Abstract:Objective To evaluate the effects of laparoscopic common bile duct exploration (LCBDE) and endoscopic retrograde cholangiopancreaticography and endoscopic sphincterotomy(ERCP/EST) combined with laparoscopic cholecystectomy(LC) in the treat-ment of cholecystolithiasis and choledocholithiasis. Methods The clinical data of patients suffering from cholecystolithiasis and choledocholithiasis from Jan.2012 to Jul.2013 were retrospectively analyzed. LC+LCBDE was performed in 124 cases and ERCP/EST combined LC was performed in 123 cases. Parameters including common bile duct (CBD) diameter, stone size, hospitalization cost, hospital stay, postoperative complications, residual bile duct stone incidence were comparatively studied. Results There was no statistical difference in the number of stone , conversion rate and residual bile duct stones incidence. But there were significant dif-ference in CBD diameter, stone size, hospitalization cost, hospital stay, and early postoperative complication. Conclusion Both of LC+LCBDE and ERCP/EST+LC were safe and reliable to treat patients with cholecystolithiasis and choledocholithiasis. The treatment for patients must be individualized. LC+LCBDE was better than ERCP/EST combined LC.
Intraoperative ERCP and endoscopic sphincterotomy combined with laparoscopic cholecystectomy for treatment of cholelithiasis with choledocholithiasis

Abstract:Objective: To evaluate the benefit of intraoperative ERCP and endoscopic sphincterotomy (EST) combined with laparoscopic cholecystectomy (LC) in the treatment of cholelithiasis with choledocholithiasis. Methods: Fifty-two patients with cholelithiasis and choledocholithiasis (as determined by intraoperative cholangiography) were randomly divided into 2 groups during LC. In group A (27 patients), common bile duct stones were extracted by intraoperative EST during LC. In group B(25 patients), common bile duct (CBD) stones were extracted by conversion to open CBD exploration and cholecyctectomy. Results: The success rate was 26/27 (96.3%) in group A and 25/25 (100%) in group B (0.25< P <0.5); The mean postoperative hospitalization was 3.32±0.56 days in group A and 17.5±4.61 days in group B ( P <0.001). In group A, two cases were complicated transient hyperamylasemia after the combined procedure. In group B, one case of bile leakage and one case of duodenal ulcer occurred after conversion to open cholecystectomy with CBD exploration. There were no retained stones in group A but 2 cases in group B. Conclusion: Intraoperative ERCP and endoscopic sphincterotomy combined with LC for treatment of cholelithiasis and chiledochlithiasis is safe, effective and results in shorter hospitalization and fewer complications than traditional open cholecystectomy with CBD exploration.
Clinical Application Effect of Different Methods of ERCP in the Treatment of Elderly Patients with Common Bile Duct Stones

Abstract:[Objective] To explore clinical application effect of different methods of ERCP in the treatment of elderly patients with common bile duct stones.[Methods]The clinical data of 158 elderly patients(≥ 60 years old)with choledo-cholithiasis treated with ERCP from June 2013 to May 2016 were analyzed retrospectively.According to the treatment method of ERCP,the patients were divided into two groups :endoscopic sphincterotomy group(EST group,n ＝68)and LEST+EPBD group(n ＝90).The operation time,hospitalization time,success rate of lithotomy and the occurrence of short-and long-term complications were observed and recorded in the two groups.[Results]There was no significant differ-ence between the two groups in hospitalization time,success rate,the number of stones,the diameter of stones and the incidence of postoperative complications(mean P ＞0.05),but the rate of mechanical lithotripsy and operative time in the EST group were significantly higher than those in the LEST+EPBD group,The lonh-term recurrence rate of stone in the EST group was significantly higher than that in LEST+EPBD group(P＜0.05).[Conclusion]ERCP is a safe and effective method for the treatment of elderly patients with choledocholithiasis.Both EST and LEST+EPBD can achieve satisfactory results.But compared with EST,LEST+EPBD can preserve the normal anatomical structure and function of sphincter of duodenal papilla and effectively reduce the risk of postoperative complications and reduce the operation time,which is worthy of clinical application.
Comparison of laparoscopic common bile duct exploration plus cholecystectomy and endoscopic retrograde cholangiopancreatography followed by laparoscopic cholecystectomy for elderly patients with common bile duct stones and gallbladder stones

