Abstract:The U-shaped association between health outcomes and ambient temperatures has been extensively investigated. However, such analyses cannot fully estimate the mortality burden of climate change because the features of the association (e.g., minimum mortality temperature) vary with human adaptation; thus, they are not generalizable to different locations. In this study, we assumed that humans could adapt to regular temperature variations; and thus examined the all-cause mortality attributable to temperature anomaly (TA), an indicator widely utilized in climate science to measure irregular temperature fluctuations, across 115 cities in the United States (US). We first used quasi-Poisson regressions to obtain the city-specific TA-mortality associations, then used meta-regression to pool these city-specific estimates. Finally, we calculated the number of TA-related deaths using the uniform pooled association, then compared it to the estimates from city-specific associations, which had been controlled for adaptation. Meta-regression showed a U-shaped TA-mortality association, centered at a TA near 0. According to the pooled association, 0.579 % (95 % confidence interval [CI]: 0.465–0.681 %), 0.394 % (95 % CI: 0.332–0.451 %), and 0.185 % (95 % CI: 0.107–0.254 %) of all-cause deaths were attributable to all anomalous temperatures (TA ≠ 0), anomalous heat (TA > 0), and anomalous cold (TA < 0), respectively. At the city level, heat-related deaths estimated from the pooled association were in good agreement with heat-related deaths estimated from the city-specific associations (R2 = 0.84). However, the cold-related deaths estimated from the two methods showed a weaker correlation (R2 = 0.07). Our findings suggest that TA constitutes a generalizable indicator that can uniformly evaluate deaths attributable to anomalous heat in distinct geographical locations.

Abstract:The increasing use of time-series analyses in exploring the relationship between daily ambient temperature and mortality has expanded our understanding of the potential health impacts of climate change. However, it raises significant concerns about the risk of overinterpretation and misattribution of statistical findings. This review examines the methodological assumptions and interpretation pitfalls prevalent in current research on ambient temperature-mortality associations. Extremely elevated ambient temperatures are well-known to elicit physiological stress and increase mortality risk; however, there is no physiological evidence for lethality risk within normal ambient temperature ranges. Despite this, many studies attribute mortality risks across the entire ambient temperature-mortality curve, including normal range ambient temperatures, thus oversimplifying complex underlying physiological processes. Overinterpretation may lead to inaccurate assessments and misguided public health policies. We caution against the tendency to extrapolate results from extreme heat conditions to milder, more typical summer ambient temperature ranges. We advocate for an interdisciplinary approach that combines physiological, clinical, and epidemiological perspectives, with a strong emphasis on the role of behavioral thermoregulation and socio-economic factors to link normal range ambient temperatures with mortality. We recommend analyses centered on excess mortality during defined heatwave periods, and to incorporate heat stress biomarkers to substantiate causal claims for temperatures below heatwaves threshold. A careful approach to interpreting ambient temperature-mortality associations is crucial for formulating evidence-based public health policies.

Abstract:The rapid growth of heatwaves' severity have increasingly endangered citizens' health in the last decade. Evidence points to the environmental injustice of heatwaves: inequal heatwave exposure among socioeconomic groups. Failing to use an adequate indicator of thermal comfort at a large scale, the previous studies have not adequately scrutinized the environmental justice of heatwaves and their variations across a large-scale territory. This study is novel in an unprecedented analysis of psychological equivalent temperature (PET), a comprehensive measure of thermal comfort, across socioeconomic groups and the urban-rural gradient of the Netherlands, as a proxy for factors affecting heatwave vulnerability. The results show that heatwave inequality (measured by the Gini coefficient) is higher in less urbanized areas. It shows that the population aged 25–44, immigrants, tenants, and females are the most heat-exposed groups across all levels of urbanization. However, the population aged 25–44 is more likely to be overexposed in urbanized areas, and immigrants are more likely to be overexposed in rural areas. The results open discussion on the necessity of location-specific policies protecting the most heat-exposed groups in different areas. It also paves the way for future studies using broader PET simulations and expanding their scope to include citizens' daily movements.

