Abstract:Objective To study the feasibility of laparoscopic resection of colorectal cancer.Methods A(retrospectively) analysis of the clinical data of 110 patients with colorectal cancer treated by laparoscopic(resection) during last three years was made.Of them,24 cases were converted to open operation.In 86 (cases),laparoscopic resection of colorectal cancer was completed.Among them,5 cases underwent right(hemicolectomy),2 cases left hemicolectomy,10 cases sigmoidectomy,22 cases Dixon′s operation,46 cases of Miles operation,and 1 case total colorectomy.Results No patient died within 30 days postoperatively in this series.The average operation time was 225(120-360) min with average 135(20-400) mL blood loss.The average number of lymph nodes excised was 8.7(1-30).The average number of positive lymph nodes was 2.2(0-24).Six cases had intraoperative complications,including 1 case of ureteral injury and 5 cases of intra-abdominal bleeding.These patients were immediately converted to open surgery.There were 6 cases with postoperative complications,among them,2 cases of urinary leakage,2 cases of massive bleeding and 2 cases of intestinal obstruction.All of them recovered after reoperation.The time of bowel funtion(recovering) was 12-72h after operation.The hospital stay after operation was 8.6(7-15)days.The median follow-up was 14.3 months(range 1-33) for 100(90.9%) patients.There was no port-site tumor(metastasis) and no tumor recurrence at the small abdominal incision.Six to fifteen months after opteration,3 cases had diffase peritoneal metastases.1 case of Miles resection had perineal metastasis 3 months after(surgery).Conclusions Laparoscopic resection of colorectal cancer is technically feasible,and have(advantages),such as less surgical trauma,less bleeding,less gastrointestinal interference and quicker(recovery).Laparoscopic radical operation for colorectal cancer can meet the requirements of safety and radical operation.

Abstract:Complete mesocolic excision (CME) has the potential to become the standard procedure for colon cancer surgery as a result of its effectiveness in reducing local recurrence rate and improving prognosis. There are two strategies to perform CME: medial and lateral. Researchers have demonstrated that laparoscopic CME via medial access is comparable to a traditional open surgery. Technically, we proposed two approaches for medial access: hybrid medial approach (HMA) and completely medial approach (CMA). HMA involves the entrance to the intermesenteric space (IMS) by an incision of the gastrocolonic ligament followed by the dissection of the middle colic vessels and the Henle trunk in a top-to-bottom fashion. The approach is capped by the dissection of the inferior edge of the pancreas, requiring the blending of both top-to-bottom and bottom-to-top approaches. CMA, on the other hand, involves a bottom-to-top approach in every step, including the entrance of IMS through transverse retrocolic space (TRCS); dissection of the middle colic vessels and the Henle trunk; and dissection of the inferior edge of the pancreas. According to the previous research of our medical center, laparoscopic CME via the total medial approach is safe and technically feasible after the precise identification of the surgical planes and spaces for the right hemicolon. Meanwhile, this procedure reduces operation time and has fewer vessel-related complications, which is a better choice for right hemicolectomy.

Abstract:Objective To explore the feasibility and safety and the long term outcome of laparoscopic assisted right hemicolectomy for colonic carcinoma.Methods The safety of operation,status of recovery,complication,oncological clearance and results of short and long term follow-up were studied in 177 patients underwent laparoscopic-assisted right hemicolectomy for colonic cancer between Feb.2002 and Mar.2009.Results No case die in this study,five cases(2.82 %) were converted to open surgery.Four cases(2.26 %) underwent laparoscopic right hemicolectomy with hand-assisted.The average operation time was(132.6±35.7) min and the blood loss was(94.2±34.1) m1.The average time for passage of flatus,food eating and hospitalization were(2.13±0.74) d,(3.21±0.45) d,(10.35±2.72) d respectively.The total number of lymph nodes removed was(15±8).Complications were observed in 24 of 177 patients(13.56 %).The median follow-up period was 39(12-97) months,1 port-site recurrence(PSR) was observed in one patient,local recurrence was found in 5 cases(2.82 %),7 cases were found distant metastasis,there were 2 pulmonary metastasis,4 hepatic metastasis and 1 bony metastasis.The cumulative survival,disease-free survival and tumor-specific overall survival were 83.69 %,80.43 % and 83.14 %.The five years’ survival rate of 3 stage(Dukes A,B,C) were 93.23 %,76.47 % and 66.67 % respectively.Conclusions Laparoscopic-assisted right hemicolectomy can be successfully performed for right colon carcinoma with all the advantages of minimally invasive surgery.Moreover the short and long term outcome is indicated effective and ideal.

