Abstract:Objective To evaluate the accuracy of three simple methods measuring insulin sensitivity in patients with impaired glucose tolerance(IGT). Method Thirteen essential hypertensives with IGT and normal fasting blood glucose were investigated, whose insulin sensitivities were evaluated by euglycemic hyperinsulinemic clamp technique. The M value obtained in “clamp” technique was compared to the results of three simple indices. Results ISI-composite and ISI-cederholm, the two insulin sensitivity indices derived from OGTT, were significantly correlated with M value by glucose clamp (r=0.687, 0.594, P0.05, respectively). The correlations were closer after adjustment for the function index of β (partial r=0.632, 0.640, P0.05). However, HOMA-IR was not in related to the M value of glucose clamp, even after adjustment for function index of β cell (r=-0.373 and -0.353, respectively, P0.05). Conclusion For clinical evaluation of insulin sensitivity in patients with IGT, ISI-composite and ISI-cederholm are closely correlated with the golden standard, the M value obtained by glucose clamp technique, even after adjustment for function index of β cell, whereas HOMA-IR is not satisfying.
Approach and Accuracy of Electrocardiogram in Diagnosis of Left Ventricular Hypertrophy in Essential Hypertension

Abstract:Objective To evaluate sensitivity and specificity of Sokolow- Lyon index, Cornell index and Chinese criteria in diagnosis of left ventricular hypertrophy in essential hypertension. Methods Two hundred eighty cases of es sential hypertension were divided into left ventricular hypertrophy group and no left ventricular hypertrophy group according to echocardiography. Diagnostic se nsitivity and specificity of Sokolow-Lyon index, Cornell index and Chinese crite ria were calculated in each group, respectively. Results Sensitivity of Chine se criteria was significantly lower than that of Cornell index (18.5% vs. 35.3%, P0.05) and similar to that of Sokolow-Lyon index (18.5% vs. 21.8%, P0.05). Sp ecificity of Chinese criteria was significantly higher than that of Cornell inde x (97.6% vs. 91.9%, P0.05) and similar to that of Sokolow-Lyon index (97.6% vs. 96.2%, P0.05). Conclusion Sokolow-Lyon index is simple and easily practiced but with low sensitivity. Cornell index needs a computer or a magnifying method to measure QRS duration and is difficult to application though it has relatively higher sensitivity than two other methods. Chinese criteria has low sensitivity and high specificity.
[Insulin Secretion and Insulin Sensitivity in 1193 Inpatients with Essential Hypertension].

Abstract:To evaluate the metabolic profiles of insulin secretion and insulin sensitivity in different categories of nondiabetic patients with essential hypertension, we made a retrospecting study in our hospital.According to the criteria of WHO and ADA, we calculated the distribution of plasma glucose levels during a 75 g oral glucose tolerance test (OGTT). Nondiabetic subjects with hypertension were classified into the following four groups: (1) normal glucose tolerance (NGT) with fasting plasma glucose (FPG) < 6.1 mmol/L and 2h-plasma glucose (2hPG) < 7.8 mmol/L, (2) Isolate impaired fasting glucose (IFG) with FPG 6.1-7 mmol/L and 2hPG < 7.8 mmol/L, (3) Isolate impaired glucose tolerance (IGT) with FPG < 6.1 mmol/L and 2hBG 7.8-11.1 mmol/L, (4) impaired fasting glucose and impaired glucose tolerance (IFG/IGT) with FPG 6.1-7 mmol/L and 2hBG 7.8-11.1 mmol/L. Then we divided the groups of NGT and IGT based on the normal 30 or 60 minute plasma glucose level during OGTT respectively. 30 minute plasma glucose level < 11.1 mmol/L and 60 minute plasma glucose level < 10.5 mmol/L were considered to be normal. HOMA-IR (insulin resistance) and composite ISI (insulin sensitivity index) were used to estimate insulin resistance and HOMA-beta and DeltaI/DeltaG index to estimate insulin secretion.11.1% of the 1193 subjects were newly diagnosed diabetic patients, in whom 57.9% had isolated postchallenge hyperglycemia only. In the IGT and IFG/IGT groups, HOMA-IR were significantly higher than that in the NGT group, whereas composite ISI and DeltaI/DeltaG were significantly lower than that in the NGT group. Whether 30 or 60 minute plasma glucose level was high or not, composite ISI in IGT group was significantly lower than that in the NGT group. In the NGT group, DeltaI/DeltaG in subjects with high 30 or and 60 minute plasma glucose level was significantly lower than that in subjects with normal 30 and 60 minute plasma glucose level.Subjects with IGT or IFG/IGT have both impaired insulin sensitivity and insulin secretion. In the NGT group, subjects with high 30 or/and 60 minute plasma glucose level have impaired beta-cell function.
Insulin Sensitivity in Hypertensives with Different Degrees of Metabolic Disorder

