Abstract:Objective To assess the efficacy and adverse effects of different doses of ACTH on infantile spasms(IS).Methods Systematic review and meta-analysis of RCTs were performed to compare the effects of different doses of ACTH on IS.The standard literature search strategy included electronic literature search and manual literature search.Electronic search was carried out in databases including PubMed,EMBASE,Ovid,ScienceDirect,China National Knowledge Infrastructure(CNKI),Wanfang Chinese periodical database,CiNii(National Institute of Informatics Scholarly and Academic Information Navigator) and Medical Online(Japanese medical literature database).As a supplement,references in previous reviews and studies identified as relevant were examined by manual search.The methodological quality of included RCTs was assessed according to randomization method,allocation concealment,blinding(participants,researchers and outcome assessors),loss of follow up,selective data reporting and other bias.Review Manager 5.0 was used for statistical analysis.Effects were expressed as Peto odds ratio and 95%confidence interval(CI) for dichotomous data.P0.05 was considered as statistically significant.Data inappropriate for combining analysis were discussed descriptively.Results A total of 4 RCTs fulfilled the inclusion criteria.The methodological quality evaluation revealed 1 RCT with moderate bias risk and 3 RCTs with high bias risk.Meta-analysis showed there was no significant difference among different doses of ACTH on cessation of spasms(OR=1.09,95%CI:0.54-2.17,P=0.82),on resolution of hypsarrhythmia(OR=1.04,95%CI: 0.43-2.51,P=0.94) and on the relapse rate(OR=1.27,95%CI: 0.64-2.54,P=0.50).Descriptive analysis showed adverse effects including hypertension and gaining weight occurred more often in high-dose group than those in low-dose group.Subgroup analysis showed similar efficacy on clinical cessation of spasms(symptomatic patients: OR=0.96,95%CI:0.43-2.17,P=0.92;cryptogenic patients: OR=2.76,95%CI:0.36-21.12,P=0.33) and EEG resolution of hypsarrhythmia(symptomatic patients: OR=0.66,95%CI: 0.20-2.23,P=0.50;cryptogenic patients: OR=2.23,95%CI:0.52-9.57,P=0.28) both in symptomatic and cryptogenic IS patients.In symptomatic IS patients,the relapse rate of different doses of ACTH also had no significant difference(OR=1.00,95%CI: 0.33-3.02,P=1.00).Conclusions In a certain range,different doses of ACTH may have similar effects on cessation of spasms,resolution of hypsarrhythmia and relapse rate in treating IS.Low-dose ACTH may have less adverse effects than high-dose.The included RCTs have relatively small sample size and low quality,so further evidence from more RCTs are needed to reach a conclusion.

Abstract:Objective To summarize clinical characteristics of infantile spasms(IS),to compare the short-term efficacy and side effects of high-dose and low-dose ACTH therapy,and to investigate influencing factors on the resolution of spasms.Methods Clinical data of IS patients who were treated with ACTH for the first time were investigated retrospectively.These patients were admitted to Peking University First Hospital from January 2005 to December 2010.These patients were divided into high-dose and low-dose groups according to initial ACTH doses.Clinical efficacy on spasms,resolution of hypsarrythmia and side effects were compared between these two groups.Logistic regression was used to explore risk factors for clinical control of spasms.Results A total of 198 patients were included into the study.Their clinical data,EEG characteristics and brain imaging were fully investigated.Complete cessation of spasms was achieved in 37.3%(25/67) of the high-dose group and 45.0%(59/131) of the low-dose group.No significant difference was found between these two groups.EEG was reevaluated in 120 patients after ACTH therapy.In the high-dose group hypsarrythmia disappeared in 42.2%(19/45) of the patients,and 58.7%(44/75) in the low-dose group,no significant difference on the resolution of hypsarrythmia was seen between the two groups.In the high-dose group,52.2%(35/67) of patients experienced side effects of ACTH while it was 32.8%(43/131) in the low-dose group,the difference being significant between the two groups.Treatment lag(TL),defined as the time from onset of spasms to the initiation of ACTH treatment,was an independent risk factor for the cessation of spasms.About 59.0%(36/61) of patients with TL no more than 1 month achieved cessation of spasms;as for patients with TL longer than 1 month,the cessation of spasms occurred in 34.8%(48/137).The difference was significant between the two groups.Conclusions The study shows similar efficacy between different doses of ACTH on cessation of spasms,and on resolution of hypsarrhythmia.High-dose ACTH has moreside effects compared with low-dose one.Treatment lag is an independent risk factor for the cessation of spasms.

