Abstract:Objective To investigate the effect of the new labor standard on maternal and fetal outcomes.Methods A retrospective study was performed in Peking University the Third Hospital.6 836 cases managed with new labor standard as the study group,and 5 385 cases with the old labor management as control group.All patients in the study had excluded absolute indications of cesarean section such as multipara,cesarean section history,twin pregnancy,placenta previa and other severe pregnancy.complications.The general condition,cesarean section indications,maternal and neonatal outcomes of the two groups were compared.Results Cesarean section rate of study group (25.19％,1 722/6 836) was significantly lower than that of control group (28.79％,1 550/5 385,P＜ 0.05).There were 757 cases of cesarean section due to stagnation of active period in control group (48.83％),while 742 cases in the study group (43.09％).93 cases of cesarean section due to prolonged second stage in control grotup (6.00％) and 64 cases in study group (3.72％).There were 113 cases.of forceps assisted delivery in study group (1.65％),110 in control group (2.04％),with no statistically difference (P＞ 0.05).534 cases of postpartum hemorrhage in the study group (7.81％),407 cases in the control group (7.56％),with no statistically difference (P ＞ 0.05).96 cases of chorion amnion inflammatory in study group (1.40％),58 cases in the control group (1.08％),P＞0.05.42 cases of neonatal asphyxia in control group (0.78％),69 cases in study group (1.01％),P＞ 0.05.9 cases of neonatal pneumonia in the study group (0.13％),and 6 in the control group (0.11％),with no statistical difference (P＞0.05).Conclusion Using the new labor management,the total cesarean section rate,cesarean due to active period stagnation and prolonged second stage were all reduced.There was no significant difference on maternal and neonatal outcomes using the new labor management.

Abstract:Objective: To discuss the influence of the new obstetrical labor standards and management in its application to maternal, newborns and the practice in delivery room. Methods: The study utilized a retrospective case-control analysis. The control group included 620 inpatients who delivered during September to November in 2013. The experimental group gathered 956 inpatients delivered during September to November in 2014. Results: We analyzed the data from the two groups. The ifrst stage of labor and the total stage of labor in the total cases, the case with PROM and the cases with DM were statistically significantly different (P<0.05).The perineum integrity and cesarean section rate (from labor room) were statistically signiifcantly different (P<0.05). Conclusion: The new obstetrical labor standards and management should be worthy to be generalized in the application, but the challenges should be noticed for the practice in delivery room.
The clinical outcomes of late preterm infants: a multi-center survey of Zhejiang, China.

Abstract:Objective: To explore birth rate, delivery mode, medical problems, requirement of respiratory support, and acute outcomes of late preterm infants in Zhejiang province in eastern China. Methods: Eleven tertiary hospitals were recruited. Clinical data of every nursery admission from January to December 2007 were collected and analyzed. Results: During the study period, 44,362 infants were born with an overall preterm birth rate of 8.9%, and late preterm birth rate of 6.2%. Late preterm infants had higher cesarean section rate than the whole population (64.9% vs. 58.2%). One-fifth of the nursery admissions were late preterm infants, of whom, 63.8% were delivered by cesarean section. Respiratory distress (42.1%) was the most common medical problem of late preterm infants. Hyperbilirubinemia (17.6%), hypoglycemia (8.7%) and sepsis (5.9%) were also common. The first three primary diagnoses of respiratory distress included pneumonia (39.5%), transient tachypnea of newborn (TTN) (22.5%) and respiratory distress syndrome (RDS) (19.0%). Compared with term infants, late preterm infants with respiratory distress needed more respiratory support with nasal continuous positive airway pressure (nCPAP) (21.4% vs. 11.6%) or with a mechanical ventilator (15.4% vs. 11.0%), and also had higher in-hospital mortality (0.8% vs. 0.4%). Conclusions: Late preterm infants are associated with very high cesarean section rate and have more medical problems and poorer short-term outcomes than term infants in China.
Labor patterns of spontaneous first-stage labor in Chinese women with normal neonatal outcomes

