Abstract:Objective: To compare the outcome of early cholecystectomy and delayed cholecystectomy in patients with gallstone pancreatitis.Study design: Randomized clinical trial.Setting: Department of East Surgical Ward, Mayo Hospital, LahoreDuration of study: Nine monthsMethodology: Total 62 patients; 12 to 60 years of age, prepared for general anesthesia and open cholecystectomy were analyzed for the study. The patients were distributed into two groups by random assignment. Group A (early cholecystectomy), Group B (delayed cholecystectomy). The clinical variables were evaluated statistically.Results: The male to female ratio was 1: 5.2. In Group A the average hospital stay was significantly less than in Group B, (p value= 0.000). The morbidity and mortality was statistically insignificant in both groups (p-values 0.185 & 0.238 respectively).Conclusion: Early cholecystectomy has a better outcome as compared to delayed cholecystectomy for gallstone pancreatitis.
Clinical Effects of Early Laparoscopic Cholecystectomy for the Patients with Acute Pancreatitis with Gallbladder Stone

Abstract:Objective To study the feasibility and timing of early laparoscopic cholecystectomy (LC) for the patients with mild-to-moderate acute pancreatitis with gallbladder stone.Methods 26 patients with mild-to-moderate acute pancreatitis with gallbladder stone underwent early laparoscopic cholecystectomy.The patients were initially treated with conservative treatment.When the diagnosis was confirmed and the patients were stable,LC was carried out within 48 hours of hospitalization.The results were compared with a delayed group of patients who received delayed LC for acute pancreatitis.Results All the LC operations were successfully carried out.There were no significant differences between the two groups in alkaline phosphatase,alanine aminotransferase,aspartate aminotransferase,blood amylase and urine armylase on postoperative day 1,3,5,7.Patients who received early laparoscopic cholecystectomy had a shorter hospital stay.There were no significant differences between the two groups in the operation time,intraoperative blood loss,postoperative complication rates and recurrence of acute pancreatifis.Conclusion After CBD stones have been ruled out,it is safe and feasible to carry out early laparoscopic cholecystectomy within 48 hours of hospital admission for patients with mild-to-moderate acute pancreatitis with gallbladder stone.

Abstract:Background: Cholecystectomy has been a subject of debate regarding its timing and utility in cases of mild and moderately severe acute pancreatitis (AP). We aimed to critically evaluate the role of early cholecystectomy in the management of mild and moderate AP, considering patient's characteristics, associated procedures, and overall impact on patient outcomes. Methods: The study compared the outcomes between patients admitted in a tertiary care surgical center undergoing early ( 96h) versus delayed ( 96h) laparoscopic cholecystectomy (LC) for mild and moderately severe acute gallstone pancreatitis between January 2019 and December 2022. Results: The study included 54 cases [mean (standard deviation) age, 59.4 (16.5) years; 31 (57.4%) years females]. All patients underwent LC, with 29 cases undergoing a two-phase therapeutic regimen for common bile duct (CBD) lithiasis, consisting of endoscopic retrograde cholangiopancreatography followed by sequential LC. The early cholecystectomy group (EC) comprised 17 patients (31.5%), while the delayed cholecystectomy group (DC) included 37 patients (68.5%). EC was significantly correlated with lower length of stay (p-value 0.0001) and significantly lower rate of ERCP usage during perioperative period. Conclusions: EC in the first 4 days after admission provides significant benefits such as prevention of recurrent pancreatitis, reduction in complications, and decreased length of stay for patients with mild and moderately severe AP.

Early versus Delayed Laparoscopic Cholecystectomy for Mild Acute Biliary Pancreatitis: A Systematic Review and Meta-Analysis

