,hospital stay,length of incision and the hemodynamic variation of pheochromocytoma underwent two types of operations were analyzed.Result:Histopathological examination confirmed adrenal adenoma in two,ganglioneuroma in ten,pheochromocytoma in 13, myelolipoma in 19,adrenal cyst combined with hemorrhage in three,schwannoglioma in two,Castleman disease in one.The mean age,size of the tumor were(45.3±15.6)years and(52.2±13.2)years(P=0.069),(7.75± 1.26)cm and(8.38±1.96)cm(P=0.098)in LA and OA group respectively.The mean operative time,blood loss,hospital stay,and length of incision were(65.2±23.4)min and(126.9±32.5)min(P0.01),(54.1± 19.5)ml and(205.0±49.6)ml(P0.01),(4.8±1.6)d and(7.6±2.1)d(P0.01),(7.56±2.06)cm and(18.28±3.25)cm(P0.01)in two groups respectively.There was no intraoperative complication and conversions to an open procedure in LA group.An unilateral nephrectomy was needed in one case with Castleman disease because of intraoperative injury of renal hilum in OA group.There was no recurrence in either group during follow-up period of three to 26months.The average peak intraoperative blood pressure and heart rate were lower in LA group than those in OA group(P0.05).Conclusion:Compared with open adrenalectomy,laparoscopic adrenalectomy is a safe and effective treatment for large benign adrenal masses because of its successful outcome in terms of operative time,blood loss,hospital stay despite of extended incision.

Abstract:To observe the clinical safety of laparoscopic adrenalectomy (LA) for larger adrenal tumors (Long diameter 5-9 cm). Methods: 41 open adrenalectomies and LAs were performed for patients with larger adrenal tumors (Long diameter 5-9 cm) in Department of Urology, Peking University First Hospital from 1995 to 2004. The perioperative clinical data were analyzed retrospectively. Results: The LAs group had lower rate of blood transfusion during operation (10.0%: 33.3%, P＜0.05), shorter time to take food and walk after operation (2.10 d: 3.28d, P＜0.01) and shorter time to stay in hospital after operation (6.90 d: 9.76 d, P＜0.001) than the open group. There were no significant differences between two groups for the operation time (151.0 min: 133.83 min, P＞0.05), the blood loss during operation (312.5 ml: 391.3 ml, P＞0.05), the patient percentage to enter into ICU (40.0%: 46.7%, P＞0.05), the time to pull out the drainage tube after operation (3.1 d: 3.5d, P＞0.05), the drainage volume (215.3 ml: 220.7 ml, P＞0.05), the blood transfusion rate after operation (10.0%: 6.7%, P＞0.05), the dosage of analgesic drugs used after operation (P＞0.05), the rate of symptomatic relief after operation (80.0%: 77.8%, P＞0.05) and the cost of hospitalization (16822.10 RMB:17247.10 RMB, P＞0.05). The complication rates for two groups were 9.1% (1/11) and 10.0% (3/30) respectively (P＞0.05). Conclusions: The larger size of adrenal tumors is not absolute contraindication of LAs for experienced endourologists. Larger adrenal tumors (long diameter 5-9 cm) can be performed by laparoscopic method safely and effectively for selected patients after sufficient preoperative preparation.
Comparison of 228 cases' clinical effects in laparoscopic adrenectomy via two approaches

Abstract:[Objective] To investigate the clinical results of the two procedures of laparoscopic adrenalectomy in embryonic natural orifice transluminal endoscopic surgery(E-NOTES) and retroperitoneal surgery.[Methods] From June 2009 to October 2011,the clinical data of 228 cases received laparoscopic adrenalectomy were analyzed retrospectively.[Results] Except for 1 case died of the original disease after 5 months,other operations were successfully performed.The rest patients were in stable blood pressure control,plasmatic hormone levels and other indicators returned to normal,and did not find obvious signs of tumor recurrence after CT inspection.E-NOTES had the same effect in the hospital days,postoperative pyrexia and complications,while the operative time is a little longer than retroperitoneal surgery.[Conclusion] E-NOTES is safe and effective,however the long-term clinical effect should be further researched and confirmed.