Abstract:Objective To evaluate the clinical significance of various metabolic dicoraders in detecting insulin resistance (IR) in non diabetics hypertensive patients. Methods Insulin sensitivity index (S I) obtained from reduced sample number modified minimal model (MMM 12 ) was validated by euglycemic hyperinsulinemic clamp technique. Analyses of S I were performed in 118 hypertensives with various metabolic disorder and 40 controls. Results S I obtained from MMM 12 was closely related to the M value by euglycemic clamps ( r =0 70, P =0 012). Logistic regression showed that high waist hip ratio (WHR), impaired glucose tolerance (IGT) and high triglyceride(TG) were independent predictors of IR . The prevalence of IR in hypertensives with one, two or three risk factors in metabolic disturbances was 64.3%, 84.1%, 100%, respectively. After adjusting for age and gender, S I was significantly lower in hypertensives with metabolic disturbances than that in controls, while no difference in S I was found between hypertensives without any metabolic disturbance and controls. Patients with high BMI had lower S I than in controls and S I was much lower in patients with high WHR . After adjusting for other factors, S I was decreased progressively with TG. Even in subgroup with TG 1.1～1.7 mmol/l, insulin sensitivity was deteriorated. Conclusions (1) Minimal model is accurate in evaluating insulin sensitivity in hypertensives with metabolic disturbances. (2) No correlation was observed between hypertension itself and IR, their relationship may be mediated by metabolic disturbances. Visceral obesity, IGT and high TG are independent predictors of IR. (3) Patients with visceral obesity are more inclined to combining IR than with whole obesity; (4) Control of TG level should be more active.
Study of Insulin Resistance among Chinese Population over 40 in Shanghai Area

