Abstract:PurposeTo compare the survival benefit, pain control and safety of low-power cumulative and traditional high-intensity focused ultrasound (HIFU) for metastatic pancreatic cancer.MethodWe retrospectively analyzed 55 patients with metastatic pancreatic cancer who received HIFU treatment between January 2008 and April 2014 in our department. 23 patients received low-power cumulative HIFU treatment (L group), 32 received the traditional HIFU treatment (T group). Performance status, cancer-related pain and serum biochemistry results were assessed before and after treatment. All patients were followed up until death. The survival rate and adverse events of the two groups were compared.ResultsThe baseline characteristics of the two groups were generally well balanced (p > 0.05). The average KPS score after treatment was significantly improved in both groups compared with the baseline score. 36 patients exhibited tumor-related pain at baseline. The pain response rate was significantly higher in the L group (92.3%) than in the T group (52.2%) (p = 0.025). The median overall survival (OS) for the L group was 7.0 months, which was significantly longer than that of the T group (p = 0.000). The 3-month and 6-month survival rates were higher in the L group. The adverse events in both groups included abdominal pain, elevated C-reactive protein (CRP) and elevated amylase. The incidence was lower in the L group than in the T group.ConclusionCompared with traditional HIFU treatment, low-power cumulative HIFU treatment showed a significantly higher pain relief rate and survival benefit with a better safety profile in patients with metastatic pancreatic cancer.

Abstract:This study reports a case of an advanced pancreatic cancer patient with liver metastasis who was treated with a combination of chemotherapy, targeted drugs, and high-intensity focused ultrasound (HIFU). The abdominal pain was successfully relieved after the HIFU therapy. The patient had an 18-month survival with satisfactory quality of life. Large-scale randomized clinical trials are necessary to evaluate the long-term efficacy of HIFU treatment with or without chemotherapy and/or targeted drug therapy for pancreatic carcinoma.

Abstract:Present study was designed to investigate the pancreatic endocrine and exocrine function damage after High Intensity Focused Ultrasound (HIFU) therapy in patients with advanced pancreatic cancer. It was a retrospective analysis of blood glucose and amylase changes in 59 advanced pancreatic cancer patients treated with HIFU from 2010 February to 2014 January. The mean glucose and amylase before HIFU treatment were 6.02mmol/L and 59.17 U/L respectively. After HIFU treatment, it was shown that the mean glucose and amylase levels were 5.66mmol/L and 57.86/L respectively. There was no statistical significance between them. No acute pancreatitis was observed. The endocrine and exocrine function of pancreatic cancer patients was not damaged by HIFU treatment. HIFU treatment for the pancreatic cancer patients seems to be safe.

Abstract:e15242 Background: To compare the safety and efficacy of concurrent chemotherapy with or without high intensity focused ultrasound (HIFU) in patients with local advanced pancreatic cancer or metastatic Pancreatic Cancer. Methods: A total of 498 patients were included from October 2002 to November 2013. 248 patients were treated with concurrent chemotherapy with HIFU, including 152 male and 96 female patients, aging from 31-91 years, the mean age was 65 years. 203 patients was diagnosed with metastatic pancreatic cancer, 45 patients with local advanced pancreatic cancer; 250 patients were treated with chemotherapy, including 144 male and 106 female patients, aging from 28-88 years, the mean age was 65 years. 212 patients was diagnosed metastatic pancreatic cancer, 38 patients with local advanced pancreatic cancer. Results: Objective response rate was 22.6%(56/248) in concurrent chemotherapy with HIFU group and 16.0%(40/250) in chemotherapy group, disease control rate was 75.8%(188/248) in concurrent chemotherapy with HIFU group and 69.2%(173/250) in chemotherapy group respectively. No significant differences in overall survival(OS;8.67 vs 7.9 months,P>0.05) between the concurrent chemotherapy with HIFU and chemotherapy groups were found in the overall population. There were no severe complications or adverse events related to concurrent chemotherapy with or without HIFU in any of the patients treated. Conclusions: Although this study may have limitations, preliminary results demonstrate the safety of clinical application of HIFU concurrent with chemotherapy for pancreatic cancer and reveal it to be a promising mode of treatment for local advanced pancreatic cancers.
Pain Management of Pancreatic Cancer Patients with High-Intensity Focused Ultrasound Therapy.