Application of peripheral blood cytokine and immunoglobulin detection in ACTH therapy for the treatment of infantile spasms

Abstract:Objective: This research aims to investigate the levels of lymphocytes, immunoglobulins, and cytokines in children with infantile spasms (IS) before and after adrenocorticotropic hormone (ACTH) therapy and to explore the application of these markers in evaluating the therapeutic effects of ACTH on infantile spasms. Methods: From May to November 2022, 35 children initially diagnosed with IS and treated at our hospital were regarded as the observation group, and 35 healthy children who underwent physical examination at our hospital during the same period were regarded as the control group. Children in the observation group received intramuscular injections of ACTH for 2 weeks. Fasting venous blood was collected from the control group and the observation group before and after ACTH therapy. Serum levels of immunoglobulins IgG, IgA, and IgM in serum were detected by immunoturbidimetry. T-cell subsets (CD3+, CD3+CD4+, and CD3+CD8+) and B-cell subsets [CD3-CD19+ and CD3-CD16+CD56+ natural killer (NK) cells] were detected by flow cytometry, and the ratio of CD3+CD4+/CD3+CD8+ was calculated. Serum levels of interleukin-1β (IL-1β), interleukin-2R (IL-2R), and interleukin-6 (IL-6) cytokines were detected by the enzyme-linked immunosorbent assay, and changes in serum cytokine and immunoglobulin levels in the two groups were compared before therapy, whereas in observation group one, these comparisons were made both before and after ACTH therapy. Results: Compared to the control group, the observation group showed significantly increased serum levels of immunoglobulins IgG and IgM before therapy, while the level of IgA was significantly decreased (p < 0.05). Also, the percentage of CD3-CD19+ B cells was significantly increased, while the percentages of CD3+ T cells and CD3+CD4+ T cells were significantly decreased (p < 0.05). The percentages of CD3+CD8+ T cells, CD3-CD16+CD56+ NK cells, and CD3+CD4+/CD3+CD8+ cells did not change significantly (p > 0.05); the levels of cytokines IL-1 β, IL-2R, and IL-6 were significantly increased (p < 0.05). Compared to levels before treatment, the serum level of immunoglobulin IgG in the observation group after ACTH therapy was significantly reduced (p < 0.05), while the IgA and IgM levels did not change significantly (p > 0.05). The percentages of CD3+ T cells and CD3+CD4+ T cells were significantly increased, while the percentages of CD3-CD16+CD56+ NK cells and CD3-CD19+ B cells were significantly decreased (p < 0.05); however, the percentages of CD3+CD8+ T cells and the CD3+CD4+/CD3+CD8+ ratio did not change significantly (p > 0.05). Furthermore, the levels of cytokines IL-1 β, IL-2R, and IL-6 were significantly reduced (p < 0.05). Conclusion: Children with IS exhibit immune dysfunction, and the changes in serological immune indices after ACTH treatment indicate that ACTH may control seizures in IS children by regulating and improving immune dysfunction. Therefore, the therapeutic effects of ACTH on IS can be evaluated by detecting the levels of cytokines and immunoglobulins.
[Effect of Prednisolone and Adrenocorticotropic Hormone in the Treatment of Infantile Spasms: a Meta Analysis].

