Abstract:We evaluated the safety and efficacy of zero ischemia, radio frequency ablation assisted tumor enucleation for renal cell carcinoma. We report the incidence of complications, positive surgical margins, local recurrence, and progression-free and disease specific survival rates.We retrospectively reviewed data on 42 patients with renal cell carcinoma treated with zero ischemia laparoscopic radio frequency ablation assisted tumor enucleation between March 2006 and November 2009. Median age was 60 years (range 37 to 82) and 31 patients (73.8%) were male. The median greatest tumor dimension was 3.4 cm (range 1.8 to 6.1). The Mann-Whitney U, chi-square and Fisher exact tests were used to compare bleeding and complications. The paired t and Mann-Whitney U tests were used to compare glomerular filtration rates. The Kaplan-Meier method was used to calculate survival.We found 32 tumors with a greatest dimension of 4 cm or less and 10 with a greatest dimension of 4 to 7 cm. Median blood loss was 82.5 ml (range 15 to 210). Overall 7 complications (16.6%) occurred, including postoperative fever in 4 cases (Clavien grade II) and prolonged urinary leakage in 3 (Clavien grade III). The PADUA (preoperative aspects and dimensions used for an anatomical) score was associated with prolonged urinary leakage (p = 0.03) but not with overall complications. No patient had positive surgical margins. The glomerular filtration rate did not differ before vs 12 months after surgery. Three-year cancer specific, cumulative and progression-free survival was 100%, 97.3% and 96.4%, respectively.Zero ischemia, laparoscopic radio frequency ablation assisted tumor enucleation of renal cell carcinoma is a safe, effective nephron sparing treatment that provides excellent oncological and functional outcomes.
Mid-to-long term oncologic and functional outcomes of zero ischemia laparoscopic microwave ablation-assisted tumor enucleation for renal cell carcinoma: a single-center experience

Abstract:Background: Zero ischemia laparoscopic microwave ablation assisted tumor enucleation (MWA-TE) has been applied to renal cell carcinoma (RCC) treatment, but the mid-to-long term follow-up results are lacking. This study aims to evaluate the mid-to-long term oncological and functional consequences of MWA-TE in RCC treatment. Methods: We conducted a retrospective analysis of 278 patients who received zero ischemia laparoscopic MWA-TE for RCC from March 2013 to June 2017. Peri- and post-operative data, renal functional and oncologic results were accumulated and evaluated. Results: The median length of operation was 86 min, with an approximate median blood loss of 50 mL. The median estimated glomerular filtration rate (eGFR) prior to and 3 months post-operation was 90.6 [interquartile range (IQR), 65.2-116.0] and 83.6 (IQR, 52.4-114.8) mL/min/1.73 m2, the median eGFR of the latest follow-up was 87.4 (IQR, 67.7-107.1) mL/min/1.73 m2 and paired t-tests showed no significant difference between preoperative, post-operative and latest eGFR (P=0.069 and P=0.071). The median change in eGFR from before surgery to the latest follow-up was -2.82 mL/min/1.73 m2. The median follow-up period was 39 months, and among all the patients, nine reported cancer recurrence or metastasis, the 3- and 5-year overall survival (OS) was 99.6% and 98.4%, respectively, and recurrence-free survival (RFS) was 98.2% and 95.8%, respectively. Conclusions: Zero ischemia laparoscopic MWA-TE is considered a feasible and effective nephron sparing surgical technique for selected renal tumors, and is accompanied by a low perioperative complication rate and promising mid-to-long term oncological and functional outcomes.

