Abstract:Objective To evaluate the diagnostic performance of shear wave elastography ( SWE) with quantitative analysis and qualitative analysis of stiff rim sign between benign and malignant breast lesions . Methods Sixty-five female patients(65 lesions) underwent breast surgery or aspiration biopsy at the Second Affiliated Hospital of Zhejiang University were included . Elastic modulus of breast lesions and its rim in 1 mm ,2 mm ,3 mm were quantitive analyzed by SWE before surgery or aspiration biopsy ,Stiff rim sign was observed at the same time . Then ,the areas of receiver operating characteristic curves( AUC) of quantitative parameters of SWE and stiff rim sign were calculated to assess their clinical values in differentiating benign and malignant breast lesions ,and the correlation between them was evaluated . Afterwards , BI-RADS category was adjusted according to the optimal parameters combining the performance of SWE feature and conventional US features ,which could be used to evatulate the diagnostic effectiveness in differentiating benign and malignant breast lesions . Results The pathological results demonstrated 32 benign cases and 33 malignant cases in all patients .The maximum cut-off point of BI-RADS category was set between 4a and 4b .AUC value was 0 .935(0 .845-0 .981) ,while the sensitivity and specificity were 84 .8％ and 90 .6％ ,respectively .Maximum elasticity of 2 mm(E2Smax) area surrounding the malignant lesions were significantly higher than those in benign lesions ( P = 0 .001 ) . Among all quantitative SWE features ,E2Smax showed the highest AUC ( 0 .773) ,with sensitivity of 78 .8％ and specificity of 71 .9％ . In addition ,the stiff rim sign as qualitative parameter showed the highest AUC (0 .800) ,with sensitivity 81 .8％ and specificity 78 .1％ . The correlation coefficient between E2Smax and stiff rim sign was 0 .63 ,which was regarded as moderate correlation . the AUC of combination of stiff rim sign or E2Smax and conventional US features was 0 .937 and 0 .902 ,and the sensitivities were 81 .8％ , 93 .9％ ,the specificities were 90 .6％ and 75 .0％ . Conclusions SWE features ,especially E2Smax and hard rim sign are found moderate correlation ,and can improve a more accurate diagnosis in differentiating benign and malignant breast lesions combining with conventional US features .
Semi-quantitating Stiffness of Breast Solid Lesions in Ultrasonic Elastography

Abstract:Rationale and Objectives. To explore whether strain ratio measurement could semi-quantitatively evaluate the stiffness of breast lesions.Materials and Methods. From January 2008 to May 2008. 148 patients with 254 solid lesions (183 benign 71 malignant) in the breast were included in the study. Ultrasound sonography found the lesions and ultrasonic elastography obtained the strain images. By using the strain ratio measurement method together with the ultrasound machine, the strain index of the lesion was calculated. Different depths of breast tissue were selected as the reference. The strain indexes of malignant and benign solid lesions were calculated with the same level of breast tissue as the reference.Results. The strain indexes of breast lesions were different compared to the same depth of breast tissue and the superior level of fat tissue (P = 0.000). The strain indexes of breast lesions were different compared to different depths of breast glandular tissues (P = 0.003). At the same level of the breast lesions. 212 lesions were glandular tissue. 11 were fat tissue, and 40 were both. In the lesion plane, six lesions had almost no glandular tissue and 220 had almost no superior fat tissue. Compared to the same depth of breast tissue, the strain indexes of benign lesions (range, 0.62-11.07) and malignant lesions (range, 3.12-39.28) were different (P = 0.000).Conclusion. Using the strain ratio measurement, stiffness of breast lesions could be semi-quantitated with the same depth of breast tissue as the reference. This method may provide another diagnostic method in addition to the 5-point scoring system used with ultrasonic elastography in the future.
Complex cystic and solid breast lesions: diagnostic performance of conventional ultrasound, strain imaging and point shear wave speed measurement.