Abstract:OBJECTIVE:To compare the effect and safety of prednisolone and adrenocorticotropic hormone (ACTH) in the treatment of infantile spasms (IS).METHODS:Cochrane Library, Embase, PubMed, China Biology Medicine Disc, CNKI, and Wanfang Data were searched for clinical studies on the comparison between prednisolone and ACTH in the treatment of IS. Literature screening, data extraction, and quality assessment were performed. Review Manager 5.3 was used for Meta analysis.RESULTS:Five clinical studies were included according to the inclusion criteria and exclusion criteria. Meta analysis showed that there was no significant difference in the spasm remission rate, spasm remission time, complicating infection rate, and irritability rate between the prednisolone and ACTH treatment groups (P>0.05), but the disappearance rate of hypsarrhythmia in the electroencephalogram was higher in the ACTH treatment group than in the prednisolone treatment group (P<0.05).CONCLUSIONS:The available evidence shows no difference in the clinical efficacy of prednisolone versus ACTH in the treatment of IS. However, ACTH is superior to prednisolone in stabilizing EEG. The two therapies have no difference in the incidence of adverse reactions such as infection and irritability.
Hormonal Therapy for Infantile Spasms: A Systematic Review and Meta-Analysis

Abstract:The limitations of adrenocorticotrophic hormone (ACTH) treatment for infantile spasms (ISs), such as high costs, limited availability, and adverse effects (AEs), make it necessary to explore whether corticosteroids are optimal alternatives. Many other compelling treatments have gone through trials due to the suboptimal effectiveness of hormonal therapy. A systematic review and meta-analysis were performed to evaluate the effectiveness and safety of hormonal therapy for patients with ISs.EMBASE, Ovid MEDLINE, Cochrane Central Register of Controlled Trials (CENTRAL), and online registers were searched through April 2021 for randomized controlled trials (RCTs).A total of 19 RCTs (N = 1,279) were included. There was no significant difference in the effectiveness of oral corticosteroids and ACTH in electro-clinical response (risk ratio [RR] = 0.85, 95% CI 0.41-1.76). Low-dose ACTH had similar effectiveness in electro-clinical response compared to usual-dose group (RR = 0.94, 95% CI 0.60-1.47) but conferred a lower risk of AEs (RR = 1.71, 95% CI 1.08-2.71). ACTH was more beneficial in controlling spasms than vigabatrin (VGB) (RR = 1.31, 95% CI 1.05-1.64) for patients without tuberous sclerosis complex (TSC). All RCTs were connected through network meta-analysis, and we found that ketogenic diet (KD), zonisamide, methylprednisolone, or combined treatment of hormonal therapy with topiramate (TPM) or pyridoxine was not different in electro-clinical response compared to usual-dose ACTH.Our analysis showed that oral corticosteroids could be optional alternatives when ACTH is not applicable, and ACTH is more beneficial for patients without TSC. Moreover, low-dose ACTH is recommended due to comparative effectiveness but lower risk of AEs. However, due to the high heterogeneity of included patients and treatment protocols, these results must be interpreted with caution. RCTs with multicentric involvement and larger sample size are needed for solid evaluation of other alternative treatments.
Efficacy of High-Dose ACTH Versus Low-Dose ACTH in Infantile Spasms: A Meta-Analysis with Direct and Indirect Comparison of Randomized Trials

Abstract:To determine the influence of dose on the efficacy and safety of adrenocorticotropic hormone (ACTH) for infantile spasms. We searched six relative databases. Randomized controlled trials and crossover studies, which meet our inclusion criteria were considered for inclusion. The quality of included trials was evaluated with respect to sequence generation, allocation concealment, blinding and incomplete data. We performed both direct and indirect meta-analysis to evaluate the efficacy and safety of high-dose and low-dose of ACTH. Results: A total of 168 reports were retrieved, and after scrutiny seven reports of trials containing three head to head comparisons, two vigabatrin comparisons and two prednisone comparisons were included. In direct comparison meta-analysis, studies were homogenous (P >; 0.10). There was a trend towards more complete cessation of spasms in the low-dose ACTH treated group (RR: 0.86, 95% confidence interval [CI]: 0.57–1.31), though statistically not significant. Relapse was more frequent in low-dose ACTH groups (RR: 0.69, 95% CI: 0.22–2.19) but without demonstrating statistic difference. No difference was present for disappearance of hypsarrhythmia between high and low-dose ACTH (RR: 0.93, 95% CI: 0.57–1.54). In indirect meta-analysis the comparison of complete cessation of spasms between high-dose and low-dose ACTH from indirect meta failed to show any significant difference. There was no significant difference between two dosages with respect to relapse rates. High-dose ACTH produced more adverse effects than low-dose group. A low-dose ACTH therapy might be preferable for its comparative efficacy and lower risk of adverse effects.
The Efficacy of Prednisone in the Treatment of Infantile Spasm and HPA Axis Mechanism Analysis

