Abstract:Extreme heat events, a leading cause of weather-related fatality worldwide, are expected to intensify, last longer, and occur more frequently in the near future. In heat health risk assessments, a spatiotemporal mismatch usually exists between hazard (heat stress) data and exposure (population distribution) data. Such mismatch is present because demographic data are generally updated every couple of years and unavailable at the subcensus unit level, which hinders the ability to diagnose human risks. In the present work, a human settlement index based on multisensor remote sensing data, including nighttime light, vegetation index, and digital elevation model data, was used for heat exposure assessment on a per-pixel basis. Moreover, the nighttime urban heat island effect was considered in heat hazard assessment. The heat-related health risk was spatially explicitly assessed and mapped at the 250 m × 250 m pixel level across Zhejiang Province in eastern China. The results showed that the accumulated heat risk estimates and the heat-related deaths were significantly correlated at the county level (Spearman's correlation coefficient = 0.76, P ≤ 0.01). Our analysis introduced a spatially specific methodology for the risk mapping of heat-related health outcomes, which is useful for decision support in preparation and mitigation of heat-related risk and potential adaptation.
Spatial variation in the joint effect of extreme heat events and ozone on respiratory hospitalizations in California

Abstract:Significance Projections show that extreme heat events and high ozone days are expected to increase under climate change. Both exposures are harmful to human health, but warning systems, typically implemented separately, do not consider the joint effects of these exposures. In this study, we propose a method to address spatial differences of the joint effects of extreme heat events and ozone peaks on respiratory hospitalizations at the zip code level in California. Zip codes with low median income and high unemployment rates are at increased risk for these joint effects. Results identified zip codes with increased vulnerability, demonstrating the importance of going beyond average measures to prioritize areas for joint warning systems and protect population health.

Abstract:Extreme heat events are more frequent and intense as a result of global climate change, thus posing tremendous threats to public health. However, extant literature exploring the multidimensional features of heat–health risks from a spatial perspective is limited. This study revisits extreme heat–health risk and decomposes this concept by integrating multi‐sourced datasets, identifying compositional features, examining spatial patterns, and comparing classified characteristics based on local conditions. Using Maryland as the focal point, we found that the components of heat–health risk are different from traditional risk dimensions (i.e., vulnerability, hazards, and exposure). Through a local‐level clustering analysis, heat–health risks were compared with areas having similar features, and among those with different features. The findings suggest a new perspective for understanding the socio‐environmental and socio‐spatial features of heat–health risks. They also offer an apt example of applying cross‐disciplinary methods and tools for investigating an ever‐changing phenomenon. Moreover, the spatial classification mechanism provides insights about the underlying causes of heat–health risk disparities and offers reference points for decision‐makers regarding identification of vulnerable areas, resource allocation, and causal inferences when planning for and managing extreme heat disasters.

Abstract:Every year, high temperatures send people to the hospital and morgue, and the combination of climate change and urbanization will increase extreme heat exposure. Cities are searching for ways to determine the most affected areas to begin addressing this pervasive issue. While we are living through the "big data" revolution, policy makers are still uncertain about what level of data is most useful. We evaluate the data loss from using data at different spatial resolutions to evaluate heat vulnerability, as both the definition of intra-urban heat and the resolution of the data affect the area identified and targeted for mitigation. Variance-based metrics provide many advantages, but when data is aggregated, these metrics are less able to represent the full range of urban heat. Using the case of Bexar County (home to San Antonio, TX), we find that increasing data aggregation increases both false positive and false negative identification of intra-urban heat islands, leading to unreliable results. Misclassification increases as aggregation increases, indicating that decisions should be made at the finest spatial resolution possible.