Abstract:Background: Friedman's standards, developed almost 50 years ago, may no longer align with the needs of today's obstetric population and current pregnancy management practices. This study aims to analyze contemporary labor patterns and estimate labor duration in China, focusing on first-stage labor data from Chinese parturients with a spontaneous onset of labor. Methods: This retrospective observational study utilized data from electronic medical records of a tertiary hospital in Changsha, Hunan. Out of a total of 2,689 parturients, exclusions were made for multiple gestations, preterm, post-term, or stillbirth, cesarean delivery, non-vertex presentation, and neonatal intensive care unit admission. Average labor curves were constructed by parity using repeated-measure analysis, and labor duration was estimated through interval-censored regression, stratified by cervical dilation at admission. We performed an analysis to assess the impact of oxytocin augmentation and amniotomy on labor progression and conducted a sensitivity analysis using women with complicated outcomes. Results: Nulliparous women take over 180 minutes for cervical dilation from 3 to 4 cm, and the duration from 5 to 6 cm exceeds 145 minutes. Multiparous women experience shorter labor durations than nulliparous. Labor acceleration is observed after 5 cm in nulliparous, but no distinct inflection point is evident in the average labor curve. In the second stage of labor, the 95th percentile for nulliparous, with and without epidural analgesia, is 142 minutes and 127 minutes, respectively. Conclusions: These findings provide valuable insights for the reassessment of labor and delivery processes in contemporary obstetric populations, including current Chinese obstetric practice.

Abstract:OBJECTIVE:To study the availability of humanized service and healthy birth and it's effect on maternal and neonatal outcomes.METHODS:A multicenteral prospective randomized control study was carried out in 9 maternity hospitals from Nov. 2000 to June 2001. 6,758 laboring women were randomly divided into study group and control group. In study group (n = 3,437), midwives accompany laboring women in whole course of birth until two hours after delivery. According WHO "Monitoring rules for normal birth" medical staff support the mothers in physiological, psychological and physical aspects. In control group (n = 3,321) mothers receive traditional management. Midwives visit and check the mothers discoutinually. In two groups husbands can company laboring women according women's requirement. A questionnaire will be filled on the second day after birth.RESULTS:In study group the spontaneous delivery rate was significant higher than that of control group (85.5% vs 66.0%, P < 0.01). The cesarean section rate, postpartum hemorrhage rate, neonatal asphyxia rate and neonatal morbidity were marked lower in study group than those of control group (P < 0.01 and P < 0.05).CONCLUSIONS:Appropriate mode of intrapartum care can decrease the cesarean section rate and promote the vaginal delivery, also it can improve the maternal and neonatal outcomes.
Impact of Adherence to a Standardized Oxytocin Induction Protocol on Obstetric and Neonatal Outcomes

Abstract:Background: Oxytocin protocols are employed to induce uterine contractions and progressive cervical changes, but they are associated with adverse maternal and neonatal outcomes. The aim of this study was to determine whether compliance with a checklist-based protocol for oxytocin administration was associated with changes in neonatal and maternal outcomes. Methods: A retrospective cohort study of 86,786 pregnant women undergoing term (> 37 weeks) induction of labor between January 2015 and December 2017 was performed. Systemwide training in the use of an oxytocin administration protocol was provided to obstetricians and nurses. Pre-use and in-use oxytocin checklists were incorporated into each unit's policies and procedures. Subsequently, charts were reviewed and individually audited by an obstetric nurse who scored each record based on the documentation of variables in an oxytocin administration protocol and ranked adherence as complete or absent. Primary outcomes were postpartum hemorrhage, neonatal ICU (NICU) admission, and delivery by cesarean section. Bivariate analyses (t-tests) were performed on adherent and nonadherent groups for comparison of selected demographic variables and the primary outcome variables. Logistic regression was completed on the primary outcome variable with eight covariates. Results: Among patients with complete adherence to the oxytocin administration protocol, the rate of cesarean section in the unadjusted analysis was 16.20%, compared to 18.54% for those with incomplete adherence; the rates of postpartum hemorrhage were 2.64% vs. 3.14%, respectively, and the rates of NICU admission were 3.03% vs. 3.86%, respectively. In the multivariable logistic regression, complete protocol adherence was associated with significantly lower odds of postpartum hemorrhage (adjusted odds ratio [OR] 0.85, 95% confidence interval [CI] 0.76-0.94) but higher odds of Cesarean section (adjusted OR 1.07, 95% CI 1.01-1.13); the adjusted OR for NICU admission was 0.90, which did not reach statistical significance (95% CI 0.81-1.00). Among the covariates, nulliparity and elective induction were the strongest predictors of the primary outcomes of cesarean section, postpartum hemorrhage, and NICU admission. Conclusion: Adherence to the oxytocin administration protocol was associated with a decrease in postpartum hemorrhage but an increased risk of delivery by cesarean section.
Development of An Evidence-Based Practice Protocol for Normal Labor and Childbirth Care in Maternity Settings

