Abstract:This study reports a case of an advanced pancreatic cancer patient with liver metastasis who was treated with a combination of chemotherapy, targeted drugs, and high-intensity focused ultrasound (HIFU). The abdominal pain was successfully relieved after the HIFU therapy. The patient had an 18-month survival with satisfactory quality of life. Large-scale randomized clinical trials are necessary to evaluate the long-term efficacy of HIFU treatment with or without chemotherapy and/or targeted drug therapy for pancreatic carcinoma.

Abstract:PurposeTo compare the survival benefit, pain control and safety of low-power cumulative and traditional high-intensity focused ultrasound (HIFU) for metastatic pancreatic cancer.MethodWe retrospectively analyzed 55 patients with metastatic pancreatic cancer who received HIFU treatment between January 2008 and April 2014 in our department. 23 patients received low-power cumulative HIFU treatment (L group), 32 received the traditional HIFU treatment (T group). Performance status, cancer-related pain and serum biochemistry results were assessed before and after treatment. All patients were followed up until death. The survival rate and adverse events of the two groups were compared.ResultsThe baseline characteristics of the two groups were generally well balanced (p > 0.05). The average KPS score after treatment was significantly improved in both groups compared with the baseline score. 36 patients exhibited tumor-related pain at baseline. The pain response rate was significantly higher in the L group (92.3%) than in the T group (52.2%) (p = 0.025). The median overall survival (OS) for the L group was 7.0 months, which was significantly longer than that of the T group (p = 0.000). The 3-month and 6-month survival rates were higher in the L group. The adverse events in both groups included abdominal pain, elevated C-reactive protein (CRP) and elevated amylase. The incidence was lower in the L group than in the T group.ConclusionCompared with traditional HIFU treatment, low-power cumulative HIFU treatment showed a significantly higher pain relief rate and survival benefit with a better safety profile in patients with metastatic pancreatic cancer.

Abstract:Present study was designed to investigate the pancreatic endocrine and exocrine function damage after High Intensity Focused Ultrasound (HIFU) therapy in patients with advanced pancreatic cancer. It was a retrospective analysis of blood glucose and amylase changes in 59 advanced pancreatic cancer patients treated with HIFU from 2010 February to 2014 January. The mean glucose and amylase before HIFU treatment were 6.02mmol/L and 59.17 U/L respectively. After HIFU treatment, it was shown that the mean glucose and amylase levels were 5.66mmol/L and 57.86/L respectively. There was no statistical significance between them. No acute pancreatitis was observed. The endocrine and exocrine function of pancreatic cancer patients was not damaged by HIFU treatment. HIFU treatment for the pancreatic cancer patients seems to be safe.

Abstract:Objective To investigate the therapeutic effect and safety of high intensity focused ultrasound(HIFU) combined with Gemcitabine in treating unresectable pancreatic carcinoma Methods Forty one patients suffering from unresectable pancreatic carcinoma were randomly divided into two groups.The patients in experimental group(n =21) were provided with HIFU in combination with gemcitabine therapy and those in control group(n =20)received HIFU treatment alone.The effect,clinical benefit rates,changes of tumor marker carbohydrate antigen 19-9(CA19-9)and adverse reactions were compared between these two groups.The median survival time,6-month and 12-month survival rates were calculated by Kaplan-Meier method and Logrank test.Results The median survival time,6-month and 12-month survival rates were 10.22 months,76.2％(16/21) and 42.9％(9/21) in experimental group,and they were 7.43 months,50.0％(10/20) and 15.0％(3/20) respectively in control group.Among them,12-month survival rates was significantly higher in experimental group than that in control group(x2 =4.00,P ＜ 0.05).The clinical benefit rates in experimental group were significantly higher than that in control group[76.2％(16/21) vs 45.0％(9/20),x2 =4.20,P ＜0.05].But there was no significant difference on pain remission rate between the two groups(66.6％ vs45.0％,P ＞ 0.05).There was significant difference on CA19-9 after treated for 2,3,4,5 and 6 months in experimental group than that in control group(t =2.225,2.133,1.743,2.599,2.278,respectively,P ＜ 0.05)Conclusion HIFU in combination with Gemcitabine therapy is better than application of HIFU treatment alone.The former may become of the most effective treatmenffor unresectable pancreatic carcinoma. Abstract:This study was performed to evaluate cancer pain control of high-Intensity focused ultrasound ablation (HIFU) and to manage the HIFU treatment pain in advanced pancreatic cancer patients with analgesics. We collected 71 locally advanced pancreatic cancer patients treated with HIFU from 2013 March to 2014 January in our hospital. The cancer pain (pre-HIFU and two weeks after HIFU) and HIFU treatment pain were evaluated respectively. The numeric rating scale (NRS) was used as the tool of pain evaluation. The related factors with pains were analyzed. The 70.42% cancer painless rate before HIFU was improved to 92.96% (P<0.05) 2 weeks after HIFU in 71 advanced pancreatic cancer patents without analgesics adjustment. The HIFU treatment pain occurred in 42 of 71 treated patients (59.15 %). The average duration was 3.93 days and pain score was 3.22. HIFU can improve cancer pain relief further in the advanced pancreatic cancer patients with third ladder analgesics, meanwhile HIFU treatment pain can be managed easy because of its short duration and low pain score.

