Abstract:Purpose To determine prevalence of refractive (RA), corneal (CA) and internal astigmatism (IA), including variation with gender and spherical equivalent refraction (SE), in a population of 12-year-old Chinese children. Methods A total of 1783 students with a mean age of 12.7 years (range 10.0-15.6 years) completed comprehensive eye examinations in the Anyang Childhood Eye Study. Data of cycloplegic refraction and corneal curvature were analysed. Results Prevalences of RA, CA and IA >= 1.0 D were 17.4% (95% CI 15.6% to 19.2%), 52.8% (50.5% to 55.1%)%) and 20.9% (19.0% to 22.8%), respectively. With different limits of astigmatism axes classification, including +/- 15 degrees, +/- 20 degrees and +/- 30 degrees, RA and CA axes were mainly ' with-the-rule' (WTR) (ie, correcting axis of negative cylinders at or near 180 degrees), while those for IA axes were mainly ' against-the-rule' (ATR) (ie, correcting axis of negative cylinders at or near 90 degrees). RA was not different between the genders, but girls had higher prevalence and greater means of CA and IA. RA and CA increased in students with higher ametropia (more myopia and more hyperopia) and were the highest in a high myopic group (SE <=-6 D), while IA was stable across refraction groups. Children with RA higher than 0.50 D were more likely to have lens corrections (51%, 57%, 61% and 69% for magnitudes of >= 0.50 D, >= 0.75 D, >= 1.0 D and >= 1.5 D, respectively). Conclusions Prevalence of RA in the Chinese 12-year-old children was relatively high compared with other studies. RA and CA had mainly ' WTR' astigmatism, while IA was mainly ATR and partially compensated for CA. Girls had greater means and prevalences of CA and IA than did boys. Both RA and CA, but not IA, increased with refractive errors away from emmetropia.
The Distribution Pattern of Ocular Residual Astigmatism in Chinese Myopic Patients

Abstract:Purpose: We aimed to investigate the distribution of ocular residual astigmatism (ORA) and its associations with age, gender, manifest refraction, and other ocular indicators in Chinese patients with myopia. Design: This is a multi-center retrospective cross-sectional study. Method: The study included 7,893 patients with myopia (7,893 eyes) aged 18-40 years from five ophthalmic centers. Anterior segment biometrics of the eyes were collected from the Pentacam. ORA and its summated vector mean were calculated using Alpins vector analysis. Compensation factor (CF) was used to evaluate the relation between ORA and corneal astigmatism. ORA in different age, gender, and refraction groups was compared. The Spearman correlation was adopted to reveal multiple ocular indicators associated with ORA, which were integrated into a multiple linear regression model to predict ORA. Results: Distribution of ORA was slightly positively skewed (Skewness= 2.111, Kurtosis = 19.660, KS P < 0.0001). Mean ORA was 0.74 ± 0.39 D (95% normal range: 0.14-1.54 D). Among all the subjects, 22.4% of the eyes had an ORA magnitude of 1.0 D or more. Undercompensation or full-compensation of anterior corneal astigmatism (ACA) by ORA prevailed in both J0 (76.99%) and J45 (58.48%). Women had higher ORA power than men (0.77 ± 0.36 D vs. 0.73 ± 0.41 D, P < 0.0001). Participants with less negative spherical equivalent (SE) or higher manifest astigmatism (MA) also had higher ORA (all P < 0.0001). ORA was significantly correlated with ACA (r = 0.405) and posterior corneal astigmatism (PCA, r = 0.356). The multivariate logistic regression analysis showed strong predictability of ORA magnitude >1.0 D using anterior segment parameters (area under the receiver operating characteristic curve: 0.739). Conclusion: ORA is present in Chinese adults with myopia and is affected by multiple ocular factors. Our findings may provide valuable information about ORA distribution in candidates for refractive surgery, helping optimize the outcome of astigmatism correction.
Internal Astigmatism and Its Role in the Growth of Axial Length in School-Age Children.

