Abstract:Pseudoaneurysms are common vascular abnormalities that represent a disruption in arterial wall continuity. Therapeutic options have evolved in recent years from the traditional surgical option or ultrasound guided compression toward a less complicated approach, ultrasound guided percutaneous thrombin injection. Although surgery is still necessary in a limited number of patients, most can be treated with ultrasound guided thrombin injection. Success exceeds 90% and complications are much lower than surgery. It could be a new way to treat pseudoaneurysms.
Mid-term Results of Coil Embolization Alone and Stent-assisted Coil Embolization for Renal Artery Aneurysms

Abstract:Background: Coil embolization (CE) alone and stent-assisted coil embolization (SCE) are two major endovascular techniques to treat renal artery aneurysms (RAAs). This study aimed at providing safety and efficacy data of CE and SCE for RAAs. Methods: Between August 2015 and June 2019, 40 RAA patients treated with CE or SCE were included in the retrospective study. Patients' demographics, clinical manifestations, aneurysm characteristics, treatment strategies, and follow-up results were collected and analyzed. Results: There were 26 and 14 patients in the CE and SCE group, respectively. The mean aneurysm diameter was 2.5 +/- 1.5 cm and 2.2 +/- 0.8 cm (CE versus SCE, P = 0.52). The neck width of the aneurysm was 0.63 +/- 0.37 cm and 1.07 +/- 0.42 cm (CE versus SCE, P = 0.021). Technical success was achieved in 97.5% patients. No death or aneurysm rupture occurred. During the perioperative period, 12% and 7.1% patients suffered partial renal infarction (CE versus SCE, P = 0.45). The mean duration of follow-up was 8.8 +/- 9.4 months and 16.1 +/- 16.3 months (CE versus SCE, P = 0.158) by imaging and 20.8 +/- 11.3 and 22.7 +/- 16.5 months by visit/telephone (CE versus SCE, P = 0.703). During the follow-up, 17.4% patients in the CE group and 30.8% patients in the SCE group suffered partial renal infarction, while their overall renal function remained normal. In addition, there was no aneurysm recurrence, sac enlargement, or death in both groups. Conclusions: Both CE and SCE were safe and effective to treat RAAs. In addition, SCE may prevent coil migration in the wide neck aneurysm in selected patients.

Abstract:A 54-year-old Chinese woman presented with a 10-year history of repeated paroxysmal palpitations. She was diagnosed with paroxysmal supraventricular tachycardia by 12-lead electrocardiogram and was advised to undergo catheter-based radiofrequency ablation. During the procedure, a rare complication occurred that was diagnosed as a right internal mammary artery penetrating injury. After appropriate emergency treatment with arterial embolization and membrane-covered stent implantation, the patient was out of immediate danger of haemorrhaging. Follow-up computed tomography angiography of the subclavian artery at 3 months after she was discharged from hospital revealed stent-graft patency with no evidence of in-stent thrombosis or stent stenosis. No problems were observed at the 6-month follow-up visit.
Assisted Coiling Techniques in Treatment of Intracraniai Complex Aneurysms

Abstract:Objective To investigate the application of assisted coiling techniques in the treatment of complex intracranial aneurysms.Methods From Jun.2007 to Aug.2010,36 patients with 41 complex intracranial aneurysms were treated by varient endovascular methods,including balloon,stent-assisted coiling and double microcatheter coiling techniques.Results Among the 33 wide - neck aneurysms,11cases underwent balloon- assisted coiling,in which 7 were totally occluded,3 subtotally occluded,1 diverted to double microcatheter treatment after failure balloon placement.15 cases underwent stent - assisted coiling,in which 10 were totally occluded,2 subtotally or partially occluded respectively,1 by single stent placement without coiling.8 cases accepted double microcatheter treatment,6 were totally occluded,1 subtotally occluded and 1 partially occluded.All 8 fusiform aneurysms were treated by stent -assisted coiling,one case was totally occluded,6 subtotally occluded,and another one was treated by single stent placement.The complications were found in 3 patients,including intra - operative thrombosis in 2 cases and dysopia in one case.Conclusion Balloon,stent - assisted coiling and double microcatheter techniques are safe and effective in treating complex intracranial aneurysms,but it should be determined by the position and morphologic characteristics of aneurysms as well as the patients' clinical condition.
Percutaneous Embolization for Ruptured Ectopic Bronchial Artery Aneurysm: a Case Report.