Abstract:OBJECTIVE: To assess the performance of conventional high frequency ultrasound (US) and US elastography in diagnosis of complex cystic and solid breast lesions. METHODS: Ninety three lesions in 93 patients underwent conventional US and US elastography, including strain elastography, acoustic radiation force impulse (ARFI) imaging, and point shear wave speed (SWS) measurement. RESULTS: Pathological examination revealed 31 (33.3%) of the 93 lesions were malignant and the remaining 62 (66.7%) were benign. Multivariate analysis showed that elder patient (OR: 25.301), internal vascularity (OR: 4.518), and not circumscribed margin (OR: 3.813) were independent predictors for malignancy, while predominately cystic lesions (OR: 0.178) was a predictor for benign lesions (all p < 0.05). Invalid SWS measurement was occurred in 19 of 31 (61.3%) malignant lesions and 16 of 62 (25.8%) benign lesions, respectively (p < 0.05). The mean SWS value for malignant lesions was significantly lower than that for benign ones, being 1.60 +/- 0.63 m/s (range, 0.68-2.70 m/s) versus 2.33 +/- 0.77 m/s (range, 0.67-3.97 m/s) (p < 0.05). Areas under the ROC curve (Azs) for Breast Imaging Reporting and Data System (BI-RADS) assessment, strain elasticity score, ARFI imaging and valid point SWS measurement were 0.844, 0.734, 0.763 and 0.778, respectively. CONCLUSIONS: US BI-RADS category, strain elastography score, ARFI imaging patterns and point SWS measurement are useful for malignancy prediction of complex cystic and solid breast lesions. The result that SWS for malignant lesions is lower than benign one should be carefully interpreted since invalid SWS measurement is excluded for analysis. The true stiffness of malignant cystic and solid lesions should be further evaluated with a new generation of two-dimensional SWS imaging.

Abstract:OBJECTIVES:To prospectively evaluate the diagnostic performance of three-dimensional (3D) shear wave elastography (SWE) for breast lesions with quantitative stiffness information from transverse, sagittal and coronal planes. METHODS:Conventional ultrasound (US), two-dimensional (2D)-SWE and 3D-SWE were performed for 122 consecutive patients with 122 breast lesions before biopsy or surgical excision. Maximum elasticity values of Young's modulus (Emax) were recorded on 2D-SWE and three planes of 3D-SWE. Area under the receiver operating characteristic curve (AUC), sensitivity and specificity of US, 2D-SWE and 3D-SWE were evaluated. Two combined sets (i.e., BI-RADS and 2D-SWE; BI-RADS and 3D-SWE) were compared in AUC. Observer consistency was also evaluated. RESULTS:On 3D-SWE, the AUC and sensitivity of sagittal plane were significantly higher than those of transverse and coronal planes (both P < 0.05). Compared with BI-RADS alone, both combined sets had significantly (P < 0.05) higher AUCs and specificities, whereas, the two combined sets showed no significant difference in AUC (P > 0.05). However, the combined set of BI-RADS and sagittal plane of 3D-SWE had significantly higher sensitivity than the combined set of BI-RADS and 2D-SWE. CONCLUSIONS:The sagittal plane shows the best diagnostic performance among 3D-SWE. The combination of BI-RADS and 3D-SWE is a useful tool for predicting breast malignant lesions in comparison with BI-RADS alone.
Virtual Touch Tissue Imaging and Quantification: Value in Malignancy Prediction for Complex Cystic and Solid Breast Lesions