Abstract:Background: Common bile duct (CBD) stones commonly occur in cholecystectomy cases. The management options include laparoscopic CBD exploration (LCBDE) or endoscopic retrograde cholangiopancreatography (ERCP) followed by laparoscopic cholecystectomy (LC). Although ERCP is fully developed, it has complications, and LCBDE is a proven alternative. This study aimed to evaluate the safety and efficacy of these treatments in elderly individuals aged ≥70 years. Methods: A retrospective study between January 2015 and July 2022 included 160 elderly patients (aged ≥70 years) diagnosed with cholelithiasis and choledocholithiasis. The patients were divided into 1-stage (LCBDE [n = 80]) or 2-stage (ERCP followed by LC [n = 80]) treatment groups. Data collected encompassed comorbidities, symptoms, bile duct clearance, postoperative complications, and long-term outcomes for systematic analysis. Results: This study analyzed 160 patients treated for CBD stones, comparing 1-stage and 2-stage groups. The 1-stage group had more female patients than the 2-stage group (57.5% vs 37.5%, respectively). The 1-stage group had a mean age of 80.55 ± 7.00 years, which was higher than the mean age in the 2-stage group. American Society of Anesthesiologists classification, Charlson Comorbidity Index, and laboratory findings were similar. Pancreatitis and cholangitis occurred after ERCP in the 2-stage group. Stone clearance rates (92.35% [1-stage group] vs 95.00% [2-stage group]) and biliary leakage incidence (7.5% [1-stage group] vs 3.0% [2-stage group]) were similar, as were postoperative complications and long-term recurrence rates (13.0% [1-stage group] vs 12.5% [2-stage group]). Conclusion: Our research indicates that both the combination of LCBDE and LC and the sequence of ERCP followed by LC are equally efficient and secure when treating CBD stones in elderly patients. Consequently, the 1-stage procedure may be considered the preferred treatment approach for this demographic.
ERCP for common bile duct stones in the elderly: refine the procedure to improve outcomes

Abstract:Removal of common bile duct stones in patients with a previous cholecystectomy was one of the first indications for ERCP with biliary sphincterotomy. Thanks to a minimally invasive procedure, patients were prevented from having a new operation. Subsequently, as the technique proved to be successful, ERCP was extended to all patients with choledocholithiasis, regardless of whether or not they had gallbladder. Also contributing was the fact that, at least in the beginnings, surgical interventions on the bile duct with laparoscopic cholecystectomy were more difficult. Nowadays, many surgeons prefer to perform cholecystectomy with a bile duct clean of stones. In this issue of the Spanish Journal of Gastroenterology, Gardenyes et al. present a study on ERCP for common bile duct stones in elderly patients. The novelty of this study is not only to analyze the ERCP procedure, which we already knew has similar success and complication rates to younger patients, but also to focus on the long-term outcome, considering the frailty that frequently accompanies aging. The study concludes that older patients may benefit from enhanced care protocols to reduce medical adverse events and improve outcomes. For us gastroenterologists and endoscopists, another conclusion that can be drawn from this study is that we should not be satisfied that ERCP in older patients has the same success and complication rates as in younger patients, but rather we should strive to ensure that the results are even better.