Abstract:Objectives. To explore the role of internal astigmatism (IA) in the growth of axial length (AL) in school-age children. Methods. Total astigmatism (TA), corneal astigmatism (CA), and AL of all children in Jing'an District 2nd Centre Primary School in Shanghai were measured. In IA, the difference between TA and CA was also calculated using vector analysis. The association of axial length with IA, genders, and age was analyzed using linear regression. The difference of IA between both eyes was also calculated. The AL between both eyes was compared using paired samples t-test when DIA = 0 D, <0.5 D, and >= 0.5 D. Results. Six hundred and twelve cases (98.23%) in 623 children aged 7-12 yrs older entered into the study. Genders, age, and IA all affected AL. This could be represented by a linear regression line in the form AL = 21.46-0.43 * gender +0.22 * age + 0.46 * IA (male = 1, female = 2; t = 7 01, P < 0 01 for sex; t = 11 6, P < 0 01 for age; and t = 6 6, P < 0 01 for IA; R-2 = 0 16). The AL in the eye with larger IA was also longer when DIA was larger than 0.5D (t = 2 65, P < 0 01). Conclusions. IA was observed to be associated with AL and might be a risk factor of the onset and progress of myopia in school-age children.
Association Between Myopia, Biometry and Occludable Angle: the Jiangning Eye Study

Abstract:BackgroundMyopia subjects usually have long axial lengths and are predisposed to deep anterior chamber depths and open angles. Jiangning eye study is a randomized, population based study in Shanghai to determine whether myopia has an effect on the prevalence of occludable angles.MethodsA total of 2478 residents aged 50 years and over were randomly selected, and 2044 (82.5%) individuals of them participated in ophthalmic examinations between November 2012 and February 2013. Eye examinations included autorefraction, noncontact tonometry, measurement of axial length, anterior chamber depth, lens thickness and spectral domain optical coherence tomography.ResultsThe crude prevalences of iris trabecular meshwork contact in Jiangning Chinese individuals with myopia, emmetropia, and hyperopia are 1.25%, 6.44%, and 7.43%, respectively. There is also iris trabecular meshwork contact and shallow anterior chamber depth in myopia subjects even with long axial lengths.ConclusionsThe prevalence of iris trabecular meshwork contact in myopia subjects is lower than their emmetropia or hyperopia counterparts. Axial length is an important factor associate with occludable angle.
Ocular Anatomical and Functional Characteristics in Anisometropic Chinese Children

Abstract:SIGNIFICANCE This research found that anterior and posterior biometrics differ in many aspects between fellow eyes of anisometropic children. This might shed light on the mechanisms underlying the onset and progression of anisometropia and myopia. PURPOSE This study aimed to investigate the ocular biometric parameters, peripheral refraction, and accommodative lag of fellow eyes in anisometropic children. METHODS Anisometropic children were recruited. Axial length (AL), vitreous chamber depth (VCD), central corneal thickness, anterior chamber depth (ACD), lens thickness (LT), simulated K readings, central and peripheral refractive errors, and accommodative lag were measured in both eyes. The subfoveal choroidal thickness, average choroidal thickness, and choroid vessel density of the 6 x 6-mm macular area were measured by optical coherence tomography. RESULTS Thirty-two children aged 11.1 +/- 1.7 years were enrolled. The average degree of anisometropia was 2.49 +/- 0.88 D. The AL, VCD, ACD, and simulated K reading values were significantly larger in the more myopic eyes, whereas the LT value was significantly smaller. Subfoveal choroidal thickness (P = .001) and average choroidal thickness (P = .02) were smaller in the more myopic eyes than in the contralateral eyes, whereas choroid vessel density (P = .03) was larger. The amount of anisometropia had a significant positive correlation with the difference in AL (r = 0.869, P < .001), VCD (r = 0.853, P < .001), and ACD (r = 0.591, P < .001) and a negative correlation with the difference in LT (r = -0.457, P = .009). CONCLUSIONS Ocular biometrics differ in many aspects between the fellow eyes of anisometropic Chinese children, and the difference is correlated with the degree of anisometropia.

