Abstract:The use ofdegradable polymers in medicine largely started around the mid 20th century with their initial use as in vivo resorbing sutures. Thorough knowledge on this topic as been gained since then and the potential applications for these polymers were, and still are, rapidly expanding. After improving the properties of lactic acid-based polymers, these were no longer studied only from a scientific point of view, but also for their use in bone surgery in the 1990s. Unfortunately, after implanting these polymers, different foreign body reactions ranging from the presence of white blood cells to sterile sinuses with resorption of the original tissue were observed. This led to the misconception that degradable polymers would, in all cases, lead to inflammation and/or osteolysis at the implantation site. Nowadays, we have accumulated substantial knowledge on the issue of biocompatibility of biodegradable polymers and are able to tailor these polymers for specific applications and thereby strongly reduce the occurrence of adverse tissue reactions. However, the major issue of biofunctionality, when mechanical adaptation is taken into account, has hitherto been largely unrecognized. A thorough understanding of how to improve the biofunctionality, comprising biomechanical stability, but also visualization and sterilization of the material, together with the avoidance of fibrotic tissue formation and foreign body reactions, may greatly enhance the applicability and safety of degradable polymers in a wide area of tissue engineering applications. This review will address our current understanding of these biofunctionality factors, and will subsequently discuss the pitfalls remaining and potential solutions to solve these problems.

Abstract:With attractive research and development of biomaterials, more and more opportunities have been brought to the treatments of human tissue repairs. The implant is usually no need to exist in the body accompanied with the recovery or regeneration of the tissue lesions, and the long-term effect of exotic substance to human body should be reduced as lower as possible. For this purpose, biodegradable materials, including polymers, magnesium alloys and ceramics, have attracted much attention for medical applications due to their biodegradable characters in body environment. This paper in turn introduces these three different types of widely studied biodegradable materials as well as their advantages as implants in applications for bone repairs. Relevant history and research progresses are summarized.

Abstract:Implants, including artificial joints, bone fixation devices, and other orthopedic implants, oral implants, and vascular interventional devices, are used to repair or replace human tissues or organs and restore their functions. Since biodegradable implants have advantage of avoiding long-term complications (including bone stress shielding, restenosis, thrombosis, and secondary surgery) while remaining safe and productive, personalized biodegradable implants will be an irresistible trend in the clinic for implantable and interventional medical devices. However, innovation of personalized biodegradable implants faces several challenges, including the interaction between the implant and its surrounding tissues or cells, the coordination of structural strength of implants and its degradation, the topological microstructure of implant and its fatigue properties, reliability, and safety. In this review, we introduced critical progresses achieved in the fields related to implants, including mechanical properties of materials, interaction between implants and host tissue, effect of stress on degradation; furthermore, we highlighted the optimized design and manufacture of implants as well as the evaluation of their reliability.

Abstract:<h3>Purpose</h3><p>To compare the advantages and disadvantages of biodegradable and non-degradable implants in orthopedic fractures.</p><h3>Methods</h3><p>Recent original articles about biodegradable and non-degradable implants for fracture fixation were reviewed extensively, and a comprehensive retrospective analysis was performed.</p><h3>Results</h3><p>Standard orthopedic treatment is to use non-degradable metal implants to fixate the fracture site. This kind of treatment not only causes rejection and stress shielding, but also requires a second surgery to remove the metal implants. In addition, this kind of treatment increases physical pain and is a serious financial burden to patients. However, biodegradable implants do not require a second surgery for removal and have good biocompatibility and osteoconductivity.</p><h3>Conclusions</h3><p>Biodegradable implants do not require a second surgery for removal and have good biocompatibility and osteoconductivity. Consequently, they are an ideal treatment and are increasingly used for orthopedic surgical patients. The most common indications for biodegradable implants include craniofacial reconstruction, anterior cruciate ligament reconstruction, meniscus repair, ankle fracture treatment, and tibia and fibula fracture treatment.</p>

Abstract:Copolymer of polycaprolactone (PCL) and polylactic acid (PLA) was synthesized and catalyzed using Y (CF3COO)3/AL (I-Bu)3. The biocompatibility was evaluated by means of biochemistry, immunocytochemistry, and by cytotoxity test. This novel orthopedic biodegradable polymer can serve as an ideal orthopedic biodegradable implants, and adjustment of molecular weight and ratio of polymers can control its degradation period.

