Application of 18F-Fdg Pet/Ct in Diagnosis and Management of Pericardial Effusion

Abstract:298 Objectives Pericardial effusion is a common finding in clinical practice either as incidental finding or manifestation of a systemic or cardiac disease. The purpose of the study was to investigate the value of 18F-FDG PET/CT in diagnosis and clinical management of pericardial effusion. Methods Between August 2010 and November 2013, 34 patients with pericardial effusion who underwent whole-body 18F-FDG PET/CT were retrospectively reviewed. The maximum standardized uptake value (SUVmax), size (mild, moderate and large) and distribution (circumferential, loculated) of pericardial effusion were recorded. The impact of 18F-FDG PET/CT on clinical management was also assessed. Histopathologic examination served as the reference gold standard. Results Of these 34 patients, the causes of pericardial effusions were tuberculosis infections (n=3), tumors (n=19, which including 13 lymphoma, 2 lung cancer, 2 breast cancer, 1 renal cell cancer and 1 peritoneal cancer), connective tissue diseases (n=5), myocardial infarction (n=2), chronic renal diseases (n=2), idiopathic pericardial effusion (n=3). 18F-FDG PET/CT was false-negative in 2 patients with breast cancer and peritoneal cancer. Diagnosis accuracy of 18F-FDG PET/CT was 94.1 % (32/34). The treatment of 5 patients was altered according to PET/CT results. The mean SUVmax of tumor group was 1.75 (range, 1.0 to 9.2) and was significantly higher than that(mean SUVmax 1.1, range, 0.7 to 2.2 ) of non-tumor group (p 0.05). Conclusions 18F-FDG PET/CT is very useful in diagnosis and clinical management of pericardial effusion. SUVmax of pericardial effusion maybe helpful in differentiating malignant from benign lesions.