Abstract:The urban heat island effect exacerbates independent climate change‐induced shifts toward longer, stronger, and more frequent heat extremes. Environmental inequity, driven by a history of racially motivated urban planning policies, has led particular demographics to bear the worst impacts of urban heat exposure and thus also climate change. These impacts cause adverse health outcomes in the form of heat emergencies. Through a novel demographic and spatial analysis of heat‐related illness Emergency Medical Services data from Richmond, Virginia, this study investigates the relationships between heat health emergencies and intra‐urban heat islands quantified through three heat exposure metrics. We also evaluate the accessibility of built refuge from urban heat in the form of public transit infrastructure, libraries, and government cooling centers in relation to these emergencies. We found that heat emergencies are inequitably distributed among racial, age, and socioeconomic groups in Richmond, particularly among residents identified as Male, Black or African American, 50+ years old, and experiencing mental health, intoxication, and/or homelessness. We found significant associations between the location of these heat emergencies and urban heat islands as estimated from remotely‐sensed surface and community science‐derived air temperature metrics, but not a co‐estimated heat index. We also found that available refuge facilities are insufficiently located to protect individuals with reduced mobility across areas with the highest number of heat‐related health emergencies. Community involvement in the mitigation and management of extreme heat threats, especially for those disproportionately impacted, is necessary to decrease the number of summertime heat illnesses.

Abstract:Heatwaves pose a serious threat and are projected to amplify with changing climate and social demographics. A comprehensive understanding of heatwave exposure to the communities is imperative for the development of effective strategies and mitigation plans. This study explores spatiotemporal characterization of heatwaves across the historically vulnerable communities in Mississippi, United States. We derive multiple heatwave metrics including frequency, duration, and magnitude based on temperature data for urban-specific daytime, nighttime, and day-night combined conditions. Our analysis depicts a rising heatwave trend across all counties, with the most extreme shifts observed in prolonged day-night events lacking overnight relief. We integrate physical heatwave hazards with a socioeconomic vulnerability index to develop an integrated urban heatwave risk index. Integrated metric identifies the counties in northwest Mississippi as heat-prone areas, exhibiting an urgent need to prioritize heat resilience and adaptive strategies in these regions. The compounding urban heatwave and vulnerability risks in these communities highlights an environmental justice imperative to implement equitable policies that protect disadvantaged populations. Although this study is focused on Mississippi, our framework is scalable and can be employed to urban regions globally. This study provides a solid foundation for developing timely heatwave preparedness and mitigation to avert preventable heat-related tragedies as extremes intensify with climate change.

Abstract:Background: The increase in the frequency and intensity of extreme heat events, which are potentially associated with climate change in the near future, highlights the importance of heat health risk assessment, a significant reference for heat-related death reduction and intervention. However, a spatiotemporal mismatch exists between gridded heat hazard and human exposure in risk assessment, which hinders the identification of high-risk areas at finer scales. Methods: A human settlement index integrated by nighttime light images, enhanced vegetation index, and digital elevation model data was utilized to assess the human exposure at high spatial resolution. Heat hazard and vulnerability index were generated by land surface temperature and demographic and socioeconomic census data, respectively. Spatially explicit assessment of heat health risk and its driving factors was conducted in the Yangtze River Delta (YRD), east China at 250 m pixel level. Results: High-risk areas were mainly distributed in the urbanized areas of YRD, which were mostly driven by high human exposure and heat hazard index. In some less-urbanized cities and suburban and rural areas of mega-cities, the heat health risks are in second priority. The risks in some less-developed areas were high despite the low human exposure index because of high heat hazard and vulnerability index. Conclusions: This study illustrated a methodology for identifying high-risk areas by combining freely available multi-source data. Highly urbanized areas were considered hotspots of high heat health risks, which were largely driven by the increasing urban heat island effects and population density in urban areas. Repercussions of overheating were weakened due to the low social vulnerability in some central areas benefitting from the low proportion of sensitive population or the high level of socioeconomic development. By contrast, high social vulnerability intensifies heat health risks in some less-urbanized cities and suburban areas of mega-cities.
Extreme Heat Vulnerability Among Older Adults: A Multilevel Risk Index for Portland, Oregon

Abstract:Background and objectives: Extreme heat is an environmental health equity concern disproportionately affecting low-income older adults and people of color. Exposure factors, such as living in rental housing and lack of air conditioning, and sensitivity factors, such as chronic disease and social isolation, increase mortality risk among older adults. Older persons face multiple barriers to adaptive heat mitigation, particularly those living in historically temperate climates. This study measures two heat vulnerability indices to identify areas and individuals most vulnerable to extreme heat and discusses opportunities to mitigate vulnerability among older adults. Research design and methods: We constructed two heat vulnerability indices for the Portland, OR, metropolitan area: one using area scale proxy measures extracted from existing regional data and another at the individual scale using survey data collected following the 2021 Pacific Northwest Heat Dome event. These indices were analyzed using principal component analysis and Geographic Information Systems. Results: Results indicate that the spatial distribution of areas and individuals vulnerable to extreme heat are quite different. The only area found among the most vulnerable on both indices has the largest agglomeration of age- and income-restricted rental housing in the metropolitan area. Discussion and implications: Due to spatial variations in heat-related risk at the individual and area scales, measures addressing heat risk should not be spatially uniform. By focusing resources on older adult individuals and areas in particular need of assistance, heat risk management policies can be both highly efficient and cost effective.
Systematic identification of heat events associated with emergency admissions to enhance the heat-health action plan in a subtropical city: a data-driven approach

