Abstract:OBJECTIVE: To identify risk factors associated with adverse airway events (AAEs) in primary oral cancer patients undergoing tumor ablation followed by free tissue transfer without prophylactic tracheostomy.METHODS: We retrospectively collected primary oral cancer patients who underwent tumor ablation surgery following free-tissue transfer without prophylactic tracheostomy during February 2017 to June 2019 in Chang Gung Memorial Hospital, Linkou Medical Center, Taiwan. 379 patients were included. Data were analysed from 2020 to 2021. Demographics, comorbidities, intraoperative variables and postoperative respiration profile were obtained from the medical record. Main outcome was postoperative AAEs, including requirement of endotracheal intubation after extubation and tracheostomy after prolonged intubation.RESULTS: Of the 379 patients, postoperative AAEs happened in 29 patients (7.6 %). In reintubation group, patients were older with more diabetes mellitus, hypertension and cerebrovascular disease. These patients had lower preoperative hemoglobin, creatinine, and albumin level with more intraoperative blood transfusion. In postoperative respiration profile, rapid shallow breathing index (RSBI) and PaO2/FiO2 (PF) ratio were poorer. On multivariate analysis, patient's age, tumor location, and cross-midline segmental mandibulectomy and a lower PF ratio were independent risk factors for postoperative AAEs.CONCLUSIONS: In head and neck cancer patients that underwent tumor ablation followed by free tissue transfer without prophylactic tracheostomy, patient's age, tumor location, cross-midline segmental mandibulectomy and P/F ratio are associated with postoperative AAEs.

[Factors Affecting Selection of Tracheostomy after Mandibular Tumor Operation: a Retrospective Study].

Abstract:PURPOSE:To assess the factors affecting selection of tracheostomy after mandibulectomy.METHODS:The clinical data of 165 patients who were divided into intubated group and tracheostomy group were collected from January 2008 to December 2012 in Shanghai Ninth People's Hospital, including demographics, smoke habits, alcohol consumption, pulmonary disease, American Society of Anesthesiologists (ASA) physical status classification, history of jaw operation, length of surgery, preoperative radiotherapy, free flap reconstruction, resection over the anterior midline, and radical neck dissection. The postoperative outcomes, such as the duration of keeping tube, the length of ICU stay, the length of hospital stay, and the number of complications and death were collected. Statistical analysis was performed using SAS version 9.2 software package.RESULTS:There were 81 patients in intubated group and 84 patients in tracheostomy group. Three factors that might be associated with selection of tracheostomy after mandibulectomy were preoperative radiotherapy (OR: 3.51, 95% CI: 1.34-9.20), free flap reconstruction (OR: 3.99, 95%CI: 1.84-8.65), and resection over the anterior midline of the jaw (OR: 20.08, 95%CI: 6.52-160.35)(P<0.05).CONCLUSIONS:Tracheostomy was suitable for patients who received preoperative radiotherapy, free flap reconstruction and resection over the anterior midline after mandibular tumor resection were factors in considering of tracheotomy.
Analysis of the risk factors and pathogenic bacteria of ventilator-associated pneumonia

Abstract:OBJECTIVE To explore the risk factors of ventilator‐associated pneumonia and the distribution of the related pathogens ,so as to provide scientific evidence for the prevention and control of ventilator associated pneu‐monia. METHODS A retrospective analysis was conducted to 172 patients who were treated with mechanical venti‐lation in ICU from Feb. 2012 to Feb. 2014. The clinical features ,risk factors and the distribution of pathogens related to ventilator‐associated pneumonia were studied. All data were analyzed by SPSS 11. 0. RESULTS In 172 patients who were treated with mechanical ventilation ,78 were infected with ventilator‐associated pneumonia ,the incidence rate was 45. 35%. The sputum specimens of them were examined and 105 strains of pathogenic bacteria were found. Of them ,62 strains were gram‐negative bacilli ,accounting for 59. 05%. 27 strains were gram‐posi‐tive bacilli ,accounting for 25. 71%. 16 strains were fungi ,accounting for 15. 24%. The risk factors of ventilator‐associated pneumonia included age ,underlying diseases ,mechanical ventilation time ,and the existence of con‐sciousness disorder , tracheotomy , invasive operation , antibiotic application and duration of hospitalization. CONCLUSION There are many risk factors for ICU patients with ventilator‐associated pneumonia so we call for a strict ventilation index ,which can provide the corresponding nursing measures considering these risk factors. Strict implementation of aseptic management ,precaution of reflux and aspiration ,and rational use of antimicrobial drugs ,can effectively reduce the incidence of ventilator‐associated pneumonia.

