METHODS 1368 patients from March 2007 to July 2013 had undergone 16-F tract PCNL in our hospital. Surgery were performed under general anesthesia in all cases. Central veinpucture suit was choosed as pucture device. Complications, residual stone, stone clearance and the need for auxiliary treatments were evaluated. Management were concluded for the rare comlications. RESULTS Complications occurred in 183/1368 (13.4%) patients. There were 102 grade 1 (7.4%),121 grade 2 (8.8%), and 32 grade 3 (2.1 %) complications, but there were no grade 4 or 5 complications. 99.7% success rates in establishing access to the kidney and 82.0% complete stone clearance for not required re-PCNL cases. Auxiliary treatments, including shockwave lithotripsy in 135 patients, re-PCNL in 49 patients and ureteroscopy in 63 patients. Four cases of rare complications occurred including 1 JJ stent translocated to chest,2 JJ stent translocated to a 16-F nephrostomy tube, 1 alimentary tract hemorrhage after operation. Severe complications during operation or postoperation should be managed immediately. CONCLUSIONS A 16-F tract PCNL guided under ultrasound is safe and convenient, especially punctured by central veinpucture suit Table 1.