Abstract:Abstract Background: There is an compensatory effect between corneal astigmatism (CA) and internal astigmatism (IA). And the refractive astigmatism (RA) decreased fast in the early 3 years. Nevertheless, little attention was paid to the change in astigmatism in preschool children. Thus, we aimed to investigate the age-dependent distribution of astigmatism and its component in Chinese preschool children from year 3 to year 6 and also, to study the relationship between ocular biometry and IA. Methods: Automated refraction, keratometry and axial length (AL) were performed in this cross-sectional study. The IA was determined as the vector difference between RA and CA. The magnitude and type (with-the-rule WTR, against-the-rule ATR, oblique) of RA, CA and IA were investigated according to the age. Spearman correlation was adopted to detect the factors that may influence IA. Results: A total of 2315 subjects (2315 right eyes) (47.38% female) with a mean age of 4.52±0.88 years (range 3-6 years) were included. The magnitude and the ratio of three kinds of CA components did not change with aging (p=0.1233 and p>0.1, respectively). Though the axial distribution of RA remained unchanged with aging, a significant reduction in RA was seen in year 6 compared to that of year 3 and year 4 (p=0.036 and 0.041, respectively). Conversely，the ATR component in IA was lower in year 6 (68.02%) than it in year 3 (81.92%) and year 4 (77.12%) (both p<0.001) accompanied by increased frequency of oblique astigmatism in year 6 than it in year 3 and year 4 (p<0.001 and p=0.012,respectively). Negative relationship was found between IA and AL. Conclusion: RA decreased from year 3 to year 6. The shift of the axis in IA from ATR to Oblique and WTR may account for the reduction in RA. Corneal and Lenticular Biometry in Chinese Children with Myopia

Abstract:Background This study aimed to evaluate the distribution characteristics of corneal and lenticular parameters in Chinese children with myopia and explored their association with other ocular components. Methods In this cross-sectional study, all ocular biometric parameters were measured using a Zeiss IOLMaster 700 Biometry. Simulations of the corneal and lenticular curvature radii were implemented using a customised MATLAB program based on cross-sectional swept-source optical coherence tomography images obtained from the same device. The associations of the calculated and simulated refractive powers of the cornea and lens with other ocular parameters were evaluated. Results In total, 119 children with myopia were recruited. Boys had a deeper anterior chamber and longer axial length (AL) than girls, while girls had steeper anterior corneal and lenticular curvatures and greater corneal and lenticular power. Children aged 10 years and older showed a larger anterior lenticular radius of curvature (sR(al)) and less lenticular power (P-L,P-OCT) than younger participants. There was a significant positive correlation between AL and the anterior corneal radius of curvature, regardless of sex or age. The sR(al) exhibited a significant increasing trend, and P-L,P-OCT exhibited a declining trend with a longer AL only in children younger than 10 years. Conclusion AL is the most influential factor in the determination of spherical equivalent refractive error, while decreases in both corneal and crystalline lens power are significantly inversely correlated with axial elongation.
Refractive and corneal astigmatism in Chinese 4–15 years old children: prevalence and risk factors

Abstract:Abstract Background To investigate the prevalence and risk factors of refractive astigmatism (RA) and corneal astigmatism (CA) in preschool children and school-aged children in Shanghai, China. Methods In this school-based, cross-sectional study, 4–15 years old children across three learning stages of kindergarten, primary school, and junior high school underwent noncycloplegic autorefraction and completed comprehensive questionnaires involving time spent on daily homework and outdoor activities. Data from the right eyes were analysed. Results Overall, 7084 children (mean ± standard deviation (SD) of age: 8.08 ± 3.11 years) were included, and the prevalence rates of RA/CA ( ≤ − 1.0 D) in children were 15.8%/64% in kindergartens, 16.5%/65% in primary schools, and 32.8%/76.9% in junior high schools. The magnitude and prevalence of RA and CA all increased with age or with learning stage (all P < 0.001). The presence of RA was associated with more myopic spherical power (odds ratio (OR) 0.956, P = 0.021), junior high school (OR 1.973, P < 0.001), longer homework time on weekdays (OR 1.074, P = 0.029), and shorter outdoor activity time on weekends (odds ratio 0.929, P = 0.013). Conclusion In the wide age range of 4 to 15 years, the magnitude and prevalence of RA and CA increased with the learning stage, and these increases mainly began at the primary school stage. Factors, including longer homework time and shorter outdoor time were correlated with the presence of RA.