Abstract:Journal of International Medical Research, Volume 52, Issue 9, September 2024. ObjectiveTo systematically evaluate potential risk factors for tracheal stenosis and to provide a reference for the prevention and management of patients with this condition.MethodsDatabases were searched to identify studies of the risk factors for tracheal stenosis, from their inception to October 2023, then a meta-analysis was performed. The study was registered with PROSPERO under the registration number CRD42023428906.ResultsTen studies of a total of 2525 patients were included. The meta-analysis showed that tracheotomy, diabetes, the duration of intubation, the duration of mechanical ventilation, respiratory tract infection, a high incision, and a ratio of intratracheal tube cuff diameter (C)/transverse diameter at the level of the clavicle (T) >150% were risk factors for the development of tracheal stenosis.ConclusionMeasures such as shortening the duration of mechanical ventilation and intubation, reducing and avoiding tracheotomy after prolonged intubation, early tracheotomy in patients with obesity who require prolonged mechanical ventilation, appropriate choices of incision location and catheter, the maintenance of appropriate C/T, the prevention of respiratory infection, and the control of diabetes mellitus should limit the risk of tracheal stenosis.

[Clinical Analysis of Selective Tracheostomy Necessary for Patients Undergoing Head and Neck Surgery with Free Flap Reconstruction].

Abstract:OBJECTIVE:To discover the factors that may affect the use of selective tracheostomy among patients who have undergone head and neck surgeries with free flap reconstruction, so that the patients will not need tracheostomy nor receive the unnecessary treatment.METHODS:Five hundred and thirty-three patients who had undergone head and neck surgery with free flap reconstruction operated by the same team of surgery at Department of Oral and Maxillofacial Surgery at Peking University School of Stomatology from 2015 to 2016 were reviewed. Three hundred and twenty-one (60.2%) of these patients underwent selective tracheostomy. All the patients' demographic information, operation-related information, prior treatments, comorbidities and complications were recorded and analyzed.RESULTS:The patients with defects of the tongue, mouth floor, oropharynx and bilateral mandible, who underwent neck dissection and with previous radiotherapy and smoking habit were more likely to get selective tracheostomy. Usage of bulky soft tissue flap might also add to the risk of airway obstruction and the need of selective tracheostomy, while other factors were not significantly related to the risk of postoperative airway obstruction and the patients could be kept safe without selective tracheostomy. Most cases without tracheostomy were kept safe except one case, while 8.39% of the patients with tracheostomy suffered from tracheostomy related complications, mainly pneumonia and hemorrhage of the tracheostomy wound, yet none led to serious consequences or even death.CONCLUSION:Selective tracheostomy is not necessary for patients who have undergone head and neck surgeries with free flap reconstruction except that there are defects at the tongue, oropharynx and mandible. Neck dissection, bulky soft tissue flap reconstruction, previous radiotherapy and smoking habit may also add to the risk of postoperative airway obstruction, while a favorable decision would involve a combination of all the above factors to assure the safety of the postoperative airway for the patients undergone head and neck surgeries with free flap reconstruction.
Risk factors for postoperative lower respiratory tract infections in elderly patients undergoing surgeries under general anesthesia and etiological analysis

