Abstract:Objective Summarize the radiographic data of 48 patients with adolescent idiopathic scoliosis (AIS) to analyze the characteristics and influencing factors of cervical sagittal alignment in AIS patients. Methods For AIS patients, The angle of the main thoracic curve (TC), the proximal thoracic curve (PTC), the thoracolumbar curve (TLC), and the lumbar curve (LC), C2~7 Cobb angle, T5~12 Cobb angle, L1~5 Cobb angle, pelvic incidence (PI), pelvic tilt (PT), sacrum slop (SS), C2~7 sagittal vertical axis (C2~7 SVA), C7~S1 sagittal vertical axis (C7~S1 SVA) were included in radiographic measurements. In addition, the C2~7 Cobb angle was measured for the young volunteers. Lenke classification was used for AIS curve type classification. Pearson correlation coefficients and T-test were used for statistical analysis. Results The incidence of cervical deformity,cervical kyphosis and cervical imbalance was 75.0%, 47.9 and 10.4%respectively. The C2~7 Cobb angle of AIS patients significantly increased than the volunteers (P=0.008). In the cervical kyphosis group, T5~12 Cobb angle was significantly lower than that of the normal group (P=0.011). The C2~7 Cobb angle of the T5~12 Cobb angle>30°group increased significantly than the T5~12 Cobb angle≤30°group (P=0.021).The C2~7 Cobb angle was significantly correlated with T5~12 Cobb angle (P=0.009) and L1~5 Cobb angle(P=0.005). There was no significant relevance between C2~7 Cobb angle and TC,PTC,TLC,LC,Lenke Curve Type,PI,PT,SS,C7~S1 SVA(P>0.05). Conclusion The cervical sagittal alignment is only related with TK and LL, especially with TK. It has no relationship with Lenke curve type, coronal angle of thoracic and lumbar spine, and pelvic parameters(PI,PT,SS).

Abstract:Purpose To characterize the sagittal alignment of each Lenke type and investigate the relationship between coronal and sagittal deformities in adolescent idiopathic scoliotic (AIS) patients. Methods A cohort of 184 subjects with AIS was retrospectively recruited. Radiographic data were measured and collected, including the Lenke types, Cobb angles of structural curves, and sagittal spino-pelvic parameters. Subjects were grouped according to their genders, Lenke curve types, lumbar modifiers and the amount of coronal structural curves. The sagittal alignment was then compared between the different groups, and correlation analysis was also taken between coronal and sagittal parameters. Besides, each subject’s Roussouly type was decided and its distribution was compared among different Lenke types. Results The cohort included 59 males and 125 females, averagely aged at 15.5 ± 3.3 years old. Most sagittal parameters except thoracic kyphosis (TK) and pelvic tilt (PT) were similar among different Lenke types, while all the sagittal parameters were similar between males and females. The groups with different lumbar modifiers had similar sagittal parameters except TK, which was also true for the groups with different amounts of coronal curves. 42.4 % of the cohort belonged to Roussouly type 3, and the distribution of Roussouly types was comparable among all Lenke types. All sagittal parameters except C7 translation ratio were significantly different among Roussouly types ( P < 0.05). Correlation analysis showed that main thoracic (MT) was negatively correlated with lumbar lordosis (LL, r = −0.324), sacral slope ( r = −0.321) and spino-sacral angle ( r = −0.363). Partial correlation analysis found that thoracolumbar/lumbar was negatively correlated with TK ( r = −0.464) and LL ( r = −0.422) when MT was controlled. Conclusions The influence of coronal deformity on sagittal parameters was limited and mainly reflected in the deviation of TK. Most coronal and sagittal parameters were not significantly correlated, and the coronal deformity types did not change the global sagittal postural patterns.
Cervical Sagittal Alignment Limited Adjustment After Selective Posterior Thoracolumbar/Lumbar Curve Correction in Patients With Lenke Type 5C Adolescent Idiopathic Scoliosis.