Abstract:OBJECTIVE:To explore the efficacy and safety of high-intensity focused ultrasound combined with other physical therapies for the treatment of pancreatic cancer, so as to provide reference for clinical application.METHODS:The PubMed, EMbase, The Cochrane Library (Issue 11, 2013), CBM, CNKI, and WanFang databases were systematically searched up to November 2013. The randomized controlled trials and clinical controlled trials about the treatment of high-intensity focused ultrasound were included. According to the inclusion and exclusion criteria, two reviewers independently screened the studies, exacted the data, and assessed the quality. The meta-analysis was performed by using the RevMan 5.0 software.RESULTS:A total of 23 studies including 19 randomized controlled trials and four clinical controlled trials were included. Of which 14 studies reported the safety. The results of meta-analyses showed that: the survival rate at 6 months and 12 months, overall efficacy, and clinical benefit rate in high-intensity focused ultrasound plus radiation and chemotherapy group were significantly higher than those in groups treated with three-dimensional conformal radiation therapy (P < 0.05), gemcitabine (P < 0.05), gemcitabine plus cisplatin (P < 0.05), and gemcitabine plus 5-fluorouracil (P < 0.05). The adverse effect caused by high-intensity focused ultrasound plus other therapy was equal to those in the control group. The major high-intensity focused ultrasound-related adverse effect was skin damage and fever.CONCLUSION:The current evidence suggested that the efficacy of high-intensity focused ultrasound for pancreatic cancer was superior to other therapies. However, the poor quality of trails may reduce the reliability of outcome to some extent. The current evidence suggested that the efficacy of HIFU for pancreatic cancer was superior to other therapies, but with poor quality. Therefore, a standard and unified criterion for the diagnosis and outcome measures of pancreatic cancer is needed, and the quality of study design and implementation should be bettered, so as to improve the high quality of evidence for its clinical application.

Abstract:High-intensity focused ultrasound (HIFU) is a non-invasive method of solid tissue ablation therapy. However, only a few studies have reported the effect of HIFU for unresectable pancreatic cancer. This study aimed to evaluate the clinical benefits, survival time and complications associated with the use of HIFU ablation in patients with unresectable pancreatic cancer.Twenty-five patients with unresectable pancreatic cancer were enrolled in our study. All patients received HIFU therapy for tumors at least once. The therapeutic effects of HIFU was evaluated in terms of Karnofsky performance status (KPS) scores, pain relief, serum CA19-9, and imaging by B-US and CT before and after the therapy. We also recorded median overall survival time and complications caused by the treatment.In the 25 patients, KPS scores were above 60, and increased KPS was observed in 23 patients after treatment. Pain relief occurred in 23 patients. Serum CA19-9 levels were significantly reduced one month after HIFU treatment and became negative in 5 patients. B-US revealed enhanced tumor echogenicity in 13 patients and decreased tumor blood supply in 9. Tumor necrosis was confirmed by CT in 8 patients one month after HIFU treatment. The median overall survival time was 10 months, and the 1-year survival rate was 42%. No severe complications were observed after HIFU treatment.HIFU can effectively relieve pain, increase KPS, decrease tumor growth and prolong the survival time of patients with unresectable pancreatic cancer.
HIFU is Safe, Effective, and Feasible in Pancreatic Cancer Patients: a Monocentric Retrospective Study among 523 Patients