Abstract:Objective To investigate the relationship between myopic refraction and ocular components.Methods Retinoscopy was performed in 533 myopic subjects (1018 eyes).K value was measured by corneal mapping,and A/B ultrasound scan to get AL,ACD and LT.VCD,AnL,RLT/AL and RAL/R were calculated.Results In myopia,AL was not associated with LT (r =0.028,P =0.364); SEQ and AL,ACD,LT,K,VCD,RAL/R,RLT/AL were all significantly correlated (P ＜0.01),and better associated with RAL/R,A1 and VCD (r =-0.815;-0.624;-0.619); ACD,LT and AnL were not statistically different between myopia groups (ANOVA:P =0.153; 0.063; 0.625),nonetheless,all the other parameter did (P ＜0.01).Conclusions Myopic severity can be featured by the significant difference of posterior segment measurement,the anterior segment is comparatively stable and statistical insignificant.
Effect of Individualized Ocular Refraction Customization Spectacle Lens Wear on Visual Performance in Myopic Chinese Children

Abstract:Purpose: Individualized ocular refraction customization (IORC) lenses can be individually adjusted depending on the initial relative peripheral refraction to determine the myopic defocus (MD). We aimed to compare visual performance of children wearing IORC lenses with different amounts of MD to determine whether higher MD resulted in greater visual compromise. Methods: This study included 184 myopic children aged eight to 12 years, and 172 completed the trial. The participants were randomly assigned to wear IORC lenses with low (IORC-L, 2.50 D), medium (IORC-M, 3.50 D), or high (IORC-H, 4.50 D) MD or single-vision spectacle lenses (SVL). Distance and near best-corrected visual acuity (BCVA), contrast sensitivity function (CSF) and questionnaires were evaluated at baseline and after six and 12 months. Results: CSF over all frequencies and distance and near BCVA were not affected by lens design (all P > 0.05). The SVL group outperformed the three IORC lens groups in terms of ghosting images at baseline, and IORC-H and IORC-M groups outperformed IORC-L group (all P < 0.001); however, no differences were observed at the six- or 12-month visit. There were no significant differences among the four groups for any other subjective variables at any of the follow-up visits regarding vision clarity, vision stability, eyestrain, dizziness, headache, or overall vision satisfaction (all P > 0.05). Conclusions: The IORC lenses with an actual MD of 4.50 D provided acceptable objective and subjective visual performance and were well tolerated by children. Translational relevance: IORC lenses with an actual MD of 4.50 D provided acceptable visual performance.
Ocular Biometrics and Uncorrected Visual Acuity for Detecting Myopia in Chinese School Students

Abstract:The study is to evaluate the performance of ocular biometric measures and uncorrected visual acuity (UCVA) for detecting myopia among Chinese students. Among 5- to 18-year-old Chinese students from two cities of China, trained eye-care professionals performed assessment of ocular biometrics (axial length (AL), corneal curvature radius (CR), anterior chamber depth) under noncycloplegic conditions using NIDEK Optical Biometer AL-Scan, distance visual acuity using retro-illuminated logMAR chart with tumbling-E optotypes, and cycloplegic refractive error using NIDEK autorefractor with administration of 0.5% tropicamide. Spherical equivalent (SER) in diopters (D) was calculated as sphere plus half cylinder, and myopia was defined as SER ≤ - 0.5 D. Performances of ocular biometrics and UCVA (individually and in combination) for detecting myopia were evaluated using sensitivity and specificity, predictive values, and area under ROC curve (AUC) in both development dataset and validation dataset. Among 3436 students (mean age 9.7 years, 51% female), the mean (SD) cycloplegic SER was - 0.20 (2.18) D, and 1269 (36.9%) had myopia. Cycloplegic SER was significantly correlated with AL (Pearson Correlation coefficient r = - 0.82), AL/CR ratio (r = - 0.90), and UCVA (r = 0.79), but was not correlated with CR (r = 0.02, p = 0.15). The AL/CR ratio detected myopia with AUC 0.963 (95% CI 0.957-0.969) and combination with UCVA improved the AUC to 0.976 (95% CI 0.971-0.981). Using age-specific AL/CR cutoff (> 3.00 for age < 10 years, > 3.06 for 10-14 years, > 3.08 for ≥ 15 years) as myopia positive, the sensitivity and specificity were 87.0% (95% CI 84.4-89.6%) and 87.8% (86.0-89.6%), respectively, in the development dataset and 86.4% (95% CI 83.7-89.1%) and 89.4% (95% CI 87.3-91.4%), respectively, in the validation dataset. Combining AL/CR and UCVA (worse than 20/32 for age < 10 years, and 20/25 for ≥ 10 years) provided 91.9% (95% CI 90.4-93.4%) sensitivity and 87.0% (95% CI 85.6-88.4%) specificity, positive value of 80.6% (95% CI 78.5-82.6%) and negative value of 94.8% (95% CI 93.8-95.8%). These results suggest that AL/CR ratio is highly correlated with cycloplegic refractive error and detects myopia with high sensitivity and specificity, AL/CR ratio alone or in combination with UCVA can be used as a tool for myopia screening or for estimating myopia prevalence in large epidemiological studies with limited resources for cycloplegic refraction.
Analysis of factors related to the development of ocular biometric parameters in Chinese children aged 6-10 years: a cross-sectional study