The aim of the study is to evaluate the safety and efficacy of 0-ischemia, radiofrequency ablation (RFA) assisted tumor enucleation (TE) for T1b renal cell carcinoma (RCC), reporting on the incidence of complications, positive surgical margins and short-term followup results. METHODS We retrospectively reviewed data for 47 patients with T1b RCC treated with 0-ischemia RFA assisted TE between March 2006 and October 2011. The mean age was 55.9 ¡À 11.0 years, and 36 (76.6%) were male. The mean greatest dimension of tumors was 5.0 ¡À 0.6 cm. Student's t-test, chi-square test and Fisher's exact test were used to compare operation time, bleeding and complications. Fisher's exact test was used to analyze the association Preoperative Aspects and Dimensions Used for an Anatomical (PADUA) score and complications. Paired t test was used to compare GFR. RESULTS We found 47 tumors with greatest dimension between 4cm to 7 cm (see Table 1). Twenty-eight patients underwent RFA assisted laparoscopic TE while 19 patients underwent open RFA assisted TE. None of these cases need clamping the renal hilar vessels during the enucleation. The mean blood loss was 130.2 ¡À 56.1 ml (laparoscopic 113.9 ¡À 44.1 ml vs open 154.2 ¡À 64.2 ml) and mean operation time was 134.2 ¡À 32.0 min (laparoscopic 126.6¡À27.1 min vs open 145.5¡À35.9 min). Overall 8 complications (17.0%) occurred including 4 postoperative fevers (Clavien-Grade II), 1 bleeding need transfusion (Clavien-Grade II) and 3 prolonged urinary leakages (Clavien-Grade III). PADUA score was associated with prolonged urinary leakage (p=0.01) but not overall complications. No patient had positive surgical margins. Glomerular filtration rate differed before and 12 months after surgery with 2.0 mL/min/1.73 m2 decline at the first year after the surgery. One patient with a 5.1cm renal tumor had recurrence near the renal sinus near the ablation zone 9 month after surgery and received laparoscopic radical nephrectomy. CONCLUSIONS Zero-ischemia, RFA assisted TE is a safe and effective nephron sparing treatment for T1b renal cell carcinoma that may provides excellent oncological and functional outcomes. Table 1. Radio frequency ablation versus partial nephrectomy for clinical T1b renal cell carcinoma: long-term clinical and oncologic outcomes.

Abstract:Purpose: We compared outcomes in patients treated with radio frequency ablation or partial nephrectomy for clinical cT1b renal cell carcinoma. Materials and Methods: We retrospectively reviewed the records of all patients who underwent radio frequency ablation or nephrectomy between February 2006 and December 2010. Radiographic followup with contrast imaging was performed 7 days, 3 and 6 months, and every 6 months thereafter after radio frequency ablation sequentially. The followup protocol for partial nephrectomy was every 6 months in the initial 3 years and annually thereafter. The Kaplan-Meier method was used to generate survival curves, which were compared with the log rank test. Multivariable regression analysis was done to determine predictors of survival. Results: A total of 56 patients who met selection criteria were included in study. Patients in the radio frequency ablation group had relatively higher mean age and a higher mean ASA (R) score than those in the partial nephrectomy group. Mean tumor diameter was significantly larger in the partial nephrectomy cohort. For radio frequency ablation vs partial nephrectomy 5-year overall, cancer specific and disease-free survival was 85.5% (95% CI 72.2-98.8) vs 96.6% (95% CI 95.9-97.3), 92.6% (95% CI 82.4-98.1) vs 96.6% (95% CI 95.9-97.3) and 81.0% (95% CI 66.2-95.8) vs 89.7% (95% CI 78.5-97.9), respectively. The percent decrease in the glomerular filtration rate was significantly lower in the radio frequency ablation group at early and last followup. Conclusions: In appropriately selected patients with stage cT1b renal cell carcinoma radio frequency ablation is an effective treatment option that provides 5-year overall, cancer specific and disease-free survival comparable to that of partial nephrectomy as well as better renal function preservation than partial nephrectomy.
Perioperative and Oncological Outcomes of Percutaneous Radiofrequency Ablation versus Partial Nephrectomy for cT1a Renal Cancers: A Retrospective Study on Groups with Similar Clinical Characteristics

