Abstract:Serum TNF, IL-2 and sIL-2R levels were measured by a "sandwich" enzyme-linked immunosorbent assay with two antibodies in 27 patients with typhoid fever and 16 normal subjects exploring the roles of these in the pathogenesis of typhoid fever. The results showed that the serum TNF and sIL-2R levels in the patients were obviously higher than those in normal controls (P < 0.01). The serum IL-2 level in the patients was obviously lower than that in normal controls (P < 0.01). Dynamic detection of serum TNF, IL-2 and sIL-2R levels at different clinical phases in 15 patients showed: dynamic changes of serum TNF and sIL-2R levels in patients correlated closely with changes of temperature, disease stages, disease condition. And serum TNF and sIL-2R levels in the patients with abnormal liver function and complications were obiously higher than those in the patients without impaired liver function and compliations. The results indicated that the clinical detection to serum TNF and sIL-2R ran important roles to judgement of patient's condition, course of disease, prognosis and the therapeutic effects besides benefiting diagnosis of typhoid fever. So the detection to TNF and sIL-2R could be served as surveillance mark for clinical observation of typhoid fever patient's condition and judgement prognosis. The detection to serum IL-2 could help for diagnosis of typhoid fever, and be less significant as surveillance mark of clinical severity.

Abstract:A 3-year-old girl presented with fever, abdominal distension and anorexia for 3 days. Physical examination showed pallor, abdominal distension and hepatomegaly (4 cm below the costal margin). Laboratory tests showed granulocytosis (17.0–31.3 × 10/L), anaemia (Hb 60–85 g/L), eosinophilia (0.58–1.89 × 10/L), raised immunoglobulin E (IgE) (>400 IU/mL), C-reactive protein (CRP) (>1600 mg/L) and erythrocyte sedimentation rate (ESR) (135 mm/h). Chest computed tomography showed multiple small nodules with sharp borders and irregular size scattered in both lung fields (Fig. 1a). Magnetic resonance imaging showed multiple nodular lesions in the liver (Fig. 1b), and enlarged abdominal aortic and superior mesenteric lymph nodes. Abstract:Eosinophilic dermatosis of hematologic malignancy is a multifaceted dermatosis with a wide morphological spectrum, presenting as pruritic, erythematous, papular and occasionally vesicular, urticarial, nodular eruptions. Histopathologically eosinophil infiltration in the super and deep dermis was found. We reported a case of eosinophilic dermatosis of hematologic malignancy presented as urticarial and vesicular lesions in a patient with chronic lymphocytic leukemia. A skin biopsy revealed a prominent subepidermal blister and a diffuse infiltrate of eosinophils with flame figures in the dermis and subcutaneous tissue. Although flame figures associated with eosinophilic dermatosis of hematologic malignancy is rarely reported, we believe that it would not seem unusual to find them in this skin disease. Eosinophilic cellulitis, which share clinical and histological features with eosinophilic dermatosis of hematologic malignancy, has also been described as showing an association with hematoproliferative diseases. In order to clearly describe eosinophilic dermatosis in patients with hematologic malignancies, the terminology eosinophilic dermatosis of hematologic malignancy, instead of eosinophilic cellulitis, would be a more suitable term in patients with eosinophilic dermatosis.
Rapidly Escalating Hepcidin and Associated Serum Iron Starvation Are Features of the Acute Response to Typhoid Infection in Humans

