Abstract:The role of the cortex in shaping automatic whole-body motor behaviors such as walking and balance is poorly understood. Gait and balance are typically mediated through subcortical circuits, with the cortex becoming engaged as needed on an individual basis by task difficulty and complexity. However, we lack a mechanistic understanding of how increased cortical contribution to whole-body movements shapes motor output. Here we use reactive balance recovery as a paradigm to identify relationships between hierarchical control mechanisms and their engagement across balance tasks of increasing difficulty in young adults. We hypothesize that parallel sensorimotor feedback loops engaging subcortical and cortical circuits contribute to balance-correcting muscle activity, and that the involvement of cortical circuits increases with balance challenge. We decomposed balance-correcting muscle activity based on hypothesized subcortically- and cortically-mediated feedback components driven by similar sensory information, but with different loop delays. The initial balance-correcting muscle activity was engaged at all levels of balance difficulty. Its onset latency was consistent with subcortical sensorimotor loops observed in the lower limb. An even later, presumed, cortically-mediated burst of muscle activity became additionally engaged as balance task difficulty increased, at latencies consistent with longer transcortical sensorimotor loops. We further demonstrate that evoked cortical activity in central midline areas measured using electroencephalography (EEG) can be explained by a similar sensory transformation as muscle activity but at a delay consistent with its role in a transcortical loop driving later cortical contributions to balance-correcting muscle activity. These results demonstrate that a neuromechanical model of muscle activity can be used to infer cortical contributions to muscle activity without recording brain activity. Our model may provide a useful framework for evaluating changes in cortical contributions to balance that are associated with falls in older adults and in neurological disorders such as Parkinson's disease. Our goal is to gain a mechanistic understanding of how cortical and subcortical systems shape motor output in whole-body movements. Using novel neuromechanical models, we identify how cortical and subcortical control mechanisms differentially contribute to balance-correcting muscle activity across reactive balance task difficulty. Bursts of balance-correcting muscle activity in young adults were decomposed into parallel sensorimotor feedback components driven by balance error but acting at different loop latencies. Corroborating the transcortical nature of the longer latency loop, we showed that evoked cortical responses to perturbations were also driven by balance error. While the contributions of the faster subcortical loop to muscle activity was present in all perturbation levels, the slower cortically-mediated burst of muscle was increasingly engaged with difficulty level. Our computational framework thus dissociates cortical and subcortical muscle activity components of balance control which may be useful to inform balance training and rehabilitation.

Effects of Acute Muscle Fatigue on the Feed-forward Motor Control of Lumbar Multifidus

Abstract:Objective: To investigate the effects of psychological expectation and local muscle fatigue on the feed-forward motor control of lumbar multifidus by means of combination of Ball-hitting Test and surface electromyography signal analysis.Method: 10 healthy male volunteers aged 18-26 participated in this study.A double factor experiment was designed in which subjects performed Ball-hitting Test both before and after finishing intermittent loading tests evoking isometric contraction of MF to fatigue.The sEMG signals of biceps brachii and lumbar multifidus(L5～S1) were sampled.The onset of the sEMG signals to the ball-hitting stimulation and average response intensity were calculated.The latency and the amplitude were surveyed of the acute muscle fatigue and subjective psychological expectation on targeted muscles.Results:(1) The MPF of lumbar MF declined 18%.(2)Psychological expectation accelerated the onset of lumbar MF significantly.(3)There was a tendency that the response of lumbar MF to Ball-hitting Test became faster under the circumstance of fatigue,though not significantly.(4)Psychological expectation could reduce the average amplitude of lumbar MF while fatigue couldn’t.Conclusion: Psychological expectation can shorten the latency of lumber MF which provides compensatory protection to lumbar stability.Acute muscle fatigue doesn’t change the reaction time and intensity of lumber MF,demonstrating that the feed-forward control of CNS over the activity of lumber postural muscles is stable.