Abstract:Objective By studying the changes of the seizures of infantile spasm(IS)、EEG and HPA axis function before and after the treatment of prednisone,to explore the efficacy of prednisone in treating infan-tile spasm,the role of HPA axis in the pathogenesis of IS,and elucidate the HPA axis mechanism of prednisone in controling seizure.Methods A total of 30 patients with IS (IS group) and 30 cases of healthy infants and young children (control group) were recruited.Number of seizures、EEG、HPA axis function was detected be-fore and after the treatment of prednisone in patients with infantile spasm.Serum cortisol,ACTH were deter-mined by the chemiluminescence analysis,serum CRH was measured by enzyme-linked immunosorbent assay. Results serum CRH levels of IS group was significantly higher than normal control group(P<0.05).Serum cortisol,ACTH in IS group were no evidently different compared with control group (P>0.05).The average number of daily ictal clusters and the average daily total seizure number positively correlated with CRH respec-tively.After the application of the prednisolone,seizure of 19 cases of the IS were controlled,11 cases were not controlled,18 cases of hyperarrhythmia were completely remited and 12 cases of hyperarrhythmia were not com-pletely remited.The average number of daily ictal clusters and The average daily total seizure number after treat-ment were significantly lower than before treatment(P<0.05);DQ after treatment was higher than DQ before treatment(P<0.05);The pathogenesis was the main influencing factor of the prednisone treatment effect,the length of the disease,the worse the treatment(P<0.05).CRH、cortisol、ACTH after treatment were significantly lower than before treatment(P<0.05).Conclusion Prednisone can effectively control the onset of infantile spasms,and early treatment is better.IS patient has HPA axis dysfunction,and prednisone can regulate HPA axis dysfunction to control spasm.
Clinical effects of low-dose ACTH combined with valproate on infantile spasms

Abstract:Objective:To observe the effects of low-dose and short-duration adrenocor-ticotrophic hormone(ACTH)combided with valproate(VPA) on infantile spasms(IS).Methods:The control group(including the symptomatic subgroup and the cryptogenic subgroup) was given oral VPA solution,while the observation group(including the symptomatic subgroup and the cryptogenic subgroup) was given with low-dose VPA solution and short-duration ACTH.The frequency of spasms,the changes of video electroencephalogram(VEEG) and the adverse drug reaction were observed.Results:The rate of successfully controlled both spasms and hypsarrhythmic EEG pattern in the observative symptomatic subgroup was significantly higher than that of the control symptomatic subgroup(P<0.05) and the rate of successfully controlled both spasms and hypsarrhythmic EEG pattern in the observative cryptogenic subgroup was obviously higher than that of the control cryptogenic subgroup(P <0.01).The rate of successfully controlled both spasms and hypsarrhythmic EEG pattern in the observative cryptogenic subgroup was significantly higher than that of the observative symptomatic subgroup(P <0.05).No cases of the serious adverse drug reaction were found.Conclusion:Combining low-dose,short-duration ACTH with VPA on IS is of curative effectsa,nd adverse drug reaction is little.
[Factors in first-time adrenocorticotropic hormone therapy and their influence on spasm control time in infantile spasms: a Cox proportional-hazards regression model analysis].

Abstract:OBJECTIVE:To investigate the factors in first-time adrenocorticotropic hormone (ACTH) therapy and their influence on spasm control time in infants with infantile spasms. METHODS:A total of 72 infants with infantile spasms who were admitted from January 2008 to October 2013 were enrolled. Their clinical data were collected, and the exposure factors for infantile spasms were selected. A Cox proportional-hazards regression model analysis was performed for these factors to analyze their influence on spasm control time. RESULTS:Clarification of the etiology (known or unexplained etiology), frequency of spasms before treatment, and presence or absence of combination therapy (ACTH used alone or in combination with magnesium sulfate) had a significant influence on spasm control time in infants with infantile spasms. The infants with a known etiology had a significantly shorter spasm control time than those with unexplained etiology, and the infants with a low frequency of spasms before treatment and receiving ACTH combined with magnesium sulfate early had a significantly longer spasm control time than their counterparts (P<0.05). CONCLUSIONS:For infants with infantile spasms at initial diagnosis, etiology should be clarified, which may helpful for evaluating prognosis. A combination of ACTH and magnesium sulfate should be given as soon as possible, which may improve their prognosis.

