Abstract:Objective To investigate the risk factors of ventilator-associated pneumonia (VAP) caused by S. maltophilia (Sm) in postoperative cancer patients. Methods The 35 postoperative cancer patients with VAP caused by Sm were enrolled as Sm VAP group from February 2001 to May 2008. At the same time, the control group was defined as 105 patients with non-Sm VAP. Several statistically significant risk factors were screened out by univariate analysis. Then, the independent predictive risk factors were determined by multivariate stepwise logistic regression analysis. Results The APACHEⅡ score, duration of mechanical ventilation and use of broad spectrum antibiotics, level of plasma albumin were significantly different between Sm VAP group and non-Sm VAP group (P0.05). Forward stepwise logistic regression showed that use of broad spectrum antibiotics 7 days was an independent predictive risk factor for Sm VAP.Conclusions APACHEⅡ10,mechanical ventilation 7 days, use of broad spectrum antibiotics 7 days, and level of plasma albumin ≤ 30 g/L are relative risk factors for postoperative cancer patients to develop Sm VAP. Use of broad spectrum antibiotics 7 days is an independent risk factor of Sm VAP in postoperative cancer patients. Actively improving the general conditions of patients, reducing duration of mechanical ventilation and limiting the use of broad spectrum antibiotics are key factors for postoperative cancer patients to prevent Sm VAP.
