Abstract:The ovarian cycle is initiated by a small rise in the follicle stimulating hormone FSH which begins during the preceding menstrual cycle. One follicle matures to ovulation and secretes estradiol which rises to a peak and falls before ovulation. Estradiol produces recognizable changes in the vaginal cytology the endometrium the cervix and the cervical secretions. Estradiol is the mechanism which initiates the midcycle surge of luteinizing hormone which triggers ovulation. Progesterone production rises along with a second rise in estradiol. Progesterone is thermogenic and causes a rise in basal body temperature (BBT). Ovulation usually occurs when the basal body temperature rises. Artificial insemination practitioners measure the LH peak by taking a number of blood samples and inseminating daily for 2-3 days as soon as they measure the first LB peak. Urinary estrogen assays have been used for 20 years to determine ovulation but the measurement of estradiol by radioimmunoassay (RIA) is a more modern method. Changes in the quality and quantity of cervical mucus have proven to be another reliable indicator of ovulation. The mucus may be dry absent in the first several days after menstrual cycle but grow thicker wetter and more profuse as levels of estradiol rise. As levels of progesterone peak the mucus changes abruptly and becomes sticky and dry. Conception may occur during any of the fertile mucous days; fertility is greatest during the last two days of the cycle.