Abstract:This review discusses and summarizes the recent developments and advances in the use of biodegradable materials for bone repair purposes. The choice between using degradable and non-degradable devices for orthopedic and maxillofacial applications must be carefully weighed. Traditional biodegradable devices for osteosynthesis have been successful in low or mild load bearing applications. However, continuing research and recent developments in the field of material science has resulted in development of biomaterials with improved strength and mechanical properties. For this purpose, biodegradable materials, including polymers, ceramics and magnesium alloys have attracted much attention for osteologic repair and applications. The next generation of biodegradable materials would benefit from recent knowledge gained regarding cell material interactions, with better control of interfacing between the material and the surrounding bone tissue. The next generations of biodegradable materials for bone repair and regeneration applications require better control of interfacing between the material and the surrounding bone tissue. Also, the mechanical properties and degradation/resorption profiles of these materials require further improvement to broaden their use and achieve better clinical results.

Abstract:The biomedical application of biodegradable polymers for addressing bone-related diseases has garnered considerable attention in recent years. Advances in material technology have expanded the repertoire of materials suitable for orthopedic implants, with nanomaterials playing a pivotal role in replicating crucial surface properties akin to natural tissues. This comprehensive review explores the evaluation of bioactive glass ceramics, shedding light on their properties and applications. The synthesis of composites through composite manufacturing has emerged as a strategy to enhance biocompatibility and biomechanical characteristics. They are addressing challenges associated with conventional implants and nanomaterials, whether in the form of functional nano coatings or nanostructured surfaces, present opportunities to refine implant techniques. Novel developments in orthopedic biomaterials, such as smart biomaterials, porous structures, and 3D implants, offer stimuli-responsive behavior to achieve desired implant shapes and characteristics. Bioactive and biodegradable porous polymer/inorganic composite materials are explored for bone tissue engineering scaffolds, aiming to promote bone formation and regeneration. As a prospective direction, the integration of stem cells into scaffolds hints at the creation of next-generation synthetic/living hybrid biomaterials, displaying high adaptability in biological settings. This review establishes a foundation for nanotechnology-driven biomaterials by elucidating fundamental design factors crucial for orthopedic implant performance and their response to cell differentiation, proliferation, and adhesion.

Abstract:Utilization of polymers as biomaterials has greatly impacted the advancement of modern medicine. Specifically, polymeric biomaterials that are biodegradable provide the significant advantage of being able to be broken down and removed after they have served their function. Applications are wide ranging with degradable polymers being used clinically as surgical sutures and implants. In order to fit functional demand, materials with desired physical, chemical, biological, biomechanical and degradation properties must be selected. Fortunately, a wide range of natural and synthetic degradable polymers has been investigated for biomedical applications with novel materials constantly being developed to meet new challenges. This review summarizes the most recent advances in the field over the past 4 years, specifically highlighting new and interesting discoveries in tissue engineering and drug delivery applications.

Abstract:Presently, orthopedic and oral/maxillofacial implants represent a combined $2.8 billion market, a figure expected to experience significant and continued growth. Although traditional permanent implants have been proved clinically efficacious, they are also associated with several drawbacks, including secondary revision and removal surgeries. Non-permanent, biodegradable implants offer a promising alternative for patients, as they provide temporary support and degrade at a rate matching tissue formation, and thus, eliminate the need for secondary surgeries. These implants have been in clinical use for nearly 25 years, competing directly with, or maybe even exceeding, the performance of permanent implants. The initial implantation of biodegradable materials, as with permanent materials, mounts an acute host inflammatory response. Over time, the implant degradation profile and possible degradation product toxicity mediate long-term biodegradable implant-induced inflammation. However, unlike permanent implants, this inflammation is likely to cease once the material disappears. Implant-mediated inflammation is a critical determinant for implant success. Thus, for the development of a proactive biodegradable implant that has the ability to promote optimal bone regeneration and minimal detrimental inflammation, a thorough understanding of short- and long-term inflammatory events is required. Here, we discuss an array of biodegradable orthopedic implants, their associated short- and long- term inflammatory effects, and methods to mediate these inflammatory events.

Abstract:BACKGROUND:Polylactic acid and its copolymers have been widely used in biomedical fields because of their good biocompatibility and biodegradability, which have become a hotspot in the research of biomaterials. OBJECTIVE:To review the biocompatibility of fully degradable polylactic acid and its copolymers. METHODS:A computer-based online retrieval of PubMed and CNKI was performed to search relevant papers published from 2006 to 2016, with the key words of "polylactide, polylactic acid, copolymer, biodegradability, biocompatibility, animal" in English and Chinese, respectively. RESULTS AND CONCLUSION:Polylactic acid and its copolymers as polyester compounds are approved by the US Food and Drug Administration for use in the biomedical field because of their good biocompatibility mainly as drug delivery carriers and temporary implants. Moreover, their side effects in clinical application have attracted attentions. Polylactide copolymers can cause some adverse reactions when used in drug delivery systems, orthopedic and skin care, and other clinical medical fields. These copolymers are deemed to have no impact on the central nervous system, eyes, cardiovessels and other tissues and organs. They also have no virtually genotoxic and carcinogenic effects. Currently, the polylactide copolymer implant mainly results in local reactions in the surrounding tissues, and no systemic reactions have been found.