Abstract:The active control of the lumbar musculature provides a stable platform critical for postures and goal-directed movements. Voluntary and perturbation-evoked motor commands can recruit individual lumbar muscles in a task-specific manner according to their presumed biomechanics. Here, we investigated the vestibular control of the deep and superficial lumbar musculature. Ten healthy participants were exposed to noisy electrical vestibular stimulation while balancing upright with their head facing forward, left or right to characterize the differential modulation in the vestibular-evoked lumbar extensor responses in generating multidirectional whole-body motion. We quantified the lumbar muscles activation on the right side using indwelling (deep multifidus, superficial multifidus, caudal longissimus (L4) and cranial longissimus (L1)) and high-density surface recordings. We characterized the vestibular-evoked responses using coherence and peak-to-peak cross-covariance amplitude between the vestibular and electromyographic signals. Participants exhibited responses in all lumbar muscles. The vestibular control of the lumbar musculature exhibited muscle-specific modulations: responses were larger in the longissimus (combined cranio-caudal) compared to the multifidus (combined deep-superficial) when participants faced forward (p<0.001) and right (p=0.011) but not when they faced left. The high-density surface recordings partly supported this observation: the location of the responses was more lateral when facing right compared to left (p<0.001). The vestibular control of muscle sub-regions within the longissimus or the multifidus was similar. Our results demonstrate muscle-specific vestibular control of the lumbar muscles in response to perturbations of vestibular origin. The lack of differential activation of lumbar muscle sub-regions suggests the vestibular control of these sub-regions is co-regulated for standing balance.

Abstract:Objective: To examine the triggering causes of inadequate neuromuscular regulation of posture and subjective imbalance in patients with phobic postural vertigo (PPV), a subtype of functional dizziness. Methods: Postural performance was assessed by center-of-pressure displacements and surface EMG of lower-limb muscles (the tibialis anterior and soleus) in 10 patients with PPV and 10 healthy controls under 4 stance conditions: standing with eyes open or closed and with or without an additional cognitive dual task. The level of muscle cocontraction and the characteristics of open- and closed-loop postural control were analyzed. Results: At baseline (i.e., standing with eyes open without dual task), patients exhibited increased muscle cocontractions (p = 0.003), which were further associated with increased open-loop diffusion activity (p = 0.022) and a lowering of the primary feedback threshold for closed-loop control (p = 0.003). However, postural performance of patients improved considerably and normalized to that of healthy controls when performing an additional dual task. Conclusions: PPV is characterized by a dissociation of subjective postural instability and objectively maintained balance capabilities. The dual-task effects on balance in patients with PPV indicate that this dissociation might result from an increased attention to postural adjustments at baseline, which is normally required only during demanding balance situations. This internal focus on balance control promotes an inappropriate neuromuscular regulation of posture, with increased muscle cocontractions, higher short-term body sway, and an oversensitivity to external stimuli. However, if patients are distracted, muscle cocontractions and balance control normalize. Such distraction may therefore be an effective coping strategy for preventing PPV attacks in susceptible patients.
Brain connectivity during simulated balance in older adults with and without Parkinson's disease

Abstract:Individuals with Parkinson's disease often experience postural instability, a debilitating and largely treatment-resistant symptom. A better understanding of the neural substrates contributing to postural instability could lead to more effective treatments. Constraints of current functional neuroimaging techniques, such as the horizontal orientation of most MRI scanners (forcing participants to lie supine), complicates investigating cortical and subcortical activation patterns and connectivity networks involved in healthy and parkinsonian balance control. In this cross-sectional study, we utilized a newly-validated MRI-compatible balance simulator (based on an inverted pendulum) that enabled participants to perform balance-relevant tasks while supine in the scanner. We utilized functional MRI to explore effective connectivity underlying static and dynamic balance control in healthy older adults (n = 17) and individuals with Parkinson's disease while on medication (n = 17). Participants performed four tasks within the scanner with eyes closed: resting, proprioceptive tracking of passive ankle movement, static balancing of the simulator, and dynamic responses to random perturbations of the simulator. All analyses were done in the participant's native space without spatial transformation to a common template. Effective connectivity between 57 regions of interest was computed using a Bayesian Network learning approach with false discovery rate set to 5%. The first 12 principal components of the connection weights, binomial logistic regression, and cross-validation were used to create 4 separate models: contrasting static balancing vs {rest, proprioception} and dynamic balancing vs {rest, proprioception} for both controls and individuals with Parkinson's disease. In order to directly compare relevant connections between controls and individuals with Parkinson's disease, we used connections relevant for predicting a task in either controls or individuals with Parkinson's disease in logistic regression with Least Absolute Shrinkage and Selection Operator regularization. During dynamic balancing, we observed decreased connectivity between different motor areas and increased connectivity from the brainstem to several cortical and subcortical areas in controls, while individuals with Parkinson's disease showed increased connectivity associated with motor and parietal areas, and decreased connectivity from brainstem to other subcortical areas. No significant models were found for static balancing in either group. Our results support the notion that dynamic balance control in individuals with Parkinson's disease relies more on cortical motor areas compared to healthy older adults, who show a preference of subcortical control during dynamic balancing.