Abstract:Background: Iron is a key pathogenic determinant of many infectious diseases. Hepcidin, the hormone responsible for governing systemic iron homeostasis, is widely hypothesized to represent a key component of nutritional immunity through regulating the accessibility of iron to invading microorganisms during infection. However, the deployment of hepcidin in human bacterial infections remains poorly characterized. Typhoid fever is a globally significant, human-restricted bacterial infection, but understanding of its pathogenesis, especially during the critical early phases, likewise is poorly understood. Here, we investigate alterations in hepcidin and iron/inflammatory indices following experimental human typhoid challenge. Methodology/principal findings: Fifty study participants were challenged with Salmonella enterica serovar Typhi and monitored for evidence of typhoid fever. Serum hepcidin, ferritin, serum iron parameters, C-reactive protein (CRP), and plasma IL-6 and TNF-alpha concentrations were measured during the 14 days following challenge. We found that hepcidin concentrations were markedly higher during acute typhoid infection than at baseline. Hepcidin elevations mirrored the kinetics of fever, and were accompanied by profound hypoferremia, increased CRP and ferritin, despite only modest elevations in IL-6 and TNF-alpha in some individuals. During inflammation, the extent of hepcidin upregulation associated with the degree of hypoferremia. Conclusions/significance: We demonstrate that strong hepcidin upregulation and hypoferremia, coincident with fever and systemic inflammation, are hallmarks of the early innate response to acute typhoid infection. We hypothesize that hepcidin-mediated iron redistribution into macrophages may contribute to S. Typhi pathogenesis by increasing iron availability for macrophage-tropic bacteria, and that targeting macrophage iron retention may represent a strategy for limiting infections with macrophage-tropic pathogens such as S. Typhi.
Colonoscopic manifestations of typhoid fever with lower gastrointestinal bleeding

Abstract:Background: Intestinal bleeding in patients with typhoid fever usually occurs in the ileum. However, endoscopic findings in such patients are not well established. We examined the colonoscopic manifestations of intestinal lesions with bleeding in patients with typhoid fever. Patients and methods: The colonoscopic findings of seven patients who presented with haematochezia due to typhoid fever were reviewed retrospectively. Typhoid fever was diagnosed when the Salmonella typhi was isolated or when the Widal test showed strongly positive reactions. Results: Clinical data and colonoscopic findings were reviewed in seven patients (four men and three women with an average age of 42 years). The most commonly involved area was the terminal ileum (100%), followed by the ileocecal valve (57%), the ascending colon (43%), and the transverse colon (29%). Left colon was intact in all cases. The most common colonoscopic finding was multiple variable-sized punched-out ulcers with slightly elevated margin, as found in five patients. In two patients, only several oedematous hyperaemic mucosal patches with haemorrhagic spots or shallow erosions were seen. Active bleeding was noticed only in one patient, who received endoscopic haemostasis twice. The remaining six patients were treated by conservative treatment including antibiotic therapy. There was no complication during or after the colonoscopic examination. Conclusions: Intestinal bleeding in typhoid fever usually occurs from the ulcers in the ileum or proximal colon, and the most common colonoscopic manifestations are multiple variable-sized punched-out ulcerations.
Secondary Hemophagocytic Lymphohistiocytosis Due to Typhoid Fever

Abstract:Hemophagocytic lymphohistiocytosis (HLH) is a potentially fatal hyper-inflammatory state that is caused by a highly activated but ineffective immune system. It can be primary or secondary to triggers like infections, malignancies, and autoimmune conditions. The authors present the case of a young male with a fever and abdominal pain due to typhoid. He continued to have a high-spiking fever and developed dyspnea, requiring oxygen therapy despite being treated with appropriate antibiotics. Laboratory evaluation revealed cytopenias and deranged liver function tests, and abdominal imaging revealed hepatosplenomegaly. These clinical and laboratory findings raised suspicion of HLH secondary to typhoid fever. Further investigations were suggestive of hyperferritinemia and hypofibrinogenemia, and bone marrow aspirates showed hemophagocytes. The patient was treated with immunosuppression (dexamethasone) and antibiotics and showed remarkable recovery. Hemophagocytic lymphohistiocytosis should be suspected in patients with tropical infections like enteric fever, tuberculosis, malaria, dengue, etc. that worsen despite appropriate treatment, as late diagnosis is associated with greater mortality.

