Abstract:OBJECTIVE:To observe the results of two techniques of posterior chamber intraocular lens (IOL) implantation by a 4-point ciliary sulcus suture fixation combined with vitrectomy.METHODS:Lensectomy and vitrectomy combined with posterior chamber IOL implantation by a 4-point scleral fixation was performed in 23 cases with complicated ocular trauma, including 9 eyes had eyeball rupture, vitreous hemorrhage, retinal contusion and traumatic cataract; 6 eyes having eyeball rupture, vitreous hemorrhage, retinal contusion, intraocular foreign body and traumatic cataract; 8 eyes having ocular blunt trauma, vitreous hemorrhage, traumatic cataract and lens dislocation. Two techniques of IOL were randomly assigned: Alcon CZ70BD PMMA suspensory IOL was performed in 12 cases (Group I); Bausch & Lomb Akreos Adapt four-haptic hydrophilic acrylic foldable IOL was performed in 11 cases (Group II). After completed vitrectomy, removed intraocular foreign body and (or) cataract extracted, one-stage IOL implantation by 4-point scleral fixation was performed and the knots of fixation sutures were rotated and buried under the sclera for all patients. The IOL position was adjusted by suture for the best centration.RESULT:The mean follow-up time was (12.4 +/-1.7) months. The postoperative best-corrected visual acuity (BCVA) was improved in 11 cases (91.7%) of Group I; and BCVA was > or =0.5 in 3 patients(25.0 %). The postoperative BCVA was improved in 11 cases (100 %) of Group II; and BCVA was > or =0.5 in 4 eyes (36.4%). There was no IOL decentration or tilting in two groups.CONCLUSION:Lensectomy and vitrectomy combined with posterior chamber IOL implantation by a 4-point scleral fixation technique is a good method to treat complicated ocular trauma. Two techniques of posterior chamber IOL have both advantages and disadvantages, and can be used accordingly.
Peripheral Iridectomy in Intrascleral Intraocular Lens Fixation: Preventing Reverse Pupillary Block

Abstract:Purpose : To compare clinical outcomes, reverse pupillary block (RPB) findings and anterior segment parameters in patients with and without intraoperative peripheral iridectomy during sutureless flattened flange intrascleral intraocular lens (IOL) fixation. Design : Retrospective comparative case series. Participants This study included 94 eyes of 82 patients who underwent IOL explantation due to vitreous-dislocated IOL at our clinic between March 2020 and September 2023, followed by flattened haptic-tipped IOL intrascleral fixation combined with pars plana vitrectomy (PPV). The patients were divided into two groups: Group 1 (n=54) underwent peripheral iridectomy during surgery, while Group 2 (n=40) did not undergo surgical peripheral iridectomy. Anterior chamber depth (ACD), anterior chamber angle (ACA) and white-to-white (WTW) distance were measured with Scheimpflug imaging. Main outcomes Visual outcomes, ACD, ACA, spherical equivalence (SE), intraocular pressure (IOP), complications, and secondary interventions were evaluated. Results : There was no significant difference between the two groups preoperatively in terms of IOP, ACD, ACA, WTW distance, and axial length (p > 0.05). Postoperatively, the mean ACD was significantly less in Group 1 (3.79 ± 0.67 mm) than in Group 2 (4.11 ± 0.75 mm) (p = 0.03). Postoperative IOP was 15.51 ± 2.48 mmHg in group 1 and 18.20 ± 4.51 mmHg in group 2 (p < 0.001). The postoperative ACA was statistically significantly shallower in Group 1 (41.72 ± 3.47 degrees) than in Group 2 (52.45 ± 17.93 degrees) (p < 0.001). Postoperatively, RPB developed in 10 eyes (25.0%) in Group 2, while it was not observed in any patient in Group 1 (p < 0.001). In Group 2, pupillary capture developed in 4 cases (10%) in addition to RPB. Laser peripheral iridotomy was performed in cases with RPB. After laser peripheral iridotomy, mean IOP, mean ACA and ACD also decreased significantly (p < 0.001). Conclusions Intraoperative peripheral iridectomy during scleral fixation surgery combined with PPV leads to significantly less RPB, a more stable anterior chamber, and fewer complications. Laser peripheral iridotomy was very useful in managing RPB that developed in the postoperative period.

