Abstract:Background The relative rarity and anatomical position of retrorectal tumors may lead to difficulty in diagnosis and surgical management. Methods This was a retrospective review of 62 patients who had resection of retrorectal tumors between 2002 and 2010. Results All patients in this study were treated by excision of the retrorectal tumors. Surgical approach included transsacral approach (52 cases), transabdominal approach (eight cases), and combined approach (two cases). A total of 48 benign lesions (77.4%) and 14 malignant lesions (22.6%) were confirmed by histological examination. The 48 benign cases included dermoid cysts (17 cases), simple cysts (eight cases), teratomas (eight cases), neurofibromas (eight cases), fibrolipomas (four cases), neurilemmomas (two cases), and synovioma (one case). The 14 malignant cases included lymphomas (four cases), malignant teratomas (three cases), fibrosarcomas (two cases), interstitialomas (four cases) and malignant mesothelioma (one case). Complications occurred in 14.5% of patients and included intraoperative bleeding (three cases), rectal injury (three cases), and presacral infection (three cases). Conclusion Primary retrorectal tumors are very rare. Successful treatment of these tumors requires extensive knowledge of pelvic anatomy and expertise in pelvic surgery.
Application and evaluation of laparoscopy in the treatment of primary retroperitoneal tumor

Abstract:The occurrence of primary retroperitoneal tumors is rare.Laparoscopic resection of retroperitoneal tumors has been described in recent years.Both transperitoneal and retroperitoneal approaches can be used in laparoscopic resection for retroperitoneal tumors.For an experienced surgeon who is familiar with laparoscopic skills and anatomy of the retroperitoneal spaces,laparoscopic resection of retroperitoneal tumors can be performed safely in selected patients.

Abstract:Objective To study the feasibility of laparoscopic resection of colorectal cancer.Methods A(retrospectively) analysis of the clinical data of 110 patients with colorectal cancer treated by laparoscopic(resection) during last three years was made.Of them,24 cases were converted to open operation.In 86 (cases),laparoscopic resection of colorectal cancer was completed.Among them,5 cases underwent right(hemicolectomy),2 cases left hemicolectomy,10 cases sigmoidectomy,22 cases Dixon′s operation,46 cases of Miles operation,and 1 case total colorectomy.Results No patient died within 30 days postoperatively in this series.The average operation time was 225(120-360) min with average 135(20-400) mL blood loss.The average number of lymph nodes excised was 8.7(1-30).The average number of positive lymph nodes was 2.2(0-24).Six cases had intraoperative complications,including 1 case of ureteral injury and 5 cases of intra-abdominal bleeding.These patients were immediately converted to open surgery.There were 6 cases with postoperative complications,among them,2 cases of urinary leakage,2 cases of massive bleeding and 2 cases of intestinal obstruction.All of them recovered after reoperation.The time of bowel funtion(recovering) was 12-72h after operation.The hospital stay after operation was 8.6(7-15)days.The median follow-up was 14.3 months(range 1-33) for 100(90.9%) patients.There was no port-site tumor(metastasis) and no tumor recurrence at the small abdominal incision.Six to fifteen months after opteration,3 cases had diffase peritoneal metastases.1 case of Miles resection had perineal metastasis 3 months after(surgery).Conclusions Laparoscopic resection of colorectal cancer is technically feasible,and have(advantages),such as less surgical trauma,less bleeding,less gastrointestinal interference and quicker(recovery).Laparoscopic radical operation for colorectal cancer can meet the requirements of safety and radical operation.

Abstract:Objective: To evaluate the feasibility and outcomes of laparoscopic surgery for primary retroperitoneal tumors (PRTs). Methods: Between January 2007 and July 2009, laparoscopic resection was attempted of PRT in 14 patients. The histological type of the tumor, operating characteristics, and outcomes were analyzed. Results: In the 14 patients (4 male), the median age was 44 years, with a median tumor diameter of 7.6 cm. Laparoscopic resection was successfully performed in 13 patients with a solely laparoscopic approach (n = 11) or combined with a hand-assistant procedure (n = 2). One case (epithelioid hemangioma) was converted to laparotomy. The median operating time for laparoscopy was 139 minutes with an estimated average blood loss of 59.2 mL. There were no transfusions or major complications. After a median follow-up of 17 months, all patients remained disease free. Conclusions: Laparoscopic surgery for the resection of PRT is feasible and effective when performed by an experienced surgeon.