Abstract:This study was aimed to evaluatethe usefulness of conventional ultrasound (US) and US elastography, including the latest virtual touch tissue imaging and quantification (VTIQ), in malignancy prediction for complex cystic and solid breast lesions. Eighty-nine complex cystic and solid breast lesions were subject to conventional US and US elastography, including strain elastography (SE), virtual touch tissue imaging (VTI) and VTIQ. Among the 89 lesions, thirty-four (38.2%) lesions were malignant and 55 (61.8%) lesions were benign. Sixteen variables were subject to multivariate logistic regression analysis. Pattern 4b in VTI (odds ratio, OR:15.278), not circumscribed margin of lesion (OR:12.346), SWS mean >4.6 m/s in VTIQ (OR:11.896), and age elder than 50 years (OR:6.303) were identified to be independent predictors for malignancy. In receiver operating characteristic (ROC) curve analyses, associated areas under the ROC curve (Az) for conventional US could be significantly elevated, from 0.649 to 0.918, by combining with US elastography (p < 0.0001). The combined diagnostic method was able to improve the specificity (32.7% vs. 87.3%, p < 0.0001) without sacrificing the sensitivity (97.1% vs. 85.3%, p = 0.075). Both conventional US and US elastography contribute substantially to malignancy prediction in complex cystic and solid lesions. The diagnostic efficacy of conventional US in terms of Az and specificity could be significantly improved by combining with US elastography.

Abstract:PURPOSETo analyze the diagnostic performance of shear wave elastography (SWE) in differentiating between benign and malignant breast lesions, with special emphasis on the value of the "stiff rim" sign, as compared with conventional ultrasonography (US).MATERIALS AND METHODSFor this ethics committee-approved retrospective study, all patients provided verbal informed consent for the analysis of their imaging data. A total of 193 consecutive women (age range, 18-82 years; mean age, 46 years) with 193 breast lesions (56 malignant, 137 benign) were included. The stiff rim sign, a qualitative SWE feature, was evaluated at the display setting of less than 180 kPa and at 180 kPa. The quantitative SWE features were assessed. Sensitivity, specificity, the area under the receiver operating characteristic curve (Az), and positive and negative likelihood ratios were calculated for conventional US features, for SWE features, and for combined conventional US and SWE features.RESULTSAmong all qualitative and quantitative SWE features, the stiff rim sign at the display setting (<180 kPa) showed the highest Az (0.918; 95% confidence interval [CI]: 0.870, 0.953), which was comparable to that for conventional US (0.891; 95% CI: 0.838, 0.931) (P = .40). The combination of the stiff rim sign at less than 180 kPa and conventional US features, with a positive likelihood ratio of 12.23 (95% CI: 11.5, 13.0) and a negative likelihood ratio of 0.02 (95% CI: 0.003, 0.1), yielded the highest Az (0.982; 95% CI: 0.951, 0.995)-higher than those for conventional US only and for any single SWE feature alone (P < .001 for all)-and yielded higher sensitivity (98.2% [55 of 56]; 95% CI: 90.4%, 100.0%) and similar specificity (92.0% [126 of 137]; 95% CI: 86.1%, 95.9%) compared with conventional US (P < .001 and P = .58, respectively). With use of this combination, the rate of recommendation of benign lesions for interventional procedures would decrease from 56.9% (78 of 137) (on the basis of conventional US features) to 15.3% (21 of 137), and 100.0% (56 of 56) (95% CI: 93.6%, 100.0%) malignant lesions would be correctly selected for biopsy.CONCLUSIONAdding SWE features, especially the stiff rim sign at the display setting (<180 kPa), to conventional US has the potential to improve the differentiation of breast lesions.
Evaluation of Tissue Stiffness Around Lesions by Sound Touch Shear Wave Elastography in Breast Malignancy Diagnosis

