Abstract:Objective To study the effect and methods of orbital septum fat redistribution or autogenous orbital septum granular fat transplantation methods on upper eyelid depression.Methods Blepharoplasty incision was used to cut open musculus orbicularis oculi and septum orbitale horizontally,exposuring orbital fat,the conjunction was relieved between orbit fat envelope and septum orbitale,displaced orbital fat was extended and replaced to the depression region of upper eyelid,and the wound was routinely sutured.For orbital septum fat volume depletion,appropriate amount of granular fat was injected into depression region and put fat backward upto supraorbital margin.Then,resetting orbital septum and closing incision were conducted with suture technique of blepharoplasty.Results Of 48 cases (96 eyes),32 cases was followed-up for 6 to 42 months with mean of 11 months.All cases got ideal outcomes,with smooth and symmetry lid folds,depression of upper eyelid were fixed and no complication occurred.Conclusions Orbital septum fat redistribution or autogenous orbital septum granular fat transplantation can anatomically reduce the fat distribution of upper eyelid.This method is easy and effective to improve the appearance of eyelid.
Sunken Upper Eyelid Deformity Correction by Orbital Fat Pad Repositioning and Orbicularis Oculi Muscle Folding in Blepharoplasty

Abstract:BACKGROUND: Sunken upper eyelid deformity results in an aged and unnatural appearance and is common among Asians along with the single eyelid. The upper eyelid volume restoration is as important as the creation of a supratarsal fold for patients with sunken eyelids. In the present study, the combination of orbital fat pad repositioning and orbicularis oculi muscle flap folding was established to correct the upper eyelid depression during blepharoplasty.METHODS: A retrospective study was conducted on patients who underwent the orbital fat pad repositioning and orbicularis oculi muscle flap folding to correct the upper eyelid depression between July 2020 and October 2021. The preoperative and postoperative sunken depths were calculated and compared, and overall patient satisfaction was performed to evaluate the operation efficiency.RESULTS: There were 40 patients, including 39 females and 1 male, who underwent surgery with an average age of 25.4 years. During the mean follow-up period of 12.7 months, the sunken depth improved from 3.2 mm preoperatively to 1.5 mm postoperatively with a statistical significance. Satisfactory results were achieved without major complication.CONCLUSIONS: For patients with mild to moderate sunken upper eyelids, the combination of orbital fat pad repositioning and orbicularis oculi muscle folding has been proved efficient and consistent to restore the upper eyelid volume and helpful to build esthetic double eyelids.

Abstract:In senile patients with sunken superior sulcus, involutional ptosis, and higher eyelid crease, a single operation to correct depression or ptosis cannot achieve good results. We demonstrated the anatomy of periorbital septum fibers, which may contribute to the levator muscle's volume depletion and dynamic power transmission disorder, and described a procedure for correcting upper-eyelid depression and blepharoptosis in senile patients. The fibrous webs in these patients connected the posterior aspect of the orbicularis and the orbital septum and extended to the orbital fat and levator aponeurosis. These fibers were dissected to release the periorbital septal fibers, and the orbital septal fat flap was transferred to the depressed region. Advancement or plication of the levator aponeurosis was performed in patients with uncorrected blepharoptosis after the procedures described above. The technique was applied to 13 Chinese patients (25 eyes) between May 2021 and April 2022. Postoperative magnetic resonance imaging revealed that the preaponeurotic fat was displaced forward and down to the upper margin of the tarsus, and the curvature of the upper-eyelid depression was significantly improved. Moreover, the superior sulcus deformity improved, the ptosis was corrected, and the uppermost crease decreased in all patients. No recurrence of ptosis or abnormal adhesion was observed. We believe this is the first study using magnetic resonance imaging to evaluate eyelid anatomy and the effects of surgery in this patient group. Releasing periorbital septum fibers is crucial for correcting a portion of the sunken eyelid and ptosis in Asians.