Abstract:Objetive To compare the effect of the peripheral iridotomy and phacoemulsification and intraocular lens implantation in the treatment of early primary angle-closure glaucoma.Methods 48 patients(54 eyes)with early primary angle-closure glaucoma were chosen and divided into two groups randomly,only those intraocular pressure could be controlled to normal by remedy were enrolled.28 patients(32 eyes)were performed with the peripheral iridotomy.The results were compared with those treated by phacoemulsification and intraocular lens implantation(20 patients,22 eyes).Intraocular pressure(IOP)measurement,slit-lamp examinations were performed before,and after operation for one time per week until 1 month,one time per month until 6 months and one time per 3 months until 12 months.The postoperative control of IOP and visual acuity were compared and analyzed.Results The average IOP was 15.23.ImmHg with the peripheral iridotomy,and 14.92.6 mmHg with the phacoemulsification and intraocular lens implantation.No statistical differences between two surgeries were identified with respect to IOP at early stage after surgery.Bestcorrected visual acuity(BCVA)of 18 eyes(81.8%)increased after phacoemulsification and intraocular lens implantation.Postoperatiive visual acuity were better than 0.4 in 63.6%patients.8 eyes' visual acuity dropped after the peripheral iridotomy There was significant difference for two methods(P<0.05).The anterior chamber depth in the peripheral iridotomy group did not change significantly, however,the anterior chamber depth increased in phacoemulsification and intraocular lens implantation group.Conclusion The phacoemulsification and intraocular lens implantation had better effect in the anterior chamber angle width,the anterior chamber depth, best—corrected visual acuity and the postoperative complications than the peripheral iridotomy surgery.

Abstract:Objective To investigate the timing of laser peripheral iridotomy(LPI)for the treatment of primary angle closure(PACS).Methods A total of 126 patients(252 eyes) diagnosed with PACS were enrolled.There were 77 females(154 eyes) and 49 males(98 eyes).Mean age of the patients was 61±4.53 years(range,47~79 years).The main criteria for diagnosis of PACS were:1.no history of attack,signs and symptoms of acute PACG;2.normal intraocular pressure;3.the depth of peripheral anterior chamber≤1/4CK;4.narrow angle width(gradeⅡ or less,Shaffer'S grading system);5.no iris-trabecular contact demonstrated by compression gonioscopy;6.no glaucomatous optic nerve damage and visual field defect.The patients were re-examined every 3 months.The average follow-up was longer than 5 years.Results 35 of 252 eyes with PACS develop into PAC.prevalence of PAC was 13.9%(35/252).Conclusion Not all patients with PACS develop PAC.In patients with any of the following features,PACS is easy to develop into PAC,and prophylactic iridotomy may be indicated in these situations:(1)Critically narrow angle(grade Ⅰor less);(2)The adhesion of angle gradually develops;(3)Narrow angle with positive dark room test;(4)Narrow angle with a well-documented family history of PACG;(5)Narrow angle with history of previous angle-closure attacks;(6)Need for repeated pupillary dilation and extensive retinal treatment(e.g.,panretinal photocoagulation for abetic retinopathy);(7)Hyperopia with senile cataract;(8)Younger patients,especially those who live in urban area away from medical care.
[Phacoemulsification with Foldable Intraocular Lens Implantation Through a Limbus Tunnel Incision].

Abstract:OBJECTIVE:To advance and evaluate the technique and therapeutic effects of phacoemulsification with foldable intraocular lens implantation through a limbus tunnel incision.METHODS:The surgery was performed through a 3.2 limbus tunnel incision on 110 eyes of 103 patients with senile, complicated, congenital and traumatic cataracts.RESULTS:Postoperatively, the visual acuities with spherical correction or without correction were 0.5 or better in 90.0% of the eyes at one day, and 1.0 or better in 42.5%, 63.7% and 68.0% at one week, one month and three months respectively. The mean postoperative astigmatism was (1.00 +/- 0.68) D at one week and only 0.07D more than the mean preoperative one (P > 0.05). There was also no statistical difference between the mean preoperative astigmatism and postoperative astigmatism at one and three months, which were respectively (0.91 +/- 0.55) D and (0.83 +/- 0.57) D. The mean postoperative astigmatism was almost the same as compared between the scleral tunnel incision and clear corneal incision groups.CONCLUSION:The phacoemulsification with foldable intraocular lens implantation through a limbus tunnel incision can reduce postoperative astigmatism and shorten the period of visual rehabilitation as the operation through a scleral tunnel or a clear corneal incision, and in the mean time the former method can overcome the shortages of the latter two methods.