Abstract:Anthropogenic climate change has resulted in a significant rise in extreme heat events, exerting considerable but unequal impacts on morbidity and mortality. Numerous studies have identified inequities in heat exposure across different groups, but social identities have often been viewed in isolation from each other. Children (5 and under) and older adults (65 and older) also face elevated risks of heat-related health impacts. We employ an intersectional cross-classificatory approach to analyze the distribution of heat exposure between sociodemographic categories split into age groups in the contiguous US. We utilize high-resolution daily air temperature data to establish three census tract-level heat metrics (i.e., average summer temperature, heat waves, and heat island days). We pair those metrics with American Community Survey estimates on racial/ethnic, socioeconomic, and disability status by age to calculate population weighted mean exposures and absolute disparity metrics. Our findings indicate few substantive differences between age groups overall, but more substantial differences between sociodemographic categories within age groups, with children and older adults from socially marginalized backgrounds facing greater exposure than adults from similar backgrounds. When looking at sociodemographic differences by age, people of color of any age and older adults without health insurance emerge as the most exposed groups. This study identifies groups who are most exposed to extreme heat. Policy and program interventions aimed at reducing the impacts of heat should take these disparities in exposure into account to achieve health equity objectives.

Abstract:Every year, high temperatures send people to the hospital and morgue, and the combination of climate change and urbanization will increase extreme heat exposure. Cities are searching for ways to determine the most affected areas to begin addressing this pervasive issue. While we are living through the "big data" revolution, policy makers are still uncertain about what level of data is most useful. We evaluate the data loss from using data at different spatial resolutions to evaluate heat vulnerability, as both the definition of intra-urban heat and the resolution of the data affect the area identified and targeted for mitigation. Variance-based metrics provide many advantages, but when data is aggregated, these metrics are less able to represent the full range of urban heat. Using the case of Bexar County (home to San Antonio, TX), we find that increasing data aggregation increases both false positive and false negative identification of intra-urban heat islands, leading to unreliable results. Misclassification increases as aggregation increases, indicating that decisions should be made at the finest spatial resolution possible.

Abstract:Cities worldwide are experiencing record-breaking summer temperatures. Urban environments exacerbate extreme heat, resulting in not only the urban heat island but also intracity variations in heat exposure. Understanding these disparities is crucial to support equitable climate mitigation and adaptation efforts. We found persistent negative correlations between daytime land surface temperature (LST) and median household income across the Los Angeles metropolitan area based on Ecosystem Spaceborne Thermal Radiometer Experiment on Space Station observations from 2018 to 2021. Lower evapotranspiration resulting from the unequal distribution of vegetation cover is a major factor leading to higher LST in low-income neighborhoods. Disparities worsen with higher regional mean surface temperature, with a $10,000 decrease in income leading to ~0.2°C LST increase at 20°C and up to ~0.7°C at 45°C. With more frequent and intense heat waves projected in the future, equitable mitigation measures, such as increasing surface albedo and tree cover in low-income neighborhoods, are necessary to address these disparities.

Abstract:Extreme heat events, a leading cause of weather-related fatality worldwide, are expected to intensify, last longer, and occur more frequently in the near future. In heat health risk assessments, a spatiotemporal mismatch usually exists between hazard (heat stress) data and exposure (population distribution) data. Such mismatch is present because demographic data are generally updated every couple of years and unavailable at the subcensus unit level, which hinders the ability to diagnose human risks. In the present work, a human settlement index based on multisensor remote sensing data, including nighttime light, vegetation index, and digital elevation model data, was used for heat exposure assessment on a per-pixel basis. Moreover, the nighttime urban heat island effect was considered in heat hazard assessment. The heat-related health risk was spatially explicitly assessed and mapped at the 250 m × 250 m pixel level across Zhejiang Province in eastern China. The results showed that the accumulated heat risk estimates and the heat-related deaths were significantly correlated at the county level (Spearman's correlation coefficient = 0.76, P ≤ 0.01). Our analysis introduced a spatially specific methodology for the risk mapping of heat-related health outcomes, which is useful for decision support in preparation and mitigation of heat-related risk and potential adaptation.

Abstract:Climate change is causing more frequent and severe extreme heat events (EHEs) in Canada, resulting in significant loss of life. However, patterns across mortality reporting for historical EHEs have not been analyzed. To address this gap, we studied deaths in Canadian EHEs from 1936 to 2021, identifying trends and challenges. Our analysis revealed inconsistencies in mortality data, discrepancies between vulnerable populations identified, difficulties in determining the cause of death, and inconsistent reporting on social vulnerability indicators. We provide some observations that could help inform solutions to address the gaps and challenges, by moving toward more consistent and comprehensive reporting to ensure no population is overlooked. Accurately accounting for affected populations could help better target evidence‐based interventions, and reduce vulnerability to extreme heat.