Abstract:OBJECTIVE:To investigate the clinical effect of laparoscopic radical resection of colon cancer.METHODS:Patients with colon cancer who underwent radical resection during January 2000 to January 2004 in Sir Run Run Shaw Hospital (Medical College of Zhejiang University) were divided into open and laparoscopic groups for a non-randomized case-control study. All the patients were followed up and their clinical and pathological results were compared.RESULTS:No difference was seen between the two groups on the patients' age, gender, lymph node numbers and Dukes staging. The laparoscopic group had a longer mean operation time [(182 +/- 62) min vs (141 +/- 37) min] and shorter hospital stay [(5.3 +/- 1.9) d vs (8.2 +/- 1.2) d] than the open group. All surgical margins were pathological negative. Post-operation follow up was 12-18 months (mean 21 months). No incision recurrence was found in neither group. As far the local recurrence and distal metastasis rate were similar in both groups.CONCLUSIONS:Laparoscopic radical resection of colon cancer has minimal invasion and shorter recuperation than open procedure, the radicalness and either local recurrence or distal metastasis rate after surgery are similar when compared with open group.

Abstract:Abstract Objective The objective of this study is to examine the safety and feasibility of laparoscopic total colonic mesenteric resection for radical right hemicolectomy, utilizing a caudal-to-dorsal priority access approach in combination with a cephalic approach.Methods This study aims to conduct a retrospective analysis of the clinical data of 56 patients diagnosed with right hemi-colon cancer, who underwent radical right hemi-colon resection through laparoscopic total colonic mesenteric resection with caudal-to-dorsal priority, combined with cephalic approach, between January 2021 and June 2022.Results All 56 cases were completed without any instances of open surgery. The average operative time was (153.8 ± 42.5) minutes, with an average intraoperative bleeding of (53.3 ± 21.3) mL. The average time to first gas after surgery was (58.3 ± 13.6) hours, while the average time to return to a liquid diet was (68.5 ± 19.6) hours. The average postoperative hospital stay was (6.1 ± 3.0) days. Following surgery, two cases of lymphatic fistula and one case of anastomotic bleeding were reported. Following conservative treatment, all cases were cured, and no perioperative deaths were reported. Post-operative pathological results indicated that all cases were diagnosed with adenocarcinoma, with an average of 20.1 ± 5.9 lymph nodes cleared. Tumor staging revealed 8 cases in stage I, 19 cases in stage IIA, 11 cases in stage IIB, 12 cases in stage IIIA, and 6 cases in stage IIIB. During the 12-month postoperative follow-up, no instances of recurrence, metastasis, or death were observed.Conclusions The utilization of a caudal-to-dorsal priority in conjunction with a cephalic approach during laparoscopic radical resection of right hemicolectomy is a secure and viable method that aligns with the fundamental principles of radical oncology. This technique can aid in reducing the learning curve for surgeons performing laparoscopic right hemicolectomy and in guaranteeing surgical safety.