Abstract:Bronchial artery aneurysm (BAA) is an uncommon disease, and spontaneous rupture of an ectopic BAA can be difficult for diagnosis and life-threatening. This case study describes a 52-year-old man who presented with acute onset of right chest pain, mild tachycardia, and hypertension. The initial diagnosis of acute myocardial infarction was made, and the patient was given nitroglycerin prior to admission to our hospital. However, the patient's symptoms deteriorated. An enhanced computed tomography scan revealed a ruptured 25-mm diameter mediastinal aneurysm under the tracheal bifurcation when he was admitted to our hospital. Bronchial arteriography further demonstrated a ruptured mediastinal BAA of a bronchial artery originated from the left subclavian artery, supplying the right lobe. Transcatheter artery embolization with polyvinyl alcohol particles and microcoils was performed successfully. The patient's symptoms were gradually relieved, and without recurrence on 1 year follow-up.This case highlights the rare variation of mediastinal BAA and the role of interventional radiology in diagnosing and treating this critical condition.
Ultrasound-guided Thrombin Injection for Rite Treatment of Coronary Intervention-related Femoral Pseudoaneurysm

Abstract:Objective To evaluate the effectivity and safety of ultrasound-guided percutaneous thrombin injection for the treatment of postcatheterization femoral pseudoaneurysm.Methods From January 2006 to April 2008,33 patients(12 male,21 female,age range 58～72 years,mean 66 years) were found to have pseudoaneurysmes associated with catheterization procedure,and 8 of them with diagnostic angiography,the other 25 patients with stent implantation.Results Thrombin was injected directly into the pseudoaneurysm with dosage 200～800 units over several seconds.Successful ablation was visualized immediately in 31 patients.The other 2 patients needed two thrombin injections and compression treatment of 2～3 minutes.The total success rate of ablating pseudoaneurysm was 100%.One patient had vasovagal syncope,12 patients had mild fever after thrombin injection.No distal embolization,infection,hemorrhage or allergic reaction occurred.Conclusions Ultrasound-guided thrombin injection may shorten hospitalization duration and it is a safe,simple,inexpensive and effective non-invasive method for the treatment of postcatheterization femoral pseudoaneurysm.Therefore it should be considered as first-line therapy.
Effect of Ultrasound Guided Thrombin Injection for Treatment of Femoral Arterial Pseudoaneurysm

Abstract:Objective To evaluate the effect and safety of ultrasound-guided percutaneous thrombin injection for the treatment of postcatheterization femoral arterial pseudoaneurysm.Methods Seventy-eight patients with femoral anterial pseudoaneurysm were included in this study.Pseudoaneurysms were treated by bedside compression in 50 of these 78 patients(group A),and by thrombin injection in 28 patients(group B).All patients were diagnosed by GE LOGIQ 9 Color Doppler Ultrasound.Ultrasound guided thrombin injection for femoral arterial pseudoaneurysme were carried out in patients in group B.Results Closure of the pseudoaneurysm was successfully in 82 percent(41/50) of the patients in group A.Six patients had vagus reflex,4 patients could not tolerate repeating compression and 5 patients were not effectively cured by compression and needed to receive remedial ultrasound-guided percutaneous thrombin injection.Closure of the pssudoareurysm by percutaneous thrombin injection was successful in all patients in group B.Successful immediate closure were noted in 25 patients and 2 patients needed second thrombin injection.One patient had vasovagal syncope and 12 patients had mild fever.No distal embolization,infection,hemorrhage or allergic reaction occurred.Conclusion Compared to compression therapy for treatment of femoral arterial pseudoaneurysme,ultrasound-guided percutaneous thrombin injection is a simple,safe and effective treatment alternative.

