Diagnostic value of combined detection of cerebrospinal fluid oligoclonal bands and IgG index in central nervous system diseases

Abstract:Objective: To study the diagnostic value of combined detection of cerebrospinal fluid oligoclonal bands and IgG index in central nervous system diseases. Methods: Eighty-one neurology patients from Ruijin Hospital were categorized into 2 groups, 38 cases of inflammatory diseases of central nervous system as the study groups and 43 cases of non-inflammatory diseases of the central nervous system served as the control group. The serum and cerebrospinal fluid(CSF)specimens were collected and oligoclonal bands(OCB) were assayed by isoelectric focusing electrophoresis, IgG in-dex(IgG I) was measured by immune scatter turbidimetry. Results: OCB was positive in 11 cases of multiple sclerosis(11/13), 2 in guillain-barre syndrome(2/9), 1 in neuro-optic myelitis(1/3), 2 in acute myelitis(2/5), 0 in viral encephalitis(0/5), 3 in other inflammatory diseases of the central nervous system(3/3). The IgG index was higher in patients of study groups than those in control group, but were not statistically significant, while only IgG I of multiple sclerosis patients(1.34 ±1.65) was significantly higher than that of control group(0.54 ±0.36, P <0.05). Conclusions: Detection of cerebrospinal fluid oligoclonal band and IgG index could be used to reflect endogenous synthesis of immunoglobulin IgG. It could be used as an auxiliary method for diagnosis of inflammatory diseases of the central nervous system.