Abstract:Objective: Benign histiocytic proliferation in effusion specimens can be found in a variety of diseases such as rheumatoid arthritis, systemic lupus erythematosus, microorganism infections, trauma, reactive eosinophilic pleuritis, and others. In addition, nodular histiocytic/mesothelial hyperplasia is another well-recognized rare cause. The previous studies have shown that proliferative histiocytes have raisinoid nuclei and abundant eosinophilic granular cytoplasm and can be confused with malignant lesions, especially metastatic carcinomas. In this study, we evaluated the cytomorphology of benign histiocytes, discussed the diagnosis and differential diagnosis, and the clinical significance of histiocytic signet ring cells in effusion cytology. Material and Methods: Seven hundred and fifty-five benign effusion cases (433 pleural effusions and 322 abdominal fluids) were found over 1 year. Among benign cases, 35 cases (28 pleural effusions and seven abdominal fluids) were included with findings of dominantly histiocytic signet ring cell morphology as well as immunohistochemical (IHC) stains. The clinical findings were also correlated. Results: In contrast to the well-documented cytomorphology of raisinoid nuclei and eosinophilic cytoplasm of proliferative histiocytes in previous studies, we find that these cells predominately presented as signet ring cell morphology with clear cytoplasm. The most characteristic findings of benign histiocytes in pleural effusions are: (1) cells are arranged in sheets and/or scattered individual cells, but no two- or three-dimensional cell clusters; (2) cells are intermediate in size and with normal N/C ratio; (3) cells have eccentric located nuclei and abundant clear cytoplasm, giving signet ring cell appearance; (4) nuclei have fine granular chromatin pattern, no hyperchromia or coarse chromatin pattern, no nuclear atypia; and (5) immunohistochemical (IHC) stains demonstrate a strongly positivity for macrophage-histiocyte lineage marker CD68, but negativity for epithelial markers and mesothelial markers. Clinically, these patients do not demonstrate nodularity or lesions in the mesothelial lining of serous cavities. Conclusion: Our study provides a detailed characterization of benign histiocytic signet ring cells in effusion cytology. The differential diagnosis of histiocytic signet ring cells is broad. The most important differential diagnoses are metastatic adenocarcinoma and epithelioid signet ring cell mesothelioma. The accurate diagnosis is critical for the appropriate clinical management of the patient. Cytopathologists should be aware of the diagnostic pitfalls of benign histiocytic signet ring cells in effusion samples in daily practice.

Abstract:A 70-year-old previously healthy man presented with fever (highest recorded temperature, 39 °C) and extreme fatigue that had persisted for 2 months. The patient had a slight cough and reported weight loss of 15 kg, but denied the presence of any other symptoms. On initial clinical evaluation, the patient was febrile (temperature, 38.5 °C) and his vital signs were within normal limits. Laboratory investigations revealed progressive pancytopenia, with a white blood cell count of 1.8 × 109/L, hemoglobin of 62 g/L, and a platelet count of 27 × 109/L. The erythrocyte sedimentation rate (56 mm/h) and the level of C-reactive protein (73 mg/L) were elevated. Liver and kidney functions were within the normal range. Blood cultures were negative for aerobic and anaerobic bacteria, mycobacteria, and fungi. Chest computed tomography scanning showed several inflammatory lesions. Epigenetic regulation in epithelial cells and innate lymphocyte responses to S. Typhi infection: insights into IFN-γ production and intestinal immunity

Abstract:Infection by Salmonella enterica serovar Typhi (S. Typhi), the cause of enteric fevers, is low in high-income countries but persistent in low- and middle-income countries, resulting in 65,400-187,700 deaths yearly. Drug resistance, including in the United States, exacerbates this issue. Evidence indicates that innate lymphocytes (INLs), such as natural killer (NK) cells, and unconventional T lymphocytes (e.g., Mucosal-associated invariant T (MAIT) cells and T-cell receptor gamma delta (TCR-γδ) cells) can impact the intestinal epithelial barrier, the primary site of exposure to S. Typhi. Moreover, INL production of IFN-γ is central in controlling S. Typhi infection. However, the impact of epithelial cells (EC) on the secretion of IFN-γ by INLs and the relationship between these events and epigenetic changes remains unknown. Epigenetic modifications in host cells are fundamental for their differentiation and function, including IFN-γ production. Herein, using a human organoid-derived polarized intestinal epithelial cell monolayer, we investigated the role of H3K4me3 and H3K27me3 epigenetic marks in intestinal immunity, focusing on the function of EC, NK, MAIT, and TCR-γδ cells in response to S. Typhi. This study builds on our previous findings that MAIT subsets exhibiting specific IFN-γ pattern signatures were associated with protection against typhoid fever and that S. Typhi infection regulates changes in chromatin marks that depend on individual cell subsets. Here, we show that cultures exposed to S. Typhi without EC exhibit a significant increase in NK and MAIT cells, and, to a lesser extent, TCR-γδ cells, expressing IFN-γ and H3K4me3 but not H3K27me3 marks, contrasting with cultures where EC is present. The influence of EC on INL H3K4me3 marks might be indirectly mediated through the modulation of IL-18 secretion via the Histone Deacetylase 6 gene during S. Typhi infection.