Abstract:OBJECTIVE:To evaluate the therapeutic effects of phacoemulsification with foldable intraocular lens implantation through a clear corneal incision.METHODS:The surgery was performed through a 3.2 mm clear corneal incision on 100 eyes of 90 patients with senile, complicated, congenital and traumatic cataracts.RESULTS:Postoperatively, the visual acuities with spherical correction or without correction were 0.5 or better in 93.0% of eyes at one day, and 1.0 or better in 56.0%, 65.1% and 67.5% at one week, one month and three months respectively. The mean postoperative astigmatism was 0.93 +/- 0.58 D at one week and 0.20 D more than the mean preoperative one (P < 0.001). There was no statistical difference between the mean preoperative astigmatism and postoperative astigmatism at one and three months which were respectively 0.78 +/- 0.60 D and 0.73 +/- 0.59 D.CONCLUSION:The phacoemulsification with foldable intraocular lens implantation through a clear corneal incision can reduce early postoperative astigmatism, shorten the period of visual rehabilitation and simplify operative procedures.

Abstract:Purpose: This study aimed to compare anterior segment parameters pre-and postoperatively in acute primary angle closure (APAC) and fellow primary angle closure suspect (PACS) eyes using anterior segment optical coherence tomography (AS-OCT) and evaluate the clinical effectiveness of cataract extraction in the treatment of APAC and fellow PACS eyes. Methods: Quantitative measurements of various parameters, including anterior chamber depth (ACD), anterior chamber volume (ACV), lens vault (LV), iridocorneal angle contact index (ITCI), iris thickness (IT), iris volume (IV), and iris curvature (IC), were obtained using Tomey CASIA2 AS-OCT on 60 eyes from 30 patients (APAC eyes and their fellow PACS eyes) before and after surgery. Simultaneous analysis of the differences between the APAC eyes and fellow PACS eyes in these parameters, visual acuity (VA), and intraocular pressure (IOP) were performed. Results: After surgery, both the APAC eyes and fellow PACS eyes (a total of 60 eyes) showed a significant increase in ACD and ACV, compared to preoperative measurements. Furthermore, LV and ITCI significantly decreased postoperatively. In the PACS group, IC significantly decreased postoperatively, while there was no statistically significant difference in the APAC group. In the APAC group, there was a significant decrease in IOP and improvement in VA at 1 day, 1 week, and the final follow-up compared to preoperative levels. The IOP values in the PACS group were within the normal range across various time points. VA in the PACS group showed significant improvement at 1 week postoperatively and at the final follow-up compared to preoperative levels. Significant differences of VA were observed in the initial, preoperative, first postoperative day, first postoperative week, and final follow-up, with better outcomes observed in the PACS group compared to the APAC group. Conclusion: Lens extraction surgery can significantly improve anterior segment crowding in APAC and PACS eyes. For APAC eyes, combined cataract extraction with intraocular lens implantation and gonioscopy-assisted goniosynechialysis under direct visualization is feasible and safe. Further, in the fellow PACS eye of APAC patients with either significant or mild cataracts, phacoemulsification can maintain or improve preoperative visual acuity to varying degrees and stabilize IOP.
Clear corneal vitrectomy combined with phacoemulsification and foldable intraocular lens implantation

