Abstract:Abstract Changing health-risk behavior has been shown to decrease morbidity and mortality and enhance quality of life. The present review aims to describe the models and theories that underpin effective interventions and the empirical studies that warrant their successful use with specific health risk-behaviors. Motivational, behavioral enactment and multi-stage models are critically discussed in the context of identifying the ingredients that help translate theories into practice by designing effective behavior change interventions. Future research directions are outlined for continuing the development of a theory and evidence based practice in health psychology and its integration with evidence-based theory and practice of cognitive-behavioral psychotherapies, as both are focused on behavioral change. Key words: health-risk behavior, models of change, effectiveness, evidencebased interventions. INTRODUCTION Human behavior plays a central role in the maintenance of health and the prevention of disease. Health-risk behavior can be defined as any activity undertaken by people with a frequency or intensity that increases risk of disease or injury (Steptoe & Wardle, 2004). The health risk behaviors might cluster together into a risky lifestyle. Much of the mortality and morbidity is caused by individual behavioral patterns, polluted environment or poverty. Statistics show that half of the premature death from the 10 leading causes in developed countries is caused by preventable factors, such as: tobacco use, alcohol abuse, physical inactivity, unhealthy dietary habits, risk sexual practices, non-adherence to effective medication regimens and to screening programs (Gray, 1993). Health risk behaviors also influence cognitive performance, emotions, and the overall quality of life (Hawkins & Anderson, 1996). Although epidemiologic data on the relationships between these behaviors and various health outcomes were available in the early 1980s, many refinements in knowledge have occurred since then. Causal conclusions have been strengthened by more sophisticated research designs, and program implementations. The impact of these risk behaviors on health is of such magnitude that it has become one of the priorities of the most important national and international health organizations (Rutter & Quine, 2004). To advance in the field of risk behaviors change, the Behavior Change Consortium (BCC) was created as a collaborative network of institutions and specialists. BCC reviews currently informs about links between health and behavior, about the influence of the social environment on these behaviors, and about interventions to improve quality of life through modifying behavior. It also addresses what must still be learned in order to answer questions and to discover and share what works, and what does not regarding health and behavior (Prochaska, 2005). There are some important issues regarding the field. First, there is an overlapping of constructs among social cognition models of health and illness. Bandura (2000) criticizes the way the proliferation of models of health behavior determines "cafeteria style research" where a mixture of theoretical concepts are used and unnecessarily multiply predictors in the name of theoretical integration. For instance, the self-efficacy concept from Social Cognitive Theory (SCT) overlaps with perceived behavioral control from the Theory of Reasoned Action and the Theory of Planned Behavior (see below) or the barriers concept from SCT overlaps with the barriers concept from Health Belief Model. More studies are needed in order to provide evidence for discriminant validity among these constructs, and their integration with already validated theories of cognitivebehavioral practice in the clinical field (David, Miclea, & Opre, 2004), as both target health promotion and/or behavioral treatment. Second, interventions in the clinical field can be considered theory and evidence-based when (1) the theory that is used for data interpretation fulfills two criteria: (a) it provides a clear account of the hypothesized underlying psychological mechanisms that generate behavior change following the intervention; (b) the theory has been empirically tested before being used with behavior modification interventions (Michie & Abraham, 2004); researchers argue for the use of evidence obtained with specific populations and particular circumstances in order to build and improve behavior change theory, and (2) research in health psychology includes randomized control trials (RTC), elaboration and process evaluations in order to measure the impact of interventions and help identify the strategies that work in health behavior change. …

Abstract:Modelling the automatic and deliberative processes that interact and underlie common-sense perceptions and behaviours for managing health and illness requires prototypes at multiple levels; specific illnesses and treatments at the 'surface' and generic acute, episodic and chronic prototypes at deeper levels. Models need to represent how multi-level processing can generate inconsistent as well as consistent expectations regarding successful management and how the limited visibility of process affects communication and symptom labelling by practitioners.