Safety and Efficiency of ERCP in the Elderly Patients

Abstract:Objective To compare the efficacy and safety of endoscopic retrograde cholangiopancreatography (ERCP) in elder and younger groups of patients. Methods Three hundred and one patients who underwent ERCP between Jan. 2006 and Dec. 2007 were studied retrospectively. One hundred and two patients(33.9%) were 70 years of age or older (Elder group) and 199 patients(66.1%) were 69 years of age or younger (Younger group). Data about ASA degree, clinical and biochemical features, primary diseases, ERCP procedures, ERCP diagnosis, complications and length of hospital stay were recorded and compared between the two groups. Results Three hundred and one cases were undergone 313 ERCP procedures. Elder group had a higher ASA degree (P0.01) than Younger group. Compared with Younger group (14.1%), Elder group patients presented with more specific symptoms (30.4%) with jaundice and biliary tract infection while Younger group with more non-specific symptoms (P0.05). Laboratory findings were similar in both groups. Selective biliary cannulation was technically successful in 96.7% of both groups (96.1% vs 97.0%). Pre-cut papillotomy was performed in 12.7% of Elder group and 14.1% of Younger group. The overall successful rate of stone extraction was 94.6% (94.8% vs 94.4%). Choledocholithiasis was the most frequent diagnosis in both groups. Malignant aetiologies and duodenal peripapilla diverticulum were more frequent in Elder group. Post-procedural complications developed in 14(4.7%) patients. There were 5(4.9%) complications in Elder group. In contrast, 9(4.5%) of the Younger group developed a complication. Conclusion Diagnostic and therapeutic ERCP have similar outcomes and complications rate in both elder and younger patients. ERCP is effective and safe in elderly patients.
Comparing percutaneous transhepatic papillary balloon dilation with endoscopic retrograde cholangiopancreatography in elderly patients for common bile duct stones: a 3-year retrospective study

Abstract:Purpose: To assess the safety and effectiveness of PTPBD for CBD stones in the elderly population. Methods: Patients aged 60 years or older, who underwent PTPBD or ERCP for CBD stones between January 2021 and December 2023, were included in this study and divided into either the PTPBD group or the ERCP group based on the procedure they underwent. Baseline characteristics, intraoperative and postoperative outcomes were collected and analyzed using SPSS 25.0. Results: A total of 145 cases were enrolled in the study. In terms of intraoperative complications, one patient in the ERCP group experienced hemorrhaging, while one patient in the PTPBD group experienced acute pain. However, neither of these patients were in a serious condition and only required observation. Stone removal procedures were successfully conducted in approximately 95% of cases in both the PTPBD and ERCP groups (95.8 and 94.8%, respectively; P > 0.999). ERCP failures were observed in one patient with a previous Billroth II anastomosis and two patients with unconventional anatomy of the esophagus and stomach. There was no statistically significant difference in postoperative complications between the PTPBD group and the ERCP group (P > 0.05). The length of hospital stays did not differ between the PTPBD group and the ERCP group (P = 0.537 > 0.05). Conclusion: PTPBD can be used in patients who have complicated anatomical issues that make the ERCP procedure difficult. In comparison, PTPBD is a similarly safe, effective, and minimally invasive technique for extracting CBD stones in elderly patients.
Therapeutic value of endoscopic retrograde cholangiopancreatography for choledocholithiasis in patients over 80 years old

Abstract:Objective:To evaluate the efficacy and safety of endoscopic retrograde cholangiopancreatography (ERCP) for the elderly patients (80 years or older) with choledocholithiasis.Methods:A retrospective study was conducted on 742 cases of choledocholithiasis who underwent ERCP at Nanjing Drum Tower Hospital Affiliated to Nanjing University Medical School from January 2014 to December 2017. The patients were divided into two groups: 196 patients aged 80 or above (the very elderly group) and 546 patients aged 60 or below (the non-elderly group). The baseline data including gender, number and size of stones of the two groups were matched using the 1∶1 propensity score matching, and the covariate equalization samples between the two groups were obtained and 196 pairs of patients were successfully matched. The treatment success rate, complication incidence, hospitalization time and cost were analyzed.Results:Comorbidities in patients of the elderly group were significantly more than those in the non-elderly group [93.4% (183/196) VS 76.5% (150/196), P<0.001]. The success rates of treatment were 96.4% (189/196) and 96.9% (190/196), respectively ( P=0.778). The incidence of postoperative complications were 8.7% (17/196) in the elderly group and 6.6% (13/196) in the non-elderly group ( P=0.447). The mean hospitalization time and the mean hospitalization cost of the elderly group and the non-elderly group were 8.20 days, 23 624.06 Yuan and 8.03 days, 22 249.64 Yuan, respectively (all P>0.05). Conclusion:ERCP is effective and safe, and can be used as the first choice for senile patients with choledocholithiasis.
Laparoscopic Common Bile Duct Exploration is a Safe and Effective Strategy for Elderly Patients