Abstract:Objective To examine the preventive effect of magnesium sulfate on hypertension caused by ACTH in the treatment of infantile spasms (IS). Methods 46 children diagnosed as IS were recruited from two hospitals during May, 2011 to October, 2013.23 patients in group A (treatment group) were treated with magnesium sulfate and ACTH in hospital A;another 23 cases in group B (control group) were treated with ACTH only in hospital B. The therapy course was 2 weeks. Results Hyperten-sion was not observed in the treatment group, while 6 children were observed with hypertension in the control group. There was signiifcant difference between the two groups (P<0.05). Conclusions Magnesium sulfate could prevent the incidence of hyper-tension in the treatment of IS with ACTH, and beneift the completion of treatment course.
Cost‐effectiveness of adrenocorticotropic hormone versus oral steroids for infantile spasms

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>To compare the effectiveness and cost‐effectiveness of adrenocorticotropic hormone (ACTH) and oral steroids as first‐line treatment for infantile spasm resolution, we performed a systematic review, meta‐analysis, and cost‐effectiveness study.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>A decision analysis model was populated with effectiveness data from a systematic review and meta‐analysis of existing literature and cost data from publicly available prices. Effectiveness was defined as the probability of clinical spasm resolution 14 days after treatment initiation.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>We included 21 studies with a total of 968 patients. The effectiveness of ACTH was not statistically significantly different from that of oral steroids (.70, 95% confidence interval [CI] = .60–.79 vs. .63, 95% CI = .56–.70; <i>p</i> = .28). Considering only the three available randomized trials with a total of 185 patients, the odds ratio of spasm resolution at 14 days with ACTH compared to high‐dose prednisolone (4–8 mg/kg/day) was .92 (95% CI = .34–2.52, <i>p</i> = .87). Adjusting for potential publication bias, estimates became even more favorable to high‐dose prednisolone. Using US prices, the more cost‐effective treatment was high‐dose prednisolone, with an incremental cost‐effectiveness ratio (ICER) of $333 per case of spasms resolved, followed by ACTH, with an ICER of $1 432 200 per case of spasms resolved. These results were robust to multiple sensitivity analyses and different assumptions. Prednisolone at 4–8 mg/kg/day was more cost‐effective than ACTH under a wide range of assumptions. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Significance</h3><p>For infantile spasm resolution 2 weeks after treatment initiation, current evidence does not support the preeminence of ACTH in terms of effectiveness and, especially, cost‐effectiveness.</p></section>

Abstract:Infantile spasms is an age-specific epileptic syndrome associated with poor developmental outcomes and poor response to nearly all traditional antiepileptic drugs except adrenocorticotropic hormone (ACTH). We investigated the protective mechanism of ACTH against brain damage. An infantile spasm rat model induced by N-methyl-D-aspartate (NMDA) in neonate rats was used. Pregnant rats were randomly divided into the stress-exposed and the non-stress exposed groups, and their offspring were randomly divided into ACTH-treated spasm model, untreated spasm model, and control groups. A proteomics-based approach was used to detect the proteome differences between ACTH-treated and untreated groups. Gel image analysis was followed by matrix-assisted laser desorption/ionization time-of-flight mass spectrometric protein identification and bioinformatics analysis. Prenatal stress exposure resulted in more severe seizures, and ACTH treatment reduced and delayed the onset of seizures. The most significantly up-regulated proteins included isoform 1 of tubulin β-5 chain, cofilin-1 (CFL1), synaptosomal-associated protein 25, malate dehydrogenase, N(G),N(G)-dimethylarginine dimethylaminohydrolase 1, annexin A3 (ANXA3), and rho GDP-dissociation inhibitor 1 (ARHGDIA). In contrast, tubulin α-1A chain was down-regulated. Three of the identified proteins, ARHGDIA, ANXA3, and CFL1, were validated using western blot analysis. ARHGDIA expression was assayed in the brain samples of five infantile spasm patients. These proteins are involved in the cytoskeleton, synapses, energy metabolism, vascular regulation, signal transduction, and acetylation. The mechanism underlying the effects of ACTH involves the molecular events affected by these proteins, and protein acetylation is the mechanism of action of the drug treatment.