Abstract:Background: We have developed a technique for the treatment of cataract and epiretinal membrane using a 25-gauge vitrectomy system through corneal ports. Design: Randomized, prospective study, Toyama Prefectural Central Hospital, Toyama, Japan. Participants: Twenty eyes of equal patients scheduled for cataract surgery combined with vitrectomy. Methods: Twenty eyes with cataract and epiretinal membrane were received treatment with our newly developed system (clear corneal vitrectomy) or the standard 25-gauge pars plana vitrectomy with corneal incision cataract surgery. The newly developed system uses 0.5-mm wide corneal side ports located at the superonasal, superotemporal and inferotemporal positions. After phacoemulsification using corneal incision, an infusion cannula was inserted from the inferotemporal port. Then core 25-gauge vitrectomy was performed using the corneal three port. After the epiretinal membrane was removed using forceps, an intraocular lens was implanted into the capsular bag. Finally, all corneal incision wounds were hydrated. Main outcome measurement: Visual acuity, intraocular pressure, corneal thickness, corneal endothelial cell and ocular inflammation were examined. Results: All procedures were uncomplicated in both groups. There was no leakage of aqueous humour from the corneal wounds in the developed system. There were no significant differences in visual acuity, corneal thickness and endothelial cell density loss. Conclusions: Clear corneal vitrectomy would be a good option for selected cases with cataract and vitreoretinal diseases.
Primary Angle Closure Observed During a House Visit: A Case Treated With Laser Iridotomy

Abstract:Laser iridotomy (LI) is an effective treatment for patients with pupillary block mechanisms. Here, we report a case of LI performed on a patient with primary angle closure (PAC) and elevated intraocular pressure (IOP), who was unsuitable for other treatments due to specific social circumstances. The patient, a 97-year-old female residing in a private nursing home, had a medical history notable only for mild dementia and was wheelchair-bound. She had not undergone ophthalmologic evaluation for over 50 years. The patient presented with intermittent tenderness and redness in the left eye. Therefore, an ophthalmologist visited the nursing home. Examination revealed visual acuity of 20/200 in the right eye and 20/100 in the left eye, IOP of 24 mmHg in the right eye and 26 mmHg in the left eye, no conjunctival hyperemia, shallow anterior chambers, and nuclear sclerosis grade 4 cataracts in both eyes. Fundus examination was challenging due to lens opacity, and both optic nerve papillae appeared pale. Given her history of episodic eye pain and hyperemia, PAC was diagnosed. Treatment options, including eye drops and cataract surgery, were discussed. However, the patient opted for LI due to her advanced age and inability to attend frequent follow-up visits. LI was successfully performed on both eyes during her visit to the clinic. One week post-procedure, IOP decreased to 12 mmHg bilaterally, with no complications. This case demonstrates that LI can be a viable option for managing PAC and high IOP in patients who are not candidates for surgery or eye drops due to social constraints.
Iris Cerclage To Treat Persistent Mydriasis Following Acute Angle Closure Glaucoma Improves Visual Quality

Abstract:Précis: We developed a modified iris cerclage technique that improves best corrected visual acuity, pupillary parameters, self-assessed photophobia, and visual function index-14 questionnaire scores in patients with acute primary angle closure and permanent mydriasis after cataract surgery. Purpose: To evaluate the efficacy of a modified iris cerclage technique in patients with acute primary angle closure (PAC) and permanent mydriasis after cataract surgery. Patients and Methods: 12 eyes of 12 patients with acute PAC and permanent mydriasis at more than 3 months after phacoemulsification combined with intraocular lens (IOL) implantation underwent modified iris cerclage. Best corrected visual acuity (BCVA), intraocular pressure (IOP), pupil diameter, and visual function index-14 (VF-14) questionnaire and self-assessed photophobia scores before surgery and at 1 week, 1 month, and 3 months postoperatively were compared. Further, the postoperative pupil position and morphology and complications were evaluated. Results: BCVA improved from 0.48±0.23 preoperatively to 0.28±0.15, 0.27±0.15, and 0.26±0.14 at 1 week, 1 month, and 3 months postoperatively, respectively ( P =0.008). No significant difference was observed between the preoperative and postoperative IOP ( P =0.974). Pupil diameter improved from 6.34±0.51 mm preoperatively to 3.59±0.33, 3.59±0.34, and 3.58±0.32 mm at 1 week, 1 month, and 3 months postoperatively, respectively ( P <0.001). Self-assessed photophobia scores improved from 3.33±0.78 preoperatively to 0.83±0.72, 0.51±0.15, and 0.45±0.14 at 1 week, 1 month, and 3 months postoperatively, respectively ( P <0.001). VF-14 scores improved from 47.6±6.1 points preoperatively to 67.9±6.2, 72.1±6.1, and 73.5±6.0 points at 1 week, 1 month, and 3 months postoperatively, respectively ( P< 0.001). Postoperatively, all pupils were centered and round. No postoperative complications, such as iris-suture slippage, iridodialysis, or exposure of the IOL’s optical zone edge, were observed. Conclusion: Modified iris cerclage creates a centered, precisely sized, round pupil. It improves visual quality in patients with acute PAC and permanent mydriasis after cataract surgery.