Abstract:Objective To develop an best-evidence-based practice protocol for normal labor and childbirth care in maternity settings.Methods The draft of evidence-based practice protocol for normal labor and childbirth care was developed by interviewing with key informants from typical maternity settings, identifying key issues in the process of normal labor management and childbirth care, systematically reviewing literature and extracting evidence.The expert consultation was conducted to identify the recommendation grades of the evidence.The final protocol and standard operating procedures were then formulated.Results The evidence-based practice protocol for normal labor and childbirth care in maternity settings contained 6 aspects, including childbirth care principles, the first-stage labor care, the second-stage labor care, the third-stage labor care, newborn care and early postpartum care for women, with a total of 90 pieces of recommendations.Conclusions The development of evidence-based practice protocol for normal labor and childbirth care is scientific and rigorous.It can help to standardize midwives’ practice of normal labor management and childbirth care in China, so as to provide guidance for subsequent clinical implementation and evaluation of this protocol.
Effect of a Text Messaging–Based Educational Intervention on Cesarean Section Rates among Pregnant Women in China: Quasirandomized Controlled Trial (Preprint)

Abstract:BACKGROUND:Consensus exists that appropriate regional cesarean rates should not exceed 15% of births, but China's cesarean rate exceeds 50% in some areas, prompting numerous calls for its reduction. At present, China's 2016 two-child policy has heightened the implications of national cesarean section trends.OBJECTIVE:This study leveraged pervasive cellular phone access amongst Chinese citizens to test the effect of a low-cost and scalable prenatal advice program on cesarean section rates.METHODS:Participants were pregnant women presenting for antenatal care at a clinic in Xi'an, China. Assignment was quasirandomized and utilized factorial assignment based on the expecting mother's birthday. Participants were assigned to one of the following four groups, with each receiving a different set of messages: (1) a comparison group that received only a few "basic" messages, (2) a group receiving messages primarily regarding care seeking, (3) a group receiving messages primarily regarding good home prenatal practices, and (4) a group receiving text messages of all groups. Messages were delivered throughout pregnancy and were tailored to each woman's gestational week. The main outcome was the rates of cesarean delivery reported in the intervention arms. Data analysts were blinded to treatment assignment.RESULTS:In total, 2115 women completed the trial and corresponding follow-up surveys. In the unadjusted analysis, the group receiving all texts was associated with an odds ratio of 0.77 (P=.06), though neither the care seeking nor good home prenatal practice set yielded a relevant impact. Adjusting for potentially confounding covariates showed that the group with all texts sent together was associated with an odds ratio of 0.67 (P=.01). Notably, previous cesarean section evoked an odds ratio of 11.78 (P<.001), highlighting that having a cesarean section predicts future cesarean section in a subsequent pregnancy.CONCLUSIONS:Sending pregnant women in rural China short informational messages with integrated advice regarding both care-seeking and good home prenatal practices appears to reduce women's likelihood of undergoing cesarean section. Reducing clear medical indications for cesarean section seems to be the strongest potential pathway of the effect. Cesarean section based on only maternal request did not seem to occur regularly in our study population. Childbirth and Early Newborn Care Practices in 4 Provinces in China: A Comparison with WHO Recommendations.

