Abstract:Severe retinal pathologies such as age-related macular degeneration and retinitis pigmentosa are still uncurable so far. A visual prosthesis is a device that can electrically stimulate a certain site on the visual pathway to replace the function of the diseased neurons, which has been proved a feasible approach to partially restore vision for blind patients. To date a number of various technical schemes by various groups in the world have been proposed and are currently at different stages of basic research or clinical trials. In this article we present recent progresses in visual prosthesis development of foreign research groups as well as that of the Chinese Project for Sight (C-sight) group in China. A breif introduction of progress in commercialization of visual prosthesis was also presented.

Abstract:Since Brindley evoked a visual sensation in a blind patient by electrical stimulation of the visual cortex, some groups have been committing to develop visual prosthesis based on electrical stimulation over the remaining structures of the visual pathway to restore lost vision caused by retinitis pigmentosa (RP) and age-related macular degeneration (ARMD). As the widespread availability of micro-manufacturing being introduced into the building of novel interfaces to the nervous system, there are four approaches to a visual prosthesis being currently pursued, including visual cortex, epi-retinal, sub-retinal and optic nerve prostheses.

Abstract:Vision is one of the most important human sensations about the surrounding world. Visual deprivation not only markedly affects the life of blind people, but also gives a heavy burden to their family and the society. A visual prosthesis is an electronic device that helps the blinds to regain visual perception by directly stimulating the visual pathway using the microelectrodes implanted into the body. In recent years, visual prostheses have been developed rapidly and some devices have already become clinically available. In this paper, we reviewed the history of visual prosthesis, introduced different visual prostheses classified according to the location of the implanted stimulating electrodes. Clinical study results as well as the functional status of the currently available visual prosthesis devices were also summarized.

Abstract:Induced by a variety of retinopathy, visual loss has become the most serious form of disability, which influences the quality of human life. With the rapid development and crossing among the information science, microelectronics, material science and biomedical disciplines, the visual prosthesis makes reparation possible for the visual blindness caused by retinitis pigmentosa, age-related macular degeneration, and other eye, retina, optic nerve and visual cortex lesions. With technology innovation, the prosthesis design, manufacturing and surgical technique are no longer the biggest obstacles to the future development of the visual prosthesis, but how to construct effective transmission of information between the brain and the prosthesis. However, due to the complex structure of the human visual system, the visual prosthesis manufacturing and visual information signal mapping are facing some difficulties. Thus, we can only study the representation strategy of image information and micro-electrode array stimulation basing on limited pixels of simulated prosthesis visual information. By studying the visual information processing of the visual prosthesis, we propose a visual prosthesis design which is based on biological, mechanical, and electronic integration.

Abstract:As the research hotspot of restoring vision for retinitis pigmentosa and age-related macular degeneration, visual prosthesis can form the visual perception and restore vision by capturing image with a microcamera, processing and encoding, transmitting the signal to the neural stimulator and delivering the current pulse to the neural system. Generally, three major types of visual prostheses are currently being developed: stimulation of the visual cortex using cortical implants, retinal devices implanted in the subretina or epiretina, and stimulation of the optic nerve. This paper will introduce the current progress and future challenge of visual prosthesis by C-sight group.

Abstract:Age-related macular degeneration(AMD) and retinitis pigmentosa(RP) are the main reasons resulting in uncurable retinal diseases of blind.Focusing on these two blinding diseases,development of visual prosthesis to enable blind people to see has become a hot topic of scientific research.The visual prosthesis stimulator,as the key module of a visual prosthesis,translates artificial visual coded information and access to effective visual evoked.This article first introduces a number of internationally renowned visual neural stimulator design,analyses their advantages and disadvantages.On this basis,we also focuses on the visual nerve stimulator challenges and future development trends,including the flexibility of the visual nerve stimulator design,low-power supply design,wireless energy and data transmission design.