Abstract:OBJECTIVE To explore the risk factors for the postoperative lower respiratory tract infections in the elderly patients undergoing upper abdominal surgeries under general anesthesia and analyze the etiological characteristics so as to take the targeted prevention measures and choose appropriate antibiotics for treatment of the lower respiratory tract infections .METHODS The clinical data of 350 patients who underwent the upper abdominal surgeries under general anesthesia from Aug 2011 to Sep 2014 were retrospectively analyzed ,then the interventions were taken to the patients with lower respiratory tract infections ,the related factors and etiological characteristics of the lower respiratory tract infections were analyzed ,and the multivariate logistic regression analysis was performed .RESULTS The lower respiratory tract infections occurred in 50 of 350 elderly patients , with the infection rate of 14 .28% .A total of 59 strains of pathogens have been isolated ,of which 77 .97% were the gram‐negative bacteria ,and there were 17 strains of multidrug‐resistant bacteria .The incidence of the lower respiratory tract infections was significantly correlated with the smoking history , complication of underlying disease ,tumor ,ICU stay ,surgical site ,operation duration ,duration of action of anesthesia ,type of intubation , and postoperative extubation time ,and the difference was statistically significant (P<0 .05) .CONCLUSION The multiple risk factors may increase the risk of the postoperative lower respiratory tract infections in the elderly patients undergoing upper abdominal surgeries under general anesthesia . It is necessary for the hospital to strengthen the control of the risk factors so as to reduce the risk of the lower respiratory tract infections .
Postoperative Prognostic Risk Factors and Treatment Strategies for Patients with T3-T4 Hypopharyngeal Squamous Cell Carcinoma

Abstract:Objective:To investigate the independent prognostic risk factors and the application of different postoperative adjuvant treatment strategies on patients with T3-T4 hypopharyngeal carcinoma. Methods:Based on the inclusion criteria, we conducted a retrospective study that analyzed the clinical data of 229 cases who underwent surgery at the Eye, Ear, Nose and Throat Hospital of Fudan University from 2003 to 2015. Risk factors for postoperative survival were analyzed by univariate and multivariate analysis and patients were stratified according to the independent risk factors. The efficacy of various postoperative adjuvant therapystrategies were compared in different risk groups. Results:Two hundred and twenty-nine patients with T3-T4 primary tumor had 3-year and 5-year overall survival rates of 39.07% and 29.03%, respectively; 3-year and 5-year recurrence-free survival rates were 40.22% and 30.29%, respectively. The results of multivariate analysis demonstrated that tumors in the posterior pharyngeal wall and posterior cricoid region, N2c-N3, lymphovascular invasion, and extranodal extension were independent risk factors. The utility of postoperative adjuvant therapy significantly improved postoperative survival in the high-risk group （P<0.05） and reduced postoperative recurrence significantly （P<0.01）. Patients received adjuvant chemoradiation had better survival than those received adjuvant radiation alone （P<0.05）. Postoperative adjuvant therapy had no significant effect on postoperative survival of patients in the low-risk group. Conclusion:Independent risk factors for patients with T3-T4 hypopharyngeal carcinoma include tumors in the posterior pharyngeal wall and posterior cricoid region, N2c-N3, lymphovascular invasion, as well as extranodal extension. Postoperative adjuvant therapy, especially adjuvant chemoradiation,is recommended for patient with risk factors mentioned above. For patients without those risk factors, postoperative follow-up and re-examination are recommended.