Abstract:Background: Women commonly experience upper eyelid dermatochalasis and upper eyelid depression with advancing age. Blepharoplasty is a suitable method for treating dermatochalasis, but not sunken eyelid. This study proposed a novel technique for eyelid rejuvenation by simultaneously correcting dermatochalasis and sunken upper eyelids in middle-aged women. Methods: Forty patients underwent subbrow blepharoplasty combined with brow fat pad transfer. Ellipse-shaped skin and subcutaneous tissue underneath the eyebrow were measured, demarcated, and excised. The orbicularis oculi muscle beneath the subcutaneous tissue was exposed and dissected in the upper-third area. The brow fat pad was turned downward using the lower edge as the pedicle and was fixed in the layer of retro-orbicularis oculi fat to fill the depressed area in the upper eyelid. The lower muscle flap was fixed to the supraorbital rim periosteum and upper musculocutaneous flaps, thereby forming a cross flap for interlocking fixation. The surgical outcomes were evaluated using a three-dimensional imaging device and the Global Aesthetic Improvement Scale. Results: The depth and volume of upper eyelid depression decreased significantly 3 months after the surgery and stabilized within 6 months. Global Aesthetic Improvement Scale scores significantly improved after the surgery, and the postoperative outcomes were acceptable. Conclusions: The novel technique is simple and effective for simultaneously correcting dermatochalasis and sunken upper eyelids in middle-aged women. The surgical outcomes are predictable and acceptable to most patients. Clinical question/level of evidence: Therapeutic, IV.

Abstract:Objective To explore the orbital fat transfer technology in the lower blepharoplasty.Methods Using the orbital septum fat transfer technology in the lower blepharoplasty to fill the canthus,infra-orbital margin,eye-nose groove.Results The 42 cases of lower blepharoplasty all gained satisfying effect.The wrinkles of canthus,the depression of infra-orbital,eye-nose groove obtained improvement and no complications occurred.Conclusion The orbital septum fat transfer technology in the lower blepharoplasty can significantly improve the shape of eyes,and might be widly utilize.

Abstract:Objective To explore the clinical effect of auto-transplantation of granulated orbital septum fat for correction of tear trough deformity. Methods From October 2015 to October 2017, 60 patients with pouch, tear trough deformity and lower eyelid skin relaxation were selected in this study. The orbital septum fat was released and removed by lower eyelid myocutaneous flap, and then was transplanted into tear trough after being cut into tiny particles. The excess skin and musculi dormitator were removed after transplantation. Results All patients were followed up for 6 months. Except for one patient who had been filled twice due to the deficiency of orbital septum fat, all eyelid pockets disappeared, the tear tough and the surrounding wrinkles were improved in appearance, and the lower eyelid area was flat and tight, without bump and no touch of nodules. Conclusion Transplantation of granulated orbital septum fat has satisfactory effect for correction of tear trough deformity and lower eyelid skin relaxation in pouch patients, the survival rate of fat is high. The operation is worth popularizing.

Abstract:Background“Sunken eyelid” is a term used to describe a deeply sunken area between the upper eyelid and upper edge of the orbital bone. The condition frequently is accompanied by blepharoptosis. Correction of a “sunken eyelid” generally is performed with a fat graft. However, surgical limitations such as a movement dysfunction of the upper eyelid or irregular skin surface often are encountered using the grafting method previously reported. An open technique is believed to be more precise than a closed procedure, and satisfactory results may be achieved by repositioning the upper eyelid fat and applying a fat graft to the retro-orbicularis orbital fat (ROOF).MethodsFat was harvested from the medial thigh area using a 10-ml syringe with minimal pressure. During the upper eyelid surgery, the orbital fat was relocated between the conjoined tendon of the levator aponeurosis and the lower orbicularis oculi muscle and skin flap. The ROOF was verified, and fat grafting then was performed from the upper medial side to the lower lateral side and from the medial aspect to the lateral direction using an 18-gauge Coleman needle. The sunken eyelids were classified into grades 1 to 4 according to the sunken depth (i.e., the distance between the most inferior orbital rim and the site of the most sunken area). The amount of fat graft and the technique of incorporating the fat were different according to sunken depth.ResultsFrom January 2006 to June 2008, 50 Korean and Chinese patients (48 women and 2 men) underwent fat repositioning and ROOF fat grafting using an open technique. The patients ranged in age from 24 to 67 years. The mean observation period was 4.7 months. The amount of fat graft for each palpebra was 0.3 to 3.3 ml, and the mean graft amount was 1.4 ml. Satisfactory results were obtained for most of the patients, but four patients required additional surgery. The reasons for the secondary surgery were a translocation of the fat graft to a lower position forming a lump (2 patients) and undercorrection (2 patients). The rate of fat absorption was relatively low and the complications very few compared with surgical cases involving a noninvasive method.ConclusionsMore precise treatment is possible by dividing the depressed area of the upper eyelid according to the stage. Satisfactory results were achieved by relocating the orbital fat using an open method and adjusting the ROOF fat graft according to the stage.