Abstract:Visual prostheses, used to assist in restoring functional vision to the visually impaired, convert captured external images into corresponding electrical stimulation patterns that are stimulated by implanted microelectrodes to induce phosphenes and eventually visual perception. Detecting and providing useful visual information to the prosthesis wearer under limited artificial vision has been an important concern in the field of visual prosthesis. Along with the development of prosthetic device design and stimulus encoding methods, researchers have explored the possibility of the application of computer vision by simulating visual perception under prosthetic vision. Effective image processing in computer vision is performed to optimize artificial visual information and improve the ability to restore various important visual functions in implant recipients, allowing them to better achieve their daily demands. This paper first reviews the recent clinical implantation of different types of visual prostheses, summarizes the artificial visual perception of implant recipients, and especially focuses on its irregularities, such as dropout and distorted phosphenes. Then, the important aspects of computer vision in the optimization of visual information processing are reviewed, and the possibilities and shortcomings of these solutions are discussed. Ultimately, the development direction and emphasis issues for improving the performance of visual prosthesis devices are summarized.

Abstract:Visual prostheses based on a stimulating microelectrode array to restore vision offer a promising approach for the blind and has become a rapidly growing scientific field in neurorehabilitation engineering. A number of research groups from major developed countries lead the research activities in this field. The goal of the C-Sight Project is to develop an implantable microelectronic medical device that will restore useful vision to blind patients. Retinitis pigmentosa (RP) and age-related macular degeneration are the two leading causes for blindness, for which there have been no effective treatments, both surgically and biologically, until now.The ultimate goal of our project is to develop a completely implantable visual prosthesis based on a penetrating microelectrode array at the optical nerve; to implement this, it requires many technical advances. However, in our study, an implantable microcamera, the wearable information processor, and the multichannel neurostimulator are investigated. Electrophysiological experiments were also performed to provide evidence for the feasibility of our approach. Some related psychophysical studies including simulated phosphene positioning and recognition of pixelized images are also reported in this article.

Abstract:Objective: The current work of the main visual prosthesis research groups abroad and at home is presented,and the focal point and the difficulty in future study are discussed.Methods: The function and characteristics of the visual prosthesis implanted in the visual cortex,the optic nerve and the retina is reviewed and introduced,and their advantages and disadvantages are analyzed.Results: The engineering along with the surgical issues is no longer a great impediment to future progress.Rather,the barrier lies in how to best introduce meaningful information to the visually deprived brain.Conclusions: Until now there is no visual prosthesis that can restore functional vision for the patients clinically,the development of sufficient visual implant still needs persistent efforts.

Abstract:Recent research on implantable system provides available technology to the retinal prosthesis. Via penetrating the microelectrodes into the brain and stimulating the optic nerve, the device could evoke visual percepts in humans, which can use to restoring the lost vision. However, how to employ component to control the process of data and energy and produce the innocent and efficient bioelectric signal finally is rather difficult task. The technique of wireless communication and programmed current source give a feasible way. The particular design for stimulus part is present in this paper. Experimental data showed that the multi-stimulator was capable of generating controllable stimulation patterns with different parameters.

Abstract:Retinal degeneration, a leading cause of irreversible low vision and blindness globally, can be partially addressed by retina prostheses which stimulate remaining neurons in the retina. However, existing electrode-based treatments are invasive, posing substantial risks to patients and healthcare providers. Here, we introduce a completely noninvasive ultrasonic retina prosthesis, featuring a customized ultrasound two-dimensional array which allows for simultaneous imaging and stimulation. With synchronous three-dimensional imaging guidance and auto-alignment technology, ultrasonic retina prosthesis can generate programmed ultrasound waves to dynamically and precisely form arbitrary wave patterns on the retina. Neuron responses in the brain's visual center mirrored these patterns, evidencing successful artificial vision creation, which was further corroborated in behavior experiments. Quantitative analysis of the spatial-temporal resolution and field of view demonstrated advanced performance of ultrasonic retina prosthesis and elucidated the biophysical mechanism of retinal stimulation. As a noninvasive blindness prosthesis, ultrasonic retina prosthesis could lead to a more effective, widely acceptable treatment for blind patients. Its real-time imaging-guided stimulation strategy with a single ultrasound array, could also benefit ultrasound neurostimulation in other diseases.

Abstract:A visual prosthesis is an auxiliary device for patients with blinding diseases that cannot be treated with conventional surgery or drugs. It converts captured images into corresponding electrical stimulation patterns, according to which phosphenes are generated through the action of internal electrodes on the visual pathway to form visual perception. However, due to some restrictions such as the few implantable electrodes that the biological tissue can accommodate, the induced perception is far from ideal. Therefore, an important issue in visual prosthesis research is how to detect and present useful information in low-resolution prosthetic vision to improve the visual function of the wearer. In recent years, with the development and broad application of computer vision methods, researchers have investigated the possibility of their utilization in visual prostheses by simulating prosthetic visual percepts. Through the optimization of visual perception by image processing, the efficiency of visual prosthesis devices can be further improved to better meet the needs of prosthesis wearers. In this article, recent works on prosthetic vision centering on implementing computer vision methods are reviewed. Differences, strengths, and weaknesses of the mentioned methods are discussed. The development directions of optimizing prosthetic vision and improving methods of visual perception are analyzed.