Abstract:Objective To investigate the risk factor of operative related hospital acquired pneumonia.Methods 210 patients admitted to Zhongshan hospital for surgery were included in our retrospective study.To investigate the risk factors of hospital acquired pneumonia used Logistic regression.ResultsMultiple regression analysis selected the following variables significantly associated with postoperative pneumonia:male,age≥60 years,the surgery intensive care unit,immunocompromised states/serious underlying disease,the presence of tracheo-intubation or tracheostomy and period longer than 12 hours,a mechanical ventilation period longer than 48 hours,the use of a cascade humidification system,prior surgery,shock in operation,location of catheter,persistence of consciousness,the kinds of antibiotic prophylactic therapy more than three and period longer than 3 days,treatment with antacids and /or H 2-receptor blocking and period longer than 3 days,period of surgery longer than 2 hours,the presence of an ultimately or rapidly fatal underlying disease,upper abdomen surgery,general anesthesia,the presence of nasogastric intubation and period longer than 6 hours.The factors high correlated with postoperative pneumonia analysis by Logistic analysis were:age≥60 years,the presence of nasogastric intubation≥6 hours,prophylaxis antibiotic therapy,administration of antacids and/or H 2-receptor blocking,immunocompromised states and serious underlying disease.Conclusion The risk factors high related with pneumonia were:age≥60 years,presence of nasogastric intubation≥6 hours,prophylaxis antibiotic therapy,administration of antacids and/or H 2-receptor blocking,immunocompromised states and serious underlying diseases.
Analysis of Procedure Factors Contributing to Airway Management after Oral-Maxillofacial Tumour Surgery

Abstract:Objective To identify the optimal management of airway in patients after maxillofacial tumor surgery. Methods Between June 2009 and June 2010,112 patients with oral-maxillofacial tumour were operated.Following data were divided into 3 groups according to the post-operative airway management strategy:extubation(32 patients),indwelling nasoendotracheal tube(29 patients),elective tracheostomy(51 patients).Tumour site,mandibulectomy or not,method of neck dissection,method of reconstruction were recorded and evaluated by logistic regression,a threshold score was then defined. Results A tracheostomy scoring system was developed to guide post -operative airway management.Tumour site, mandibulectomy or not,method of neck dissection,method of reconstruction were identified as significant parameters with different weightings.When threshold score was above 4-5,an elective tracheostomy was needed.Conclusion Whether an elective tracheotomy in maxillofacial tumour surgery is necessary can be facilitated using the score system.
Analysis of risk factors of postoperative complication for non-small cell lung cancer

Abstract:Background: The relationship between risk factors of common postoperative complications after pulmonary resection, such as air leakage, atelectasis, and arrhythmia, and patient characteristics, including nutritional status or perioperative factors, has not been sufficiently elucidated. Methods: One thousand one hundred thirty-nine non-small cell lung cancer patients who underwent pulmonary resection were retrospectively analyzed for risk factors of common postoperative complications. Results: In a multivariate analysis, male sex (P = 0.01), age ≥ 65 years (P < 0.01), coexistence of chronic obstructive pulmonary disease (COPD) (P < 0.01), upper lobe (P < 0.01), surgery time ≥ 155 min (P < 0.01), and presence of lymphatic invasion (P = 0.01) were significant factors for postoperative complication. Male sex (P < 0.01), age ≥ 65 years (P = 0.02), body mass index (BMI) < 21.68 (P < 0.01), coexistence of COPD (P = 0.02), and surgery time ≥ 155 min (P = 0.01) were significant factors for severe postoperative complication. Male sex (P = 0.01), BMI < 21.68 (P < 0.01), thoracoscopic surgery (P < 0.01), and surgery time ≥ 155 min (P < 0.01) were significant risk factors for postoperative air leakage. Coexistence of COPD (P = 0.01) and coexistence of asthma (P < 0.01) were significant risk factors for postoperative atelectasis. Prognostic nutrition index (PNI) < 45.52 (P < 0.01), lobectomy or extended resection more than lobectomy (P = 0.01), and surgery time ≥ 155 min (P < 0.01) were significant risk factors for postoperative arrhythmia. Conclusion: Low BMI, thoracoscopic surgery, and longer surgery time were significant risk factors for postoperative air leakage. Coexistence of COPD and coexistence of asthma were significant risk factors for postoperative atelectasis. PNI, surgery time, and surgical procedure were revealed as risk factors of postoperative arrhythmia. Patients with these factors should be monitored for postoperative complication. Trial registration: The Institutional Review Board of Kanazawa Medical University approved the protocol of this retrospective study (approval number: I392), and written informed consent was obtained from all patients.
Analysis of Risk Factors of Ventilator-Associated Pneumonia Caused by Stenotrophomonas Maltophilia in Postoperative Cancer Patients