Abstract:Objective To evaluate the reliability of homeostasis model assessment (HOMA) for estimating in sulin sensitivity, as well as to investigate the insulin resistance of base-line survey people by HOMA-IR. Methods The hyperinsulinemic-euglycemic clamp combined with 3-3H glucose infusion were performed in 29 Chinese individuals to determine the glucose disposal rate and assess the sensitivity, specificity and accuracy of HOMA-IR (HOMA-insulin resistance index). The current status of insulin sensitivity in Shanghai Huayang community survey( n = 1873)was esti mated by HOMA-IR. Results 1. In comparison with the glucose disposal rate by glucose clamp, the sensitivity、speci ficity、accuracy of HOMA-IR was 86.4% 、71.4% and 82.8%, respectively. 2. In diabetes、impaired glucose tolerance and/or impaired fasting glucose、hypertriglyceridemia and/or low high-density-lipoprotein cholesterol(H-TG/L-HDL)and hypertensives, about 62.5%、41.7%、30.8% and 32.0% suffered from insulin resistance respectively. 3. With the rise of BMI or metabolic disorders, the frequency of insulin insensitivity was also increased (P for trend, P ＜ 0.001 ). 4. Results of logistic regression analysis revealed that insulin resistance had a major effect on hyperglycemia, and that on H-TG/L-HDL being the next. Conclusion HOMA-IR in the estimation of insulin resistance has a higher accuracy, while insulin resistance is common among the adults, especially in those with metabolic disorders. Abstract:Objective To inv estigate the insulin sensitivity in hype rtensive patients with or without lacuna r infarction.Methods Study i ncluded 30 normal subjects (control grou p)and 143 hypertension patients.All subj ects were divided into three groups:cont rol group (n=30);according to the result s of MRI,143 hypertension patients were again divided into two subgroups:hyperte nsive patients with lacunar infarction ( n=103) and without lacunar infarction (n =40).All subjects condrcted oral glucose tolerance test (OGTT) and insulin relea se test (InRT).All data were compared wi th three different calculation methods:1 /(FPG×FIns),AUC OGTT /AUC InRT and Ce derholm-ISI.Then to analyse whether insu lin resistance is a risk factor in hyper tensive patients with lacunar infarction .Results (1)1/(FPG×FIns),AUC OGTT /AUC InRT and Cederholm-ISI had positive correlation with each other (r =0 306,r=0 534,r=0 547, P 0 01,respectively).( 2)The significant difference of cederhol m-ISI was observed in hypertensive patie nts with and without lacunar infarction ( P 0 05).Conclusions (1)Ceder holm-ISI seems more sensitive than 1/(FP G×FIns) and AUC OGTT /AUC InRT to eva luate the insulin-sensitivity in hyperten sive patients.(2)Insulin resistance may be the risk factor of cerebral microvasc ular disease.

Study on Insulin Sensitivity and Early Β Cell's Function in Patients with Metabolic Syndrome

Abstract:Objective To study insulin sensitivity and early islet β cell's function in simple obesity, simple hypertension, obesity with impaired glucose tolerance(IGT), hypertension with IGT and obesity with type 2 diabetes (T2DM). Methods Insulin sensitivity index (S I),and glucose effectiveness (S G) were assessed by the reduced sample number of the Bergman's minimal model method with the frequently sampled intravenous glucose tolerance (FSIGT) test in the subjects of each target group. Meanwhile the early islet β cell's function was also evaluated by the following two parameters:I p-b stands for the difference between the peak insulin value and the basal insulin value after glucose injection in FSIGT. Results 1. There was no significant different S G in the control, simple obesity and simple hypertension groups. The values of S G in the three groups were significantly higher than those in the obesity with IGT, hypertension with IGT and T2DM groups, and there was no significant different S G in the latter three groups. 2. The S I in the controls was significantly bigger than that in the 5 pathogenic groups. 3. Compared with the normal group, the values of I p-b in the simple obesity and simple hypertension groups were significantly elevated; and the values of I p-b were markedly decreased in the obesity with IGT and T2DM groups, and there was significant difference between the latter twogroups (P0.02). Conclusion The early islet β cell's function is normal in simple hypertension, and hyperergasia appears in the simple obesity and hypertension with IGT groups. In obesity with IGT, the first-phase insulin secretion deficiency of β cells is apparent, and in T2DM this defect is markedly aggravated.
Evaluation of insulin sensitivity by hyperinsulinemic-euglycemic clamp technique in Chinese patients with type 1 diabetes mellitus