Abstract:Study Design. A retrospective clinical and radiographic study. Objective. The aim of this study was to analyze the radiographic change in cervical sagittal alignment (CSA) and identify the possible factors influencing the postoperative CSA in selective posterior thoracolumbar/lumbar (TL/L) curve correction. Summary of Background Data. The correlation between the CSA and thoracic sagittal alignment (TSA) is a well-recognized phenomenon in patients with thoracic idiopathic scoliosis. However, little has been published about the influence of TL/L curve instrumentation on CSA. Methods. Thirty patients with Lenke type 5C adolescent idiopathic scoliosis (AIS) received the selective posterior TL/L curves fusion by pedicle screws. Preoperative and postoperative radiographic and clinical parameters were analyzed. C2-C7 lordosis less than 0 was considered a lordotic cervical spine (LCS), whereas more than 0 was considered a kyphotic cervical spine (KCS). Results. Preoperatively, 43% of patients showed a KCS, whereas the remaining 57% showed a LCS. C2-C7 lordosis was correlated with T1-slope, thoracic kyphosis (TK), and global-TK (Gl-TK) (P<0.05). The patients in the KCS group showed a smaller T1-slope, TK, and Gl-TK than those in the LCS group. Logistic regression analysis revealed that the T1-slope [odds ratio (OR) = 0.86, P = 0.008] and Gl-TK (OR = 0.90, P = 0.019) were risk factors for developing KCS. In the LCS group, 5 of 12 patients with LCS exhibited KCS after surgery. The T1-slope and Gl-TK in the changed group were considerably smaller than those in the maintained group (P = 0.004 and 0.035, respectively). In the KCS group, 6 of 13 patients with KCS unrecovered to LCS after the operation. The T1-slope in the unrecovered group was smaller than that in the recovered group (P = 0.045). Conclusion. C2-C7 lordosis was strongly correlated with the T1-slope. C2-C7 lordosis of Lenke 5C AIS was related to the global TSA rather than TK and proximal TK. As time progresses, spontaneous adjustment of CSA is limited because of the inherent rigidity of the cervical spine.
Anterior Selective Lumbar Fusion Saving More Distal Fusion Segments Compared with Posterior Approach in the Treatment of Adolescent Idiopathic Scoliosis with Lenke Type 5: A Cohort Study with More Than 8-Year Follow-up

Abstract:Objective To investigate whether anterior selective fusion (ASF) could save more distal fusion segments compared with posterior approach in the treatment of Lenke type 5 adolescent idiopathic scoliosis with long term follow-up. Methods A retrospective cohort study. From 2008 to 2011, 22 AIS girls with Lenke type 5 who underwent ASF or posterior selective fusion (PSF) with more than 8-year follow-up, were extracted from the database. 13 girls in the ASF group had an average age of 14.3 +/- 1.3 years and Risser sign of 3.3 +/- 1.1; 9 PSF girls had an average age of 16.2 +/- 3.6 years and Risser sign of 3.8 +/- 1.5. The radiographic outcome was compared between groups preoperatively, 6-month postoperatively, 8-year postoperatively and at last follow-up (>8 years). Results The average follow-up duration was 8.7 +/- 0.4 (ASF) and 8.8 +/- 0.5 (PSF) years, respectively. There was no significant difference at baseline in age, Risser sign and preoperative curve pattern in the coronal and sagittal plane between the groups (P > 0.05). The ASF group had significantly shorter fusion segments (5.1 +/- 0.6 vs. 7.0 +/- 1.3) and decreased upper instrumented vertebra (UIV) (T-11 +/- 0.8 vs. T-10 +/- 0.8) than the PSF (P < 0.05); while no significant difference was found in the lower instrumented vertebra (LIV) and distal reserved segments (P > 0.05), which suggested that ASF could shorten the fusion segments by lowering UIV. The distal compensatory curve in the ASF group (9.0 degrees +/- 3.9 degrees) was significantly larger than in the PSF group (3.3 degrees +/- 2.4 degrees, P = 0.003), despite of no significant difference in the incidence of coronal imbalance (P > 0.05), indicating that both two approaches could obtain satisfactory correction in the coronal plane. In the sagittal plane, PSF patients had significantly larger lumbar lordosis (LL, 59.1 degrees +/- 10.5 degrees), thoracic kyphosis (TK, 37.2 degrees +/- 13.3 degrees) and proximal junctional angle (PJA, 13.3 degrees +/- 6.1 degrees) at the last follow-up than the ASF (LL: 43.4 degrees +/- 9.4 degrees; TK: 20.7 degrees +/- 8.4 degrees; PJA: 4.7 degrees +/- 3.4 degrees; P < 0.05), but without significant difference in proximal junctional kyphosis (PJK) and sagittal vertical axis (SVA) (P > 0.05). After controlling for age, Risser sign, and radiographic parameters related to the primary curve pattern, shorter fusion segments and more distal reserved segments still remained significant in the ASF group with greater Risser sign (P < 0.05). No major intra- or post-operative complications occurred. Conclusions Both ASF and PSF could obtain satisfactory coronal and sagittal correction for Lenke 5 AIS; compared with PSF, ASF could shorten the fusion segments by lowering UIV, and save more distal fusion segments only in patients with greater skeletal maturity.
[Correlation Analysis Between the Sagittal and Coronal Parameters of Spino-Pelvic in Lenke Type 1 Adolescent Idiopathic Scoliosis].