Abstract:Infantile spasms is an epileptic encephalopathy of early infancy with specific clinical and electroencephalographic (EEG) features, limited treatment options, and a poor prognosis. Efforts to develop improved treatment options have been hindered by the lack of experimental models in which to test prospective therapies. The neuropeptide adrenocorticotropic hormone (ACTH) is effective in many cases of infantile spasms, although its mechanism(s) of action is unknown. This review describes the emerging candidate mechanisms that can underlie the therapeutic effects of ACTH in infantile spasms. These mechanisms can ultimately help to improve understanding and treatment of the disease. An overview of current treatments of infantile spasms, novel conceptual and experimental approaches to infantile spasms treatment, and a perspective on remaining clinical challenges and current research questions are presented here. This summary derives from a meeting of specialists in infantile spasms clinical care and research held in New York City on June 14, 2010.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Aim</h3><p>To compare the efficacy and safety of prednisolone/prednisone and adrenocorticotropic hormone (ACTH) in the treatment of infantile spasms using a meta‐analysis of randomized controlled trials (RCTs).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Method</h3><p>In a systematic literature search of electronic databases (MEDLINE, Embase, the Cochrane Library), we identified RCTs that assessed prednisolone/prednisone compared with ACTH/tetracosactide in patients with infantile spasms. The electroclinical response and adverse events were evaluated.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Six RCTs (616 participants) were included in the meta‐analysis. Compared with prednisolone/prednisone, ACTH/tetracosactide was not superior in terms of cessation of spasms at day 14 (relative risk 1.19, 95% confidence interval [CI] 0.74–1.92), day 42 (relative risk 1.02, 95% CI 0.63–1.65), and resolution of hypsarrhythmia on electroencephalogram (relative risk 1.14, 95% CI 0.71–1.81); the incidences of common adverse reactions caused by ACTH/tetracosactide were not lower than that of prednisolone/prednisone for irritability (relative risk 0.79, 95% CI 0.57–1.10), increased appetite (relative risk 0.78, 95% CI 0.57–1.08), weight gain (relative risk 0.86, 95% CI 0.56–1.32), and gastrointestinal upset (relative risk 0.60, 95% CI 0.35–1.02), though it seemed less frequent.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Interpretation</h3><p>Prednisolone/prednisone elicits a similar electroclinical response as ACTH for infantile spasms, which indicates that it can be an alternative to ACTH for treating infantile spasms.</p></section>