Abstract:This study was performed to evaluate cancer pain control of high-Intensity focused ultrasound ablation (HIFU) and to manage the HIFU treatment pain in advanced pancreatic cancer patients with analgesics. We collected 71 locally advanced pancreatic cancer patients treated with HIFU from 2013 March to 2014 January in our hospital. The cancer pain (pre-HIFU and two weeks after HIFU) and HIFU treatment pain were evaluated respectively. The numeric rating scale (NRS) was used as the tool of pain evaluation. The related factors with pains were analyzed. The 70.42% cancer painless rate before HIFU was improved to 92.96% (P<0.05) 2 weeks after HIFU in 71 advanced pancreatic cancer patents without analgesics adjustment. The HIFU treatment pain occurred in 42 of 71 treated patients (59.15 %). The average duration was 3.93 days and pain score was 3.22. HIFU can improve cancer pain relief further in the advanced pancreatic cancer patients with third ladder analgesics, meanwhile HIFU treatment pain can be managed easy because of its short duration and low pain score.

Abstract:Objective To evaluate the efficacy of high intensity focused ultrasound ( HIFU) in combination with S-1 in treat-ment of advanced pancreatic carcinoma. Methods A total of 120 patients with unresectable advanced pancreatic carcinoma were ran-domly assigned into group A( n=61) and group B( n=59) . All patients received oral S-1, Oral S-1 was administered at 80mg/day for body surface area( BSA) less than 1?25m2 , 100mg/day for BSA 1?25-1?5m2 , and 120mg/day for BSA more than 1?5m2 on days 1-14 every 3 weeks. Only group A received HIFU in combination with S-1. The response evaluation criteria for solid tumor( RECIST) 1?0 were adopted to evaluate recent curative effect and calculate the response rate( RR) and disease control rate( DCR) . The toxicity was assessed using NCI CTC 3?0. The pain relief rate was analyzed and patients were followed up. Results All patients were included in the objective response evaluation. The treatment achieved CR in 3 cases, PR in 15 cases, SD in 28 cases and PD in 15 cases of group A and CR in 1 case, PR in 5 cases, SD in 15 cases and PD in 38 cases of group B. The RR and DCR in group A were 29?5% and 75?4%, higher than 10?2% and 35?6% in group B with significant difference( P<0?05) . The median overall survival time was 11?2 months and 6-, 12-months survival rates were 80?0% and 40?0% in group A, were superior to 7?9 months, 70?0% and 10?0% in group B with significant difference(P<0?05). The pain relief rate in group A was 57?4%(35/61), higher than 20?3%(12/59) in group B with significant difference( P<0?05) . No significant difference was observed on grade 3-4 toxicities between both groups. No significant difference was observed on grade 1-2 toxicities, including anemia, neutropenia, thrombocytopenia, nausea, diarrhea, hand-foot syndrome and neurotoxicity( P>0?05) . Conclusion The HIFU in combination with S-1 shows a favorable benefit and can be well tolerated in patients with advanced pancreatic carcionoma.

Abstract:Objective To evaluate the clinical effect and safety of high intensity focused ultrasound (HIFU) for the treatment of advanced pancreatic carcinoma. Methods 21 cases of advanced pancreatic carcinoma were treated by FEP BY01 pyrotherapier (HIFU). Results After HIFU, abdominal pain was relieved in 88% of patients (15/17), ultrasonic echo of carcinoma tissue was enhanced in all patients, the bloodstream decreased significantly or disappeared, the tumor size and CT value had no significant change, PET examination showed that carcinoma was inactivated effectively, although the tumor size as evaluated by CT scan did not experience a shrinkage. There were no complications such as skin burns、pancreatic fistula、bleeding、 pancreatitis and perforation during the process. The mean survival time of the patients after HIFU was 7 6 months. Conclusion HIFU as a new local treatment effects a satisfactory palliation for patients with advanced pancreatic carcinoma.
Early Clinical Experience Using High Intensity Focused Ultrasound for Palliation of Inoperable Pancreatic Cancer