Abstract:OBJECTIVE:To explore the relationship between spino-pelvic sagittal and coronal parameters in Lenke 1 group of adolescent idiopathic scoliosis (AIS).METHODS:The subjects were retrospectively collected from 2005 to 2013. On the posteroanterior and lateral radiographs, apical vertebra (AV), Cobb angle of main thoracic curve (MT), pelvic incidence (PI), C7 translation ratio (C7TR) and other sagittal parameters were measured and recorded. Comparison and correlation studies were conducted between these parameters using specific softwares.RESULTS:In the study, 51 subjects, including 18 males and 33 females, were recruited, aged (14.9±2.0) years averagely. The apical vertebra ranged from T7 to T11,with mean MT being 49.6°±16.7°, and mean PI 44.7°±6.7°. Significant correlation existed between PI and PT, SS, LL, as well as between LL and SS, TK (P <0.05). Significant differences were found in TK, LL and SS among the different LM groups, but no difference in the other sagittal parameters. AV had no significant correlation with any sagittal parameter. MT was significantly correlated with TK, LL and SS, but its correlation with PI was not significant.CONCLUSION:Most of sagittal parameters were significantly correlated in Lenke 1 adolescent idiopathic scoliosis, forming a regulation chain of spine-pelvic sagittal balance on the basis of PI. Significant correlation exists between some sagittal and coronal parameters.
Analysis of the cervical spine sagittal alignment in young idiopathic scoliosis: a morphological classification of 120 cases

Abstract:Purpose To analyze the relationship between the cervical spine and global spinal-pelvic alignment in young patients with idiopathic scoliosis based on a morphological classification, and to postulate the hypothesis that cervical kyphosis is a part of cervico-thoracic kyphosis in them. Methods 120 young patients with idiopathic scoliosis were recruited retrospectively between 2006 and 2011. The following values were measured and calculated: cervical angles (CA), cervico-thoracic angles (CTA), pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS), spinal sacral angle (SSA), hip to C7/hip to sacrum, thoracic kyphosis (TK), lumbar lordosis (LL), Roussouly sagittal classification, Lenke Type Curve and Lumbar Modifier. The cervical curves were classified as lordosis, straight, sigmoid and kyphosis. They were categorized into four groups as cervical non-kyphosis group (CNK Group), cervical kyphosis group (CK Group), cervical-middle-thoracic kyphosis group (CMTK Group), and cervical-lower-thoracic kyphosis group (CLTK Group) according to their morphological characters of sagittal alignments. All parameters were compared and analyzed among groups. Results The incidence of cervical kyphosis was 40 % (48/120). The CA and the CTA were in significant correlation ( r = 0.854, P = 0.00). The cervical spine alignments were revealed to be significantly different among groups ( r = 85.04, P = 0.00). Significant differences among groups in CA, CTA and TK were also detected. A strong correlation between the group type and Lenke Lumbar Modifier was still seen ( P < 0.05). Fisher’s exact test revealed that the individual vertebral body kyphosis and wedging were directly related to the overall cervical kyphosis ( P = 0.00, respectively). Conclusion The cervical kyphosis is correlated with global sagittal alignment, and is a part of cervico-thoracic sagittal deformity in young patients with idiopathic scoliosis. Despite the deformity in cervical alignment, the global spine could still be well-balanced with spontaneous adjustment. The correlation between our grouping based on the morphological characteristics of the sagittal alignments and Lenke Lumbar Modifier suggests that the coupled motion principle be appropriate to explain the modifications both in coronal and sagittal planes.
Sagittal Profile Response of Cervical Spine after Posterior Correction in Thoracic and Lumbar Adolescent Idiopathic Scoliosis: Correlation with Thoracic Kyphosis?

Abstract:OBJECTIVE: To analyze postoperative changes in cervical alignment in patients with adolescent idiopathic scoliosis (AIS) with different curve patterns. METHODS: Radiographic data were retrospectively reviewed in 43 patients with AIS with right thoracic major curve versus 39 with major lumbar curve with a minimum of 2 years' follow-up. Radiographic parameters analyzed in this study included cervical sagittal parameters (cervical lordosis [CL], T1 slope, T1 slope minus C2-C7 lordosis, T1-spine, T1 pelvic angle, and C2-C7 sagittal vertical axis) and spinopelvic sagittal parameters obtained from radiographs. Paired t tests were used for comparison with 0.05 as a statistically significant threshold. RESULTS: At baseline, larger CL (5.69 degrees vs. - 5.12 degrees, P = 0.002) and smaller T1 slope minus C2-C7 lordosis (9.26 degrees vs. 17.09 degrees , P = 0.001) was noted in patients with lumbar-curve adolescent idiopathic scoliosis (l-AIS), whereas preoperative thoracic kyphosis (TK) was not different between the 2 groups. When the immediate postoperative sagittal profiles were compared between the 2 groups, larger TK (23.72 degrees vs. 18.86 degrees, P = 0.009) and more obvious CL (7.26 degrees vs. -2.60 degrees, P = 0.001) were noticed in the L-AIS group. During the follow-up, larger TK and CL were still maintained in the L-AIS group. In addition, a significant correlation was found between the improvement of CL and TK restoration in patients with L-AIS (r = - 0.473, P = 0.002). CONCLUSIONS: Correlations between the improvement of CL and TK highlight the importance of restoration of patients with normal TK or AIS. Reciprocal changes in cervical alignment may happen if the TK was also simultaneously restored in patients with AIS. For patients with different curve patterns, the cervical sagittal parameters tend to be similar during follow-up.
Sagittal alignment of the cervical spine: radiographic analysis of 111 asymptomatic adolescents, a retrospective observational study