Abstract:Bone cements such as polymethyl methacrylate and calcium phosphates have been widely used for the reconstruction of bone. Despite their remarkable clinical success, the low degradation rate of these materials hampers a broader clinical use. Matching the degradation rate of the materials with neo bone formation remains a challenge for bone-repairing materials. Moreover, questions such as the mechanism of degradation and how the composition of the materials contribute to the degradation property remain unanswered. Therefore, the review provides an overview of currently used biodegradable bone cements such as calcium phosphates (CaP), calcium sulfates and organic-inorganic composites. The possible degradation mechanism and clinical performance of the biodegradable cements are summarized. This paper reviews up-to-date research and applications of biodegradable cements, hoping to provide researchers in the field with inspirations and references.

Abstract:Background: Due to the history of using permanent implants and the ability of adaptations of polymers to physiological environments such as the body environment, the need to design a polymer implant with a new formulation for orthopedic applications was felt. Methods: Polymer joints in this study were made by solvent casting method. The mechanical properties of the samples were investigated by bending tests, before and after immersion in simulated body fluid (SBF). Morphology of nanocomposites, bioactivity of samples and initiation of degradation process were performed by field emission scanning electron microscope (FESEM). Toxicity test was performed to evaluate the toxicity of nanocomposites. The antibacterial properties of the samples were investigated by examining the zone of inhibition and measuring the photometric concentration. Biodegradability test was performed to prove the biodegradability of polymer joints. Results: It was found that the mechanical properties of nanostructures increased with the addition of nanoparticles. Also, the presence of oxide and graphene nanoparticles affected the antibacterial properties of the composite nanostructure. Immersion in SBF solution showed that the nanostructures were biodegradable and bioactive. The results of this study indicate that the optimal nanocomposite PLA-PCL-HA-1% ZNO-1% GR has a Young’s modulus close to spongy bone and reduces the stress shielding phenomenon. The flexural Yang modulus of the PLA-PCL-HA nanocomposite was 2139.037 ± 381.312 MPa. The presence of zinc oxide and graphene nanoparticles increased the Young’s modulus to 4363.636 ± 127.498 MPa. The optimal sample has the necessary lethality against two strains of gram-positive and gram-negative bacteria and due to its bioactivity is a suitable option for use in spongy bone tissue. In this study, the viability of fibroblast cells in the vicinity of the polymer matrix versus the optimal matrix increased from 22.14 ± 0.623 to 82.96 ± 1.101% after 72 hours. Conclusions: Improving cell viability indicates a reduction in the optimal matrix toxicity compared to the polymer matrix.

Based on the results of the induced carcinoma, collagenous encapsulation and chronic inflammation around implanted polymers, the effects of polymers topography on biocompatibility are discussed, and the importance of topographical compatibility of materials is emphasized.

Abstract:Biodegradable polymers have been widely used and have greatly promoted the development of biomedical fields because of their biocompatibility and biodegradability. The development of biotechnology and medical technology has set higher requirements for biomedical materials. Novel biodegradable polymers with specific properties are in great demand. Biodegradable polymers can be classified as natural or synthetic polymers according to the source. Synthetic biodegradable polymers have found more versatile and diverse biomedical applications owing to their tailorable designs or modifications. This review presents a comprehensive introduction to various types of synthetic biodegradable polymers with reactive groups and bioactive groups, and further describes their structure, preparation procedures and properties. The focus is on advances in the past decade in functionalization and responsive strategies of biodegradable polymers and their biomedical applications. The possible future developments of the materials are also discussed.

Abstract:<p>The metamorphosis of biodegradable polymers in biomedical applications is an auspicious myriad of indagation. The utmost challenge in clinical conditions includes trauma, organs failure, soft and hard tissues, infection, cancer and inflammation, congenital disorders which are still not medicated efficiently. To overcome this bone of contention, proliferation in the concatenation of biodegradable materials for clinical applications has emerged as a silver bullet owing to eco-friendly, nontoxicity, exorbitant mechanical properties, cost efficiency, and degradability. Several bioimplants are designed and fabricated in a way to reabsorb or degrade inside the body after performing the specific function rather than eliminating the bioimplants. The objective of this comprehensive is to unfurl the anecdote of emerging biological polymers derived implants including silk, lignin, soy, collagen, gelatin, chitosan, alginate, starch, <em>etc.</em> by explicating the selection, fabrication, properties, and applications. Into the bargain, emphasis on the significant characteristics of current discernment and purview of nanotechnology integrated biopolymeric implants has also been expounded.
