Repeated Endorectal Ultrasonographies Predict the Tumor Response of Preoperative Chemoradiation Therapy in Rectal Cancer

Abstract:To assess the value of repeated endorectal ultrasonographies in predicting the effects of preoperative chemoradiation (CRT) for locally advanced rectal cancer. Patients with histopathologically proven, clinical stage II/III rectal adenocarcinoma were enrolled prospectively. Endoscopic ultrasound measurements of primary tumor maximum diameter were performed before (EUS1), during (EUS2) and post (EUS3) CRT in all patients. The ratios for EUS2/EUS1 and EUS3/EUS1 were calculated. In addition, correlations between relative values in primary tumor maximum diameter and tumor regression grade, the down-staging rate and the pathological complete response (pCR) rate, were assessed. From October 2010 to October 2012, 44 patients were enrolled. EUS1, EUS2 and EUS3 measurements of 37 patients with median age of 54 years old were completed. There were 20 males and 17 females patients enrolled. pCR and down-staging rate was 21.6% and 48.6%, respectively. The median primary tumor maximum thickness of EUS1, EUS2 and EUS3 were 13.1 mm (5.7-41.2 mm), 10.6 mm (5.5-25.4 mm) and 8.8 mm (4.3-12.6 mm), respectively. There was no significant correlation between value of EUS2/EUS1 and tumor regression grade (p = 0.074). However, the value of EUS3/EUS1 correlated with the pCR rate and tumor regression grade (p = 0.000, p = 0.046). In addition, the value of EUS3 correlated with the pCR rate (p = 0.010). ROC curve analysis indicated that the pCR diagnostic cutoff point for EUS3/EUS1 was 52.2% (area under the curve (AUC) = 0.710), and EUS3 was 8.1 mm (AUC = 0.891). Repeated endorectal ultrasonographies before, during and after concurrent chemo-radiation therapy may predict therapeutic effect of preoperative chemoradiation therapy for rectal cancer.