Abstract:Laparoscopic cholecystectomy is considered the most effective treatment for gallstones. However, there is no agreement on the treatment of common bile duct stones, which are a common cause of abdominal pain. This article aims to evaluate and analyze the results of common bile duct exploration in elderly patients and compare the results of open and laparoscopic groups. We analyzed a case series of all elderly patients over 70 years who underwent common bile duct exploration in the Department of Hepatobiliary and Pancreatic Surgery between January 2020 and February 2021. All the operating procedures were carried out by the same surgical team. A total of 104 common bile duct explorations were performed on elderly patients with a mean age of 77.4 years (range, 70–87 years). Of these, 50 patients were in the open common bile duct exploration group, while the laparoscopic common bile duct exploration group included 54 patients, with 7 patients converted to open surgery and excluded from analysis. There were no statistically significant differences between the two groups in terms of risk factors before operation, such as hypertension, diabetes, heart diseases, cerebral vascular disease, and previous abdominal surgery. The duration of surgery for the laparoscopic common bile duct exploration group was significantly shorter than that of the open common bile duct exploration group (149.3 ± 49.0 min versus 191.7 ± 109.9 min; p = 0.015). Moreover, the laparoscopic common bile duct exploration group had a shorter postoperative hospital stay than the open common bile duct exploration group (10.6 ± 4.9 days versus14.0 ± 6.8 days; p = 0.006). Postoperative complications in the open common bile duct exploration group were higher than in the laparoscopic common bile duct exploration group. Laparoscopic common bile duct exploration can be considered a safe and effective treatment for elderly patients with common bile duct stones, not only for young people. Safety analysis of endoscopic retrograde cholangiopancreatography and its associated treatments in the elderly

Abstract:Objective:To investigate the safety of endoscopic retrograde cholangiopancreatogra-phy(ERCP)and its associated treatments in the elderly aged 65 years and over, and analyze the related factors leading to postoperative complications.Methods:Totally 512 patients who received ERCP and its associated treatments in the Department of Gastroenterology, Peking University People's Hospital from January 2013 to January 2019 were included retrospectively.The clinical data, operative procedures and postoperative complications were collected.The differences in underlying diseases, operative procedures and postoperative complications between the elderly group(≥65 years old, n=301)and the non-elderly group(n=211)were compared, and the correlations between them were analyzed.Results:The proportion of hypertension, diabetes, heart disease and diverticulum beside the nipple was higher in the elderly group than in the non-elderly group(all P<0.01). The proportion of preoperative medication of anticoagulant and antiplatelet drugs was higher in the elderly group than in the non-elderly group(all P<0.01). There were no significant differences in cholelithiasis, history of biliary surgery, history of ERCP, immunity disease and tumor between the two groups(all P>0.05). Endoscopic sphincterotomy was the most common ERCP-associated treatments in both groups, followed by the order of endoscopic balloon dilation of the nipple, endoscopic biliary stent drainage, pancreatic duct intubation, and pancreatic duct stenting.The proportions of endoscopic papillary balloon dilation and endoscopic biliary stent drainage were 34.6%(104 cases)and 18.9%(57 cases)respectively, in the elderly group, which were statistically significantly higher than 25.6%(54 cases)and 11.4%(24 cases)in the non-elderly group(all P<0.05). The common bile duct diameter and maximum stone diameter were(1.32±0.43)cm and(1.04±0.53)cm, respectively in the elderly group, which were statistically significantly higher than those in the non-elderly group(1.16±0.40)cm and(0.81±0.03)cm respectively, ( t values were -4.23 and -4.76, respectively, all P<0.01). The proportions of endoscopic papillary balloon dilation and biliary stent drainage were statistically significantly higher in elderly patients than in non-elderly patients(all P<0.05). The incidence of intraoperative bleeding was 4.0%(12 cases)and 5.7%(12 cases)in the elderly group and non-elderly group, respectively, with no statistical significance( χ2=0.08, P>0.05). There was no significant difference in the incidence of postoperative pancreatitis, bleeding, perforation, infection and contrast agent-related complications(all P>0.05). Conclusions:Clinically widely used high-risk endoscopic papillary balloon dilation and endoscopic biliary stent drainage(ERCP-associated treatments)are frequently performed in elderly patients, and do not increase intraoperative bleeding and postoperative complications in the elderly.Therefore, ERCP-associated treatments are safe for the elderly.
Clinical Effect of Endoscopic Retrograde Cholangiopancreatography for Elderly Patients with Periampullary Diverticula Accompanied with Choledocholithiasis