Abstract:Objective To investigate pathologic effects combined with postoperative results on right colon cancer with laparoscopic complete mesocolic excision(CME) via dorsal-medial hybrid approach compared with traditional medial approach. Methods A total of 68 patients with right colon cancer undergoing laparoscopic CME in our hospital between Ja-nuary and June 2017 were studied retrospectively. There were two groups including 37 cases in dorsal-medial hybrid group and 31 cases in traditional medial group. CME quality evaluated with the method proposed by West et al along with safety, short-term results and long-term prognosis were compared between two groups. Results CME completion rate was 86.5% in dorsal-medial hybrid group and 80.6% in traditional medial group without statistical difference(P=0.53). Laparoscopic dissection time in dorsal-medial group was significantly shorter than that in traditional medial group[(62.70±12.22) min vs.(70.39±11.98) min, P=0.01]. There was no statistical difference in operation time, blood loss, postoperative flatus time, postoperative complications and long-term prognosis of the patients between two groups. Conclusions Laparoscopic CME via dorsal-medial hybrid approach for right colon cancer could have less laparoscopic dissection time, and two approaches were similar in CME quality, safety and prognosis.

Abstract:Objective:To investigate the feasibility and clinical efficacy of laparoscopic radical cholecystectomy(LRC) for gallbladder cancer.Methods:The clinical data of 247 patients with gallbladder cancer who underwent radical resection from Jan 2013 to Dec 2019 at Department of General Surgery, Sir Run Run Shaw Hospital affiliated to Zhejiang University School of Medicine was analyzed retrospectively. After propensity score matching, 54 patients were included in laparoscopic group and 103 in laparotomy group. The clinicopathological characteristics and the short- and long-term outcomes were compared.Results:Compared to the laparotomy group, patients in the laparoscopic group had less intraoperative blood loss [100(50,200)ml vs. 200(100,300) ml, Z=4.105, P<0.001], earlier postoperative oral diet[1.0(1.0,2.0) d vs. 2.0(1.0,4.0) d, Z=4.157, P<0.001]and drainage removal[6.5(4.0,12.5) d vs. 9.0(6.0,16.0) d, Z=2.769, P=0.006], shorter hospital stay[7.0(5.0,9.3) d vs. 9.0(8.0,14.0) d, Z=3.923, P<0.001]. The number of lymph node dissection in laparoscopic group was significantly lesser than that in open group [6(4,9) vs. 8(5,12), Z=2.639, P=0.008]. There were no significant differences between the two groups in postoperative complications, short-term and long-term survival outcomes. Conclusions:Laparoscopic radical surgery for gallbladder cancer is as safe and feasible, and identical survival prognosis as open surgery, and moreover a less traumatic procedure.

Abstract:BackgroundWhether extended lymphadenectomy for right colon cancer leads to increased perioperative complications or improves survival is still controversial. This trial aimed to compare the efficacy and safety of complete mesocolic excision (CME) versus D2 dissection in laparoscopic right hemicolectomy for patients with right colon cancer. This article reports the early safety results from the trial.MethodsThis randomised, controlled, phase 3, superiority, trial was done at 17 hospitals in nine provinces of China. Eligible patients were aged 18–75 years with histologically confirmed primary adenocarcinoma located between the caecum and the right third of the transverse colon, without evidence of distant metastases. Central randomisation was done by means of the Clinical Information Management-Central Randomisation System via block randomisation (block size of four). Patients were randomly assigned (1:1) to CME or D2 dissection during laparoscopic right colectomy. Central lymph nodes were dissected in the CME but not in the D2 procedure. Neither investigators nor patients were masked to their group assignment but the quality control committee were masked to group assignment. The primary endpoint was 3-year disease-free survival, but the data for this endpoint are not yet mature; thus, only the secondary outcomes—intraoperative surgical complications and postoperative complications within 30 days of surgery, graded according to the Clavien-Dindo classification, mortality (death from any cause within 30 days of surgery), and central lymph node metastasis rate in the CME group only—are reported in this Article. This early analysis of safety was preplanned. The outcomes were analysed according to a modified intention-to-treat principle (excluding patients who no longer met inclusion criteria after surgery or who did not have surgery). This study is registered with ClinicalTrials.gov, NCT02619942. Study recruitment is complete, and follow-up is ongoing.FindingsBetween Jan 11, 2016, and Dec 26, 2019, 1072 patients were enrolled and randomly assigned. After exclusion of 77 patients, 995 patients were included in the modified intention-to-treat population (495 in the CME group and 500 in the D2 dissection group). The postoperative surgical complication rate was 20% (97 of 495 patients) in the CME group versus 22% (109 of 500 patients) in the D2 group (difference, −2·2% [95% CI −7·2 to 2·8]; p=0·39); the frequency of Clavien-Dindo grade I–II complications were similar between groups (91 [18%] vs 92 [18%], difference, −0·0% [95% CI −4·8 to 4·8]; p=1·0) but Clavien-Dindo grade III−IV complications were significantly less frequent in the CME group than in the D2 group (six [1%] vs 17 [3%], −2·2% [−4·1 to −0·3]; p=0·022); no deaths occurred in either group. Of the intraoperative complications, vascular injury was significantly more common in the CME group than in the D2 group (15 [3%] vs six [1%], difference, 1·8 [95% CI 0·04 to 3·6]; p=0·045). Metastases in the central lymph nodes were detected in 13 (3%) of 394 patients who underwent central lymph node biopsy in the CME group; no patient had isolated metastases to central lymph nodes.InterpretationAlthough the CME procedure might increase the risk of intraoperative vascular injury, it generally seems to be safe and feasible for experienced surgeons.FundingThe Capital Characteristic Clinical Project of Beijing and the Chinese Academy of Medical Sciences.
Laparoscopic Vs. Open Abdominal Radical Hysterectomy for Cervical Cancer: A Single-Institution, Propensity Score Matching Study in China