Abstract:Background: Surgical approaches for retrorectal tumors (RRT) are either abdominal, dorsal trans-sacrococcygeal, or perineal. Very few cases have been reported so far concerning a laparoscopic approach. The aim of this study was to assess the results of laparoscopy for the treatment of RRT. Methods: All patients who underwent laparoscopy for RRT between 2003 and 2012 were reviewed. Data included patient and tumor characteristics, surgical morbidity, and mortality. Results: A total of 12 consecutive women with a median age of 55 years underwent laparoscopy for RRT. Median operative time was 145 min (range 70-215). Two conversions in laparotomy occurred, and one patient needed a temporary diverting ileostomy for rectal injury. Postoperatively, two patients presented urinary tract infections. Median length of stay was 8 days (range 4-16). Pathological examinations showed complete resections (R0) for ten benign tumors and one malignant tumor (Ewing sarcoma). One lesion was incompletely resected, a colloid sarcoma (R1 status), and was re-operated on by laparotomy after neoadjuvant chemoradiation. No local recurrence was observed after a median follow-up of 34 months (range 12-79) for benign lesions, and 28 and 71 months for the two patients who underwent resection of Ewing and colloid sarcoma, respectively. Conclusions: Laparoscopic resection for RRT seems feasible and safe. It allows complete excision of tumors located in the retrorectal space with low morbidity. Thus, this approach can be a valid alternative to standard Kraske or open abdominal approaches for the treatment of RRT.

Abstract:Objective To evaluate the safety and advantage of laparoscopic resection for retrorectal tumors. MethodsMedical records of 8 patients with retrorectal tumors were reviewed retrospectively.The patients were diagnosed with MRI and received laparoscopic resection in our hospital from October 2005 to June 2010.During the operation,pneumoperitoneum was established with a 10-cm cannula through the inferior umbilical fold.At the level of the sacral promontory,the retroperitoneum was opened to expose the perirectal space so that to separate and remove the tumor. Results In the 8 cases,the procedure was completed under laparoscope in 6 patients within 90 to 300 minutes(mean,211.2 minutes).During the operation,a mean of 121.2 ml blood loss was measured(20-200 ml).After the procedure,pelvic cavity drainage was maintained for 0 to 6 days(mean,2 days).The mean hospital stay of the patients was 4 days(2-8 days).None of them had intestinal injury,hemorrhage,pelvic infection or urine retention.The other two cases were converted to open surgery because of adhesion between levator ani and ureter.The 6 patients who underwent laparoscopy were followed up for 12 to 58 months(median,17.5 months),none of them showed recurrence. Conclusion Laparoscopic resection is safe and less traumatic for retrorectal tumors.

Laparoscopic Diagnosis and Treatment for Small Intestinal Stromal Tumors

Abstract:Objective To evaluate the laparoscopic management in diagnosing and treating small intestinal stromal tumors.MethodsSeventeen patients with small intestinal stromal tumors,who underwent laparoscopic diagnosis and surgical procedures,were retrospectively analyzed with the size of tumor,operative time,blood loss,length of incision,time for passage of flatus,post-operative hospital stay,operative complications and result of follow-up.Results All the 17 patients were diagnosed and treated by laparoscopic partial intestinal resections.The mean diameter of tumors was(3.6±1.3)cm,operative time(62.1±25.7)min,blood loss(17.1±15.2)mL,length of incision(3.4±1.1)cm,time for passage of flatus(2.2±1.2)d and post-operative hospital stay(7.8±2.0)d.As for the complications,one(5.9%)anastomosis bleeding was found and was treated by non-surgical procedure.After follow-up for 2-34 months,no recurrent tumor was found.Conclusion Laparoscopic exploration is useful to diagnose small intestinal stromal tumor accurately,and laparoscopic partial intestinal resection is a safe,effective and less invasive procedure for small intestinal stromal tumors.
Clinical Evaluation of Laparoscopic Radical Resection of Colon Cancer