Abstract:The aim of the study described here was to assess the evaluation of tissue stiffness around lesions by sound touch shear wave elastography (STE) in breast malignancy diagnosis. This was an institutional ethics committee–approved, single-center study. A total of 90 women with breast masses examined with conventional ultrasound and STE were eligible for enrollment from December 2020 to July 2021. The maximum and mean elastic values of masses, Emax and Emean, were determined. Shell function was used to measure the maximum and mean elastic values of tissues around masses in annular shells 0.5, 1.0, 1.5 and 2.0 mm wide, recorded as corresponding Emax-shell and Emean-shell. All parameters were analyzed and compared with histopathologic results. Receiver operating characteristic curves were constructed to assess diagnostic performance. Logistic regression analysis was conducted to determine the best diagnostic model. Collagen fiber content of tissues around breast lesions was evaluated using Masson staining and ImageJ software. Ninety women with breast masses were included in this study; 50 had benign (mean diameter 15.84 ± 4.39 mm) and 40 had malignant (mean diameter 17.40 ± 5.42 mm) masses. The diagnostic value of Emax-shell-2.0 was the highest (area under the curve = 0.930) with a sensitivity of 87.5% and specificity of 88%. According to stepwise logistic regression analysis, Emax-shell-2.0 and age were independent predictors of malignancy. Emax-shell-2.0 was also found to be highly correlated with the collagen fiber content of tissue in the malignant group (r = 0.877). Tissue stiffness around lesions measured by STE is a useful metric in identifying malignant breast masses by reflecting collagen fiber content, and Emax-shell-2.0 performs best.

Abstract:Purpose: To compare the diagnostic performance of 5-point scoring system and strain ratio by sonoelastography in the assessment of solid breast lesions. Material and methods: One hundred and eighty-seven solid masses in 155 patients were scanned by two-dimensional ultrasonography and sonoelastography. Elasticity scores were determined with a 5-point scoring method, and the strain ratio was based on the comparison of the average strain measured in the lesion with the adjacent breast tissue in the same depth. Pathological results were taken as gold standards to compare the diagnostic efficacy of two methods with clinical diagnostic test and receiver operating characteristic (ROC) curves. Results: Among 187 lesions, 130 were benign and 57 were malignant. The mean scores (1.62 ± 0.69 vs 4.07 ± 0.26, P < 0.05) and strain ratios (2.06 ± 1.27 vs 6.66 ± 4.62, P < 0.05) were significantly higher of malignant than benign lesions. The area under the curve for the 5-point scoring system and for strain ratio-based elastographic analysis was 0.892 and 0.909, respectively (P > 0.05). For 5-point scoring, sonoelastography had 84.2% sensitivity, 84.6% specificity, 84.5% accuracy, 70.6% positive predictive value and 92.4% negative predictive value. When a cutoff point of 3.06 was used, sensitivity, specificity, accuracy, positive and negative predictive values were 87.7%, 88.5%, 88.2%, 76.9% and 94.3%, respectively for the strain ratio (P > 0.05). Conclusions: The 5-point scoring system and strain ratio has similar diagnostic performance, and the strain ratio could be more objective to differentiate the masses when those masses were difficult to be judged by using 5-point scoring system in sonoelastographic images. © 2012 Elsevier Ireland Ltd. The value of peri-lesional tissue stiffness and stiff rim sign in the differential diagnosis between benign and malignant breast lesions

Abstract:Abstract Background Shear wave elastography (SWE) is an important method in the diagnosis of breast lesions. The purpose of this study was to evaluate the value of tissue stiffness around breast lesion and stiff rim sign for the differentiation of benign and malignant lesions. Methods 192 patients (mean age, 44.6 ± 13.6 years) with 199 breast lesions proven by pathological examination underwent shear wave elastography (SWE). We first observed if there was a stiff rim sign. Then the shell around the breast lesion on SWE was automatically drawn by machine, with width of 1mm, 2mm and 3mm. Elasticity modulus of the lesion and surrounding tissue were recorded, including maximum elasticity (Emax), mean elasticity (Emean), minimum elasticity (Emin) and elasticity ratio (shell/lesion ratio). The optimal thresholds of elasticity modulus were calculated according to receiver operating characteristic (ROC) curve. Results There were 75 malignant lesions and 124 benign lesions. The average Emax, Emean of lesion and shell were significantly higher in the malignant group than in the benign group (P<0.05). The optimal cut-off value of Emax for diagnosing malignant lesion was 101.7 Kpa, with a sensitivity of 66.3% and specificity of 87.9%. The optimal cut-off value of Emean was 29.1 Kpa, with a sensitivity of 65.3% and specificity of 79.8%. The stiff rim sign had a highest diagnostic performance for malignancy than other elastic parameters, with an accuracy of 88.4%. However, measuring peritumoral tissue stiffness can achieve a relatively high sensitivity, whereas specificity was not improved significantly. Conclusion The stiffness of tissue surrounding breast malignancy was significantly higher than benign lesion. Stiff rim sign has the potential to improve the diagnostic performance of breast lesions.
Strain Elastography Features In Invasive Breast Cancer: Relationship Between Stiffness And Pathological Factors