Abstract:Fluctuations in brain state alter how we perceive our body and generate movements but have not been investigated in functional whole-body behaviors. During reactive balance control, we recently showed that evoked brain activity is associated with balance ability in healthy young individuals. Further, in individuals with Parkinson's disease, impairments in whole-body motion perception in reactive balance are associated with clinical balance impairment. Here we investigated brain activity during whole-body motion perception in reactive balance in healthy young adults. We hypothesized that flexibility in brain states underlies successful perception and movement during whole-body movement. We characterized two cortical sensorimotor signals using electroencephalography localized to the supplementary motor area: 1) the "N1 response", a perturbation-evoked potential that decreases in amplitude with expectancy and is larger in individuals with lower balance function; and 2) pre-perturbation beta oscillatory activity, a rhythm that favors maintenance of the current sensorimotor state and is inversely associated with perception in seated somatosensory perceptual tasks. In a two-alternative forced choice task, participants judged whether pairs of backward support-surface perturbations during standing were in the "same" or "different" direction. As expected, lower whole-body perception was associated with lower balance ability. Within a perturbation pair, N1 attenuation was larger on correctly perceived trials and associated with better balance, but not perception. In contrast, pre-perturbation beta power was higher on incorrectly perceived trials and associated with poorer perception, but not balance. Taken together, flexibility in different cortical processes influences perceptual accuracy but have distinct associations with balance and perceptual ability.
Age- and Parkinson-related differences in trunk and lower-limb muscle activation during four balance tasks: A case-control pilot study

Abstract:Background Poor postural control has been reported in people with Parkinson's disease, which could be explained by the changes in muscular activation patterns related to antigravitational muscles. This study aims to measure the muscle activation of antigravitational muscles during balance tasks in individuals, with and without Parkinson's. Methods Sixteen (16) participants (9 with Parkinson's), aged ≥65 yrs., performed 2 × 30-s trials of 4 balance tasks (bipodal and semi-tandem opened eyes and closed eyes) on a force platform (center of pressure measurement); while surface electromyography measurements were obtained bilaterally on the multifidus at L5, biceps femoris and medialis gastrocnemius. Electromyography amplitude analysis was processed by the Root Mean Square (250 ms window epochs) and normalized by the peak of activation during the balance task, to determine each muscle's activity level. Findings The Parkinson's group reported lower muscle activation than control across tasks (in mean for multifidus = 8%, biceps femoris = 16%, gastrocnemius = 7%), although not statistically significant. Parkinson's reported significantly poorer postural control than control, mainly for the center of pressure sway ellipse area ( p = 0.016) from challenge balance tasks such as semi-tandem. Interpretation Poor postural control was confirmed in the Parkinson's group, but not significantly associated by the changes from muscle activation of trunk and lower limbs, during balance performance.