Abstract:OBJECTIVES:As a part of the process of implementing Early Essential Newborn Care (EENC) in China, which are evidence-based interventions recommended by the World Health Organization (WHO), we sought to understand whether current hospital policies are consistent with WHO-recommended standards and what factors influence their use. Data from the study will help inform policy changes needed to support the introduction of evidence-based childbirth and early newborn care practices effectively and to inform further scale up of EENC nationwide.METHODS:Ten randomly selected hospitals in 4 early-introducing provinces participated in the study. We collected data from 20 simulated delivery scenario observations and focus group discussions and individual interviews with 10 hospital management staff. Policies, protocols, and guidelines related to childbirth and newborn care practice were also collected and reviewed at each hospital. Additionally, a survey was emailed to 15 childbirth and newborn experts from the 4 selected provinces and completed by 13. Data were compared with WHO EENC evidence-based standards to calculate the agreement rates. Barriers to introducing evidence-based guidelines were identified in focus groups and key informant interviews, then combined into common categories.FINDINGS:Hospital policies were not consistent with WHO recommendations in 10 (59%) of the 17 delivery and early newborn care practices. Delayed cord clamping was recommended by 30% of hospital protocols and prolonged skin-to-skin contact by 13%, neither of which were observed in the delivery simulations. Kangaroo mother care (KMC) for stable preterm babies was required in only 17% of the hospitals; no preterm babies had KMC initiated, with all immediately separated from their mothers and admitted to neonatal intensive care units. Newborn resuscitation equipment was required to be placed within 2 meters of the delivery bed in 84% of hospital protocols, but was prepared in only 40% of cases. Immediate drying after birth was required in 48% of hospital protocols, but was initiated in only 20% of observed cases.CONCLUSIONS:Current childbirth and early newborn care policy and practice in China is not aligned with WHO recommendations for some major interventions. To make it easier and safer for hospital workers to practice EENC, expert working groups and national policies must be established to address inconsistencies and cultural beliefs and provide a strong, evidence-based set of guidelines for hospitals and health workers to follow.

A study on implementation of WHO labour care guide in low-risk pregnant women and its impact on maternal and perinatal outcome

Abstract:Background: WHO Labour Care Guide is a labour monitoring tool that is developed to support respectful maternity to provide positive childbirth experience with evidence-based recommendations.Methods: This is an observational study comparing WHO labour care guide with WHO modified partograph in low-risk pregnant women who are in active phase of labour. The study included 80 labouring women who are divided into two groups with 40 participants in each group. In group 1 WHO modified partograph was used and in group 2 WHO labour care guide was used as a tool to assess the course of labour. Maternal and perinatal outcomes were analysed for both the groups.Results: There is no significant difference in the caesarean section rates and instrumental deliveries between the two groups. There is a rise in the incidence of PPH and meconium-stained liquor in group 2 compared to group 1. None of the participants had labour beyond 12 hours. NICU admissions were higher in group 2.Conclusions: In our study, the WHO modified partograph was found as equivalent to WHO labour care guide in identifying prolonged labor. Also, maternal and perinatal outcomes were equally identified by both types of partograph. WHO labour care guide did not prove to be beneficial over WHO modified partograph in terms of maternal and perinatal outcomes in Indian parturient women.
Establishment and evaluation of a standardization management model for pregnant women after caesarean sections

Abstract:Objective To establish a midwife-obstetrician collaboration-based management model for pregnant women after caesarean sections and to evaluate its effectiveness on women′s childbirth outcomes. Methods A panel of experts including obstetricians and midwives was established. A revised management scheme for women after cesarean sections was finally formed through multiple expert consultation method. A historical control method was implemented in our study. Women giving birth before the implementation of collaboration-based management scheme between 2011 and 2013 were recruited in the control group;and women giving birth afterwards between 2014 and 2016 were recruited in the intervention group. Childbirth outcomes such as the mode of delivery, rate of trial of labor after cesarean sections, and vaginal birth rate after cesareans were compared between the two groups. Results A total of 3326 women in intervention group, of those women 281 (8.4%) under trial of labor after cesarean, of the women who under trial of labor 264(94.0%) had successful vaginal birth. A total of 1625 women in control group, of those women 28(1.7%) under trial of labor after cesarean, of the women who under trial of labor 22(78.6%) had successful vaginal birth. The trial of labor after cesarean sections rate and the vaginal birth rate after cesareans rate was significantly higher in intervention group than that in control group (P<0.01). Conclusions The midwife-obstetrician collaboration-based management model can promote vaginal birth for women after cesarean sections, and has no adverse effect on other childbirth outcomes, which is worthy of being implemented more widely in China.