Abstract:Our study aimed to investigate the relationship between stiffness and pathological factors in invasive breast cancer. We evaluated stiffness by strain elastography indicators of elasticity score and hardness percentage (HP) in tumors that histologically confirmed as invasive breast cancers. The relationship between stiffness and pathological factors including histologic type, histologic grade and molecular subtypes were analyzed, and the most influential factor was further determined by logistic regression analysis. Of 291 invasive breast cancers, 230 (79.0%) tumors were stiff according to elasticity score evaluation, and the average value of HP was (82.32 +/- 15.72)%. Based on the elasticity score result, the cutoff value of HP for hard tumors was 80.0% (sensitivity 90.1%, specificity 100.0%), lesions with inconsistent evaluation results by two indicators were almost grade III tumors (19/21). Differences in stiffness could be found in histologic grades (both P < 0.001) and molecular subtypes (score P=0.003, HP P < 0.001) but were not obvious in histological types (score P=0.034, HP P=0.131). Grade I or grade II (low grade) type tumors and luminal A type tumors were harder, while histologic grade was the only independent factor for tumor stiffness. The grade III group, which had a mean value of HP (65.68 +/- 18.02)%, was significantly softer than the low grade group (P < 0.001). The cutoff value of HP for low grade tumors was 80.59% (sensitivity 88.2%, specificity 84.4%). Our study revealed a close relationship between tumor stiffness and the histologic grade of invasive breast cancer. Strain elastography may have potential applications to provide prognostic information for patients with breast cancer noninvasively.
Quantitative Evaluation of Peripheral Tissue Elasticity for Ultrasound-Detected Breast Lesions.

