Abstract:Background and study aims: Although peroral endoscopic myotomy (POEM) is credited with high success rates in the treatment of achalasia, persistent/recurrent symptoms may occasionally develop afterwards. Our purpose was to evaluate the feasibility, safety, and efficacy of repeat peroral endoscopic myotomy (Re-POEM) as salvage therapy after initial POEM failure.Patients and methods: Fifteen patients with persistence/recurrence of symptoms after previous POEM (Eckardt symptom score >= 4) were retrospectively selected from a prospectively maintained database housing a total of 1454 consecutive patients with achalasia. The primary endpoint was symptom relief during follow-up, defined by an Eckardt score of <= 3. Secondary outcome measures were procedure-related adverse events, change in manometric lower esophageal sphincter (LES) pressure, and reflux symptoms before and after Re-POEM.Results: All patients underwent successful Re-POEM a mean of 13.5 months (range 4-37 months) after execution of their primary POEM procedures. Mean operative time was 41.5 minutes (range 28-62 minutes). One instance of submucosal tunnel infection was successfully managed with conservative treatment. During a mean follow-up period of 11.3 months (range 3-18 months), therapeutic success was achieved in all patients. The mean symptom score pretreatment was 5.6 (range 4-8), compared with a post-treatment mean of 1.2 (range 0-3; P<0.001). Mean LES pressure also declined from 25.0 mm Hg to 9.5 mm Hg after Re-POEM (P<0.001). The overall clinical reflux complication rate of Re-POEM was 33.3 %.Conclusions: Re-POEM appears safe and effective as a salvage option after initial POEM failure, conferring short-term symptom relief and being free of serious complications in all patients.
Clinical value of repeated peroral endoscopic myotomy for persistent/ recurrent achalasia

Abstract:Objective To evaluate the feasibility, safety and efficacy of repeated peroral endoscopic myotomy (Re-POEM) as a make-up therapy after POEM failure. Methods Thirty-three patients with persistent / recurrent symptoms after first POEM (Eckardt symptom score≥4) were selected from a database of a total of 2516 consecutive patients with achalasia. The main outcome measures was Eckardt score during follow-up; the secondary outcome measures were procedure-related adverse events, changes in manometric lower esophageal sphincter (LES) pressure, and reflux symptoms before and after Re-POEM. Results All patients successfully underwent Re-POEM in mean 18. 6 months (ranging 3-55 months) after their first POEM procedures. The mean symptom score before Re-POEM was 5. 5 (ranging 4-8). Mean operation time was 45. 1 minutes ( ranging 28-64 minutes). Submucosal tunnel infection occurred in 1 patient who recovered with conservative treatment. During a mean follow-up period of 36. 6 months (ranging 12-58 months), symptom relief was achieved in all patients. Eckardt score reduced to 1. 3 (ranging 0-3), significantly different from that before (P< 0. 001). Mean LES pressure also declined from 26. 0 mmHg (1 mmHg = 0. 133 kPa) to 9. 6 mmHg after Re-POEM (P<0. 001). The incidence of gastroesophageal reflux of Re-POEM was 33. 3％(11/ 33). Conclusions Re-POEM appears safe and effective as a make-up option after POEM failure.

Abstract:Background and Aims Repeat peroral endoscopic myotomy (Re-POEM) appeared to be a promising salvage option for patients with persistent/recurrent symptoms after initial POEM, but it may be more technical challenging. Here we aim to evaluate the safety and technical difficulty of Re-POEM. Methods Between July 2012 and October 2023, 158 achalasia patients underwent Re-POEM were retrospectively enrolled. Another 2978 patients without prior myotomy were selected for naïve POEM. After propensity score matching (PSM), procedure-related parameters were compared between the two groups. Univariable and multivariable analyses were performed to reveal risk factors for difficult Re-POEM (defined as procedure time ≥ 90 min). Results With similar baseline characteristics between two groups after PSM, the procedure time was comparable between Re-POEM and naïve POEM groups (61.4±27.0 vs. 59.3±29.9, p=0.496). The Re-POEM group showed shorter esophageal myotomy length (7.0±2.1 vs. 8.2±1.1, p<0.001) and longer gastric myotomy length (2.3±0.6 vs. 2.1±0.4, p=0.017), compared with the naïve POEM group. Mucosal injury in Re-POEM group was slightly higher, which was not statistically significant (20.3% vs. 13.9%, p=0.135). No differences were found in the incidence of gas-related adverse events (AEs), major AEs, and technical difficulty. Multivariate analysis determined mucosal edema (OR=4.942, 95% CI, 1.554-15.714, p=0.007) and submucosal fibrosis (OR=3.817, 95% CI, 1.333-10.931, p=0.013) to be independent risk factors for difficult Re-POEM. Conclusions Re-POEM appears safe and feasible as a salvage option after initial POEM failure, with comparable procedure duration, incidence of AEs and technical difficulty to naïve POEM. Mucosal edema and submucosal fibrosis were associated with difficult Re-POEM.
Peroral Endoscopic Remyotomy for Failed Heller Myotomy: a Prospective Single-Center Study