Abstract:According to the United Nations Office for Disaster Risk Reduction (UNDRR), a heat-health action plan should address various impacts of hazards at different levels, including an early warning system to monitor risks and behaviour enhancement to increase disaster preparedness. It is necessary to comply with guidelines regarding heat duration/intensity. In this study, we developed a data-driven approach to rapidly and systematically estimate the impacts of various heat events on emergency admissions among the adult population (n = 7,086,966) in Hong Kong in order to enhance the heat-health action plan. Immediate, short-term, and long-term impacts determined by 1-day, 4-day, and 8-day windows were estimated to identify specific heat events suitable for early warnings. In addition, underestimated risk, determined by a continuous increase in heat risk after days without significant emergency admissions, was estimated to evaluate potential maladaptive behaviours among a specific subpopulation. Based on age- and gender-specific analyses, 1D, 1D1N, and 2D2N were observed to have a stronger immediate impact on emergency admissions. 1D1N and 2D2N also showed notable short-term and long-term impacts. Based on heat vulnerability factors (age and gender), 2D2N was a higher-priority extreme heat event for early warning measures than 1D1N. Furthermore, men aged 19 to 64 had the highest underestimated risk. Specifically, they had IRR values of 1.113 [1.087, 1.140], 1.061 [1.035, 1.087], and 1.069 [1.043, 1.095] during lag days 3-5 of 3D2N, respectively, possibly due to a lack of adaptive behaviour. By adopting our approach, the duration of heat events with significant health impacts can be identified in order to further enhance relevant heat stress information. This framework can be applied to other cities with a similar background for rapid assessment.
Neighborhood effects on heat deaths: social and environmental predictors of vulnerability in Maricopa County, Arizona

Abstract:Background: Most heat-related deaths occur in cities, and future trends in global climate change and urbanization may amplify this trend. Understanding how neighborhoods affect heat mortality fills an important gap between studies of individual susceptibility to heat and broadly comparative studies of temperature-mortality relationships in cities. Objectives: We estimated neighborhood effects of population characteristics and built and natural environments on deaths due to heat exposure in Maricopa County, Arizona (2000-2008). Methods: We used 2000 U.S. Census data and remotely sensed vegetation and land surface temperature to construct indicators of neighborhood vulnerability and a geographic information system to map vulnerability and residential addresses of persons who died from heat exposure in 2,081 census block groups. Binary logistic regression and spatial analysis were used to associate deaths with neighborhoods. Results: Neighborhood scores on three factors-socioeconomic vulnerability, elderly/isolation, and unvegetated area-varied widely throughout the study area. The preferred model (based on fit and parsimony) for predicting the odds of one or more deaths from heat exposure within a census block group included the first two factors and surface temperature in residential neighborhoods, holding population size constant. Spatial analysis identified clusters of neighborhoods with the highest heat vulnerability scores. A large proportion of deaths occurred among people, including homeless persons, who lived in the inner cores of the largest cities and along an industrial corridor. Conclusions: Place-based indicators of vulnerability complement analyses of person-level heat risk factors. Surface temperature might be used in Maricopa County to identify the most heat-vulnerable neighborhoods, but more attention to the socioecological complexities of climate adaptation is needed.