Abstract:Transcatheter arterial embolization (TAE) is the best non-laparotomy choice for solid visceral organs rupture and bleeding nowadays. In our previous study, a new biodegradable macromolecule material thrombin-loaded alginate-calcium microsphere (TACM) was prepared and its characteristics were investigated preliminarily. In this study, we further investigated the biocompatibility of TACMs, as well as physical characteristic, application method and effect of TACMs with thrombus (embolic agent). The in vivo results attested that TACMs were non-irritating and non-genotoxic with desired biocompatibility, although brought about a slight and temporary inflammation. Application research showed that the function of thrombin was inhibited by common contrast agents, and it was impracticable to add contrast agents in TACMs with thrombus for tracing under X-rays in TAE. Then, a novel delivery method was developed. In addition, stress resistance test indicated that the TACMs with thrombus was significantly stronger than single autologous thrombus, the optimized ratio of TACMs to whole blood was 2:3 for forming mixed thrombus. Finally, large animal experiment revealed that the novel embolic agent - TACMs mixed thrombus was effective and safe in treating hemorrhage of solid abdominal viscera by TAE.
Transvenous Endovascular Treatment for Scalp Arteriovenous Fistulas: Results with Combined Use of Onyx and Coils

Abstract:BACKGROUND:Scalp arteriovenous fistulas (AVFs) are rare lesions that may occur spontaneously or secondary to head trauma. A standard treatment strategy for these lesions has not been established to date. We present 3 cases of successful treatment of scalp AVFs using a combination of Onyx-18 and coils via a transvenous approach.METHODS:The patient database at Huashan Hospital, Fudan University, Shanghai, China was reviewed, and patients with scalp AVFs treated with Onyx-18 and coils via a transvenous approach were identified for analysis.RESULTS:Between 2014 and 2016, 3 consecutive patients with scalp AVFs were treated transvenously with Onyx embolization in combination with coil placement at our hospital. No procedure-related complications were noted. Postembolization angiography demonstrated successful and complete occlusion of the AVFs immediately after treatment. Clinical follow-up showed resolution of symptoms.CONCLUSIONS:The technique of transvenous "armored concrete" embolization using a combination of coils and Onyx is an effective treatment modality for scalp AVFs, particularly in type C cases.
Ultrasound‐guided percutaneous thrombin injection for the management of upper extremity pseudoaneurysms: 20 years of tertiary care center experience

Abstract:In this study, the success of ultrasound‐guided percutaneous thrombin injection for the management of iatrogenic upper extremity pseudoaneurysms related to venous line placement or arterial access was 85.7%, with an overall success rate in the series of 75%. Purpose To evaluate the safety and efficacy of ultrasound‐guided percutaneous thrombin injection for the treatment of upper extremity pseudoaneurysms. Methods An institutional database containing 8,316,467 radiology reports was searched for suitable cases over a 241‐month period. Fourteen female and 10 male patients, average age of 69.7 years (range 29–93) underwent a total of 26 procedures for the management of upper extremity pseudoaneurysms, involving the radial (n = 9), brachial (n = 9) or other upper extremity arteries (n = 6). Baseline demographic and pseudoaneurysm characteristics were documented, together with primary and secondary success, failures, and complications. All procedures were performed with real‐time ultrasound guidance. Results The mean pseudoaneurysm volume was 9.93 cm3 (range 0.06–111.62 cm3). Twelve cases were related to central line placement or arterial access. Primary success was obtained in 50% (n = 12) after a single ultrasound‐guided thrombin injection, and secondary success was achieved in an additional six (for a total success of 75%). Success was highest for the treatment of brachial artery pseudoaneurysms (87.5%), and in those who were diagnosed within 7 days of the inciting event, findings that were statistically significant (p‐value 0.046 and 0.002, respectively). Conclusions Ultrasound‐guided percutaneous thrombin injection is safe and effective for managing upper extremity pseudoaneurysms.

