Abstract:Peroral endoscopic myotomy (POEM) originally developed by Inoue et al. is shown to be effective as a lesser invasive but curative treatment for esophageal achalasia. POEM has not yet been standardized and it is like endoscopic submucosal dissection technically difficult and time consuming. These limitations may be reduced by the use of the new HybridKnife®(Erbe, Germany), combining high pressure water-jet for needleless submucosal injection and electrosurgical interventions like submucosal tunnelling and endoscopic myotomy of inner circular muscle bundles.

Abstract:Achalasia is a rare primary esophageal motility disorder, which is featured with impaired relaxation of the lower esophageal sphincter.Nowadays there is no ideal medication treatment for this disease, while operation is too risky with multiple complications .Over the past few years,peroral endoscopic myotomy ( POEM) has been widely adopted as a clinical approach in practice,and has gradually become the first-line treatment,for it brings affirmative effects and less complications, equaling the operation.However, there remains no consensus on the indications and definite effects of POEM at home and abroad .
Bypassing Large Submucosal Vessels During Peroral Endoscopic Myotomy Reduces Operative Time and Intraoperative Blood Loss in Patients with Achalasia

Abstract:Objective: peroral endoscopic myotomy (POEM), a relatively minimally invasive endoscopic procedure, is the first-line treatment for achalasia. The aim of this study was to compare procedure-related parameters and clinical outcomes between bypassing and performing prophylactic electrocoagulation of large submucosal vessels during POEM. Methods: one hundred and twelve patients with achalasia who had undergone POEM at our hospital between April 2017 and March 2023 were retrospectively enrolled. Large submucosal vessels were bypassed to avoid injury during submucosal tunneling in the bypass group, whereas large submucosal vessels were prophylactically treated by electrocoagulation in the prophylactic electrocoagulation group. Procedure-related parameters, Eckardt score and complications were compared between the two groups. Results: the bypass group showed a significant reduction in operative time and amount of intraoperative blood loss than the prophylactic electrocoagulation group (37.11 +/- 9.96 min vs 58.80 +/- 17.90 min, and 1 [interquartile range: 1-2] ml vs 5 [interquartile range: 3-8] ml; p < 0.001). Eleven (17.5 %) and 44 (89.8 %) patients in the bypass and prophylactic electrocoagulation groups, respectively, required hemostatic forceps ( p < 0.001). Furthermore, lower operative and hospitalization costs were recorded in the bypass group than in the prophylactic electrocoagulation group ( p < 0.05). No statistically significant difference was found between the two groups in terms of submucosal tunnel length, myotomy length, clinical efficacy, or complications. Conclusions: bypassing large submucosal vessels during POEM can reduce the operative duration and intraoperative blood loss, with no difference in clinical outcomes than treatment.

Abstract:OBJECTIVE:To evaluate the efficacy and the feasibility of peroral endoscopic myotomy (POEM) for esophageal achalasia (AC).METHODS:The clinical data of 42 patients diagnosed as AC and received POEM in the Zhongshan Hospital of Fudan University between August 2010 and March 2011 were reviewed retrospectively. The key procedures of POEM included esophageal mucosal incision, submucosal "tunneling" by endoscopic submucosal dissection(ESD) technique, endoscopic myotomy of the circular muscle, and closure of mucosal incision by hemostatic clips.RESULTS:The mean age of 42 cases was 43.9(range 10-70) years. The mean duration of disease was 8.7 years (range, 3 months to 50 years). The mean operative time was (68.5 ± 25.5) (range 23-180) minutes with a mean submucosal tunneling length of(10.5 ± 1.5)(range 8-15) cm. The average length of endoscopic myotomy of inner circular muscle was(9.5 ± 2.5)(range 7-13) cm. No serious complications related to POEM were encountered. The median follow-up period was 2.5(range 1-6) months. Dysphagia symptom was relieved significantly during the follow-up period in 41 patients; one patient had dysphagia and vomiting 15 days after the operation. Endoscopic observation showed a submucosal fistula which was managed by endoscopic incision.CONCLUSIONS:As a novel minimally invasive therapy for AC, POEM appears to have definite short-term outcomes and can rapidly alleviate dysphagia symptom. Further observations and long follow-up are needed to evaluate long-term outcome and complications.