Training Postural Balance Control with Pelvic Force Field at the Boundary of Stability

Abstract:This study characterizes the effects of a postural training program on balance and muscle control strategies in a virtual reality (VR) environment. The Robotic Upright Stand Trainer (RobUST), which applies perturbative forces on the trunk and assistive forces on the pelvis, was used to deliver perturbation-based balance training (PBT) in a sample of 10 healthy participants. The VR task consisted of catching, aiming, and throwing a ball at a target. All participants received trunk perturbations during the VR task with forces tailored to the participant's maximum tolerance. A subgroup of these participants additionally received assistive forces at the pelvis during training. Postural kinematics were calculated before and after RobUST training, including (i) maximum perturbation force tolerated, (ii) center of pressure (COP) and pelvic excursions, (iii) postural muscle activations (EMG), and (iv) postural control strategies (the ankle and hip strategies). We observed an improvement in the maximum perturbation force and postural stability area in both groups and decreases in muscle activity. The behavior of the two groups differed for perturbations in the posterior direction where the unassisted group moved towards greater use of the hip strategy. In addition, the assisted group changed towards a lower margin of stability and higher pelvic excursion. We show that training with force assistance leads to a reactive balance strategy that permits pelvic excursion but that is efficient at restoring balance from displaced positions while training without assistance leads to reactive balance strategies that restrain pelvic excursion. Patient populations can benefit from a platform that encourages greater use of their range of motion.
Lower extremity muscle activity during reactive balance differs between adults with chronic traumatic brain injury and controls

Abstract:Background: Control of reactive balance is key to achieving safe independent walking and engagement in life activities. After traumatic brain injury (TBI), motor impairments and mobility challenges are persistent sequelae. To date, no studies have explored muscle activity of individuals with chronic TBI during a task that requires reactive control of balance. Objective: To investigate lower extremity muscle activity during a reactive balance test performed by adults with chronic severe TBI and matched controls. We hypothesized that abnormal activity of lower extremity muscles would be related with poorer reactive balance performance. Also, we performed an exploratory analysis for those with TBI investigating the impact of unilateral versus bilateral lower extremity involvement in the control of reactive balance. Methods: Ten adults with chronic severe TBI who were independent community ambulators and ten matched controls performed the computerized reactive balance test (Propriotest®) while lower extremity muscle activity was recorded. Electromyographic (EMG) activity was contrasted (Mann-Whitney U Test) between groups across each 10 s epoch of the 120 s test. Additionally, test scores were correlated (Spearman) with lower extremity composite EMG activity to distinguish muscle activity patterns related with reactive balance performance. Lastly, reactive balance test scores were correlated with reactive balance test scores and clinical functional measures only for the TBI group. Results: Although the TBI group exhibited greater EMG activity across the entire test compared with the control group, significant differences were not observed. Greater composite EMG activity correlated significantly with poorer reactive balance performance across most of the 10 s windows of the test. Conclusion: Greater muscle activity exhibited during the reactive balance test by individuals with chronic severe TBI compared to those without disabilities, particularly at small unexpected perturbations, highlights the greater physiologic effort required to control reactive balance even after independent ambulation is achieved.

Abstract:The control of posture is guided by the integration of sensory information. Because blind individuals cannot apply visual information to control posture as sighted individuals do they must compensate by the remaining senses. We therefore hypothesize that blind individuals alter their brain activation in the sensorimotor cortex during postural control to compensate for balance control without vision by the increased integration of somatosensory information. Ten blind and ten sighted (matched) individuals controlled posture during conditions with (I) eyes closed / open, and (II) stable / unstable surface conditions. Postural sway was recorded by applying a pressure distribution measuring plate. Brain activation was collected by functional Near InfraRed Spectroscopy (fNIRS) above motor-sensory cortices of the right and left hemispheres. Blind individuals showed significantly increased postural sway when balancing with open eyes on an unstable surface and when compared to sighted individuals. Whereas blind individuals showed significantly increased brain activation when balancing with open eyes on stable and unstable surface conditions, sighted individuals increased their brain oxygenation only during closed eyes and unstable surface conditions. Overall conditions, blind individuals presented significantly increased brain activation in two channels of the left and right hemispheric motor-sensory cortex when compared to sighted individuals. We therefore conclude that sighted individuals increase their brain oxygenation in the sensorimotor cortex during postural control tasks that demand sensory integration processes. Blind individuals are characterized by increased brain activation overall conditions indicating additional sensory integration during postural control. Thus, the sensorimotor cortex of blind individuals adapts to control posture without vision.