Abstract:Objective To explore the effects of whole-stage labor analgesia on maternal and neonatal outcomes after the implementation of the new partogram.Methods A retrospective cohort study was performed in Peking University First Hospital.From June to December 2016,907 nulliparae with full-term singleton cephalic pregnancies that were expected to deliver vaginally and adhered to the new partogram were selected as the study group,and 982 cases that adhered to the old partogram (Friedman standard) from June to December 2012 were selected as the control group.Maternal basic characteristics and analgesic,obstetric and neonatal data were collected.Maternal and neonatal outcomes between the two groups were analyzed with independent sample t-test,rank sum test or Chi-square test (Fisher's exact test).Results (1) Maternal age and the proportion of gravidas of advanced maternal age in the study group were both higher than those in the control group [(30.2±3.0) vs (29.64-2.9) years,t=3.823;8.2％ (74/907) vs 4.2％ (41/982),x2=13.087;both P＜0.001].No statistically significant difference in the other basic characteristics was found between the two groups.(2) Women in the study group had significantly smaller cervical dilatation [M(P25-P75)] than that in the control group when analgesia was commenced [2(1-2) vs 2(1-3) cm,Z=-3.752].The intensity of pain quantified with numerical rating scale (NRS) before analgesia,and during the second stage of labor in the study group were less than that in the control group [8(8-9) vs 8(8-10) points,Z=-14.441;5(4-5) vs 6(5-7) points,Z=-16.495].The study group had longer median duration of analgesia than the control group [520(340-750) vs 300(200-453) min,Z=-17.801,P＜0.001].The overall dose of analgesics in injection pump in the study group was significantly higher compared with that used in the control group [68 (35-84) vs 30 (18-48) ml,Z=-18.004,P＜0.001].However,there was no significant difference in the incidence of analgesia-related complications (hypotension,accidental dura puncture,lower extremity numbness,pruritus and urinary retention) between the two groups.(3) The study group showed a higher percentage of spontaneous vaginal delivery,and a lower rate of converted cesarean section compared with the control group [71.8％ (651/907) vs 63.2％ (621/982),x2=15.623;13.6％ (123/907) vs 20.5％ (201/982),~=18.831;both P＜0.001].The study group had lower forceps-assisted vaginal delivery rate than the control group without statistically significant difference [14.8％ (133/907) vs 15.3％ (150/982),x2=0.093,P=0.797].The duration of the first,second and third stage of labor in the study group were all significantly longer than that of the control group [680 (470-900) vs 480 (360-660) min,Z=-12.490;56 (31-89) vs 37 (24-58) min,Z=-9.964;7 (5-10) vs 6 (5-8) min,Z=-6.673;all P＜0.001].Women in the study group had a lower rate of artificial rupture of membranes when comparing with the control group [55.2％ (501/907) vs 63.2％ (621/982),x2=12.516,P＜0.001].The incidence of fever,pathologically confirmed infection and postpartum hemorrhage were significantly higher in the study group than that in the control group [10.8％ (98/907) vs 6.4％ (63/982),x2=11.652;9.6％ (87/907) vs 3.7％ (36/982),x2=27.201;12.6％ (114/907) vs 5.7％ (56/982),x2=27.144;all P＜0.05].There was no significant difference in the rate of oxytocin use during labor or blood transfusion between the two groups.(4) Compared with the control group,the proportion of cesarean section due to arrested active stage of labor in the study group was significantly lower [5.7％ (7/123) vs 50.2％ (101/201),x2=68.173,P＜0.05],but the incidences of cesarean section due to intrauterine infection and relative cephalopelvic disproportion were higher in the study group [61.0％ (75/123) vs 30.8％ (62/201),x2=2.680;20.3％ (25/123) vs 8.0％ (16/201),x2=10.555;both P＜0.05].There was no significant difference in the proportion of fetal distress or other indications for cesarean section between the two groups.(5) There was no significant difference between the two groups in birth weight,macrosomia,fetal distress,neonatal asphyxia,or the proportion of neonatal intensive care unit admission.Conclusions After the implementation of the new partogram,analgesic pain relief covering the whole labor prolongs the average analgesic time and reduces the rate of cesarean section due to arrested active stage of labor.Although the risk of postpartum hemorrhage is slightly increased,analgesia itself does not pose additional risks on forceps-assisted delivery,maternal blood transfusion and neonatal asphyxia.
Implementation of Normal Childbirth Guidelines among Midwives: Adherence and Influencing Factors.

