Abstract:Intra-abdominal and pelvic abscesses are common and result from various illnesses. Percutaneous drainage applies limitedly to well-localized abscesses with appropriate density while surgical drainage usually causes significant physiological disturbance. We herein illustrated an innovative choice “sump drainage with trocar puncture” for the management of intra-abdominal abscesses and compare it with conventional percutaneous and surgical drainage in terms of clinical outcomes and prognosis.
Design and application of a new type of medical drainage device in the measurement of ICU intra-abdominal pressure

Abstract:Objective:To explore the clinical application of a new type of medical drainage device designed to measure the intra-abdominal pressure of ICU patients.Methods:The 65 patients with severe acute pancreatitis treated in our hospital from April to September 2018 were selected as the experimental group; the 54 patients with severe acute pancreatitis treated from October 2017 to March 2018 were selected as the control group. Patients in the control group used traditional drainage bags to measure intra-abdominal pressure, while patients in the experimental group used a new-designed medical drainage device to measure intra-abdominal pressure. Compare the cost of consumables used for the first time in the experimental group with the control group, the incidence of acupuncture injuries, the incidence of urethral leakage, the incidence of rupture of the catheter balloon, and the satisfaction of the nurses. The 32 patients admitted from January to March 2018 in the control group were measured again using a new drainage device after the abdominal pressure measurement operation was stopped in order to compare the accuracy of the two methods.Results:The per capita consumable cost of the experimental group was 5.71 yuan, 0 cases of needle stick injury, 0 case of catheter leakage, which were lower than the control group (22.36 yuan, 1 case, 7 cases), the difference was significant ( P<0.05). The nurse operation satisfaction score was 13.85±0.93, which was higher than the control group (10.00±1.05). The difference was statistically significant ( t value was -20.323, P<0.05). Conclusion:In the operation of intra-abdominal pressure measurement, the use of a new type of medical drainage device can ensure the accuracy of the measurement, reduce the cost of consumables, needle stick injuries and the incidence of urinary catheter leakage, and improve nurse operation satisfaction.

Abstract:Objective To study the effects of placement of double cannula using trocar puncture for intra-abdominal abscess drainage. Methods A retrospective study was performed to investigate the clinical data of 32 patients undergoing intra-abdominal abscess drainage with double cannula placed using trocar puncture between June 2010 and December 2010. Techniques: the location and size of the abscess was evaluated by ultrasound and CT. Placement of double cannula using trocar puncture was performed under CT or ultrasound guidance. Results Trocar puncture was successful in all the patients . One patient died of liver metastasis and multiple organ failure after surgery for pancreatic cancer. One patient required laparotomy and drainage because non-localization of sepsis from intestinal fistula. The remaining 30 patients experienced alleviation of septic symptoms after drainage and eventually cured. The mean healing time was (7±3) days. Two patients developed subcutaneous bleeding and were management by local compression. Conclusions Placement of double cannula using trocar puncture for intra-abdominal abscess drainage results in satisfactory outcomes. This technique is especially suitable for abscesses with viscous drainage, those with the presence of phlegmon or necrotic debris, and those with multiple large cavities.

Abstract:We aimed to introduce the puncture technique based on a novel device for preperitoneal tunneling in laparoscopic PD catheter placement and to evaluate the safety and efficacy of this technique. This novel device was used in our center from May 2016. We conducted a retrospective analysis of patients undergoing laparoscopic PD catheter placement. The traditional method was performed in 20 patients and the novel procedure in 18 patients. A straight Tenckhoff PD catheter was placed in all patients. No intraoperative complications were encountered in both groups. Compared the traditional technique group, the procedure based on the novel device had a shorter operative time (49.2 ± 11.8 vs 53.9 ± 12.5). One patient in the traditional tunneling group underwent catheter obstruction. There were no pericatheter leakage, exit site and subcutaneous infection, hernia and peritonitis in the early postoperative days. No mortality was observed in these patients. The 6-month survival rate of the catheter was 100%. Laparoscopic preperitoneal tunneling technique is an effective way to implant intra-abdominal catheter. Our method based on a novel puncture device for preperitoneal tunneling is safe and efficient.
Trocar Puncture with a Sump Drain for Crohn's Disease with Intra-Abdominal Abscess: A 10-Year Retrospective Cohort Study