Abstract:OBJECTIVE:To investigate the clinical effect of laparoscopic radical resection of colon cancer.METHODS:Patients with colon cancer who underwent radical resection during January 2000 to January 2004 in Sir Run Run Shaw Hospital (Medical College of Zhejiang University) were divided into open and laparoscopic groups for a non-randomized case-control study. All the patients were followed up and their clinical and pathological results were compared.RESULTS:No difference was seen between the two groups on the patients' age, gender, lymph node numbers and Dukes staging. The laparoscopic group had a longer mean operation time [(182 +/- 62) min vs (141 +/- 37) min] and shorter hospital stay [(5.3 +/- 1.9) d vs (8.2 +/- 1.2) d] than the open group. All surgical margins were pathological negative. Post-operation follow up was 12-18 months (mean 21 months). No incision recurrence was found in neither group. As far the local recurrence and distal metastasis rate were similar in both groups.CONCLUSIONS:Laparoscopic radical resection of colon cancer has minimal invasion and shorter recuperation than open procedure, the radicalness and either local recurrence or distal metastasis rate after surgery are similar when compared with open group.
Laparoscopic versus open surgery for rectal neuroendocrine tumors: a multicenter real-world study

Abstract:Background: Owing to the lack of evidence-based medical studies with large sample sizes, the surgical approach for the radical resection of rectal neuroendocrine tumors remains controversial. Methods: We retrospectively collected the medical records of patients with rectal neuroendocrine tumors who underwent radical resection at 17 large tertiary care hospitals in China between January 1, 2010, and April 30, 2022. All patients were divided into laparoscopic and open surgery groups. After propensity score matching to reduce confounders, the postoperative and oncologic outcomes were compared between the groups. Results: We enrolled 174 patients with rectal neuroendocrine tumors who underwent radical surgery. After random matching, 124 patients were included in the comparison (62, laparoscopic surgery group; 62, open surgery group). The laparoscopic surgery group had fewer complications (14.5% vs. 35.5%, P = 0.007) and superior relapse-free survival (P = 0.048). Subgroup analysis revealed that the laparoscopic surgery group had fewer complications (10.9% vs. 34.7%, P = 0.004), shorter postoperative hospital stays (9.56 ± 5.21 days vs. 12.31 ± 8.61 days, P = 0.049) and superior relapse-free survival (P = 0.025) in the rectal neuroendocrine tumors ≤ 4 cm subgroup. Conclusions: Laparoscopic surgery was associated with improved postoperative outcomes and oncologic prognosis for patients with rectal neuroendocrine tumors ≤ 4 cm; it can serve as a safe and feasible option for radical surgery of rectal neuroendocrine tumors.
Diagnosis and surgical treatment of primary retroperitoneal tumors: report of 63 cases

Abstract:Objective To evaluate the experience in the diagnosis and surgical treatment of primary retroperitoneal tumor (PRT). Methods Clinical data of PRT 63 cases from January 1990 to March 2007 confirmed by pathology were retrospectively analyzed including clinical manifestation, surgical procedures, pathological examination and follow-up. Results Tumors were benign in 25 patients and malignant in 38. The main symptoms and signs were abdominal mass. The Youdon's index of CT examination for distinguishing benign from malignant tumors was 85%. The complete surgical resection rate was 88% for benign tumors and 68% for malignances. Removal of the involved organs or vessels was needed in 40% patients in complete surgical resection group. Postoperative recurrence was high in those the tumor was not completely removed and those the tumor was malignant. The 5-year survival rate for complete surgical resection of benign tumors was 83.6% ,and that of malignant tumors was 27.3%. After a median follow-up of 36 months (range 5 ～ 168)for the resection of malignant tumors, the local recurrence rate was 53%. Median time between initial surgery and recurrence was 25 months (range 3 ～ 108). For local recurrence of malignant tumor, the complete surgical resection rate was 62.5%. One patient died of intraabdominal hemorrhage in the first day after operation. Conclusion For PRT patients, early diagnosis is often difficult. Preoperative imaging results are essential to predict the surgical resectability. The optimal treatment of patients with PRT is radical resection, an bloc organ resection if necessary. Regularly postoperative follow-up is mandatory for early finding recurrence.