Abstract:Background: Heat-waves have a substantial public health burden. Understanding spatial heterogeneity at a fine spatial scale in relation to heat and related mortality is central to target interventions towards vulnerable communities. Objectives: To determine the spatial variability of heat-wave-related mortality risk among elderly in Paris, France at the census block level. We also aimed to assess area-level social and environmental determinants of high mortality risk within Paris. Methods: We used daily mortality data from 2004 to 2009 among people aged >65 at the French census block level within Paris. We used two heat wave days' definitions that were compared to non-heat wave days. A Bernoulli cluster analysis method was applied to identify high risk clusters of mortality during heat waves. We performed random effects meta-regression analyses to investigate factors associated with the magnitude of the mortality risk. Results: The spatial approach revealed a spatial aggregation of death cases during heat wave days. We found that small scale chronic PM10 exposure was associated with a 0.02 (95% CI: 0.001; 0.045) increase of the risk of dying during a heat wave episode. We also found a positive association with the percentage of foreigners and the percentage of labor force, while the proportion of elderly people living in the neighborhood was negatively associated. We also found that green space density had a protective effect and inversely that the density of constructed feature increased the risk of dying during a heat wave episode. Conclusion: We showed that a spatial variation in terms of heat-related vulnerability exists within Paris and that it can be explained by some contextual factors. This study can be useful for designing interventions targeting more vulnerable areas and reduce the burden of heat waves.

Abstract:Extreme heat and wildfire smoke events are increasingly co-occurring in the context of climate change, especially in California. Extreme heat and wildfire smoke may have synergistic effects on population health that vary over space. We leveraged high-resolution satellite and monitoring data to quantify spatially varying compound exposures to extreme heat and wildfire smoke in California (2006–2019) at ZIP Code Tabulation Area (ZCTA) level. We found synergistic effects between extreme heat and wildfire smoke on daily cardiorespiratory hospitalizations at the state level. We also found spatial heterogeneity in such synergistic effects across ZCTAs. Communities with lower education attainment, lower health insurance coverage, lower income, lower proportion of automobile ownership, lower tree canopy coverage, higher population density, and higher proportions of racial/ethnic minorities experienced higher synergistic effects. This study highlights the need to incorporate compound hazards and environmental justice considerations into evidence-based policy development to protect populations from increasingly prevalent compound hazards.

Abstract:The bicuspid aortic valve is the most frequent congenital anomaly of the heart in adults. Since the 1970s, with the advent of two-dimensional echocardiography, the in-vivo diagnosis of bicuspid aortic valve has become easier and greater attention has been paid to this anomaly in normal young people, particularly in sportsmen because, from a theoretical point of view, physical exercise may stress the abnormal aortic valve, favouring an early deterioration of the valve itself, as well as a dilation of the ascending aorta. Although the bicuspid aortic valve may remain without clinical consequences for a lifetime, it may be responsible for severe complications such as aortic stenosis, aortic insufficiency, endocarditis and aortic dilation/dissection, sometimes requiring surgery. Moreover, the bicuspid aortic valve may be associated with other cardiovascular anomalies, mainly aortic coarctation. At present there are no prospective studies dealing with effect of physical training and competitive sports on the natural course of the bicuspid aortic valve. However, in order to take any decision about sports eligibility, sports physicians should perform an initial accurate staging of the bicuspid aortic valve, taking into account haemodynamic factors, aortic complications and associated significant cardiovascular anomalies. A strict follow-up, with serial cardiological controls, is mandatory as well as antibiotic prophylaxis for endocarditis, particularly in subjects engaged in contact sports.

Abstract:The increase in the frequency and intensity of extreme heat events (EHEs), potentially associated with climate change in the near future, highlights the importance of heat health risks assessment, which is an important starting point for the reduction of heat-related mortality and sustainable development. However, there is a spatiotemporal mismatch between hazard data (at pixel-level) and exposure data (at census unit level) in heat health risks assessment. Based on multisensor remote sensing data and demographic and socioeconomic statistical data, we used a human settlement index to assess heat exposure. Heat health risks and its driving factors were spatially explicitly assessed and mapped at the 250 m × 250 m pixel-level across the Yangtze River Delta region (YRD). The visual inspection suggests that the high risks areas were mainly distributed in urbanized areas of YRD, including the downtown of Shanghai, Changzhou, Hangzhou, Ningbo, Wuxi, Jiaxing and Taizhou, which was mostly driven by the high human exposure and heat hazard index. In less-urbanized cities and the suburban and rural areas of mega-cities, the heat health risks come second. Even though the human exposure index was low in other less-developed areas, the heat health risks in those areas were high due to the high heat hazard and human vulnerability index. It's of great importance to identify the driving factors of high heat health risks to provide science-based support for adaptation strategies and emergency planning.
Mapping Urban Heat Vulnerability of Extreme Heat in Hangzhou via Comparing Two Approaches