Abstract:Objective To investigate insulin sensitivity in Chinese adult patients with type 1 diabetes mellitus through hyperinsulinemic?euglycemic clamp technique. Methods Adult patients with type 1 diabetes mellitus (n=10,group T1DM) and age, sex, body mass index, waist and waist?hip ratio matched normal glucose tolerance subjects (n=10,group NGT) were recruited from the third affiliated hospital of Sun Yat?sen University between June 2011 and June 2013. There were 5 male and 5 female in each group. The average age was (27 ± 5) and (27 ± 8) yrs in each group,respectively. Hyperinsulinemic?euglycemic clamp were performed to measure glucose disposal rate as insulin sensitivity. Differences between groups were analyzed by Student′s t test. Results No significant differences were found in age, waist, waist?hip ratio, body mass index, systolic blood pressure, diastolic blood pressure, triglyceride, low?density lipoprotein?cholesterol, high?density lipoprotein?cholesterol and uric acid between type 1 diabetes mellitus group and normal glucose tolerance group (t=-1.248-0.973, all P>0.05), except for glycated hemoglobin A1c ((7.5 ± 0.8)% vs (5.2 ± 0.3)%, t=-6.318, P<0.001). Glucose disposal rate in type 1 diabetes mellitus and normal glucose tolerance groups were (7.8±1.9) mg·kg?1·min?1 and (14.0±1.7) mg·kg?1·min?1 (t=7.769, P<0.001), respectively. Conclusion Insulin resistance may be present in Chinese patients with adult type 1 diabetes mellitus.
Exploration and Validation of the Performance of Hemoglobin A1c in Detecting Diabetes in Community-Dwellers With Hypertension

Abstract:BACKGROUND: Diabetes can complicate hypertension management by increasing the risk of cardiovascular disease (CVD) and all-cause mortality. Studies targeting diabetes detection in hypertensive individuals demonstrating an increased risk of diabetes are lacking. We aimed to assess the performance of hemoglobin A1c (HbA1c) and its cut-off point in detecting diabetes in the abovementioned population.METHODS: Data from 4,096 community-dwellers with hypertension but without known diabetes were obtained from the Study on Evaluation of iNnovated Screening tools and determInation of optimal diagnostic cut-off points for type 2 diaBetes in Chinese muLti-Ethnic (SENSIBLE) study; these data were randomly split into exploration (70% of the sample) and internal validation (the remaining 30%) datasets. The optimal HbA1c cut-off point was derived from the exploration dataset and externally validated using another dataset from 2,431 hypertensive individuals. The oral glucose tolerance test was considered the gold-standard for confirming diabetes.RESULTS: The areas under the ROC curves for HbA1c to detect diabetes were 0.842, 0.832, and 0.829 for the exploration, internal validation, and external validation datasets, respectively. An optimal HbA1c cut-off point of 5.8% (40 mmol/mol) yielded a sensitivity of 76.2% and a specificity of 74.5%. Individuals who were not diagnosed as having diabetes by HbA1c at 5.8% (40 mmol/mol) had a lower 10-year CVD risk score than those diagnosed as having diabetes (P=0.01). HbA1c≤5.1% (32 mmol/mol) and ≥6.4% (46 mmol/mol) could indicate the absence and presence of diabetes, respectively.CONCLUSIONS: HbA1c could detect diabetes effectively in community-dwellers with hypertension.

Correlation Between Blood Pressure and Insulin Resistance and Secretion in Population with Normal Glucose Tolerance.

Abstract:Objective To determine the correlation between blood pressure and insulin resistance and insula B cell function in population with normal glucose tolerance.Methods We did a population-based study in Jiangsu province using multi-stage,stratified,randomized cluster sampling method.955 consecutive subjects aged 18 years as having normal oral glucose tolerance were enrolled on the basis of oral glucose tolerance test and were further assigned into normotensive group,prehypertension group,grade 1 hypertension group,grade 2 hypertension group and grade 3 hypertension group,respectively,according to grading of blood pressure.Insulin sensitivity was measured by using Matsuda index(ISIM) and homeostasis model assessment of insulin resistance(1/HOMA-IR),whilst insula B cell function was determined by HOMA-β,InsAUC30/GluAUC30 and InsAUC120/GluAUC120.Results Prehypertension group,grade 1 hypertension group,grade 2 hypertension group and grade 3 hypertension group yielded a reduction of 11.1%,14.8%,24.1% and 33.3% in 1/HOMA-IR(all P0.001) and a decreased of 10.3%,15.5%,27.6% and 37.1% in ISIM(all P0.001),respectively,as compared with normotensive group.This was associated with an increase of 11.1%,41.3%,42.9% and 101.2% in InsAUC30/GluAUC30(all P0.001) and the elevation of 13.2%,46.5%,54.2% and 96.9% in InsAUC120/GluAUC120(all P0.001),respectively.All groups were noted as having increased HOMA-β(all P0.05).Additionally,distinction in the disposition index(DI),including baseline DI(DI0),early-phase DI(DI30) and total-phase DI(DI120),did not reach statistical significance among groups(all P0.05).Conclusion Increased blood pressure in population with normal glucose tolerance is correlated with gradual reduction in insulin sensitivity and hence,development of insulin resistence.This may be responsible for progressive increase in insulin secretion,but not impaired insula secretion capacity.
Validation of Insulin Sensitivity Surrogate Indices and Prediction of Clinical Outcomes in Individuals with and Without Impaired Renal Function.