Abstract:Abstract Background Laparoscopic cholecystectomy (LC) is widely accepted as the definitive therapeutic approach for MABP, primarily aiming to reduce recurrent biliary complications. However, the optimal timing of LC remains a matter of debate. The aim of this study was to evaluate the efficacy and safety of early laparoscopic cholecystectomy (ELC) versus delayed laparoscopic cholecystectomy (DLC) in patients with mild acute biliary pancreatitis (MABP). Methods The PubMed, Embase, Scopus, and CENTRAL databases were systematically searched for randomized clinical trials (RCTs) comparing ELC with DLC in patients with MABP from inception to August 1, 2023. The primary outcome was recurrent biliary events. Secondary outcomes included postoperative complications, readmissions, and length of stay (LOS). The pooled risk ratio (RR) or standardized mean difference (SMD) of data was calculated using the random-effects model. Results Fifteen RCTs with 1616 patients enrolled between 2010 and 2022 were included. High-certainty evidence from nine RCTs demonstrated a significantly lower risk of recurrent biliary events in patients who underwent ELC during the waiting time compared to DLC (RR=0.128[0.063, 0.262], P <0.001). For secondary outcomes, ELC in patients with MABP was associated with a significantly lower readmission rate (RR=0.382[0.182, 0.801], P =0.011) and shorter LOS (SMD=-2.026[-2.854, -1.198], P <0.001) than DLC. Subgroup analysis indicated that patients with ELC had a significantly lower rate of postoperative complications in accordance with the Atlanta criteria, MABP, and >4 weeks subgroup. Conclusions ELC emerges as a generally safe and effective strategy in reducing recurrent biliary events, postoperative complications, readmission, and length of stay for patients with MABP.

Timing of Cholecystectomy for Acute Calculous Cholecystitis: A Meta-Analysis

Abstract:OBJECTIVES: To compare early with delayed cholecystectomy for the treatment of acute lithiasic cholecystitis: a meta-analysis of prospective randomized trials.METHODS: Pertinent studies were selected from the Medline, Embase, Cancerlit, HealthSTAR and Cochrane Library Databases, references from published articles, and reviews. Twelve prospective randomized trials (9 addressing open cholecystectomy and 3 laparoscopic cholecystectomy) were selected. Conventional meta-analysis according to the DerSimonian and Laird method was used for the pooling of the results. The rate difference (RD) (95% CI) and the number needed to treat (NNT) were used as a measure of the therapeutic effect.RESULTS: Cumulative operative and perioperative mortality and morbidity were 0.9% and 17.8%, respectively, for open cholecystectomy and 0% and 13.1%, respectively, for laparoscopic cholecystectomy. The pooled RD for operative complications in early surgery was 1.37% (95% CI =-3.78% to 6.53%; p= 0.2) for open cholecystectomy and 3.11% (95% CI =-15.10% to 8.87%; p= 0.6) for laparoscopic cholecystectomy. In laparoscopic cholecystectomy the cumulative conversion rate to open cholecystectomy was 21.5%. The pooled RD for conversion rate in early laparoscopic cholecystectomy was -7.99% (95% CI =-18.46% to 2.47%; p= 0.1; NNT = 13). Total hospital stay (mean +/- SD) was significantly shorter in the early surgery group (9.6 +/- 2.5 days vs 17.8 +/- 5.8 days; p < 0.0001). More than 20% of patients referred to delayed surgery fail to respond to conservative management or suffer recurrent cholecystitis in the interval period.CONCLUSIONS: Early operation (open or laparoscopic) does not carry a higher risk of mortality and morbidity compared to delayed operation and should be the preferred surgical approach for patients with acute lithiasic cholecystitis.

Laparoscopic cholecystectomy for acute pancreatitis accompanying gallstones

Abstract:Objective To investigate the feasibility and timing of laparoscopic cholecystectomy(LC) for the treatment of acute pancreatitis accompanying gallstones. Methods A retrospective analysis was made on clinical data of 21 patients with acute pancreatitis accompanying gallstones treated by LC between January 1999 and August 2003 in this hospital. Results The LC was performed 20~40 days after the onset of pancreatitis and 15 days after the recovery of serum and urinary amylase levels.An abdominal drainage tube was placed in 9 patients.Postoperative recovery was uneventful in 19 patients,who got out of bed on the surgery day or on the first postoperative day and began to take food 12 hours after operation,with the abdominal drainage tube removed 24~72 hours postoperatively and no complications encountered.Recurrence of acute pancreatitis happened in 2 patients on the 3 postoperative days and was relieved from symptoms with conservative treatment.The duration of postoperative hospital stay was 4~20 days(mean,7 days).Follow-up checkups in the 21 patients for 6 months ~ 5 years(mean,19 months) found no recurrence. Conclusions Laparoscopic cholecystectomy is feasible and safe for patients with acute pancreatitis accompanying gallstones in the period of 20~40 days after the onset of pancreatitis.Careful patient selection,proper peri-operative treatment,strict adherence to techniques,and use of postoperative drainage are considered essential.