Abstract:Study Objective: To compare the surgical and oncologic outcomes between open abdomen radical hysterectomy (ARH) and laparoscopic radical hysterectomy (LRH) for cervical cancer. Methods: Retrospective observational study with propensity score matching was used to ensure balanced groups for ARH and LRH. One-hundred-and-ninety-eight women with cervical cancer, 99 treated using ARH and 99 using LRH, between January 2012 and December 2014. Outcomes included disease-free survival (DFS), overall survival (OS), intra-operative factors, post-operator recovery, urinary retention, and adverse events. Moreover, the inverse probability of the treatment weighting (IPTW) method was also used. Main Results: Compared with ARH, LRH was associated with a lower volume of blood loss (P < 0.001) and transfusion rate (P < 0.001), with a broader resection of the parametrium (P < 0.001). Post-operatively, the time to first flatus was shorter for LRH than ARH (P < 0.001) but the rate of urinary retention was higher for LRH (22.2%) than ARH (8.1%; P = 0.009). DFS and OS were similar between groups. By IPTW, laparoscopy was also not associated with poorer survival in terms of DFS (HR 1.52, CI 0.799-2.891, P = 0.202) or OS (HR 0.942, HR 0.425-2.09, P = 0.883). Conclusion: Compared with ARH, LRH provided better intra-operative and post-operative outcomes, with no significant difference in oncologic outcomes and survival. Urinary retention remains a clinical issue to improve with LRH. The technology of LRH has been improved in China to address the inconsistent results of oncologic outcomes in previous studies. Whether these improvements could be effective needs to be investigated in the future.
[Efficacy of Complete Mesocolic Excision in Radical Operation for Right Colon Cancer].