Abstract:A 48-year-old male was admitted to Peking Union Medical College Hospital presented with intermittent fever for two years. The maximum body temperature was 39 ℃, and could spontaneously relieve. The efficacy of antibacterial treatment was poor. He had no other symptoms and positive signs. He had a significant weight loss, and the serum lactate dehydrogenase increased significantly. It was highly alert to be lymphoma, but bone marrow smear and pathology, and PET-CT had not shown obvious abnormalities. Considering high inflammatory indicators, increased ferritin and large spleen, the patient had high inflammatory status, and was treated with methylprednisolone. Then the patient's body temperature was normal, but the platelet decreased to 33×109/L. During hospitalization, he had suddenly hemoperitoneum and hemorrhagic shock. He was found spontaneous spleen rupture without obvious triggers, and underwent emergency splenectomy. The pathological diagnosis of spleen was diffuse large B-cell lymphoma.

Abstract:Salmonella enterica serovar Typhi ( S . Typhi), a human-restricted pathogen, invades the host through the gut to cause typhoid fever. Recent calculations of the typhoid fever burden estimated that more than 10 million new typhoid fever cases occur in low and middle-income countries, resulting in 65,400-187,700 deaths yearly. Interestingly, if not antibiotic-treated, upon the resolution of acute disease, 1%-5% of patients become asymptomatic chronic carriers. Chronically infected hosts are not only critical reservoirs of infection that transmit the disease to naive individuals but are also predisposed to developing gallbladder carcinoma. Nevertheless, the molecular mechanisms involved in the early interactions between gallbladder epithelial cells and S . Typhi remain largely unknown. Based on our previous studies showing that closely related S . Typhi strains elicit distinct innate immune responses, we hypothesized that host molecular pathways activated by S . Typhi strains derived from acutely and chronically infected patients would differ. To test this hypothesis, we used a novel human organoid-derived polarized gallbladder monolayer model, and S . Typhi strains derived from acutely and chronically infected patients. We found that S . Typhi strains derived from acutely and chronically infected patients differentially regulate host mitogen-activated protein kinase (MAPK) and S6 transcription factors. These variations might be attributed to differential cytokine signaling, predominantly via TNF-α and IL-6 production and appear to be influenced by the duration the isolate was subjected to selective pressures in the gallbladder. These findings represent a significant leap in understanding the complexities behind chronic S . Typhi infections in the gallbladder and may uncover potential intervention targets.

Abstract:OBJECTIVE:To investigate the T lymphocyte subgroups and the levels of serum zinc (Zn), selenium (Se), iron (Fe), copper (Cu) in patients with diarrhea type of irritable bowel syndrome (D-IBS). METHODS:A total of 30 D-IBS patients and 30 control subjects were enrolled in this study, and their peripheral blood samples were collected. The percentage of peripheral CD3, CD4, CD8 T lymphocytes were analyzed by flow cytometry, and the ratio of CD4/CD8 was calculated. Serum Zn, Fe and Cu levels were determined by atomic absorption spectrometry(AAS), and the Se level by atomic fluorometry. RESULT:Compared with control group,the percentage of CD4 T lymphocyte and the ratio of CD4/CD8 in D-IBS group were significantly lower (P<0.01). However, there was no significant difference in serum Zn, Se, Fe, Cu levels between two groups (P>0.05). CONCLUSION:The declines of peripheral blood CD4 T lymphocytes and the ratio of CD4/CD8 may suggest a cellular immune abnormality in D-IBS patients. There was no significant difference in trace elements levels between the two groups.
Changes in Intestinal Microflora in Patients with Chronic Severe Hepatitis

