Abstract:The most frequent causes of hemoptysis are infectious diseases, malignant tumors, cardiovascular disorders, and inflammatory diseases. Lobular capillary hemangioma (LCH) has rarely been reported as a cause of hemoptysis. This vascular lesion usually presents on the skin or mucous membranes, and lesion predominantly affects children with a mean age of approximately six years. It typically presents as a solitary glistening red vascular papule or nodule, prone to ulceration and bleeding. The following case illustrates the occurrence of an LCH of the tracheal mucosa treated with interventional bronchoscopy.

Abstract:OBJECTIVE:to highlight the clinical manifestation, histologic characteristics, diagnostic and therapeutic method of tracheal lobular capillary hemangioma (LCH).METHODS:the clinical, auxiliary examinational and pathological data of two patients with tracheal LCH were presented, and relevant literatures were reviewed.RESULTS:LCH is a polypoid form of capillary hemangioma which usually occurs on skin or muscosal surface of oral and nasal, rarely be seen in trachea. The most common presenting symptom of tracheal LCH is recurrent haemoptysis, CT scan can help to make diagnosis. Transbronchoscopic interventional therapy is the most effective treatment for tracheal LCH. Histologic examination can help to make the extreme diagnosis.CONCLUSIONS:tracheal LCH is a scarce benign lesion of tracheal. There isn't any typical clinical manifestation and auxiliary examination. Histologic examination can make a definite diagnosis, and bronchoscopes plays the most effective part in diagnosis and therapy.
Primary Carcinoma Ex Pleomorphic Adenoma Of The Bronchus: Report Of Two Cases And Review Of The Literature

Abstract:Primary carcinoma ex pleomorphic adenoma in the tracheobronchial system is an exceedingly rare neoplasm arising from minor salivary glands. Two cases of this unusual entity were presented. All the lesions were associated with bronchial system, while neither of the patients had a history of a head and neck salivary gland neoplasm. Both cases were followed up for more than 3.5 years. Thus, further data concerning biological behavior and prognosis were made availably. In addition, we provided a review of all cases published to date and summarized the data presently known, for the first time, with the emphasis on the computed tomography (CT) findings. The diagnosis of carcinoma ex pleomorphic adenoma should be considered in elderly patients with slowly progressive symptoms of dyspnea and cough. The continued clinical evaluation and long-term follow-up seems to be of utmost importance for better diagnosis and treatment of primary carcinoma ex pleomorphic adenoma of the bronchus.

Abstract:Background. The aim of this study was to evaluate the correlation of clinical and radiologic characteristics and pathologic features in small pulmonary nodules and to evaluate the success rate and safety of a computed tomography (CT)-guided hook wire system. Methods. One hundred sixty-eight pulmonary nodules that were 20 mm or less in diameter in 161 patients were percutaneously localized by a CT-guided hook wire system, followed by pulmonary resection. The general clinical data, computed tomographic characteristics, and pathologic findings were evaluated, and the major side effects during the localization procedure were reviewed. Results. Of the targeted nodules, 71 (42.3%) nodules were pure ground-glass opacities (GGOs), 59 (35.1%) nodules were partially solid GGOs, and 38 (22.6%) nodules were solid lesions. Pathologically, 49.3% of the pure GGOs were atypical adenomatous hyperplasia (AAH), 54.2% of the partially solid GGOs were lung cancer, and 65.8% of the solid nodules were benign lesions. Lung cancer tended to develop in older patients with larger nodule diameters. Subsolid nodules greater than 10 mm showed a statistically significant correlation with malignancy (p < 0.001). The hook wires were successfully placed in 164 (97.6%) nodules, and no serious complications occurred. Conclusions. In resected specimens with a CT-guided hook wire, pulmonary subsolid nodules tend to be AAH or lung cancer, whereas small solid nodules tend to be benign lesions. Pulmonary subsolid nodules larger than 10 mm are more frequently malignant. Preoperative localization of small pulmonary nodules with a hook wire system has a high success rate and acceptable utility. (C) 2015 by The Society of Thoracic Surgeons
Tracheobronchial Amyloidosis Mimicking Tracheal Tumor