Microcoaxial Phacoemulsification Combined with Viscogoniosynechialysis for Patients with Refractory Acute Angle-Closure Glaucoma

Abstract:Purpose:To evaluate the safety and efficacy of microcoaxial phacoemulsification combined with viscogoniosynechialysis in managing refractory acute angle-closure glaucoma (AACG) unresponsive to conventional therapy.Methods:Seventeen consecutive eyes of patients with AACG>270 degrees peripheral anterior synechiae were treated with microcoaxial phacoemulsification combined with viscogoniosynechialysis. After intraocular lens implantation, a heavy viscoelastic agent was used to deepen the anterior chamber, and then injected near the angle for 360 degrees without touching any ocular structure, to release the peripheral anterior synechiae under gonioscopy. The viscoelastic agent was then removed.Results:Intraocular pressure was reduced from a median of 45.0 mm Hg initially to 15.0 mm Hg after the combined procedures at the final follow-up visit (P<0.001). All angles showed exposure of the trabecular meshwork over 360 degrees postoperatively without residual synechiae. The mean best corrected visual acuity improved from 0.7 to 0.18 logMAR at 6-month follow-up (P<0.001). Surgically induced astigmatism was 0.29 at 6-month follow-up. There was a 6.4% reduction in the mean corneal endothelial cell count. No severe complications were observed in any patient.Conclusions:Microcoaxial phacoemulsification combined with viscogoniosynechialysis is an effective and safe treatment for managing refractory AACG. Using a 2.2-mm incision caused less surgically induced astigmatism and also improved visual quality.
Combined Laser Technique for Advanced Chronic Angle-closure Glaucoma

Abstract:Purpose To investigate the efficiency of laser peripheral iridoplasty,laser peripheral iridectomy (LPI)and the combination of laser trabeculoplasty tot advanced primary angle-closure glaucoma(PACG).Methods La- ser peripheral iridoplasty and LPI was taken for 19 eyes of 17 patients who were diagnosed as advanced PACG.Four eyes with high intraocular pressure(IOP)and coefficient of facility(C)less than 0.12 had laser trabeculoplasty.Results The chamber angle was widened.The follow-up time was 3～18 months.The IOP decreased from 20.1～35 nun Hg pre- treatment to 13.6～22.8 mm Hg after the treatment with 31% decrease in average.During the follow-up time there was no more visual field lost.Conclusions Laser peripheral iridoplasty and LPI combined with laser trabeculoplasty might be safe and effective for the treatment of patients with advanced PACG who refused or couldn't tolerate trabeculectomy. Effect of Different Clear Corneal Incision Sites on Surgery Efficacy and Anterior Segment Parameters in Patients Undergoing Phacoemulsification

Abstract:OBJECTIVE:To determine the safe distance range of clear corneal incision (CCI) from the corneal limbus and how different CCI sites affect surgery efficacy and anterior segment parameters in patients undergoing phacoemulsification.METHODS:This retrospective case-control study was conducted in 44 patients (44 eyes) undergoing phacoemulsification and IOL implantation. The patients were divided into two groups with CCI distances ranging from 1 mm to 1.5 mm (group A, n= 22) and from 0.5 mm to 1.0 mm (group B, n= 22). The visual acuity, surgically induced astigmatism (SIA), corneal aberration, and anterior segment parameters were analyzed.RESULTS:Compared with the preoperative data, all the patients showed significant improvements in the postoperative uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), anterior chamber depth (ACD), and anterior chamber angle (ACA) after the surgery (P < 0.05). No significant differences were found between the two groups in postoperative UCVA, BCVA, SIA, total corneal aberration RMS, lower- and higher-order aberration RMS, spherical aberration (Z40), horizontal three leaf clover (Z33), vertical three leaf clover(Z3-3), horizontal coma(Z31), vertical coma(Z3-1), ACD, ACA, anterior chamber volume, or central corneal thickness (P>0.05).CONCLUSIONS:Phacoemulsification is an effective therapy for cataract with a CCI distance range either of 1-1.5 mm or 0.5-1.0 mm. These two CCI distance ranges produce no significant differences in the visual quality following phacoemulsification, indicating that a CCI distance range of 0.5-1.5 mm can be safe for phacoemulsification.
Treatment of Residual Angle Closure with Small Pupil and Severe Flat Anterior Chamber after Peripheral Laser Iridotomy