Abstract:Over the recent years, progress in imaging techniques has led to an increased detection of kidney tumours, including small renal masses. While surgery is still the standard of care, there is a growing interest in minimally invasive methods. Ultrasound (US)-guided percutaneous ablation is particularly attractive because it is a safe and relatively simple procedure. In this study, we investigated the results of US-guided percutaneous radiofrequency ablation (RFA) and partial nephrectomy (PN) in the treatment of cT1a renal cancers. Between August 2016 and February 2022, 271 patients with renal tumours underwent percutaneous RFA as initial treatment in our institution. In the same period, 396 patients with renal tumours underwent surgical tumour excision. For the purpose of this study, only patients with confirmed renal cancer with matched age and tumour characteristics (size, location) were selected for both groups. Thus, a group of 44 PN patients and 41 RFA patients were formed with the same qualification criteria for both groups. Parameters such as procedure length, blood loss, hospital stay, analgesics used, and pre- and post-procedural serum creatinine were compared between these groups. Patients followed up with contrast-enhanced CT. There was no significant difference in age, tumour size, tumour location, and creatinine levels between these groups. All procedures were generally well tolerated. During a median follow-up of 28 months, two cases of recurrence/residual disease were found in each group. The overall survival was 100% in both groups, and all patients were disease-free at the end of observation. Percutaneous RFA was associated with a significantly shorter procedure length and hospital stay, lower blood loss, and lower analgesics used than PN. In the selected group of renal cancer patients, US-guided percutaneous RFA was associated with a shorter hospital stay, less analgesics used, and a shorter procedure length than PN, without differences in the oncological results or kidney function.

Tumor Enucleation Vs. Partial Nephrectomy for T1 Renal Cell Carcinoma: A Systematic Review and Meta-Analysis

Abstract:Purpose: Tumor enucleation (TE) and partial nephrectomy (PN) have both become main treatment strategies for T1 renal cell carcinoma (RCC), despite the discrepancy between their safety margin. We performed a meta-analysis on all the relevant trials in order to compare the clinical efficacy and safety of TE with those of PN for RCC treatment. Methods: In this meta-analysis, randomized controlled trials or retrospective studies were included if they compared TE and PN therapy in patients with localized renal cancer. The main outcomes extracted were perioperative data and post-operative outcomes. Subgroups for analyses were undertaken according to tumor size and duration of follow up. Data were pooled using the generic variance method with a fixed or random effects model and expressed as mean differences or odds ratios with 95% CI. Results: A total of 13 studies containing 1,792 patients undergoing TE and 3,068 undergoing PN were identified. Our study showed that the patients received TE had significantly shorter operative time (MD = −28.46, 95% CI = −42.09, −14.83, P < 0.0001), less hospital day (MD = −0.68, 95% CI = −1.04, −0.31, P = 0.0003), less estimate blood loss (MD = −59.90, 95% CI = −93.23, −26.58, P = 0.0004) and smaller change in estimated glomerular filtration rate (fixed effect: MD = 4.66, 95% CI = 1.67, 7.66, P = 0.002), fewer complications (fixed effect: OR = 0.65, 95% CI = 0.50, 0.85, P = 0.001) compared with those received PN. However, there were no significant differences in terms of warm ischemic time, positive margin rates, recurrence rates and survival rates between the two groups. All the subgroup analyses presented consistent results with the overall analyses. Conclusions: Our findings suggested that TE is not only less-traumatizing and beneficial for recovery, but also better for renal function protection. Moreover, it did not show the evidence of an increase relapse rate or mortality rate when compared with PN.
Radiofrequency Ablation of Renal Cell Carcinoma: Part 1, Indications, Results, and Role in Patient Management over a 6-Year Period and Ablation of 100 Tumors