Abstract:Background: Few studies have examined variation in the associations between heat waves and mortality in an international context. Objectives: We aimed to systematically examine the impacts of heat waves on mortality with lag effects internationally. Methods: We collected daily data of temperature and mortality from 400 communities in 18 countries/regions and defined 12 types of heat waves by combining community-specific daily mean temperature ≥90th, 92.5th, 95th, and 97.5th percentiles of temperature with duration ≥2, 3, and 4 d. We used time-series analyses to estimate the community-specific heat wave-mortality relation over lags of 0-10 d. Then, we applied meta-analysis to pool heat wave effects at the country level for cumulative and lag effects for each type of heat wave definition. Results: Heat waves of all definitions had significant cumulative associations with mortality in all countries, but varied by community. The higher the temperature threshold used to define heat waves, the higher heat wave associations on mortality. However, heat wave duration did not modify the impacts. The association between heat waves and mortality appeared acutely and lasted for 3 and 4 d. Heat waves had higher associations with mortality in moderate cold and moderate hot areas than cold and hot areas. There were no added effects of heat waves on mortality in all countries/regions, except for Brazil, Moldova, and Taiwan. Heat waves defined by daily mean and maximum temperatures produced similar heat wave-mortality associations, but not daily minimum temperature. Conclusions: Results indicate that high temperatures create a substantial health burden, and effects of high temperatures over consecutive days are similar to what would be experienced if high temperature days occurred independently. People living in moderate cold and moderate hot areas are more sensitive to heat waves than those living in cold and hot areas. Abstract:Abstract Worsening temperature extremes are among the most severe impacts of human-induced climate change. These extremes are often defined as rare events that exceed a specific percentile threshold within the distribution of daily maximum temperature. The percentile-based approach is chosen to follow regional and seasonal temperature variations so that extremes can occur globally and in all seasons, and frequently uses a running seasonal window to increase the sample size for the threshold calculation. Here, we show that running seasonal windows as used in many studies in recent years introduce a time-, region-, and dataset-depended bias that can lead to a striking underestimation of the expected extreme frequency. We reveal that this bias arises from artificially mixing the mean seasonal cycle into the extreme threshold and propose a simple solution that essentially eliminates it. We then use the corrected extreme frequency as reference to show that the bias also leads to an overestimation of future heatwave changes by as much as 30% in some regions. Based on these results we stress that running seasonal windows should not be used without correction for estimating extremes and their impacts.

Abstract:Many studies have explored the impact of extreme heat on health, but few have investigated localized heat-health outcomes across a wide area. We examined fine-scale variability in vulnerable areas, considering population distribution, local weather, and landscape characteristics. Using 36 different heat event definitions, we identified the most dangerous types of heat events based on minimum, maximum, and diurnal temperatures with varying thresholds and durations. Focusing on California's diverse climate, elevation, and population distribution, we analyzed hospital admissions for various causes of admission (2004-2013). Our matching approach identified vulnerable zip codes, even with small populations, on absolute and relative scales. Bayesian Hierarchical models leveraged spatial correlation. We ranked the 36 heat event types by attributable hospital admissions per zip code and provided code, simulated data, and an interactive web app for reproducibility. Our findings showed high variation in heat-related hospitalizations in coastal cities and substantial heat burdens in the Central Valley. Diurnal heat events had the greatest impact in the Central Valley, while nighttime extreme heat events drove burdens in the southeastern desert. This spatially informed approach guides local policies, prioritizing dangerous heat events to reduce the heat-health burden. The methodology is applicable to other regions, informing early warning systems and characterizing extreme heat impacts.
Premature Deaths Due To Heat Exposure: The Potential Effects of Neighborhood‐Level Versus City‐Level Acclimatization Within US Cities