Abstract:Objective To explore the clinical effect of endoscopic retrograde cholangiopancreatography (ERCP) for elderly patients with periampullary diverticula accompanied with choledocholithiasis.Methods The retrospective cross-sectional study was conducted.The clinical data of 297 elderly patients with age ＞70 years and periampullary diverticula accompanied with choledocholithiasis who were admitted to the Zhongshan Hospital affiliated to Fudan University between January 2013 and January 2016 were collected.All the patients received lithotomy by ERCP after completion of preoperative preparation,and then underwent symptomatic treatment.Observation indicators included:(1) treatment results:cannulation time,success rate of cannulation and success rate of stones removed;(2) surgical complications:hemorrhage,perforation and pancreatitis;(3) follow-up results.All patients were followed up by outpatient examination and telephone interview up to December 2016.Follow-up included postoperative delayed perforation,patients' survival,further attack of acute cholangitis and reoperation for removing stones.Measurement data with normal distribution were represented as x ± s,and measurement data with skewed distribution were described as M (range).Results (1) Treatment results:all the 297 patients underwent successful lithotomy by ERCP,with a cannulation time of (5±4)minutes and a success rate of cannulation of 100.00％ (297/297).Of 297 patients,292 had one-off success of stones removed,with a one-off success rate of 98.32％ (292/297),and 5 received partial stones removed due to choledocholithiasis combined with acute cholangitis.(2) Surgical complications:4 patients were complicated with immediate hemorrhage during intraoperative endoscopic sphincterotomy (EST).Bleeding of 1 patient was cauterized by biopsy forceps,and 3 patients had simplex JHY-BAL compression hemostasis.Two patients with postoperative delayed hemorrhage had successful hemostasis by biopsy forceps with metal hemostatic clip under endoscope after emergency duodenoscopy.One patient with intraoperative perforation was cured by conservative treatment.Two patients with severe acute pancreatitis underwent continuous gastrointestinal decompression combined with maintenance therapy of somatostatin,and then received endoscopic ultrasound-guided puncture and drainage for pseudocyst around the pancreas,with a stable symptoms of pancreatitis after 4 weeks.(3) Follow-up results:all the 297 patients were followed up for 6-12 months,with a median time of 8 months.During follow-up,292 patients had healthy survival,without recurrences of cholangitis and bile duct stone,and no delayed perforation and death were detected.Five patients underwent the second time lithotomy by ERCP after 2-3 months postoperatively.Conclusion Lithotomy by ERCP is safe and effective in the treatment of elderly patients with periampullary diverticula accompanied with choledocholithiasis.
Analysis of Safety of Therapeutic ERCP in 90 Years of Age and Older