Abstract:OBJECTIVE: The objectives of our article are to review our experience with radiofrequency ablation of renal cell carcinoma and to assess size and location as predictors of the ability to achieve complete necrosis by imaging criteria.MATERIALS AND METHODS: Over a 6-year period, 100 renal tumors in 85 patients underwent radiofrequency ablation at a single institution. The absence of enhancement on CT or MRI after radiofrequency ablation was interpreted as complete coagulation necrosis. Results were analyzed by tumor size and location using multivariate analysis. A p value of 0.05 or less was considered significant.RESULTS: All 52 small (3 cm) and all 68 exophytic tumors underwent complete necrosis regardless of size, although many large tumors (> 3 cm) required a second ablation session. Using multivariate analysis, we found that both small size (p < 0.0001) and noncentral location (p = 0.0049) proved to be independent predictors of complete necrosis after a single ablation session. Location was a significant predictor (p = 0.015) of complete necrosis after any number of sessions, whereas size showed a strong trend (p = 0.059) toward predicting success after any number of sessions. Complications were either self-limited or readily treated and included hemorrhage (major, n = 2; minor, n = 3), inflammatory track mass (n = 1), transient lumbar plexus pain (n = 2), ureteral injury (n = 2), and skin burns (n = 1).CONCLUSION: Radiofrequency ablation is a promising minimally invasive therapy for renal cell carcinoma in patients who are not good operative candidates. Small size and noncentral location are favorable tumor characteristics, although large tumors can sometimes be successfully treated with multiple ablation sessions.

Abstract:Objectives To evaluate the impact of contrast-enhanced ultrasonography (CEUS) during percutaneous radiofrequency ablation (PRFA) procedure in renal cell carcinoma (RCC). Methods From January 2008 to July 2010, 73 patients with sporadic unilateral RCC were enrolled to our study (57 men and 16 women, age range: 37–78 years, mean age 57.9 years). The diameter of the tumor was 1.7–5.8, 3.4 cm on average. The patients were divided into two groups depending on the intraoperative ultrasonography type: CEUS group and conventional ultrasound group. Patients in CEUS group received CEUS before insertion of the electrode, and the second CEUS was performed right after the initial ablation to dynamically evaluate the images. If there was highly suspicious residue, additional ablation and repeated CEUS were applied. Patients in the conventional ultrasound group received PRFA guided by gray-scale ultrasound. All of these patients received contrast-enhanced computed tomography (CT) examination 7 days after the procedure (patients in CEUS group received CEUS conducted with each CT scan), with subsequent CT and CEUS assessment at 3, 6, and every 6 months thereafter. Results The mean follow-up period was 22 months (range: 12–42 months). All tumors were biopsied before RFA. The local tumor control rate was 94.6% (35/37) in the CEUS group and 86.1% (31/36) in the conventional ultrasound group ( P < 0.05); the cancer-specific survival rate and the overall survival rate were 100%. The post-RFA (12 months) mean GFR levels were 84.7 ± 27.5 mL/min/1.73 m 2 ( P > 0.05, compared with pre-GFR: 86.4 ± 26.2 mL/min/1.73 m 2 ) in the CEUS group and 81.9 ± 22.8 mL/min/1.73 m 2 ( P > 0.05, compared with pre-GFR: 83.5 ± 23.7 mL/min/1.73 m 2 ) in the conventional ultrasound group. Conclusion Intraoperative CEUS can “real-time” monitor the ablated area during PRFA procedure. This technique can help to achieve a higher success rate compared with conventional ultrasound. No impact of intraoperative CEUS has been found on GFR level.