Abstract:ABSTRACT Human and planetary health goals have coalesced within consumer food choices. Food‐based dietary guidelines provide expert recommendations on what to eat, however, adherence is low and few include sustainability recommendations. This systematic review sought to extract consumer behavioural findings specifically related to ‘healthy sustainable diets’ to gain insights into the levers of food‐related behavioural change. Data analysed from 57 studies reveal that while 27 discrete behavioural theories underpinned conceptualisation of research, just three dominate this literature: Theory of Planned Behaviour, Transtheoretical Model and Social Practice Theory. Correspondingly, studies were mainly concerned with motivation, attitudes, intentions and practices. Promising research directions are emerging from investigations into the roles of emotions, meat attachment and types of knowledge. Those sustainable healthy food behaviours (SuHeFB) most investigated were meat consumption reduction, plant‐based food consumption, and alternative‐to‐animal protein food product acceptance. In addressing the perplexity surrounding consumer inertia in altering dietary habits, this review provides a comprehensive SuHeFB construct typology and a justification for directing research attention towards the phenomenon of amotivation. In addition, 31 future research questions are posited under six related SuHeFB themes.

Public engagement with air quality data: using health behaviour change theory to support exposure-minimising behaviours

Abstract:Exposure to air pollution prematurely kills 7 million people globally every year. Policy measures designed to reduce emissions of pollutants, improve ambient air and consequently reduce health impacts, can be effective, but are generally slow to generate change. Individual actions can therefore supplement policy measures and more immediately reduce people's exposure to air pollution. Air quality indices (AQI) are used globally (though not universally) to translate complex air quality data into a single unitless metric, which can be paired with advice to encourage behaviour change. Here we explore, with reference to health behaviour theories, why these are frequently insufficient to instigate individual change. We examine the health behaviour theoretical steps linking air quality data with reduced air pollution exposure and (consequently) improved public health, arguing that a combination of more 'personalised' air quality data and greater public engagement with these data will together better support individual action. Based on this, we present a novel framework, which, when used to shape air quality interventions, has the potential to yield more effective and sustainable interventions to reduce individual exposures and thus reduce the global public health burden of air pollution.

I do that in three ways. First, I make a few comments on the general idea that there are theories of health competing in the field of philosophy of medicine; second, I offer specific comments on the phenomenological approach; and finally, I comment on Barnes' claim that health is messy. I do not provide an overview of the book but a response to some of its themes.

Abstract:Interventions to change health-related behaviours typically have modest effects and may be more effective if grounded in appropriate theory. Most theories applied to public health interventions tend to emphasise individual capabilities and motivation, with limited reference to context and social factors. Intervention effectiveness may be increased by drawing on a wider range of theories incorporating social, cultural and economic factors that influence behaviour. The primary aim of this paper is to identify theories of behaviour and behaviour change of potential relevance to public health interventions across four scientific disciplines: psychology, sociology, anthropology and economics. We report in detail the methodology of our scoping review used to identify these theories including which involved a systematic search of electronic databases, consultation with a multidisciplinary advisory group, web searching, searching of reference lists and hand searching of key behavioural science journals. Of secondary interest we developed a list of agreed criteria for judging the quality of the theories. We identified 82 theories and 9 criteria for assessing theory quality. The potential relevance of this wide-ranging number of theories to public health interventions and the ease and usefulness of evaluating the theories in terms of the quality criteria are however yet to be determined.