Abstract:The C-Sight visual prosthesis is based on optical nerve stimulation with a penetrating electrode array. A silicon-based microprobe by MEMS process techniques and Pt-Ir microwire arrays by electrochemical etching were fabricated in our project. Noise and impedance analyses were applied to optimize the electrode configuration. A multichannel microcurrent neural electrical stimulator and an implantable CMOS-based micro-camera were developed for neural stimulation and image acquisition, respectively, with a DSP-based system processing the captured image. Electrical evoked potentials (EEPs) from rabbit models were recorded using multichannel stainless-steel screws Mounted oil the primary visual cortex. The mean charge threshold density was 20.99 +/- 5.52 mu C/cm(2) considering the exposed Surface of the stimulating electrode. Current threshold decreased as the pulse duration of the stimulus increased while the corresponding charge threshold increased. The amplitude of PI increased when the Pulse duration increased from 0.4 to 1.0 ms while the latency of PI changed little. Experiments also showed that different distribution maps of EEPs were elicited by different pairs of stimulating electrodes. The stimulating electrode pair along the axis of the optic nerve elicited cortical responses with much lower thresholds than that perpendicular to the axis of the optic nerve. The visual prosthesis with stimulating electrodes penetrating into the optic nerve has been validated in animal experiments.

Abstract:This review paper explores the current state of retinal prostheses technology and its potential for restoring vision in individuals with degenerative retinal diseases. It discusses the strengths and limitations of existing implantable devices and optogenetic approaches, and emphasizes the role of image‐processing and deep learning techniques in improving visual perception. The paper also highlights the need for future research to enhance the informative quality of phosphene simulations to facilitate safer navigation and increased interaction with the environment. Background Retinal prostheses offer hope for individuals with degenerative retinal diseases by stimulating the remaining retinal cells to partially restore their vision. This review delves into the current advancements in retinal prosthesis technology, with a special emphasis on the pivotal role that image processing and machine learning techniques play in this evolution. Methods We provide a comprehensive analysis of the existing implantable devices and optogenetic strategies, delineating their advantages, limitations, and challenges in addressing complex visual tasks. The review extends to various image processing algorithms and deep learning architectures that have been implemented to enhance the functionality of retinal prosthetic devices. We also illustrate the testing results by demonstrating the clinical trials or using Simulated Prosthetic Vision (SPV) through phosphene simulations, which is a critical aspect of simulating visual perception for retinal prosthesis users. Results Our review highlights the significant progress in retinal prosthesis technology, particularly its capacity to augment visual perception among the visually impaired. It discusses the integration between image processing and deep learning, illustrating their impact on individual interactions and navigations within the environment through applying clinical trials and also illustrating the limitations of some techniques to be used with current devices, as some approaches only use simulation even on sighted‐normal individuals or rely on qualitative analysis, where some consider realistic perception models and others do not. Conclusion This interdisciplinary field holds promise for the future of retinal prostheses, with the potential to significantly enhance the quality of life for individuals with retinal prostheses. Future research directions should pivot towards optimizing phosphene simulations for SPV approaches, considering the distorted and confusing nature of phosphene perception, thereby enriching the visual perception provided by these prosthetic devices. This endeavor will not only improve navigational independence but also facilitate a more immersive interaction with the environment.

Abstract:Loss of vision alters the day to day life of blind individuals and may impose a significant burden on their family and the economy. Cortical visual prosthetics have been shown to have the potential of restoring a useful degree of vision via stimulation of primary visual cortex. Due to current advances in electrode design and wireless power and data transmission, development of these prosthetics has gained momentum in the past few years and multiple sites around the world are currently developing and testing their designs. In this review, we briefly outline the visual prosthetic approaches and describe the history of cortical visual prosthetics. Next, we focus on the state of the art of cortical visual prosthesis by briefly explaining the design of current devices that are either under development or in the clinical testing phase. Lastly, we shed light on the challenges of each design and provide some potential solutions.