Abstract:Purpose: This study aimed to evaluate the clinical value of high-intensity focused ultrasound (HIFU) combined with gemcitabine (GEM) in treating unresectable pancreatic ductal adenocarcinoma (PDAC). Patients and methods: A total of 523 unresectable PDAC patients were recruited from December 30, 2007 to January 30, 2015 at Fudan University Shanghai Cancer Center. Among them, 347 received HIFU combined with GEM (with regional intra-arterial chemotherapy [RIAC] or with systemic chemotherapy) and the remaining patients received GEM only. Postoperative complications were observed, and overall survival was recorded. Results: The median overall survival of patients who received HIFU combined with GEM vs GEM alone was 7.4 vs 6.0 months (P=0.002); the 6-month, 10-month, 1-year, and 2-year survival rates for patients in these two groups were 66.3% vs 47.5% (P<0.0001), 31.12% vs 15.9% (P< 0.0001), 21.32% vs 13.64% (P=0.033), and 2.89% vs 2.27% (P=0.78), respectively. In the combined therapy group, the most obvious survival benefits were obtained among patients who received HIFU plus RIAC and systemic chemotherapy (used in the intervals between RIAC treatments). There were no severe complications in patients undergoing HIFU treatment. Conclusion: We demonstrated the survival benefit of HIFU among PDAC patients treated with GEM. The benefit was most obvious in PDAC patients treated with HIFU plus RIAC and systemic chemotherapy.

Abstract:Objective To evaluate the analgesic efficacy and effect of high-intensity focused ultrasound (HIFU) in the treatment of pancreatic cancer.Methods 29 patients with pancreatic cancer were treated by HIFU.According to some parameters,such as the clinical symptoms,the basis of main organs functional tests and imaging examinations to assess the efficacy of HIFU.Moreover,we compared dose intensity and efficiency of HIFU under general anesthesia or sedation-analgesia.Results 68.2% patients' (15/22) serum Ca19-9 decreased.The cancer pain was relieved in 69.2% patients(18/26).Coagulated-necrosis by CT/MRI examination was observed.1-year survival rate was 23.1%.Median survival time was 8.5 months.There was no obvious difference (P>0.05) in dose intensity of HIFU under two anesthetic methods.The efficiency of HIFU under sedation-analgesia was higher than that under general anesthesia (P<0.05).Conclusions Extracorporeal HIFU can effectively ablate pancreatic cancer and relieve pain.The mode sedation-analgesia is better applied for HIFU treatment with pancreatic cancer.

Abstract:Objective:To observe the proximate efficacy and toxicity of thermotherapy of high intensive focus ultrasound (HIFU) in combination with chemotherapy (Gemcitabine and Fluorouracil) on treating advanced pancreatic cancer. Methods: Forty patients with advanced pancreatic cancer were randomly divided into group A (chemotherapy+HIFU)and group B (chemotherapy alone). Results:The effective rate in group A was 50% and 31.25% in group B. There was a significant difference between two groups (P0.05). The main toxicities had Icukopenia and thrombocytopenia. Conclusion: Thermotherapy of HIFU in combination with chemotherapy is a feasible and well-tolerated scheme on treating advanced pancreatic cancer.

Abstract:Objective To evaluate the clinical effect and safety of high intensity focused ultrasound (HIFU) for the treatment of advanced pancreatic carcinoma. Methods 21 cases of advanced pancreatic carcinoma were treated by FEP BY01 pyrotherapier (HIFU). Results After HIFU, abdominal pain was relieved in 88% of patients (15/17), ultrasonic echo of carcinoma tissue was enhanced in all patients, the bloodstream decreased significantly or disappeared, the tumor size and CT value had no significant change, PET examination showed that carcinoma was inactivated effectively, although the tumor size as evaluated by CT scan did not experience a shrinkage. There were no complications such as skin burns、pancreatic fistula、bleeding、 pancreatitis and perforation during the process. The mean survival time of the patients after HIFU was 7 6 months. Conclusion HIFU as a new local treatment effects a satisfactory palliation for patients with advanced pancreatic carcinoma.