Abstract:BACKGROUND:Clinical practice guidelines on normal childbirth were issued worldwide with a view to achieve evidence-based, cost-effective and universal intrapartum care. Effective implementation of guideline recommendations is impossible without a full evaluation and understanding of current practice and factors influencing adherence among midwives.AIM:This study aimed to explore midwives' adherence to clinical practice guidelines on normal childbirth and factors influencing the implementation of guideline recommendations in mainland China.DESIGN:We used a mixed-method sequential explanatory design.METHODS:A national level cross-sectional survey was conducted among 718 midwives to investigate their adherence to guideline recommendations on normal childbirth. Based on the findings of quantitative study phase, we developed the interview outline and performed semi-structured interviews with thirteen midwives to explore potential factors influencing their implementation of normal childbirth guidelines. An inductive thematic analysis was undertaken to identify themes, which were then deductively mapped to the Theoretical Domains Framework (TDF).RESULTS:Midwives' adherence to the guideline recommendations was relatively low, with non-adherence to thirteen guideline recommendations (41.94%) being observed. Six domains on the TDF and one additional theme were found to be factors influencing midwives' implementation of guideline recommendations on normal childbirth, with behavioural regulation, beliefs about consequences, professional roles and responsibilities being identified as barriers for interventions recommended against use, knowledge, environmental context and resources being identified as enablers for interventions recommended for use, and skills and women's preference being identified as barriers/enabler for both.CONCLUSION:Guideline adherence can be improved by multifaceted efforts at professional, organizational and maternal levels. The identification of barriers and enablers of guideline implementation provides a solid foundation for further reducing non-evidence-based intrapartum interventions.
Effects of Integrating Nursing Early Warning Systems with Doula-Assisted Childbirth on Natural Childbirth Rates and Neonatal Outcomes: A Single Center Prospective Study

Abstract:Objective: This study aims to evaluate the efficacy of integrating nursing early warning systems with doula-assisted childbirth nursing on natural childbirth rates and associated outcomes. Methods: A total of 150 women who underwent childbirth in the obstetrics and gynecology department of our hospital between September 2021 and March 2023 were enrolled as participants. They were randomly allocated into either the observation group or the control group, each comprising 75 individuals. The control group received standard nursing care, while the observation group received nursing early warning systems combined with doula-assisted childbirth nursing. Comparison between the two groups included modes of childbirth, the intensity of childbirth pain, neonatal Apgar scores, the incidence of postpartum hemorrhage, duration of the first and second stages of labor, and maternal satisfaction with nursing care. Results: Our findings indicated a higher natural childbirth rate in the observation group compared to the control group (P < .05). Furthermore, the SAS score of the observation group post-nursing was significantly lower than that of the control group. Additionally, neonates in the observation group exhibited higher Apgar scores compared to those in the control group. Moreover, mothers who underwent natural childbirth in the observation group experienced lower childbirth pain scores and reduced postpartum hemorrhage incidence (P < .05). The duration of both the first and second stages of childbirth was shorter in the observation group than in the control group. Furthermore, overall satisfaction with care was significantly higher in the observation group than in the control group, with a statistically significant difference (P < .05). Conclusions: This study underscores the effectiveness of implementing nursing early warning systems combined with doula-assisted nursing in enhancing maternal satisfaction, reducing postpartum hemorrhage, and improving maternal well-being and neonatal outcomes. The findings advocate for the widespread adoption of this intervention in clinical practice.
Implementation of the Practice Programme for Upright Positions in the Second Stage of Labour and the birth experience of Chinese women: A qualitative study