Abstract:OBJECTIVE To report our experience of laparoscopic radiofrequency ablation (RFA) on patients with renal tumors. RFA has been increasingly applied in the management of small renal tumors. However, it was performed mostly via percutaneous approach, with limited cases and a short follow-up period.METHODS From February 2006 to March 2008, laparoscopic RFA was performed on 106 renal tumors (size range: 0.9-5.5 cm) in 106 selected patients (74 men and 32 women, age range: 25-81 years). Initial contrast-enhanced computed tomography (CT) examination was performed seven days after the procedure, with subsequent CT assessment at three months, six months, and every six months thereafter. Serum creatinine measurement was conducted along with each time CT examination.RESULTS The mean follow-up period was 32 months (range: 12-48). All 106 tumors were biopsied before RFA, of which 90 were diagnosed as renal cell carcinoma (RCC) (84.90%). There was one incomplete ablation. One case with radiographic local recurrence was then proved by pathologic analysis of the nephrectomy specimen to have no cancer cells. The local tumor control rate was 98.1% (104/106). Of the 90 RCC cases, the disease-free survival rate was 97.8% (88/90); both the cancer-specific and the overall survival rate were 100%. No death or renal failure after the procedure has yet been found.CONCLUSIONS Our results showed that the laparoscopic RFA on small renal mass was safe, with outcomes of patients comparable with those by partial nephrectomy and percutaneous RFA. The Comparison of R.E.N.A.L., PADUA and Centrality Index Score in Predicting Perioperative Outcomes and Complications after Laparoscopic Radio Frequency Ablation of Renal Tumors

Abstract:PURPOSE:We assessed the association between the R.E.N.A.L., PADUA and centrality index scores and perioperative outcomes, and determined the efficacy of each scoring system over the simple parameter of tumor size.MATERIALS AND METHODS:We conducted a retrospective review of the records of patients who underwent laparoscopic radio frequency ablation of renal tumors from February 2006 to April 2014 at our institution. Tumors were categorized using the R.E.N.A.L., PADUA and centrality index score, which were determined by analyzing preoperative imaging. The relationships between each variable and change in glomerular filtration rate, estimated blood loss, operative time and postoperative complications were examined.RESULTS:Laparoscopic radio frequency ablation was performed in 215 selected patients with preoperative imaging available. The interclass correlations coefficient of the 3 observers was 0.87, 0.84 and 0.82 for the C-Index, the R.E.N.A.L. and the PADUA scoring system, respectively. R.E.N.A.L. score, PADUA score and PADUA complexity were significantly associated with postoperative complications (p=0.040, 0.015 and 0.006, respectively). However, major complications were only associated with high PADUA complexity (chi-square 23.851, p <0.05). Each scoring system had a significant association with operative time and estimated blood loss (p <0.05), except that the C-index score had no relationship with estimated blood loss (p=0.083). The accuracy of each scoring system in predicting perioperative outcomes and postoperative complications was superior to that of tumor size.CONCLUSIONS:Each scoring system has an advantage over tumor size in describing tumor complexity and may help with surgical decision making. Further studies are required to validate their efficacy in evaluating the intraoperative and postoperative results after renal laparoscopic radio frequency ablation.

Abstract:Purpose: To assess the functional and oncologic outcomes of laparoscopic radiofrequency ablation (RFA) with intraoperative contrast-enhanced ultrasonography in treatment of T1b renal tumors. Patients and Methods: We performed a retrospective review of 51 patients with unilateral T1b renal tumors who underwent laparoscopic RFA from January 2007 to April 2012 with a mean follow-up of 31.5 months. The tumors were laparoscopically dissected and ablated with cool-tip RFA system under the guidance of contrast-enhanced ultrasound. Routine follow-up included contrast-enhanced computed tomography/magnetic resonance imaging (CT/MRI) and renal function tests. Results: Mean diameter of the treated renal tumors was 5.1cm (range 4.1-6.4cm). Initial ablation success rate was 46/51 (90.2%). Of five incompletely ablated patients, four experienced up to two additional sessions of percutaneous RFA and achieved complete ablation. After at least 12 months contrast-enhanced CT/MRI follow-up after RFA, one patient experienced local tumor recurrence and another developed pulmonary and bone metastases. The 3-year disease-free survival in patients with biopsy-proven cancer was 85.7%. There was no significant difference between the estimated glomerular filtration rate (eGFR) pre- and post-RFA. Mean change in eGFR following RFA was -2.2mL/minute. No patients required dialysis in the periprocedural period. Major complications (Grade 3) only developed in two (3.9%) cases. Conclusions: Our initial experience with this technique in T1b renal tumors was favorable with good renal function preservation and oncologic outcomes. It may be a good alternative to partial nephrectomy for selected T1b renal tumors.