Abstract:OBJECTIVE:To investigate the efficacy of complete mesocolic excision (CME) in the radical operation for right hemicolon cancer.METHODS:Clinical data of 336 cases of right hemicolon cancer undergoing radical resection, including 218 cases of CME surgery group and 118 cases of traditional surgery group, from January 2005 to December 2014 in Department of Gastrointestinal Surgery, Renji Hospital, School of Medicine, Shanghai Jiaotong University were retrospectively analyzed. Intraoperational events, perioperative status and postoperative survival were compared between the two groups.RESULTS:The baseline information was not significantly different between the two groups (all P>0.05). The number of harvested lymph nodes in CME and traditional group was 11.4±0.3 and 9.3±0.5 respectively(P=0.000) and the proportion of greater than or equal to 12 lymph nodes per case was 47.3%(103/218) and 28.8%(34/118)(P=0.002), which both were significantly different. The operation time in CME and traditional group was (147.2±2.9) and (148.8±3.9) minutes, which was not significantly different (P>0.05), whereas operative blood loss was (125.7±7.5) and (305.1±20.5) milliliters in CME and traditional group with significant difference (P=0.000). Postoperative hospital stay was (12.9±0.9) and (16.3±1.0) days in CME and traditional group with significant difference (P=0.018), while the time to postoperative liquid intake and normal diet was not significantly different between two groups (both P>0.05). The morbidity of postoperative complication of CME group was lower compared to traditional group (14.2%, 31/218 vs. 24.6%, 29/118), which was significantly different (P=0.018). Among them, infection occurred in 19 (8.7%) cases and 21 (17.8%) cases with significant difference between the two groups (P=0.014). The average time of follow-up was (34.5±1.2) months and (27.9±1.5) months in CME and traditional group, and the five-year survival rate was 85.6% and 78.0% with significant difference(P=0.043). Moreover, 102 cases underwent laparoscopic-assisted CME and 116 cases underwent open CME in CME group. The 5-year survival rate was 89.8% and 82.2% in laparoscopic and open group with significant difference (P=0.048).CONCLUSION:Compared with traditional radical resection, CME radical resection for right hemicolon cancer can harvest more lymph nodes, decrease operative blood loss, lower the riskof postoperative complication, shorten the postoperative hospital stay, and increase the 5-year survival rate. Furthermore, laparoscopic-assisted CME has more advantages.

Abstract:AIM: Laparoscopic surgery, especially laparoscopic rectal surgery, for colorectal cancer has been developed considerably. However, due to relatively complicated anatomy and high requirements for surgery techniques, laparoscopic right colectomy develops relatively slowly. This study was designed to compare the outcomes of laparoscopic right hemicolectomy (LRH) with open right hemicolectomy (ORH) in the treatment of colon carcinoma.METHODS: Between September 2000 and February 2003, 30 patients with colon cancer who underwent LRH were compared with 34 controls treated by ORH in the same period. All patients were evaluated with respect to surgery-related complications, postoperative recovery, recurrence and metastasis rate, cost-effectiveness and survival.RESULTS: Among 30 LRH, 2 (6.7%) were converted to open procedure. No significant differences were observed in terms of mean operation time, blood loss, post-operative complications, and hospital cost between LRH and ORH groups. Mean time for bowel movement, hospital stay, and time to resume early activity in the LRH group were significantly shorter than those in the ORH group (2.24 +/- 0.56 vs 3.25 +/- 1.29 d, 13.94 +/- 6.5 vs 18.25 +/- 5.96 d, 3.94 +/- 1.64 vs 5.45 +/- 1.82 d respectively, P<0.05). As to the lymph node yield, the specimen length and total cost for operation and drugs, there was no significant difference between the two groups. Local recurrence rate and metachronous metastasis rate had no marked difference between the two groups. Cumulative survival probability at 40 mo in LRH group (76.50%) was not obviously different compared to the ORH group (74.04%).CONCLUSION: LRH in patients with colon cancer has statistically and clinically significant advantages over ORH. Hand-Assisted Laparoscopic Surgery Versus The Open Approach In Curative Resection Of Rectal Cancer

Abstract:This prospective, randomized study compared the outcomes of hand-assisted laparoscopic surgery (HALS) with open surgery for the resection of rectal cancer. The main outcome measures were procedure time, blood loss, post-operative pain, time to oral intake, return of bowel function, length of hospital stay, morbidity and functional recovery. Patients in each group were similar with regard to general status, procedure types and the histopathological features of tumours. Procedure times were significantly longer with HALS versus open surgery. Analgesic requirements, surgical blood loss, time to first passing flatus, time to first oral fluids and postoperative hospital stay length were all significantly shorter in the HALS group. At a median follow-up of 16.3 months, local recurrence of tumour was not observed in either group. In this study, the HALS approach for curative resection of rectal cancer was safe and effective and may offer several potential advantages to patients in their post-operative recovery.