Abstract:Objective: Upright positions in the second stage of labour are recommended by many international organizations. However, they have not been widely used worldwide, especially in China. One of the important factors is the absence of a practice programme based on the best available evidence. We thus developed a Practice Programme for Upright Positions in the Second Stage of Labour following the UK Medical Research Council framework. Under the guidance of the programme, whether the use of upright positions can improve the maternal birth experience is a question of great concern. This study aimed to explore the birth experience of Chinese women who adopted upright positions in the second stage of labour. Design: This qualitative descriptive study was conducted as part of an implementation study that developed an evidence-based intervention and used strategies to integrate the evidence-based intervention into routine obstetric clinical practice. Setting: The maternity department of a tertiary comprehensive hospital in Hebei Province, China. Participants: Semi-structured interviews with twelve eligible women who adopted upright positions in the second stage of labour were conducted between March and April 2022. Qualitative data were analyzed by using conventional content analysis. Findings: The average age of included women was 26.5 ± 3.5 years, and ten of them were primiparous women. Eight women adopted epidural analgesia during labour to relieve labour pain. All women gave birth in at least one type of upright position in the passive second stage of labour and adopted the semi-recumbent position in the active second stage of labour. Through conventional content analysis, we found that the use of upright positions in the second stage of labour could possibly promote an overall positive birth experience. Women giving birth in upright positions generally perceived they were more involved in their birthing process, and had greater physical and mental capacity to cope with childbirth. Key conclusions: Women have a positive birth experience when using upright positions in the second stage of labour. Implications for practice: This study suggests upright positions could improve women's birth experience and have the potential to be widely applied in clinical practice.
The influence of normalized postoperative abortion services on contraceptive behavior of women

Abstract:Objective To study the impact of standardizing post -abortion services on women's contraceptive behavior .Methods The 1200 women, those who received abortion in Shenzhen hospital affiliated to Peking university during January 2014 and June 2014, were enrolled in this study .They were randomly divided into experimental group and control group , 600 cases in each group .Women in experimental group were given standardizing post -abortion service , however , the women in control group were given routine education measures .The patients were surveyed immediately on contraceptive use ( one day post-procedure ) , and followed-up to understand use of contraception persistently at 3 months, 6 months and 12 months after the operation . Results The rate of contraceptive use at different time point in experimental group were 95.33%, 85.67%, 70. 82%and 52.83% respectively, which were significantly higher than those of the control group (4.67%, 56. 83%, 45.33%and 29.00%), the differences were statistically significant (P<0.05).In the control group, unmarried women , those with junior high school education or less , factory or service workers , and those with an average income less than 3000 yuan had fewer consistent contraceptive use , differences were statistically significant (P<0.05).Conclusions Standardized abortion services can improve the implementation rate of female contraceptives use , and post -abortion service efforts should be focused on low income , low education level and unmarried transient population .