Abstract:Devices and programs using digital technology to foster or support behavior change (digital interventions) are increasingly ubiquitous, being adopted for use in patient diagnosis and treatment, self-management of chronic diseases, and in primary prevention. They have been heralded as potentially revolutionizing the ways in which individuals can monitor and improve their health behaviors and health care by improving outcomes, reducing costs, and improving the patient experience. However, we are still mainly in the age of promise rather than delivery. Developing and evaluating these digital interventions presents new challenges and new versions of old challenges that require use of improved and perhaps entirely new methods for research and evaluation. This article discusses these challenges and provides recommendations aimed at accelerating the rate of progress in digital behavior intervention research and practice. Areas addressed include intervention development in a rapidly changing technological landscape, promoting user engagement, advancing the underpinning science and theory, evaluating effectiveness and cost-effectiveness, and addressing issues of regulatory, ethical, and information governance. This article is the result of a two-day international workshop on how to create, evaluate, and implement effective digital interventions in relation to health behaviors. It was held in London in September 2015 and was supported by the United Kingdom's Medical Research Council (MRC), the National Institute for Health Research (NIHR), the Methodology Research Programme (PI Susan Michie), and the Robert Wood Johnson Foundation of the United States (PI Kevin Patrick). Important recommendations to manage the rapid pace of change include considering using emerging techniques from data science, machine learning, and Bayesian approaches and learning from other disciplines including computer science and engineering. With regard to assessing and promoting engagement, a key conclusion was that sustained engagement is not always required and that for each intervention it is useful to establish what constitutes "effective engagement," that is, sufficient engagement to achieve the intended outcomes. The potential of digital interventions for testing and advancing theories of behavior change by generating ecologically valid, real-time objective data was recognized. Evaluations should include all phases of the development cycle, designed for generalizability, and consider new experimental designs to make the best use of rich data streams. Future health economics analyses need to recognize and model the complex and potentially far-reaching costs and benefits of digital interventions. In terms of governance, developers of digital behavior interventions should comply with existing regulatory frameworks, but with consideration for emerging standards around information governance, ethics, and interoperability.

The application of psychological theory to nutrition behaviour change

Abstract:Changing individuals' health behaviour seems to be the key to solving many of the world's health problems. Although there is a multitude of potential influences, many interventions to improve health seek to change intrinsic psychological determinants of health behaviour. To date, most attention has been paid to cognitions, such as attitudes and beliefs, and a number of social cognition models (SCM) are in current use. SCM all describe cognitions as determinants of behaviour, thereby implying that changes in cognitions will lead to changes in behaviour. Although SCM are widely used to predict a range of health behaviours, they are associated with a number of important limitations, including poor levels of predictive power, particularly in relation to eating behaviour, and limited guidance about the operationalisation of theoretical constructs. These limitations may explain why very few interventions to change behaviour are explicitly theory-based, despite the widely-held view that having a clear theoretical underpinning will improve effectiveness. Ultimately, advances in understanding and changing health behaviour will come about only if psychological theory and practice are integrated. The recently-published taxonomy of behaviour-change techniques used in interventions is a good example of integrated research, but more work of this type is essential and will require respectful collaboration between researchers and practitioners working from a range of different disciplines such as health psychology, public health nutrition and health promotion.

Abstract:This paper starts by briefly reviewing the history, theory and practice of the settings approach to promoting public health--highlighting its ecological perspective, its understanding of settings as dynamic open systems and its primary focus on whole system organization development and change. It goes on to outline perceived benefits and consider why, almost 20 years after the Ottawa Charter advocated the approach, there remains a relatively poorly developed evidence base of effectiveness. Identifying three key challenges--relating to the construction of the evidence base for health promotion, the diversity of conceptual understandings and real-life practice and the complexity of evaluating ecological whole system approaches--it suggests that these have resulted in an ongoing tendency to evaluate only discrete projects in settings, thus failing to capture the 'added value' of whole system working. It concludes by exploring the potential value of theory-based evaluation and identifying key issues that will need to be addressed in moving forward--funding evaluation within and across settings; ensuring links between evidence, policy and practice; and clarifying and articulating the theories that underpin the settings approach generically and inform the approach as applied within particular settings.

Abstract:Despite rapid theoretical expansion in conceptualising individual and environmental processes, the examination of social processes associated with health behaviours has a less cohesive theoretical landscape. The purpose of this mapping review and content analysis was to develop a taxonomy of social dimensions applicable to health behaviours. Michie et al (2014) 'ABC of Theories of Behaviour Change' text, which includes 83 behaviour change theories, was used as the data-set, whereby an iterative concurrent content analysis was undertaken with respect to all relational/interpersonal psychological dimensions. The analysis resulted in a social dimensions of health behaviour (SDHB) framework of 10 dimensions, including seven sub-types of social appraisal dimensions and three-sub-types of social identification dimensions. The SDHB revealed that specific dimensions, such as descriptive norm, are prevalent in behavioural theories, while other dimensions have seen less attention. Further, while most social constructs in behavioural theories are represented by only one social dimension in the SDHB, other constructs have complex representation. This version 1.0 of the SDHB framework should assist in specifying the core social dimensions in health behaviour, provide a common lexicon to discuss relational constructs in psychological theories, amalgamate the disparate social constructs literature and identify opportunities for further research to advance theory development and interventions.