Abstract:Objective To summarize the outcomes of treating renal tumors with ultrasound-guided percutaneous radiofrequency ablation(RFA).Methods Ninteen consecutive patients with renal tumors located in the dorsolateral zone or middle and low pole of the kidney underwent ultrasound-guided percutaneous RFA.Results The operations in all patients were successful without serious complications of perirenal hemorrhage,effusion and injury on adjacent organs.The time of operation was 30 to 50 minutes and hospital stay was 2 to 5 days.Pathological diagnosis was renal cell carcinoma(RCC ) in 14 cases,renal hamartoma in 3 cases and no definite agnosis in 2 case. One patient was confirmed having residual tumor by review,who recieved second percutaneous RFA. There were no cases with local tumor recurrence or distant metastasis during follow-up for 5 to 19 months.ConclusionUltrasound-guided percutaneous RFA of renal tumors has the advantages of less trauma and rapid postoperative recovery and is a good choice of minimally invasive treatment method for the tumors located in the dorsolateral zone or middle and low pole of the kidney that is unsuitable for surgery or unwilling to receive operation.

Abstract:Purpose: To compare outcomes in patients treated with radiofrequency ablation (RFA) and partial nephrectomy (PN) for clinical T-1a renal-cell carcinoma (RCC) in a propensity-score matched cohort. Methods: We conducted a retrospective review of the records of all patients who underwent RFA or nephrectomy between February 2005 and December 2009 in our institution. The Kaplan-Meier method was used to generate the survival curves that were compared with the log-rank test. Univariable and multivariable regression analyses were performed to determine predictors of survival. Results: A total of 90 patients were included in the final study after being matched by propensity scores (RFA 45; PN 45). The 5-year overall survival (95% confidence interval [CI]) was 90.2% (78.6-98.8) vs 93.2% (85.8-98.9); 5-year cancer-specific survival (95% CI) was 95.6% (89.5-98.1) vs 97.7% (93.4-99.3); 5-year disease-free survival (95% CI) was 86.7% (89.5-96.7) and 88.5% (79.1-97.9); 5-year recurrence-free survival (95% CI) was 95.4% (89.3-98.1) vs 97.7% (93.3-99.2); and 5-year metastasis-free survival (95% CI) was 95.5% (89.4-98.0) vs 95.5% (89.4-98.0). Age was the only factor that could predict the disease-free survival (P=0.044). The percentage decrease in the glomerular filtration rate was significantly lower in the RFA group at the time of last follow-up (P=0.001). Conclusions: In the propensity-score matched cohort of patients with clinical T-1a RCC, we observed that RFA was an effective treatment option that provided comparable 5-year oncologic outcomes and better preservation of renal function than PN.

Abstract:Objective To investigate the role of intraoperative ultrasonography in radiofrequency ablation(RFA) for renal tumors.Methods A total of 82 consecutive patients with renal tumors underwent ultrasound-guided RFA,of whom 22 patients underwent open radiofrequency abalation and 60 patients underwent laparoscopic radiofrequency abalation.Intraoperative ultrasonography was applied to evaluate the renal lesions and guide the process of RFA.ResultsTherapeutic procedures were performed successfully in all patients.A complete ablation was achieved in 81 patients.A residual tumor in one patient was confirmed by review,who was given oral sunitinib daily thereafter.Conclusion In the radiofrequency abalation surgery for renal tumors,intraoperative ultrasonography takes a key role in selecting the suitable extent of ablation and protecting the normal kidney.