Abstract:OBJECTIVE:To investigate changes in intestinal microflora in patients with chronic severe hepatitis (CSH), and their role in this life-threatening disease.METHODS:We classified nineteen patients with chronic severe hepatitis as the CSH group, thirty patients with chronic hepatitis (CH) as the CH group and thirty-one healthy volunteer as the control group. Fecal flora from all subjects were analyzed. Concentrations of plasma endotoxin, serum cytokines tumor necrosis factor alpha (TNF-alpha) and interleukin-1 beta (IL-1 beta) and liver function were assessed.RESULTS:The number of fecal bifidobacterium (P < 0.001, P < 0.05 respectively), as well as bacteroidaceae (P < 0.001, P < 0.01 respectively) were significantly deceased in patients with chronic severe hepatitis compared with the CH and control groups, while the number of enterobacteriaceae (P < 0.001, P < 0.05 respectively) and yeasts (P < 0.01, P < 0.05 respectively) were significantly increased. Levels of plasma endotoxin, serum TNF-alpha, IL-1 beta and total bilirubin (TBiL) were significantly increased in the CSH group. The concentration of endotoxin positively correlated with levels of both TNF-alpha, IL-1 beta and TBiL (P < 0.001, respectively). Levels of plasma endotoxin were positively correlated with the number of fecal enterobacteriaceae and negatively correlated with bifidobacterium (P < 0.05, P < 0.001, respectively).CONCLUSION:Intestinal flora in patients with chronic severe hepatitis were severely disturbed and gut microbiological colonization resistance was impaired. Changes in intestinal flora may have a pivotal role in both the elevation of plasma endotoxin and further hepatic lesions resulting in liver failure.
Flea-Borne Typhus Presenting with Acalculous Cholecystitis and Severe Anemia

Abstract:Background: Flea-borne typhus (FBT), an uncommon illness in the United States, typically presents as a high continuous fever with commonly associated symptoms including headache, myalgias, and rashes on the trunk and extremities. Patients infected with FBT may also present with atypical symptoms. As such, the combination of its relatively low incidence in the United States coupled with its variability in associated symptoms poses a diagnostic challenge for clinicians; early empiric treatment with doxycycline is warranted prior to a definitive diagnosis to reduce the risk of damage to vital organs. Case Report. This case describes a 54-year-old male who presented to an emergency room in Houston, Texas, with one week of constant right upper quadrant abdominal pain and fevers up to 40°C. The patient was initially diagnosed with Grade III severe acute cholangitis after abdominal ultrasound revealed gallbladder sludge and wall thickening without ductal dilatation, but a subsequent endoscopic retrograde cholangiopancreatography was unremarkable. Following intermittent fevers and worsening anemia, the patient was started on oral doxycycline for atypical infection, and an infectious disease workup subsequently returned a positive titer for Rickettsia typhi. He experienced rapid symptomatic and clinical improvement, and the patient was discharged home with a final diagnosis of flea-borne typhus. Conclusion: Albeit uncommon, the presentation of this patient's symptoms and final diagnosis of flea-borne typhus demonstrates the importance of (1) keeping atypical infections such as FBT in the differential diagnosis and (2) beginning empiric treatment to prevent damage to vital organs if suspicion of FBT is high.

