Analysis of oligocional band, IgG index and IgG synthesis rate in patients with multiple sclerosis

Abstract:Aim: To investigate the clinical significances of oligoclonal band (OCB), IgG index and IgG synthesis rate in patients with multiple sclerosis (MS). Methods: Samples of cerebral spinal fluid (CSF) and matched serum were obtained from 32 patients with CSF treated in the Department of Neurology, Third Hospital Affiliated to Sun Yat-sen University from October 2004 to December 2005 for observation, including 18 patients with traditional CSF and 14 patients with neuromyelitis optica. Forty-two patients with infectious disease in central nervous system (CNS), 38 patients with CNS non-infection diseases and 32 patients with peripheral nervous system disease were selected as the control group. The concentrations of IgG and albumn (Alb) in paired serum and CSF were detected with nephelometry, and the IgG index and IgG synthesis rate were calculated according to IgG Delpech index formula and IgG Tourtellotte synthesis rate formula. OCB were detected by isoelectrical focusing technique and avidin biotin-peroxidase complex method. Results: A total of 144 recruited patients were involved in the analysis of results. 1 The positive rate of OCB in MS patients was higher than patients with CNS non-infection diseases and peripheral nerve diseases with significant differences (37.50%,5.26% and 0 respectively, P < 0.05).2 The sensitivity of OCB, IgG index and IgG synthesis rate in MS group were significantly higher than those in the control group (37.50%,10.71%; 31.25%,7.14%;37.50%,8.92% respectively, P < 0.05).3 White blood cell count (WBC) counting and total protein concentration in CSF of OCB in positive MS patients were significantly higher than those in negative MS patients [(10.93±5.41)×106, (2.99±1.33)×106; (0.27±0.06),(0.19±0.06) g/L respectively, P < 0.05). There were no significant differences in IgG index between the two groups (P > 0.05). Conclusion: The positive rate of OCB in Chinese MS patients is low, which is also found in OCB of non-MS patients. There are certain significance of OCB combined with IgG index and IgG synthesis in diagnosis of MS, and there are differences in WBC counting and total protein concentration in CSF of OCB between positive and negative MS patients.