Abstract:Object To observation the acesodyne effect of high intensity focused ultrasound (HIFU) on the treatment of advanced pancreatic carcinoma treated by HIFU .Methods 19 cases of advanced pancreatic carcinoma were treated by FEP-BYO2 pyrothrapier (HIFU). Results The pain were disappeared in 6 cases,the pain were relieved in 10 cases, the pain had not change in 3 cases. 2 caes who had no pain before be cured, there were no complications such as skin burns . Pancreatic fistula bleeding pancreatics and perforation during the prose the result has significant difference ( P<0.05).Conclusion HIFU can relieved the pain in advanced pancreatic carcinoma effectively.

Abstract:Objective To evaluate the effect and feasibility of high intensity focused ultrasound(HIFU) in the treatment of pancreatic cancer. Methods Eleven patients with advanced pancreatic cancer were treated with HIFU. Seven cases were diagnosed by pathology and others were diagnosed by clinical manifestation. The vital signs were observed during treatment. The change of symptoms, laboratory index, imaging and complications were observed after treatment. Results Steady vital signs during treatment and pain relief after treatment were observed in all patients. Coagulation necrosis appearance in CT or MRI and the obvious decrease of CA-199 were observed in part patients. No serious complication such as acute pancreatitis was observed. Conclusion The clinical application of HIFU in the treatment of advanced pancreas cancer is effective and safe.

Abstract:Objective To evaluate the analgesic efficacy and effect of high intensity focused ultrasound (HIFU) in the treatment of advanced pancreatic cancer.Methods 128 patients with pancreatic cancer ( Ⅲ stage 41 case,Ⅳ stage 87 case) were treated by HIFU.According to some parameters,such as the clinical symptoms,imaging examinations and survival analysis to assess the efficacy of HIFU treatment.Results 22.9％ patients’ serum Ca19-9 decreased.The cancer pain was relieved in 72.5％ patients.Coagulated-necrosis by CT/MRI examination was observed.PR,SD and PD of all were 11.7％,70.3％ and 18.0％ respectively.1-year survival rate was 16.7％ for all patients,and median survival time was 7.0 months.1-year survival rate and median survival time for Ⅲ stage group were 28.5％ and 9.0 months,that of 10.8％ and 6.0 months for Ⅳ stage group respectively.Conclusion HIFU may benefit to advanced pancreatic cancer patient by stabilizing tumors,relieving pain and prolonging life expectancy with less side effects.

Abstract:OBJECTIVE:To evaluate the pain-alleviating action, feasibility and efficacy of high intensity focused ultrasound (HIFU) for palliation of inoperable pancreatic cancer in humans.METHODS:Forty patients with advanced pancreatic cancer were treated with HIFU. There were 13 patients with stage III, and 27 patients with stage IV disease. The locations of the tumours were as follows: head of pancreas in 9 patients, body and/or tail of pancreas in 31 patients. Pain relief, local tumour control rate, median survival and complications were monitored after HIFU treatment. The primary endpoint was to assess pain relief rate and pain relief time (PRT). Secondary endpoints included local progression-free survival time, overall survival (OS), and side effects.RESULTS:There were no severe complications or adverse events related to HIFU therapy in any of the patients treated. Pain relief was achieved in 87.5% of patients, median PRT was 10 weeks. The median local progression-free survival time for all patients was 5 months. The median overall survival time was 10 months for patients with stage III disease, and 6 months for patients with stage IV disease. The median OS time, 6-month and 1-year survival rate for patients as a whole were 8 months, 58.8% and 30.1%, respectively.CONCLUSIONS:Although this study may have limitations, preliminary results demonstrate the safety of clinical application of HIFU for pancreatic cancer and reveal it to be a promising mode of treatment for palliation of pain associated with pancreatic cancers.

