Abstract:To the editor: Pulmonary arteriovenous malformation (PAVM) is abnormal communications between the pulmonary arteries and veins, and has been reported to be sporadic or to occur in association with hereditary hemorrhagic telangiectasia (HHT).1 The patients of PAVM may be asymptomatic, present with hypoxemia, paradoxical thromboembolism, or hemoptysis. Ference and coworkers2 reported only 6 patients with hemoptysis in a review of 143 patients with PAVM. A 50-year old man was referred to our hospital with a 10-day history of hemoptysis, productive of 2 to 3 spoons of bloody sputum daily. There was no recent history of fever or thoracic trauma. This patient denied dyspnea on exertion, cyanosis, or family history of HHT. All obtained laboratory results were normal. Chest radiography revealed a mass in the lower right lung. Contrast enhanced computed tomography (CT) demonstrated a 2 cm × 2 cm nodular lesion located in the lower lobe of the right lung (Figure 1A). Spiral CT with 3-D reconstruction indicated a PAVM supplied by a right lower lobe arterial branch with venous reflux (Figure 1B). Bronchoscopy demonstrated clot adherent to the bronchial wall of anterolateral segment of the right lower lobe (Figure 1C).Figure 1.: Chest CT scan shows PAVM located in the right lower lobe (A). Clots are seen in the wall of anterolateral segmental bronchi in right lower lobe by bronchoscopy (C) and the diffused hematoma under the surface of whole right lower lobe (D).After detailed discussion on the choice of treatments, including transcatheter embolization and surgical resection of lesion, the patient preferred the later approach. A right lower lobectomy was performed through a standard lateral thoracotomy and the postoperative course was uneventful. Findings in the operating room included a diffuse hematoma under the surface of the entire right lower lobe (Figure 1D). The mass of the PAVM was palpably pulsatile. The pathological examination confirmed an AVM with hyperplasia and consolidation of arterialized venous vessels. PAVM is rare pulmonary vascular anomalies, and the prevalence is estimated to be 38 per 100 000 individuals.3 The current preferred treatment for the majority of patients with a PAVM is percutaneous embolotherapy, which has largely replaced surgical intervention.4 However, failures of transcatheter embolization for PAVM occur more often in aneurysms because of a high risk of inadvertent coil migration as well as a significant instability of the guiding catheter during coil deployment. The decision of surgical resection of PAVM on this patient was based on the personal choice of patient himself. In conclusion, we reported a case of a PAVM presenting hemoptysis in which spiral CT successfully visualized the abnormal vascular communications between a feeding pulmonary artery and two return pulmonary veins, thus confirming the diagnosis of PAVM. The diffuse hematoma under the surface of entire lower lobe convinced us of the urgency of management in patients with PAVM and hemoptysis.