Abstract:Extreme heat is the leading cause of heat-related mortality around the world. Extracting heat vulnerability information from the urban complexity system is crucial for urban health studies. Using heat vulnerability index (HVI) is the most common approach for urban planners to locate the places with high vulnerability for intervention and protection. Previous studies have demonstrated that HVI can play a vital role in determining which areas are at risk of heat-related deaths. Both equal weight approach (EWA) and principal component analysis (PCA) are the conventional methods to aggregate indicators to HVI. However, seldom studies have compared the differences between these two approaches in estimating HVI. In this paper, we evaluated the HVIs in Hangzhou in 2013, employing EWA and PCA, and assessed the accuracies of these two HVIs by using heat-related deaths. Our results show that both HVI maps showed that areas with high vulnerability are located in the central area while those with low vulnerability are located in the suburban area. The comparison between HVI(EWA)and HVI(PCA)shows significantly different spatial distributions, which is caused by the various weight factors in EWA and PCA. The relationship between HVI(EWA)and heat-related deaths performs better than the relationship between HVI(PCA)and deaths, implying EWA could be a better method to evaluate heat vulnerability than PCA. The HVI(EWA)can provide a spatial distribution of heat vulnerability at intracity to direct heat adaptation and emergency capacity planning.

Abstract:It is usually accepted that the predisposition to atopic disease encompasses at least 20 % of the population. They are consequently vulnerable to a wide spectrum of disease, ranging from trivial to life-threatening. Moreover, the implication on the part of the individual for a normal life, at school, work or play, may be considerable. It is beyond argument that the additional cost to society of providing health care and social benefits to those afflicted is very substantial. In the United Kingdom, the cost of providing primary and secondary care for those with asthma alone was estimated in 1990 to be 400 million pounds per annum• The additional consequence of social benefits and absence from employment might mean the real cost of asthma to the economy is closer to one billion pounds per annum. Thus, there is a clear incentive, both for the individual and the state, to try to diminish or prevent allergic disease. At present, there is no acceptable method of approaching the genetic influence into the equation, so attention must be directed at identifying the high risk families and attempting to influence sensitization to allergens. It is a reasonable assumption that there is a stage in the development of the atopic individual when he or she has no specific IgE and therefore has not been sensitized. Although, it is known that the fetus produces endogenous IgE at the end of the first trimester, most researchers have found few or no infants to have measurable specific IgE in cord blood• As a population is followed through infancy into the first years of life, the percentage demonstrating a specific IgE response either to skin prick testing or to some method of quantitative assay of IgE such as the Radioallergosorbent test (RAST) increases• Initially, this response is primarily to food antigens but rapidly aero-allergens produce sensitization. Abstract:BackgroundHeat stroke is a serious heat-related illness, especially among older adults. However, little is known regarding the spatiotemporal variation of heat stroke admissions during heat waves and what factors modify the adverse effects.MethodsWe conducted a large-scale national study among 23.5 million Medicare fee-for-service beneficiaries per year residing in 1,916 US counties during 1999–2010. Heat wave days, defined as a period of at least two consecutive days with temperatures exceeding the 97th percentile of that county’s temperatures, were matched to non-heat wave days by county and week. We fitted random-effects Poisson regression models to estimate the relative risk (RR) of heat stroke admissions on a heat wave day as compared to a matched non-heat wave day. A variety of effect modifiers were tested including individual-level covariates, community-level covariates, meteorological conditions, and the intensity and duration of the heat wave event.ResultsThe RR declined substantially from 71.0 (21.3–236.2) in 1999 to 3.5 (1.9–6.5) in 2010, and was highest in the northeast and lowest in the west north central regions of the US. We found a lower RR among counties with higher central air conditioning (AC) prevalence. More severe and longer-lasting heat waves had higher RRs.ConclusionsHeat stroke hospitalizations associated with heat waves declined dramatically over time, indicating increased resilience to extreme heat among older adults. Considerable risks, however, still remain through 2010, which could be addressed through public health interventions at a regional scale to further increase central AC and monitoring heat waves.