Characterization of Human Balance through a Reinforcement Learning-based Muscle Controller

Abstract:Balance assessment during physical rehabilitation often relies on rubric-oriented battery tests to score a patient's physical capabilities, leading to subjectivity. While some objective balance assessments exist, they are often limited to tracking the center of pressure (COP), which does not fully capture the whole-body postural stability. This study explores the use of the center of mass (COM) state space and presents a promising avenue for monitoring the balance capabilities in humans. We employ a musculoskeletal model integrated with a balance controller, trained through reinforcement learning (RL), to investigate balancing capabilities. The RL framework consists of two interconnected neural networks governing balance recovery and muscle coordination respectively, trained using Proximal Policy Optimization (PPO) with reference state initialization, early termination, and multiple training strategies. By exploring recovery from random initial COM states (position and velocity) space for a trained controller, we obtain the final BR enclosing successful balance recovery trajectories. Comparing the BRs with analytical postural stability limits from a linear inverted pendulum model, we observe a similar trend in successful COM states but more limited ranges in the recoverable areas. We further investigate the effect of muscle weakness and neural excitation delay on the BRs, revealing reduced balancing capability in different regions. Overall, our approach of learning muscular balance controllers presents a promising new method for establishing balance recovery limits and objectively assessing balance capability in bipedal systems, particularly in humans.

Abstract:This short narrative review article moves from early papers that described the behaviour of healthy subjects balancing on a motorized platform continuously translating in the antero-posterior direction. Research from the laboratories of two of the authors and related investigations on dynamic balancing behaviour are briefly summarized. More recent findings challenging time-honoured views are considered, such as the statement that vision plays a head-in-space stabilizing role. The time interval to integrate vision or its withdrawal in the balancing pattern is mentioned as well. Similarities and differences between ageing subjects and patients with peripheral or central disorders are concisely reported. The muscle activities recorded during the translation cycles suggest that vision and amplitude changes of the anticipatory postural activities play a predominant role in controlling dynamic balance during prolonged administration of the predictable perturbation. The potential of this paradigm for rehabilitation of balance problems is discussed.
Physiology and pathophysiology of basalganglia: Impact on motor system function

Abstract:Non-pyramidal muscular initiation by/of basalganglia differs from pyramidal, central, and spinal motor system regulation, but their synergistic cooperation increases the efficiency of central muscular motor functions. Corticostriatal pathways of neurons from motor areas in cortex excite by glutamate striatal GABA(gamma-aminobutyric acid)ergic neurons via their receptors, causing the release of the inhibitory neurotransmitter GABA. Over direct and indirect projections GABAergic impulses reach several nuclei of basalganglia, while GABAergic impact is modulated by dopamine (DA) through inhibitory or excitatory DA receptor subtypes. Adequate DA release from nigrostriatal pathways of DAergic neurons of substantia nigra is essential for well adjusted propagation to motor exits of basalganglia. From basalganglia exit projections are forwarded to motor nuclei of thalamus whose basal activity can be attenuated by inhibition or increased by disinhibition. Impulses are projected from thalamus to motor areas in cortex, in which information on the conducted circuit through basalganglia is returned to basalganglia by corticostriatal loops, adapting the following projections from basalganglia according to the need. Simultaneously, thalamic output is projected via supplementary motor area to primary cortex and, thus, may also affect centrifugal pyramidal output. In addition to basalganglic motor pathways via thalamus to motor cortex, there also exist projections from substantia nigra reticularis to several areas of motor nuclei in the brain stem, which influence spinal motor system function, especially that of the γ-motoneuron regulating extention of muscle spindle. Understanding of the physiological function of basalganglia has been immensely enriched by clarification of the pathophysiology of Parkinson's disease (PD) and its therapy. PD is characterized by resting tremor, rigidity, bradykinesia or slowness, gait disturbance, and postural instability. Hypokinetic, rigid syndrome of PD is caused by striatal disinhibition after degeneration of DAergic neurons in the substantia nigra pars compacta, which are essential for the control of motor performance. The cardinal symptomatology of the above mentioned motor system impairments in PD is also affected by the changes in motor behavior, particularly in the case of implicit learning and memory dysfunctions in striatum. In addition to the implicit deficit, cerebral dementia increases with duration of PD. Thus, explicit learning and memory function declines. Especially in the case of increasing bradyphrenia, anticipation of prefrontal association cortex as well as its propagation via frontal-striatal pathways to associative sensomotor and motor areas with connection to motor cortex area 4, where centrifugal pyramidal projections originate, are disturbed. This process promotes the reduction of motor anticipative behavior in cortex and impairs the continuation of proper motor movements as well as their automatization. During execution of movements additional impairment of fluctuation in centripetal as well as centrifugal impulses cannot be corrected volitionally or by reflex in PD. They generally intensify the decline of non-pyramidal and pyramidal synergistic cooperation. With increasing cardinal symptoms in PD, consequent specific medicamentous or invasive treatments are required. The systematic addressing of motor behavior in special reeducation programs can positively influence motor system functions. Particularly tremor and rigor, which are pronounced in the case of implicit motor system dysfunction, can partly be improved by increased explicit learning and memory function, supported by explicit mental movement initiation. Special training of prefrontal associations and a consecutive increase of central-motor behavior associations can also reduce cerebral automatization deficits. By this, even in long existing PD, non-pyramidal motor impairments can be affected and striatal dysfunctions symptomatically improved. Each motor input and output level and their central representation has to be evaluated individually and be corrected in this learning state, so that the summation of mistakes in the final phase is kept as small as possible. Thus, special didactical learning strategies should be applied with frequent controls.