Abstract:Objective To evaluate the curative effect and safety of emergency therapeutic endoscopic retrograde holangiopancreatography (ERCP) and treatment strategy of mobidity and combined diseases for patients aged over 90. Methods The clinical and follow-up data of 116 cases treated by ERCP from January 2002 to December 2006 were analyzed retrospectively. Results The success rate was 97.41% for biliary drainage. The occurrence rate of mobidity was 21.24% (24/113),of which 6 cases of acute pancreatitis (25.00%),2 cases of upper gastrointestinal bleeding (8.33%), 12 cases of electrolyte disorders (50.00%),acid-base balance disorders in 4 cases (16.67%). Except the higher incidence of hypokalemia disorders of emergency group than out-patient group (P=0.003), the rest of the mobidity rates were similar in the two groups. The mortality rate and deterioration rate of combined disease between the two groups were also similar. Conclusion Simplify operations,rapid drainage,positive preoperative preparation can effectively reduce the incidence of mobidity and avoid the aggravation of combined diseases. Emergency ERCP for treatment of patients aged over 90 is safe and effective.
Analysis of 48 Cases of Emergency ERCP in Elderly Patients with Acute Suppurative Cholangitis

Abstract:Objective To explore the effect and safety of elder patients with acute purulent cholangitis treated by emergency ERCP.Methods 48 elder patients(≥70years) with acute purulent cholangitis were reviewed retrospectively.The patients were divided into two groups;the ENBD group: 30 patients who underwent emergency ENBD only,and the EST group: 18 patients who underwent emergency ERCP included stones removal after EST and subsequent ENBD.The drainage effect,stone removal success rate and complication were recorded in both groups.Results Drainage efficiency was got 97.9% success rate in total 48 cases,which ENBD group 29 cases,the success rate of 96.7%,18 cases of EST Group,the success rate of 100%.There was no significant difference between the two groups(P0.05).The success rate of stone removal one-time in EST group was 83.3%(15/18),ENBD group 96.7%(29/30).The ENBD group was higher than the EST group(P0.05).The ERCP-related complications occurred 1 cases(3.3%) in the ENBD group,2 cases(11.1%) in the EST group,respectively.the EST group was higher than the ENBD group in complications(P0.05).No death in both groups.Conclusion The emergency ERCP is an effective and safe method for elderly patients with acute suppurative cholangitis,and should be the preferred treatment option.

Abstract:Background: Recently, the incidence of acute obstructive cholangitis caused by bile duct stones in patients aged 90 years and older (super-old) has been increasing, for which urgent endoscopic retrograde cholangiopancreatography (ERCP) treatment may be required. The aim of this study was to evaluate the efficacy and safety of urgent ERCP in super-old patients with acute cholangitis caused by bile duct stones. Methods: A total 147 consecutive patients aged between 75 and 99 years who underwent urgent ERCP for acute cholangitis caused by bile duct stones were analyzed in a retrospective manner. They were divided into the old (age 75-89 years, control) and super-old (age 90-99 years) groups. Urgent ERCP efficacy and safety, including general status, ERCP-related findings and outcomes, cardiopulmonary monitoring during ERCP, and mortality, were compared between the groups. Results: The physical status of the super-old group was worse than that of the old group. The success rates for biliary drainage and complete clearance of bile duct stones at the first attempt in the super-old group were lower as compared to the old group, while those after two attempts increased in the super-old group and were nearly the same as in the old group. No fatal cardiopulmonary complications during ERCP were observed in either group. Mortality rate within 2 months was higher in the super-old group, though recovered to the same level as in the old group after 2 months. Conclusions: Efficacy and safety of urgent ERCP treatment in super-old patients were comparable to those seen in old patients, though the overall trend indicated greater difficulty. Urgent ERCP treatment can be useful for acute cholangitis caused by bile duct stones in super-old patients.