Abstract:Objective To investigate the risk factors of ventilator-associated pneumonia (VAP) caused by S. maltophilia (Sm) in postoperative cancer patients.Methods The 35 postoperative cancer patients with VAP caused by Sm were enrolled as Sm VAP group from February 2001 to May 2008. At the same time,the control group was defined as 105 patients with non-Sm VAP. Several statistically significant risk factors were screened out by univariate analysis. Then,the independent predictive risk factors were determined by multivariate stepwise logistic regression analysis. Results The APACHEⅡ score,duration of mechanical ventilation and use of broad spectrum antibiotics,level of plasma albumin were significantly different between Sm VAP group and non-Sm VAP group (P0.05). Forward stepwise logistic regression showed that use of broad spectrum antibiotics 7 days was an independent predictive risk factor for Sm VAP.Conclusions APACHEⅡ10,mechanical ventilation 7 days,use of broad spectrum antibiotics 7 days,and level of plasma albumin ≤ 30 g/L are relative risk factors for postoperative cancer patients to develop Sm VAP. Use of broad spectrum antibiotics 7 days is an independent risk factor of Sm VAP in postoperative cancer patients. Actively improving the general conditions of patients,reducing duration of mechanical ventilation and limiting the use of broad spectrum antibiotics are key factors for postoperative cancer patients to prevent Sm VAP.
The Analysis of Pulmonary Infect Flora Distribution and Risk Factors of Stroke Patients with Tracheotomy in Intensive Care Unit

Abstract:Objective To investigate pulmonary infect flora distribution characteristics of stroke patients with tracheotomy in Intensive care unit (ICU) and analyze the risk factors.Methods A total of 792 cases of ICU stroke patients was selected,including 426 cases of tracheotomy patients,and 366 cases of non-tracheotomy patients.The incidence of lung infections was compared.Bacteriological examination was used for tracheotomy bacteriological studies for lung infected stroke patients with tracheotomy.Bacteria infection's characteristics was observed.The risk factors were analyzed.Results (1) The tracheotomy patient 's lung infection rate was 23.00％,higher than 9.56％ of the non-tracheotomy patients,the difference was statistically significant (X2 =19.125,P ＜0.05);(2) For lung infection-occurred patients with tracheotomy of ICU,gram-negative bacteria infection rate was 63.97％,significantly higher than 19.85％ of gram-positive bacteria and 16.18％ of fungi,the difference was statistically significant (x2 =18.255,17.042,P ＜ 0.01);(3) Lung infection rate of ICU stroke tracheotomy patients with unconscious,dysphagia,hospitalization time ＞ 14 d,blood glucose levels ≥≥7.8 mmol/L,and based diseases was significantly higher than that of patients with conscious,non-dysphagia,hospitalization time ≤＜14 d,blood sugar level ＜7.8 mmol/L,and no based diseases (P ＜ 0.05);(4) Consciousness,dysphagia,hospitalization,blood sugar levels,and the underlying disease were the independent risk factors of lung infection in ICU stroke tracheotomy patients (OR1 =11.528,OR2 =8.046,OR3 =15.174,OR4 =7.795,ORs =10.784,P ＜0.05).Conclusions Gram-negative bacteria is the main reason for pulmonary infections in ICU stroke tracheotomypatients patients with stroke.State of consciousness,invasive treatment,hospitalization,blood sugar levels,and the underlying disease are the independent risk factors.
Preoperative tracheotomy as reflection of tumor size impacting oncologic outcomes of patients with advanced stage glottic carcinoma

