Abstract:Objective To identify the risk factors of ventilator-associated pneumonia(VAP) after open-heart surgery in infants and young children with congenital heart diseases(CHD).Methods It was retrospectively analyzed that the infants and young children who were mechanically ventilated ≥ 48h after open-heart surgery from January 2006 to December 2008.A total of 143 patients were included in this study. The clinical data of the patients with VAP were compared with the patients without VAP.Case-control method,single-factor analysis with Chi-square test and multiple logistic regression were performed to determine the risk factors of postoperative ventilator-associated pneumonia in infants and young children with CHD.Results There were 59 episodes of VAP in 143 patients who were mechanically ventilated ≥ 48 hours. The incidence rate of VAP was 41.3%.Single-factor analysis showed that preoperative antibiotics use, duration of cardiopulmonary bypass(CPB),re-intubation,duration of mechanical ventilation, usage of histamine2-receptor blockade or antacid, postoperative low cardiac output syndrome and sepsis were related to the development of VAP(P<0.05).Multiple logistic regression analysis showed that preoperative antibiotics use,re-intubation,duration of mechanical ventilation, usage of histamine2-receptor blockade or antacid were significantly associated with the development of VAP(P<0.05).Conclusion The incidence rate of VAP after open-heart surgery in infants and young children with congenital heart diseases is high.Preoperative antibiotics use,re-intubation,duration of mechanical ventilation, usage of histamine2-receptor blockade or antacid are significantly associated with the development of VAP.A comprehensive strategy of prevention and treatment may be the best way to manage VAP.