Abstract:Objective To evaluate the safety and efficacy of high intensity focused ultrasound for palliation of inoperable pancreatic cancer in humans. Patients Eighty-nine patients with advanced pancreatic cancer were treated with high intensity focused ultrasound. There were 4 patients with stage II, 39 patients with stage III, and 46 patients with stage IV disease. The location of the tumors was as follows: head of pancreas 34 patients, body and/or tail of pancreas 55 patients. Methods Pain relief, local tumor control rate, median survival and complications were observed after high intensity focused ultrasound treatment. Results In the clinical treatments in humans the following local tumor control was seen: complete response, 0%; partial response, 14.6%; no change, 57.3%; progressive disease, 28.1%. Pain relief was achieved in 80.6% of patients who had pain prior to high intensity focused ultrasound therapy. The median survival was 26.0 months for patients with stage II disease, 11.2 months for patients with stage III disease, and 5.4 months for patients with stage IV disease. One-year survival rate was as follows: stage II, 100%; stage III, 41.0%; and stage IV, 6.5%. Two-year survival rate was as follows: stage II, 75.0%; stage III, 10.3%; and stage IV, 0%. Complications included superficial skin burns (3.4%), subcutaneous fat sclerosis (6.7%), and an asymptomatic pancreatic pseudocyst (1.1%). There were no severe complications or adverse events related to high intensity focused ultrasound therapy seen in any of the patients treated. Conclusions Although this retrospective study has significant limitations, preliminary results suggest that the clinical application of high intensity focused ultrasound for pancreatic cancer appears to be safe and is a promising modality of treatment for palliation of pain related to pancreatic cancer.

Methods: Patients with pancreatic cancer TNM stage III or IV were included. Magnetic resonance imaging was performed 2 weeks before and after the HIFU. The ablating tumor volume was calculated by ratio of the non-perfused necrotic area of the planned area on contrast-enhanced T1-weighted image on post-HIFU magnetic resonance imaging. The ablation results were stratified into 4 ranges: 100% to 90% unenhanced area of targeting area, 90% to 50%, within 50%, and no change. Results: High-intensity focused ultrasound treatment was performed without severe adverse event in 52 patients, 53 times (male/female = 28/24; mean age, 62.7 ± 12.1; TNM stage 3/stage 4 = 21/31). Average size of the pancreatic cancer lesion was 4.5 ± 1.6 cm (1.4 - 9.5 cm). After HIFU treatment, ablating tumor volume was as follows: 90% to 100% in 41 lesions, 90% to 50% in 11, and within 50% in 3. Overall median survival (S1) from initial pancreatic cancer diagnosis was 13.1 months. Overall survival (S2) rates at 6, 12, and 18 months from HIFU were 53.1%, 31.6%, and 23.5%, respectively, with a median survival of 7.4 months. Conclusions: High-intensity focused ultrasound is safe and effective, which induced excellent local tumor control in most patients with advanced pancreatic cancer.

Overall survival (S2) rates at 6, 12, and 18 months from HIFU were 52.2%, 30.4%, and 21.79%, respectively, with a median survival of 7.0 monthsCONCLUSIONS: High-intensity focused ultrasound is safe and effective, which induced excellent local tumor control in most patients with advanced PC.

Abstract:Objective To evaluate the analgesic efficacy and effect of high intensity focused ultrasound (HIFU) in the treatment of advanced pancreatic cancer.Methods 128 patients with pancreatic cancer ( Ⅲ stage 41 case,Ⅳ stage 87 case) were treated by HIFU.According to some parameters,such as the clinical symptoms,imaging examinations and survival analysis to assess the efficacy of HIFU treatment.Results 22.9％ patients’ serum Ca19-9 decreased.The cancer pain was relieved in 72.5％ patients.Coagulated-necrosis by CT/MRI examination was observed.PR,SD and PD of all were 11.7％,70.3％ and 18.0％ respectively.1-year survival rate was 16.7％ for all patients,and median survival time was 7.0 months.1-year survival rate and median survival time for Ⅲ stage group were 28.5％ and 9.0 months,that of 10.8％ and 6.0 months for Ⅳ stage group respectively.Conclusion HIFU may benefit to advanced pancreatic cancer patient by stabilizing tumors,relieving pain and prolonging life expectancy with less side effects.