Abstract:AIM:This study aimed to assess the safety and feasibility of laparoscopic curative resection for colorectal cancer through the clinical practice and basic research.MATERIAL AND METHODS:From September 2001 to September 2002, 47 patients with colorectal cancer were treated using laparoscopic approach, compared with 113 patients underwent traditional operation. The length of intestinal segment excised, size of tumour, clearance of lymph nodes, local recurrence and distant metastasis rate during the period of follow-up in both groups were compared. The other part of the study involved the detection of exfoliated tumour cells in the peritoneal washing before and after surgery; flushing of the instruments was performed in both groups and the results compared. For the laparoscopic cases, the filtrated liquid of CO(2) pneumoperitoneum was also checked for tumour cells.RESULTS:No significant differences existed in tumour size, operative site and manner between the two groups. The exfoliated tumour cell was not detected in the CO(2) filtrated liquid. Between both groups there was no difference in the incidence of exfoliated tumour cells in peritoneal washing before and after surgery as well as in the fluid used for flushing the instruments. The total number of lymph nodes harvested was 13.71±9.57 for the laparoscopic group and 12.10±9.74 for the traditional procedure. Similar length of colon was excised in both groups; this was (19.38±7.47) cm in the laparoscopic and (18.60±8.40) cm in the traditional groups. The distal margins of resection for rectal cancer were (4.19±2.52) cm and (4.16±2.00) cm respectively. The local recurrence rate was 2.13% (1/47) and 1.77% (2/113) with the distant metastasis rate 6.38% (3/47) and 6.19% (7/113) respectively. Both the statistics were comparable between the laparoscopic and traditional surgery for the colorectal cancer.CONCLUSION:Laparoscopic curative resection for colorectal cancer can be performed safely and effectively. In the treatment of colorectal malignancy, laparoscopic resection can achieve similar radicalilty as compared to the traditional laparotomy.

Abstract:Objective To compare the efficacy of laparoscopic complete mesocolic excision (CME) for right colon cancer with open CME. Methods The clinical data of the patients with right colon cancer in our department from January 2010 to December 2014 was retrospectively analyzed. There were 102 patients with laparoscopic CME as laparoscopic group and 116 patients with open CME as open group. Peri-operative outcomes and survival were compared between two groups. Rusults The baseline information was not significantly different between two groups (P>0. 05). Compared with open group, the patients in laparoscopic group had longer operating time [(155. 20±4. 17) min vs (140. 10±4. 00) min, P=0. 009 6], but less bleeding [(102. 60±7. 37) mL vs (145. 90±12. 23) mL, P=0. 003 7], more retrieved lymph nodes [(12. 17±0. 39) vs (10. 78±0. 42), P=0. 016 8], faster recovery course including shorter time to liquid diet [(2. 91±0. 47) day vs (3. 62±0. 41) day, P=0. 034] and shorter postoperative hospital stay [(10. 59±0. 57) day vs (14. 13±0. 52) day, P=0. 041]. No significant difference was observed in the postoperative complications between two groups. The period of follow-up had no significant difference between two groups [(38. 83±1. 73) month vs (30. 74±1. 60) month, P>0. 05]. Three-year survival rate of the patients in laparoscopic group was significantly higher than that in open group (89. 81％ vs 82. 22％, P=0. 048 2). Conclusions Laparoscopic CME provides the patients of right colon cancer with faster recovery and better overall survival.