Abstract:High intensity focused ultrasound (HIFU) is a novel therapeutic modality. Several preclinical and clinical studies have investigated the safety and efficacy of HIFU for treating solid tumours, including pancreatic cancer. Preliminary studies suggest that HIFU may be useful for the palliative therapy of cancer-related pain in patients with unresectable pancreatic cancer. This review provides a brief overview of HIFU, describes current clinical applications of HIFU for pancreatic cancer, and discusses future applications and challenges.

Abstract:339 Background: The aim of this study was to evaluate safety and efficacy of high-intensity focused ultrasound (HIFU) for advanced pancreatic cancer. Methods: Patients with pancreatic cancer TNM stage III or IV were included. Magnetic resonance imaging was performed 2 weeks before and after the HIFU. The ablating tumor volume was calculated by ratio of the non-perfused necrotic area of the planned area on contrast-enhanced T1-weighted image on post-HIFU magnetic resonance imaging. The ablation results were stratified into 4 ranges: 100% to 90% unenhanced area of targeting area, 90% to 50%, within 50%, and no change. Results: High-intensity focused ultrasound treatment was performed without severe adverse event in 52 patients, 53 times (male/female = 28/24; mean age, 62.7 ± 12.1; TNM stage 3/stage 4 = 21/31). Average size of the pancreatic cancer lesion was 4.5 ± 1.6 cm (1.4 - 9.5 cm). After HIFU treatment, ablating tumor volume was as follows: 90% to 100% in 41 lesions, 90% to 50% in 11, and within 50% in 3. Overall median survival (S1) from initial pancreatic cancer diagnosis was 13.1 months. Overall survival (S2) rates at 6, 12, and 18 months from HIFU were 53.1%, 31.6%, and 23.5%, respectively, with a median survival of 7.4 months. Conclusions: High-intensity focused ultrasound is safe and effective, which induced excellent local tumor control in most patients with advanced pancreatic cancer.

Abstract:Objective To investigate the clinical effect of high intensity focused ultrasound therapy(HIFU)on patients with advanced hepatocellular carcinoma(HCC).Methods Patients with surgically unresectable HCC received either HIFU plus supportive treatment(HIFU group,n=151),or supportive treatment(control group,n=30),according to their willingness.Short-term efficacy including the improvement in tumour imaging parameters,decrease in the serum α-fetoprotein(AFP)levels,symptom relief(i.e.Karnofsky Performance Status)and the response rates,and long-term efficacy including the increase of the survival rates and improvement of quality of life(QOL)were monitored.Results The tumour imaging parameters,serum AFP levels and symptom scores were significantly improved in HIFU group compared with the control group(P0.05).In HIFU group,a complete and a partial response were achieved in 28.5%(n=43)and 60.3%(n=91),while the rates were 0 and 16.7%(n=5),respectively in the control group.The overall response rate(88.8%)was significantly greater in the HIFU group than that(16.7%)in control group(P0.01).In addition,the one-and two-year survival rates were 50.0% and 30.9%,respectively,in the HIFU group,which were significantly greater than those(3.6% and 0%,respectively)in the control group(P0.01).The QOL score was 83.1±8.0 in 3 months after HIFU,which was significantly greater than the pre-HIFU score 67.7±5.9 and the score in 3 months after treatment 69.0±8.5 in control group(P0.05).No severe complications occurred during and after HIFU.Conclusion HIFU is an effective and safe ablation therapy with satisfactory short-and long-term efficacy for patients with advanced HCC.

Abstract:Objective To evaluate the efficacy and clinical feasibility of high-intensity focused ultrasound (HIFU) in the treatment of pancreatic cancer.Methods Seven patients with pancreatic cancer were treated by HIFU.According to clinical symptoms,basis of main organ functional tests,imaging examinations and complications,the efficacy and clinical feasibility of HIFU in the treatment was assessed.Results Symptom alleviation,serum Ca 19-9 decrease,coagulated-necrosis on MRI after HIFU treatment were observed.No complication of gastrointenstinal perforation was seen.Conclusions HIFU in the treatment of pancreatic cancer is feasible,effective and noninvasive.