Abstract:Introduction: This study was performed to analyze the safety of high-intensity focused ultrasound (HIFU) for treating pancreatic cancer. Methods: 224 cases with advanced pancreatic cancer were enrolled into this study. Real-time sonographic images were taken, and vital signs, liver and kidney function, skin burns, local reactions, and systemic effects were monitored and recorded before, during, and after HIFU. Computed tomography or magnetic resonance imaging (MRI) was also performed before and after HIFU. Results: Serum amylase level increased in 16 cases (7.1%) 1 day after HIFU treatment, and 9 of these cases also had abnormal urinary amylase levels. Gastrointestinal (GI) dysfunction such as abdominal distension and anorexia with slight nausea was observed in 10 cases (4.5%) after HIFU treatment. 1 case with pancreatic head cancer developed obstructive jaundice 2 weeks after HIFU treatment. Vertebral injury, identified by MRI, occurred in 2 cases, although no symptoms were seen. No severe complications such as skin burns, lesion bleeding, GI tract bleeding or GI perforation were observed in any of the cases. Conclusion: For specific patients, HIFU treatment is a safe, non-invasive treatment for pancreatic cancer but requires careful preoperative preparation and exact operative performance.

Pain relief, local tumour control rate, median survival and complications were monitored after HIFU treatment. The primary endpoint was to assess pain relief rate and pain relief time (PRT). Secondary endpoints included local progression-free survival time, overall survival (OS), and side effects.RESULTS:There were no severe complications or adverse events related to HIFU therapy in any of the patients treated. Pain relief was achieved in 87.5% of patients, median PRT was 10 weeks. The median local progression-free survival time for all patients was 5 months. The median overall survival time was 10 months for patients with stage III disease, and 6 months for patients with stage IV disease. The median OS time, 6-month and 1-year survival rate for patients as a whole were 8 months, 58.8% and 30.1%, respectively.CONCLUSIONS:Although this study may have limitations, preliminary results demonstrate the safety of clinical application of HIFU for pancreatic cancer and reveal it to be a promising mode of treatment for palliation of pain associated with pancreatic cancers.
Real-world Response of Patients with Locally Advanced Pancreatic Adenocarcinoma to High Intensity Focused Ultrasound Treatment: a Single-Center, Observational Study in China

Abstract:Background: The purpose of this study was to evaluate the feasibility of high intensity focused ultrasound (HIFU) ablation for tumor response and local control in selected patients with unresectable locally advanced pancreatic cancer (LAPC). Methods: Forty-six malignant cases of unresectable pancreatic cancer were treated in our center from February 2013 to December 2016, and were included in this analysis. Response was determined with the response evaluation criteria in solid tumors (RECIST v1.1). Overall survival (OS), including 12-month survival rates and complications were also recorded and analyzed. Results: Clinical responses (ablation obtained) were observed in 43.48% of the cases, and disease control [complete response (CR), partial response (PR), and stable disease (SD)] was observed in 43 (93.48%) of 46 cases. Stratified analyses suggested that receiving radiotherapy per-HIFU might affect the response rate. Median OS was 9.7 months, with 95% CI, 7.9–13.2 months. The 1-year survival rate was estimated to be 46.69% (95% CI, 31.71–60.34%). Age <65 years, tumors located in the tail of pancreas, stage III, receiving targeted therapy per-HIFU, time to HIFU from diagnose <2 months, and receiving HIFU therapy for more than 10 times was detected to be significant associated with the improvement of median survival time (MST) and 1-year survival rate by univariate analysis. However, multivariate analysis did not support the association. Major complications included fatigue [2], abdominal pain [2], fever [1], GI bleeding [1]. No deaths were registered during the course of the treatment. Conclusions: HIFU is a potentially effective and safe modality for the treatment of malignant tumors. HIFU proves to have a survival advantage in treating unresectable pancreatic cancer.

Abstract:Objective To evaluate the analgesic efficacy and effect of high-intensity focused ultrasound (HIFU) in the treatment of pancreatic cancer.Methods 29 patients with pancreatic cancer were treated by HIFU.According to some parameters,such as the clinical symptoms,the basis of main organs functional tests and imaging examinations to assess the efficacy of HIFU.Moreover,we compared dose intensity and efficiency of HIFU under general anesthesia or sedation-analgesia.Results 68.2% patients' (15/22) serum Ca19-9 decreased.The cancer pain was relieved in 69.2% patients(18/26).Coagulated-necrosis by CT/MRI examination was observed.1-year survival rate was 23.1%.Median survival time was 8.5 months.There was no obvious difference (P>0.05) in dose intensity of HIFU under two anesthetic methods.The efficiency of HIFU under sedation-analgesia was higher than that under general anesthesia (P<0.05).Conclusions Extracorporeal HIFU can effectively ablate pancreatic cancer and relieve pain.The mode sedation-analgesia is better applied for HIFU treatment with pancreatic cancer.
An Experimental Study of High Intensity Focused Ultrasound on Pig's Pancreas and the Early Clinical Experience on Pancreatic Cancer