Abstract:Purpose The aim of this study was to evaluate the influence of preoperative tracheotomy on oncologic outcomes of advanced stage glottic carcinoma patients, and to explore the potential reason. Methods We retrospectively analyzed 413 consecutive advanced stage glottic carcinoma patients from January 2005 to December 2010. The correlation of preoperative tracheotomy and potential impacting factor of tumor size involving tumor diameter and tumor area with overall survival (OS) and disease-free survival (DFS) was fully assessed. Results Our cohort consisted of 302 (73.1%) patients with T3 and 111 (26.9%) patients with T4, and 98 (23.7%) patients received preoperative tracheotomy. The OS and DFS rates of patients receiving preoperative tracheotomy were worse than those without (5-year OS: 49.3% versus 69.8%; 5-year DFS: 45.3% versus 61.0%). The mean tumor diameter and tumor area of patients with preoperative tracheotomy were greater than those without (3.3 cm versus 2.4 cm, 8.9 cm 2 versus 4.7 cm 2 ). The optimal cutoff values of tumor diameter and tumor area for tracheotomy were 2.85 cm and 6.64 cm 2 . Tumor diameter and tumor area were correlated with tracheotomy intervention. Furthermore, when considering the potential effect of tumor area in multivariate model, we found that it was a significant factor in survival outcomes but variable of preoperative tracheotomy was not. Conclusion This study indicates that tumor size is correlated with preoperative tracheotomy, and tracheotomy intervention may be reflection from effect of great tumor size that is a true adverse factor influencing oncologic outcomes of advanced stage glottic carcinoma patients.
Incidence of Pneumothorax Experienced after Orthognathic Surgery.

Abstract:The purpose of this study is to evaluate the frequency of pneumothorax following orthognathic surgery and describe its clinical presentation. A retrospective analysis of a hospital database was carried out on consecutive patients with normal presurgical clinical assessment, laboratory findings, and chest X-ray who underwent orthognathic surgery from January 2007 to September 2018 in the Department of Oral and Craniomaxillofacial Surgery, Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine. Two patients (0.038%), 1 male (age 20 years) and 1 female (age 32 years), complained of respiratory difficulty and chest pain and were clearly diagnosed with postoperative pneumothorax by radiographic chest X-rays from a sample of 5229 consecutive patients during the study period. Intercostal drainage under local anesthesia was performed immediately and the treatment effects for both patients were satisfactory. The present study findings indicate that although orthognathic surgeries can be safely performed in patients with craniofacial anomalies, some unexpected complications such as pneumothorax may occur. Therefore, accurate postoperative follow-up must be done in every patient to monitor possible clinical complications. Patients who experience respiratory difficulty and postoperative chest pain may have pneumothorax, and once it is diagnosed, treatment should be promptly carried out to eliminate further severe sequelae.
Clinical characteristics and influencing factors of nosocomial infections of respiratory system after general anesthesia

Abstract:OBJECTIVE To explore the clinical characteristics and influencing factors of nosocomial infections of respiratory system after general anesthesia with tracheal intubation so as to provide reference for reducing infections .METHODS Clinical data were collected of 1 200 patients undergoing general anesthesia with tracheal intubation from Jun .2011 to Jun .2014 .The nosocomial infection rate was recorded ,and the infectious pathogens and influencing risk factors were analyzed .RESULTS In 1 200 cases of patients with general anesthesia ,42 cases had postoperative respiratory system infections with the infection rate of 3 .50% ,including 13 cases (30 .95% ) of upper respiratory tract infections ,29 cases (69 .05% ) of lower respiratory tract infections .Totally 58 strains of pathogenic bacteria of nosocomial infections were detected , including 34 strains (58 .63% ) of gram positive bacteria ,19 strains (32 .75% ) of gram negative bacteria ,5 strains (8 .62% ) of fungi .The univariate analysis showed that the nosocomial infection rate was significantly high in patients with old age ,long duration of general anesthesia and postoperative catheter indwelling ,smoking history ,no operative analgesia and nervous system operation (P<0 .05) .The multivariate analysis showed significantly increased nosocomial infection rate in patients without postoperative analgesia .The risk of nosocomial infections of the respiratory system was significantly high in patients with longer duration of general anesthesia and catheter indwelling and older age ( P< 0 .05 ) . CONCLUSION The main pathogenic bacteria for nosocomial infections of respiratory system in patients undergoing general anesthesia with tracheal intubation were gram positive .Abolishment of postoperative analgesia ,long dura‐tion of general anesthesia and catheter indwelling and old age were the independent risk factors for infections .

