Abstract:Objective:To determine the variation of regulatory T cells and B lymphocyte subset in peripheral blood and B lymphocytes in renal tissue of patients with idiopathic membranous nephropathy(IMN) and explore the relationship between the dysregulation of peripheral blood regulatory T cells,B cells and distribution of B lymphocytes in renal tissue and the clinical features of IMN. Methodology:Sixty-six patients diagnosed membranous nephropathy by renal biopsy,45 male and 21 female with age from 15 to 71 years old,excluded possible secondary factors,without large dose immunosuppressive agent using recently,were enrolled in the study.Another 40 age-and sex-matched healthy volunteers served as healthy controls(HC).The ABSolute cell numbers of peripheral blood regulatory T cells(CD4+CD25+Foxp3+ Treg cells),B lymphocyte(CD20+ cells) and T lymphocyte subsets(CD3+,CD4+ and CD8+ cells) were determined in IMN patients as compared to HC by flow cytometry.According to the level of urine protein(Upro),the IMN patients were divided into two groups,the first group with Upro≥3.5 g/d and the second group with Upro<3.5 g/d.The variation of peripheral blood regulatory T cells,B lymphocyte and T lymphocyte subsets was compared between the patients with IMN and HC,as well as between two subgroups of IMN patients.24 in 66 IMN patients of which B lymphocytes >30/mm2 in renal tissue were divided into two groups according to whether there is a focal aggregation of B lymphoctes.The variantion of Upro,NAG,RBP and periphral blood B lymphcytes are compared between the two groups. Results:(1) The ABSolute cell number of peripheral blood regulatory T cells was significantly lower in patients with IMN than that in HC [(23.2±10.7) cells/μl vs(31.4±7.29) cells/μl,P<0.01].(2) Comparing with HC,the ABSolute cell number of peripheral blood B cells in patients with IMN was elevated apparently [(208±111) cells/μl vs(137±64) cells/μl,P<0.01].(3) No significant differences in the ABSolute cell numbers of CD4+ cells [(715±267) cells/μl vs(657±214) cells/μl,P>0.05] and CD8+ cells [(393±178) cells/μl vs(464±200) cells/μl,P>0.05] could be found between IMN patients and HC,however,the ratio of CD4+/CD8+ cells of IMN patients was higher remarkably in patients with IMN as compared with that in HC(2.04±0.92 vs 1.52±0.48,P<0.01).(4) The ABSolute cell number of peripheral blood B cells was significantly higher in the first group(Upro≥3.5 g/d IMN patients) than that in the second group(Upro<3.5 g/d IMN patients) [(246±117) cells/μl vs(186±108) cells/μl,P<0.05].The ABSolute cell numbers of regulatory T cells,CD4+ cells,CD8+ cells and the ratio of CD4+/CD8+ cells between two subgroups showed no statistical difference(P>0.05).(5) The numbers of B lymphocytes infiltrated in renal tissue was higher in the IMN patients than that in HC.NAG of IMN patients with focal aggregation of B lymphocytes was increased significantly. Conclusion:The decrease of regulatory T cells,the up-regulation of B lymphocyte and the disequilibrium of CD4+/CD8+ cells in peripheral blood occurs in patients with IMN.The focal aggregation of B lymphocytes in renal tissue could present more serious acute tubular-intersitial damage,and the amount of peripheral B cells may be associated with the degree of proteinuria.Those parameters may predict the disease activity of IMN.