Abstract:Online health behaviour interventions have great potential but their effectiveness may be hindered by a lack of formative and theoretical work. This paper describes the process of formative research to develop theoretically and empirically based health messages that are culturally relevant and can be used in an online intervention to promote healthy lifestyle behaviours among new university students. Drawing on the Theory of Planned Behaviour, a three-phase programme of formative research was conducted with prospective and current undergraduate students to identify (i) modal salient beliefs (the most commonly held beliefs) about fruit and vegetable intake, physical activity, binge drinking and smoking, (ii) which beliefs predicted intentions/behaviour and (iii) reasons underlying each of the beliefs that could be targeted in health messages. Phase 1, conducted with 96 pre-university college students, elicited 56 beliefs about the behaviours. Phase 2, conducted with 3026 incoming university students, identified 32 of these beliefs that predicted intentions/behaviour. Phase 3, conducted with 627 current university students, elicited 102 reasons underlying the 32 beliefs to be used to construct health messages to bolster or challenge these beliefs. The three-phase programme of formative research provides researchers with an example of how to develop health messages with a strong theoretical- and empirical base for use in health behaviour change interventions.

Abstract:The term 'environment' is not uniformly defined in the public health sciences, which causes crucial inconsistencies in research, health policy, and practice. As we shall indicate, this is somewhat entangled with diverging pathogenic and salutogenic perspectives (research and policy priorities) concerning environmental health. We emphasise two distinct concepts of environment in use by the World Health Organisation. One significant way these concepts differ concerns whether the social environment is included. Divergence on this matter has profound consequences for the understanding of health and disease, for measures derived from that understanding targeting health promotion and disease prevention, and consequently, for epistemic structures and concept development in scientific practice. We hope to improve the given situation in public health by uncovering these differences and by developing a fruitful way of thinking about environment. Firstly, we side with the salutogenic conception of environment as a health resource (as well as a source of health risks). Secondly, we subdivide the concept of environment into four health-oriented environmental categories (viz., natural, built-material, socio-cultural, and psychosocial) and we link these with other theoretical notions proposed in the health sciences literature. Thirdly, we propose that in public health 'environment' should be understood as consisting of all extrinsic factors that influence or are influenced by the health, well-being, and development of an individual. Consequently, none of the four categories should be excluded from the concept of environment. We point out the practical relevance and fruitfulness of the conception of environment as a health source and frame this in causal terms, representing individual health environments as causal networks. Throughout, we side with the view that for the design of human health-promoting settings, increased attention and consideration of environmental resources of salutogenic potential is particularly pressing.

Abstract:Non-communicable diseases, such as hypertension, diabetes, or depression, result from an interplay of physiological, genetic, behavioral, and environmental aspects. Together with climate change, they are arguably among the most significant challenges mankind faces in the 21st century. Additionally, the bidirectional influences of climate change and health on each other are undisputed. Behavioral changes could curb both climate change and the spread of non-communicable diseases. Much effort has been put into information campaigns in both fields, but success has been limited. In the following, the knowledge action gap is compared and analyzed in healthy and climate-friendly behavior from a practical point of view and the supporting theoretical models are highlighted. The analysis shows that self-efficacy plays an essential role in both areas of research for effecting behavioral changes. The models of ‘Planned Behavior’ and ‘Stages of Change’ seems helpful and can be applied and adapted to explain behavioral changes in health and climate changes settings. We compared two previously unrelated research fields to uncover new avenues for further study and stimulate fruitful transdisciplinary discussion. Future directions on how behavioral medicine and climate change research can learn from each other are discussed.