Abstract:Objective: To investigate the feasibility and safety of high intensity focused ultrasound (HIFU) treatment of in vivo pig pancreases, and to evaluate the safety and efficacy of HIFU in the clinical treatment of pancreatic cancer in humans. Methods: HIFU was performed in 12 domestic pig pancreases in vivo with varying acoustic energies. The safety of HIFU treatments was assessed by necropsy. The pathology and microstructure of the treated pancreases were evaluated using standard histology and transmission electron microscopy. Following the animal studies 62 patients with advanced pancreas cancer were treated with 250 420 W of acoustic power. There were 3 patients with stage 11, 23 patients with stage 111, and 36 patients with stage IV disease. Results: In animal studies, precise regions of coagulation necrosis were identified on pathology in 8 specimens that were treated with 420 W or 645 W acoustic power. Treatment effects were unable to be identified in 4 specimens treated with 300 similar to 340 W acoustic power; however, damages to the cells microstructure and apoptosis were identified on electron microscopy. Damage to the stomach and colon were seen in some animals treated with 645 W. In the clinical treatments in humans the following were seen: local tumor control: complete response (CR) 0%, partial response (PR) 17.7%, no change (NC) 54.8%, progressive disease (PD) 27.5%. Pain relief was achieved in 87.1% of patients. The median survival for stage II and III patients was 11.2 months and median survival for stage IV patients was 5.6 months. The total median survival was 8.6 months. The survival rate at I year was as follows: stage II and III 42.3%, stage IV 5.6%. The survival rate at 2 years was as follows: stage 11 and III 15.4%, stage IV 0%. There were no severe complications or adverse events related to HIFU therapy seen in any of the patients treated. Conclusions: This study supports the feasibility of HIFU in the treatment of pancreatic cancer. The clinical application of HIFU to pancreas cancer was safe and effective especially in palliating pain. xiongliulin@sina.com
Sequential High-Intensity Focused Ultrasound Treatment Combined with Chemotherapy for Inoperable Pancreatic Cancer: a Retrospective Analysis for Prognostic Factors and Survival Outcomes

Abstract:Objective: To evaluate the effect of HIFU (High-Intensity Focused Ultrasound) therapy on the survival and prognosis of patients with inoperable pancreatic cancer, and the clinical application of serological prognostic indicators.Methods: We retrospectively analyzed the clinicopathological features, laboratory tests and follow-ups of 192 patients. Among the patients, 57 were treated with HIFU prior to chemotherapy (HIFU-priority), and 135 patients received chemotherapy followed by HIFU (HIFU-second). Univariate and multivariate Cox regression analysis was used to determine the prognostic value of tumor inflammation-related serological markers. A nomogram model was established based on the identified prognostic factors.Results: Univariate analysis showed that receiving the treatment regimen in HIFU-priority was a significant protective factor for overall survival (OS, p < 0.001). Tumor stage, high C-reactive protein (CRP), high gamma-glutamyl transferase(gamma GT) high carbohydrate antigen 125 (CA125), high neutrophil-to-lymphocyte ratio (NLR), high lymphocyte-to-monocyte ratio (LMR) and liver metastasis were significant risk factors for poor prognosis (p < 0.05). CRP combined with normal tumor marker CA125 (CRP + CA125) was associated with longer OS (p = 0.005). Multivariate analysis shows that HIFU-priority is a protective factor for OS (Hazard Ratio, HR: 0.38; 95% confidence interval(CI): 0.25-0.57), tumor stage (HR: 1.61; 95% CI: 1.12-2.31), CRP + CA125 (HR: 1.46; 95% CI: 1.02-2.08) and gamma GT (HR: 1.44; 95% CI: 1.04-1.98) are risk factors for OS and serve as independent prognostic factors in the nomogram.Conclusion: Early application of HIFU treatment improves the OS of patients with inoperable pancreatic cancer. CRP + CA125 and gamma GT are independent prognostic factors.
