3D Printing Individual Templates Guided Brachytherapy in Head and Neck

Abstract:PurposeTo enhance the precise and reduce the difficulty of radioactive seed implants in head and neck, a kind of 3D printing individual template based on preplan was designed to transfer preplan to clinical implantation procedure and to constrain the site and orientation of implantation needles. And in this study the accuracy of the template was evaluated in vivo.Materials and MethodsFrom December 2010 to April 2013, 187 patients with malignant tumors of head and neck underwent 125I radioactive seeds implantation under the guide of 3D printing individual templates in Peking University School and Hospital of Stomatology. Nearly all sites of head and neck were involved in this study, such as parotid and masseter region, retromandibular region, maxillary and paranasal region, skull base, base of tongue, floor of mouth and neck. Before 125I implants, patients received computed tomography (CT) scans based on 0.75mm thickness. And the brachytherapy treatment planning system (BTPS) software was used to make the preplan based on the CT images. Mimics software and Geomagic software were used to read the data containing CT images and preplan, and to design the individual template. Then the individual template containing the information of needle pathway and face features simultaneously was made through 3D printing technique. All patients received 125I seeds interstitial implantation under the guide of the individual template [Fig.1]. And CT scan was made after needles insertion to evaluate the accuracy of needles. The positions and orientations of the simulated needles in the preplan CT data set and the actual needles in the intermediate CT data set with needles were compared and measured using Mimics software and Geomagic software in eighteen patients containing 348 needles. And the time cost by the implantation procedure was record and calculated. After implants, the seeds and dosages distribution (D90, V100, V150) evaluation was made using CT and TPS.ResultsThe individual templates were positioned easily and were stable. The seeds and dosages distribution (D90, V100, V150) were well meet the treatment requirement. The mean entrance point positioning deviation was 0.883 mm (range 0.002 to 2.943 mm) in different sites. The mean orientation deviation were 1.977° (range 0.536 to 4.566°). All needles arrived the preplanned position in 1 attempt manually.