Abstract:AIM: To evaluate the diagnostic performance of the quantitative measurement of peripheral tissue elasticity using shear-wave elastography (SWE) in differentiating between benign and malignant breast lesions detected by ultrasonography (US).MATERIALS AND METHODS: This study was approved by institutional review board; informed consent was signed from all patients. From June 2012 to April 2014, conventional B mode US and SWE were performed in 205 breast lesions (78 malignant, 127 benign) of 205 consecutive women (mean age, 41.9 +/- 12.3 years; age range, 18-76 years). For each lesion, a rim around its contour was constructed by using a computer-aided tool, including adequate adjacent peripheral tissue. Quantitative elastographic features of the rim (elasticity mean, maximum, standard deviation, and elasticity ratio) as well as Breast Imaging Reporting and Data System.(BI-RADS) categories were assessed according to the final histopathological results. Sensitivity, specificity, positive and negative predictive values, and receiver operating characteristic (ROC) curve analysis were performed to evaluate the diagnostic performances for the three data sets (conventional B-mode US alone, SWE features alone, and combined B mode US and SWE features).RESULTS: SWE features of the peripheral tissue differed significantly between benign and malignant breast lesions (mean +/- standard deviation [SD]: elasticity mean, 16.1 +/- 6.6 versus 43.6 +/- 17.8; maximum, 55.4 +/- 31.4 versus 150.3 +/- 44.6; SD, 8.1 +/- 4.1 versus 30 +/- 13.8 and elasticity ratio, 1.1 +/- 0.2 versus 2 +/- 0.7, respectively, all p<0.001), and enabled significantly higher specificities compared with conventional B-mode US alone (92.9%, 84.3%, 94.5%, and 93.7% versus 44.1%, respectively, all p<0.001). The combinations of SWE assessment (including individual features and the logistic regression model of all four features) and conventional B-mode US for BI-RADS.3 and 4a lesions significantly improved the diagnostic performance compared with B mode US alone, with significantly increased specificity (85%, 82.7%, 81.9%, 86.6%, and 85% versus 44.1%; all p<0.001) and AUC (0.942, 0.940, 0.940, 0.935, and 0.953 versus 0.916, all p<0.05).CONCLUSION: Quantitative evaluation of the elastic properties of the peripheral tissue exhibited good discriminatory ability in differentiating US-detected breast lesions and could be used to further stratify low-suspicion lesions within BI-RADS category 3 and 4a to avoid unnecessary biopsy. Abstract:Objective To evaluate the accuracy of the strain ratio by ultrasonic elastography in diagnosis of benign and malignant breast masses.Methods One hundred and seventy-three breast lesions in 162 cases were examined with high frequency sonography and elastography.The strain ratio(E2/E1) between normal breast tissue and mass was measured and compared with pathological diagnosis.Results The area under the ROC curve(AUC)of strain ratio method in diagnosis of benign and malignant breast tumors are 0.950(95% CI,0.916～0.983).The cut-off point of maximum Yuden index was established(0.779)and then the critical point of strain ratio for the diagnosis of benign and malignant breast masses was 0.355.The value of E2/E1 of benign and malignant masses were 0.57±0.15 and 0.29±0.08,repectively,which were significantly differently(P0.05).There was no significant difference between strain ratio method and conventional ultrasonography for diagnosis of breast masses.But the combined application of the two methods could significantly improve positive detection rate than single method(P0.05).Conclusion The strain ratio method has a highly accuracy in diagnosis of benign and malignant breast masses and can improve the detection rate of malignant breast tumor when combined with conventional ultrasonogrphy.
Quantitative Analysis of Peri-Tumor Tissue Elasticity Based on Shear-Wave Elastography for Breast Tumor Classification

Abstract:For shear-wave elastography (SWE) images, the most common site of tumor-associated stiffness is generally in the surrounding stroma rather than the tumor itself. The aim of this study is to assess the value of the peri-tumor tissue elasticity in the classification of breast tumors. SWE images of 106 breast tumors (65 benign, 41 malignant) were collected from 82 consecutive patients. By applying the image processing method, 5 elastographic features of the peri-tumor area (elasticity modulus mean, maximum, standard deviation, hardness degree and elasticity ratio) were computed to represent peri-tumor tissue elasticity. B-mode Breast Imaging Reporting and Data System (BI-RADS) were used for comparing the diagnostic performances between the grayscale US and color SWE images. Histopathologic results were used as the reference standard. The t-test, point biserial correlation coefficient and receiver operating characteristic (ROC) curve analysis were performed for statistical analysis. As a result, the Az values (area under ROC curve) were 0.92, 0.95, 0.94, 0.91, and 0.98 for the classifiers using the five elastographic features respectively, and 0.91 for BI-RADS assessment. The results showed that the peri-tumor tissue elasticity could provide valuable information for breast tumor classification.
Diagnostic value of strain ratio measurement in breast neoplasm