Abstract:Background Peroral endoscopic myotomy (POEM) has recently been introduced as a promising alternative to laparoscopic Heller myotomy for idiopathic achalasia. Several proposed technical modifications are yet to be tested in randomized trials. Objective The objective of our study was to evaluate efficacy and safety of water-jet (WJ) assisted POEM versus the conventional (C) technique. The clinical trial registration number is NCT01742494. Methods A prospective randomized trial was carried out in Zhongshan Hospital, Fudan University (Shanghai, China), in 100 consenting achalasia patients between August 2011 and April 2012. Patients eligible for POEM were randomized to use of either the HybridKnife (WJ group) or the conventional technique using injection and triangle tip knife interchangeably (C group). Results A total of 100 patients with comparable characteristics between groups were included. Procedure time was significantly shorter for the WJ group (22.9 ± 6.7 vs. 35.9 ± 11.7 min; p < 0.0001), mostly due to less replacement of accessories (2.0 ± 2.4 vs. 19.2 ± 7.6; p < 0.0001). Injection volume was larger in the WJ group (45.3 ± 10.2 vs. 35.2 ± 9.5 ml; p < 0.0001) and was associated with fewer minor bleeding episodes (3.6 ± 1.8 vs. 6.8 ± 5.2; p < 0.0001). No severe complications occurred; one case of cutaneous emphysema occurred in the WJ group, and four cases occurred in the C group ( p = 0.17), three cases of pneumonia were encountered in the C group and none in the WJ group ( p = 0.24). Treatment success (Eckardt score ≤3) was achieved in 96.5 % of patients, with no significant differences between groups. Conclusions The use of the HybridKnife leads to a significant decrease in POEM procedure time and facilitates reinjection, possibly contributing to a lower rate of minor intra-procedural bleeding.
Peroral Endoscopic Myotomy for Treating Achalasia Performed by a Gastroenterologist: 4 Yearsʼ Experience from a Single Endoscopy Center

Abstract:Introduction: Esophageal achalasia is most commonly treated with endoscopic dilation or laparoscopic Heller myotomy. Peroral endoscopic myotomy (POEM) has recently been described as a novel treatment for achalasia in humans. The aim of this study was to assess the clinical effectiveness and safety of POEM for treating esophageal achalasia performed by a gastroenterologist in a single endoscopic center within 4 years.Table 1: Baseline and clinical characteristics of patientsTable 2: Clinical outcomes of POEM procedureMethods: Between June 2011 to November 2015, POEM was performed in 153 consecutive patients with achalasia. POEM procedure consisted of the following step: firstly, submucosal tunnel was created and extended below the lower esophageal sphincter (LES) onto the gastric cardia after a mucosal incision was made; then endoscopic myotomy of circular muscle bundles was done; finally, the mucosal entry was closed by hemostatic clips (Figure 1). The Eckardt score and manometry were used to evaluate the outcomes.Figure 1Results: POEM was successfully performed in all cases. Mean procedure time was 51.3 ± 16.6 min and mean myotomy length was 8.4 ± 3.3 cm. Mucosal perforations occurred in 6 (3.9%) patients during submucosal tunnel creation, major bleeding occurred in 9 (5.9%) patients, and 7 (4.6%) patients suffered pneumothorax immediate after procedure. All the complications were managed conservatively. During a mean follow-up period of 25 months (range 6-59.4 months), 20 patients were lost, and treatment success was achieved in 124/133 patients (93.2%). Mean LES pressure was 45.1 mmHg and 15.3 mmHg before and after the procedure (P = 0.000), respectively. Mean Eckardt score was 8.3 and 1.4 before and after POEM, respectively (P = 0.000). 23 patient (17.3%) developed mild reflux symptoms and required intermittent medication with proton pump inhibitors during the follow-up. Conclusion: Our study demonstrated that POEM is a safe, and effective treatment for achalasia during the long-term follow-up. Further studies are warranted to compare POEM with other treatment modalities.

Abstract:Background/aims: Achalasia is a rare esophageal motility disorder. Recently, a novel endoscopic technique, peroral endoscopic myotomy (POEM), was introduced as an alternative treatment for achalasia. We report the results and short term outcomes of POEM for patients with achalasia. Methods: POEM was performed in 13 patients with achalasia. The procedure consisted of creating a submucosal tunnel followed by endoscopic myotomy of circular muscle bundles. The mucosal entry was closed by conventional hemostatic clips. A validated clinical symptom score (Eckardt score) and high resolution manometry were used to evaluate the outcomes. Results: Both the clinical score of achalasia, as well as the resting lower esophageal sphincter (LES) pressure, were significantly reduced after POEM. Mean posttreatment Eckardt score was 0.4±0.7, compared to 6.4±1.9 prior to the treatment (p=0.001). The mean pretreatment and posttreatment LES pressure was 30.3 and 15.3 mm Hg, respectively (p=0.007). Following POEM, symptomatic relief from dysphagia without reflux symptoms was observed in all patients (13/13). No serious complications related to POEM were encountered. Conclusions: Based upon our initial experience, the authors believe that POEM is a feasible, safe, and effective treatment and may possibly substitute established treatments of refractory achalasia.