Abstract:Purpose To evaluate the feasibility of peritoneal laparoscopic excision of adrenal pheochromocytoma. Methods Peritoneal laparoscopic excision of adrenal pheochromocytoma was performed on 15 patients, the control group included 42 patients with adrenal pheochromocytoma who underwent open surgery.The postoperative outcomes were compared between the 2 groups. Results Fourteen peritoneal laparoscopic excisions of pheochromocytoma were successful, only 1 was converted to open surgery due to intraoperative bleeding. The mean largest dimension of the tumor was (3.4±1.8) cm (range,1.5 to 7.0) in laparoscopic surgery group vs (3.8±2.2) (1.5-10.5) cm in open surgery group (P0.05). The mean operative time was (94±34) min (range,75 to 170) in laparoscopic surgery group vs (131±38) (80-250) min in open surgery group (P0.01).The mean volume of blood loss was (45±36) (10-600) mL in laparoscopic surgery group vs (198±108) (50-2 000) mL in open surgery group (P0.01).Postoperative hospital stay was (6.2±1.1) (5-8) days in laparoscopic surgery group vs (9.3±1.3) (7-14) days in open surgery group (P0.01). All cases have been followed up 9 months to 3 years. Tumor recurrence was found in 1 case in open surgery group. Conclusions Based upon the experience and skills of the surgeons, peritoneal laparoscopic surgery for pheochromocytoma is feasible and safe, with no increase in the operation risks, and can be considered the preferred choice for pheochromocytoma.
Comparing perioperative outcomes of robotic-assisted laparoscopic adrenalectomy and laparoscopic adrenalectomy in the treatment of large adrenal tumors (≥5 cm): a propensity-matched score analysis

Abstract:Abstract Introduction: At present, the advantages of robotic-assisted laparoscopic adrenalectomy over laparoscopic adrenalectomy for large adrenal tumors (≥ 5 cm) are still controversial. Our study aimed to clarify whether robotic-assisted laparoscopic adrenalectomy has significant advantages. Materials and methods A total of 244 patients with adrenal tumors ≥ 5 cm who underwent minimally invasive surgery in our hospital from 2010 to 2021 were included in this study. There were 77 patients in the robotic-assisted laparoscopic adrenalectomy group and 167 patients in the laparoscopic adrenalectomy group. After propensity score matching, each group included 77 patients. Results There were no significant differences between the groups, in postoperative morbidity (p = 0.125)、estimated blood loss (p = 0.678)、operative time (p = 0.451), and postoperative hospital stay ( p = 1.000). The rate of conversion to open surgery in the robotic-assisted laparoscopic adrenalectomy group was significantly lower than that in the laparoscopic adrenalectomy group [0(0.0%) vs 6(7.8%), p = 0.031]. However, the total hospitalization cost in the robotic-assisted laparoscopic adrenalectomy group was significantly higher than that in the laparoscopic adrenalectomy group [53452 (46717, 60499) vs 33196 (27317, 41521), p < 0.001]. Conclusions Both robotic-assisted laparoscopic adrenalectomy and laparoscopic adrenalectomy are safe and effective in the treatment of large adrenal tumors. If the risk of conversion to open surgery is high, robotic-assisted laparoscopic adrenalectomy should be the first choice.

Abstract:Objective:To investigate the clinical efficacy of retroperitoneal laparoscopic excision of adrenal pheochromocytoma.Methods:Retroperitoneal laparoscopic resection of adrenal pheochromocytoma was successfully performed on 15 patients(retroperitoneal laparoscopic group).18 cases with adrenal pheochromocytoma underwent open surgery(open surgery group).The outcomes of the two groups were compared.Results:15 cases of the laparoscopic surgery group were successfully performed,tumor maximum diameter was 2.1-8.5 cm,average(4.2 ±1.8) cm;operation time was 55-180 min,average(85±35) min;bleeding volume was 25-105 ml,average(45±25) ml;intraoperative hypertension occurred in 2 cases,postoperative analgesics were applied for one time;getting out of bed after operation was 2-4 d,average(2.5 ±1.5) d;post operative hospital stay was 5-8 d,average(6.0±1.5) d.18 cases of open surgery group were successfully performed;tumor maxi mum diameter was 2.0-10.5 cm,average(4.5±2.5) cm;operation time was 90-250 min,average(140±50) min;bleeding vol ume was 95-650 ml,average(350±150) ml;blood transfusion in 6 cases,intraoperative hypertension occurred in 10 cases,postoperative analgesics were applied for 4 times;getting out of bed after operation was 5-7 d,average(5.5 ±1.5) d;postoperative hospital stay was 7-12 d,average(10.5±1.5) d.Conclusion:Based on the experience and skills of the surgeons,retroperitoneoscopic adrenalectomy for pheochromocvtoma has advantages such as shorter operation time,less blood loss,minimal invasion,better intraoperative control of blood pressure and similar clinical efficacy with open surgery.It is therefore a safe and effective method in treating pheochromocytoma.