Abstract:BackgroundTraditional percutaneous catheter drainage (PCD) and surgical intervention could not always achieve satisfactory results for patients with Crohn's disease (CD) who have complications with intra-abdominal abscess. We proposed a trocar puncture with sump drainage for the treatment of CD with intra-abdominal abscess and compared it with the conventional PCD and surgical intervention.MethodsCrohn's disease patients with intra-abdominal abscess and admitted to our hospital from 2011 to 2020 were identified by reviewing the electronic medical records. We divided them into Trocar, PCD, and fecal diverting (FD) groups, according to the ways of treating an abscess. Outcomes, risk factors for abscess recurrence, and postoperative complications were compared among the three groups.ResultsA total of 69 patients were included and they were divided into Trocar (n = 18), PCD (n = 29), and FD (n = 22) groups. Four patients in the PCD group were transferred to receive the FD surgery due to the failure of initial treatment. The incidence of abscess recurrence was significantly higher in the PCD (48%) and FD (50%) groups compared to the patients using the trocar puncture with the sump drain (Trocar group) (16.7%). There were 8 patients in Trocar, 22 in PCD, and 20 s in the FD group who received enterectomy. None of the patients in the Trocar had an ultimate stoma and the incidence of postoperative complications was statistically lower [0% (Trocar) vs. 31.8% (PCD) vs. 45% (FD), P < 0.05]. The way of initial treating of the abscess was significantly correlated with the abscess recurrence and postoperative complications.ConclusionsTrocar puncture with a sump drain had a lower incidence of abscess recurrence, abdominal adhesions, postdrainage, and postoperative complications compared to the conventional PCD or surgical intervention.

Abstract:Background. Intra-abdominal abscess is a common complication in Crohn’s disease (CD). Traditional percutaneous catheter drainage (PCD) and surgical intervention could not obtain satisfactory results in some cases. We herein demonstrate a novel management option and compare it with traditional strategies. Methods. A total of 77 patients were retrospectively collected into 3 groups. Postoperative complication, postoperative recurrence of abscess, subsequent surgery, ultimate stoma creation rate, and survival rate were analyzed. Results. Patients were divided into the trocar group (n = 21), PCD group (n = 25), and surgery group (n = 31). The incidences of postoperative complication as well as the incidence of recurrent abscess were lowest in trocar group, and ultimate stoma creation rate was highest in the surgery group. Subsequent surgery after initial intervention and survival rate during the follow-up period were similar among the 3 groups. Conclusions. Trocar puncture with sump drain had lower incidence of postoperative complication, postoperative recurrence of abscess, and ultimate stoma creation compared with conventional PCD and surgical interventions. This novel technique might be an optimal option in the management of intra-abdominal abscesses in CD.
Ultrasound-guided Percutaneous Drainage for Peripancreatic Abscess

Abstract:Objective To investigate the feasibility of ultrasound-guided percutaneous drainage for the treatment of peripancreatic abscess. Methods The clinical data of 36 patients with peripancreatic abscess who were admitted to the General Hospital of the Chengdu Military Command were retrospectively analyzed. All the puncture sites were designed according to the region, range and shape of the abscess, and then the angle and the direction of the needle penetration were determined according to the spatial relationship between the puncture site and the abscess. Finally, the drainage tubes were placed under the guidance of the ultrasound. Results The technique was successfully performed on all the patients, and 33 patients were cured with the cure rate of 92%.The mean healing time was 37 days. Three patients were converted to laparotomy because of the unsatisfied therapeutic effects. Enterocutaneous fistula was detected in 3 patients after the surgery and they were cured after receiving nonoperative management. All patients were followed up for 3-48 months, and neither residual abscess nor recurrence was detected. Two patients were complicated with type one diabetes, one with dyspepsia, two with gallstone, and they were cured by symptomatic treatment. The body weights of 27 patients were increased compared to those before the operation. Conclusion Ultrasound-guided percutaneous drainage is effective for the treatment of peripancreatic abscess.

Abstract:Rationale:Currently, the use of double-lumen irrigation-suction tube for drainage has become increasingly more common. However, the insertion process is complex, and the position of the double cannula placed in this manner is not accurate. We developed a method for placing the drainage tube and use it in the treatment of an abdominal infection.Patient concerns:A 51-year-old man with an abdominal infection due to colonic anastomotic fistula was admitted. Routine laboratory tests revealed an elevated white blood cell count (17x10(9)/L) and C-reactive protein level (78mg/L). Computed tomography (CT) revealed that the peritoneal cavity was filled with fluid.Diagnoses:The patient was diagnosed with colonic anastomosis fistula by gastrointestinal radiography. Abdominal infection was diagnosed based on CT scan, inflammatory markers, and patient signs and symptoms.Interventions:Two punctures were performed. After skin expansion, the source of infection was drained with a suction catheter (diameter = 1.0cm) under continuous negative pressure of 150 to 200 millibars, along with continuous saline irrigation at 300mL/h.Outcomes:Pus in the abdomen drained completely. The abdominal infection was controlled. There were no adverse events.Lessons:Abdominal infection in fistulas is a fatal disease. The main therapeutic target is full drainage at an early stage. Precise positioning of the tube, continuous negative pressure irrigation and drainage are key points in the treatment.