Abstract:Objectives Emmetropia depends on the precise coordination of ocular biometry, including axial length (AL), corneal curvature, lens thickness and anterior chamber depth (ACD). Disruption of this coordination leads to refractive errors such as myopia. This article aimed to determine the factors affecting ocular biometry and myopia development in young children. Design A cross-sectional study. Setting This study was conducted in a primary school in the Yanqing district of Beijing, China. Participants 792 students in grades 1–3 without hyperopia (>+2.00 D), strabismus, or amblyopia were selected. Exclusions: students had conditions affecting best corrected visual acuity and whose guardians refused to provide informed consent. Ocular biometric measurements and non-cycloplegia autorefraction were performed. The questionnaire addressed factors such as perinatal factors and environmental factors. Interventions None. Primary and secondary outcomes Ocular biometry and myopia. Results According to the multivariate logistic regression analysis, electronic screen use >2 hours/day (OR=2.175, p=0.013), paternal myopia (OR=1.761, p=0.002), maternal myopia (OR=1.718, p=0.005), taller height (OR=1.071, p<0.001), maternal education (OR=0.631, p=0.012) and maternal gestational hypertension (OR=0.330, p=0.042) were associated with myopia. AL was affected by female sex (OR=0.295, p<0.001), older age (OR=1.272, p=0.002) and taller height (OR=1.045, p<0.001). Female sex (OR=0.509, p<0.001), taller height (OR=1.046, p 2 hours each day (OR=3.596, p 2 hours each day (OR=0.431, p=0.001) influenced ACD incidence. Central corneal thickness (CCT) was associated with older age (OR=1.113, p=0.008), paternal education (OR=1.474, p=0.007), premature birth (OR=0.494, p=0.031), history of blue light therapy in infancy (OR=0.636, p=0.041) and history of incubator therapy in infancy (OR=0.263, p=0.009). Only sex influenced corneal curvature. Conclusions The factors associated with myopia were partly related to ACD and AL, and perinatal factors were associated with myopia and CCT. Results Of 1327 children (66.8 ± 3.4 months, 53.2% male), older age of the child (OR = 0.95 for per month increase; P = 0.03), older paternal age at child birth (OR = 1.04 for per year increase; P = 0.03), paternal astigmatism (OR = 1.89; P = 0.003), maternal astigmatism (OR = 1.73, P = 0.008), and second-hand smoke exposure during pregnancy (OR = 1.64; P = 0.03) were associated with higher risk of TA, while partial breastfeeding (OR = 0.49, P = 0.006) or formula feeding (OR = 0.46, P = 0.003) were associated with lower risk of TA. Larger ratio of axial length to corneal radius (OR = 16.16 for per unit increase; P = 0.001), maternal working during pregnancy (OR = 1.27; P = 0.04), and cesarean delivery (OR = 1.68, P = 0.04) were associated with higher risk of ACA, while formula feeding was associated with lower risk of ACA (OR = 0.57, P = 0.01). Paternal astigmatism (OR = 0.50, P = 0.01) and assisted reproduction (OR = 0.56, P = 0.03) were associated with lower risk of horizontal or vertical internal compensation. More outdoor activity time (OR = 1.15 for per hour increase, P = 0.01) was associated with higher risk of oblique internal compensation while more nighttime sleep on weekends (OR = 0.83 for per hour increase, P = 0.01) was associated with lower risk of oblique internal compensation. Conclusions Our study confirmed some previously reported risk factors and identified some novel risk factors for astigmatism including formula feeding for lower risk of both ACA and TA, and older paternal age at child birth for higher risk of TA.

Relationship Between Ocular Wavefront Aberrations and Refractive Error in Chinese School Children.