Abstract:We reported 2 cases of refractory residual angle closure glaucoma patients aged over 50 years with small pupil,severe flat anterior chamber and lens opacification,and more than 5 years after laser peripheral iridotomy(LPI) or iridoectomy.The purpose of this case discusion is to pay attention to long-term follow-up of primary angle-closure patient after LPI,to promote personalized treatment for residual glaucoma,and to present our experience of cataract surgery in patients with small pupil and severe flat anterior chamber.

Abstract:ABSTRACT Purpose: To retrospectively assess the effectiveness of combined phacoemulsification with viscogoniosynechialysis for patients with different subtypes of primary angle closure (PAC). Methods: Forty-three eyes of 37 patients diagnosed with PAC (12 eyes), PAC with glaucoma (PACG, 20 eyes), and acute PAC (11 eyes) were treated with phacoemulsification and viscogoniosynechialysis between November 2010 and October 2012. Main outcome measures were anterior chamber depth (ACD), intraocular pressure (IOP), the number of IOP-lowering medication, extent of peripheral anterior synechia (PAS), and visual acuity preoperatively and one month postoperatively. Results: All operations were successfully accomplished and no intra-operative complications were observed. The ACD, IOP, PAS, and visual acuity were all significantly improved postoperatively, especially in the acute PAC group, which resulted in few glaucoma drugs being needed. The IOP was decreased into equal or less than 21 mmHg in most patients postoperatively, except four patients had IOP slightly higher than 21 mmHg. However, they were restored to normal spontaneously or by medication at last follow-up. The extent of PAS was all reduced to less than 90 or 180°, with the visual acuity restored to more than 0.1. Conclusions: Combined phacoemulsification with viscogoniosynechialysis seems to be an effective surgical procedure in the treatment of various subtypes of PAC.
Sutureless intrascleral intraocular lens fixation and modified iris cerclage pupilloplasty for aphakia and traumatic mydriasis

Abstract:Purpose: To evaluate the safety and efficacy of sutureless intrascleral intraocular lens (IOL) fixation combined with modified iris cerclage pupilloplasty for treating aphakia and traumatic mydriasis. Methods: Five patients with aphakia and traumatic mydriasis were operated on by the same surgeon. All patients underwent sutureless intrascleral IOL fixation combined with modified iris cerclage pupilloplasty and were followed up for ≥6 months. Best-corrected visual acuity (BCVA) was measured using the logarithm of the minimum angle of resolution (logMAR). BCVA, intraocular pressure (IOP), pupil diameter, and corneal endothelial cell count (CECC) preoperatively and postoperatively were statistically analyzed. The pupil shape, photophobia, IOL position, and surgical complications were evaluated. Results: The mean BCVA was significantly improved 6 months postoperatively (0.26 ± 0.17 logMAR, P = 0.042) than preoperatively (0.50 ± 0.30 logMAR). No significant difference was observed between the preoperative and postoperative IOP ( P = 0.138). The mean pupil diameter significantly reduced postoperatively than preoperatively (3.44 ± 0.35 mm vs. 7.28 ± 0.35 mm, P = 0.043). There was no significant decrease in CECC postoperatively ( P = 0.225). The pupil shape was round-like, and photophobia disappeared in all patients. No intraoperative or postoperative complications occurred. Conclusion: Sutureless intrascleral IOL fixation combined with modified iris cerclage pupilloplasty is a safe and efficient procedure for treating aphakia traumatic mydriasis patients without sufficient capsular support.

Abstract:BackgroundAphakic glaucoma is a common complication after congenital cataract extraction, especially in those who have surgery during infancy. This case report describes a case of bilateral pupillary block glaucoma diagnosed with intraoperative ultrasound biomicroscopy (UBM) after removal of congenital cataract.Case presentationWe present a case report of a 9-month-old infant with bilateral corneal enlargement and ocular hypertension after uneventful removal of congenital cataracts. Initial and follow-up examination findings were reviewed. The infant was suspected to have developmental glaucoma and schemed to have bilateral trabeculotomy until pupillary obstruction by vitreous herniation and angle closure with iris bombé were detected by intraoperative UBM. Anterior vitrectomy and goniosynechialysis were then performed as treatment.ConclusionPupillary block glaucoma is a rare type of infantile aphakic glaucoma. Application of intraoperative UBM can assist in the differential diagnosis of aphakic glaucoma in infants.