Abstract:For the population of a given US city, the risk of premature death associated with heat exposure increases as temperatures rise, but risks in hotter cities are generally lower than in cooler cities at equivalent temperatures due to factors such as acclimatization. Those living in especially hot neighborhoods within cities might therefore suffer much more than average if such adaptation is only at the city‐wide level, whereas they might not experience greatly increased risk if adjustment is at the neighborhood level. To compare these possibilities, we use high spatial resolution temperature data to evaluated heat‐related deaths assuming either adjustment at the city‐wide or at the neighborhood scale in 10 large US cities. On average, we find that if inhabitants are adjusted to their local conditions, a neighborhood that was 10°C hotter than a cooler one would experience only about 1.0–1.5 excess heat deaths per year per 100,000 persons. By contrast, if inhabitants are acclimatized to city‐wide temperatures, the hotter neighborhood would experience about 15 excess deaths per year per 100,000 persons. Using idealized analyses, we demonstrate that current city‐wide epidemiological data do not differentiate between these differing adjustments. Given the very large effects of assumptions about neighborhood‐level acclimatization found here, as well as the fact that current literature is conflicting on the spatial scale of acclimatization, more neighborhood‐level epidemiological data are urgently needed to determine the health impacts of variations in heat exposure within urban areas, better constrain projected changes, and inform mitigation efforts.

Abstract:Abstract. Towards the end of June 2021, temperature records were broken by several degrees Celsius in several cities in the Pacific northwest areas of the U.S. and Canada, leading to spikes in sudden deaths, and sharp increases in hospital visits for heat-related illnesses and emergency calls. Here we present a multi-model, multi-method attribution analysis to investigate to what extent human-induced climate change has influenced the probability and intensity of extreme heatwaves in this region. Based on observations and modeling, the occurrence of a heatwave with maximum daily temperatures (TXx) as observed in the area 45° N–52° N, 119° W–123° W, was found to be virtually impossible without human-caused climate change. The observed temperatures were so extreme that they lie far outside the range of historically observed temperatures. This makes it hard to quantify with confidence how rare the event was. In the most realistic statistical analysis, which uses the assumption that the heatwave was a very low probability event that was not caused by new nonlinearities, the event is estimated to be about a 1 in 1000 year event in today’s climate. With this assumption and combining the results from the analysis of climate models and weather observations, an event, defined as daily maximum temperatures (TXx) in the heatwave region, as rare as 1 in a 1000 years would have been at least 150 times rarer without human-induced climate change. Also, this heatwave was about 2 °C hotter than a 1 in 1000-year heatwave that at the beginning of the industrial revolution would have been (when global mean temperatures were 1.2 °C cooler than today). Looking into the future, in a world with 2 °C of global warming (0.8 °C warmer than today), a 1000-year event would be another degree hotter. It would occur roughly every 5 to 10 years in such global warming conditions. Our results provide a strong warning: our rapidly warming climate is bringing us into uncharted territory with significant consequences for health, well-being, and livelihoods. Adaptation and mitigation are urgently needed to prepare societies for a very different future.

Abstract:Using administrative criminal records from Texas, we show how high temperatures affect the decision-making of police officers, prosecutors, and judges. We find that police reduce the number of arrests made per reported crime on the hottest days and that arrests made on these days are more likely to be dismissed in court. For prosecutors, high temperature on the day they announce criminal charges does not appear to affect the nature and severity of the charges. Judges, however, dismiss fewer cases, issue longer prison sentences, and levy higher fines when ruling on hot days. Our results suggest that the psychological and cognitive consequences of exposure to high temperatures have meaningful consequences for criminal defendants as they interact with the criminal justice system.
Effects of High Temperature on COVID‐19 Deaths in U.S. Counties

Abstract:The United States of America (USA) was afflicted by extreme heat in the summer of 2021 and some states experienced a record-hot or top-10 hottest summer. Meanwhile, the United States was also one of the countries impacted most by the coronavirus disease 2019 (COVID-19) pandemic. Growing numbers of studies have revealed that meteorological factors such as temperature may influence the number of confirmed COVID-19 cases and deaths. However, the associations between temperature and COVID-19 severity differ in various study areas and periods, especially in periods of high temperatures. Here we choose 119 US counties with large counts of COVID-19 deaths during the summer of 2021 to examine the relationship between COVID-19 deaths and temperature by applying a two-stage epidemiological analytical approach. We also calculate the years of life lost (YLL) owing to COVID-19 and the corresponding values attributable to high temperature exposure. The daily mean temperature is approximately positively correlated with COVID-19 deaths nationwide, with a relative risk of 1.108 (95% confidence interval: 1.046, 1.173) in the 90th percentile of the mean temperature distribution compared with the median temperature. In addition, 0.02 YLL per COVID-19 death attributable to high temperature are estimated at the national level, and distinct spatial variability from -0.10 to 0.08 years is observed in different states. Our results provide new evidence on the relationship between high temperature and COVID-19 deaths, which might help us to understand the underlying modulation of the COVID-19 pandemic by meteorological variables and to develop epidemic policy response strategies.