Abstract:Background Nowadays, people's pace of life continues to rapid up, and many bad habits will accelerate the aging of the eye periphery, and patients with sunken upper eyelids are to be found in younger people. In young Asians, single eyelids are often accompanied by upper eyelid depression, so correcting the upper eyelid depression during blepharoplasty becomes a higher challenge for plastic surgeons. Current surgical methods for upper eyelid depression include three major categories: tissue repositioning, injection and filling, and combined use. According to grades 1 and 2 are mild or moderate upper eyelid sunken. The sunken can be well corrected only by repositioning the orbital fat pad, while the correction effect for severe upper eyelid sunken in grades 3 and 4 is Poor, need to be used in combination to achieve the desired effect. Purpose The authors sought to determine whether, for patients with single eyelids and severe upper eyelid depression of grades 3 and 4, combined with orbital fat pad repositioning and autologous fat transplantation during blepharoplasty, an aesthetic and youthful blepharoplasty can be achieved. Methods This study included 79 patients with single eyelids and severe upper eyelid depression of grades 3 and 4 who received treatment between June 2020 and July 2022. All patients underwent double eyelid surgery plus orbital fat repositioning and autologous fat grafting. Results After a minimum follow-up period of 1 year, overall patient satisfaction was 92%. The recurrence rate within the first year was 6% and the complication rate was 5%. Conclusion This combined surgery may be an option for young Asians with single eyelids and severe upper eyelid depression. In this study, the surgery resulted in natural-looking double eyelids and younger-looking eye sockets in most patients. A combination of different surgical methods based on the patient's preoperative condition is critical to achieving long-term correction.

Transconjunctival Fat Reposition With Septal Reset For The Treatment Of Tear Trough

Abstract:To offer an alternative operation as a method of fat repositioning via a transconjunctival approach. This is a safe and effective alternative surgical approach to Hamara's transcutaneous approach when obvious external scars are not acceptable. This approach should be limited to patients who do not have any obvious excessive lower eyelid skin or muscle laxity. The method of fat reposition with septal reset in transconjunctival lower blepharoplasty was used to correct tear trough deformity. Lower eyelid fat repositioning with septal reset during transconjunctival lower blepharoplasty was used to correct tear trough deformities on young Asian patients who did not want external scars. In the 34 patients of Barton's grade I, with 30 cases (88.2%) improving to grade 0, and 4 cases (11.8%) having no improvement. In the 38 patients of Barton's grade II, with 29 cases (76.3%) improving to grade 0, 9 cases (23.7%) improving to grade I. In the 12 patients of Barton's grade III, with 3 patients (25%) improving to grade 0, 8 cases (66.6%) improving to grade I, 1 cases (8.4%) improving to grade II. The case study demonstrated that the method of fat reposition with septal reset in transconjunctival lower blepharoplasty is optimal. However, it is important to notice that the indication is narrow.
Surgical Correction of Depressed Scars After Upper Blepharoplasty: A Turnover Orbicularis-Septum Composite Flap Technique