Abstract:Spastic CP is characterized by four interrelated neuromuscular impairments, two that result from reduced descending neural activation: weak muscle and reduced muscle growth relative to skeletal growth resulting in short muscle and joint contracture; as well as two neuromuscular impairments that result from reduced neural inhibition: muscle spasticity characterized by increased sensitivity to stretch, and impaired selective motor control characterized by flexion and extension synergies that interfere with voluntary movements. (Zhou et al., 2017). Mirror movements also impact motor control and are thought to result from increased dependence on preserved motor pathways, (Hruby A et. al., 2023).While interventions for spastic CP often focus on tone, mobility is more often limited by weak and short muscle due to reduced neural activation and muscle growth. Further, short muscles, especially two-joint muscles, are particularly vulnerable to spasticity, the increased sensitivity to stretch as the muscle extends over both joints. Pain, illness, inflammation and anxiety can be misdiagnosed and treated as 'increased spasticity', especially in individuals who have limited mobility and limited communication, highlighting the need for improved health monitoring and exercise. Misunderstanding interrelations between neuromuscular impairments such as short muscle and muscle spasticity, leads to imprecise diagnosis, over treating muscle spasticity and under recognition of the impacts of sedentary life and reduced muscle growth on mobility. For children with spastic CP, muscle growth is inadequately monitored in the clinic and warrants focused clinical research. Current treatments for joint contracture, such as surgical tendon lengthening and skeletal realignment address musculoskeletal deformities that are years in the making, and therefore, have suboptimal outcomes. Early improved treatments for the underlying neuromuscular impairments of muscle weakness and reduced muscle growth could normalize musculoskeletal growth and prevent joint contracture and skeletal malalignment, minimizing the need for surgery. Analysis of muscle activation in lower extremity for static balance

Abstract:Balance plays an important role for human bipedal locomotion. Degeneration of balance control is prominent in stroke patients, elderly adults and even for majority of obese people. Design of personalized balance training program, in order to strengthen muscles, requires the analysis of muscle activation during an activity. In this paper we have proposed an affordable and portable approach to analyze the relationship between the static balance strategy and activation of various lower extremity muscles. To do that we have considered Microsoft Kinect XBox 360 as a motion sensing device and Wii balance board for measuring external force information. For analyzing the muscle activation pattern related to static balance, participants are asked to do the single limb stance (SLS) exercise on the balance board and in front of the Kinect. Static optimization to minimize the overall muscle activation pattern is carried out using OpenSim, which is an open-source musculoskeletal simulation software. The study is done on ten normal and ten obese people, grouped according to body mass index (BMI). Results suggest that the lower extremity muscles like biceps femoris, psoas major, sartorius, iliacus play the major role for both maintaining the balance using one limb as well as maintaining the flexion of the other limb during SLS. Further investigations reveal that the higher muscle activations of the flexed leg for normal group demonstrate higher strength. Moreover, the lower muscle activation of the standing leg for normal group demonstrate more headroom for the biceps femoris-short-head and psoas major to withstand the load and hence have better static balance control.