Abstract:Typhoid fever is a systemic infection caused by Salmonella enterica serovar Typhi (S. Typhi) bacteria invading through the gut lumen. Transmission occurs through ingestion of contaminated food and water, particularly in settings with poor water, sanitation and hygiene infrastructure, resulting in over 10 million illnesses annually. It is well established that vaccines and natural infections can stimulate protective immunity against S. Typhi. As the pathogen invades through the gastrointestinal tract, it is plausible that the gut microbiome may also influence the outcome of S. Typhi exposure. There is some evidence that bacteria producing short-chain fatty acids (SCFAs) may create an environment unfavourable to invasive Salmonella, but data from humans is very limited. To investigate the association between gut microbiome and typhoid fever, we analysed samples collected from three cohorts recruited in a prospective surveillance study conducted in parallel in three settings where typhoid fever is endemic (Dhaka, Bangladesh; Blantyre, Malawi; and Kathmandu, Nepal). Cohorts consisted of acute typhoid fever patients (n=92), asymptomatic household contacts of typhoid fever patients (representing individuals who were likely exposed to S. Typhi but did not develop disease, n=97), and asymptomatic serosurvey participants with high Vi antibody titres (representing individuals who were exposed to S. Typhi and may be carriers, n=69). The stool microbiomes of each cohort were characterised using shotgun metagenomics, and bacterial composition and diversity were compared. We identified four bacterial species and 28 metabolic gene clusters (MGCs) that were significantly lower in abundance in typhoid fever patients compared with household contacts (i.e. probably exposed), in two of the three host populations (Bangladesh and Malawi). These may represent taxa that provide protection against development of clinical infection upon exposure to S. Typhi, and include the inflammation-associated species Prevotella copri clade A and Haemophilus parainfluenzae, and seven MGCs involved in SCFA metabolism. The putative protection provided by SCFA metabolism was supported by data from a controlled human infection model conducted in a UK population, in which participants who did not develop typhoid fever following ingestion of S. Typhi had significantly higher abundance of a putative SCFA-metabolising MGC (q-value = 0.22). This study identified recurring associations between taxonomic and functional microbiota characteristics and non-susceptibility to typhoid fever across multiple cohorts. Future research should explore the potential functional role of SCFAs and inflammation-associated bacteria in resistance to S. Typhi and other enteric infections.
Acute Abdominal Pain and Rapidly Accumulating Ascites as an Unusual Presentation of Salmonella Typhi: A Case Report

Abstract:Salmonella typhi (S. typhi) infections typically present with fever and gastrointestinal symptoms. This case report on S. typhi enteritis documents atypical clinical, radiological, and endoscopic findings raising diagnostic challenges. A 31-year-old male in the Kingdom of Saudi Arabia (KSA) presented with severe abdominal pain, vomiting, bloody diarrhea, and no fever. Initial diagnosis included amebiasis and other gastroenteritis infections. Despite treatment with ciprofloxacin and metronidazole, the patient's condition did not improve, and he kept having intractable abdominal pain and vomiting. Subsequent investigations, including abdominal ultrasound and esophagogastroduodenoscopy, revealed extensive and rapidly progressive intestinal inflammation with wall thickening and ascites. Stool culture eventually identified a multidrug-resistant strain of S. typhi, sensitive only to ceftriaxone. Treatment with ceftriaxone and continuous infusion of proton pump inhibitor (PPI) led to significant improvement. The absence of fever in the context of bloody diarrhea, and the rapid development of ascites not improving with first-line treatment of gastroenteritis, led to the search for other diagnoses such as inflammatory bowel syndromes or tuberculosis. The presentation of diffuse intestinal wall thickening with intractable vomiting, bloody diarrhea, and progressively increasing ascites is not frequently encountered with S. typhi. The case also underscores the growing concern of antibiotic-resistant S. typhi strains. The patient's response to targeted antibiotic therapy emphasizes the importance of accurate microbial identification and susceptibility testing in managing infectious diseases. This case report illustrates an atypical presentation of S. typhi enteritis with progressively increasing ascites and increased intestinal wall thickening. The uncommon complicated clinical picture led to challenges in diagnosis and management. It emphasizes the need for high clinical suspicion and comprehensive diagnostic approaches in atypical cases of common infections, especially in the context of increasing antibiotic resistance.

Abstract:Salmonella enterica serovar Typhimurium, a Gram‐negative pathogen, causes human gastroenterocolitis. In this study, we examined the role of flagellin in the apoptosis of Peyer's patch (PP) cells during Salmonellosis. Apoptosis is a crucial mechanism that eliminates cells without releasing noxious components. We used both enterocolitis and systemic typhoid fever models in C57BL/6 mice, with various isogenic Salmonella strains. In the enterocolitis model, an aflagellate C−B− mutant increased apoptosis of PP CD4+ T cells at 12 h post‐infection compared to the wild‐type strain, whereas the aflagellate mutant slightly decreased cell death at 24 h. In the typhoid model, the aflagellate mutant modulated CD4+ T cell apoptosis differently in PP and spleen on day 4 post‐infection. As ICAM‐1 is a costimulatory molecule involved in T cell activation, we examined whether the expression of this adhesion molecule was modified in the absence of flagellin. Interestingly, the aflagellate mutant modified ICAM‐1 expression on macrophages only in PPs but not in mesenteric lymph nodes (MLN). We also observed that the absence of flagellin elicits an increase of macrophages in PPs and MLN as well as an increased bacterial load in these organs. In conclusion, flagellin seems to be involved in the apoptosis of CD4+ T cells in several organs. However, its role appears to depend on the model, the organ and the stage of the infection.