Abstract:Objective: Depressed scarring is a common complication after incisional upper blepharoplasty and frequently contributes to patient dissatisfaction. Correcting this deformity presents a significant challenge for oculoplastic surgeons. This study aims to investigate the clinical effectiveness of employing the turnover orbicularis-septum composite flap technique in correcting depressed scars after double eyelid surgery. Methods: This is a retrospective study of 118 patients who underwent revision blepharoplasty with depressed scar from November 2020 to February 2023. During the revision procedure, the adhesions of the original scar were meticulously dissected, and the residual orbital fat was thoroughly released. The orbicularis-septum composite flap was then inverted downward and smoothly laid over the depressed scar area to address the tissue deficit. After surgery, patient satisfaction was evaluated by assessing the improvement of the depressed scars and the shape of the double eyelid folds. Results: Follow-up assessments were conducted over a period of 6 to 24 months postoperatively. The results were judged as fully satisfied in 78 cases (66.1%), basically satisfied in 32 cases (27.1%), and unsatisfied in 8 cases (6.8%). Among the unsatisfied patients, 5 patients complained of eyelid fold shallow or disappear, and 3 patients complained of asymmetry. All patients exhibited varying degrees of improvement in the depressed scars. Conclusions: The turnover orbicularis-septum composite flap technique provides an effective approach for the treatment of depressed scars with a high satisfaction rate.

[One Stage Correction of Sunken Eyes Combined with Ptosis].

Abstract:OBJECTIVE:To investigate the technique and therapeutic effect for correction of sunken eyes combined with ptosis.METHODS:In order to adjust the levator muscle tension and the relationship between levator aponeurosis and tarsus plate, multiple individualized treatment was selected, including levator aponeurosis restoration, levator aponeurosis plication, or shorten, or combination. Then the orbital fat was transferred to the depressed area, or autologous fat particles were collected and injected into the depressed area within the orbital fat fascia. After the orbital septum fascia was restored, the incision was closed primarily.RESULTS:15 cases (30 eyes) were treated. 11 cases were followed up for 6-40 months (average, 9.5 months) with satisfactory cosmetic and functional result. No recurrence of ptosis happened.CONCLUSIONS:One-stage correction of sunken eyes combined with ptosis can be achieved with autologous fat injection or orbital fat transposition. Good cosmetic and functional result can be achieved.
Orbital septorhaphy for the correction of baggy upper and lower eyelids

Abstract:Eyelid bags are the result of relaxation of lid structures like the skin, the orbicularis muscle, and mainly the septum, with subsequent protrusion or pseudo herniation of intraorbital fat contents. The logical treatment of baggy upper and lower eyelids should therefore include repositioning the herniated fat into the orbit and strengthening the attenuated septum in the form of a septorhaphy as a hernia repair. The preservation of orbital fat results in a more youthful appearance. The operative technique of the orbital septorhaphy is demonstrated for the upper and lower eyelid. A prospective series of 60 patients (50 upper and 90 lower blepharoplasties) with a maximum follow-up of 17 months were analyzed. Pleasing results were achieved in 56 patients. A partial recurrence was noted in 3 patients and widening of the palpebral fissure in 1 patient. Orbital septorhaphy for baggy eyelids is a rational, reliable procedure to correct the herniation of orbital fat in the upper and lower eyelids. Tightening of the orbicularis muscle and skin may be added as usual. The procedure is technically simple and without trauma to the orbital contents. The morbidity is minimal, the rate of complications is low, and the results are pleasing and reliable.

Abstract:UNLABELLED:OBJECTIVE To investigate an operative method of combined resection of preseptal fat: and partial retro-orbicularis oculus fat (ROOF) for correction of upper eyelid heaviness, and evaluate the efficacy and safety of the method. METHODS:Preseptal fat lies widely under the orbicularis oculi in the upper eyelid, and retro-orbicularis oculus fat (ROOF) lies in the lateral supraorbital area. Combined resection of preseptal fat and partial ROOF was performed in patients selected by examination. The efficacy and safety were evaluated by follow-up study. RESULTS:From May 2011 to July 2013, 38 selected patients received the treatment with 3 months to 28 months follow up. The heaviness of upper eyelid improved in all cases. One patient developed postoperative hematoma, and another patient had a transient numbness over the lateral upper brow region. 37 patients were satisfied with the result. CONCLUSIONS:Combined resection of preseptal fat and partial ROOF was effective in reducing the heaviness of upper eyelid, without major complications. The operative method should be an important adjunct for selected patients undergoing blepharoplasty.
Application of orbital fat flap combined with free fat in correction of moderately or severe sunken upper eyelid and multiple folds