Abstract:Background and study aims: Recurrence/persistence of symptoms occurs in approximately 20% of patients after Heller myotomy for achalasia. Controversy exists regarding the therapy for patients in whom Heller myotomy has failed. The aim of the current study was to evaluate the efficacy and feasibility of peroral endoscopic myotomy (POEM), a new endoscopic myotomy technique, for patients with failed Heller myotomy.Patients and methods: A total of 12 patients with recurrence/persistence of symptoms after Heller myotomy, as diagnosed by established methods and an Eckardt score of >= 4, were prospectively included. The primary outcome was symptom relief during follow-up, defined as an Eckardt score of <= 3. Secondary outcomes were procedure-related adverse events, lower esophageal sphincter (LES) pressure on manometry, reflux symptoms, and medication use before and after POEM.Results: All 12 patients underwent successful POEM after a mean of 11.9 years (range 2-38 years) from the time of the primary Heller myotomy. No serious complications related to POEM were encountered. During a mean follow-up period of 10.4 months (range 5-14 months), treatment success was achieved in 11/12 patients (91.7%; mean score pre- vs. post-treatment 9.2 vs. 1.3; P<0.001). Mean LES pressure was 29.4 mmHg pre-treatment and 13.5 mmHg post-treatment (P<0.001). One patient developed mild reflux symptoms and required intermittent medication with proton pump inhibitors.Conclusions: POEM seems to be a promising new treatment for failed Heller myotomy resulting in short-term symptom relief in >90% of cases. Previous Heller myotomy may make subsequent endoscopic remyotomy more challenging, but does not prevent successful POEM.

Abstract:Objective: peroral endoscopic myotomy (POEM), a relatively minimally invasive endoscopic procedure, is the first-line treatment for achalasia. The aim of this study was to compare procedure-related parameters and clinical outcomes between bypassing and performing prophylactic electrocoagulation of large submucosal vessels during POEM. Methods: one hundred and twelve patients with achalasia who had undergone POEM at our hospital between April 2017 and March 2023 were retrospectively enrolled. Large submucosal vessels were bypassed to avoid injury during submucosal tunneling in the bypass group, whereas large submucosal vessels were prophylactically treated by electrocoagulation in the prophylactic electrocoagulation group. Procedure-related parameters, Eckardt score and complications were compared between the two groups. Results: the bypass group showed a significant reduction in operative time and amount of intraoperative blood loss than the prophylactic electrocoagulation group (37.11 +/- 9.96 min vs 58.80 +/- 17.90 min, and 1 [interquartile range: 1-2] ml vs 5 [interquartile range: 3-8] ml; p < 0.001). Eleven (17.5 %) and 44 (89.8 %) patients in the bypass and prophylactic electrocoagulation groups, respectively, required hemostatic forceps ( p < 0.001). Furthermore, lower operative and hospitalization costs were recorded in the bypass group than in the prophylactic electrocoagulation group ( p < 0.05). No statistically significant difference was found between the two groups in terms of submucosal tunnel length, myotomy length, clinical efficacy, or complications. Conclusions: bypassing large submucosal vessels during POEM can reduce the operative duration and intraoperative blood loss, with no difference in clinical outcomes than treatment.