Abstract:Typhoid fever caused by infection with Salmonella enterica subspecies enterica serotype Typhi (S. Typhi), an important public health problem in many low- and middle-income countries, is transmitted by ingestion of water or food contaminated by feces or urine from individuals with acute or chronic S. Typhi infection. Most chronic S. Typhi carriers (shedding for ≥12 months) harbor infection in their gallbladder wherein preexisting pathologies, particularly cholelithiasis, provide an environment that fosters persistence. Much less appreciated is the existence of non-gallbladder hepatobiliary chronic S. Typhi carriers and urinary carriers. The former includes parasitic liver flukes as a chronic carriage risk factor. Chronic urinary carriers typically have pathology of their urinary tract, with or without renal or bladder stones. Even as the prevalence of multidrug-resistant and extensively drug-resistant S. Typhi strains is rising, global implementation of highly effective typhoid vaccines is increasing. There is also renewed interest in identifying, monitoring, and (where possible) treating chronic carriers who comprise the long-term reservoir of S. Typhi.

Abstract:INTRODUCTION: Chronic diarrhea is a common clinical presentation in GI practice and numerous causes need to be considered during the evaluation including infectious, inflammatory, and metabolic or functional bowel disorders. CASE DESCRIPTION/METHODS: A 58-year old white, obese male patient was admitted because of 2-year history of recurrent episodes of non-bloody, watery diarrhea with up to 12 daily bowel movements associated with crampy abdominal pain. He noted nausea and vomiting with a 30-pound weight loss but denied dysphagia, odynophagia, and GERD symptoms. There was no sick contact and no travel. His past medical history was significant for diarrhea-predominant irritable bowel syndrome (IBS), chronic pancreatitis, and seasonal allergies. He lived alone with several dogs, cats, and rabbits. Two years ago, EGD and colonoscopy showed unspecific duodenitis and gastritis, and 2 colonic adenomas were removed. Persistent peripheral eosinophilia of 15-25% ( > 1,500 Eosinophils/mm3) was noted for the previous 2 years. The physical examination was unremarkable. Tests for helminths, parasites, and myeloid neoplasms remained negative. Colonoscopy was performed and showed normal mucosa. Congested duodenal mucosa and mild gastric erythema were noted during EGD (Figure 1). Dense eosinophilic infiltrates along the entire length of the digestive tract were noted without evidence of pathological organisms, celiac disease, and microscopic colitis (Figure 2). Abundant mast cells were identified in corresponding segments of the GI tract (Figure 3). Tryptase levels were normal, IgE was > 10,000 IU/ml (nl 0-100), and skin testing was positive to cat standard, ragweed, and rye, but negative for food allergies. RAST showed positive results with dog and cat dander and dairy. The patient was advised to exclude dairy products from his diet. In follow up, his symptoms improved, and eosinophilia was no longer detectable. DISCUSSION: Episodic chronic watery diarrhea represented a diagnostic challenge in this case because of underlying IBS-D and chronic pancreatitis. Furthermore, the persistent moderate eosinophilia and the highly unusual intense eosinophilic inflammation of the entire intestinal mucosa resulted in comprehensive testing to exclude helminthic and parasitic infections and hematologic malignancies. Resolution of symptoms and peripheral eosinophilia upon exclusion of dairy suggested that concurrent eosinophilic esophagitis, gastroenteritis, and colitis were due to an allergic reaction to dairy.Figure 1.: Representative endoscopic images of the gastrointestinal tract.Figure 2.: Representative photomicrographs (hematoxylin-eosin stain) of histopathologic changes with unusually dense eosinophilic infiltrate in the lamina propria, epithelium, and muscularis.