Abstract:Protection motivation theory is a pre-eminent health behaviour theory purposed to predict participation in health protection and risk behaviours. It has been widely applied across multiple behaviours, populations and contexts. In this conceptual review, we summarise research applying the theory and identify shortcomings and evidence gaps that limit reported inferences and impede theory and intervention development. Accordingly, we provide recommendations for best practices and suggestions for future research to resolve these limitations. Limitations identified include a dearth of comprehensive theory tests, sparse evidence of theory sufficiency, a lack of studies including additional constructs, overuse of correlational and cross-sectional research designs, a paucity of intervention studies and tests of theory-consistent mechanisms of action, few tests intrapersonal and environmental moderators of theory effects and measurement concerns. We provide recommendations to address these limitations including conducting comprehensive theory tests in support of nomological validity; incorporating past behaviour and other constructs to establish theory sufficiency and extend its scope; adopting cross-lagged panel and factorial experimental research designs to test directional effects, permit better causal inference and test mechanisms of action; testing effects of moderators to identify conditions that may affect theory applicability and developing measurement standards for study constructs and adopting non-self-report behaviour measures.

Abstract:The benefits of exposure to nature for health and wellbeing have been demonstrated across multiple disciplines. Recent work has sought to establish one “dose” or type of nature exposure that is universally beneficial, which has proven difficult. We use the principles of psychopharmacology to look beyond the use of dose as a concept for prescribing nature. Instead, we posit a multidimensional model of bioavailability of nature to shift the focus beyond universal effects, and instead consider the relationship between health and nature as dynamic, changeable, and heavily contextual. We propose that the bioavailability of nature interactions is constructed through understanding route of administration, dose, and concentration. By delineating the mechanisms of health benefit derived from the type of behavioural interaction (through being, doing, living), the route of administration of nature interactions may be highly variable not just between, but also within, individuals. We propose concentration as being a meeting between the subjective aspects of the individual and the subjective qualities of the nature at that specific time and place. We use a “green equation”, for mapping the processes and pathways that belie the interaction between the person and their environment. Here, the nature/health association as a dynamic interaction, and we operationalise this within a multidimensional construct of bioavailability. We provide an overview of this testable model, and summarise with preliminary evidence as well as a research agenda for the future.

Abstract:This review provides a critical overview of current evidence on psychological health behavior determinants and its value in informing intervention and future determinants research. The review begins with work labeling and classifying the myriads of determinants available in the extant research to arrive at core groups of determinants. Next, the conceptual bases of these determinant groups are identified, and the weight of the evidence for their purported effects on health behavior, including belief-based determinants (e.g., outcome expectancies, capacity beliefs), determinants representing self-regulatory capacity (e.g., planning, action control) and nonconscious processes (e.g., habit, implicit cognition), and dispositional determinants (e.g., personality, regulatory control), is critically evaluated. The review also focuses on the theory-based mechanisms underpinning determinant effects and moderating conditions that magnify or diminish them. Finally, the review recommends a shift away from research on determinants as correlates, outlines how determinants can inform intervention development and mechanisms of action tests, suggests alternatives to predominant individualist approaches, and proposes future research directions.

Abstract:Background: It is challenging to predict long-term outcomes of interventions without understanding how they work. Health economic models of public health interventions often do not incorporate the many determinants of individual and population behaviours that influence long term effectiveness. The aim of this paper is to draw on psychology, sociology, behavioural economics, complexity science and health economics to: (a) develop a toolbox of methods for incorporating the influences on behaviour into public health economic models (PHEM-B); and (b) set out a research agenda for health economic modellers and behavioural/ social scientists to further advance methods to better inform public health policy decisions. Methods: A core multidisciplinary group developed a preliminary toolbox from a published review of the literature and tested this conceptually using a case study of a diabetes prevention simulation. The core group was augmented by a much wider group that covered a broader range of multidisciplinary expertise. We used a consensus method to gain agreement of the PHEM-B toolbox. This included a one-day workshop and subsequent reviews of the toolbox. Results: The PHEM-B toolbox sets out 12 methods which can be used in different combinations to incorporate influences on behaviours into public health economic models: collaborations between modellers and behavioural scientists, literature reviewing, application of the Behaviour Change Intervention Ontology, systems mapping, agent-based modelling, differential equation modelling, social network analysis, geographical information systems, discrete event simulation, theory-informed statistical and econometric analyses, expert elicitation, and qualitative research/process tracing. For each method, we provide a description with key references, an expert consensus on the circumstances when they could be used, and the resources required. Conclusions: This is the first attempt to rigorously and coherently propose methods to incorporate the influences on behaviour into health economic models of public health interventions. It may not always be feasible or necessary to model the influences on behaviour explicitly, but it is essential to develop an understanding of the key influences. Changing behaviour and maintaining that behaviour change could have different influences; thus, there could be benefits in modelling these separately. Future research is needed to develop, collaboratively with behavioural scientists, a suite of more robust health economic models of health-related behaviours, reported transparently, including coding, which would allow model reuse and adaptation.