Abstract:Peroral endoscopic myotomy (POEM) originally developed by Inoue et al. is shown to be effective as a lesser invasive but curative treatment for esophageal achalasia. POEM has not yet been standardized and it is like endoscopic submucosal dissection technically difficult and time consuming. Results: With similar baseline characteristics between both groups after PSM, the salvage POEM group presented with shorter tunnel length (11.8 +/- 2.2 cm vs 12.8 +/- .9 cm, P < .0001) and myotomy length (9.8 +/- 2.0 cm vs 10.4 +/- 1.0 cm, P < .0001) than the naive POEM group. No significant differences were found in procedure-related adverse events between patients of salvage POEM and naive POEM. The primary outcome of treatment success occurred in 175 of 193 patients (90.7%) in the salvage POEM group versus 362 of 374 patients (96.8%) in the naive POEM group (P = .0046). At a 2-and 5-year follow-up, significantly higher rates of clinical failures were observed in the previous HM subgroup than in the previous POEM subgroup (P = .0433 and P = .0230, respectively). Conclusions: Salvage POEM after a previous myotomy failure, especially after a POEM failure, is a promising treatment option because it has a durable clinical relief rate.

Abstract:Introduction: Esophageal achalasia is most commonly treated with endoscopic dilation or laparoscopic Heller myotomy. Peroral endoscopic myotomy (POEM) has recently been described as a novel treatment for achalasia in humans. The aim of this study was to assess the clinical effectiveness and safety of POEM for treating esophageal achalasia performed by a gastroenterologist in a single endoscopic center within 4 years.Table 1: Baseline and clinical characteristics of patientsTable 2: Clinical outcomes of POEM procedureMethods: Between June 2011 to November 2015, POEM was performed in 153 consecutive patients with achalasia. POEM procedure consisted of the following step: firstly, submucosal tunnel was created and extended below the lower esophageal sphincter (LES) onto the gastric cardia after a mucosal incision was made; then endoscopic myotomy of circular muscle bundles was done; finally, the mucosal entry was closed by hemostatic clips (Figure 1). The Eckardt score and manometry were used to evaluate the outcomes.Figure 1Results: POEM was successfully performed in all cases. Mean procedure time was 51.3 ± 16.6 min and mean myotomy length was 8.4 ± 3.3 cm. Mucosal perforations occurred in 6 (3.9%) patients during submucosal tunnel creation, major bleeding occurred in 9 (5.9%) patients, and 7 (4.6%) patients suffered pneumothorax immediate after procedure. All the complications were managed conservatively. During a mean follow-up period of 25 months (range 6-59.4 months), 20 patients were lost, and treatment success was achieved in 124/133 patients (93.2%). Mean LES pressure was 45.1 mmHg and 15.3 mmHg before and after the procedure (P = 0.000), respectively. Mean Eckardt score was 8.3 and 1.4 before and after POEM, respectively (P = 0.000). 23 patient (17.3%) developed mild reflux symptoms and required intermittent medication with proton pump inhibitors during the follow-up. Conclusion: Our study demonstrated that POEM is a safe, and effective treatment for achalasia during the long-term follow-up. Further studies are warranted to compare POEM with other treatment modalities.

Abstract:BACKGROUND AND AIMS:Per-oral endoscopic myotomy (POEM) has received wide acceptance as a highly effective and safe treatment for esophageal achalasia. Short-term and small-scale studies are ample, but long-term large-scale studies are few. The aim of the study was to systematically analyze our long-term results of POEM, with particular emphasis on POEM failures and associated risk factors. METHODS:In this single-center study, consecutive patients treated with POEM between August 2010 and December 2012 were included. The Kaplan-Meier survival function was used to estimate clinical success rate at each year. The Cox proportional hazards model was used to analyze risk factors related to recurrence. RESULTS:A total of 564 patients were included. Major perioperative adverse events occurred in 36 patients (6.4%). After a median follow-up of 49 months (range, 3-68), the Eckardt score and lower esophageal sphincter (LES) pressure were significantly decreased (median Eckardt score, 8 to 2 [P < .05]; median LES pressure, 29.7 mm Hg to 11.9 mm Hg [P < .05]). Fifteen failures occurred within 3 months, 23 between 3 months and 3 years, and 10 after 3 years. The estimated clinical success rates at 1, 2, 3, 4, and 5 years were 94.2%, 92.2%, 91.1%, 88.6%, and 87.1%, respectively. Multivariate Cox regression revealed long disease duration (≥10 years) and history of prior interventions to be risk factors for recurrence. Clinical reflux occurred in 37.3% of patients (155/ 416). CONCLUSIONS:POEM is a highly safe and effective treatment for esophageal achalasia with favorable long-term outcomes.