Abstract:As chronic kidney disease (CKD) progresses with abnormalities in glucose and insulin metabolism, commonly used insulin sensitivity indices (151s) may not be applicable in individuals with CKD. Here we sought to validate surrogate ISls against the glucose disposal rate by the gold-standard hyperinsulinemic euglycemic glucose clamp (HEGC) technique in 1074 elderly men of similar age (70 years) of whom 495 had and 579 did not have CKD (estimated glomerular filtration rate (eGFR) under 60 ml/min per 1.73 m2 (median eGFR of 46 ml/min per 1.73 m2)). All ISls provided satisfactory (weighted K over 0.6) estimates of the glucose disposal rate in patients with CKD. ISls derived from oral glucose tolerance tests (OGTTs) agreed better with HEGC than those from fasting samples (higher predictive accuracy). Regardless of CKD strata, all ISls allowed satisfactory clinical discrimination between the presence and absence of insulin resistance (glucose disposal rate under 4 mg/kg/min). We also assessed the ability of both HEGC and ISls to predict all-cause and cardiovascular mortality during a 10-year follow-up. Neither HEGC nor ISIs independently predicted mortality. Adjustment for renal function did not materially change these associations. Thus, ISls can be applied in individuals with moderately impaired renal function for diagnostic purposes. For research matters, OGTT-derived ISls may be preferred. Our data do not support the hypothesis of kidney function mediating insulin sensitivity (I5)-associated outcomes nor a role for IS as a predictor of mortality

Abstract:Objective To study insulin sensitivity and first-phase insulin secretion in obese subjects with impaired glucose regulation (ICR) in Shanghai. Methods A total of 129 subjects [38 lean controls and 91 obese subjects with ICR including 64 isolated impaired glucose tolerance (IGT) ,8 isolated impaired fasting glucose (IFC) and 19 IFC + ICT] underwent 75 g oral glucose tolerance test and insulin-modified reduced sample number (n = 12) of Bergman's minimal model method with frequently sampled intravenous glucose tolerance test (FSIGTT). Insulin resistance was determined from the insulin sensitivity index (S1) of the FSIGTT. Insulin secretion was determined during the FSIGTT by the acute insulin response to glucose (AIRg). The disposition index (DI) , the product of AIRg and S1, was used to determine whether AIRg was adequate to compensate for insulin resistance. Results (1) Compared with normal controls, the value of S1 was significantly decreased in 3 groups with ICR (all P<0.01) and there was no significant difference in S1 among the 3 groups; (2) AIRg in control group was similar to that of the IGT group and was significantly greater than those in IFG and IFG + IGT group ( P < 0.05 or P < 0.01), and the difference between IGT and IFG + IGT groups was significant (P < 0.01); (3) Compared with normal group, the DI value was significantly decreased in the 3 groups with ICR (all P< 0.01) , and there was no significant difference among these 3 ICR groups; (4) S1 was negatively correlated with fasting insulin, body mass index, and serum uric acid (adjusted r2 = 0. 568, P < 0. 01), AIRg was positively correlated with 2 h insulin and negatively correlated with fasting plasma glucose, 2 h plasma glucose and age (adjusted r2 =0.402, P<0.01). Conclusion Newly diagnosed obese patients with ICR (including isolated IGT, isolated IFG and IFG + ICT) in Shanghai have a similar degree of insulin resistance, after adjusting to insulin resistance, AIRg is still normal in individuals with IGT but lower in those with IFG and IFG + IGT, yet the compensatory function for insulin resistance forβcells is lost in the IGR groups to the same extent. Insulin resistance andβcell dysfunction coexist in patients with IGT. Compared with non-obese IGT patients, the IGT patients with obesity show severe insulin resistance but higher first phase insulin secretion.
Development and validation of machine learning-augmented algorithm for insulin sensitivity assessment in the community and primary care settings: a population-based study in China