Abstract:BACKGROUND Postoperative abdominal infections are an important and heterogeneous health challenge. Many samll abdominal abscesses are resolved with antibiotics, but larger or symptomatic abscesses may require procedural management. CASE SUMMARY A 65-year-old male patient who suffered operation for the left hepatocellular carcinoma eight months ago, came to our hospital with recurrent abdominal pain, vomit, and fever for one month. Abdominal computed tomography showed that a big low-density dumbbell-shaped mass among the liver and intestine. Colonoscopy showed a submucosal mass with a fistula at colon of liver region. Gastroscopy showed a big rupture on the submucosal mass at the descending duodenum and a fistula at the duodenal bulb. Under colonoscopy, the brown liquid and pus were drained from the mass with “special stent device”. Under gastroscopy, we closed the rupture of the mass with a loop and six clips for purse stitching at the descending duodenum, and the same method as colonoscopy was used to drain the brown liquid and pus from the mass. The symptom of abdominal pain, vomit and fever were relieved after the treatment. CONCLUSION The special stent device could be effectively for draining the abdominal abscess respectively from colon and duodenum.

Abstract:Intra-abdominal abscesses usually originate from the gastrointestinal tract, with 70% occurring in the postoperative period. The mortality rate can reach 50%. These abscesses most commonly develop in the subphrenic and subhepatic spaces. Treatments include percutaneous drainage or surgical drainage. In this report, we present a minimally invasive video-assisted trans-diaphragmatic drainage (MIVTD) method through a simple incision using a right intercostal approach. This method was successfully performed on a patient who underwent Graham patch repair with laparotomy due to a diagnosis of peptic ulcer perforation and subsequently developed a right subphrenic multiloculated collection after unsuccessful percutaneous drainage.
Use Of A Closed Suction, Perfused Drainage System For Peripancreatic Drainage After Pancreaticoduodenectomy

Abstract:Objective: To use a novel drainage system in pancreaticoduodenectomy. Method: The data of 150 patients who underwent pancreaticoduodenectomy from January 2009 to July 2011 were analyzed, and all the surgeries were performed by a single team. First 93 surgeries were performed a novel drainage tube, the other 57 patients were placed the traditional one. The incidence and grade of postoperative pancreatic fistula (POPF), and the incidence of biliary leakage, delayed gastric emptying, bleeding and death within 30 days after the surgery were monitored. Result: In the first group, there were 22 patients had POPF (23.66%), the proportion of Grade A, B and C was 23.66%, 20.43% and 0%, respectively. In the contrary group, there were 17 patients had POPF (29.82%), the proportion of Grade A, B and C was 12.28%, 12.28% and 5.26%, respectively. Grade B and Grade C POPF rates between the two groups were statistically significance (P < 0.05). Regarding the incidence of postoperative biliary leakage, delayed gastric emptying, bleeding and death, no significant differences were observed (P > 0.05). Conclusion: The application of the novel drainage system in pancreaticoduodenectomy was safe, and could reduce the incidence of high grade POPF.

Abstract:Backgroud: Pancreatic fistula is a leading cause of morbidity and mortality after pancreaticoduodenectomy. We designed a modified technique, by which the pancreatic stump was invaginated into the jejunal loop by purse-string suture.Methods: From June 2008, 18 cases of patients were performed with this modified technique, and the morbidity and mortality were calculated.Results: All cases recovered well from pancreaticoduodenectomy, and none of them occured pancreatic fistula. The complications included: hemorrhage, wound infection, pulmonary infection, and ascite.Conclusions: Invagination anatomosis by purse-string suture could be performed safely and reduced the leakage rate of pancreaticojejunostomy. Abstract:Background: Post-operative anastomotic leak (PAL) is the most feared complication after abdominal surgery. Timely drainage of enteric effluent is beneficial in the healing of PAL. Methods: We introduced a new and feasible approach for early active drainage of PAL using fine tube bundles (FTBs). The therapeutic effects of FTBs were observed prospectively and compared with the traditional drainage tube without FTBs in a non-blinded randomized controlled trial. Results: Sixty patients with PAL in two tertiary hospitals in China from 2010 to 2016 were included in this study. Of these patients, 30 received FTBs and 30 were treated with a traditional drainage tube. The implantation failure rate was zero in the FTB group. No statistical difference was observed between the two groups in terms of demographic data. After these interventions, patients in the FTB group showed a faster decline in infection-related indictors, a higher ratio of spontaneous PAL closure, and shorter treatment duration of antibacterial agents compared with those in the traditional drainage tube group. Fatal complications and financial cost were also reduced in the FTB group. Conclusion: Fine tube bundles may contribute to the healing of PAL through active drainage. This method should be validated by further clinical trials for wider use.