Abstract:Background: Antenatal care is an important component of maternity care. In many parts of the world, midwives are the primary caregivers for childbearing women, providing a high level of continuity of care during a normal pregnancy. While in China, obstetricians are the primary providers of antenatal care for all childbearing women; and midwives only provide intrapartum care to labouring women. Today midwifery as a profession in China has been marginalised. Pregnant women usually lack individualised continuity of care from midwives during the perinatal period. There have been few randomised controlled trials of midwifery care practice in mainland China.Objective: (1) To develop and implement a model of Chinese midwives' antenatal clinic service and (2) to explore its effect on childbirth outcomes, psychological state and satisfaction, for primiparae.Design and methods: Two-group randomised controlled trial. One hundred and ten pregnant women were assessed for eligibility and invited to participate in either the intervention group (midwives' antenatal clinic service) or the control group (routine antenatal care) in the Obstetrics and Gynaecology Hospital of Fudan University from September 2011 to December 2011. Baseline data were collected, and then women were randomised to individual midwives' antenatal clinic care (intervention group) or regular antenatal clinic service by obstetricians and obstetric nurse (control group). The research hypothesis was that compared with regular obstetrician-led antenatal care, the midwives' antenatal clinic service would decrease the caesarean section rate, produce more favourable birth outcomes and women's greater satisfaction with care. Data were collected by retrospective review of case records and self-report questionnaires. The sample size of 110 was calculated to identify a decrease in caesarean birth from 70% to 40%. Birth outcomes, satisfaction and anxiety score in the two groups were compared.Setting: The midwives' antenatal clinic in the Obstetrics and Gynaecology Hospital of Fudan University, Shanghai, China. Participants: 55 women, attending the midwives' antenatal clinic (the intervention group) and 55 women, entering the control group.Results: Women in the intervention group were more likely than women in the control group to have a vaginal birth (35 [66.04%] versus 23 [43.40%]; 95% Cl for difference 3.69-41.60). Women in the intervention group had a higher perinatal satisfaction but lower anxiety score than those in the control group. No differences were seen in neonatal Apgar score and in the amount of bleeding 2 h post partum.Conclusion and implications for practice: The midwives' antenatal clinic can decrease the rate of caesarean section and enhance women's satisfaction with midwifery care. Further research needs to be conducted to implement this model of care more widely. We will attempt to make midwifery care a true choice for Chinese women. (C) 2013 Elsevier Ltd. All rights reserved.
Maternal characteristics and birth outcomes under different family planning policy

Abstract:Objective To assess the potential impact of recent family planning policy adjustments on maternal characteristics and birth outcomes in China.Methods Pregnant women who delivered in six hospitals in Hubei and Gansu provinces in 2013-2016 were surveyed.The hospitals included one tertiary and two secondary hospitals in each province.Information on major maternal characteristics and birth outcomes were extracted from hospital delivery registration form.Three phases were categorized based on the implement of family planning policy:One-Child Policy period refers to January,2013 to September,2014;Selective Two-Child Policy period refers to October,2014 to July,2016 and Universal Two-Child Policy period refers to August,2016 to December,2016,and the secular trends in the major maternal characteristics and birth outcomes were described accordingly.Results In total,29 895 women (30 532 neonates) were included in the analysis.The overall percentage of women with advanced age (aged ≥35 years) was 9.6％,and 39.0％ were multiparous.The overall prevalence rate of preterm delivery and low birth weight was 9.0％ and 7.7％,respectively.Over the three phases,the percentage of women with advanced age increased significantly from 8.6％ to 9.6％ and it's 12.3％ in the Universal Two-Child Policy period,and the percentage of multiparous women was 34.9％,39.7％ and 46.9％ respectively.Over the study period,there were no significant increase in the prevalence of preterm delivery (9.5％,8.4％ and 9.7％),low birth weight (7.5％,7.9％ and 7.6％),and the mean 1-minter Apgar scores [(8.7 ± 0.9),(8.8 ± 1.0),(8.9 ± 1.5)].Further stratified by the institutions,compared with One-Child Policy period,there was a 24.3％～97.4％ and 28.1％～75.2％ increase in the percentage of women with advanced maternal age at the last period in 2 tertiary and 2 secondary hospitals respectively.While the remaining two midwifery institutions (one of them has the annual delivery of around 100 cases,the other was close to tertiary midwifery institutions) has no corresponding upward trend.In all the 6 hospitals,there were no increases in the rate of preterm delivery and low birth weight,and no decreases in the mean 1-minter Apgar scores over the study period.Conclusion Maternal characteristics have changed substantially after relaxation of the One-Child policy,especially after implementation of the universal Two-Child Policy,typically characterized by a substantial increase in the percentage of women with advanced age and that of multiparous women.However,no evidence showed the increase in the prevalence of preterm delivery and low birth weight,and decreases in the mean 1-minter Apgar scores.The safety of mothers and neonates has been effectively guaranteed in the early stage of family planning policy adjustments,although the number of high risk pregnancies increases after adjustment of family planning policy.However,follow up study was still necessary for continued monitoring the potential impacts of the Universal Two-Child Policy,in order to optimize allocation of health resources and guarantee the safety of mothers and neonates.