Abstract:Tracheobronchial amyloidosis is a rare presentation and accounts for about 1% of benign tumors in this area. The diagnosis of disease is delayed due to nonspecific pulmonary symptoms. Therapeutic approaches are required to control progressive pulmonary symptoms for most of the patients. Herein, we report a case of a 68-year-old man admitted with progressive dyspnea to our institution for further evaluation and management. He was initially diagnosed with and underwent management for bronchial asthma for two years but had persistent symptoms despite optimal medical therapy. Pulmonary computed tomography scan revealed severe endotracheal stenosis. Bronchoscopy was performed and showed endotracheal mass obstructing 70% of the distal trachea and mimicking a neoplastic lesion. The mass was successfully resected by mechanical resection, argon plasma coagulation (APC), and Nd-YAG laser during rigid bronchoscopy. Biopsy materials showed deposits of amorphous material by hematoxylin and eosin staining and these deposits were selectively stained with Congo Red. Although this is a rare clinical condition, this case indicated that carrying out a bronchoscopy in any patient complaining of atypical bronchial symptoms or with uncontrolled asthma is very important.

Abstract:We present the case of a 60-year-old female patient with no prior history of any systemic disease. She suffered from a prolonged cough that lasted more than 3 months, associated with poor appetite and weight loss of 5 kg. The pathology report of the pre-operative transbronchial needle biopsy was consistent with a neurogenic tumour. Chest computed tomography (CT) revealed a right lower lobe (RLL) mass-like consolidation of 8.67 cm with obstructive pneumonitis and suspicious posterior mediastinal invasion. The tumour was surgically resected with bronchial reconstruction, and the pathological diagnosis was intrabronchial schwannoma located inside the bronchus, a rare tumour that should be included as one of the differential diagnoses of primary bronchial tumours. The possibility of a surgical completed resection should be considered in patients with airway obstruction symptoms.

Abstract:Abstract Objective With advancements in respiratory interventional techniques, bronchoscopic intervention technology has emerged as a viable approach for managing postintubation tracheal stenosis (PITS). However, there was a paucity of research investigating the potential impact of stenosis characteristics and morphology (such as stenosis degree, length, type, and morphology) on bronchoscopic intervention treatment prognosis for PITS patients. This study was to assess the impact of various preoperative stenosis characteristic factors on the bronchoscopic cure rate among patients. Methods This is a retrospective study analyzing the medical records of patients with PITS who received bronchoscopic intervention at the tertiary interventional pulmonology center. Results Among the cases, 115 individuals achieved a in a success rate of 79.86% for bronchoscopic intervention therapy and were assigned to Group S. On the other hand, 29 cases required surgical intervention, accounting for a surgical treatment rate of 20.14% and were assigned to Group F. The stenosis in the Group F predominantly exhibited irregular shapes with scar granulation accompanied by tracheal chondromalacia collapse. Patients in group S experienced fewer total procedures, rigid bronchoscopy treatment, intraoperative hypoxemia, needed emergency re‐bronchoscopy in 24 h and transferred to ICU postoperatively. Patients with pure scar and granuloma, the rate of bronchoscopic success cure was higher than patients with scar granulation accompanied by tracheal chondromalacia (odds ratio: 8.208; 95% confidence interval: 2.755–24.459), and regular stenosis morphology was associated with a higher bronchoscopic success cure rate (odds ratio: 9.463; 95% confidence interval: 3.128–28.623). Conclusion Irregular airway stenosis, chondromalacia or airway collapse are key factors affecting the success rate of bronchoscopic treatment for post‐intubation tracheal stenosis. Abstract:Objective:To investigate the clinical features,diagnosis and surgical treatment of primary malignant tumors of the trachea and main bronchus.Methods:Clinical data of 35 cases with primary malignant tumors of the trachea and main bronchus in Zhongshan Hospital,Fudan University between January 2005 and December 2015 were analyzed retrospectively.Objective:The clinical symptoms were cough,sputum,bloody sputum,hemoptysis,shortness of breath,chest tightness,thoracalgia,and fever.There were 13 cases(37.1％)proved to be misdiagnosed in the early stage.All patients underwent surgical treatment without complications,and 3 patients were received postoperative radiotherapy.Among the 31 cases received postoperative follow-up,26 cases survived,5 cases died.The average disease-free interval was(34±16.2)months(16 to 49 months)and the median survival time was 50 months(16-138 months).Conclusions:Primary malignant tumors of the trachea and main bronchus without special clinical manifestations are easy to be misdiagnosed so that early bronchoscopy and chest CT examination are necessary.Active surgery treatment for primary malignant tumors of the trachea and main bronchus can achieve generate good prognosis.
Bronchus-blocked ultrasound-guided percutaneous transthoracic needle biopsy (BUS-PTNB) for intubated patients with severe lung diseases