Abstract:Background: Infantile spasms (IS) is an age-specific and severe epileptic encephalopathy that occurs in infancy and early childhood and is usually refractory to conventional antiepileptic drugs. Adrenocorticotropic hormone (ACTH) has been the treatment of choice for IS, but ACTH use has been associated with infection and hypertension. Magnesium ion is an N-methyl-D-aspartic acid (NMDA) noncompetitive antagonist that might inhibit NMDA activity and has antiepileptic and neuroprotective effects.Objective: This study compared the efficacy and tolerability of ACTH + magnesium sulfate (MgSO4) versus ACTH monotherapy for the treatment of IS.Methods: This 24-week, randomized, open-label follow-up study enrolled male and female infants with IS. Patients were randomly assigned to receive ACTH 25 U/d + MgSO4 0.25 g/kg/d, or ACTH 25 U/d only (control), intravenously for 3 weeks. Efficacy was assessed over a period of 24 weeks based on seizure frequency, EEG, and Gesell testing of psychomotor skills (subscales: language, motor, adaptive, and personal-social skills; measured using a developmental quotient [DQ]). Tolerability was assessed by monitoring for adverse events using laboratory analysis and clinical evaluation.Results: Thirty-eight infants were enrolled (23 male, 15 female; median age, 9.2 months; 19 patients per group). At 12 weeks, 14 patients (73.7%) who received ACTH + MgSO4 and 9 patients (47.4%) in the control group were seizure free. At 24 weeks, seizure-free rates were 12 (63.2%) in the ACTH + MgSO4 group and 10 (52.6%) in the control group. On EEG, 9 patients (47.4%) in the ACTH + MgSO4 group achieved complete recovery (normalized EEG), 5 (26.3%) attained partial improvement (multifocal spike wave), and 5 (26.3%) had no improvement (hypsarrhythmia or modified hypsarrhythmia). At 4 weeks, in the control group, 5 patients (26.3%) achieved complete recovery, 6(31.6%) achieved partial improvement, and 8(42.1%) had no improvement. Of the 12 patients who were seizure free at 24 weeks in the ACTH + MgSO4 group, 11(91.7%) had complete recovery (normalized EEG); this rate was 7 of 10 (70.0%) in the control group. In the ACTH + MgSO4 group, the change from baseline to 24 weeks in mean (SD) personal-social DQ was significant (from 48.6 [6.4] to 65.2 [7.1], respectively; P < 0.05). In the control group, the difference before and after treatment was nonsignificant (47.7 [6.0] vs 49.9 [4.4]). None of the other Gesell test findings were significant versus baseline. The most common AEs included upper respiratory tract infection and pyrexia (both, 3 [15.8%] per group); diarrhea (2 [10.5%] per group); and hypertension, insomnia, and irritability (all, 0 in the ACTH + MgSO4 group and 2 [10.5%] in the control group). None of the between-group differences in the prevalences of AEs were significant between the 2 groups.Conclusions: In this study in infants with IS, the proportions of patients who were seizure free from 4 to 24 weeks were significantly greater in the ACTH + MgSO4 group compared with the ACTH monotherapy group. Personal-social neurodevelopment was significantly improved from baseline in the group that received combination treatment. Both treatments were generally well tolerated. Abstract:OBJECTIVES To study the factors influencing the short-term (28 days) efficacy of initial adrenocorticotropic hormone (ACTH) therapy for infantile epileptic spasms syndrome (IESS), as well as the factors influencing recurrence and prognosis. METHODS The clinical data were collected from the children with IESS who received ACTH therapy for the first time in the Department of Pediatric Neurology, Xiangya Hospital of Central South University, from April 2008 to January 2018 and were followed up for ≥2 years. The multivariate logistic regression analysis was used to evaluate the factors influencing the short-term efficacy of ACTH therapy, recurrence, and long-term prognosis. RESULTS ACTH therapy achieved a control rate of seizures of 55.5% (111/200) on day 28 of treatment. Of the 111 children, 75 (67.6%) had no recurrence of seizures within 12 months of follow-up. The possibility of seizure control on day 28 of ACTH therapy in the children without focal seizures was 2.463 times that in those with focal seizures (P<0.05). The possibility of seizure control on day 28 of ACTH therapy in the children without hypsarrhythmia on electroencephalography on day 14 of ACTH therapy was 2.415 times that in those with hypsarrhythmia (P<0.05). The possibility of recurrence within 12 months after treatment was increased by 11.8% for every 1-month increase in the course of the disease (P<0.05). The possibility of moderate or severe developmental retardation or death in the children without seizure control after 28 days of ACTH therapy was 8.314 times that in those with seizure control (P<0.05). The possibility of moderate or severe developmental retardation or death in the children with structural etiology was 14.448 times that in those with unknown etiology (P<0.05). CONCLUSIONS Presence or absence of focal seizures and whether hypsarrhythmia disappears after 14 days of treatment can be used as predictors for the short-term efficacy of ACTH therapy, while the course of disease before treatment can be used as the predictor for recurrence after seizure control by ACTH therapy. The prognosis of IESS children is associated with etiology, and early control of seizures after ACTH therapy can improve long-term prognosis.