Abstract:Objective To explore the clinical value of strain ratio measurement with ultrasound elastography(UE) for diagnosis of breast neoplasm.Methods A total of 109 breast masses in 103 patients for diagnosis in our hospital were randomly examined with 2-dimensional ultrasonography and elastosonography.The strain ratio of the lesion and nearby breast tissue was calculated,and the results were compared with final pathological findings.Results There were 71 benign and 38 malignant lesions in the 109 breast masses.The strain ratios between benign lesion(2.26±1.39) and malignancy(6.95±4.08) were significantly different(P<0.05).The sensitivity,specificity and accuracy of 2-dimensional ultrasonography and strain ratio for breast cancer detection was 81.58%,80.28%,80.73% and 86.84%,88.73%,88.07% respectively(P>0.05).The rate of diagnostic sensitivity and accuracy was increased to 97.37% and 93.58% respectively(P<0.05) by combination of 2-dimensional ultrasonography and strain ratio measurement.Conclusions Combined 2-dimensional ultrasonography with ultrasonic elastographic diagnostic method can elevate the coincidence rate of preoperative diagnosis of breast neoplasm.
Stiffness in Breast Masses with Posterior Acoustic Shadowing: Significance of Ultrasound Real Time Shear Wave Elastography.

Abstract:BACKGROUND:To assess the stiffness of benign breast masses in ultrasound images with posterior acoustic shadowing (PAS) and malignant lesions, and explore the significance of differential diagnosis using ultrasound real time shear wave elastography. MATERIAL AND METHODS:All 117 mammary masses (98 patients) with PAS were assessed by using routine ultrasound examination, and elastic modulus values were obtained with the real time shear wave elastography mode. All breast lesions were confirmed by surgery or biopsy. The significance of differences in ultrasound elastography values between breast benign and malignant masses with posterior acoustic shadowing was assessed, and the ROC curves of elasticity modulus values were analyzed. RESULTS:Among the 117 masses, 72 were benign and 45 were malignant. The two types of breast masses showed significant differences in size, margin, internal echo, calcification, and blood flow characteristics (P < 0.05), although the difference in orientation was not significant (P > 0.05). Emean, Emax and Esd obtained with real time shear wave elastography showed statistically significant differences between benign masses with posterior acoustic shadowing and breast cancer (P < 0.05), while Emin showed no significant difference between them (P = 0.633). Ultrasound real time shear wave elastography showed higher sensitivity and specificity than conventional ultrasound. CONCLUSIONS:Benign and malignant breast masses with PAS show different ultrasound manifestations. Real time shear wave elastography can facilitate the differential diagnosis and treatment planning for these breast masses.

Abstract:Objectives-To develop and evaluate a set of quantifiable elastographic features based on ultrasound real-time strain elastography (SE) in differentiating between benign and malignant breast lesions. Methods-The SE and conventional B-mode ultrasound images of 226 breast lesions (81 malignant, 145 benign) were obtained from 226 consecutive women. By using a computer-aided tool, four elastographic features (elasticity score, lesion stiffness degree, lesion-to-fat strain ratio, and elastography-to-B-mode lesion area ratio) were respectively calculated and evaluated. Histopathologic results were used as the reference standard. B-mode Breast Imaging Reporting and Data System categorization was used to compare the perfonnances between B-mode ultrasound and SE. Sensitivity, specificity, positive and negative predictive values, and receiver operating characteristic curve analyses were performed to evaluate the diagnostic performances for three data sets (conventional B-mode ultrasound alone, SE features alone, combined SE features). Results-Quantifiable SE features for malignant lesions all showed significantly higher values than those for benign lesions (all P <.001). The evaluation with any individual SE feature significantly improved the specificity in breast lesion differentiation compared with B-mode ultrasound (all P <.001). The logistic regression model combing SE features significantly improved the diagnostic performance compared with B-mode US, with significantly increased specificity (95.2% versus 54.5%; P <.001) and area under the receiver operating characteristic curve 0.988 versus 0.921, P <.001). Conclusions-Computer-aided tool with SE provided further elasticity information for breast characterization. Evaluation using quantifiable SE features showed better diagnostic performance than conventional B-mode ultrasound in breast lesion differentiation.