Abstract:BACKGROUND:The relationship between ocular wavefront aberrations and refractive error in children's eyes remains controversial. The purpose of this study is to re-examine this relationship in Chinese school children under natural distance accommodation.METHODS:Ocular wavefront aberrations were measured in 86 Chinese children with spherical equivalent refraction (SER) between +0.5 D and -6.0 D and astigmatism less than -1.00 D. Wavefront aberrations were calculated using an objective method based on the Hartmann-Shack principle. Refractive error was obtained using a phoropter after cycloplegia. Subjects were categorised into three groups based on the mean SER: emmetropia (SER from -0.50 D to +0.50 D), mild myopia (SER greater than -0.50 D to -3.00 D) and moderate myopia (SER greater than -3.00 D to -6.00 D). Of the 86 participants, 22 were emmetropic, 43 were mildly myopic and 21 were moderately myopic. The root mean square (RMS) values of higher-order aberrations, Zernike coefficients (third-, fourth- and fifth-order aberrations) and R(j) (the ratio of third-, fourth- or fifth-order aberrations to total higher-order aberrations) were compared across the three refractive groups.RESULTS:No significant correlations were found between the RMS values of total higher-order aberrations, third-order aberrations, fourth-order aberrations, fifth-order aberrations, spherical aberration or coma and SER. No significant differences in the RMS values of total higher-order aberrations or R(j) were observed among the groups. The difference in fifth-order aberrations was statistically significant among the groups (p = 0.022); no other differences in higher-order aberration were found. Aside from C (3,1), no other differences were observed for Zernike coefficients.CONCLUSION:Ocular wavefront aberrations are similar among Chinese school children with different refractive errors under natural accommodation for a distance target. There is no evidence that myopes have a different amount of ocular higher-order aberrations than emmetropes.
Astigmatism in school students of eastern China: prevalence, type, severity and associated risk factors

Abstract:Abstract Background China has been undergoing dramatic economic development, accompanied by increased education load on the young children. This study is to investigate the prevalence, type, severity, and associated risk factors of astigmatism in school students in eastern China. Method In this cross-sectional school-based study, students underwent refraction using NIDEK non-cycloplegic autorefractor. Astigmatism was defined as cylinder 1.5 diopter (D) or greater, and high astigmatism was defined as cylinder 3.0 D or greaterMultivariate regression models were used to determine factors associated with astigmatism. Results Among 4801 children (55% male) with mean age (±standard deviation) 12.3 (±3.8) years, 680 (14.2, 95% confidence interval (95% CI): 13.2–15.2%) had astigmatism (85% were with-the-rule) and 103 (2.2, 95% CI: 1.8–2.6%) had high astigmatism. The prevalence rate of astigmatism was 7–8% in grades 4 or below, 15–16% in grades 6–8, 20% in grade 9, and 20–25% in grade 10 or above. In multivariate analyses, higher grade and male gender were associated with higher prevalence of astigmatism (all p < 0.0001) and high astigmatism ( p = 0.04 for grade, p = 0.001 for gender). When multivariate models were further adjusted by spherical equivalent, only gender remained statistically associated with astigmatism (odds ratio (OR) = 1.65, p < 0.0001) and high astigmatism (OR = 2.21, p = 0.0004), myopic and hyperopic refractive error were significantly associated with higher risk of astigmatism and high astigmatism (all p < 0.0001). Conclusion Astigmatism is common in Chinese school-age children and increases with grade. Majority of astigmatism is with-the-rule. Male gender and myopic or hyperopic refractive error are significantly associated with higher prevalence and severity of astigmatism.

Abstract:BACKGROUND Myopia and high myopia are global public health concerns. Patients with high myopia account for 0.5%-5.0% of the global population. AIM To examine diopters, axial length (AL), higher-order aberrations, and other ocular parameters in Chinese children with myopia, to analyze the influence of structural parameters associated with myopia on visual quality, and to provide a theoretical basis for the prevention and treatment of childhood myopia and high myopia. METHODS This study included 195 children aged 6-17 years with myopia. The AL was measured with an ultrasonic ophthalmic diagnostic instrument, and the aberrations, corneal curvature (minimum K1, maximum K2, and average Km), central corneal thickness, anterior chamber depth, and anterior chamber angle were measured using a Sirius three-dimensional anterior segment analyzer. Using a standard formula, the corneal radius of curvature R (337.3/Km) and AL/R values were obtained. RESULTS The diopter of high myopia compared with low-middle myopia was correlated with age and AL (r = -0.336, -0.405, P < 0.001), and AL of high myopia was negatively correlated with K1, K2, and Km (r = -0.673, -0.661, and -0.680, respectively; P < 0.001), and positively correlated with age and the anterior chamber depth (r = 0.214 and 0.275, respectively; P < 0.05). AL/R was more closely related to diopter than AL in children with myopia, and 94.4% of children with myopia had an AL/R of > 3.00. CONCLUSION The ocular structural parameters of children change because of different diopters. AL/R is more specific and sensitive than AL in evaluating the refractive status of myopia in children. An AL/R of > 3.00 may be used as a specific index of myopia in children. There are differences in AL/R between high myopia and low-middle myopia, which can be used for the classification of ametropia. The degree of myopia has a certain influence on higher-order aberrations.
Longitudinal change in components of astigmatism and its association with axial length-corneal radius ratio in Chinese young children: the Nanjing Eye Study