Abstract:PURPOSE:To demonstrate the feasibility, safety, and technical strategies of hand-assisted laparoscopic complete mesocolic excision (HAL-CME) and to compare oncological outcomes between HAL-CME and the open approach (O-CME) for right colon cancers.METHODS:Patients who were scheduled to undergo a right hemicolectomy were divided into HAL-CME and O-CME groups. Measured outcomes included demographic variables, perioperative parameters, and follow-up data. Demographic variables included age, sex distribution, body mass index (BMI), American Society of Anesthesiologists (ASA) physical status classification, previous abdominal surgery, tumor localization, and potential comorbidities. Perioperative parameters included incision length, operative time, blood loss, conversion rate, postoperative pain score, postoperative first passage of flatus, duration of hospital stay, total cost, number of lymph nodes retrieved, TNM classification, and postoperative complications. Follow-up data included follow-up time, use of chemotherapy, local recurrence rate, distant metastasis rate, and short-term survival rate.RESULTS:In total, 150 patients (HAL-CME, 78; O-CME, 72) were included. The groups were similar in age, sex distribution, BMI, ASA classification, history of previous abdominal surgeries, tumor localization, and potential comorbidities. Patients in the HAL-CME group had shorter incision lengths, longer operative times, less operative blood loss, lower pain scores, earlier first passage of flatus, shorter hospital stay, higher total costs, similar numbers of lymph nodes retrieved, similar TNM classifications, and a comparable incidence of postoperative complications. The 2 groups were also similar in local recurrence rate, distant metastasis rate, and short-term survival rate.CONCLUSION:The results demonstrate that the HAL-CME procedure is a safe, valid, and feasible surgical method for right hemicolon cancers.
Cephalo-medial to lateral laparoscopic total mesorectal excision with left colic artery preserving for rectal cancer: a retrospective study

Abstract:Objective To assess the safety,feasibility,radical resection,short term outcome and surgical technique of cephalo-medial to lateral approach in laparoscopic radical rectal resection with left colic artery preserving for rectal cancer.Methods Retrospective analysis was performed with rectal patients in our hospital from July 2016 to December 2016.There were 32 cases of laparoscopic radical rectal resection with left colic artery preserving using cephalo-medial to lateral approach as CML group and 29 cases with traditional-medial to lateral approach as TML group.We aimed to investigate the surgical technique by comparison between two groups.Results There was no case needing conversion to open,with intraoperative severe complications and mortality.The operative time was (103.6±16.9) min in CML group with (17.2±2.7) min of lymph node dissection time,(95.3±21.5) mL blood loss and (17.1±7.2) lymph node harvest,which were similar to those in TML group.No.253 lymph node harvest was (3.3±1.2),which was significantly more than that in TML group (1.9±1.2,P＜0.05).There was no statistical difference of postoperative morbidity,postoperative pass flatus time and hospital stay between two groups (P＞0.05).Conclusions Cephalo-medial to lateral approach was safe and feasible for laparoscopic radical rectal resection with left colic artery preserving of rectal cancer.The operative time of cephalo-medial to lateral approach was comparable to the conventional approach.It could provide better No.253 lymph node dissection during the surgery.Further study with larger sample is required to evaluate the impact of cephalo-medial to lateral approach for laparoscopic radical rectal resection with left colic artery preserving of rectal cancer.

Abstract:Objective To investigate the safety and feasibility of transumbilical single-incision laparoscopy in radical operation for right colonic cancer. Methods A retrospective study was conducted in 60 patients who underwent laparosco-pic radical right hemicolectomy for colonic cancer from January 2019 to June 2020 in Department of Gastrointestinal Surgery Changshu Second People’s Hospital combined with Department of Surgery Ruijin Hospital Shanghai. The patients were divided into group of single-incision laparoscopic surgery(SILS) (n=30) and group of conventional five-port laparoscopic surgery(CLS) (n=30). Patient demographics, operative details and postoperative complications were compared between two groups. Results No significant differences were observed in length of incision, intraoperative blood loss, largest diameter of tumor, postoperative feeding time, extraction time of drainage tube, postoperative hospital stay, pathologic stage and lymph node harvest between SILS group and CLS group. Compared with CLS group, SILS group showed that all longer operative time [(185.17±12.28) min vs. (127.83±16.70) min], lower mean pain score (visual analog scale) [(3.30±0.79) vs. (3.97±0.72)] on the first postoperative day and earlier postoperative evacuation time [(2.70±0.54) d vs. (3.97±0.49) d)] with statistical significant difference (P<0.05). There were incision infections 2 cases in SILS group and 1 case in CLS group (P>0.05). One case with postoperative anastomotic leakage in both SILS group and CLS group (P>0.05). No tumor recurrence was present in 2 groups during the period of follow-up (11.0±0.6) months. Conclusions Clinical efficacy in single-incision laparoscopy would be similar to CLS for radical resection of right colonic cancer.