Intrascleral Sewing-Machine Technique: A Grooveless/flapless Cyclopexy Technique for Large Traumatic Cyclodialysis Cleft Repairs in Pars Plana Vitrectomy

Abstract:Purpose: To describe a minimally invasive technique for the repair of large traumatic cyclodialysis clefts using intrascleral sewing machine suture and overhand friction knot techniques in pars plana vitrectomy. Methods: This prospective, noncomparative, interventional case series included seven eyes of seven patients with a large traumatic cyclodialysis cleft. The sewing machine technique was modified by an intrascleral approach. The procedure was transconjunctival or subconjunctival performed without scleral flaps/grooves. An overhand friction knot was used to lead the cutting ends of the suture buried in the scleral tunnel. Results: The closure of the cyclodialysis cleft was achieved in seven eyes. The mean follow-up duration was 49.1 ± 15.6 weeks (range, 30–70 weeks). The intraocular pressure increased from 7.3 ± 2.1 mm Hg (range, 5–11 mmHg) preoperatively to 13.6 ± 2.4 mm Hg (range, 10–17 mmHg) postoperatively (P < 0.01). The best-corrected visual acuity improved from a mean of 2.76 ± 2.77 logarithm of the minimum angle of resolution preoperatively to 0.63 ± 0.82 logarithm of the minimum angle of resolution at the final follow-up (P < 0.01). Conclusion: In conclusion, the present technique is safe and effective in the treatment of large traumatic cyclodialysis clefts with minimal surgical trauma and a decreased surgical duration.

Abstract:To explore the effect of phacoemulsification along with intraocular lens (IOL) implantation on the treatment of primary angle closure glaucoma (PACG), a total of 50 patients (54 eyes) with PCG who had undergone surgery were selected as the research objects, in which, there were 16 eyes with the acute PACG in pre-clinical stage, 20 eyes with acute PACG during acute attack, and 18 eyes with chronic PACG, and all patients received phacoemulsification along with IOL implantation therapy. The patient’s best corrected visual acuity, anterior chamber (AC), anterior depth, intraocular pressure (IOP), visual field, and fundus conditions were measured during the operation, after the operation, and during the follow-up period. The postoperative visual acuity of the patients with three types of PACG was greatly corrected, in which 30 eyes had corrected visual acuity above 0.5, and there were statistically significant differences (SSDs) ( P < 0.05); angle closure of 54 eyes had different degrees of improvement, the opening degree of the acute angle closure is better than that of the chronic angle closure, and there were SSDs ( P < 0.05); each of 54 eyes showed different degrees of deepening in the central AC, and there were SSDs ( P < 0.05); postoperative IOP decreased greatly in all 54 eyes, and there were SSDs ( P < 0.05). In conclusion, the IOL implantation based on phacoemulsification can effectively treat the PACG, at the same time reduce IOP, promote angle opening, enhance vision field of the patients, and ultimately achieve the purpose of improving surgical success.

Abstract:OBJECTIVE: To examine the effects and safety of doubled-frequency Nd:YAG laser peripheral iridoplasty(LPI) for angle-closure glaucoma (ACG) with relieving the pupillary block. METHODS: Thirty-seven eyes of 29 patients of ACG with mean intraocular pressure (IOP) of (33.5+/-9.3)mmHg, were treated by LPI. In the follow-up of 5 approximate, equals 35 months, the IOP, angle and complications from LPI were examined. RESULTS: The mean IOP of the patients was reduced to (17.2+/-5.3)mmHg (P<0.0001). Thirty-three eyes of 25 patients, who had negative of dark room provocative test, maintained the angle opened without adjunctive antiglaucoma treatment. Four eyes of 4 patients continued to developed angle closure and had a high IOP. CONCLUSION: Peripheral iridoplasty doubled frequency Nd:YAG laser is a safe and effective method for treatment of ACG.