Abstract:Objective: To assess short-term and long-term outcomes following robotic enucleation (REn) of tumors in the proximal pancreas. Background: Despite the advantages of preserving function via pancreatic enucleation, controversies persist, since this can be associated with severe complications, such as clinically relevant postoperative pancreatic fistula, especially when performed near the main pancreatic duct. The safety and efficacy of REn in this context remain largely unknown. Methods: A retrospective analysis was performed of all patients who underwent REn for benign and low-grade malignant neoplasms in the pancreatic head and uncinate process between January 2005 and December 2021. Clinicopathologic, perioperative, and long-term outcomes were compared with a similar open enucleation (OEn) group. Results: Of 146 patients, 92 underwent REn with a zero conversion-to-open rate. REn was superior to OEn in terms of shorter operative time (90.0 minutes vs 120.0 minutes, P< 0.001), decreased blood loss (20.0 mL vs 100.0 min, P =0.001), and lower clinically relevant postoperative pancreatic fistula rate (43.5% vs 61.1%, P =0.040). Bile leakage rate, major morbidity, 90-day mortality, and length of hospital stay were comparable between groups. No post-REn grade C POPF or grade IV/V complication was identified. Subgroup analyses for uncinate process tumors and proximity to the main pancreatic duct did not demonstrate inferior postoperative outcomes. In a median follow-up period of 50 months, REn outcomes were comparable to OEn regarding recurrence rate and pancreatic endocrine or exocrine function. Conclusions: REn for pancreatic head and uncinate process tumors improved clinically relevant outcomes without increased major complications compared to OEn, while demonstrating comparable long-term oncological and functional outcomes.

Abstract:Objectives Tumor enucleation (TE) optimizes parenchymal preservation with promising short-term oncologic outcomes compared with standard partial nephrectomy (SPN). However, researches/literatures about long-term oncologic outcomes for TE after minimally invasive surgery are scarce. We aim to analyze long-term oncologic outcomes after laparoscopic and robotic tumor enucleation for renal cell carcinoma (RCC). Patients and Methods We retrospectively analyzed 146 patients who underwent TE with either laparoscopic or robotic approach for localized RCC in our center. Local recurrence, cancer specific survival (CSS), recurrence free survival (RFS), and overall survival (OS) were the main outcomes. Survival curves were generated using a Kaplan-Meier method. Perioperative outcomes and pathological outcomes were also analyzed. Results Overall, 98 male and 48 female patients were eligible for the study. The median tumor size was 3.4 cm with a median R.E.N.A.L. score of seven. Warm ischemia was used in 143 patients with a median ischemia time of 20 min and three patients had zero ischemia. Five patients (3.4%) had major complications (> Clavien IIIa) and only two were related to urinary system. The median global glomerular filtration rate (GFR) preserved after surgery was 93%. Pseudocapsule invasion was reported in 50 tumors (34%) and positive surgical margins were found in 3/146 (2.1%) tumors. At a median follow-up of 66 months, local recurrence happened in two patients (1.4%), and systemic recurrence happened in six patients (4.2%). The 5-year CSS, RFS, OS were 95.7, 89.6, and 91.9%, and the 10-year CSS, RFS, OS were 93.8, 89.6, and 90.0%, respectively. Conclusion This study indicates that tumor enucleation with laparoscopic or robotic approach in experienced hands for the treatment of RCC appears oncologically safe with a median follow-up of more than 5 years. Prospective studies with more patients and longer follow-up will be required to further evaluate oncologic safety after TE.

Abstract:Objective To assess the intraoperative efficacy of contrast-enhanced ultrasonography (CEUS) in the radiofrequency ablation (RFA) of renal tumors.Methods RFA was performed on 110 renal tumors (size range:1.5-8.6 cm,49 located in the left,55 right and 3 bilateral) in 107 selected patients.Conventional tumor biopsy was conducted for pathological diagnosis using 18G biopsy needle after RFA.CEUS was performed in all patients before,during and after RFA to assess the necrotic area.Once suspicious residual lession was discovered by CEUS,another round of RFA was taken immediately.Initial follow-up enhanced CT and CEUS was performed seven days after the procedure,with subsequent CEUS and enhanced CT after one month,three months,and six months to assess the necrotic area.Technical success was defined as elimination of areas that enhanced at imaging within the entire tumor.Results With CEUS performed during RFA,110 tumors (100％) were successfully ablated during one operation.Of these,five tumors was subjected to additional RFA treatment for the suspicious residual lesion detected by CEUS right after initial RFA.Elimination of areas covered the entire tumor after the second RFA.No residual or recurrence tumors was confirmed by both enhanced CT and CEUS during follow-up.All patients survived in the follow-up phase which ranged from 3 to 18 months(mean 11 months).Conclusions With the application of intraoperative CEUS,there were less residual tumors after RFA.