Abstract:Summary The aim of this review is to provide an overview of per-oral endoscopic myotomy (POEM) and its utilization in non-achalasia disorders of the esophagus. POEM, a relatively novel endoscopic technique, involves submucosal tunneling to access esophageal muscle layers, enabling selective myotomy and mitigating the consequences of motor disorders of the esophagus. POEM is an effective treatment modality for diffuse esophageal spasm providing resolution of chest pain and dysphagia in a majority of patients who have refractory symptoms despite medical therapy. The results of POEM are more equivocal compared to esophagogastric junction outflow obstruction (EGJOO). POEM in EGJOO has been shown to have a 93% clinical success rate in 6 months. POEM appears to be more effective in motor disorders that affect the lower esophageal sphincter, such as EGJOO and opioid-induced esophageal dysfunction. While the current data for POEM in other entities such as DES and HE are positive, more supportive data are required to make POEM a consistent recommendation for patients.

Effect of peroral endoscopic myotomy on achalasia with ineffective prior treatment

Abstract:Objective:To evaluate the effect of peroral endoscopic myotomy (POEM) on perioperative and follow-up outcomes of achalasia patients undergoing unsuccessful prior treatment.Methods:A retrospective analysis was performed on 849 patients with achalasia who underwent POEM and completed follow-up at endoscopy center, Zhongshan Hospital, Fudan University, from August 2010 to December 2014. The effect of POEM was compared between patients with prior treatment (n=245) and those without (n=604). Risk factors for perioperative major adverse events, postoperative clinical reflux and failure were analyzed.Results:A total of 34 patients (4.0%, 34/849) experienced major adverse events, such as pneumothorax, hydrothorax, delayed bleeding. No statistical difference in major adverse events was found between the two groups [4.5% (11/245) VS 3.8% (23/604), χ2=0.21, P=0.65]. Multivariate logistic regression analysis demonstrated that procedure duration ≥60 min was the independent risk factor for perioperative major adverse events ( OR=3.82, 95% CI: 1.81-8.08, P<0.05), while endoscopist experience >100 cases was the independent protective factor for major adverse events ( OR=0.23, 95% CI: 0.07-0.76, P<0.05). During a median follow-up of 23 months (ranging 1-71 months), clinical reflux occurred in 203 patients (23.9%, 203/849), and clinical failure occurred in 94 patients (11.1%, 94/849). Patients with prior treatment had a clinical reflux rate of 26.9% (66/245), which was comparable with those without (22.7%, 137/604) ( P=0.19). Multivariate logistic regression analysis demonstrated that Heller myotomy was the independent risk factor for clinical reflux ( OR=1.49, 95% CI: 0.98-2.29, P=0.07). Clinical failure rate in patients with prior treatment at 5 years after POEM was 18.0% (44/245), and that in those without was 8.3% (50/604) ( P<0.05). Multivariate cox analysis revealed that disease duration ≥10 years ( HR=1.62, 95% CI: 1.04-2.52, P=0.03) and prior treatment ( HR=1.90, 95% CI: 1.26-2.88, P<0.05) were independent risk factors for clinical failure. Conclusion:POEM is an effective and safe rescue for patients who underwent unsuccessful prior treatment, with a low rate of perioperative major adverse events and good long-term effects. Prior treatment may increase the risk for clinical failure of POEM, it may not significantly increase severe perioperative complications or clinical reflux though.

Abstract:A circular muscle myotomy preserving the longitudinal outer esophageal muscular layer is often recommended during peroral endoscopic myotomy (POEM) for esophageal achalasia, which is different from the usual full-thickness myotomy performed surgically. Because the longitudinal muscle fibers of the esophagus are extremely thin, unintentional splitting frequently occurs during POEM. Given the fact that an incomplete myotomy with time progresses healing of myotomy edges by fibrosis may be considered as the major reason for postoperative recurrence, an intentional full-thickness myotomy may reduce the incidence of fusion by further spreading of the cut muscular wall and subsequently induce long-term symptom remission.