Abstract:Objective To evaluate the safety and efficacy of therapeutic ERCP for patients above 90 years of age.Methods The data of 37 patients of above 90 years who underwent 42 ERCP procedures from January 2001 to December 2009 were studied retrospectively and compared with those of 152 matched patients ( 168 procedures) below 65 years old at a 1∶4 ratio for success rate and complications.Results The rate of complete success,partial success,and failure in observation group was 73.81％ (31/42),19.05％(8/42) and 2.38％ (1/42),respectively,which were similar (P ＞0.05) with those in control group,with complete success rate at 85.12％ ( 143/168),partial success rate at 12.50％ (21/168) and failure rate at 2.38％ (4/168).The rate of terminated operation in observation group (4.76％,2/42) was significantly higher than that of the control group (0.00％,0,P =0.039).The overall rate of complication in observation group was 7.14％ ( 3/42 ),slightly higher than that of the control group ( 6.55％,11/168,P ＞0.05 ).There was no significant difference between the two groups regarding the rates and severity of such complications as pancreatitis,hemorrhage and infection ( P ＞ 0.05 ).No perforation or death was observed.Conclusion Therapeutic ERCP for patients of 90 years or older is safe and effective.Adverse events related to chronic concomitant diseases need early detection and proper management.
Endoscopic Retrograde Cholangiopancreatography for Biliary Stricture with Bile Duct Stone after Liver Transplantation

Abstract:Objective To investigate the efficacy and safety of ERCP for biliary stricture with duct stone after liver transplantation.Methods Clinical data of 60 patients undergoing ERCP for biliary stricture with duct stone after liver transplantation between January 2013 and June 2014 were retrospectively analyzed.Results ERCP was successfully performed 78 times in 60 patients.Bile duct stenosis was cured in 24 cases (40％),improvement was observed in 27 cases (45％) and was not obvious in 9 cases (15％).Bile duct stones were successfully removed in 39 patients (65％).Incidence of post-ERCP complications was 13％ (8/60),including mild pancreatitis in 1 case,hyperamylasemia in 5 cases,biliary tract infection in 2 cases,which were all resolved after conservative treatment.Conclusion ERCP is a safe and effective treatment for biliary stricture with duct stone following liver transplantation.

Abstract:Objective: The treatment of choledocholithiasis with duodenal stenosis is a clinical difficult problem. This study aimed to investigate the efficacy and safety of ERCP via gastroscopy in the treatment of choledocholithiasis and duodenal stenosis. Methods: From January 2015 to December 2020, 21 patients with choledocholithiasis with duodenal stenosis who underwent ERCP treatment under gastroscopy in our hospital were enrolled. The patients' case characteristics, ERCP status, and complication rate were analyzed. Results: Among the 21 patients, 17 cases were successful in ERCP, and a total of 29 times ERCPs were performed, with an average of 1.71 times per patient. Among the failures of ERCP, selective deep intubation of common bile duct was unsuccessful in 4 cases. Six patients underwent multiple lithotomies, after the operation, of which 4 patients underwent secondary ERCP lithotomy and 2 patients underwent triple ERCP lithotomy. All patients successfully completed the balloon dilation without serious complications. Two patients developed mild acute pancreatitis after ERCP, and all recovered after medication. Conclusion: In patients with choledocholithiasis and duodenal stenosis, ERCP treatment by gastroscopy has a higher success rate and does not increase the incidence of complications, but there is a problem of cholecystolithiasis recurrence.
EUS- Versus ERCP-guided Biliary Drainage for Malignant Biliary Obstruction: Evidence from Randomized Controlled Studies