Abstract:Background: Examinations based on lung tissue specimen can play a significant role in the diagnosis for critically ill and intubated patients with lung infiltration. However, severe complications including tension pneumothorax and intrabronchial hemorrhage limit the application of needle biopsy. Methods: A refined needle biopsy technique, named bronchus-blocked ultrasound-guided percutaneous transthoracic needle biopsy (BUS-PTNB), was performed on four intubated patients between August 2020 and April 2021. BUS-PTNB was done at bedside, following an EPUBNOW (evaluation, preparation, ultrasound location, bronchus blocking, needle biopsy, observation, and withdrawal of blocker) workflow. Parameters including procedure feasibility, sample acquisition, perioperative conditions, and complications were observed. Tissue specimens were sent to pathological examinations and microbial tests. Results: Adequate specimens were successfully obtained from four patients. Diagnosis and treatment were correspondingly refined based on pathological and microbial tests. Intrabronchial hemorrhage occurred in patient 1 but was stopped by endobronchial blocker. Mild pneumothorax happened in patient 4 due to little air leakage, and closed thoracic drainage was placed. During the procedure, peripheral capillary hemoglobin oxygen saturation (SPO2), blood pressure, and heart rate of patient 4 fluctuated but recovered quickly. Vital signs were stable for patient 1-3. Conclusions: BUS-PTNB provides a promising, practical and feasible method in acquiring tissue specimen for critically ill patients under intratracheal intubation. It may facilitate the pathological diagnosis or other tissue-based tests for intubated patients and improve clinical outcomes.

Abstract:A 34-year-old male presented with lung shadow and was asymptomatic during medical examination. The patient had a prior history of thyroid tumors. Imaging manifestation showed a nodule in the medial segment of the right middle lobe, with partial obstruction of the distal bronchus within the lesion. Ground-glass and inflammatory nodules were observed in the anterior segment of the right upper lobe, as well as chronic inflammatory changes in the lower lobe of the right lung. Lung histopathological examination suggested invasive adenocarcinoma. A morphological examination of the bronchoalveolar lavage fluid revealed the presence of Tropheryma whipplei (TW) and Nocardia . Although TW infection has been reported in cancer patients, co-infection with Nocardia is a unique occurrence in this case. Opportunistic pathogens are common in immunocompromised patients but in this case, the patient was a young adult with normal immunity and an early-stage tumor with TW and Nocardia co-infection. We demonstrated the presence of rare microorganisms through imaging findings, combined with different staining methods of bronchoalveolar lavage fluid and lung tissue sections and evaluation of morphological characteristics. The aim of the present study was to provide early diagnosis and treatment of patients by improving microbial morphological detection.

Abstract:OBJECTIVETo explore the clinical characteristics of primary tracheal tumors and therefore to improve the diagnosis and treatment.METHODSA retrospective analysis of the clinical data of 8 patients with primary tracheal tumors admitted to our department during the period of May 1994 to May 2006 was performed, with detailed description of the clinical manifestations, chest radiography, fiberoptic bronchoscopy, lung function measurements, treatment and prognosis for 4 cases.RESULTSSeven patients presented with irritable cough and progressive inspiratory dyspnea, mostly misdiagnosed as asthma or chronic bronchitis. Examination by fiberoptic bronchoscopy confirmed the diagnosis of tracheal tumor in all the cases. One case with benign neurinoma and 2 cases with adenoid cystic carcinomas had a long-term postoperative remission. Two cases of squamous cell carcinoma with severe tracheal stenosis got rapid symptom remission after intervention of tracheal stent by fiberoptic bronchoscopy and argon plasma coagulation. One patient with adenoid cystic carcinoma refused any treatment. One patient with squamous cell carcinoma lost follow-up after surgery. One patient with adenocarcinoma died of progressive deterioration after 2 month chemotherapy.CONCLUSIONSPrimary tracheal tumors occur infrequently and early clinical symptoms are unspecific. Early diagnosis can be made by chest CT and fiberoptic bronchoscopy. Benign tracheal tumors can be resected, and for some low-grade malignant tumors surgical resection and postoperative radiotherapy can improve long term survival. Intervention with fiberoptic bronchoscopy to unresectable diseases can lead to symptom remission and thus improve patient's life quality.