Abstract:Purpose To describe the cervical spine morphology and explore its relationship to global sagittal alignment parameters in the asymptomatic adolescent population. Methods A total of 111 adolescent subjects were included. Sagittal alignment parameters, including C7 Slope, C2-C7 Cobb, C2-7 plumb line (PL), C2-S1 Sagittal Vertical Axis (SVA), C7-S1 SVA, T5-12 Cobb, T10-L2 Cobb, L1-S1 Cobb, pelvic incidence (PI), pelvic tilt (PT) and sacral slope (SS), were obtained from lateral radiographs. Results Forty-four males and sixty-seven females with a mean age of 16.12 ± 2.40 years were included in this study. The mean values of C7 Slope, C2-7 Cobb and C2-7PL were 20.45 ± 8.88°, -7.72 ± 12.10°, and 13.53 ± 11.63 mm, respectively. C2-7 Cobb, C7 Slope showed significant differences between the male and female groups. Correlation analysis showed that C7 slope was significantly correlated with C2-7 Cobb ( r = -0.544, P < 0.001), C2-S1 SVA ( r = 0.335, P < 0.001), and C7-S1 SVA ( r = 0.310, P = 0.001), but not lumbosacral parameters(L5-S1 Cobb, PI, PT, SS). Using a modified method of Toyama to describe the cervical spine morphology, there were 37 cases (33.3%) in the Lordotic group, and C7 slope, C2-7 Cobb and C2-7PL showed significant differences between groups. According to C2-C7 Cobb, there were 80 Lordotic cases (72.1%). C7 slope and C2-7PL were significantly different between the two groups. Conclusion The cervical spine morphology of asymptomatic adolescents varies widely, from lordotic to kyphotic. Combining different classification methods provides a better understanding of the morphology of the cervical spine. C7 slope is an important predictor of global sagittal balance and C2-7PL is a key parameter for restoring cervical lordosis, which should be considered pre-operatively and for conservative treatment. Cervical regional sagittal alignment parameters are not correlated with lumbosacral parameters, and C2-7 Cobb, C7 Slope showed significant differences between males and females.
Occipitocervical sagittal alignment in normal adolescents and adolescents with idiopathic scoliosis

Abstract:Objective To compare the occipitocervical sagittal alignment in normal adolescents and age-and gender-matched patients with adolescent idiopathic scoliosis (AIS) and analyze the relationship between occipitocervical parameters and cervical sagittal balance.Methods A total of 100 adolescent volunteers and 80 patients with AIS at the Affiliated Drum Tower Hospital of Nanjing University Medical School from March 2012 to March 2014 were recruited.Occipital incidence (OI),occipital slope (OS),occipital tilt (OT),C0-C2 angle,C2-C7 angle and C0-C7 angle were measured on sagittal occipitocervical image.The occipitocervical parameters were compared between the two groups and the relationship with age and gender was further evaluated.Correlations between occipitocervical parameters and cervical sagittal alignment in two groups were analyzed with Pearson coefficient.Results The mean OI,OS,OT were 36.12° ± 2.55° (30°-44°),26.34° ± 8.41° (15°-46°),-10.06° ± 7.51° (-22°-11°) in normal group and 35.62° ± 3.01 ° (31 °-42°),24.27° ± 8.49° (7°-42°),-11.52° ± 9.23° (-28°-10°) in AIS group,respectively.Occipitocervical parameters in two groups were not significantly different (t =0.878,t =1.014,t =1.306,all P values ＞0.05).Furtherly,no significant differences were noted between males and females,or between younger (≤ 14 years) and older subjects (＞ 14 years) (all P values ＞ 0.05).Significant correlations were observed between OI and C0-C2 angle (r =0.307,P ＜ 0.05),OI and C0-C7 angle(r =0.298,P ＜0.05),OS and C2-C7 angle(r =0.402,P ＜0.05),OS and C0-C7 angle (r =0.560,P ＜ 0.05),OT and C2-C7 angle (r =0.428,P ＜ 0.05) and OT and C0-C7 angle (r =0.550,P ＜ 0.05) in normal group.However,only the correlations between OI and C0-C2 angle(r =0.532,P ＜ 0.05),OS and C0-C2 angle (r =0.620,P ＜ 0.05) were significant in AIS group.Conclusions There is no significant difference in occipitocervical parameter between normal adolescents and those with AIS,with no influence by age and gender.OI,OS and OT are significantly correlated with cervical sagittal alignment in normal adolescents.However,the occipitocervical parameters are only correlated with the upper cervical sagittal alignment in patients with AIS.