Abstract:Background: Endoscopic retrograde cholangiopancreatography-guided biliary drainage (ERCP-BD) is the first-line technique for the management of malignant biliary obstruction. Previous evidence demonstrated that endoscopic ultrasound-guided biliary drainage (EUS-BD) is technically safe and feasible, and EUS-BD has been used as the alternative palliative treatment when ERCP fails. We aimed to compare the therapeutic efficacies and procedure-associated complications of EUS-BD and ERCP as the first treatment option. Methods: Studies comparing efficacies and complications of EUS-BD and ERCP-BD for the management of malignant biliary obstruction published before January 25, 2019 were searched. The patient characteristics, variables regarding to the therapeutic efficacies and procedure-associated complications of EUS-BD were compared to that of ERCP-BD by meta-analyses. Results: After screening 821 studies, three randomized clinical trials (RCTs) comparing the therapeutic efficacies and complications of EUS-BD and ERCP-BD for the palliative treatment of malignant obstructive jaundice were included. No significant difference was found between EUS-BD and ERCP-BD regarding to the technical success rate, clinical success rate, procedure time, and stent patency at 3 months and 6 months. The incidence of overall adverse events was also similar between the two groups. EUS-BD showed higher frequency of stent patency at 12 months. In addition, EUS-BD showed significant decreased incidence of procedure-associated pancreatitis [odds ratio (OR) =0.08, 95% confidence intervals (CI): 0.01 to 0.62, P=0.02) and reintervention rate (OR =0.25, 95% CI: 0.12 to 0.54, P=0.004). Conclusions: Compared to ERCP-BD, EUS-BD showed equivalent therapeutic efficacy with decreased incidence of pancreatitis and reintervention rate. EUS-BD, like ERCP-BD, could be the first-line technique for the management of malignant biliary obstruction.

Abstract:Background Endoscopic retrograde cholangiopancreatography (ERCP) is recommended by major guidelines for the removal of common bile duct (CBD) stones but is technically challenging in patients with low cardiopulmonary reserve and anatomic abnormalities of the upper gastrointestinal (GI) tract. Purpose To compare percutaneous transhepatic papillary balloon dilation (PTPBD) with ERCP for CBD stone removal. Materials and Methods Participants with one to three CBD stones (largest stone ≤30 mm) and without intrahepatic bile duct or gallbladder stones were eligible for this prospective cohort study. PTPBD was recommended in participants with low cardiopulmonary reserve or definitive anatomic abnormalities of the upper GI tract. Otherwise, both procedures were offered without preference. Follow-up, including abdominal CT, was conducted at 1-week and 1-, 3- and 6-month follow-up, and every 6 months thereafter. US and MR cholangiopancreatography were conducted if recurrence could not be confirmed with CT. Technical success rate was the primary outcome. Results A total of 531 participants were analyzed: there were 360 undergoing PTPBD (median age, 76 years; interquartile range [IQR], 64-82 years; 163 men) and 171 undergoing ERCP (median age, 66 years; IQR, 57-74 years; 94 men). The technical success rate was 99% (355 of 360) in the PTPBD group and 98% (167 of 171) in the ERCP group (relative risk, 1.02; = .12). The incidence of overall complications was 4% (13 of 360) for PTPBD and 8% (13 of 171) for ERCP (relative risk, 0.27; 95% CI: 0.12, 0.61; < .001). The PTPBD group showed a longer fluoroscopy time and a higher radiation exposure, with adjusted differences of 28.7 minutes (95% CI: 22.2, 35.2) and 384.3 mGy (95% CI: 296.5, 472), respectively. A propensity score-matching analysis ( = 123 per group) indicated that PTPBD had a slightly higher technical success rate and significantly fewer complications. Conclusion When compared with endoscopic retrograde cholangiopancreatography, percutaneous transhepatic papillary balloon dilation has a similar technical success rate and fewer perioperative complications but a higher radiation exposure. © RSNA, 2021 . Abstract:Objective To evaluate the clinical efficiency of stepwisely removing huge common bile duct stones by endoscopic treantment(ERCP and EST).Motheds 183 patients with common bile duct stones no less than 1.5 cm were selected to take the endoscopic treatment,such as mechanical lithotripsy,removing stones by basketnet with/or balloon,and bile duct drainage(endoscopic nasobiliary drainage,Teflon plastic stents).The size of the dissected papillary muscle and diameters of the calculus,the times of the operation and the complications caused by ERCP and EST were recorded.Results While the size of the calculus is bigger,the times of removing stone increased and bile duct drainage and surgical intervention were more needed.However,other complications didn′t rise apparently.Conclusion Removing the common bile duct stones step by step by endoscopic treatment(ERCP and EST,lithotripsy,etc) is efficient,safe and reliable.