Abstract:A 46-year-old woman presented with cough for over 20 days, unrelieved by anti-inflammation treatment. Diffuse wall thickening and mildly luminal stenosis of the lower trachea and both main bronchi were observed on chest CT examination. The pathology after bronchoscopy confirmed non-Hodgkin lymphoma (mucosa-associated lymphatic tissue, MALT). FDG PET/CT was performed to evaluate the extent of the disease, which demonstrated the abnormal FDG accumulation in the diffusely thicken wall of trachea and bronchi, without lymph nodes and other extranodal tissue involvement.
Primary localized tracheobronchial amyloidosis presenting with massive hemoptysis: a case report and literature review

Abstract:Primary localized tracheobronchial amyloidosis (TBA) is a rare respiratory tract dysfunction, which is a heterogeneous group of diseases involving abnormal extracellular deposition of amyloid and autologous fibrillar protein material in β-pleated sheets. A 64-year-old man was referred to our hospital because of hemoptysis. Physical examination showed decreased breath sounds in the right lung on auscultation. Chest computed tomography scan displayed multiple nodules with varied size in main bronchia around bilateral hilus of the lung. After admission, bronchoscopy was performed for this patient, and roughness of mucosa in trachea and multiple nodules in respiratory tract were observed. Through further tissue biopsy, the diagnosis of primary TBA was confirmed.

A Case of COPD with Left Endobronchial Hamartoma by Bronchoscopic Intervention

Abstract:Objective: Exploration for chronic obstructive pulmonary disease with branched air lumen hamartoma by bronchoscoptic treatment.Material and Method: An old man was referred to our hospital with shortness of breath for many years.Clinical evidence emphysema on respiratory examination, lung function and chest X-ray gave rise to concern.He was diagnosed as COPD and treated with ICS + LABA.But the shortness of breath is still existed.Initial assessment by CT suggested a left lower lobe collapse.Then bronchoscope identified a solid abnormality in main left bronchus.The pathology showed a benign neoplasm.Because of intolerance of operation, the patient was treated by brochoscopic intervention.The endoscopic intervention included resection by electrosurgical snare, electrocautery, argon plasma coagulation (APC).Result: After intervention, the neoplasm was partly removed and the pathological result was endobrochial hematoma.After treatment, the patient's panting had taken a turn for the better.After one month later, the patient's lung function improved a lot.Conclusion: In conclusion, endobronchial hamartomas are one of benign neoplasms.It can cause persistent bronchial obstruction and recurrent pneumonias.In this case, invasive endoscopic treatment provides an excellent outcome.
A Rare Case of Tracheal Schwannoma Successfully Treated With Endoscopic Resection and Cryoablation Under Rigid Bronchoscopy

Abstract:We present a rare case of tracheal schwannoma, the first reported in Hong Kong, emphasizing the diagnostic challenges and treatment outcomes. A 54-year-old woman with respiratory symptoms underwent evaluations revealing a tracheal mass causing luminal narrowing. Emergency operation with rigid bronchoscopy and cryoablation successfully removed the tumor. Follow-up bronchoscopies showed a gradual reduction in residual tumor size, with no evidence of recurrence after 3.5 years postoperation. Tracheal schwannomas are exceedingly rare, often resulting in delayed diagnosis. Clinicians should maintain a high suspicion of tracheal tumors in patients with unexplained respiratory symptoms. Spirometry and flow volume loop analysis aid in identifying upper airway obstruction. Rigid bronchoscopy is preferred for diagnosis and treatment, ensuring airway stability and obtaining tissue samples. Surgical resection remains the mainstay, but observation after endoscopic resection may be considered. This case highlights the successful management of tracheal schwannoma through endoscopic resection and cryoablation, emphasizing the need for further studies and case reports on this rare entity.

Abstract:Objective To explore the efficacy and safety in treatment of tracheobrochial neoplasms by transbronchoscope high-frequency electrocoagulation.Methods High-frequency electrocoagulation was performed by electrocautery stick or wire through bronchoscope.Results After transbronchoscope high-frequency electrocoagulation,tracheobrochial neoplasms decreased and dyspnea alleviated immediately in all of 16 patients.With the exception of a small amount of hemoptysis in a patient,no other complication occurred.Conclusion High-frequency electrocoagulation is an effective technique in treating tracheobrochial neoplasms.It can rapidly relieve symptom resulted from obstruction of tracheobrochial neoplasms and can avoid thoracotomy in patients with benign tracheobrochial neoplasms.