Abstract:Background: Infantile spasms is a rare disease characterized by distinct seizures and hypsarrhythmia. Adrenocorticotropic hormone (ACTH) is available as a natural product (repository corticotropin injection, [RCI]; Acthar® Gel) and as synthetic analogs. RCI is a naturally-sourced complex mixture of purified ACTH analogs and other pituitary peptides approved by the United States Food and Drug Administration as a monotherapy for the treatment of infantile spasms. RCI is commonly used in the United States. Outside the United States, synthetic analogs of ACTH-synthetic ACTH1-24 (tetracosactide) and synthetic ACTH1-39 (corticotropin carboxymethyl-cellulose [CCMC])-are used. The efficacy of RCI may differ from that of synthetic ACTH treatments based on the structure of peptide; however, no head-to-head clinical trials have compared the efficacy of RCI and synthetic ACTH treatments. Objective: A systematic review and indirect treatment comparison of clinical trials was conducted to assess the comparative efficacy of RCI and synthetic ACTH treatments in infantile spasms. Methods: A search was conducted in MEDLINE, EMBASE, and Cochrane databases through September 30, 2020. Relevant clinical trials on RCI or synthetic ACTH therapy and reporting either cessation of spasms or resolution of hypsarrhythmia, separately or as a combined outcome were included. A Bayesian indirect treatment comparison using a fixed-effects model was used for comparative efficacy. Results: Of 473 citations screened, 21 studies were reviewed qualitatively. In the indirect treatment comparison of six eligible clinical trial studies, the odds of achieving efficacy outcomes were five to eight times greater with RCI than with tetracosactide and 14 to 16 times greater than CCMC. This translated to a risk reduction of 10% to 14% and 40% to 50% with RCI versus tetracosactide and CCMC, respectively. For every two to five patients treated, RCI improved efficacy outcomes in one additional patient compared to synthetic ACTH (adjusted number needed-to-treat). Conclusions: Based on the available limited evidence, results suggest RCI may be more efficacious for infantile spasms than synthetic ACTH treatments. Our findings provide a blueprint to inform the design of future prospective studies for the treatment of infantile spasms.
An Open-Label Clinical Study on the Efficacy and Tolerability of Magnesium Sulfate Combined with Adrenocorticotropic Hormone Treatment on Infantile Spasm

Abstract:Background Infantile spasm (IS) is characterized by spasm seizure, hypsarrhythmia in EEG, and psychomotor retardation. It is refractory to conventional antiepileptic drugs (AEDs). As firstline therapy for IS, adrenocorticotropic hormone (ACTH) has side effects, such as infection and hypertension. This study aims to evaluate the efficacy and tolerability of magnesium sulfate combined with ACTH treatment by open-label clinical trial. Methods An open-label clinical study was designed. According to inclusion and exclusion criteria, objects were inpatients with IS. A total of 55 patients with IS were enrolled. After clinical data collection, standard magnesium sulfate combined with ACTH was given to the patients for 2 weeks. Efficacy and safety were assessed regularly. The evaluation parameters of efficacy included change of spasm seizure frequency and EEG after treatment. During administration process, vital signs were monitored, laboratory items were tested regularly, and adverse events were daily recorded by guardian. Results After treatment of magnesium sulfate combined with ACTH for 2 weeks, seizure control efficiency was 78.18%(43/55), and the improvement rate of EEG was 55.77% (29/52), wherein 25 patients (45.45%) acquired seizure free, 6 patients (11.54%) presented normal in EEG examination. There were no statistical differences of the effect of different onset ages (≤ 4 months, > 4 months) on seizure control efficiency rate (χ2 = 0.595, P = 0.441) and EEG improvement rate (χ2 = 1.325, P = 0.250), the effect of different courses of disease (≤ 2 months, > 2 months) on seizure control efficiency rate (Fisher's exact test: P = 0.735) and EEG improvement rate ( χ2 = 2.668, P = 0.102), and the effect of different etiologies (idiopathic or cryptogenic IS, symptomatic IS) on seizure control efficiency rate (Fisher's exact test: P = 1.000) and EEG improvement rate ( χ2 = 2.215, P = 0.145). No adverse events, such as hypertension, occurred. After long-term (1 month, 3 months and 12 months) follow-up for 29 patients, seizure free rate remained roughly unchanged (χ2 = 0.945, P = 0.815).