Abstract:PurposeTo assess the longitudinal change in components of astigmatism from age 4 to 7 years and its association with axial length-corneal radius ratio (AL/CR).MethodsChildren born between September 2011 and August 2012 in Yuhuatai District of Nanjing were invited to participate in the Nanjing Eye Study for a comprehensive eye examination annually since 2015. The data presented in this paper were obtained in 2016, 2017, and 2019. At each study encounter, noncycloplegic autorefraction and ocular biometric parameters were measured. Changes of total astigmatism (TA), corneal astigmatism (CA), anterior corneal astigmatism (ACA), residual astigmatism (RA), and internal astigmatism (IA) were analyzed in clinical notation (Cyl) and vector notation (J0, J45).ResultsNine hundred fifty-four children (mean ± standard deviation of baseline age: 4.63 ± 0.29 years, 53.7% boys) had complete data and were included in this study. Mean slopes of longitudinal changes in Cyl notation were significantly negative for TA, CA, and ACA, but positive for IA. TA, CA, ACA, and RA of J0 notation had a shift toward increasing with-the-rule (WTR) astigmatism and/or decreasing against-the-rule (ATR) astigmatism. TA of J45 notation showed an increase in astigmatism at axis 135° and/or a decrease in astigmatism at axis 45°, while CA and ACA of J45 notation showed an opposite change. Longitudinal changes in ACA and IA were negatively correlated in J0 notation, but not in J45 notation. Based on compensation factor (CF, defined as the minus ratio of IA and ACA), the compensation proportions for J0 in varying degrees (CF: 0.1–2) in 2016, 2017, and 2019 were 91.3%, 93.5%, and 90.0%, respectively, while these for J45 were 74.9%, 76.5%, and 34.6%, respectively. Higher AL/CR increase was associated with less decrease or more increase in CA and ACA of Cyl notation, and a shift toward increasing WTR and/or decreasing ATR in these of J0 notation.ConclusionsThe compensatory role of IA was persistent and prominent from 4 to 7 years old for J0 notation in Chinese young children. The progression of AL/CR was correlated with astigmatism originated from the cornea.

Abstract:Purpose: The purpose of this project is to explore the changes in and factors associated with asymmetry between the superior and inferior hemiretina in children with different refractive levels. Materials and Methods: A total of 2964 children from 12 primary and middle schools in Shanghai, China, were included. The mean thickness of the GCL+ layer (ganglion cell layer+ inner plexiform layer), the GCC layer (ganglion cell complex, a combination of the retinal nerve fiber layer and the GCL+ layer), and the neural retina was measured according to the sectors defined by the Early Treatment of Diabetic Retinopathy Study (ETDRS) grid with swept-source optical coherence tomography. The asymmetry index (AI), calculated via the formula AI = |log(10) (superior/inferior hemiretinal layer thickness) |, was used to assess the asymmetry of each retinal layer. Results: The mean age of the children was 11.67 (3.45) years old. The AIs of all retinal layers were larger in myopes than in hyperopes and emmetropes in the perifoveal region (all p < 0.001), and the AIs became even larger in the GCL+ layer and neural retina in children with a higher degree of myopia. However, no statistical differences were observed between various refractive groups in the parafoveal region. Regression analyses indicated that with increasing myopia, the asymmetry of retinal layers increased in the perifoveal region (all p < 0.001) and slightly decreased in the parafoveal region (all p < 0.05). Conclusions and relevance: The vertical asymmetry of retinal layers increased in the perifoveal region in myopic children, and the change in asymmetry was more obvious in the GCL+ layer and neural retina than in the GCC layer.