Abstract:Objective To explore the experience in treatment of primary retroperitoneal tumors (PRPT).Methods The clinical data of 62 cases with primary relroperitoneal tumors from 2001 to 2007were studied retrospectively.Results The abdominal mass was the main symptom in PRPT.There were 29 cases of benign tumors and 33 cases of malignanat tunlors respectively.Complete resections were performed in 47 patients(76%),partial resection was conducted for 10 patients.Resection could not be performed in 5 patients so only biopsy was conducted,12 patients accepted combined organ resections,2 patients died.Conclusion Adequate preoperative preparation and excellent surgical technique are the keys to compele tumour resection. Abstract:Objective:To summarize our experience on the diagnosis and surgical treatment of primary retroperitoneal tumor(PRPT).Methods:The clinical data of 315 patients with primary retroperitoneal tumor(from Jan.2000 to Jun.2008) were retrospectively analyzed,and the clinical experience on diagnosis and treatment was summarized.Results:The ratio of benign to malignant tumors in our group was 0.55:1.Totally 294 patients received operation,including 161 cases of radical resection,69 cases of combined resection,and 64 partial palliative resection.Twenty-one patients received no operation.The prognoses of patients who received radical resection for malignant tumors were significantly improved compared with those of patients who received partial palliative resection(P0.05).Conclusion:Imaging diagnosis can effectively identify PRPT.Complete resection is the key for treatment of PRPT,and reoperation is an effective treatment for recurrent PRPT.

Abstract:Objective:To summarize the clinical characteristics of primary retroperitoneal tumors (PRT).Methods:All PRT cases undergoing surgical resection during recent 10 years at our center were retrospectively analyzed.Results:Tumors in all 92 cases were of malignant in 64 cases, borderline in 10 and benign PRT in 18, among which liposarcoma and leiomyosarcoma were the most common types. The tumor size and Ki-67 was significantly higher in malignant compared to borderline or benign PRT. The multifocal rate was 50%, en-bloc resection rate was 72%, R 0 rate was 61%, and combined organ resection rate was 41% in malignant PRTs. Small intestine and the colon were the most frequently resected organs. During 9.3 years of follow-up period, the 1-, 3- and 5-year cumulative reoperation rate of malignant PRT was 10.6%, 44.7% and 62.9%, respectively, and the median re-operation period was 4.0 years. The 1-, 3- and 5-year cumulative survival rate was 90.1%, 73.0% and 64.2%, respectively, and the median survival period was 6.1 years. None of postoperative borderline or benign PRT recurred or needed re-operation or deceased. Conclusion:Most of PRTs were malignant, presenting themsehies as a challenge to surgery with unfaror prognosis.

Abstract:[Objective] To summarize and analyze the important factors of treating the primary retroperitoneal tumors with laparoscope successfully.[Methods] The clinical information of 71 cases the primary retroperitoneal tumors were analyzed in Peking University Third Hospital from Apr.1994 to Jun.2010 retrospectively.The important factors of treating the primary retroperitoneal tumors with laparoscope successfully were analyzed.[Results] The incidence and cases of the primary retroperitoneal tumors is lower and fewer.The surgery is not a kind of common operation.Sixty-one cases underwent operation through open abdomen and ten cases through laparoscope selectively in sixteen years.[Conclusions] According to principle of benefiting the patients,the important factors of treating the primary retroperitoneal tumors with laparoscope successfully are the more experience of operation through open abdomen,fine laparoscopic technology and preoperation evaluation by image examination.The number of laparoscopic resection of primary retroperitoneal tumors would be more and more performed year by year.

Abstract:Objective To investigate the surgical treatment of primary retroperitoneal tumor(PRT). Methods 42 cases of pathologically confirmed PRT from May 1998 to April 2005 were retrospectively reviewed, including malignant in 17 cases and benign in 25 cases. Result Complete resection was achieved in 32 cases in which combined organ resection carried out in 13 cases, 7 cases underwent partial resection, and 2 cases had biopsy only. Reoperation performed for recurrent malignant tumor in 6 cases including complete resection in 3 cases. The 1-, 3-, 5-year survival rates were 58.8%, 41.2% and 24.1% respectively. Conclusion Survival can be improved by suitable procedure, complete resection and recurrent tumor excision.