Abstract:Objective:To compare the safety between open and laparoscopic surgery of adrenal pheochromocytoma.Methods:Laparoscopic excision of pheochromocytoma was performed on 26 patients with pheochromocytoma,which were compared with 26 patients with pheochromocytoma underwent open surgery at the same term.Results:All operations were successful in 2 groups,no patients recrudescence.Conclusions:Following some principles,based upon the experience and skills of the surgeons,laparoscopic surgery for pheochromocytoma less than 3.5cm is safe.

Abstract:OBJECTIVE:To compare the long-term results of laparoscopic and open radical resection for colorectal carcinoma.METHODS:Two hundred and fifteen patients with colorectal cancer from January 1996 to September 2000 were non-randomly divided into laparoscopic and open operation groups. Local recurrence, distant metastasis, 5-year survival rate and long-term postoperative complications were compared between the two groups.RESULTS:Eighty-seven cases received laparoscopic resection and 128 cases received open operation. There were no statistical differences in age, sex and tumor stage between the two groups (P > 0.05). The 5-year-survival rate was 70% in open operation group, and 78% in laparoscopic group (P > 0.05). There were no significant differences in the incidences of local recurrence, distant metastasis, incision seeding, and incision hernia between the two groups (P > 0.05). The complication rate of postoperative adhesive intestinal obstruction was significantly lower in laparoscopic group than that in open operation group (P< 0.05).CONCLUSIONS:Long-term results of laparoscopic resection are similar to those of open resection for colorectal carcinoma, but laparoscopic surgery has less long-term complications.
Comparison of Oncologic Clearance and Long-Term Outcomes of Laparoscopic Surgery Versus Open ;surgery in Radical Resection of Rectal Cancer

Abstract:OBJECTIVE:To compare the oncologic clearance and long-term outcomes between laparoscopic surgery and open surgery in radical resection of rectal cancer.METHODS:Clinicopathological and follow-up data of 1184 cases with rectal cancer undergoing radical resection from July 2005 to December 2011 were analyzed retrospectively. According to the surgical method, cases were divided into laparoscopy group (104 cases) and open group(1080 cases). Demographics, number of harvested lymph nodes, distance between distal margin and tumor, incidence of anastomotic complications, disease-free survival (DFS) and overall survival(OS) were compared between the two groups.RESULTS:There were no significant differences in the number of harvested lymph nodes (15.5 vs. 14.4, P>0.05), length of distal margin (2.5 cm vs. 2.1 cm, P>0.05) and incidence of anastomotic complications (1.9% vs. 1.9%, P>0.05) between the two groups. And there were no significant differences in DFS and OS between the two groups (both P>0.05). The 3-year and 5-year DFS in laparoscopy group were 79.0% and 69.3%, and were 78.0% and 72.5% in open group. The 3-year and 5-year OS in laparoscopy group were 93.5% and 81.2%, which were 87.6% and 80.7% in open group. There were no significant differences in DFS and OS after stratification by TNM stage.CONCLUSION:The oncologic clearance and long-term outcomes after laparoscopic surgery are comparable with open surgery in radical resection of rectal cancer.

Abstract:Objectives: To compare our modified retroperitoneoscopic adrenalectomy with the standard procedure. Methods: A total of 68 patients underwent modified retroperitoneoscopic adrenalectomy. Simultaneously, another 23 patients were treated using the standard procedure. The demographic data and perioperative outcomes of the patients were obtained for statistical analysis. Results: No significant differences in age, evaluated blood loss, oral intake, and hospital stay were observed. The mean operation time in the modified group was shorter than that in the standard group (58.3±16.4 vs. 79.5±23.6 min, P<0.05). The median total length of incision was shorter in the modified group than in the standard group (4.5±0.8 vs. 6.4±1.1 cm, P<0.05). The modified group also showed significantly better pain control (3 vs. 6 diclofenac sodium use, P<0.05) and cosmetic outcome (score, 9.1±1.8 vs. 8.2±2.1, P<0.05) than the standard group. Conclusions: Our modified retroperitoneoscopic adrenalectomy is a safe and alternative procedure for adrenal tumors. This procedure offers superior cosmetic outcome, better pain control, and shorter operation time compared with the standard retroperitoneal adrenalectomy.
Comparison of the Long-Term Outcomes of Patients Who Underwent Laparoscopic Versus Open Surgery for Rectal Cancer