To investigate the preoperative spinopelvic sagittal alignment in Lenke 5 patients with adolescent idiopathic scoliosis (AIS), and analyze how it alters after posterior correction. Summary of Background Data. The structural thoracolumbar or lumbar curve may change the local sagittal alignment thereby altering the sagittal balance in Lenke 5 patients with AIS. However, few studies have evaluated the spinopelvic sagittal alignment before and after the surgery in these patients. Methods. Forty-eight Lenke 5 patients with AIS who underwent posterior correction and fusion were included in this study. Preoperative and postoperative radiographs were reviewed measuring both the coronal and sagittal parameters. Three pelvic sagittal states (anteverted, normal, or retroverted) were evaluated according to the magnitude relationship of individual pelvic tilt with pelvic incidence (PI). Both the coronal and sagittal parameters between different pelvic sagittal states were compared. The alterations of these parameters by surgery would also be analyzed. Results. The mean follow-up was 1.8 years. Preoperatively, the mean PI was 44.3 degrees with a pelvic tilt of 4.1 degrees. There was 48% patients showing the anteverted pelvis, whereas the remaining 52% showing normal. The patients with anteverted pelvis showed a smaller PI and more distal lower end vertebra than normal pelvis ones. Logistic regression analysis revealed PI (odds ratio [ OR] = 0.62, P = 0.024) and lower end vertebra (OR = 2.1, P = 0.037) were significantly associated with the risk of developing anteverted pelvis. The pelvic tilt was significantly increased and 61% of patients with preoperative anteverted pelvis had recovered. Logistic regression analysis revealed PI (OR = 0.7, P = 0.034) and lower instrumented vertebra (OR = 6.5, P = 0.002) were significantly associated with the risk of postoperative uncovered of anteverted pelvis. Conclusion. Anteverted pelvis appears in almost half of Lenke 5 patients with AIS, especially in who have smaller PI or distal lower end vertebra. The abnormal pelvic sagittal state will be generally corrected by posterior correction surgery except for patients with a PI less than 39 degrees or a lower instrumented vertebra that extends to L5.
Sagittal Plane Analysis of the Spine and Pelvis in Adult Idiopathic Scoliosis.

Abstract:BACKGROUND:There has been an increasing recognition of the importance of sagittal spinopelvic alignment in patients with scoliosis as it relates to clinical outcomes. However, the changes seen in sagittal spinopelvic alignment in adult idiopathic scoliosis patients is poorly defined. This study was conducted to evaluate the sagittal alignment of pelvis and spine in adult idiopathic scoliosis patients.METHODS:The sagittal parameters of the spine and pelvis were analyzed in lateral standing radiographs of 124 patients (mean age 47.4 years) with adult idiopathic scoliosis, including thoracic kyphosis (TK), thoracolumbar junction kyphosis (TLJ), lumbar lordosis (LL), pelvic incidence (PI), sacrum slope (SS), pelvic tilt (PT) and C7 plumb line (C7PL). The patients were divided into three groups according to the age: 20 - 40 years, 41 - 64 years, and ≥ 65 years. The parameters were compared with those in normal adults and adolescent idiopathic scoliosis (AIS) patients. The relationship between all parameters as well as age and sagittal parameters were analyzed.RESULTS:The PI in patients with adult idiopathic scoliosis was 58.1° ± 13.0°, which was significantly higher than that in normal adults. The PT (19.9° ± 10.6°) was also higher than that in both normal adults and AIS patients, while the SS (38.1° ± 12.0°) was similar or smaller. As age increased, C7PL, PT and TJL increased while LL decreased. There was no relationship between age and both PI and TK. PT had the strongest statistical association with the C7PL.CONCLUSIONS:PI is higher in adult idiopathic scoliosis than normal subjects. The PT is the most relevant pelvic parameter to the global sagittal alignment of the spine. Age significantly influences sagittal parameters of the spine and pelvis except the PI and TK.