Abstract:The pelvic floor functions as a holistic entity. The organs, bladder, bowel, smooth and striated muscles, nerves, ligaments and other connective tissues are directed cortically and reflexly from various levels of the nervous system. Such holistic integration is essential for the system's multiple functions, for example, pelvic girdle stability, continence, voiding/defecation, and sexuality. Pelvic floor dysfunction (PFD) is related to a variety of pelvic pain syndromes and organ problems of continence and evacuation. Prior to treatment, it is necessary to understand which part(s) of the system may be causing the dysfunction (s) of Chronic Pelvic Pain Syndrome (CPPS), pelvic girdle pain, sexual problems, Lower Urinary Tract Symptoms (LUTS), dysfunctional voiding, constipation, prolapse and incontinence. The interpretation of pelvic floor biomechanics is complex and involves multiple theories. Non-surgical treatment of PFD requires correct diagnosis and correctly supervised pelvic floor training. The aims of this review are to analyze pelvic function and dysfunction. Because it is a holistic and entirely anatomically based system, we have accorded significant weight to the Integral Theory's explanations of function and dysfunction.
Bimanual force control strategies in chronic stroke: finger extension versus power grip

Abstract:Stroke leads to motor asymmetries in the flexor and extensor muscles of the hand. Typically, the strength deficits in the extensors are greater than the flexors. The impact of differential motor abilities of these muscle groups on the execution of bimanual force control tasks in individuals with stroke is unknown. The primary purpose of this study was to determine the influence of task constraints on visually guided bimanual force control in chronic stroke. Stroke survivors and age-matched individuals performed bimanual isometric contractions for 20s to match target submaximal force levels. Online visual feedback of the total force (sum of the forces produced by both hands) was provided. The task constraints were manipulated by (a) finger extension, and (b) finger flexion (power grip). Force asymmetry was indexed by the proportion of force contributed by the paretic hand to the total force. The stroke group demonstrated task-specific asymmetry in bimanual force control. Specifically, the paretic hand contributed less force than the non-paretic hand in finger extension whereas this relationship was reversed in power grip. Importantly, regardless of the nature of the task, reduction in motor impairments was associated with increased symmetry and coordination in bimanual tasks. Further, bimanual submaximal grip force control revealed asymmetry and coordination deficits that are not identified by investigating bimanual maximal force production alone. The motor control strategy adopted to optimize performance on bimanual tasks revealed the altered force production of the paretic hand due to the combined effect of extensor weakness and enhanced flexor bias following stroke. Bimanual asymmetries in stroke survivors highlight the need for identifying and treating the task-specific impairments for maximizing motor recovery post stroke.

Abstract:Despite the abundance of studies on the control of standing balance, insights about the roles of biomechanics and neural control have been limited. Previous work introduced an analysis combining the direction and orientation of ground reaction forces. The “intersection point” of the lines of actions of these forces exhibited a consistent pattern across healthy, young subjects when computed for different frequency components of the center of pressure signal. To investigate the control strategy of quiet stance, we applied this intersection point analysis to experimental data of 15 healthy, young subjects balancing in tandem stance on a narrow beam and on the ground. Data from the sagittal and frontal planes were analyzed separately. The task was modeled as a double-inverted pendulum controlled by an optimal controller with torque-actuated ankle and hip joints and additive white noise. To test our prediction that the controller that minimized overall joint effort would yield the best fit across the tested conditions and planes of analyses, experimental results were compared to simulation outcomes. The controller that minimized overall effort produced the best fit in both balance conditions and planes of analyses. For some conditions, the relative penalty on the hip and ankle joints varied in a way relevant to the balance condition or to the plane of analysis. These results suggest that unimpaired quiet balance in a challenging environment can be best described by a controller that maintains minimal effort through the adjustment of relative ankle and hip joint torques.