Abstract:With its unique position, primary health care (PHC) can provide health promotion and prevention services, including lifestyle behavioural counselling. Unhealthy lifestyle behaviours are very prevalent among patients attending PHC, with many patients unwilling to change or in the precontemplation stage. While patients in the contemplation stage are better managed using the 5As approach of motivational interviewing counselling, those unwilling or not ready for change necessitate a different approach, such as the 5Rs of motivational interviewing (MI) counselling. The 5Rs MI approach holds promise in motivating unwilling individuals to consider embarking on the journey of behavioural change. The 5Rs approach is not a stand-alone checklist of tasks implemented in isolation but is best integrated within a theoretical behavioural change framework. Of the four health-related behavioural change theoretical frameworks that are frequently used, the transtheoretical stages of the change model are the most used. This continued professional development article provides a summary review of the literature on behavioural change theories as they apply to lifestyle health behaviour change and presents the 5Rs approach as a feasible and practical approach to manage patients who are unwilling to change or in the precontemplation stage. This offers a beacon of hope for improved patient outcomes in a PHC system saddled with high prevalence of modifiable unhealthy lifestyle behaviours.
Health science knowledge translation: Critical appraisal of online physical activity promotion material

Abstract:Limited research exists on how often health behavior theory concepts, principles, and techniques are embedded in the design of material to promote physical activity and exercise, such as persuasive communications disseminated via the Internet. This cross-sectional study examined 139 web articles on physical activity and exercise. We assessed how often article messages targeted recommended motivational determinants of health behavior: that is, outcome expectancy, self-efficacy, and self-regulation. Moreover, exploratory analyses were performed, which compared patterns in article messages by organization type. Results showed the largest categories were other messages and self-efficacy, each representing nearly one-third of article messages. The frequency that article messages targeted motivational determinants of physical activity and exercise differed by organization type. Our results suggest web articles that could promote physical activity and exercise motivation exist; however, they can be better constructed to meet the needs of people. For example, over half of article messages targeted a recommended motivational determinant, but the least targeted determinant was self-regulation. We present steps nurse educators and clinicians may take to improve the design or selection of physical activity and exercise promotion material.

Abstract:BackgroundHealth behaviors exhibit diverse characteristics and types, leading to health behavior theories with varied focuses. However, an integrated framework elucidating the intrinsic mechanisms of different behavior types is notably absent.PurposeThis study aimed to establish such a framework offering a comprehensive basis for explanations and behavior interventions.MethodsA systematic review and meta-synthesis of qualitative research concerning health behavior change and maintenance were conducted.ResultsA total of 556 records were identified and screened, with 170 meeting the inclusion criteria. Thematic synthesis, a form of meta-synthesis, generated 23 themes, with the qualitative relationships between them presented in a conceptual model, forming an integrated framework.DiscussionThis framework provides a dynamic and hierarchical platform that enables the interpretation and analysis of various health behaviors within a unified structure. Furthermore, the recognition of the dynamicity of constructs and the multiplicity of behavior routes offers certain implications for intervention design, including behavior setting guidance, immediate benefits provision, and behavior interactions utilization.Translation to Health Education PracticeThe study provides an integrated framework for health educators/promoters. It offers a comprehensive view of health behavior change mechanisms from multiple perspectives and provides insights for the design, implementation and evaluation of behavior intervention practices.