Abstract:OBJECTIVE:To explore the risk factors of pulmonary infection after D2 radical gastrectomy of gastric cancer in order to guide clinical measures to reduce the incidence of pulmonary infection.METHODS:Clinical data of 371 patients undergoing D2 radical gastrectomy at Division 2 of Gastrointestinal Cancer Center from October 2014 to October 2016 were collected for retrospective cohort study. Associated risk factors of pulmonary infection after D2 radical gastrectomy of gastric cancer were analyzed. Diagnosis criteria of pulmonary infection: occurrence of new pulmonary rales; new infiltrative change or consolidation in chest by imaging examination; with at least one of the following: temperature ≥38.5centi-degree, emergence of new purulent sputum or sputum character change, isolated pathogens from bronchial brush biopsy or tracheal secretion culture.RESULTS:Of 371 patients, 265 were males and 106 were females. The average age was 59.1(22-80) years old. There were 38(10.2%) cases of pulmonary infection after radical resection of gastric cancer. Univariate analysis showed that smoking history, intra-operative blood loss ≥200 ml, total gastrectomy, and gastric intubation ≥6 d were associated with pulmonary infection after D2 radical gastrectomy (all P<0.05). Six cases quitted smoking 2 weeks before operation with pulmonary infection incidence of 8.1%(6/74), and 16 patients did not quit smoking 2 weeks before operation with pulmonary infection incidence of 21.1%(16/76), and the difference was statistically significant (χ2=4.0387, P=0.0445). Multivariate Logistic regression analysis showed that postoperative gastric intubation ≥6 d (OR=4.335, 95%CI: 1.088 to 4.586, P=0.05), smoking history (OR=3.469, 95%CI: 1.056 to 5.252, P=0.043) and intra-operative blood loss ≥200 ml (OR=3.931, 95%CI: 1.350 to 10.574, P=0.013) were independent risk factors of pulmonary infection after D2 radical gastrectomy of gastric cancer.CONCLUSION:For gastric cancer patients undergoing D2 radical gastrtectomy with smoking history, greater blood loss during operation and postoperative gastric intubation ≥6 d, surgeons must pay attention to the prevention of postoperative pulmonary infection.
Multivariate Analysis of Risk Factors for Pulmonary Infection after Radical Gastrectomy for Gastric Cancer

Abstract:OBJECTIVE:To explore the major risk factors for pulmonary infection after radical gastrectomy in patients with gastric cancer.METHODS:From November 2010 to February 2014, a total of 765 patients undergoing radical gastrectomy at our hospital were divided into 2 groups based upon the presence of postoperative pulmonary infection (n = 32, 4.2%) or not (n = 733, 95.8%). Their clinicopathological data were retrospectively analyzed by Logistic regressive analysis with a case-control study model.RESULTS:Comparing with the control group, the patients had longer surgical duration (245.7 ± 66.7 vs 210.9 ± 47.2 min, P < 0.01), higher rates of requiring intensive care (12.50% vs 2.86%, P = 0.02) and longer post-operative hospital stays (21.9 ± 24.9 vs 14.2 ± 4.2 days, P < 0.01) in the postoperative pulmonary infection group.Univariate Logistic regressive analysis found that age ≥ 60 years, smoking ≥ 400 year by cigarette, diabetes mellitus, chronic obstructive pulmonary disease, proximal or total gastrectomy, combined organ resection, surgical duration ≥ 240 min, intra-operative blood loss ≥ 300 ml, peri-operative transfusion, transfusion ≥ 3 unit packed red blood cell, post-operative transfusion and post-operative complications other than pulmonary infections were associated with postoperative pulmonary infection (all P < 0.05).Further multivariate analysis identified 4 independent risk factors for pulmonary infection after radical gastrectomy, including diabetes mellitus (OR = 4.77, 95%CI:1.18-19.23), post-operative complications other than pulmonary infections (OR = 3.15, 95%CI:1.25-7.90), intra-operative blood loss ≥ 300 ml (OR = 2.63, 95%CI:1.17-5.90) and post-operative nasogastric tube ≥ 5 days (OR = 2.30, 95%CI:1.02-5.21).CONCLUSION:Correcting the modifiable risk factors may reduce the incidence of pulmonary infection and shorten the length of hospital stays and costs after radical gastrectomy in patients with gastric cancer.
Preoperative Tracheotomy in the Treatment of Upper Airway Obstruction of Patients with Advanced Stage Supraglottic Carcinoma.

