THE IMPORTANCE OF ENDOBRONCHIAL ULTRASOUND-GUIDED FINE-NEEDLE ASPIRATION IN CRYPTOCOCCOSIS DIAGNOSIS

Abstract:TOPIC: Chest Infections TYPE: Medical Student/Resident Case Reports INTRODUCTION: Cryptococcosis is an opportunistic infection caused by Cryptococcus neoformans that affects immunocompromised patients. It could present as a pulmonary or disseminated infection that is associated with a high mortality rate. CASE PRESENTATION: A 30-year-old male without significant past medical history presented to the hospital with shortness of breath. A chest CT scan revealed diffuse bilateral nodular lesions along with mediastinal lymphadenopathy. Fungus culture from endobronchial ultrasound-guided fine-needle aspiration (EBUS-FNA) of subcarinal lymph node (station 7) was positive for Cryptococcus neoformans. There were subsequent multiple negative serum cryptococcus antigen tests along with negative cryptococcal antigen retrieved from bronchoalveolar lavage (BAL). Moreover, the patient was found to be HIV positive with a CD4 count of 21/mm3 and a viral load of 293,000 copies/mL. A lumbar puncture to further assess for possible cryptococcus meningitis showed negative cerebrospinal fluid (CSF) cryptococcal antigen. The tissue sample from a subsequent liver biopsy was also unremarkable for cryptococcus involvement. DISCUSSION: Serum cryptococcus antigen is one of the most sensitive diagnostic tools used for the detection of cryptococcal infection. Research data from a multicenter study revealed that the antigen is 95.2% sensitive in HIV patients with cryptococcal infection and is 100% sensitive in disseminated cryptococcal infection [1]. A retrospective study showed that 92.6% of the patients with pulmonary cryptococcosis had cryptococcal antigens detected in the BAL [2]. Another similar case detailed the utilization of EBUS-FNA in diagnosing mediastinal lymphatic cryptococcosis via subcarinal lymph node biopsy. However, our patient had negative serum and CSF cryptococcal antigen despite the similarity in the diagnostic method [3]. CONCLUSIONS: EBUS-FNA is a useful minimally invasive procedure that could help to diagnose both malignant and non-malignant mediastinal pathologies. Our case not only detailed a rare presentation of cryptococcosis but also accentuated the importance of this procedure as it had guided the rest of our patient's clinical workup.