Abstract:Objective:To evaluate the use of orbital fat flap combined with free fat for the treatment of the moderately or severe sunken upper eyelid and multiple folded upper eyelids.Methods:From March 2017 to March 2019, orbital fat release and replacement and free abdominal fat transfer graft were adopted to treat 20 patients (1 male, 19 females, aged 20-47 years, with an average age of 29 years) with sunken upper eyelids of multiple folds in Department of Plastic and Reconstructive Surgery, the First Affiliated Hospital of Xiamen University Siming Branch.Results:Twenty cases were treated successfully. The swelling gradually disappeared at 3 month postoperatively. Patients were followed up for 3 months to 12 months with sustained aesthetic results; no complications occurred. Free fat graft survived well, and no obvious scar, sags, or crests were present in the upper eyelid.Conclusions:It is a reasonable method for correcting sunken upper eyelid and multiple folds with orbital fat flap and free fat. Survival rate of the free fat is quite high in sunken upper eyelid. It is worth spreading.

Abstract:Background: Tear trough deformity is one of the most common complaints in clinical settings. The correction of this groove is challenging in facial rejuvenation. The lower eyelid blepharoplasty varies with different conditions. A novel approach of using orbital fat in the lower eyelid to increase the volume of the infraorbital rim with granule fat injection has been applied in our institution for more than 5 years. Objectives: This article aims to describe the detailed steps of our technique and verify its effectiveness by a cadaveric head dissection after surgical simulation. Methods: In this study, a total of 172 patients with tear trough deformity underwent lower eyelid orbital rim augmentation with fat filling in the sub-periosteum pocket. According to Barton's grades, 152 patients underwent lower eyelid orbital rim augmentation with orbital fat filling, 12 patients had it combined with autologous granule fat from other body parts, and 8 patients received only transconjunctival fat removal to correct tear trough. Results: The modified Goldberg score system was used to compare preoperative and postoperative photographs. Patients were satisfied with the cosmetic results. Excessive protruding fat was released, and the tear trough groove was flattened by using autologous orbital fat transplantation. The lower eyelid sulcus deformities were well-corrected. To further illustrate the anatomical structure of the lower eyelid area and injection layers, six cadaveric heads were used for surgical simulation and demonstrated the effectiveness of our technique. Conclusions: This study indicated that the infraorbital rim could be increased by transplanting orbital fat to the pocket, which was dissected under the periosteum, and the procedure has been verified as reliable and effective. Abstract:The evolution of lower blepharoplasty has shifted from simply removing of orbital septum fat to smoothing of the lid-cheek junction through fat repositioning. This paper adopts a novel technique to transpose and stabilize intraorbital fat during transconjunctival lower blepharoplasty. The tear trough and nasal alar base were filled to correct the pouch while ensuring the blood supply of the fat flap.

Abstract:Objective To explore the method of the repairing of zygomatic soft tissue depression deformities with tissue flap combined with autologous fat transplantation through lower eyelid edgeincision route. Methods We can take two methods to reparie it according to the parts of depression: ①the projection of depression above zygomatic fat pad under the skin.Through lower eyelid edge incision route,release the orbicularis oculi muscle with scar and contracture.Mattress suture both ends,before releasing the orbital fat,fix and fill the orbital fat between orbicularis oculi muscle and orbital. ②the projection of depression in zygomatic fat pad under the skin.Through lower eyelid edge incision route,strip till the depression under the skin.Fully release the deep scar adhesion tissue and then Draw the surrounding fat tissue and fascia forming tissue flap,mattress suturing them. Results All 11 cases got a better aesthetic result.The patients were followed up for 6 months to 2 years with satisfactory results with good appearance and function.The depression deformity of zygomatic soft tissue has disappeared. Conclusion It is one of good methods to repair zygomatic small soft tissue depression deformity with tissue flap combined with autologous fat transplantation through lower eyelid edge incision route.