Abstract:Background: Preoperative tracheotomy is an effective option that secures upper airway patency in laryngeal carcinoma patients suffering from upper airway obstruction, but the influence of this treatment on oncologic outcomes of laryngeal carcinoma remains controversial. The purpose of this study was to determine the impact of preoperative tracheotomy on overall survival in supraglottic carcinoma patients with tumor obstruction of the upper airway, and explore the potential causes. Materials and methods: This retrospective study collected 243 consecutive patients with advanced stage supraglottic carcinoma from 2005 to 2010. Preoperative tracheotomy in the management of upper airway obstruction in patients with supraglottic carcinoma was analyzed. Results: The mean age was 60.9 years at diagnosis, with men accounting for 98.4% of all patients. Thirty nine (16.0%) patients presenting with tumor obstruction of the upper airway required preoperative tracheotomy. T4 stage patients had higher rate of tracheotomy than those of patients with T3 stage (36.8% vs 12.2%). Patients with upper airway obstruction presented with greater tumor area compared with patients without (13.7 cm(2) vs 9.0 cm(2)). The optimal cutoff value of tumor area for tracheotomy and OS rate were both at 10 cm(2). Supraglottic patients with upper airway obstruction receiving preoperative tracheotomy had poorer OS rate compared with patients without. T stage and tumor area were correlated with upper airway obstruction, and these two variables were independent predictors of OS rate in supraglottic carcinoma patients. Conclusions: Advanced stage supraglottic carcinoma patients with upper airway obstruction undergoing preoperative tracheotomy experienced worse overall survival. Advanced T stage and greater tumor size were associated with upper airway obstruction, indicating that the negative influence of tumor obstruction on survival may be cause by these two preoperative variables. Therefore, preoperative tracheotomy acts only as an alternative procedure, and is not a prognostic agent.

Abstract:Lung cancer remains a leading cause of cancer‐related mortality, with surgical resection as a primary treatment modality. However, postoperative wound infections (PWIs) pose significant risks following thoracoscopic radical resection. This study aims to identify the risk factors and pathogenetic characteristics associated with PWIs in lung cancer surgery. A comprehensive retrospective study was conducted from August 2021 to June 2023 at our institution. The study included 30 patients who developed PWIs and 60 controls who did not, following thoracoscopic radical resection for lung cancer. We evaluated various factors including age, hospital stay, intraoperative blood loss, body mass index (BMI), operation time, prophylactic antibiotic use, diabetes mellitus and tumour staging. Diagnostic criteria for PWIs were based on clinical signs and microbiological confirmation. Statistical analysis was performed using SPSS software, utilizing chi‐square tests, and univariate and multivariate logistic regression analyses. The predominant pathogens identified in PWIs were Escherichia coli, Klebsiella pneumoniae and Pseudomonas aeruginosa. Univariate analysis revealed operation time (≥4 h) and diabetes mellitus as significant risk factors for PWIs, while prophylactic antibiotic use was associated with a lower incidence of PWIs. Multivariate analysis further confirmed these findings, highlighting prolonged operation time and diabetes as significant predictors of PWIs, and antibiotic use as a protective factor. Prolonged operation time and diabetes mellitus significantly increase the risk of PWIs following thoracoscopic radical resection for lung cancer, whereas prophylactic antibiotics play a protective role. These findings underscore the importance of tailored preventive strategies in clinical practice to minimize the occurrence of postoperative infections and improve surgical outcomes in lung cancer patients.