Abstract:STUDY DESIGN:A retrospective radiographical study investigated the sagittal alignment in healthy Chinese girls and Chinese girls with idiopathic thoracic scoliosis (T-AIS). OBJECTIVE:To evaluate the sagittal alignment of the pelvis and spine in Chinese girls with idiopathic scoliosis and healthy girls and to assess whether the pelvic morphology differed between white and Chinese girls with AIS. SUMMARY OF BACKGROUND DATA:It has been shown that patients with AIS have an abnormal spinopelvic balance and pelvic morphology. Race is a determinant factor of sagittal spinal alignment and serves as a reminder when planning surgical reconstruction for spinal deformity. Until now, there have been no studies documenting the sagittal lumbosacral spine morphology in Chinese girls with T-AIS. METHODS:In this study, 95 patients with T-AIS and 33 healthy age-matched adolescents were recruited consecutively. Sagittal spinal and pelvic parameters were measured from the standing lateral radiograph, including thoracic kyphosis (TK), lumbar lordosis (LL), upper arc of LL, lower arc of LL, pelvic incidence (PI), sacrum slope (SS), and pelvic tilt (PT). Analysis of variance was used in the comparison of each dependent variable between patients with AIS and healthy adolescents. The relations between all parameters were determined via Pearson correlation coefficient (r). RESULTS:For all the sagittal parameters, only the TK and the upper arc of the LL showed significant differences between girls with AIS and healthy girls. The LL, lower arc of the LL, and 3 pelvic parameters were similar for both groups. The TK was found to be strongly correlated with LL and the upper arc of the LL in both groups. However, the TK was not related to the lower arc of the LL, nor were the 3 pelvic parameters in either group. In addition, LL was found to be associated with PI and SS in both groups. The lower arc of the LL was also correlated with PI for both groups. The PI was related to PT and SS in both groups; however, no correlation was found between PT and SS. In this study, the TK (15.7°), SS (35.1°), and PI (44.2°) were found to be significantly lower in Chinese patients with T-AIS than the values reported in the AIS cohort. CONCLUSION:In our study, the Chinese girls with T-AIS had similar PI, PT, and SS values when compared with the age-matched healthy girls. There were significant differences in pelvic morphology between Chinese and white girls with AIS. These results suggest that race may influence an individual's spinopelvic morphology. Although we have shown that the TK could affect LL through the upper arc of the LL directly, the evaluation of the thoracolumbar morphology of T-AIS before surgery is important for surgical planning.
An Increased Kyphosis of the Thoracolumbar Junction is Correlated to More Axial Vertebral Rotation in Thoracolumbar/Lumbar Adolescent Idiopathic Scoliosis

Abstract:Study Design. A retrospective correlation study.Objective. To identify radiographically if there is a correlation of the sagittal thoracolumbar alignment to the axial vertebral rotation and coronal scoliotic curvature in thoracolumbar/lumbar (TL/L) adolescent idiopathic scoliosis (AIS).Summary of Background Data. The consistent positive correlation between coronal angulations and axial rotation has been defined as a typical feature of AIS. However, the correlation of sagittal alignment transformation to axial and/or coronal deformity has not been definitely described, especially for primary TL/L AIS.Methods. On standard anteroposterior and lateral radiographs of 43 patients with TL/L AIS, coronal and sagittal parameters including primary TL/L and compensatory thoracic curve, thoracic kyphosis, thoracolumbar junctional sagittal curve (TLJS), and lumbar lordosis, were evaluated using the Cobb method. Apical vertebral rotation (AVR) was evaluated using the Perdriolle torsionmeter. The correlation was investigated between all parameters. After the patients were divided into 2 subgroups according to the TLJS curvature, the average AVR and coronal TL/L Cobb were compared between the 2 groups.Results. The average TLJS Cobb was 4.5 degrees +/- 8.8 degrees (range, -12.5 degrees-24.3 degrees) with 28 cases (65.1% of all cases) in TLJS+ (kyphosis) group and 15 cases in TLJS- (lordosis) group. Significant correlations were observed between AVR and coronal TL/L curvature (P < 0.001), and between AVR and sagittal TLJS Cobb angle (P < 0.001). These correlations still existed when the partial correlation analysis was conducted. The correlation between sagittal TLJS and coronal TL/L curvature became nonsignificant (P = 0.405) when the partial correlation analysis was conducted with AVR as control variable. Besides, a significant difference of 7.0 degrees was observed in average AVR between TLJS+ and TLJS+ subgroups (P < 0.001).Conclusion. From the standard anteroposterior and lateral radiographs, an increased kyphosis of the thoracolumbar junction was observed in TL/L AIS, and it was demonstrated to be positively correlated to more axial vertebral rotation of the scoliotic spine.