Abstract:Traditionally abdominal abscesses have been treated with either surgical or radiologically guided percutaneous drainage. Surgical drainage procedures may be associated with considerable morbidity and mortality, and serious complications may also arise from percutaneous drainage. Endoscopic ultrasound (EUS)-guided drainage of well-demarcated abdominal abscesses, with adjunctive endoscopic debridement in the presence of solid necrotic debris, has been shown to be feasible and safe. This multicenter review summarizes the current status of the EUS-guided approach, describes the available and emerging techniques, and highlights the indications, limitations, and safety issues.
Step-up Approach of Source Control for Intra-abdominal Infection

Abstract:Intra-abdominal infection (IAI) is a clinical challenge,if combined with sepsis or multiple organ dysfunction,mortality rate of IAI will increase to 30％～40％.How to improve the cure rate of patients with severe IAI is still a matter of clinical concern.Early diagnosis,risk assessment and appropriate treatment for IAI are key factors in determining the prognosis of IAI.The source control is considered fundamental of the treatment in most IAIs,and the step-up approach of source control is recommended.The first step is to set up the pathway of active drainage with minimally invasive;if the active drainage cannot achieve the purpose of controlling the source of infection,the second step with laparotomy should be executed,and in this process,the principle of damage control surgery should be considered;open abdomen surgery as third step should be implemented for IAIs accompanied with intra-abdominal hypertension.
Sonographically-guided Parasacrum Infrapiriformis Drainage of Deep Pelvic Abscesses: An Anatomical Safety Study Using SYNAPSE VINCENT

Abstract:Objectives: Deep pelvic abscesses are surrounded by the pelvic bones, bladder, gynecological organs, intestinal tract, and nerve and vascular systems, and are approached by various routes for drainage. The transgluteal approach is often performed under computed tomography guidance; however, if ultrasonography can be used to confirm the approach, it is considered more effective because it reduces radiation exposure and allows for real-time puncture under sonographic and fluoroscopic guidance. Methods: This retrospective study was conducted at Tobata Kyoritsu Hospital (Fukuoka, Japan) between April 1, 2021, and December 31, 2022. Sonographically guided transgluteal drainage with fluoroscopy was performed in five consecutive cases of deep pelvic abscesses using a 3D image analysis system (SYNAPSE VINCENT) to study the anatomy for safe puncture. Results: Three patients had postoperative abscesses from colorectal cancer, one caused by perforation of the appendicitis, and one caused by sigmoid diverticulitis. The average drainage duration was 11 days (SD = 6.7). No complications, such as bleeding or nerve damage, were observed. Conclusions: We constructed a 3D image of the puncture route of the trans greater sciatic foramen using SYNAPSE VINCENT to objectively comprehend the anatomy and puncture route. The ideal transgluteal approach is to insert the catheter as close to the sacrum as possible at the level of the infrapiriformis. The parasacrum infrapiriformis approach can be performed safely and easily using ultrasound guidance and fluoroscopy.

Abstract:Objective To evaluate the therapeutic efficacy of percutaneous drainage of solitary retroperitoneal abscess under CT guidance.Methods A retrospective analysis was made for percutaneous drainage of solitary retroperitoneal abscess with CT guidance in 13 patients including 4 at the tail of pancreas, 3 nearby the head of pancreas,3 in perirenal space,2 in posterior renal space and 1 case was by the side of psoas muscle.The maximum diameters of retroperitoneal abscess were between 3.5cm and 8cm.Results Out of 13 patients,8 with one time drainage,3 with twice drainages and 2 with thrice drainages.The mean duration of drainage was 16 days with no recurrence or residual lesions during clinical follow up.Conclusion CT guided percutaneous drainage of solitary retroperitoneal abscess is effective and minimally invasive.(J Intervent Radiol,2007,16:828-830)