Abstract:Background and objectives: Extreme heat is an environmental health equity concern disproportionately affecting low-income older adults and people of color. Exposure factors, such as living in rental housing and lack of air conditioning, and sensitivity factors, such as chronic disease and social isolation, increase mortality risk among older adults. Older persons face multiple barriers to adaptive heat mitigation, particularly those living in historically temperate climates. This study measures two heat vulnerability indices to identify areas and individuals most vulnerable to extreme heat and discusses opportunities to mitigate vulnerability among older adults. Research design and methods: We constructed two heat vulnerability indices for the Portland, OR, metropolitan area: one using area scale proxy measures extracted from existing regional data and another at the individual scale using survey data collected following the 2021 Pacific Northwest Heat Dome event. These indices were analyzed using principal component analysis and Geographic Information Systems. Results: Results indicate that the spatial distribution of areas and individuals vulnerable to extreme heat are quite different. The only area found among the most vulnerable on both indices has the largest agglomeration of age- and income-restricted rental housing in the metropolitan area. Discussion and implications: Due to spatial variations in heat-related risk at the individual and area scales, measures addressing heat risk should not be spatially uniform. By focusing resources on older adult individuals and areas in particular need of assistance, heat risk management policies can be both highly efficient and cost effective.
Mortality in extreme heat events: an analysis of Los Angeles County Medical Examiner data

Abstract:Objectives: Climate change is intensifying heat events, and local governments are working to absorb and mitigate the associated costs. To develop effective responses, local data on the relationship between climate and health are crucial. This study investigates the impact of heat events on unexpected mortality, focusing on deaths investigated by the Medical Examiner in Los Angeles County. Study design: A retrospective observational study. Methods: We estimate the associations between the National Weather Service's HeatRisk index and deaths investigated by the Medical Examiner in Los Angeles County using negative binomial count models with controls for time trends and seasonality. In subgroup analyses, we explore how these effects vary for those who are homeless or living in care facilities. Results: Compared to days with no HeatRisk, days with moderate, major, or extreme HeatRisk were associated with death increases of 6.7% [CI: 1.9-11.7%], 15.3% [CI: 2.9-29.1%], and 65.5% [CI: 34.9-102.1%], respectively. Effects were more pronounced for individuals who were homeless or in care facilities. Major or extreme heat days were associated with a 59.3% [CI: 19.8-109.4%] increase in deaths among homeless individuals and a 91.4% [CI: 19.0-198.6%] increase in deaths among those in care facilities. Conclusions: Heat events have a significant impact on mortality investigated by the Medical Examiner, especially among vulnerable groups. Local governments may consider using the warning tools provided by the National Weather Service to focus their resources on the most intense heat events, especially to target those living in care facilities or who are homeless.
Heat, Health, and Habitats: Analyzing the Intersecting Risks of Climate and Demographic Shifts in Austrian Districts

Abstract:The impact of hot weather on health outcomes of a population is mediated by a variety of factors, including its age profile and local green infrastructure. The combination of warming due to climate change and demographic aging suggests that heat-related health outcomes will deteriorate in the coming decades. Here, we measure the relationship between weekly all-cause mortality and heat days in Austrian districts using a panel dataset covering $2015-2022$. An additional day reaching $30$ degrees is associated with a $2.4\%$ increase in mortality per $1000$ inhabitants during summer. This association is roughly doubled in districts with a two standard deviation above average share of the population over $65$. Using forecasts of hot days (RCP) and demographics in $2050$, we observe that districts will have elderly populations and hot days $2-5$ standard deviations above the current mean in just $25$ years. This predicts a drastic increase in heat-related mortality. At the same time, district green scores, measured using $10\times 10$ meter resolution satellite images of residential areas, significantly moderate the relationship between heat and mortality. Thus, although local policies likely cannot reverse warming or demographic trends, they can take measures to mediate the health consequences of these growing risks, which are highly heterogeneous across regions, even in Austria.