Abstract:Objective: Insulin plays a central role in the regulation of energy and glucose homeostasis, and insulin resistance (IR) is widely considered as the "common soil" of a cluster of cardiometabolic disorders. Assessment of insulin sensitivity is very important in preventing and treating IR-related disease. This study aims to develop and validate machine learning (ML)-augmented algorithms for insulin sensitivity assessment in the community and primary care settings. Methods: We analyzed the data of 9358 participants over 40 years old who participated in the population-based cohort of the Hubei center of the REACTION study (Risk Evaluation of Cancers in Chinese Diabetic Individuals). Three non-ensemble algorithms and four ensemble algorithms were used to develop the models with 70 non-laboratory variables for the community and 87 (70 non-laboratory and 17 laboratory) variables for the primary care settings to screen the classifier of the state-of-the-art. The models with the best performance were further streamlined using top-ranked 5, 8, 10, 13, 15, and 20 features. Performances of these ML models were evaluated using the area under the receiver operating characteristic curve (AUROC), the area under the precision-recall curve (AUPR), and the Brier score. The Shapley additive explanation (SHAP) analysis was employed to evaluate the importance of features and interpret the models. Results: The LightGBM models developed for the community (AUROC 0.794, AUPR 0.575, Brier score 0.145) and primary care settings (AUROC 0.867, AUPR 0.705, Brier score 0.119) achieved higher performance than the models constructed by the other six algorithms. The streamlined LightGBM models for the community (AUROC 0.791, AUPR 0.563, Brier score 0.146) and primary care settings (AUROC 0.863, AUPR 0.692, Brier score 0.124) using the 20 top-ranked variables also showed excellent performance. SHAP analysis indicated that the top-ranked features included fasting plasma glucose (FPG), waist circumference (WC), body mass index (BMI), triglycerides (TG), gender, waist-to-height ratio (WHtR), the number of daughters born, resting pulse rate (RPR), etc. Conclusion: The ML models using the LightGBM algorithm are efficient to predict insulin sensitivity in the community and primary care settings accurately and might potentially become an efficient and practical tool for insulin sensitivity assessment in these settings.

Abstract:Objective To evaluate the insulin resistance status of patients with different glucose tolerance by using frequently sampled intravenous glucose tolerance test(FSIGT).Methods Total 160 non-diabetic patients with different glucose tolerance in our hospital from June 2008 to June 2009 were selected and divided into the normal glucose tolerance(NGT) group,impaired fasting glucose(IFG) group,impaired glucose tolerance(IGT) group and IFG+IGT group.The glucose tolerance status was analysed by oral glucose tolerance test,and then the degree of insulin resistance was evaluated by FSIGT.Results Among the four groups,there were no significant differences in age,body mass index(BMI),waist-to-hip ratio(WHR),systolic blood pressure,diastolic blood pressure,smoking history and lipid parameters.The insulin sensitivity index(ISI) was lower in IFG,IGT and IFG+IGT groups than that in NGT group.But significant difference was only found between IGT group and NGT group(P<0.05).Conclusion Detection of insulin resistance status by using FSIGT has important guiding significance in IGT screening,early diagnosis and prevention of diabetes.