Abstract:Transcatheter arterial embolization (TAE) has played a huge role in the interventional treatment of organ bleeding and accidental bleeding. Choosing bio-embolization materials with good biocompatibility is an important part of TAE. In this work, we prepared a calcium alginate embolic microsphere using high-voltage electrostatic droplet technology. The microsphere simultaneously encapsulated silver sulfide quantum dots (Ag 2 S QDs) and barium sulfate (BaSO 4 ), and fixed thrombin on its surface. Thrombin can achieve an embolic effect while stopping bleeding. The embolic microsphere has good near-infrared two-zone (NIR-II) imaging and X-ray imaging effects, and the luminous effect of NIR-II is better than that of X-rays. This breaks the limitations of traditional embolic microspheres that only have X-ray imaging. And the microspheres have good biocompatibility and blood compatibility. Preliminary application results show that the microspheres can achieve a good embolization effect in the ear arteries of New Zealand white rabbits , and can be used as an effective material for arterial embolization and hemostasis. This work realizes the clinical embolization application of NIR-II combined with X-ray multimodal imaging technology in biomedical imaging, achieving complementary advantages and excellent results, more suitable for studying biological changes and clinical applications.

Abstract:Interventional Neuroradiology, Ahead of Print. ObjectiveTo investigate the therapeutic effect of intra-arterial microguidewire electrocoagulation on intracranial vascular diseases.MethodsData from 10 patients with cerebral aneurysms between May 2018 and September 2022 were analysed. Patients were treated with endovascular coil embolisation and microguidewire electrocoagulation. XperCT scans were conducted to identify new intracranial haemorrhage, infarction and hydrocephalus. Follow-up examinations were conducted 1, 3, 6 and 12 months after discharge.ResultsAfter the patients received electrocoagulation for different durations, Raymond Grade 1 embolisation was achieved in all 10 patients. No complications, such as haemorrhage, infarction or hydrocephalus, were found during or after surgery. Ten patients were followed up for 6–12 months, and none had any symptoms or new neurological dysfunction 1 month after their operation. Among them, nine were followed up for 12 months, and digital subtraction angiography showed no recurrence of aneurysms or occlusion of parent arteries.ConclusionIntra-arterial microguidewire electrocoagulation can be used as a supplementary treatment for cerebral aneurysms. In cases of incomplete lesion embolisation and cases where tamponade treatment cannot continue, immediate thrombosis may occur. Thus, intra-arterial microguidewire electrocoagulation can help achieve patients' treatment goals.

The effect of transcatheter bronchial artery embolization in five patients with bronchial artery aneurysm

Abstract:INTRODUCTION: Due to its rarity and the lack of animal models for this disease, the etiology of bronchial artery aneurysm (BAA) is poorly understood.AIM: To investigate the effect of transcatheter artery embolization (TAE) with coils, spherical polyvinyl alcohol (PVA), and gel foam in patients with BAA.MATERIAL AND METHODS: From June 2015 to November 2018, 5 patients diagnosed with BAA with massive hemoptysis after undergoing bronchial arteriography and receiving TAE therapy in the Department of Interventional Radiology were enrolled in this study. Post-embolization angiography was used to demonstrate total occlusion of the BAAs and bronchiectatic engorged vessels. The patients' follow-up was conducted 1-16 months after TAE.RESULTS: All 5 patients were cured by the TAE treatment, during which PVA, coils, and gel foam were applied. The patients also received successful TAE via a microcatheter. Hemoptysis disappeared in all patients. No special or severe complications occurred, and no patient experienced a recurrence during the follow-up.CONCLUSIONS: Overall, TAE is the best choice for patients with hemoptysis and BAA due to its effectivity and reliability. Thorough and complete embolization of the bleeding arteries is the key to successful treatment. Though PVA and coils are the most common materials for embolization, gel foam strips can also be applied.
Transcather Embolization of Splenic Artery Aneurysms by Drifting-Coils Method