Abstract:OBJECTIVETo determine the diagnostic performance of conventional ultrasound (US) and two-dimensional shear wave elastography (2D SWE) in the differential diagnosis between mastitis and breast malignancy.METHODSBetween January 2016 and March 2017, 105 patients with 105 pathologically proven breast lesions were enrolled. All the lesions were subject to conventional US and 2D SWE examinations. In 2D SWE, the qualitative parameter of stiff rim sign and quantitative parameter of maximal shear wave velocity (SWV) were obtained. The diagnosis performances of US and combination of US and 2D SWE were evaluated, including sensitivity, specificity and the area under the receiver operating characteristic curve (AUROC). The AUROC of US and the combined method were also evaluated in subgroups with different diameters.RESULTSPathologically, 26 breast lesions were confirmed to be mastitis and 79 were malignant. The cut-off value for maximal SWV was 6.75 m/sec. The AUROC of stiff rim sign and maximal SWV were 0.701 (95% CI: 0.587-0.815) and 0.753 (95% CI: 0.659-0.832) respectively. Compared with US, the specificity and AUROC of the combined method increased significantly (specificity: 11.5% vs. 96.1%, AUROC: 0.520 vs. 0.752; both P < 0.05).CONCLUSIONSThe combination of US and 2D SWE improved the diagnostic performance in the differential diagnosis between mastitis and breast malignancy in comparison with the conventional US alone.
Role Of Strain Elastography And Virtual Touch Tissue Quantification Technique In The Diagnosis Of Breast Solid Lesions

Abstract:Objective: This study aimed to compare the diagnostic performance of the strain elastography and virtual touch tissue quantification (VTQ) technique in the differentiation of malignant and benign breast solid lesions, and determine which one is better than the other for a certain pathological type or a certain case. Materials and methods: 52 breast solid lesions (27 benign and 25 malignant) in 49 consecutive female patients (mean age, 46.7 +/- 11.8 years) were included in this study. Conventional ultrasound, subsequently strain elastography and VTQ examination were performed. The diagnostic performance were evaluated by receiver operating characteristic analysis. Results: There was significant difference in strain ratio (SR) (P < 0.001) and shear wave velocity (SWV) (P < 0.001) between benign and malignant lesions. For strain elastography, the optimal cutoff value of SR was 0.499 (AUC(SR) = 0.919) with a sensitivity of 93.6%, specificity of 72.0%, PPV of 78.8%, NPV of 94.7% and accuracy of 84.6%. For VTQ technique, the optimal cutoff value of SWV was 2.90 m/s (AUC(SWV) = 0.796) with a sensitivity of 68.0%, specificity of 92.6%, PPV of 89.5%, NPV of 75.8% and accuracy of 80.8%. Z test results showed that there was no significant difference in area under the curve (P = 0.057). Conclusions: Although the diagnostic performance of strain elastography and VTQ technique in the differentiation of breast solid lesions was almost equally. VTQ technique showed higher specificity but lower sensitivity than strain elastography. These two methods could be used to help to re-regulate the BI-RADS category of breast lesions, especially for equivocal BI-RADS category 3 and category 4 breast lesions.

Abstract:Objective To investigate the diagnostic value of strain ratio for differentiation breast benign and malignant solid lesions. Methods Two hundred and sixty-nine patients with 377 lesions (278 benign, 99 malignant) were included in the study. Ultrasonography found the lesions, then ultrasonic elastography got the strain images. By the strain ratio measure method equipped with the ultrasonic machine, strain ratio of the lesion was calculated. Compared with the pathologic diagnosis, the cut-off point of strain ratio was determined with receiver operating characteristic curve. Results There was significant difference between the strain ratio of benign and malignant lesions( P <0. 05). The cut off point of strain ratio was determined as 3. 08, Youden's index was 0. 88,sensitivity was 97. 38% ,and specificity was 91. 33%. Conclusions Strain ratio could be used to differentiate benign and malignant lesions in breast and would be a new diagnostic standard in future.