Abstract:OBJECTIVESTo evaluate the influence of thoracic kyphosis to sagittal alignment and balance of the lumbosacral vertebrae in thoracic adolescent idiopathic scoliosis patients.METHODSStanding posteroanterior and lateral x-rays of a cohort of 55 patients with thoracic adolescent idiopathic scoliosis were obtained. The patients were classified according to their thoracic kyphosis, the first group TK < 10 degrees and the second group 10 degrees < or = TK < or = 40 degrees . The following parameters were measured: lumbar lordosis (LL), upper and lower arc of lumbar lordosis, sagittal vertical axis, sacral slope (SS), pelvic incidence (PI), pelvic tilt (PT). Sagittal plane parameters were analyzed using t-test between two groups, with significance set at P < 0.05. Linear correlations between parameters were calculated using Pearson correlation coefficients, with significance set at P < 0.01.RESULTSThere were smaller LL and upper arc of lumbar lordosis in the first group. Significant linear correlations were found between each single adjacent shape parameter. Significant correlations were also found between TK, LL and upper arc of lumbar lordosis, as well as between PT, SS and PI.CONCLUSIONSSagittal alignment and balance of the lumbosacral vertebrae may influence the thoracic kyphosis in AIS patients. The mechanism of this influence may through the adaptation of upper arc of lumbar lordosis. This influence must be considered in thoracic adolescent idiopathic scoliosis patients who undergo selective posterior thoracic fusion.
Global sagittal alignment after surgery of right thoracic idiopathic scoliosis in adolescents and adults with and without thoracic hypokyphosis

Abstract:This study aimed to characterize global sagittal alignment in adolescent idiopathic scoliosis (AIS) with normal kyphosis (NTK, kyphosis > 10°) and with thoracic hypokyphosis (THK, kyphosis < 10°), before and after posterior spinal fusion, and compare them with asymptomatic controls. 27 AIS girls and young adults with right thoracic curves were included (seventeen with age ≤ 18 years, then age > 21). Biplanar radiographies were acquired at baseline, immediate post-operatively, 1-year and 2-year follow-up, and 3D reconstruction of the spine and pelvis was performed. NTK and THK showed different global sagittal alignment, as well as differences compared to controls. AIS with THK at baseline had higher SVA/SFD (2.0 ± 2.9 vs - 0.4 ± 1.9; P < 0.05) and OD-HA (0.2 ± 1.4° vs - 1.3 ± 1.6°; P < 0.05) than controls, indicating that THK had compensated balance with unusual forward leaning posture. Immediately post-operation, SVA/SFD remained high (1.3 ± 3.0) while OD-HA reversed (- 1.2 ± 1.7°), indicating that THK patients had found partially compensated balance. After 2-yeas, both SVA/SFD (- 1.3 ± 2.1) and OD-HA (- 1.4 ± 0.9°) were normalized. The changes in global sagittal alignment and mechanism of balance are different in AIS with or without THK. As the head plays a critical role on balance during immediate and delayed post-operation, OD-HA can be complementary parameter for assessing global balance during post-operative follow-up of AIS patients with THK.
Sagittal spino-pelvic alignment in non-dystrophic thoracic scoliosis secondary to NF-1:a comparison with AIS and normal control

Abstract:Objective To compare sagittal spino-pelvic alignment in neurofibromatosis type-1(NF-1) associated with non-dystrophic thoracic scoliosis,adolescent idiopathic scoliosis(AIS),and normal control.Methods:Fifteen NF-1 patients with non-dystrophic thoracic scoliosis(group NF-1) less than 70 degrees were reviewed.Fifteen AIS patients(group AIS) matched for age,gender,curve pattern and Cobb angle were selected for comparison.In addition,age-and gender-matched healthy adolescents were recruited as control(group CTL,n=30).On standing full-spine lateral X-rays,the following parameters were measured: thoracic kyphosis(TK),thoracolumbar junctional angle(TLJ),lumbar lordosis(LL),pelvic incidence(PI),pelvic tilt(PT),sacral slope(SS),spinal tilt(ST) and spinal-sacral angle(SSA).Results:For the NF-1 group,the TK of 13 patients was within the normal range(10°~40°) with hyperhyphosis and hypohyphosis presenting in 1 each.The average TK of the NF-1 group was significant larger than that in the AIS group(5.6°± 7.6°versus 16.5°± 5.4°,P<0.05),and significant difference was found between the two groups in LL(-53.8°versus-47.8°,P<0.05).However,no significant differences were noted between the NF-1 and the CTL group in all eight sagittal parameters.ConclusionThe sagittal profile in non-dystrophic thoracic scoliosis secondary to NF-1 is similar to that in age-and gender-matched healthy adolescents,that is,either significant hypohyphosis,an obvious characteristic in AIS patients,or obvious hyperhyphosis occurs in NF-1 group.The postoperative sagittal decompensation might not be related to the preoperative sagittal alignment in NF-1 patients with non-dystrophic thoracic scoliosis.
Predictors of Ultimate Postoperative Cervical Sagittal Alignment in Main Thoracic Adolescent Idiopathic Scoliosis