Abstract:OBJECTIVE:To investigate the impact of dyslipidaemia on insulin resistance in Chinese individuals in Shanghai.METHODS:830 individuals aged over 40 years (male 300, female 530) were divided into normal and dyslipidaemia groups. The latter included 7 subgroups as follows: low high density lipoprotein (L-HDL)-subgroup I, high triglycerides (H-TG)-subgroup II, high total cholesterol or high low density lipoprotein (H-TC/H-LDL)-subgroup III, low high density lipoprotein with high triglycerides (L-HDL + H-TG)-subgroup IV, low high density lipoprotein with high total cholesterol or high low density lipoprotein (L-HDL + H-TC/H-LDL)-subgroup V, high triglycerides with high total cholesterol or high low density lipoprotein(H-TG + H-TC/H-LDL)-subgroup VI, low high density lipoprotein with high triglycerides and high total cholesterol or high low density lipoprotein (L-HDL + H-TG + H-TC/H-LDL)-subgroup VII. Homeostasis model assessment (HOMA) was applied to estimate the degree of insulin resistance (IR).RESULTS:(1) Adjusted with age, sex and body mass index (BMI), HOMA-IR was increased in the dyslipidaemia subgroups with high triglycerides. (2) Hypertriglyceridemia was more closely related to raised BMI and waist-hip ratio (WHR). (3) Both body fat (BMI, WHR) and triglycerides were independent risk factors accounting for HOMA-IR.CONCLUSION:Hypertriglycerides could be regarded as an independent risk factor for insulin resistance.
Evaluating the Clinical Accuracy of a Non-invasive Single-Fasting-Calibration Glucometer in Patients with Diabetes: A Multicentre Study

Abstract:Introduction: The aim of this study was to evaluate the stability and accuracy of glucose measurements determined using the metabolic heat conformation (MHC)-based non-invasive glucometer in a multicentre, self-controlled clinical trial. This device is the first to obtain a medical device registration certificate awarded by the National Medical Products Administration of China (NMPA). Methods: The multicentre clinical study was conducted at three sites and enrolled 200 subjects whose glucose was measured with a non-invasive glucometer (the Contour Plus blood glucose monitoring system) and by venous plasma glucose (VPG) measurements, in a fasted state and at 2 and 4 h after meals. Results: Based on both the non-invasive and VPG measurements, 93.9% (95% confidence interval 91.7-95.6%) of the blood glucose (BG) values fell within consensus error grid (CEG) zones A + B. The measurements obtained in a fasted state and at 2 h after meals were more accurate, with 99.0% and 97.0% of the BG values, respectively, falling within zones A + B. Compared to those subjects who received insulin, the proportion of values in zones A + B and the correlation coefficients were 3.1% and 0.0596 higher, respectively. The accuracy of the non-invasive glucometer was influenced by the level of insulin resistance calculated by the homeostatic model assessment method, which had a correlation coefficient with the mean absolute relative difference of - 0.1588 (P = 0.0001). Conclusion: The MHC-based non-invasive glucometer assessed in the present study demonstrates generally high stability and accuracy in the glucose monitoring of people with diabetes. The calculation model needs to be further explored and optimised for patients with different diabetes subtypes, levels of insulin resistance and insulin secretion capacity. Detection of diabetes and prediabetes using hemoglobin A1c in Chinese adults living in Shanghai: a prospective analysis.