Abstract:Background: Achalasia is a disease caused by neuromuscular dysfunction in the esophagogastric junction, with a relatively low incidence among children. Peroral endoscopic myotomy (POEM) is a new endoscopic procedure for the treatment of achalasia; however, it is rarely applied in children. Objectives: We aim to study the feasibility, safety, and effectiveness of POEM as a treatment for pediatric achalasia. Methods: A total of 21 pediatric patients (ages from 11 months to 18 years) diagnosed as having achalasia and treated with POEM from October of 2014 to October of 2016 in our hospital were included in our study. Indices such as clinical manifestations, nutritional status, Eckardt scores, high-resolution esophageal manometric measurements, gastroduodenoscopic findings, and upper gastroenterographic findings after treatment were respectively compared with those before treatment to analyze the effectiveness and safety of POEM. Results: All of the 21 children successfully received POEM. The patients were followed up for duration ranged from 3 to 24 months. Among these children, symptoms such as vomiting and dysphagia were significantly alleviated or resolved. The Eckardt scores had an average drop of 7 points after treatment compared with those before treatment. Postoperative gastroduodenoscopy and upper gastroenterography showed that the opening of the cardiac orifice was significantly enlarged and obstruction at the lower esophagus was significantly relieved. Conclusions: POEM is a mini-invasive technique and a promising new treatment for pediatric patients, making it worthy of further clinical research and application.

Abstract:Achalasia is a primary esophageal motility disorder characterized by a hypertensive non-relaxing lower oesophageal sphincter (LES) and absence of esophageal body peristalsis. Therapeutic options for achalasia include pharmacologic therapy, endoscopic balloon dilation, endoscopic injection of botulinum toxin, surgical thoracotomy, thoracoscopic or laparoscopic myotomy. Abstract:Background Peroral endoscopic myotomy (POEM) is a minimally invasive yet challenging procedure for achalasia. Additional technological innovations and improvements are important for simplifying the procedure. Methods We report the successful use of a modified POEM procedure, which utilized a “Push and Pull” technique, on a patient with achalasia. Results Our modifications resulted in a short operation time of only 35 min. No complications arose during or after the procedure, up to a follow-up period of 6 months, and symptoms were significantly and quickly improved. Conclusions This modified procedure shortens operation time and lowers the difficulty of the operation, while leaving the safety and efficacy uncompromised.

Abstract:We present a case report of peroral endoscopic myotomy (POEM) in the treatment of pediatric patient with achalasia and annular stenosis in lower esophagus. A 9-year-old girl was newly diagnosed with achalasia. After assessment, the patient underwent POEM procedure and the retrievable stent was implanted in the esophagus. Postoperatively, lung CT suggested esophagopleural fistula, while gastroscopy showed that the metal stent was displaced to the esophageal lumen. A fistula was found in the lower esophageal segment after the stent was removed. Emergency operation of endoscopic perforation repair and esophageal stent replacement were performed. The patient recovered well in the next 6 months of follow-up.

Abstract:Objective:To solve the symptoms of achalasia complicated with osteogensis imperfecta through peroral endoscopic myotomy(POEM).Methods: The 21-year-old male patient,being diagnosed as achalasia complicated with osteogensis imperfecta,underwent POEM under general anesthesia after preoperative assessment of achalasia(Eckardt score of 7).The mucosal incision was made at a distance of 28 cm from the incisor,then the gastroscope entered the submucosal layer and tunnel in it,and myotomy was performed about 29 cm from the incisor extending 2 cm into the cardia.Electrocoagulation was performed to stop bleeding and the mucosal incision was then closed.Results:The operation was completed successfully in about 28 minutes.The stomach tube was removed 30 h after surgery.The patient was discharged in a stable condition on the fourth postoperative day.The follow-up result showed no recurrence in symptom with Eckardt score of 0 two weeks after operation.Conclusions:POEM is a feasible and effective way for achalasia of cardia complicated with osteogenesis imperfecta.