Abstract:Objective:To evaluate the efficacy and safety of the techniques of transcatheter embolization by drifting-coils in treatment of splenic artery aneurysms(SAAs).Methods: The Clinical data of 25 patients with SAAs between 2005 and 2010 for treated with catheter-based techniques were reviewed retrospectively.Sixteen were women,the median age was 54 years.Among 25 patients,28 were SAAs,4 were pseudoaneurysms,19 were solitary aneurysms,25 were saccular forms,of which 9 involved the middle portion of splenic artery,11 involved the splenic hilum and 8 involved the proximal portion of the splenic artery.And 36% of them had portal hypertension.The median aneurysm diameter was 32mm(range,15–65 mm).Transcatheter endovascular coil embolization was used in 22 procedures,the techniques of transcatheter embolization by Drifting-coils was used in 15 of them.Stent graft placements were used to treat 2 patients with combined embolization of distal splenic artery.Results: Technical success was achieved in 100% of 24 of procedures.No significant complication was observed except for 10 slight post operative embolization symptoms and 7 slight splenic infarctions.An average follow-up duration of 18.1 months revealed no recurrence of areurysms and Doppler ultrasound scan showed patency of 2 stent grafts.One SAA with no intervention showed no changes.No significant difference between the embolization by drifting-coils group and no drifting-coils in embolization group was found by evaluating their technical success,30-day mortality,post-embolization symptoms,splenic infarction,the reperfusion of sac during follow-up period.Conclusions: The techniques of transcatheter embolization by drifting-coils are safe and effective methods for the treatment of splenic artery aneurysms.
Balloon assisted, ultrasound guided percutaneous thrombin injection of a large radial artery pseudoaneurysm using a trans-venous approach via an Ipsilateral Arteriovenous Fistula

Abstract:Background: Pseudoaneurysm formation is known to complicate arteriovenous haemodialysis access. Ultrasound guided thrombin injection is a recognised treatment option, but is not possible in pseudoaneurysms with no measurable neck. Balloon assisted techniques have been described in such cases, which transiently obstruct flow out of the pseudoaneurysm and thereby prevent non-target embolization during ultrasound guided percutaneous thrombin injection. We describe a balloon assisted technique for the treatment of a radial artery pseudoaneurysm, via retrograde access from the draining cephalic vein of an arteriovenous fistula. Method: A 61-year-old male with a radio-cephalic fistula was found on duplex ultrasound to have a large radial artery pseudoaneurysm with no measurable neck, as well as a juxta-anastomotic cephalic vein stenosis. Endovascular treatment was selected over open surgery. Retrograde cephalic venous access was established, which allowed for concurrent treatment of both the venous stenosis and the arterial pseudoaneurysm. After balloon dilation of the juxta-anastomotic stenosis, a percutaneous transluminal angioplasty balloon catheter was advanced across the arteriovenous anastomosis and inflated across the neck of the radial artery pseudoaneurysm, to transiently obstruct blood flow. This allowed for safe injection of thrombin into the pseudoaneurysm by direct ultrasound guided sac puncture; thereby achieving thrombosis. Conclusions: Balloon assisted ultrasound guided thrombin injection is an endovascular treatment option that can obviate the need for open surgery in cases involving pseudoaneurysms with no measurable neck. The technique described allowed both concurrent treatment of a juxta-anastomotic venous stenosis and treatment of an arterial pseudoaneurysm from a single venous puncture. This technique avoided arterial access and its inherent complications.
Endovascular treatment of ectopic bronchial artery aneurysm with brachiocephalic artery stent placement and coil embolization: A case report and literature review.