Abstract:Background This study aimed to evaluate the discriminative abilities of glycosylated hemoglobin (HbA1c) and to examine the optimal HbA1c cutoff values for diabetes and prediabetes in Chinese adults. Methods Data of a population-based cohort of Chinese adults aged >= 40 years living in Jiading District in Shanghai were used. At baseline, 9389 and 7241 participants were included to identify the optimal HbA1c cutoff values for diabetes and prediabetes, respectively using the 1999 World Health Organization criteria as reference. In addition, the follow-up data on incident diabetes of 4538 participants were used to determine the HbA1c cutoff value for prediabetes using the development of diabetes as reference. The discriminative abilities of HbA1c were evaluated using receiver operating characteristic (ROC) curves, and the optimal cutoff values were determined by Youden's index. Results The areas under the ROC curves were 0.849 for diabetes, 0.614 for prediabetes using baseline data, and 0.648 for prediabetes using follow-up data. An HbA1c cutoff value of 6.0% had the largest Youden's index to diagnose diabetes with a sensitivity of 70.2% and a specificity of 87.4%. An HbA1c cutoff value of 5.6% was indicated for prediabetes using both baseline and follow-up data. However, the sensitivity and specificity were both low (55.4% and 61.1% using an oral glucose tolerance test as reference, 64.6% and 57.1% using incident diabetes as reference). Conclusions An HbA1c value >= 6.0% could be used to detect diabetes in Chinese adults aged >= 40 years. However, although an HbA1c value of 5.6% to 5.9% was indicated in this study, the overall discrimination of HbA1c for prediabetes was poor.

Abstract:Objective To validate the accuracy of finger blood glucose test result by comparing the results of finger blood sample and venous blood sample,so as to provide evidence for self monitoring of blood glucose(SMBG) and blood glucose management in diabetes patients.Methods A total of 108 diabetic patients,who visited out endocrinology outpatient department,were randomly selected.The finger tip capillary blood and synchronous venous blood samples were collected under fasting condition and 2h postprandial using 3 glucose meters of different brands.Results The fasting plasma glucose was(6.28±1.76) mmol/L and the 2 h postprandial plasma glucose was(9.69±4.43) mmol/L in the venous blood samples.The fasting and 2 h postprandial finger blood glucose levels by 3 different meters were(6.53±1.56),(10.41±4.79) mmol/L;(6.28±1.44),(9.49±4.0) mmol/L;and(6.49±1.44),(9.88±3.96) mmol/L,respectively.The finger tip blood glucose values were only slightly higher than the venous plasma glucose values(all P>0.05).The coefficients of the 3 levels detected by 3 different meters with the venous plasma glucose values were 0.959,0.974,and 0.962,respectively.Conclusion Finger tip glucose detection can be used as a satisfactory method for diabetes patients for self-monitoring and management of their blood glucose.(Shanghai Med J,2010,33: 570-571)

Abstract:The prevalence of gestational diabetes mellitus (GDM) is increasing annually, which poses substantial harm to the health of both mothers and children. Therefore, selection of clinically applicable and easily detectable indicators in the assessment of maternal insulin secretory function and insulin sensitivity in pregnant women undoubtedly holds great importance in evaluating the risk of GDM, guiding the choice of GDM therapy modalities, and improving the ability to provide early warning of adverse pregnancy outcomes. Compared with the classic clamp technique, many simple indices are more suited for use among pregnant women due to the low frequency of blood sampling and simple administration involved. While indices derived from fasting blood glucose and fasting insulin levels are most readily available, they are unable to provide information on the ability of insulin to manage the glucose load during pregnancy. Although the indices derived from the insulin and glucose values at each time point of the oral glucose tolerance test can provide a more comprehensive picture of the insulin sensitivity and insulin secretory function of the body, their application is constrained by the complexity of the procedure and associated high costs. Concomitantly, the findings from different studies are influenced by a variety of confounding factors, such as the gestational age during testing, race, and detection method. Furthermore, insulin secretory function and insulin sensitivity in pregnant women differ from those in non-pregnant women in that they change significantly with prolonged pregnancy; hence, there is an urgent need to develop a pregnancy-specific reference range. This article reviews the progress in the application of simple indices to help clinicians better understand their potential application in detecting GDM.