Abstract:Purpose: Survival data of patients who underwent laparoscopic surgery compared with those who underwent open surgery for rectal cancer are limited. We compared long-term outcomes of laparoscopic surgery with those of open surgery in patients with rectal cancer.Methods: Between April 2007 and December 2013, a series of 879 patients with rectal cancer underwent surgery with curative intent (287 patients via laparoscopic surgery and 592 patients via open surgery). Age, gender, body mass index (BMI), American Society of Anesthesiologists (ASA) score, clinical stage and type of resection were matched by propensity scoring, and 382 patients (191 patients by laparoscopic surgery and 191 patients by open surgery) were selected for analysis. Intraoperative factors, postoperative data, long-term survival were evaluated.Results: There was no significant difference in preoperative data between the two patient groups. Blood loss was significantly lower in the laparoscopy group than in the open group, while operation time was significantly longer in the laparoscopy group than the open group. There were no significant differences in overall survival or recurrence-free survival. The patterns of recurrence were not different between the two groups.Conclusion: Laparoscopic surgery for patients with rectal cancer produces the same long-term results compared with open surgery in terms of survival and recurrence.
Clinical Efficacy Analysis of Different Surgical Methods for Giant Adrenal Tumors (≥10 cm): A Single-Center Study of 44 Cases

Abstract:Objective: The objective of this study was to compare the efficacy and safety of open adrenalectomy (OA) and laparoscopic adrenalectomy (LA) by summarizing the clinical features and treatment experience of giant adrenal tumors. Methods: The clinical data of 44 patients with adrenal tumors ≥10 cm admitted to the Second Hospital of Lanzhou University from January 2006 to August 2022 were retrospectively analyzed. The mean tumor diameter was 11.6 ± 1.8 cm. Regular follow-up was performed to observe the outcome of symptoms and the recurrence after operation. Results: All the 44 patients successfully completed the operation and were divided into the OA group (21 cases) and the LA group (11 cases of transabdominal laparoscopic adrenalectomy [TLA] and 12 cases of retroperitoneal laparoscopic adrenalectomy [RLA]) according to different operation methods. The analysis results showed that the proportion of estimated blood loss (EBL) >100 mL and the postoperative length of stay (PLOS) in the LA group were superior to those in the OA group, and there were no significant differences in other surgical indicators (P > .05); subgroup analysis was conducted for patients in the LA group according to different surgical pathways, and the results showed that there were no significant differences in each surgical indicator between TLA and RLA (P > .05). Among the 44 patients, 15 (34.1%) had intraoperative complications, 19 (43.2%) had postoperative complications, and 5 (21.7%) were converted from LA to OA. The median follow-up time was 68 (5-162) months. Conclusions: For adrenal tumors with diameter ≥10 cm, both LA and OA have relatively large damage to the patients, and the incidence of conversion to open surgery is also high. However, LA is superior to OA in EBL and PLOS. Given the fact that giant adrenal tumors are mainly pheochromocytoma and paraganglioma and have an increased possibility of malignancy, we suggest that the relationship between tumors and peripheral important blood vessels and organs should be carefully evaluated before operation, and the operative methods should be selected according to the experience of the surgeon.
Robotic Versus Laparoscopic Retroperitoneal Resection of Large Adrenal Tumors: A Propensity Score-Matched Cohort Analysis

Abstract:Abstract Background Laparoscopic adrenalectomy is the first-line treatment for benign adrenal tumors, but when the tumor is larger than 6 cm, its resection is controversial. The purpose of this study was to understand and compare the effects of robot-assisted and laparoscopic adrenalectomy on large adrenal tumors via a retroperitoneal approach. Methods We reviewed the perioperative data of patients who underwent robot-assisted and laparoscopic retroperitoneal adrenalectomy at the First Affiliated Hospital of Nanchang University between 2011 and 2020. After propensity score matching, the clinical data were analyzed by using the t test and chi-square test, as well as univariate and multivariate analysis. Results There is no significant difference between the two groups in operation time (RRA 191 ± 136min vs. 165 ± 83.8min, P = 0.158), postoperative complications (18.2% vs. 14.4%, P = 0.536), estimated blood loss (181 ± 167ml vs. 214 ± 341ml, P = 0.359), conversion rate (1.5% vs. 1.5%, P = 1.000) and postoperative hospital stay (6.55 ± 2.12 vs. 6.06 ± 2.39, P = 0.149). Multivariate analysis showed that larger tumors were associated with longer operative times. Female gender and postoperative complications were associated with longer hospital stays. BMI, conversion, history of abdominal surgery, and pathological type of patients affected the occurrence of postoperative complications. Conclusion it is safe and feasible for experienced surgeons to perform robotic adrenalectomy and laparoscopic adrenalectomy after the possibility of infiltration is ruled out on imaging before operation. However, when there is evidence of infiltration, we still recommend open surgery.