Abstract:Background:Bronchial artery aneurysm (BAA) is an uncommon but potentially life-threatening disease, and multiple BAAs are even rarer. Clinically, the tortuous and short neck of a BAA may present significant challenges for invasive intervention.Methods:This report describes the detailed process of diagnosis and treatment and includes a literature review of the etiology, clinical presentation, and therapeutic management of BAA.Results:A rare case of multiple BAAs, with one having an inflow artery arising from the brachiocephalic trunk, was referred to our hospital. The patient was successfully treated with coil embolization and brachiocephalic artery stent placement. In addition, we conducted a literature review involving 63 cases of BAA. BAA was most commonly associated with bronchiectasis and was located predominantly in the mediastinum. There was no significant difference between the diameters of the ruptured aneurysms and those of the nonruptured aneurysms (P=0.115). Transcatheter arterial embolization was the most commonly adopted technique to treat BAA, while thoracic aortic endovascular repair was selected if the neck between the aneurysm and the aorta was short. Subgroup analysis suggested that patients with >1 BAA were significantly more likely to be female than male ((2) test, P=0.034).Conclusion:Transcatheter coil embolization combined with stent placement could be a reasonable treatment option for BAAs with a tortuous and short neck. According to our literature review, patients with multiple BAAs display distinctive clinical characteristics compared with patients with a single BAA.

Abstract:Vascular embolism is a severe complication following hyaluronic acid (HA) filler injections, with hyaluronidase injection being the most effective treatment. The nasal region, including the nose and nasolabial fold, is the most common site of vascular embolism after facial HA injections. Currently, there is insufficient clinical evidence regarding the benefits of facial artery thrombolysis.

Treatment of arterial pseudoaneurysms with percutaneous ultrasound-guided thrombin injection

Abstract:Background: The pseudoaneurysms or false aneurysms are one of the most common complications after arterial invasive manipulation. Different methods of treatment have been described, such as surgical repair, ultrasound-guided compression and endovascular procedures. Ultrasound-guided thrombin injection has shown efficacy, safety and low cost, and, in some cases, the procedure is considered the treatment of choice. Preliminary results are presented. Design: Case series. We studied adult patients with iatrogenic arterial pseudoaneurysms demonstrated clinically and corroborated with ultrasound. We describe the technique used for the management of the arterial pseudoaneurysms by ultrasound-guided thrombin application, as well as complications and follow-up. Results: Twelve patients were included with a mean age of 63 years (SD 12.4), 33% were male and 67% female. Associated comorbid states were hypertension 83%, 50% diabetes, obesity 66%. All pseudoaneurysms were located in the femoral artery (2:1 right/left ratio). The mean size of the pseudoaneurysm was 48.2 mm SD 11.9, (range of 30 to 65 mm) by 44.3 mm SD 9.3, (range 20 to 53 mm). The average neck diameter was 2.2 mm SD 0.54 (range of 1.5 to 3 mm). Eleven cases were unilobulated (87.5%), and one bilobulated (12.5%). All reported arterial integrity with complete occlusion after one thrombin application without recurrence and morbidity after 90 days of follow up. Conclusions: In our study we obtained a 100% of thrombosis without pseudoaneurysm recurrence or complications. However a longer follow-up time and more number of cases are required to confirm these results.

Abstract:To date, transcatheter arterial embolization (TAE) has become a standard treatment to control intracavitary bleeding as an alternative to surgery. Due to excellent biocompatibility and no residual in vivo, biodegradable materials are preferred in TAE. However, gelfoam is the only commercially available biodegradable embolic material used to treat blunt trauma of solid abdominal viscera until now, and controversial on its stability and reliability never stopped in the past five decades. In this study, a new biodegradable macromolecule material (thrombin-loaded alginate-calcium microspheres, TACMs) was prepared using electrostatic droplet techniques and a special method was developed for hemostatic embolization. Thrombin was successfully loaded into microspheres with high encapsulation efficiency and drug loading capacity. A burst release of TACMs was observed at early stage and sustained release later on, with the activity of thrombin preserved well. The strength of TACMs mixed thrombus, which was used as embolic agent, increased in a dose-dependent manner after TACMs were added. In addition, the TACMs were verified to be of no cytotoxicity and systemic toxicity, and biodegradable in vivo. Finally, the results of preliminary applications revealed that the TACMs could serve as an effective and promising embolic material for blunt trauma and hemorrhage of solid abdominal viscera.