Abstract:This is a retrospective study. The aim of this study was to access sagittal compensatory mechanism of the cervical spine in thoracic adolescent idiopathic scoliosis (T-AIS) before and after posterior spinal fusion and to identify preoperative or immediate postoperative radiographic parameters that can predict the ultimate cervical sagittal alignment (CSA) after long-term follow-up. A retrospective study was performed on 44 T-AIS patients treated with posterior spinal fusion and with at least 5 years of follow-up. Preoperative, immediate postoperative and latest follow-up radiographs were reviewed measuring cervical lordosis (CL), cervical sagittal vertical axis (CSVA), upper thoracic kyphosis (UTK), main thoracic kyphosis (MTK), global thoracic kyphosis (GTK), lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), and sacral slope (SS). Pearson correlation analysis, stepwise multilinear regression analysis, and receiver operator characteristic (ROC) curve were performed to define the relationship between ultimate CL and preoperative or immediate postoperative radiographic parameters. CL significantly improved from 6.6 +/- 8.8 degree kyphosis preoperatively to 3.8 +/- 8.7 degree kyphosis immediate postoperatively and to 0.5 +/- 7.3 degree lordosis at the latest follow-up. Pre-and postoperative CSVA showed no significant difference. Pearson correlation coefficient test showed that CL was only correlated to T1 slope and UTK before surgery, whereas it was correlated to T1 slope, UTK, and GTK after surgery. The following equation was developed to estimate the ultimate CL: ultimate CL = -2.792+ 0.510 x Preop CL+ 0.531 x Postop T1 slope. Furthermore, ROC curve showed that preoperative CL = >= -4.5 degree was strongly predictive and postoperative T1 slope >= 11.3 degree was moderately predictive of lordotic cervical spine after long-term follow-up. For T-AIS patients, CL significantly increased after surgery with the restoration of the global and regional sagittal profile. The sagittal compensatory mechanism of the cervical spine before surgery is different from that after surgery. In these patients, preoperative CL and immediate postoperative T1 slope could be predictors of the ultimate CSA after long-term follow-up.

Abstract:Objective To investigate the long-term effect of interbody cage support on the sagittal profile reconstruction after anterior selective correction in adolescent idiopathic scoliosis (AIS) patients with a Lenke type 5 curve. Methods Totally 40 AIS patients with a Lenke type 5 curve who received anterior selective single rod instrumentation and fusion were divided into two groups. During grafting in the surgery, a structural interbody cage was placed in group A, including 19 girls and 1 boy, aging 12-18 yrs; while morsellized autogenous rib or iliac bone in group B, including 17 girls and 3 boys aging 13-20 yrs. All pa- tients were followed up for more than one year and a half. The coronal correction of the scoliosis and the sagittal profile reconstruction were evaluated retrospectively. Results At final follow-up, more than 70% correction was achieved for the primary curve and a spontaneous correction for the secondary curve in both groups. No instrumentation-related complications or pseudarthrosis occurred. And similar postoperative changes were found between group A and group B, such as the increase in thoracic kyphosis, slight change in thoracohmbar junctional kyphosis, no significant loss of lumbar lordosis, and mild increase in proximal junctional measurement, as well as mild lordosis loss of the instrumented segments. In group A, the sagittal vertical axis improved from -24 mm postoperatively to -15 mm, and the distance between L1 and C7 plumb line decreased from -24 mm postoperatively to -12 mm, while there were no obvious changes in group B. Conclusion Anterior selective single rod instrumentation and fusion is a recommended method for treating AIS of Lenke type 5. Regarding the sagittal reconstruction, a structural interbody cage can bring a better to- tal balance during the long-term follow-up.
Imaging study of coccygeal morphology in adolescent idiopathic scoliosis

Abstract:The correlation between scoliosis and sagittal curvature of the cervical, thoracic, and lumbar spine have already been reported in previous studies. However, as a part of the spine, the change in coccygeal morphology in AIS patients has not yet been studied. In this study, a retrospective analysis was performed on 400 patients who were divided into a non-scoliotic group (206 patients) and an AIS group (194 patients). The Postacchini coccygeal radiological classification that was modified by Nathan was used to observe and compare the sagittal coccygeal morphology between the two groups. The results showed that the non-scoliotic group had the highest percentage (52.4%) of patients with type I and the lowest (3.4%) proportion of patients with type V; moreover, the AIS group had the highest percentage (69.1%) of patients with type I and the lowest (1.5%) proportion of patients with type V. The coccygeal morphology was significantly different between the non-scoliotic group and the AIS group ( P = 0.001). No significant differences in coccygeal morphology were found between the males and females in the two groups (mild and moderate scoliosis and different segmental scoliosis). In addition, a significant correlation between coccygeal morphology and scoliosis ( P = 0.035) was found. In conclusion, coccygeal morphology significantly differs between AIS patients and non-scoliotic adolescents. There was a smaller proportion of patients with a type I coccyx and a larger proportion of patients with a type II or type III coccyx in the AIS group than in the non-scoliotic group. In other words, the presence of a more pronounced coccygeal curve in AIS patients may be caused by an incorrect sitting position and an imbalance in the contraction of the pelvic muscles. It should be further studied whether correcting the sitting position and muscular imbalances could change coccygeal morphology and subsequently affect the development of AIS.