Simple enucleation has proven to be oncologically safe. In this study, we used our large institutional experience to compare perioperative results and early oncological outcomes with modified robot-assisted simple enucleation (MRASE) and robot-assisted partial nephrectomy (RAPN) for treating localized renal tumors. METHODS We evaluated 445 consecutive patients who underwent MRASE or RAPN for renal tumors in our institution from September 2014 to July 2017 in terms of perioperative pathologic and oncologic outcome variables. In simple enucleation, the surgeon used the pseudocapsule as the anatomical landmark to enucleate the tumor by blunt dissection, with no visible rim of normal parenchyma. The single layer suture technique was performed for renal reconstruction. RESULTS In total, 310 patients underwent MRASE and 135 underwent RAPN. Mean operative time was 181.3 and 192.3 min, respectively (p=0.038). Warm ischemic time was significantly lower in the MRASE than RAPN group (20.9 vs 24.3 min; p=0.000). The estimated blood loss was similar (p=0.381). Tumor bed suturing was performed in 8.7% and 79.3% of MRASE and RAPN cases (P=0.000). No hilar clamping was performed in 42 MRASE patients (13.5%) and 8 RAPN patients (5.9%) (p=0.019). Grade III and IV complications were reported in 6 (1.9%) MRASE patients and 9 (6.7%) RAPN patients (p=0.024). The incidence of positive surgical margins was comparable between the MRASE and RAPN groups (1.9% and 5.2%, p= 0.118). After a median follow-up of 18 months, recurrence did not differ between the 2 groups: 9 (2.9%) MRASE patients and 5 (3.7%) RAPN patients (p=0.881). CONCLUSIONS MRASE may confer shorter warm ischemic time, almost no need for tumor bed suturing and less Grade III and IV complications than RAPN, with similar oncologic outcomes. Comparative efficacy of cryoablation versus robot-assisted partial nephrectomy in the treatment of cT1 renal tumors: a systematic review and meta-analysis

Abstract:Purpose: This study utilizes a meta-analytic approach to investigate the effects of cryoablation and robot-assisted partial nephrectomy on perioperative outcomes, postoperative renal function, and oncological results in patients. Methods: This study followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. Four electronic databases (PubMed, Embase, Web of Science, and the Cochrane Library database) were systematically searched to identify relevant studies published in English up to November 2023. The primary outcomes were perioperative results, complications, postoperative renal function and oncologic outcomes. Review Manager 5.4 was used for this analysis. Results: This study included a total of 10 studies comprising 2,011 patients. Compared to RAPN (Robot-Assisted Partial Nephrectomy), the CA (Cryoablation) group had a shorter hospital stay [MD -1.76 days; 95% CI -3.12 to -0.41; p = 0.01], less blood loss [MD -104.60 ml; 95% CI -152.58 to -56.62; p < 0.0001], and fewer overall complications [OR 0.62; 95% CI 0.45 to 0.86; p = 0.004], but a higher recurrence rate [OR 7.83; 95% CI 4.32 to 14.19; p < 0.00001]. There were no significant differences between the two groups in terms of operative time, minor complications (Clavien-Dindo Grade 1-2), major complications (Clavien-Dindo Grade 3-5), changes in renal function at 12 months post-operation, RFS (Recurrence-Free Survival), and OS (Overall Survival). Conclusion: The evidence provided by this meta-analysis indicates that the therapeutic effects of Cryoablation (CA) are similar to those of Robot-Assisted Partial Nephrectomy (RAPN) in terms of perioperative outcomes and renal function. However, the recurrence rate of tumors treated with CA is significantly higher.
